Abstract

Purpose: An innovative strategy to improve the sensitivity of positron emission tomography (PET)- based treatment verification in ion beam radiotherapy is proposed. Methods: Low counting statistics PET images acquired during or shortly after the treatment (Measured PET) and a Monte Carlo estimate of the same PET images derived from the treatment plan (Expected PET) are considered as two frames of a 4D dataset. A 4D maximum likelihood reconstruction strategy was adapted to iteratively estimate the annihilation events distribution in a reference frame and the deformation motion fields that map it in the Expected PET and Measured PET frames. The outputs generated by the proposed strategy are as follows: (1) an estimate of the Measured PET with an image quality comparable to the Expected PET and (2) an estimate of the motion field mapping Expected PET to Measured PET. The details of the algorithm are presented and the strategy is preliminarily tested on analytically simulated datasets. Results: The algorithm demonstrates (1) robustness against noise, even in the worst conditions where 1.5-104 true coincidences and a random fraction of 73% are simulated; (2) a proper sensitivity to different kind and grade of mismatches ranging between 1 and 10 mm; (3) robustness against bias due to incorrect washout modeling in the Monte Carlo simulation up to 1/3 of the original signal amplitude; and (4) an ability to describe the mismatch even in presence of complex annihilation distributions such as those induced by two perpendicular superimposed ion fields. Conclusions: The promising results obtained in this work suggest the applicability of the method as a quantification tool for PET-based treatment verification in ion beam radiotherapy. An extensive assessment of the proposed strategy on real treatment verification data is planned.

Abstract

The article looks at a study regarding phenotypic variability in xeroderma pigmentosum group G and combining the skin alterations with neurological and systemic abnormalities of Cockayne syndrome. It mentions chest X-rays showed mildly enlarged heart but no electrical defect was observed at electrocardiography (ECG). It also mentions magnetic resonance imaging revealed severe white matter abnormalities and no signs of photosensitivity.

Abstract

OBJECTIVE: Two cases of T1D children are reported with similar pattern, characterized by remission of the disease just after the start of insulin therapy and, through supplements of high dosage of vitamin D and omega 3, a subsequent persistence of optimal metabolic control, respectively one and two years after, with minimal contribution of basal insulin (0.1 IU/kg/die) in a single evening injection. The immunomodulatory and anti-inflammatory effects of the supplements are likely determinants. From similarities in genotyping and autoantibodies, patterns of the two cases, we tried to identify which T1D subjects could benefit from this supportive therapy. The double supplement represents a sustainable approach to childhood T1D therapy to preserve the recovery of endogen insulin secretion for the absence of side effects. We do not know how remissions can last, but for now these represent a significant advantage for the child, and over time a further favorable benefit-cost intervention reducing hypoglycemia and complications. 18. 164. Dose evaluation and image quality assessment of mega-voltage CT Tomotherapy acquisitions.

Abstract

Abstract Purpose To assess dose and image quality in phantom with HiArt TomoTherapy and to evaluate the feasibility to acquire pre-treatment images with a pitch Coarse with a 3 mm slice thickness (ST) instead of the currently used 6 mm ST. Methods Multiple Scan Average Dose ( MSAD w) was calculated according to the literature using a Exradin ion-chamber in a TomoPhant placed at the isocenter. Measurements were repeated 5 times for each combination pitch -ST reconstruction (Fine 1 and 2 mm, Normal 2 and 4 mm and Coarse 3 and 6 mm) for 2 different scan lengths (5 and 10 cm). Image quality was assessed scanning a Catphan500 for 5 times with each combination of pitch and ST and the collected data were evaluated in terms of contrast linearity, signal uniformity, noise, Signal-to-noise (SNR) ratio. In addition, spatial resolution was evaluated both qualitatively and quantitatively. Results MSAD w for the 10-cm scan length was of 1.03 ± 0.01 cGy for pitch Coarse, 1.48 ± 0.01 cGy for pitch Normal and 2.83 ± 0.01 cGy for pitch Fine. We found slight lower values for the 5-cm scan length (0.94 ± 0.01 cGy, 1.36 ± 0.01 cGy and 2.67 ± 0.01 cGy for pitch Coarse, Normal and Fine, respectively) due to the lower scattered radiation. Considering image quality, signal uniformity was comparable among the different pitch-ST combinations; noise ranged from 2.4% to 2.5% with no significant differences changing pitch or ST. Signal-to-noise ratio ranged from 40.0 ± 2.0 from pitch Coarse-6 mm to 42.1 ± 1.2 for Normal-4 mm but we found no statistical significance in these differences. MTF 50 % was of 3.3 lp/cm for Coarse, 2.7 lp/cm for Normal and 2.5 lp/cm for Fine. Anyway these differences were not empirically detectable on the images, in which we observed up to 4 lp/cm for all the pitch-ST combinations. Conclusions The acquisitions of pre-treatment images with pitch Coarse and 3 mm ST reconstruction is feasible and justified according to our results. [ABSTRACT FROM AUTHOR]

Abstract

Background and purpose In clinical applications of Positron Emission Tomography (PET)-based treatment verification in ion beam therapy (PT-PET), detection and interpretation of inconsistencies between Measured PET and Expected PET are mostly limited by Measured PET noise, due to low count statistics, and by Expected PET bias, especially due to inaccurate washout modelling in off-line implementations. In this work, a recently proposed 4D Maximum Likelihood (ML) reconstruction algorithm which considers Measured PET and Expected PET as two different motion phases of a 4D dataset is assessed on clinical 4D PET-CT datasets acquired after carbon ion therapy. Material and methods The 4D ML reconstruction algorithm estimates: (1) Measured PET of enhanced image quality with respect to the conventional Measured PET, thanks to the exploitation of Expected PET; (2) the deformation field mapping the Expected PET onto the Measured PET as a measure of the occurred displacements. Results Results demonstrate the desired sensitivity to inconsistencies due to breathing motion and/or setup modification, robustness to noise in different count statistics scenarios, but a limited sensitivity to Expected PET washout inaccuracy. Conclusions The 4D ML reconstruction algorithm supports clinical 4D PT-PET in ion beam therapy. The limited sensitivity to washout inaccuracy can be detected and potentially overcome.
