Abstract

International audience; INTRODUCTION Catheter ablation is used to treat ventricular tachycardia (VT). It uses radiofrequency energy to destroy a small part of heart tissue that is causing rapid and irregular heartbeats. Automated localization of VT exit sites can facilitate the long and often challenging ablation procedures but current methods are not accurate enough, cannot be used in some conditions, and often require detailed information about the patient's anatomy. The aim of this study was to optimize the accuracy of a previously proposed computer-based method for localization of arrhythmia exit sites. The effectiveness of the method was tested using simulated ECG data. We looked for optimal settings of the method allowing to apply it in clinical conditions.METHODS The proposed algorithm works on any set of 3 or more ECG leads. The QRS complex integral (QRSi) of an ectopic beat is reduced to principal components (PCs) treated as coordinates of the exit site in ECG space and then projected to real space by a linear transformation based on a small number of QRSis paced at known locations. The accuracy of the method was tested on 8 patient-tailored models of the human heart and torso. For each model ~500 simulations were run, each for a different stimulus location. A set of training points was randomly chosen and all other locations were then estimated from simulated surface ECGs. The absolute and relative (to a neighboring stimulation site) localization errors (in mm) were computed for a 252-lead ECG, and Frank VCG and using different numbers of training points and principal components.RESULTS The localization error depended on the size of the training set. By using patient’s mean transform matrix of stimulus position from ECG space to real space and Frank XYZ leads we found 15.5 ± 6.4 mm of mean absolute error. Starting from 9 pacing positions available and 3 PCs used we reached a similar level of mean error (15.22 ± 3.5 mm). With 20 stimulus points available and 7 PCs we got 10 ± 2 mm of error. Added noise had no significant influence on the results; even a 2 dB signal/noise ratio increased the error by only 1 mm.DISCUSSION This study suggests that the proposed method can predict exit sites with a precision in the order of a centimeter. By dynamically switching the settings of the algorithm it is possible to obtain better accuracy.

Furthermore, it cannot represent gap junctions accurately. We introduce a microscopic model. We prove the existence of a solution of this model.

Abstract

International audience; Computational modeling and simulation is extensively used to investigate diseases in cardiac electrophysiological activity and also drug effects, side effects and interactions. Human embryonic stem cell-derived cardiomyocytes (hESC-CMs) have been recently considered as a promising tool in regenerative medicine: their major role in repairing damaged tissue is due to pluripotency and ability to differentiate. These pluripotent cells are also used in early stages of drugs development. Pharmaceutical companies use the MultiElectrode Array (MEA) device in order to perform many in vitro experiments on hESC-CMs. The goal of our study is to derive a mathematical model and to simulate these in vitro experiments.

Abstract

Microscopic alterations of the heart tissue (fat, fibre invasion, fibre crossing responsible of non-anisotropic behavior) are responsible of alterations of the electric wave propagation. It leads to macroscopic heart diseases.The purpose of my work is to provide a better understanding of the consequences at the macroscopic scale of these alterations that occur at the microscopic scale.This poster introduces the current work/results of my PhD work as of July 2015.

Abstract

The bidomain model is nowadays one of the most accurate mathematical descriptions of the electrical activity in the heart. From now on, it was believed to be the only model for accurate simulations of cardiac muscle stimulation in a clinically relevant manner, i.e. through extracellular electrodes. In this paper, we develop a computationally efficient and accurate approximation of the bidomain model that allows for extracellular stimulation and accounts for unequal anisotropy ratios between intra-and extra-cellular media: the current-lifted monodomain model. We prove its use in the isolated heart by reproducing four types of extracellular activation that exhibit virtual electrode polarization. The simplicity of the code implementation and the fact that the computational cost is equal to the standard monodomain model henceforth give an excellent alternative to the bidomain model for expensive simulations like arrhythmia, fibrillation, and their respective treatments, e.g. signal guided catheter ablation and defibrillation.

Abstract

International audience; We present a new mathematical model of the electric activity of the heart. In the standard bidomain model we can distinguish the intra- and the extracellular space with different conductivities for excitable cells and the fibrotic tissue around them. The main drawback is that it assumes the existence of excitable cells everywhere in the heart, while it is known that there exist non small regions where fibroblasts take place. The fibroblasts are equally distributed and since they are non excitable cells, they can be considered as a diffusive part. Hence we extend the standard bidomain model as follows: we assume that we have periodic alternation of the healthy tissue (linear bidomain model) and fibrotic extracellular space (diffusive part). We use homogenization techniques to derive our macroscopic partial differential equations. Interestingly, we obtain again a bidomain type model with modified conductivities that involve the volume fraction of the diffusive domain. Preliminary numerical experiments will conclude on the influence of these diffusive inclusions.

Abstract

International audience; OBJECTIVES: The aim of this study was to describe a new familial cardiac phenotype and to elucidate the electrophysiological mechanism responsible for the disease. BACKGROUND: Mutations in several genes encoding ion channels, especially SCN5A, have emerged as the basis for a variety of inherited cardiac arrhythmias. METHODS: Three unrelated families comprising 21 individuals affected by multifocal ectopic Purkinje-related premature contractions (MEPPC) characterized by narrow junctional and rare sinus beats competing with numerous premature ventricular contractions with right and/or left bundle branch block patterns were identified. RESULTS: Dilated cardiomyopathy was identified in 6 patients, atrial arrhythmias were detected in 9 patients, and sudden death was reported in 5 individuals. Invasive electrophysiological studies demonstrated that premature ventricular complexes originated from the Purkinje tissue. Hydroquinidine treatment dramatically decreased the number of premature ventricular complexes. It normalized the contractile function in 2 patients. All the affected subjects carried the c.665G>A transition in the SCN5A gene. Patch-clamp studies of resulting p.Arg222Gln (R222Q) Nav1.5 revealed a net gain of function of the sodium channel, leading, in silico, to incomplete repolarization in Purkinje cells responsible for premature ventricular action potentials. In vitro and in silico studies recapitulated the normalization of the ventricular action potentials in the presence of quinidine. CONCLUSIONS: A new SCN5A-related cardiac syndrome, MEPPC, was identified. The SCN5A mutation leads to a gain of function of the sodium channel responsible for hyperexcitability of the fascicular-Purkinje system. The MEPPC syndrome is responsive to hydroquinidine.

Abstract

International audience; Investigating the causes of failure of Fontan circulation in individual patients remains challenging despite detailed combined inva-sive cardiac catheterisation and magnetic resonance (XMR) exams at rest and during stress. In this work, we use a biomechanical model of the heart and Fontan circulation with the components of systemic and pulmonary beds to augment the diagnostic assessment of the patients undergoing the XMR stress exam. We apply our model in 3 Fontan patients and one biventricular "control" case. In all subjects, we obtained important biophysical factors of cardiovascular physiology-contractil-ity, contractile reserve and changes in systemic and pulmonary vascular resistance-which contribute to explaining the mechanism of failure in individual patients. Finally, we used the patient-specific model of one Fontan patient to investigate the impact of changes in pulmonary vas-cular resistance, aiming at in silico testing of pulmonary vasodilation treatments.
