Abstract

We used a two-phased mixed methods study with an explanatory sequential design to understand how frequently paramedics attend patients who, on paramedic assessment with the Gold Standards Framework Proactive Identification Guidance, are end-of-life and have advance care planning. We subsequently explored paramedic views on paramedic screening of patients to assess if they are end-of-life and onward referral to their General Practitioner for advance care planning. Paramedics screened and recorded 14.9% of patients as end-of-life and 44.3% of these patients were assessed to have no advance care plan in place. When paramedics screened patients and they did have an advance care plan in place, 36.8% had only a Do Not Attempt Cardiopulmonary Resuscitation. Paramedics found using the Gold Standards Framework Proactive Identification Guidance to screen patients for end-of-life status useful and straightforward and considered themselves well-placed to complete this task. Future research is required to address the practicalities of implementing a paramedic screening and referral tool for end-of-life care that results in the intended outcome of supporting effective advance care planning. Abstract

This article discusses the optimal surgical approach for the treatment of functional mitral regurgitation (FMR). The study analyzed 390 patients who underwent mitral valve surgery and compared the outcomes of mitral valve repair versus replacement. The results showed that younger patients (<65 years) with an ischemic origin of FMR, poor ejection fraction, severe left ventricle remodeling, and atrial fibrillation had a significantly higher mortality risk if they underwent mitral valve replacement. Therefore, the study suggests that a tailored treatment approach should prioritize mitral valve repair in younger patients with FMR. [Extracted from the article]

Abstract

The neutron time of flight (n_TOF) facility at CERN is a neutron spallation source, its white neutron energy spectrum ranges from thermal to several GeV, covering the full energy range of interest for nuclear astrophysics, in particular for measurements of the neutron capture cross-section required in s-process nucleosynthesis. This contribution gives an overview on the astrophysical program made at n_TOF facility, the results and the implications will be considered.

Abstract

The purpose of this pilot study was to investigate the effects of the transfusion of one erythrocyte concentrate on the number of circulating red blood cell extracellular vesicles (RBC‐EVs) and their clearance time. Six, healthy volunteers donated their blood and were transfused with their RBC concentrate after 35–36 days of storage. One K2EDTA and one serum sample were collected before donation, at four timepoints after donation and at another six timepoints after transfusion. RBC‐EVs were analyzed on a Cytek Aurora flow cytometer. A highly significant increase (p < 0.001) of RBC‐EVs from an average of 60.1 ± 19.8 (103/μL) at baseline to 179.3 ± 84.7 (103/μL) in the first 1–3 h after transfusion could be observed. Individual differences in the response to transfusion became apparent with one volunteer showing no increase and another an increased concentration at one timepoint after donation due to an influenza infection. We concluded that in an individualized passport approach, increased RBC‐EVs might be considered as additional evidence when interpreting suspicious Athletes Biological Passport (ABPs) but for this additional research related to sample collection and transport processes as well as method development and harmonization would be necessary. Abstract

The laboratory axolotl (Ambystoma mexicanum) is widely used in biological research. Recent advancements in genetic and molecular toolkits are greatly accelerating the work using axolotl, especially in the area of tissue regeneration. At this juncture, there is a critical need to establish gene and transgenic nomenclature to ensure uniformity in axolotl research. Here, we propose guidelines for genetic nomenclature when working with the axolotl. Key Findings: Axolotl has become the favored model organism for tetrapod tissue regeneration.
