Abstract

Resume Le TEP scanner au 18F-fluoro desoxy glucose (18F-FDG-PET-CT) a montre son interet dans le diagnostic de la pseudo polyarthrite rhizomelique (PPR) et permet d’evaluer l’activite metabolique de l’ensemble de l’appareil locomoteur et en particulier des structures musculaires. L’objectif de cette etude etait d’evaluer l’atteinte musculaire a l’aide du 18F-FDG-PET-CT dans les cas de PPR, par rapport a une population non-PPR. Methodes Il s’agit d’une etude retrospective monocentrique de patients atteints de PPR (criteres ACR/EULAR 2012) qui ont eu un examen au 18F-FDG-PET-CT. Un groupe « temoin » compose de sujets sans manifestations rhumatologiques ayant eu un tel examen dans le cadre d’une recherche de neoplasie ou d’un suivi de maladie neoplasique a egalement ete evalue. L’evaluation TEP comprenait 17 sites suggerant une PPR, comme indique precedemment. Les zones d’hypermetabolisme musculaire ont ete classees de la meme facon selon la meme classification semi quantitative. Les sites d’activite musculaire ont ete identifies. Une comparaison des patients atteints de PPR avec et sans lesion musculaire a ete effectuee a l’aide du test exact de Mann–Whitney ou de Fisher. Resultats Deux cent un cas ont ete examines, impliquant 101 PPR (âge moyen de 68,6 ans) et 100 « temoins » (âge moyen de 67,7 ans). Dans l’ensemble, des lesions musculaires par TEP ont ete observees dans 34 cas (34 %) chez les PPR et 10 cas (10 %) chez les temoins (p = 0,004). Les lesions sont bi ou multifocales dans la moitie des cas. Les sites musculaires affectes sont : les muscles spinaux 19, la ceinture scapulaire 14, la ceinture pelvienne 13, la cuisse 6. Une fasciite a ete retrouvee dans 3 cas. Chez les patients atteints de PPR, l’atteinte musculaire TEP n’etait pas associee a l’âge, a la CRP ou au score TEP global de la PPR. Conclusion Les lesions musculaires evaluees par TEP-CT au 18F-fluorodesoxyglucose sont courantes dans la PPR (1/3 des cas), situees aux sites habituels des symptomes de la maladie, sans association avec l’âge, les niveaux de CRP ou le score global de la PPR par TEP. Le muscle doit etre evalue avec soin lors d’un examen TEP dans les cas de PPR.

Abstract

International audience; Purpose: This study aimed at determining the diagnostic implications of indirect signs of infection at FDG-PET-i.e., hypermetabolisms of the spleen and/or bone marrow (HSBM)-when documented in patients with known or suspected infective endocarditis (IE).Methods: HSBM were defined by higher mean standardized uptake values comparatively to that of the liver on FDG-PET images from patients with a high likelihood of IE and prospectively included in a multicenter study.Results: Among the 129 included patients, IE was ultimately deemed as definite in 88 cases. HSBM was a predictor of definite IE (P = 0.014; odds ratio (OR) 3.2), independently of the criterion of an abnormal cardiac FDG uptake (P = 0.0007; OR 9.68), and a definite IE was documented in 97% (29/30) of patients showing both HSBM and abnormal cardiac uptake, 78% (7/9) of patients with only abnormal cardiac uptake, 67% (42/63) of patients with only HSBM, and 37% (10/27) of patients with neither one.Conclusion: In this cohort with a high likelihood of IE, HSBM is an additional albeit indirect sign of IE, independently of the criterion of an abnormal cardiac uptake, and could reinforce the suspicion of IE in the absence of any other infectious, inflammatory, or malignant disease.

Abstract

Resume Introduction Depuis le plan cancer, l’evaluation des pratiques professionnelles est indispensable pour s’assurer de la mise en œuvre de soins de haute qualite. Evaluer la satisfaction des patients est un des piliers pour des soins de haute qualite. L’objectif principal de l’etude etait d’evaluer la satisfaction de patientes atteintes d’un cancer du sein de stade precoce traitees par hormonotherapie, l’objectif secondaire etant d’identifier les facteurs associes a la satisfaction. Methode Le questionnaire EORTC OUT-PATSAT 35 modifie a ete envoye a un echantillon de patientes franc-comtoises afin d’evaluer neuf dimensions de satisfaction parmi lesquelles les qualites relationnelles, les informations fournies, la satisfaction globale. Pour chaque dimension, un score de satisfaction compris entre 0 (satisfaction nulle) et 100 (satisfaction elevee) a ete mesure. Des modeles de regression logistique ont permis d’etudier les facteurs associes a la satisfaction. Resultats Le score moyen de satisfaction globale des 280 patientes ayant repondu etait de 73 [0–100]. Pratiquer une activite extra-professionnelle etait associe a une satisfaction plus elevee pour plusieurs dimensions (odds ratio entre 2,80 et 4,12 ; p Discussion Les patientes etaient satisfaites et plusieurs facteurs impactant leur satisfaction ont ete identifies, sur la base d’un questionnaire qui doit evoluer pour prendre en compte l’aspect ambulatoire de leur prise en charge. Lors des consultations, une attention particuliere sera portee sur ces facteurs.

Abstract

Background Epithelioid peritoneal malignant mesothelioma (EPMM) is the most common subtype of this aggressive tumor. We compared two antibodies against PD-L1, a recent theranostic biomarker, and evaluated the prognostic value of PD-L1 expression by mesothelial and immune cells in EPMM. Methods Immunohistochemistry was performed on 45 EPMM. Clinical and pathological data were extracted from the RENAPE database. Using E1L3N and SP142 clones, inter-observer agreement, PD-L1 expression by mesothelial and immune cells and inter-antibody agreement were evaluated. The prognostic relevance of PD-L1 expression was evaluated in 39 EPMM by univariate and multivariate analysis of overall survival (OS) and progression-free survival (PFS). Results Inter-observer agreement on E1L3N immunostaining was moderate for mesothelial and immune cells, and fair for mesothelial and poor for immune cells using SP142. Using E1L3N, 31.1% of mesothelial and 15.6% of immune cells expressed PD-L1, and 22.2% of mesothelial and 26.7% of immune cells using SP142. Inter-antibody agreement was moderate. In most positive cases, 1–5% of tumor cells were positive. Using E1L3N, PD-L1 expression by lymphocytes was associated with better OS and PFS by both univariate and multivariate analysis. Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy predicted better prognosis than other treatments. Solid subtype was an independent prognostic factor for worse OS. Conclusion E1L3N appeared easier to use than SP142 to evaluate PD-L1 expression. A minority of EPMM expressed PD-L1, and only a few cells were positive. PD-L1 expression by immune cells evaluated with E1L3N was an independent prognostic factor in EPMM.

Abstract

International audience; Epidemiological studies have demonstrated that most humans infected with Echinococcus spp. exhibit resistance to disease. When infection leads to disease, the parasite is partially controlled by host immunity: in case of immunocompetence, the normal alveolar echinococcosis (AE) or cystic echinococcosis (CE) situation, the metacestode grows slowly, and first clinical signs appear years after infection; in case of impaired immunity (AIDS; other immunodeficiencies), uncontrolled proliferation of the metacestode leads to rapidly progressing disease. Assessing Echinococcus multilocularis viability in vivo following therapeutic interventions in AE patients may be of tremendous benefit when compared with the invasive procedures used to perform biopsies. Current options are F18-fluorodeoxyglucose-positron emission tomography (FDG-PET), which visualizes periparasitic inflammation due to the metabolic activity of the metacestode, and measurement of antibodies against recEm18, a viability-associated protein, that rapidly regresses upon metacestode inactivation. For Echinococcus granulosus, similar prognosis-associated follow-up parameters are still lacking but a few candidates may be listed. Other possible markers include functional and diffusion-weighted Magnetic Resonance Imaging (MRI), and measurement of products from the parasite (circulating antigens or DNA), and from the host (inflammation markers, cytokines, or chemokines). Even though some of them have been promising in pilot studies, none has been properly validated in an appropriate number of patients until now to be recommended for further use in clinical settings. There is therefore still a need to develop reliable tools for improved viability assessment to provide the sufficient information needed to reliably withdraw anti-parasite benzimidazole chemotherapy, and a basis for the development of new alternative therapeutic tools.

Abstract

Objectives To compare 18 f-Fluoro-Dexoxyglucose Positron Emission Tomography (FDG-PET/CT) findings in patients with polymyalgia rheumatica (PMR) and controls without rheumatologic disease. Methods We have retrospectively included 50 patients with a diagnosis of PMR (EULAR/ACR criteria), who have had a FDG-PET/CT. For comparison, 53 patients who have had a FDG-PET/CT for initial staging or follow-up of neoplasm have been included as controls. 17 sites have been analyzed (2 shoulders, 2 acromio-clavicular and 2 sterno-clavicular joints, 2 greater trochanters, 2 hips, 2 ischial tuberosities, 2 iliopectinal bursitis, 2 pubic symphysis enthesis and only the interspinous space with the most FDG uptake). An FDG uptake score has been used (semi-quantitative scoring system, by Goerres and al. to evaluate visually FDG uptake (0 to 3 compared to liver uptake) into these sites. We calculated the FDG uptake score corresponding to the mean of intensity over all sites, and the number of sites with significant uptake (FDG uptake 2 or 3) for each patient. The cut offs for number of sites with high activity and for FDG uptake score were assessed using ROC curves and Odds Ratios. Results The two groups were comparable for the median patient age (69.3 years for PMR vs. 68.1 for controls). Significant differences between the two groups were found for FDG uptake score (1.12 vs 0.34, p The presence of 3 or more sites with significant uptake was correlated with the diagnosis of PMR with 74% sensitivity, 79% specificity [OR =10.8]. For the FDG uptake score the cut off is 0.53 [Se 80%, Sp 77%; OR 13.6]. We found significant differences in all sites for FDG uptake score, and number of sites with significant uptake, particularly marked for shoulders, ischial tuberosities and interspinous bursitis (p Conclusions We found significant uptake in all sites studied in PMR compared to controls. Abstract

International audience; Human alveolar echinococcosis is a parasitic zoonosis with intermediate (rodents and small lagomorphs) and final (carnivores) hosts. The latter can transmit the parasite to humans, by their feces, which are contaminated by the oncospheres of Echinococcus multilocularis, the larvae of which develop in the liver like a slow cancer. Political, socioeconomic, and ecological factors can affect the intermediate and final hosts and thus influence the long-term emergence and stability of endemic areas. The past 20 years have been marked by: Epidemiologic changes: discovery of an endemic area in China, the largest in the world; extension of the European endemic range tothe east and north; extension of the French endemic range to the west and south; and the settlement of contaminated foxes in urban centers, which will modify the populations at risk over time. Progress in diagnosis: initial use of liver ultrasound to screen for asymptomatic disease; value of magnetic resonance imaging to confirm diagnosis and assess extension of disease before beginning treatment; mastery of immunological and genetic diagnosis; and the introduction of positron emission tomography combined with computed tomography to assess the functional activity of the lesions and monitor treatment. Consensus has defined the main directions of a multidisciplinary treatment approach: radical liver resection when possible; avoidance of palliative surgical procedures; use of interventional radiology or endoscopy to treat biliary and vascular complications, whenever possible; consideration of liver transplantation only when all other treatment possibilities have been exhausted; and treatment of all patients, without exception, by albendazole for at least two years after radical surgery and long term (at least several years) in other cases. In France, survival and quality of life have improved very substantially, due to earlier diagnosis, clinical classification of cases, and rigorous follow-up as part of the FrancEchino network. Better understanding of the immunological and immunogenetic mechanisms that underlie the course of disease may open up the possibility of immunomodulation that could replace or round out the current treatments. These are far from satisfactory, in view of the constraints of prolonged, most often life-long treatment by benzimidazoles and their side effects.

Abstract

We report the 30-yr history of a well-documented human case of alveolar echinococcosis, with a lung lesion at presentation followed by the discovery of a liver lesion, both removed by surgery. Subsequently, within the 13 years following diagnosis, metastases were disclosed in eye, brain and skull, as well as additional lung lesions. This patient had no immune suppression, and did not have the genetic background known to predispose to severe alveolar echinococcosis; it may thus be hypothesized that iterative multi-organ involvement was mostly due to the poor adherence to benzimidazole treatment for the first decade after diagnosis. Conversely, after a new alveolar echinococcosis recurrence was found in the right lung in 1994, the patient accepted to take albendazole continuously at the right dosage. After serology became negative and a fluoro-deoxy-glucose-Positron Emission Tomography performed in 2005 showed a total regression of the lesions in all organs, albendazole treatment could be definitively withdrawn. In 2011, the fluoro-deoxy-glucose-Positron Emission Tomography showed a total absence of parasitic metabolic activity and the patient had no clinical symptoms related to alveolar echinococcosis. The history of this patient suggests that multi-organ involvement and alveolar echinococcosis recurrence over time may occur in non-immune suppressed patients despite an apparently “radical” surgery. Metastatic dissemination might be favored by a poor adherence to chemotherapy. Combined surgery and continuous administration of albendazole at high dosage may allow alveolar echinococcosis patients to survive more than 30 years after diagnosis despite multi-organ involvement.

Abstract

International audience; Liver transplantation (LT) is currently contraindicated in patients with residual or metastatic alveolar echinococcosis (AE) lesions. We evaluated the long-term course of such patients who underwent LT and were subsequently treated with benzimidazoles. Clinical, imaging, serological, and therapeutic data were collected from 5 patients with residual/recurrent AE lesions who survived for more than 15 years. Since 2004, [(18) F]-2-fluoro-2-deoxyglucose (FDG)-positron emission tomography (PET) images were available, and the levels of serum antibodies (Abs) against Echinococcus multilocularis-recombinant antigens were evaluated. Median survival time after LT was 21 years. These patients were from a prospective cohort of 23 patients with AE who underwent LT: 5 of 8 patients with residual/recurrent AE and 4 of 9 patients without residual/recurrent AE were alive in September 2009. High doses of immunosuppressive drugs, the late introduction of therapy with benzimidazoles, its withdrawal due to side effects, and nonadherence to this therapy adversely affected the prognosis. Anti-Em2(plus) and anti-rEm18 Ab levels and standard FDG-PET enabled the efficacy of therapy on the growth of EA lesions to be assessed. However, meaningful variations in Ab levels were observed below diagnostic cutoff values; and in monitoring AE lesions, images of FDG uptake taken 3 hours after its injection were more sensitive than images obtained 1 hour after its injection. In conclusion, benzimidazoles can control residual/recurrent AE lesions after LT. Using anti-rEm18 or anti-Em2(plus) Ab levels and the delayed acquisition of FDG-PET images can improve the functional assessment of disease activity. The potential recurrence of disease, especially in patients with residual or metastatic AE lesions, should not be regarded as a contraindication to LT when AE is considered to be lethal in the short term.
