Abstract

Silver is used in various industrial applications, but also in confectioneries and for therapeutic use due to its antibiotic properties. Its toxicity is not well documented and most often only in the context of professional exposure. Here we report two cases of high silver concentrations in biological samples in two women: the first patient presented grey marks around her cuticles, probably due to her consumption of silvered sweets and the second patient presented agranulocytosis and thrombocytopenia occurring within 24 h after the topical application of a cream containing sulfadiazine and silver to burns over a large surface area. Silver concentrations were determined in blood and urine samples and sweets using inductively coupled plasma- mass spectrometry (ICP-MS). The silver concentrations were elevated compared to population reference values and confirmed the hypotheses for the patients: the significant consumption of sweets coated with silver nanoparticles and the topical application of a cream containing silver to burns over a large area. After initial questioning by the dermatologist, Patient 1 explained that she consumed more than 30 bags of the sweets per year. She decreased her consumption of the sweets and the control performed one year later showed a plasma silver concentration of 1.5 μg/L. For Patient 2, the absorption of silver through burns over a large area appeared relatively significant, whereas it is very low through undamaged skin. The haematological cells counts returned to normal levels quickly and no other major effects were highlighted. To apply these findings to a larger population, further investigation to determine sulfadiazine and silver concentrations in plasma and urine have been initiated in a cohort of patients with burns over a large area. Abstract

Introduction Candida glabrata est une espèce de levures fréquemment isolée lors des infections profondes affectant les patients de réanimation chirurgicale. Depuis leur commercialisation, les échinocandines sont apparues comme la première option thérapeutique vis-à-vis des souches qui présentent des concentrations minimales inhibitrices (CMIs) élevées au fluconazole. Cependant, il a été récemment rapporté que l’usage croissant de cette classe d’antifongiques pouvait conduire à l’apparition de mutations sur les séquences FKS1 et FKS2 , qui codent les sous-unités de la protéine-cible, la β-D-Glucane synthétase de la paroi fongique. Contexte et objectifs Suite à l’installation en 2010 de l’activité de transplantation hépatique au CHU de Tours, la consommation d’échinocandines en réanimation chirurgicale est passée, en l’espace de 36 mois, de 67 journées de traitement par an à 392. L’objectif de ce travail était donc d’effectuer une « photographie » locale des mutations apparues sur les hot spots des gènes FKS1 et FKS2 de toute souche de C. glabrata isolée en réanimation chirurgicale entre novembre 2013 et avril 2014. Matériel et méthodes L’identification mycologique a été réalisée jusqu’à l’espèce par les méthodes usuelles. L’ADN des souches identifiées comme C. glabrata a été extrait par le kit QIAmp DNA Mini Kit ® (Qiagen). Les séquences des deux hot spots (HS1 et HS2) des gènes FKS1 et FKS2 ont été amplifiées par PCR (Thompson GR 3rd., et al. AAC, 2008). Les séquences obtenues ont été alignées (Codon Code Aligner ® ), puis comparées à une séquence de référence (GenBank accession number : AGP03610.1 ). Les CMIs pour la caspofungine ont été mesurées par technique E-test ® (BioMérieux). Résultats Sur la période d’étude, 21 souches de C. glabrata ont été isolées chez 14 patients, dont 42,9 % exposés à la caspofungine (en moyenne, 15,0 jours de traitement ± 6,6). Toutes, sauf une, présentaient des séquences conservées ( wild type ) dans les domaines FKS1 et FKS2 , et quand réalisées, les CMIs étaient basses (0,24 μg/mL ± 0,09). Une souche, issue des urines sondées d’une patiente ayant bénéficié d’un traitement préalable par caspofungine pendant 21 jours, présentait la mutation F659S sur HS1 du gène FKS2 et une CMI à 1,0 μg/mL. Pour la même patiente, l’isolat antérieur provenant des urines présentait une CMI à 0,25 μg/mL. Discussion et conclusion Dans notre contexte, ces résultats ne sont pas en faveur de l’émergence large de phénomènes de résistance génétique vis-à-vis des échinocandines. Néanmoins, la détection d’une mutation isolée apparaît comme un signal qui devrait nous inciter à une vigilance et une surveillance épidémiologique régulière. [ABSTRACT FROM AUTHOR]
