ABSTRACT: The present study demonstrates successful herd sanitation and eradication of contagious mastitis caused by Staphylococcus aureus genotype B (GTB) in an entire Swiss district (Ticino) including 3,364 dairy cows from 168 farms. Herd sanitation included testing of all cows using a highly GTB-specific and sensitive real-time quantitative PCR (qPCR) assay, implementation of related on-farm measures, appropriate antibiotic therapy of GTB-positive cows, and culling of therapy-resistant animals, respectively. A treatment index was used as an objective criterion to select GTB-positive cows eligible for culling and replacement payment. Sixty-two herds (37%) were initially GTB-positive with a cow prevalence between 10% and 100% and were submitted to sanitation. Twenty months after the start of the campaign, all of these herds were free from S. aureus GTB, whereby 73% of them were sanitized during the first 7 mo. At the cow level, a total of 343 animals were infected. Fifty of them were immediately culled and farmers were financially compensated based on their treatment index value. The remaining 293 cows were intramammarily treated with antibiotics either during lactation using the combination of cephalexin-kanamycin or penicillin-gentamicin or at dry-off using cloxacillin. Out of these cows, 275 (93.9%) were treated successfully, meaning that their milk was twice GTB-negative by qPCR after therapy. For lactational treatment, control samples were taken ≥10 and ≥20 d after treatment, for dry-off treatment ≥14 and ≥24 d after parturition. Neither lactation number nor SCC before treatment of the cow nor the type of therapy was associated with therapeutic cure. Using data of 30 GTB-positive and 71 GTB-negative herds (1,855 observations), the effect of GTB sanitation on bulk tank milk SCC (BTSCC) was evaluated by applying a linear mixed statistical model. In the year before sanitation, BTSCC was always higher in GTB-positive than in GTB-negative herds. After the start of the campaign, BTSCC declined rapidly in the herds under GTB sanitation and achieved values that no longer differed statistically from those of GTB-free herds after only 2 mo, remaining very similar for the rest of the campaign. The farmers were very satisfied with the outcome of the campaign because all GTB-positive herds could be sanitized rapidly, sanitation was sustainable, and milk quality increased.

Healthy, untreated cows of nine dairy herds from the Swiss Canton Tessin were analyzed three times within one year to identify the most abundant species of the intramammary bacteriome. Aseptically collected milk samples were cultured and bacteria identified using MALDI-TOF. Of 256 cows analyzed, 96% were bacteriologically positive and 80% of the 1,024 quarters were positive for at least one bacterial species. 84.5% of the quarters were healthy with somatic cell counts (SCC)

El presente trabajo tiene por objeto conocer la realidad política e institucional de las Reales Aduanas del Río de la Plata. Se busca comprender el sistema político de la monarquía española a fines del siglo XVIII y principios del XIX. Se estudia la comisión secreta y por vía reservada de Francisco Ximénez de Mesa en la Real Aduana de Montevideo en 1786. Las competencias jurisdiccionales de los asuntos del Real Erario en manos del Superintendente de Real Hacienda, Francisco de Paula Sanz, permitieron organizar tal pesquisa. En1788, tras la reestructuración de la superintendencia, se retoman los informes de la comisión originando la revisión de los estados contables, llevando a la quiebra al administrador Antonio Quintana y Lasso de la Vega. Para este caso se consultaron expedientes de la sección Criminales del Archivo General de la Nación, documentación de Reales Órdenes, reglamentos y ordenanzas vinculados a la organización administrativa del ramo aduanero.

Abstract Background The bedside head impulse test (bHIT) is a clinical method of assessing the vestibulo-ocular reflex (VOR). It is a critical component of the bedside assessment of dizzy patients, and can help differentiate acute stroke from vestibular neuritis. However, there is evidence showing the bHIT is often not performed in appropriate clinical settings or is performed poorly. To date, there have been no studies evaluating the bHIT competence of graduating physicians. Methods 23 final year Otolaryngology –Head &Neck Surgery (OTL-HNS) residents in Canada were evaluated on the use of bHIT using a written multiple-choice examination, interpretation of bHIT videos, and performance of a bHIT. Ratings of subject bHIT performance were completed by two expert examiners (DT, DL) using the previously published Ottawa Clinic Assessment Tool (OCAT). Results Using a cut-off of an OCAT score of 4 or greater, only 22% (rater DT) and 39% (rater DL) of residents were found able to perform the bHIT independently. Inter-rater reliability was fair (0.51, interclass correlation). The mean scores were 65% (14.1% standard deviation) on the video interpretation and 71% (20.2% standard deviation) on the multiple-choice questions. The scores on multiple choice examination did not correlate with bHIT ratings (Pearson r = 0.07) but there was fair correlation between video interpretation and bHIT ratings (Pearson r = 0.45). Conclusion Final year OTL-HNS residents in Canada are not adequately trained in performing the bHIT, though low interrater reliability may limit the evaluation of this bedside skill. Multiple choice examinations do not reflect bHIT skill. These findings have implications for development of competency-based curricula and evaluations in Canada in critical physical exam skills.

This study was conducted in Armação de Búzios, Brazil, a municipality where ecosystem degradation has been observed following large increases in tourism and population growth. The goal of this study was to determine seasonal variations in three Búzios populations of the long snout seahorse Hippocampus reidi. Monthly dives were conducted from November 2011 to October 2013. All three subpopulations had low densities of seahorses and no seasonality. The sex ratio differed at each site. The most commonly used microhabitats were the sponge Aplysina fulva and the seaweed Sargassum sp. There was no significant difference in temperature and salinity. The environmental trends could not explain the variation in seahorse density at the three beaches. The population showed no seasonality and no further decline.

Background: The high incidence of pediatric acute otitis media (AOM) makes the implications of overdiagnosis and overtreatment far-reaching. Quality indicators (QIs) for AOM are limited, drawing from generalized upper respiratory infection QIs, or locally developed benchmarks. Recognizing this, we sought to develop pediatric AOM QIs to build a foundation for future quality improvement efforts. Methods: Candidate indicators (CIs) were extracted from existing guidelines and position statements. The modified RAND Corporation/University of California, Los Angeles (RAND/UCLA) appropriateness methodology was used to select the final QIs by an 11-member expert panel consisting of otolaryngology—head and neck surgeons, a pediatrician and family physician. Results: Twenty-seven CIs were identified after literature review, with an additional CI developed by the expert panel. After the first round of evaluations, the panel agreed on 4 CIs as appropriate QIs. After an expert panel meeting and subsequent second round of evaluations, the panel agreed on 8 final QIs as appropriate measures of high-quality care. The 8 final QIs focus on topics of antimicrobial management, specialty referral, and tympanostomy tube counseling. Conclusions: Evidence of variable and substandard care persists in the diagnosis and management of pediatric AOM despite the existence of high-quality guidelines. This study proposes 8 QIs which compliment guideline recommendations and are meant to facilitate future quality improvement initiatives that can improve patient outcomes. [ABSTRACT FROM AUTHOR]

The present study investigated the occurrence of seahorses in Araruama hypersaline lagoon using a combination of rapid assessment surveys and data from citizen science. We conducted transects at each of the six study sites, where we recorded a series of population parameters. Local citizen scientists also provided records of occurrence through social media. A female-biased population of Hippocampus reidi Ginsburg, 1933 occupies the lagoon, where it reaches high densities. Most of the seahorses were found attached to artificial holdfasts, and ammonia and nitrite concentrations were above the level recommended by environmental agencies. The reports provided by the citizen scientists represented a valuable contribution to the database on the occurrence of seahorses around the lagoon. Our findings provide new insights into the biological potential of seahorses to colonize new areas and tolerate extreme conditions, as well as the importance of partnerships with citizen scientists to expand ecological knowledge. The primary objective of this single- and multiple-dose pharmacokinetic study was the investigation of rifamycin SV’s pharmacokinetic profile in plasma and urine. All the 18 enrolled healthy men and post-menopausal women received modified release tablets containing 600 mg of the oral non-absorbable antibiotic, rifamycin SV, according to a multiple dose regimen: one tablet three times a day (daily intake: 1800 mg) for 14 consecutive days. Blood sampling and urine collection were performed up to 24 h post-dose after the first dose on Days 1 and 7. On average, on Day 1, Cmax,0–24 was 5.79 ± 4.24 ng/mL and was attained in a median time of 9 h. On Day 7, all the subjects had quantifiable levels of rifamycin SV in plasma at each sampling time. After a peak concentration attained 2 h post-dose (mean ± SD concentration: 10.94 ± 16.41 ng/mL), rifamycin SV decreased in plasma to levels similar to those of Day 1. The amounts of rifamycin SV excreted in urine paralleled the plasma concentration at the corresponding times. On Day 1, the total amount excreted in urine was 0.0013%, and was 0.0029% on Day 7. The study results confirmed those of the previous Phase I study: the systemic absorption of rifamycin SV was also proved negligible after 7 days of the 600 mg t.i.d. dose regimen of the newly formulated tablets, currently under development for the treatment of several small and large intestinal pathologies, including diarrhea-predominant irritable bowel syndrome, hepatic encephalopathy, and others. Marine environments are visual domains restricted regarding light characteristics. Overall, blue monochromatic spectrum prevails in offshore areas especially below 15m depth, since long wavelengths are quickly attenuated. Light intensity is even more constrained in coastal waters, particularly those of tropical estuaries and bays, because further scattering through dissolved and suspended materials. Biofluorescence, which is the ability of organisms to absorb light and reflect it in a different wavelength, has been reported for many marine fish. In this paper, biofluorescence was recorded for the first time for the longsnout seahorse Hippocampus reidi, under natural conditions at Ilha Grande bay, Brazil, and both adult, juvenile and fry individuals kept in captivity. Although displaying the same colour emissions, seahorses differed in relation to body lighting, colour patterns, and age wherein fluorescence occurs. Newborn seahorses exhibit green biofluorescence only in the eyes and stomach. Further experiments are necessary to address whether H. reidi can change the patterns of biofluorescence emission for sensorial and social purposes.

Dinámica de la población del hipocampo en peligro de extinción Hippocampus reidi Ginsburg, 1933 en un arrecife rocoso tropical Este estudio se realizó en Armação de Búzios, en Brasil, un municipio en el que se ha observado la degradación de algunos ecosistemas tras el gran aumento del turismo y el crecimiento demográfico. El objetivo fue determinar las variaciones estacionales de tres poblaciones de Búzios del caballito de mar, Hippocampus reidi. Las inmersiones mensuales se realizaron entre noviembre de 2011 y octubre de 2013. Las tres subpoblaciones tenían baja densidad de caballitos de mar y carecían de estacionalidad. La razón de sexos fue diferente para cada sitio. Los microhábitats más utilizados fueron la esponja Aplysina fulva y el alga Sargassum sp. No hubo diferencias significativas ni en temperatura ni en salinidad. Las tendencias ambientales no pudieron explicar la variación de la densidad de caballitos de mar en las tres playas. La población no mostró estacionalidad y no disminuyó más.

Of 256 cows analyzed, 96% were bacteriologically positive and 80% of the 1,024 quarters were positive for at least one bacterial species. 84.5% of the quarters were healthy with somatic cell counts (SCC) < 200,000 cells/mL, whereas 15.5% of the quarters showed a subclinical mastitis (SCC = 200,000 cells/mL). We could assign 1,288 isolates to 104 different bacterial species including 23 predominant species. Non-aureus staphylococci and mammaliicocci (NASM) were most prevalent (14 different species; 73.5% quarters). Staphylococcus xylosus and Mammaliicoccus sciuri accounted for 74.7% of all NASM isolates. To describe the intramammary resistome, 350 isolates of the predominant species were selected and subjected to short-read whole genome sequencing (WGS) and phenotypic antibiotic resistance profiling. While complete genomes of eight type strains were available, the remaining 15 were de novo assembled with long reads as a resource for the community. The 23 complete genomes served for reference-based assembly of the Illumina WGS data. Both chromosomes and mobile genetic elements were examined for antibiotic resistance genes (ARGs) using in-house and online software tools. ARGs were then correlated with phenotypic antibiotic resistance data from minimum inhibitory concentration (MIC). Phenotypic and genomic antimicrobial resistance was isolate-specific. Resistance to clindamycin and oxacillin was most frequently observed (65 and 30%) in Staphylococcus xylosus but could not be linked to chromosomal or plasmid-borne ARGs. However, in several cases, the observed antimicrobial resistance could be explained by the presence of mobile genetic elements like tetK carried on small plasmids. This represents a possible mechanism of transfer between non-pathogenic bacteria and pathogens of the mammary gland within and between herds. The-to our knowledge-most extensive bacteriome reported and the first attempt to link it with the resistome promise to profoundly affect veterinary bacteriology in the future and are highly relevant in a One Health context, in particular for mastitis, the treatment of which still heavily relies on antibiotics. Objective: To determine risk factors for surgical site infection (SSI) after cochlear implantation (CI) in pediatric patients. Study Design: Case‐control study. Setting: A total of 150 hospitals contributing data to the ACS‐NSQIP Pediatric database (American College of Surgeons National Surgical Quality Improvement Program) in North America and worldwide. Methods: Pediatric patients (aged <18 years) undergoing CI during the years 2012 to 2017 were identified in the ACS‐NSQIP Pediatric database. Uni‐ and multivariable logistic regression analyses were used to determine the odds ratios (ORs) of SSI (including superficial incisional, deep incisional, organ/space) occurring up to 30 days postoperatively. Results: A total of 79 SSIs occurred over a 5‐year period (n = 5146). Longer operative time significantly increased the odds of SSI (OR, 1.965; 95% CI, 1.205‐3.289). Younger age was also found to raise the odds of SSI, with decreased odds associated with each 6‐month increase in age (OR, 0.887; 95% CI, 0.814‐0.958). Conclusion: Longer operative time and younger age appear to significantly increase the odds of SSI in pediatric CI. Body mass index, recent steroid use, American Society of Anesthesiologists class, bilateral vs unilateral implantation, and hospital length of stay do not appear to significantly influence SSI risk. These findings must be interpreted in the context of the limitations inherent to adverse events reporting, which are mitigated by the stringent manner of data collection by the ACS‐NSQIP, and those inherent to the definition of SSI. Future prospective studies should investigate the impact of reducing operative time on the risk of SSI and other complications in pediatric CI. [ABSTRACT FROM AUTHOR]

One of the consequences of climate change is an increase in the temperature of the oceans, which is considered to be one of the greatest impacts on biodiversity. Fish may respond to this impact in several ways, including shifts in their patterns of occurrence. The present study investigated the variation in the structure of a H. reidi population between 2015 and 2017 in the northern Guaíba Island area, highlighting a possible relationship to thermal anomaly associated with the El Niño phenomenon. The seahorse population monitoring was performed monthly, recording sex ratio, abundance, juvenile and adult proportion, depth of occurrence, total length and the holdfast which the seahorse were found attached. The influence of the El Niño event on the study population was evaluated by the correlation of the thermal anomaly data reported for the Tropical South Atlantic Index. Seahorse density on northern Guaíba island was positively and significantly correlated with water temperature, but the sex ratio and number of juveniles were not. The diversity of holdfasts used increased over the study period and was inversely proportional to the thermal anomaly. These results suggest that the thermal anomalies caused by the El Niño in the South Atlantic might trigger migration behaviour in the study species, providing a large aggregation during that period in Guaíba island. Intersex has been reported regularly in wild populations of gonochoristic fish. The authors investigated the possible feminization of seahorses in a population of Hippocampus reidi in Brazil, as well as report a case of a brood pouch reduction after reproductive stimuli. The female seahorses of this population showed two kinds of morphological anomalies. The proportion of the animals affected in the study population was consistent and was uncorrelated with the sex ratio. The study indicates the changes in the sexual profile of the individual. Purpose: A novel fixed-dose combination of 150 mg of econazole with 6 mg of benzydamine formulated in vaginal ovules was investigated in a randomised, double-blind, four-parallel group, tolerability, and pharmacokinetic Phase I study in healthy women.Methods: The fixed-dose combination was compared to econazole and benzydamine single-drug formulations and with placebo after daily applications for 3 consecutive days. Safety and tolerability were evaluated recording the adverse drug reactions, local and general tolerability scores, clinical laboratory assays, and vital signs. Econazole, benzydamine, and its metabolite benzydamine N-oxide pharmacokinetics were investigated after single and multiple applications.Results: Local reactions were generally absent. Pruritus and pain at the application site were infrequently reported. According to the subjects' evaluations, the overall tolerability of the ovules was rated as excellent or good. No significant effect of any treatment on laboratory parameters, vital signs, body weight, vaginal pH, or ECG was observed. Very low econazole, benzydamine, and benzydamine-N-oxide concentrations were measured in plasma, though quantifiable in almost all samples.Conclusion: The tested fixed-dose combination showed a good safety profile consistently with the known tolerability of both active substances. In addition, the confirmed low bioavailability of the drugs excludes the possibility of any accumulation effects and limits the risk of undesired systemic effects. The article reports that A 16-year-old female presented to the Emergency Department with a tongue laceration. She was playfighting with her brother who swung a samurai sword and cut her tongue as she tried to dodge it. She denied headaches, constitutional symptoms, weakness, or loss of consciousness.
