The continued scaling-up of antiretroviral therapy (ART) in Sub-Saharan Africa provides an opportunity to further study its impact on sexual behaviors among people living with HIV/AIDS (PLWHA). We explored time trend and correlates of sexual activity among PLWHA initiating ART in Cameroon and compared sexual risk behaviors between patients sexually active before and after initiating ART and those resuming sexual activity after ART initiation. Analyses were based on longitudinal data collected within the randomized trial (n = 459) conducted in nine rural district hospitals in Cameroon. Sexual activity was defined as reporting at least one sexual partner during the previous 3 months. Inconsistent condom use (ICU) was defined as reporting to have never, sometimes, or nearly always used condoms at least once with a partner(s) either HIV-negative or of unknown HIV status during the same period. Mc Nemar tests were used to assess time trend, while mixed-effect logistic regressions were conducted to analyze the effect of time since ART initiation on sexual activity. The proportion of sexually active patients significantly increased over time: from 31.8% at baseline to 40.2 and 47.1% after 6 and 12 months of ART, respectively (p=0.001), to 55.9% after 24 months (p=0.02). After adjustment for behavioral and psychosocial factors, time since ART initiation was independently associated with reporting sexual activity (AOR [95% CI] = 1.30 [1.171.46] per 6-month increase, p=0.001). Since the start of the new century, development aid targeted on health care has seen an unprecedented rise, driven by the fight against AIDS. This article shows how this struggle has been accompanied with a renewal of the economic paradigms governing international action in favour of health care in developing countries: the idea that an improvement in health care constitutes an unavoidable prerequisite to macroeconomic growth, rather than a consequence; the insistence on the founding of mechanisms for health insurance to finance the costs of health care, rather than covering the costs at the point of use by the health care users; a concern to impose price differentials for access to medicine in developing countries, and to introduce flexibility in the regulation of international intellectual property law; the priority to vertical programmes targeted on certain illnesses, thought to act as levers for a global reinforcement of health care systems. This article discusses the pertinence of these new paradigms in light of the evolution of the AIDS/HIV epidemic, and the international context. To cite this article: J.R Moatti, C R. This text presents the second part of the book. In the first two decades of the HIV Epidemic, the strategy in developing world was focused on preventive methods. The challlenge is now give HIV treatment access for the benefit of People Living With HIV/Aids (PLWH) and for curbing the epidemic. Experiences in Brazil, Chile, Thailand are implemented to achieve this goal. In some African countries as Cote d'Ivoire, Senegal and Uganda, pilot projects have proved the feasibility and efficency of Antiretroviral treatments in subsaharian countries. These experiences show that a strong involvement of governments to promote or to regulate the delivery of ARV drugs is needed to face an eventual "antiretroviral anarchy". Scaling up access to ART will often take place in a context of failing government systems. The economic arguments against access to HIV treatment in developing countries are critically reviewed and the book brings evidence that treatment is rational as economic decision. Furthermore, the books stands the economic impact of the epidemic is underestimated with a new conceptual approach which highlights the "involution trap" where countries can fall in the long run. Some "optimistic" results of previous models, as a reduced pressure on land and capital, are artefacts. So the main rationale of HIV Treatment is to prevent falling in this trap : more resources must be allocated to scaling up programs. The book shows how countries can mobilize new resources. Every country must find the best balance between public subsidies, private insurance, private fundings and patients participation., Ce texte présente la seconde partie du livre. Dans les deux premières décennies de l'épidémie Sida, la stratégie dans les pays en développement a été centrée sur les méthodes préventives. Le défi maintenant est de permettre l'accès au traitement pour les personnes vivant avec le VIH/SIDA et pour ralentir l'épidémie. Les expériences du Brésil, du Chili, de la Thaïlande ont été réalisées dans ce but. Dans certains pays africains comme la Côte d'Ivoire, le Sénégal, l'Ouganda, des projets pilotes ont démontré la faisabilité et l'efficacité du traitement ARV dans les pays sub-sahariens. Ces expériences montrent qu'un fort engagement des gouvernements pour promouvoir ou réglementer la distribution des médicaments ARV est nécessaire pour éviter une éventuelle "anarchie d'antirétroviraux". La montée en puissance de l'accès à ces traitements se situera souvent dans un contexte de systèmes publics défaillants. Les arguments économiques contre le traitement du sida sont examinés et critiqués, et le livre apporte les preuves que le traitement est une décision économique rationnelle. En outre, le livre soutient la thèse que l'impact économique de l'épidémie a été sous-évalué, avec une approche conceptuelle nouvelle qui souligne le piège d'effondrement dans lequel les pays peuvent tomber à long terme. Certains résultats "optimistes" dans les modèles antérieurs, comme une réduction de la pression sur la terre et sur le capital, sont des artefacts. Il en résulte que le principal argument en faveur du traitement par ARV est de prévenir ce piège : plus de ressources doivent être allouées au passage à l'échelle des programmes. Le livre montre comment les pays peuvent mobiliser de nouvelles ressources : chaque pays doit trouver le meilleur équilibre pour lui entre des financements publics, l'assurance privés, des donations privées et la participation des patients.

The purpose is to analyse the conditions to improve access to drugs against AIDS in developing countries, to urge on international agencies determinedly support patients treatment. The first part deals with the market of antiretrovirals and patents : How the prices drop taked place in 1998-2002, what were the debates about Intellectual Property Rights and WTO, what happened in countries as Brazil and Thailand ? The second part deals with the impact of AIDS epidemic in the developing world and the role of therapeutic strategies to lower this impact. The book backs up the argument that the impact has been undersestimated and theses strategies are feasible.; Cet ouvrage réunit des travaux effectués principalement dans le cadre de l'Agence Nationale de Recherche contre le Sida. Il a pour objectif d'analyser les conditions de l'amélioration de l'accès aux médicaments traitant le Sida dans les pays en développement pour que les pays et les agences internationales s'engagent résolument dans le traitement des malades. La première partie traite du marché des antirétroviraux et de la question des brevets : comment s'est passé la baisse des prix dans les années 1998-2002, quels ont été les débats à propos de la protection des brevets et de l'OMC, que s'est-il passé dans des pays-phares comme le Brésil ou la Thailande ? La seconde partie porte sur l'impact du Sida dans les pays en développement et le rôle de stratégies thérapeutiques pour réduire cet impact. La thèse soutenue est que cet impact a été sous-évalué et que des stratégies sont possibles.

In 1994, the Uruguay Round culminated by the creation of WTO which extends worldwhile patent protection for new products, including drugs. In 1996, an efficent Aids treatment was available in the world market : antiretroviral combined therapy. The coexistence of these two series of events is obviously coincidal, but has fueled intensive debates about their conflictual interaction. A focus is made on the growing tension between the dimension of the collective good of public health and profit-seeking by private firms involved in that domain. The TRIPS Agreement, part of WTO has generated new constraints to face to AIDS epidemic and other public health issues as well.Thailand and Brazil succeeded to manufacture generic drug for the benefit of domestic patients through many international obstacles. A special chapter is devoted to the prices : how are they determined ? how have the main protagosnists in this field been behaving? What can we expect in the future ?, En 1994, l'Uruguay Round s'est achevé avec la création de l'OMC qui étend au monde entier la protection par brevet des innovations, et en particulier des nouveaux médicaments. En 1996, apparaît sur le marché mondial un nouveau traitement du Sida : la trithérapie par ARV. La coïncidence de ces deux séries d'événements est évidemment une coïncidence, mais a alimenté d'intenses débats sur leurs interactions conflictuelles. Est analysée la tension croissante entre la dimension de bien collectif qu'est la santé publique et la recherche du profit par les firmes engagées dans ce domaine. L'accord ADPIC, partie de l'OMC, a créé de nouvelles contraintes pour lutter contre le Sida, et d'autres problèmes de santé publique. La Thaïlande et le Brésil on réussi à fabriquer des médicaments génériques pour traiter les malades de ces pays malgré de nombreux obstacles internationaux. Ce chapitre est consacré aux prix : comment sont-ils déterminés, comment se sont comportés les principaux protagonistes, que pouvons nous attendre dans l'avenir ?

National audience; L'article s'appuie sur une recherche anthropologique menée dans les centres de dépistage anonyme et gratuit du SIDA à Marseille en 1990. Les auteurs montrent que les stratégies de dépistage s'organisent à partir d’événements de vie puis se recroisent avec des informations médiatiques ou médicales. L'intérêt de mieux connaitre les évènements de vie déclenchant la vulnérabilité psychologique au risque semble évident pour atteindre une meilleure efficacité vis à vis des personnes à risque d'infection par le VIH.
