2. Psychotherapeutic research and practice: Do we need to talk about 'efficacy'?

In this article, we address how standardisation in psychotherapeutic research and practice can create a 'procrustean bed' which leads to conformity rather than an ethical response to what emerges in the moment. Articulated, three points of conflict in which we discuss how the therapist or researcher may be rendered 'ineffective' in the name of 'efficacy'. We explore some of the implications for idiosyncratic practices in psychotherapy, which are rooted in a foundation of ethics and philosophy, when the paradigm of empirically 'evidence-based practices' (EBP), which are rooted in measurement, takes centre stage and are overwhelmingly favoured in the industry of funded therapies. The first point of conflict is in relation to conversation itself; exploring the place of hermeneutics and interpretation within therapeutic dialogue and research. The second point explores what it means to take up a discovery-oriented stance and the implications this has for 'evidence'; further delving into the way our demand for 'evidence' may be impacting how psychotherapeutic research and practice is carried out. Chronic respiratory diseases like COPD pose a significant global health challenge. Respiratory function tests, such as F/V Spirometry, are crucial for diagnosis and management but require active patient cooperation. Lack of standardized explanations can cause inefficiencies and increased costs. This project proposes using tutorial videos in waiting rooms to improve understanding of respiratory procedures and device use, reducing execution times and enhancing diagnostic quality. Patients will be divided into two groups: one receiving nurse explanations, the other viewing tutorial videos. Expected outcomes include reduced maneuver execution times and shorter clinic stays. Il contributo ragiona sulle direttive recentemente adottate dal legislatore europeo per contrastare il fenomeno dell’ambientalismo di facciata e rafforzare la tutela di consumatori e operatori economici, nell’ambito di una più ampia strategia volta al raggiungimento di una economia sempre più sostenibile. La lettura combinata dei due provvedimenti, seguendo anche le modifiche apportate nel corso del processo di approvazione definitiva, consente di evidenziare alcuni aspetti peculiari della tecnica legislativa europea che, nel voler dettare criteri per armonizzare la disciplina di determinati settori a livello UE, tende ad essere particolarmente minuziosa nella definizione di diritti e oneri e ad abbondare nella emanazione di atti normativi i cui contenuti si sovrappongono parzialmente. I rischi sono quelli di creare più dubbi che certezze nei destinatari e di dare origine a prassi applicative frammentate.

Questo lavoro riporta alcuni risultati preliminari di un progetto di ricerca volto a indagare il rapporto tra il benessere percepito e il lavoro personale in psicoterapia della Gestalt. Nello specifico, presentiamo nel seguente articolo, i risultati relativi alla dinamica e ai cambiamenti che alcuni pazienti, che hanno liberamente partecipato alla ricerca, hanno mostrato riguardo al proprio senso di benessere, costrutto qui indagato attraverso l’uso di una scala autovalutativa nella sua versione adattata e standardizzata per la popolazione italiana. METODOLOGIA. Si tratta di un disegno sperimentale longitudinale qualitativo che ha coinvolto cinque pazienti per tre mesi, tutti presi in carico da psicologi psicoterapeuti di approccio Gestalt fenomenologico-esistenziale. I dati sono stati raccolti ogni quattro sedute individuali.

Il presente lavoro vuole costituire una lettura critica della Consensus conference sulle terapie psicologiche per ansia e depressione che è oggi un documento ufficialmente assunto dal Ministero della Salute e dunque fa da riferimento per gli operatori del settore. Questa autorevolezza formale rende opportuno che il documento venga conosciuto e valutato in modo approfondito. Vengono qui analizzati i principali ambiti trattati distinguendo fra la relazione per la giuria e le raccomandazioni assunte dalla giuria stessa. Come vedremo vi è grande distanza fra le due parti come i giudici stessi hanno colto. Ma altri aspetti riteniamo meritino attenzione: l'insufficiente approfondimento sull'infanzia e l'adolescenza, la prospettiva ristretta assunta rispetto al tipo di studi ritenuti utili, le raccomandazioni sulla formazione. Limiti che definiamo gravi e che rendono il documento, pur animato dalle migliori intenzioni, almeno in apparenza, un testo scarsamente convincente che necessita di ampie revisioni per raggiungere standard accettabili. Lo scopo principale del presente elaborato è quello di determinare con precisione la natura e la funzione del concetto di illiceità all'interno della scienza del diritto canonico. Per giungere a tali obiettivi, è necessario evincere la caratteristica del termine dai testi normativi dei due codici di diritto canonico. Inoltre, l'Autore ha messo in evidenza che l'atto illecito non deve essere per forza nullo. L'indagine ha pure dimostrato l'impiego alternativo del termine in altri contesti giuridici. Secondo l'opinione dell'Autore, il corretto intendimento della nozione può essere raggiunto grazie al suo accostamento all'efficacia dell'agire. Lungi dall'essere esaustivi, lo studio documenta l'ambiguità dell'impiego del termine in diversi ambiti del vissuto ecclesiale puntando che il suo corretto luogo interpretativo si attiene all'ambito sacramentale. Questo articolo approfondisce il tema del rapporto tra ricerca in psicoterapia e pratica clinica. Nonostante gli avanzamenti nell’ambito della ricerca negli ultimi venti anni, permangono ancora diversi ostacoli nel processo di disseminazione di tale conoscenza. a tal fine, verranno messe in evidenza le differenze tra l’evidence-based research, volta a determinare quale trattamento terapeutico sia più efficace per i singoli disturbi psichiatrici e la practice-based research, che prova ad ottenere dei risultati più vicini alla pratica professionale quotidiana, focalizzandosi su aspetti come la relazione terapeutica, le caratteristiche del terapeuta e del paziente ed i fattori contestuali. È possibile costruire una pratica clinica che non sia sganciata dalle evidenze scientifiche su vari aspetti del lavoro terapeutico. infine, verrà evidenziato come le terapie psicodinamiche abbiano ottenuto varie prove di efficacia che le rendono comparabili a trattamenti di altri orientamenti, anche se la complessità dello studio della relazione terapeutica in tale setting richiede uno sforzo sempre maggiore per integrare ricerca quantitativa e qualitativa.

The year 2021 will mark the tenth anniversary of the opening of the Pitigliano Hospital Centre for Integrated Medicine, where the first centre for the integration of complementary medicine (homeopathy and acupuncture) with integrated medicine settings has been set up for inpatients and for outpatients suffering from chronic diseases. The Centre also carries out training activities for students following university and professional courses in complementary medicine disciplines. In this article some studies on the effectiveness of integrated care are illustrated, which have also been the subject of scientific publications, and an integrated model of care for patients suffering from chronic diseases is also proposed. This article aimed to explore the experience of 39 patients who continued group psychotherapy in the online setting during the period of restrictions due to the pandemic. Through a structured questionnaire, with both open and closed answers, different areas were investigated: the setting, which includes the initial reactions to the need for online movement, perception in showing and being able to see the private spaces of the participants and therapists, and the difficulties relating to the maintenance of privacy; clinical efficacy, connected to the positive and negative aspects of the new modality; the therapeutic relationship, which explores the alliance with therapists and the group, as well as emerging representations; and the therapist's management style and group dynamics. A recent paper by Dominguez and Lee (2017) discussed several methodological and statistical errors made by the American Psychological Association (APA) in defining Eye Movement Desensitisation and Reprocessing (EMDR) efficacy for PTSD as weaker than cognitive behavioural therapy (CBT), cognitive processing therapy (CPT), cognitive therapy (CT) and exposure therapy. In this commentary, starting from the study of Dominguez and Lee (2017) we are going to express our position and state that the limits which make the results on EMDR treatment efficacy controversial are not merely methodological and statistical but, rather, theoretical. Chronic pain resulting from nervous system injury is, in almost all cases, difficult to treat. In 1995 the first trial on the efficacy of gabapentin for neuropathic pain appeared. In 2004 pregabalin was introduced, approved as adjunctive therapy in epilepsy and the treatment of generalized anxiety disorder and peripheral and central neuropathic pain in adults (for example pain originating from post-herpetic neuralgia, neuropathy associated with neoplastic disease, post-stroke pain or spinal paralysis, polyneuropathy, multineuropathy, mononeuropathy, and painful diabetic neuropathy). Studies comparing the two drugs indicate a greater power of action of pregabalin with a dose efficacy ratio of 3 to 1. This allows pregabalin to be released at lower doses without the need of lengthy initial titration to achieve therapeutic range. The bioavailability of pregabalin is constant while that of gabapentin decreases with the dose; gabapentin bioavailability at 1600 mg is only 27% while for pregabalin bioavailability is > 90% at all doses. Pregabalin in direct comparison has proved clinically more effective than gabapentin and is also effective in patients unresponsive to gabapentin. Gabapentin absorption takes place only in the small intestine while that of pregabalin even in the ascending portion of the colon. This leads to a higher risk of gabapentin interactions with drugs affecting small intestine motility, such as opioids, often associated in pain therapy. For the same reason, patients with bowel resections benefit best from treatment with pregabalin.

Physical therapy modalities are widely used in musculoskeletal rehabilitation, particularly to reduce painful symptoms. This would be accomplished through their action on local inflammation and specific stimulation of the cellular metabolism by means of different forms of energy. Spinal pain is one of the most prevalent musculoskeletal disorders. Numerous papers have studied the efficacy of physical modalities such as therapeutic ultrasound, laser therapy and TENS, in comparison to placebo. Objectives:Group-based Cognitive–Behavioral Therapy of children with aggressive behavior has resulted in significant reductions of behavior problems with small to medium effect sizes. We report the efficacy of an individualized Treatment Program for Children with Aggressive Behavior.Method:A within-subject design with two phases (waiting, treatment) was chosen. Sixty boys aged 6–12 years with peer-related aggressive behavior were included. The course of the outcome measures (growth rates) during a 6-week waiting phase was compared with those in the subsequent treatment phase (24 weekly child sessions together with an average of 8 parent contacts) by multilevel modeling. Primary outcome was peer-related aggressive behavior rated by parents. Further outcome measures included parent ratings and patient self-reports of aggressive and prosocial behavior.Results:During the treatment, growth rates for all parent-rated outcome measures were significant (p < .001) and comparison with the waiting phase indicated a stronger decrease in aggressive behavior and a stronger increase in prosocial behavior. For all self-rated outcome measures, growth rates during the treatment were significant (p < .01), but comparison with the waiting phase indicated a stronger decrease only for disturbance of social interaction.Conclusions:The treatment program is an effective intervention for children with peer-related aggressive behavior. OBJECTIVE: Allergic rhinitis (AR) is a disease with high and increasing prevalence. The management of AR includes allergen avoidance, anti-allergic drugs, and allergen specific immunotherapy (AIT), but only the latter works on the causes of allergy and, due to its mechanisms of action, modifies the natural history of the disease. Sublingual immunotherapy (SLIT) was proposed in the 1990s as an option to traditional, subcutaneous immunotherapy. MATERIAL AND METHODS: We reviewed all the available controlled trials on the efficacy and safety of SLIT. RESULTS AND CONCLUSION: Thus far, more than 60 trials, globally evaluated in 6 meta-analyses, showed that SLIT is an effective and safe treatment for AR. However, it must be noted that to expect clinical efficacy in the current practice SLIT has to be performed following the indications from controlled trials, that is, sufficiently high doses to be regularly administered for at least 3 consecutive years.

The use of a complementary medicine approach, and specifically of the popular medicine Oscillococcinum, for the treatment of influenza-like syndromes remains controversial. This brief paper analyses the currently available literature on this homeopathic preparation and the Cochrane Collaboration’s 2006 systematic review, along with other recent studies, in order to clarify certain fundamental aspects of its use in the treatment of influenza. In the light of the reported findings, and applying the rigorous criteria of evidence-based medicine, we suggest that this medicine should be placed in category "BI".

Objective:Up to 50% of psychotherapeutic treatments end without significant improvements. While there is first evidence about predictors of nonresponse in outpatient psychotherapy, there are currently no studies investigating predictors of nonresponse in inpatient settings. Based upon a previous systematic literature review, we analyzed the predictive value of initial patient characteristics on nonresponse in symptom distress.Methods:Treatment episodes from 546 patients, treated for at least 4 weeks, were assessed under naturalistic conditions. Nonresponse status (i.e., lack of a reliable improvement in symptom distress) was investigated at four different time points: at week 4, at discharge, and at a two follow-ups (3 and 12 months after discharge). Hierarchical binary logistic regression models were used to predict nonresponse. Sociodemographic data, clinical variables, and the previous response status were entered subsequently in the model.Results:A moderate or functional level of initial symptom distress, a comorbid personality disorder, and previous nonresponse were the most consistent predictors of nonresponse.Conclusions:The results point to the importance of early outcome assessment and suggest the implementation of more symptom-specific treatments. 39. Efficacy of systemic therapy on adults with mental disorders: A meta-analysis.

Objective: To evaluate the efficacy of systemic therapy on psychiatric disorders in adulthood. Methods: This meta-analysis integrates results of 37 randomized controlled trials (RCT) of therapy with an explicit systemic focus on adults with psychiatric disorders. Studies were identified through systematic searches in electronic databases and cross-referencing. Results: On average, systemic therapy had stronger short-term (g =.51) and long-term (g =.55) efficacies than control groups without alternative treatment and stronger short-term effects than alternative active treatments (g =.25). In addition, efficacy of systemic therapy was similar to those of other bona fide psychotherapies. Individuals receiving systemic therapy plus medication showed stronger improvements at posttest (g =.71) and follow-up (g =.87) than those receiving only medication. Illness-specific analyses showed positive short-term efficacy of systemic therapy on eating disorders, mood disorders, obsessive–compulsive disorders, schizophrenia, and somatoform disorders. At follow-up, efficacy of systemic therapy was only found on eating disorders, mood disorders, and schizophrenia. In addition, systemic therapy had lower dropout rates than alternative treatments. For certain comparisons, effect sizes were moderated by participant age, study quality, and year of publication. Conclusions: We conclude that the present meta-analysis found some evidence for the efficacy of systemic therapy on five disorders, but the number of available RCT is still limited. More research is needed on systemic therapy of other disorders, such as anxiety disorders and substance use disorders. [ABSTRACT FROM AUTHOR]

41. Acceptance and Commitment Therapy e Terapia Cognitivo- Comportamentale a confronto: una revisione della letteratura.

During the last decade a considerable number of studies have been conducted showing that Acceptance and Commitment Therapy is an effective treatment for a wide range of psychological disorders. However, the debate about the empirical status of Acceptance and Commitment Therapy and its different characteristics compared to Cognitive Behavioural Therapy remains controversial and open. This study aims to shed some light on the issue by conducting a systematic review of the studies that have compared Acceptance and Commitment Therapy and Cognitive Behavioural Therapy. Fifteen studies were identified using the databases PubMed and Scopus in November 2014. Studies were included in the review if they compared individual treatments focused on mood and anxiety disorders. The two treatments were equivalently effective at post-treatment according to measures of symptoms and global functioning. Some differences emerged in long-term outcomes. Studies that have investigated mediating mechanisms have produced mixed results. Whereas some studies have found evidence that Acceptance and Commitment Therapy works using different processes from Cognitive Behavioural Therapy, other studies have showed that the two treatments shared similar mechanisms of action. Viene ricordato come storicamente si tenda a sovrastimare l’effetto positivo delle tecnologie sull’apprendimento; la ricerca ha invece rilevato i limiti della loro efficacia (da ricercare in determinate aree) e la rilevanza del rischio del sovraccarico cognitivo che la loro introduzione può comportare. Si propongono alcuni suggerimenti per i decisori scolastici rispondendo alle due domande “Quali criteri per la politica tecnologica?”, “In che modo usare le tecnologie per apprendere a scuola?”.

L'agire secondo i principi di appropriatezza, efficacia e sostenibilità, rappresenta l'impostazione corretta per un management efficace ed efficiente delle principali patologie soprattutto in ambito internistico con pazienti polipatologici e complessi. È fondamentale far crescere tra gli internisti un ragionamento critico alla luce di conoscenze e evidenze su test e procedure mediche che possono non essere necessarie o addirittura dannose, stimolandolo ad evitare quelle decisioni che non porterebbero tangibili vantaggi per il paziente. Necessario coinvolgere le altre componenti sanitarie (altri specialisti, infermieri, ecc.), i livelli dirigenziali e soprattutto dei pazienti. Research on effectiveness shows a widely accepted, significant and large effect of psychotherapy. Few published process studies exist for adolescents, due to complexity of the subject, and the absence of instruments sensitive enough to empirically capture its nuances. Minotauro have developed a therapeutic model for adolescents, integrating psychoanalysis and developmental psychopathology. Specific interventions for antisocial behaviors, suicidal attempts, eating disorders, learning problems, interpersonal withdrawal, internet addiction have been deepened. We used Adolescent Psychotherapy Q-Set, previously used to capture prototypical aspects of different treatments (psychoanalysis, psychodynamic psychotherapy, CBT, MBT) to describe this model. Using the APQ, 30 psychotherapists (M = 14 years of experience) from Minotauro were asked to rate their 'actual' practice with adolescents. To study typical features of developmental psychotherapy we used descriptive statistics. To explore similarities between developmental psychotherapy and other approaches we used Spearman correlations. To investigate different therapist responses we used Q-Factor analysis. Results showed that APQ is a meaningful tool for describing the adolescent psychotherapy process and for identifyng similarities between different therapeutic approaches. Limits and perspectives of the present study regarding instruments' improvement (i.e. addictional items for APQ) and the need to link this methology to "real" clinical practice are also discussed. This study aims to perform a quantitative synthesis of research literature yielding empirical evidence of the efficacy of System Training for Emotional Predictability and Problem Solving (STEPPS; Blum, Bartels, John and Pfohl, 2002) in the treatment of Borderline Personality Disorder (BPD). A search in the SCOPUS and PsychInfo databases yielded 7 studies (4 longitudinal, «open label» and 3 RCT), which provided data concerning the efficacy of STEPPS in reducing BPD symptom severity, as well as in reducing self-report depressive and general psychiatric symptoms. Although heterogeneity statistics were highly significant, cumulative effect size estimates (Cohen d) evidence high efficacy of STEPPS in reducing the severity of both self-reported (... = 0.91) and observer-rated (... = 1.24) BPD symptoms, as well as self-reported depressive symptoms (... = 0.89); STEPPS showed moderate cumulative efficacy in reducing self-reported general psychiatric symptoms (... = 0.65). Although the number of available studies on STEPPS efficacy was at best moderate, the results of our meta-analysis support the clinical utility of STEPPS in treating BPD. [ABSTRACT FROM AUTHOR]
