Highlights This study investigates the unknown prognostic influence of peripheral blood CD4+ T lymphopenia in non-small cell lung cancer (NSCLC) with long-term follow-up CD4+ but not CD8 + T lymphopenia was associated with poor survival in patients with localized NSCLC The CD4+ T lymphopenia appears to be an independent prognostic factor for poor overall survival in local/loco-regional NSCLC. CD4+ T lymphocyte count ≤500/μL in peripheral blood is associated with a high risk of death in NSCLC
