4. Attending to cross-border sociocultural competence in bilingual programs in the Polish-Czech border region: An exploratory study.

The aim of this research was to explore whether and how various components of cross-border sociocultural competence were attended to in bilingual schools serving Polish minorities in the Zaolzie border region. Zaolzie is on the Czech side of the Polish-Czech border region, where the Polish minority play an important role in cross-border relations. Based on a review of the extant literature on the development of sociocultural competence among students enrolled in bilingual programs, researchers developed a 19-item survey capturing three distinct components of cross-border sociocultural competence: cognitive, social, and political. The survey was sent to teachers in the 14 Zaolzie region schools, with 123 teachers (44%) participating and asked participants to indicate the extent to which different facets of cross-border sociocultural competence were part of their school's instructional program. Most participants were female-identifying and had over 10 years of teaching experience, and most were born on the Czech side of the border but identified as Polish nationals. Results from an exploratory factor analysis (EFA) revealed two unique factors representing different components of sociocultural competence included in bilingual programs in the Zaolzie region: (1) the development of students' intrapersonal skills, motivation, and cross-linguistic understanding, and (2) the development of students' capacity to engage interpersonally with and to understand others in the region. These findings suggest that schools in the Zaolzie region attend to cross-border sociocultural competence through both an intra- and interpersonal lens, emphasizing individual students' linguistic and cultural development as well as their ability to interact in their highly politicized cross-border context. Educators in the region can thus consider assessing students' progress in these two areas over time as measures of program effectiveness. Given that the researchers found that cognitive, social, and political aspects of sociocultural competence were integrated across intra- and interpersonal dimensions, more research is needed to understand if this integration is unique to the teaching of cross-border sociocultural competence in the Zaolzie region or is taught similarly in other contexts. Dissociative disorders (DD) are mental disorders that involve experiencing a disconnection and lack of continuity between thoughts, memories, surroundings, actions, and identity. Transient dissociative episodes are a common phenomenon during childhood that generally decrease during adolescence to relatively low levels in adults. There is strong evidence that traumatic events play a key role in the development of dissociation, but non-abusive and genetic factors are also considered. In the article, we discuss a few complementary theoretical models of the origin of DD that view them from attachment theory, affect theory, or developmental perspectives. Diagnosing DD among children is especially hard because of their frequent co-occurrence with other disorders, their broad differential diagnosis spectrum, and the fact that symptoms may be unnoticed for a long time. 6. Circadian Blood Pressure Profile in Pediatric Patients with Primary Hypertension.

Our study aimed to evaluate factors affecting circadian BP profile and its association with hypertension-mediated organ damage (HMOD) in pediatric patients with primary hypertension (PH). The study included 112 children (14.7 ± 2.1 age, 79 boys, 33 girls) with untreated PH. Non-dipping was defined as a nocturnal drop in systolic or diastolic BP (SBP, DBP) < 10%, and a nocturnal drop >20% was defined as extreme dipping. The nocturnal SBP drop was 10.9 ± 5.9 (%), and the DBP drop was 16.2 ± 8.5 (%). Non-dipping was found in 50 (44.6%) children and extreme dipping in 29 (25.9%) patients. The nocturnal SBP decrease correlated with BMI Z-score (r = −0.242, p = 0.010) and left ventricular mass index (LVMI) (r = −0.395, p = 0.006); diastolic DBP decrease correlated with augmentation index (AIx75HR) (r = 0.367, p = 0.003). Patients with a disturbed blood pressure profile had the highest LVMI (p = 0.049), while extreme dippers had the highest augmentation index (AIx75HR) (p = 0.027). Elevated systolic and diastolic BP dipping were risk factors for positive AIx75HR (OR 1.122 95CI (1.009–1.249) and OR 1.095 95CI (1.017–1.177). We concluded that disturbed circadian BP profile was common in children with PH and should not be considered a marker of secondary hypertension. A disturbed circadian BP profile may be associated with higher body weight. In pediatric patients with PH, non-dipping is associated with increased left ventricular mass, and extreme dipping may be a risk factor for increased arterial stiffness. [ABSTRACT FROM AUTHOR]

7. Digitalisation of Schools from the Perspective of Teachers' Opinions and Experiences: The Frequency of ICT Use in Education, Attitudes towards New Media, and Support from Management.

The digitalisation of education has become an irreversible process, and Poland is no exception. However, the issue of ICT usage in education raises many concerns and controversies, posing numerous methodological challenges at the same time. In the interpretation of our research, one of the most frequently used and validated models in empirical research—the Unified Theory of Technology Acceptance and Use (UTAUT) model—was used. The aim of the research was to show the frequency of use of popular hardware, software and websites among teachers from Poland (in the Silesia Province) and finding answers to the questions pertaining to the main determinants of digital teaching aids. The frequency of ICT use in education was juxtaposed with the attitudes towards new media in the school environment as well as with the support of managers in that area. Quantitative research was conducted on a sample of N = 258 in 2020. A questionnaire in the form of a digital diagnostic survey was used to collect data. Based on the data collected, it was noted that: (1) Teachers use virtual systems (eRegisters) and interactive whiteboards most often, while educational podcasts and software for learning foreign languages are used least often; (2) approximately 40% of teachers use ICT often or very often in their school teaching; (3) in public schools, digital teaching aids are used slightly more often than in non-public institutions; (4) teachers are consistent in their use of digital teaching aids, so the individuals experimenting with and implementing ICT at school do so regardless of the type of software and hardware; (5) active support from school head teachers strengthens the frequency of ICT use in education; and (6) in schools where the use of smartphones by students is prohibited, the frequency of ICT use in education is at a lower level. Primary hypertension (PH) is the most common cause of arterial hypertension in adolescents; many of these patients present with isolated systolic hypertension (ISH). The study presents a case of a 16-year-old male adolescent with PH. The patient had ISH in both 24-hourambulatory and office measurements. Secondary causes and hypertension-mediated organ damage were excluded. Evaluation of the vascular phenotype revealed low central systolic blood pressure (109 mm Hg), high peripheral augmentation index (27 mm Hg), and high arterial elasticity. The boy was started on non-pharmacological treatment. The manuscript discusses up-to-date knowledge on the pathogenesis and evolution of ISH in young people. A diagnostic and therapeutic approach according to the European Society of Hypertension is presented. Adolescents with ISH require evaluation of the vascular phenotype. There is a need for further studies on the evolution and long-term outcome in young people with this form of arterial hypertension. [ABSTRACT FROM AUTHOR]

13. Ambulatory blood pressure monitoring and subclinical inflammation in children with chronic kidney disease.

Background. Children with chronic kidney disease (CKD) are characterized by increased risk of hypertension and chronic low-grade inflammation. The aim of the study was the analysis of relation between parameters of ambulatory blood pressure monitoring (ABPM) and subclinical inflammation in children with CKD. Material and methods. Study group included 27 paediatric patients (age 14.23 ± 3.57 years) with CKD stage 2-5; 18 children with previously recognized hypertension. In all patients we evaluated ABPM, office blood pressure, complete blood count and selected clinical and biochemical parameters. Results. In the study group, GFR was from 7.05 to 86.73, mean 40.88 ± 25.82 mL/min/1.73 m2. All 9 children without hypertension had normal blood pressure in ABPM, but ABPM detected poor blood pressure control in 7 among 18 (38.9%) children with previously recognized and treated hypertension. Abnormal circadian blood pressure profile was found in 12 (44.4%) children: 9/18 (50.0%) with hypertension and 3/9 (33.3%) with normal BP. Systolic, diastolic, mean blood pressure and diastolic blood pressure load correlated with neutrophil count, neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio (r = 0.39-0.49, p = 0.010-0.044); diastolic and mean blood pressure and diastolic blood pressure load with parathormone (r = 0.48-0.57, p = 0.005-0.023); diastolic blood pressure load with phosphate and calcium-phosphorus product (r = 0.44-0.47, p = 0.021-0.030); diastolic blood pressure dipping with phosphate (r = -0.43, p = 0.034). Conclusions. 1. Ambulatory blood pressure monitoring should be used in children with chronic kidney disease on a regular basis, especially in those with arterial hypertension. 2. 14. Vitamin D in children with primary hypertension.

Introduction: Recent evidence suggests that vitamin D plays a role in pathogenesis of arterial hypertension. The aim was to assess vitamin D in children and adolescents with arterial hypertension. Material and methods: In 49 children (14.29 ± 3.17 years) with arterial hypertension we evaluated vitamin D status (according to Polish 2018 Guidelines), serum calcium, phosphorus, parathormone, alkaline phosphatase, office blood pressure, ABPM (including ambulatory arterial stiffness index [AASI]), BMI, GFR, uric acid, lipids and albuminuria. None of the children were supplemented with vitamin D. Results: Mean vitamin D concentration was 19.74 ± 9.68 ng/mL. Vitamin D severe deficiency (0-10 ng/mL) was found in 5 (10.2%), deficiency (> 10-20 ng/mL) in 29 (49.0%), suboptimal concentration (> 20-30 ng/mL) in 17 (34.7%), optimal concentration (> 30 to 50 ng/mL) in 1 (2.0%), and high concentration (> 50 to 100 ng/mL) in 2 (4.1%) children. Vitamin D was higher in spring-summer vs. autumn-winter (21.79 ± 10.19 vs. 15.53 ± 7.08 ng/mL, p = 0.03). Vitamin D correlated with height Z-score (r = 0.39, p < 0.01), BMI Z-score (r = -0.34, p = 0.02), uric acid (r = -0.31, p = 0.04), triglycerides (r = -0.37, p = 0.01), but not with office blood pressure and ABPM parameters except for heart rate (r = -0.38, p < 0.01). In 24 children treated with antihypertensive medications vitamin D correlated with AASI (r = 0.50, p = 0.04). Conclusions: 1. Inadequate vitamin D supply is ubiquitous in children with arterial hypertension. 2. Vitamin D deficiency should be suspected especially in autumn-winter period and among obese and short children. 3. The relation between vitamin D status and ambulatory arterial stiffness index suggests negative influence of vitamin D on arterial wall but requires further investigations. [ABSTRACT FROM AUTHOR]

The article discusses the issue of Public Relations (PR) and its use in Polish social welfare institutions. At present, we can observe that Polish social welfare industry faces serious problems related to its public image. This is due to: insufficient experience in Public Relations, lack of systemic actions towards improving the prestige of social work as well as creating the image of social welfare. The text offers a comparison of beliefs and opinions expressed by the employees of the Polish public welfare institutions; these opinions and views are related to PR and communication practices applied by their organisations. The opinions were collected through a nationwide survey. The results include very interesting findings: for instance, they confirm the thesis about the lack of regular and constructive communication and image-related activities carried out on the institutional level and low evaluation of their effectiveness. The conclusions, however, also point out that the respondents revealed that they are highly aware of the importance of PR operations as well as of the correlations between these operations and how their institutions are seen by the general public and by the media. For today's pupils the Internet is something obvious, a natural environment and simultaneously one of the most typical tools for obtaining knowledge and information, communication with peers and the source of entertainment. Very different or even completely opposite are the attitudes towards the new media presented by teachers, pedagogues and managers of educational institutions. Due to the significant role of new technologies in the socialization of young people as well as in education and communication, it is noticeable that there is a barrier between the generations of pupils and pedagogues and it has a major impact on the quality of the education process. An important aspect is, in this context, the level of knowledge among educators on using the Internet by their pupils: so called models of "presence in the net" and, what in the perspective of the upbringing role of the schools is of special significance, the awareness of the real dangers connected with using computer and the Internet by young people, including netoholism. The article presents data and information regarding the level of knowledge among the management staff of educational institutions for children and youth in the Silesian voivodship, regarding the area of misuse of the new media by the pupils and the threat of Internet addiction. The information has been provided by the project study "Netoholism among children and youth" carried out since 2008 in cooperation of the Higher School of Administration in Bielsko-Biała with the Silesian Education Administrative Body in Katowice (Śląskie Kuratorium Oświaty w Katowicach). Recent studies showed the significance of the canonical Wnt/beta-catenin pathway and its inhibitor—sclerostin, in the formation of arterial damage, cardiovascular morbidity, and mortality. The study aimed to assess serum sclerostin concentration and its relationship with blood pressure, arterial damage, and calcium-phosphate metabolism in children and adolescents with primary hypertension (PH). Serum sclerostin concentration (pmol/L) was evaluated in 60 pediatric patients with PH and 20 healthy children. In the study group, we also assessed calcium-phosphate metabolism, office peripheral and central blood pressure, 24 h ambulatory blood pressure, and parameters of arterial damage. Serum sclerostin did not differ significantly between patients with PH and the control group (36.6 ± 10.6 vs. 41.0 ± 11.9 (pmol/L), p = 0.119). In the whole study group, sclerostin concentration correlated positively with height Z-score, phosphate, and alkaline phosphatase, and negatively with age, peripheral systolic and mean blood pressure, and central systolic and mean blood pressure. In multivariate analysis, systolic blood pressure (SBP) and height expressed as Z-scores were the significant determinants of serum sclerostin in the studied children: height Z-score (β = 0.224, (95%CI, 0.017–0.430)), SBP Z-score (β = −0.216, (95%CI, −0.417 to −0.016)). Experimental studies suggest that periostin is involved in tissue repair and remodeling. The study aimed to evaluate serum periostin concentration as potential biomarker in pediatric patients with primary hypertension (PH). We measured serum periostin, blood pressure, arterial damage, biochemical, and clinical data in 50 children with PH and 20 age-matched healthy controls. In univariate analysis, children with PH had significantly lower serum periostin compared to healthy peers (35.42 ± 10.43 vs. 42.16 ± 12.82 [ng/mL], p = 0.038). In the entire group of 70 children serum periostin concentration correlated negatively with peripheral, central, and ambulatory blood pressure, as well as with aortic pulse wave velocity (aPWV). In multivariate analysis, periostin level significantly correlated with age (β = −0.614, [95% confidence interval (CI), −0.831–−0.398]), uric acid (β = 0.328, [95%CI, 0.124–0.533]), body mass index (BMI) Z-score (β = −0.293, [95%CI, −0.492–−0.095]), high-density lipoprotein (HDL)-cholesterol (β = 0.235, [95%CI, 0.054–0.416]), and triglycerides (β = −0.198, [95%CI, −0.394–−0.002]). Neither the presence of hypertension nor blood pressure and aPWV influenced periostin level. To conclude, the role of serum periostin as a biomarker of elevated blood pressure and arterial damage in pediatric patients with primary hypertension is yet to be unmasked. Problematic Internet Use (PIU) has of late come to the major attention of researchers of risky behaviours. For parents and teachers in turn, the various pathological forms of Internet use have become more and more noticeable. In recent years, the operationalisation of the term PIU has also been discussed and attempts to measure its various phenomena have been made. In our study, we used the quantitative method—a diagnostic survey designed on the basis of a tool of the Supreme Audit Office and Scientific and Academic Computer Network (Naukowa i Akademicka Sieć Kompureowa NASK), The survey was conducted in the first half of 2017 in Poland, among 3569 adolescents (median 16, SD = 1.28). The purpose of the research was to investigate the scale of PIU among Polish youths. For most young people, PIU is a major social problem. The majority also declare that more than half of their friends have problems with using electronic media (smartphones and the Internet in particular). On the basis of the data collected, it is estimated that 60% of adolescents use media in a functional way, whereas 40% of young electronic media users show visible PIU symptoms, of which 5% do so at a destructive level (all symptoms). Gender does not co-occur with PIU; however, family structure (where both parents are present), the setting of rules for the use of electronic media, and the place of residence are all protective factors. There is a need for constantly improved tools for measuring PIU, as well as preventive programs focused on developing self-control and helping young people understand their own emotions. The geographical presentation of various data on the spatial functioning of society has been present in fields such as social policy and security for almost 200 years. New technologies, in particular computerization and the GIS tool, not only make the mapping of social problems or security threats more accurate, but also allow for the interactive creation of those maps by society and public institutions. This article is a qualitative analysis of solutions used in the Silesian Voivodeship, a region of Poland with a highly specific nature and density in terms of the problems discussed. The tools presented, including a "National Map of Security Threats" and "Maps of Social Problems and Resources in the Local Environment", demonstrate—apart from their obvious benefits, such as increasing public awareness of threats and enhancing cooperation between society and public institutions in combatting them—the challenges and problems that these solutions bring and may also create in the future. Introduction: the immune system can trigger an inflammatory process leading to blood pressure elevation and arterial damage. the aim of the study was to assess the relation between subclinical inflammation and arterial damage in pediatric patients with primary hypertension (PH) and to establish the usefulness of neutrophil-to-lymphocyte (NLR) and platelet-to-lymphocyte (PLR) ratios, and mean platelet volume (MPV) as markers of arterial damage in these subjects. Material and methods: in 119 children with Ph (14.94 ±2.76 years) and 45 healthy children (14.91 ±2.69 years) we analyzed markers of subclinical inflammation (NLR, PLR, MPV), clinical and biochemical parameters, office blood pressure, ambulatory blood pressure monitoring (ABPM), central blood pressure, aortic pulse wave velocity (APWV), augmentation index corrected for heart rates 75 (aix75hr), carotid intima media thickness (CIMt), and common carotid artery stiffness (e-tracking). Results: Children with Ph were characterized by significantly higher neutrophil (3.9 ±1.7 vs. 3.0 ±1.0 [1000/μl], p < 0.001) and platelet counts (271.9 ±62.3 vs. 250.3 ±60.3 [1000/μl], p = 0.047), nLr (1.9 ±1.5 vs. 1.3 ±0.4, p = 0.010), PLr (131.4 ±41.9 vs. 114.7 ±37.6, p = 0.020), aPwV (5.36 ±0.88 vs. 4.88 ±0.92 m/s, p = 0.004), and ciMt (0.46 ±0.07 vs. 0.43 ±0.07 mm, p = 0.002) compared to healthy children. in Ph children nLr correlated positively (p < 0.05) with: systolic, diastolic and mean blood pressure in ABPM (r = 0.243, r = 0.216, r = 0.251), aPwV [m/s] (r = 0.241), aPwV z-score (r = 0.204), and common carotid artery PWVbeta [m/s] (r = 0.202). Conclusions: there is a link between arterial stiffness and subclinical inflammation in pediatric patients with primary hypertension. Neutrophil-to-lymphocyte ratio may serve as a promising marker of arterial stiffness in pediatric patients affected by primary hypertension. [ABSTRACT FROM AUTHOR]

Introduction: Adult and pediatric data suggest a positive relationship between the extent of sub-clinical inflammation, blood pressure, and hypertension-mediated organ damage (HMOD) in primary hypertension (PH). 24-hour (24-h) ambulatory blood pressure (ABPM) and central blood pressure (CBP) are strong predictors of HMOD. our study aimed to analyze the relationship between 24-h central ABPM, subclinical inflammation, and clinical data in adolescents with pH. Material and methods: In 28 untreated adolescents with PH (14.50 ±2.27 years) and 25 healthy peers (14.76 ±2.83 years), we analyzed 24-h peripheral and central ABPM, markers of subclinical inflammation (neutrophil-to-lymphocyte ratio -- NLR, platelet-to-lymphocyte ratio -- PLR, mean platelet volume -- MPV), and clinical and biochemical data. Results: Patients with PH had higher 24-h peripheral and central blood pressure than healthy peers. In all 53 patients, we found significant (p < 0.05) positive correlations between NLR, PLR and 24-h central systolic, diastolic, and mean blood pressure (24-h cSBP, 24-h cDBP, 24-h cMAP), between 24-h central augmentation index corrected for heart rate 75 (24-h cAIx75HR) and platelet count. In 28 patients with PH, 24-h cAIx75HR correlated with low-density lipoprotein (LDL) cholesterol (R = 0.442), and ambulatory arterial stiffness index with body mass index (BMI) (R = 0.487), uric acid (R = 0.430), and high-density lipoprotein (HDL) cholesterol (R = -0.428). Conclusions: Increased central 24-h blood pressure may be associated with immune system activation in adolescents with primary hypertension. In adolescents with primary hypertension, dyslipidemia and hyperuricemia are risk factors for increased arterial stiffness. Further studies on central and peripheral blood pressure in terms of their relationship with inflammation in these patients are needed. [ABSTRACT FROM AUTHOR]

Introduction: Adhesion molecules: E-selectin and intercellular adhesion molecule 1 (ICAM-1) are well-established markers of endothelial injury. The aim of the study was to assess the relation between E-selectin and ICAM-1 and clinical and biochemical parameters in children and adolescents with primary hypertension (PH). Material and methods: In 77 patients with PH (15.04 ±2.62 years, 50 boys, 27 girls) we evaluated serum E-selectin, ICAM-1, and selected clinical and biochemical parameters including inflammatory indicators and ambulatory blood pressure monitoring (ABPM). Results: The E-selectin concentration was 55.63 ±26.49 ng/ml and the ICAM-1 concentration was 302.17 ±67.14 ng/ml. E-selectin and ICAM-1 correlated (p < 0.05) with BMI Z-score (r = 0.24, r = 0.29), ICAM-1 also with uric acid (r = 0.35), HDL-cholesterol (r = --0.28), platelet-to-lymphocyte ratio (r = 0.26), and systolic and mean blood pressure variability (r = 0.24, r = 0.24); in boys ICAM-1 correlated with mean platelet volume (r = 0.29). In multivariate analysis the only significant predictor of E-selectin was mean arterial pressure during 24 hours (β = 0.329, 95% CI: 0.012-0.646) and of ICAM-1 -- uric acid (β = 0.430, 95% CI: 0.040-0.819). In 27 children with newly diagnosed PH E-selectin correlated negatively with diastolic blood pressure dipping (r = --0.54, p = 0.004) and positively with ambulatory arterial stiffness index (r = 0.51, p = 0.012). Conclusions: Elevated mean arterial pressure and hyperuricemia are risk factors of endothelial damage in paediatric patients with primary hypertension. In children with untreated primary hypertension there may be a relation between endothelial damage and disturbed circadian blood pressure profile. [ABSTRACT FROM AUTHOR]
