Objectives: This clinical study on therapy of peri-implantitis aimed to compare the marginal bone loss of implants treated with different surgical approaches: implantoplasty and peri-implant resective surgery only., Material and Methods: Over a period of 6 years, 10 patients (20 implants) were treated with implantoplasty (test group) and 9 had resective surgery (control group). A computerized analysis of radiographs was performed to calculate marginal bone loss (MBL) values mesial and distal to the implants. The measurement system was set by means of known implant sizes. Data on MBL were collected at the time of peri-implantitis diagnosis, 1, 2 and 3 years after surgery., Results: There was no difference between the mean MBL values three years after implantoplasty in the test group: 0 and 0.01 mm of MBL mesial and distal to the implant were found (P>0.05). Conversely, the mean MBL values recorded in the control group were statistically different: 1.44 and 1.54 mm of MBL mesial and distal to the implant were found (P<0.05) 3 years after resective surgery. Moreover, the variation of peri-implant marginal bone after peri-implantitis surgical treatment was significantly lower in the test group than in the control group (P<0.05)., Conclusions: The results of this radiographic research suggested that implantoplasty was an effective treatment of peri-implant infections and peri-implantitis progression.

Purpose: The purpose of this prospective study on humans were to evaluate (a) the clinical outcome of alveolar distraction osteogenesis for the correction of vertically deficient edentulous mandibular ridges, (b) the clinical outcome of dental implants placed in the distracted areas, and (c) the quality and quantity of the bone that had formed in the distraction gap., Material and Methods: Seven patients presenting vertically deficient edentulous ridges were treated by means of distraction osteogenesis with an intraoral alveolar distractor. Approximately 3 months after consolidation of the distracted segments, 20 ITI solid screw SLA implants were placed in the distracted areas. Three to 4 months later, abutments were connected and prosthetic loading of the implants started. During implant site preparation, bone biopsies were taken at the implant sites with trephine burrs for histologic and histometric analyses., Results: The mean follow-up after the initial prosthetic loading was 18 months (range 12-24 months). The mean bone gain obtained at the end of distraction was 7 mm (range 5-9 mm). The cumulative success rate of implants 2 years after the onset of prosthetic loading was 95%, whereas the survival rate of implants was 100%. The newly formed bone consisted of woven bone reinforced by parallel-fibered bone with bone marrow spaces between the bone trabeculae. The bone area fraction in the distraction region ranged from 21.6% to 57.8% (38.5+/-11.7%)., Discussion and Conclusions: Results from this study showed that (a) distraction osteogenesis is a reliable technique for the correction of vertically deficient edentulous ridges, (b) the regenerated bone withstood the functional demands of implant loading, (c) survival and success rates of implants placed in the distracted areas were consistent with those of implants placed in native bone, and (d) there is sufficient bone volume and maturity in the distracted region for primary stability of the implant.

Objectives: The aim of this narrative review was to explore the application of digital technologies (DT) for the simplification and improvement of bone augmentation procedures in advanced implant dentistry. Material and methods: A search on electronic databases was performed to identify systematic reviews, meta‐analyses, randomized and non‐randomized controlled trials, prospective/retrospective case series, and case reports related to the application of DT in advanced implant dentistry. Results: Seventy‐nine articles were included. Potential fields of application of DT are the following: 1) the use of intra‐oral scanners for the definition of soft tissue profile and the residual dentition; 2) the use of dental lab CAD (computer‐aided design) software to create a digital wax‐up replicating the ideal ridge and tooth morphology; 3) the matching of STL (Standard Triangulation Language) files with DICOM (DIgital COmmunication in Medicine) files from CBCTs with a dedicated software; 4) the production of stereolithographic 3D models reproducing the jaws and the bone defects; 5) the creation of surgical templates to guide implant placement and augmentation procedures; 6) the production of customized meshes for bone regeneration; and 7) the use of static or dynamic computer‐aided implant placement. Conclusions: Results from this narrative review seem to demonstrate that the use of a partially or fully digital workflow can be successfully used also in advanced implant dentistry. However, the number of studies (in particular RCTs) focused on the use of a fully digital workflow in advanced implant dentistry is still limited and more studies are needed to properly evaluate the potentials of DT. A simplified surgical technique is presented for closure and soft tissue augmentation of fresh extraction sockets that utilizes a xenogeneic collagen matrix as a substitute for a combined onlay-interpositional connective tissue graft. Ten alveoli receiving a socket preservation procedure (n = 5) or an immediate submerged implant (n = 5) were sealed with a xenogeneic collagen matrix, stabilized under small palatal and buccal full-thickness envelope flaps. Eight weeks after surgery, full wound closure was achieved in 9 out of 10 sites with satisfactory esthetic integration (mean ΔE score: 3.76). Results from this descriptive observational study suggest that this technique may represent a valuable and minimally invasive solution to simplify closure and soft tissue augmentation of fresh extraction sockets. Objectives: To present the results of guided bone regeneration (GBR) of atrophic edentulous ridges with customized CAD/CAM titanium meshes. Material and methods: Forty‐one patients, presenting with 53 atrophic sites, were enrolled between 2018 and 2019. GBR was obtained with titanium meshes filled with autogenous bone chips and bovine bone mineral (BBM). After a mean of 7 months (range: 5–12 months), meshes were removed and 106 implants placed. After a mean of 3.5 months (range: 2–5 months), implants were uncovered and prosthetic restorations started. The outcomes were vertical and horizontal bone augmentation changes, biological complications and implant survival. Results: Out of 53 sites, 11 underwent mesh exposure: eight of them were followed by uneventful integration of the graft, while three by partial bone loss. The mean vertical and horizontal bone gain after reconstruction was 4.78 ± 1.88 mm (range 1.00–8.90 mm) and 6.35 ± 2.10 mm (range 2.14–11.48 mm), respectively. At the time of implant placement, mean changes of initial bone gain were −0.39 ± 0.64 mm (range −3.1 to + 0.80 mm) and −0.49 ± 0.83 mm (range –3.7 to +0.4 mm), in the vertical and horizontal dimensions, respectively. Reduction of bone volume was significantly higher (p <.001 for both dimensions) in the exposed sites. The mean follow‐up of implants after loading was 10.6 ± 6.5 months (range: 2–26 months). The survival rate of implants was 100%. Conclusion: Customized titanium meshes can represent a reliable tool for GBR of severely atrophic sites, with simplification of the surgical phases. Objectives: To report the clinical and radiographic outcomes of patients presenting with edentulous and atrophic ridges and treated with autogenous mandibular bone blocks and rehabilitated with implant‐supported prostheses. Materials and methods: From 1997 to 2015, 75 patients presenting with bone defects of the jaws were grafted with autogenous mandibular bone blocks. One‐hundred eighty‐two implants were placed 4 to 12 months later and loaded 3 to 10 months later: the mean follow‐up was 10 years (range: 3–16 years). The following outcomes were recorded: (a) complication rate of the reconstructive procedure; (b) bone graft resorption before implant placement; (c) peri‐implant bone resorption; (d) implant‐related complications; and (e) implant survival and success rates. Results: Postoperative recovery was uneventful in the majority of patients. An early dehiscence occurred in three patients, but with no significant bone loss, while 3 experienced temporary paraesthesia. The mean vertical and horizontal bone resorption before implant placement was 0.18 mm (standard deviation [SD] = 0.43) and 0.15 mm (SD = 0.42), respectively. The mean peri‐implant bone loss ± standard deviation was 1.06 ± 1.19 (range: 0.00–5.05) at patient level and 1.11 ± 1.26 (range: 0.00–5.20) at implant level. Two implants in 2 patients lost integration and were removed; 10 implants in 7 patients developed peri‐implantitis, but healed after surgical treatment. The cumulative implant survival and success rates were 98.11% and 85.16%, respectively. Purpose: This review evaluated (1) the success of different surgical techniques for the reconstruction of edentulous deficient alveolar ridges and (2) the survival/success rates of implants placed in the augmented areas. Materials and Methods: Clinical investigations published in English involving more than 10 consecutively treated patients and mean follow-up of at least 12 months after commencement of prosthetic loading were included. The following procedures were considered: onlay bone grafts, sinus floor elevation via a lateral approach, Le Fort I osteotomy with interpositional grafts, split ridge/ridge expansion techniques, and alveolar distraction osteogenesis. Full-text articles were identified using computerized and hand searches by key words. Success and related morbidity of augmentation procedures and survival/success rates of implants placed in the augmented sites were analyzed. Results and Conclusion: A wide range of surgical procedures were identified. However, it was difficult to demonstrate that one surgical procedure offered better outcomes than another. Moreover, it is not yet known if some surgical procedures, eg, reconstruction of atrophic edentulous mandibles with onlay autogenous bone grafts or maxillary sinus grafting procedures in case of limited/moderate sinus pneumatization, improve long-term implant survival. Every surgical procedure presents advantages and disadvantages. Priority should be given to those procedures which are simpler and less invasive, involve less risk of complications, and reach their goals within the shortest time frame. Purpose: The purposes of this prospective multicenter study were to evaluate the use of vertical distraction osteogenesis in the correction of vertically deficient alveolar ridges and to evaluate whether the vertical bone gained by distraction osteogenesis was maintained over time when dental implants were placed in the distracted areas. Materials and Methods: Thirty-seven patients presenting vertically deficient edentulous ridges were treated in 4 different centers by means of distraction osteogenesis with an intraoral alveolar distractor. Two to 3 months after consolidation of the distracted segments, 138 dental implants were placed in the distracted areas. Four to 6 months later, abutments were connected and prosthetic loading of the implants began. Results: The mean follow-up after initial prosthetic loading was 34 months (range 15 to 55 months). The mean bone gain obtained by distraction was 9.9 mm (range 4 to 15 mm). The cumulative success rate of the implants 4 years after the onset of prosthetic loading was 94.2%, while the implants' cumulative survival rate was 100%. No statistically significant differences were found between the different centers as far as survival and success rates of implants were concerned. Discussion and Conclusion: The results of this study appear to demonstrate that distraction osteogenesis is a reliable technique for the correction of vertically deficient edentulous ridges. The regenerated bone appeared to withstand the functional demands of implant loading. The survival and success rates of the implants placed in the distracted areas were consistent with those reported in the literature regarding implants placed in native bone in this patient population. This study examined the opportunities offered by intraoral distraction osteogenesis to vertically elongate insufficient alveolar ridges and thereby improve local anatomy for ideal implant placement. Eight patients presenting with vertically deficient edentulous ridges were treated by means of the distraction osteogenesis principle with an intraoral alveolar distractor. Two to 3 months after consolidation of the distracted segments, 26 implants were placed in the distracted areas. Four to 6 months later, abutments were connected and prosthetic loading of the implants was started. The mean follow-up after initial prosthetic loading was 14 months. In all patients, the desired bone gain was reached at the end of distraction (mean vertical bone gain of 8.5 mm). Probing depth, Bleeding Index, and Plaque Index around implants were evaluated, and Periotest values were also calculated. The cumulative success rate of implants was 100%. Radiographic examinations 12 months after functional loading of implants showed a significant increase in the density of the newly generated bone in the distracted areas. This technique seems to be reliable, and the regenerated bone has withstood the functional demands of implant loading. Success rates of implants, periodontal indices of peri-implant soft tissues, and Periotest values were consistent with those reported in the literature regarding implants placed in native bone. The possibility of placing endosseous implants in the edentulous maxilla is frequently reduced by inadequate bone volume of the residual ridge. In totally edentulous maxillae with knife-edge conformation, insufficient thickness is frequently associated with insufficient height of the residual ridge in the posterior maxilla because of pneumatization of the maxillary sinuses. This surgical method combines grafting of the maxillary sinuses, onlay grafts on the buccal side of maxillary posterior segments, and sagittal osteotomy of the anterior maxilla with interpositional bone grafts. Five to six months after maxillary reconstruction, Brånemark implants were placed and, after osseointegration occurred, implant-supported dental prostheses were fabricated. Three patients have been treated with this method and 22 implants have been placed. The mean follow-up after final prosthetic rehabilitation has been 16 months; survival rate has been 100%. Purpose: To evaluate: (1) the survival rate of Straumann® Tissue Level and Bone Level implants placed in atrophic edentulous jaws previously reconstructed by means of autogenous onlay bone grafts; (2) to compare peri-implant bone resorption values over time. Materials and Methods: From 2005 to 2010, 50 patients presenting with vertical or tridimensional defects of the edentulous ridges were treated with autogenous bone grafts. Three to 7 months afterward, 192 implants were placed (Group A: 97 Tissue Level implants; Group B: 95 Bone Level implants) in the reconstructed areas. After a further waiting period of 2 to 3 months, patients were rehabilitated with implant-supported fixed prostheses. The follow-up ranged from 12 to 68 months after the start of prosthetic loading (mean: 33 months). Results: No implants were removed (survival rate: 100%), but in Group B 13 implants (8 placed in iliac grafts, 2 placed in ramus grafts, and 3 placed in calvarial grafts) presented peri-implant bone resorption values higher than those proposed by Albrektsson and colleagues. for successful implants: the overall implant success rate was then 100% for Group A and 86.8% for Group B. No prosthetic failures were recorded, thus leading to a 100% prostheses success rate. Conclusion: No significant differences were found between the two types of implants as far as implant survival rate is concerned, but results from this study seem to demonstrate that Tissue Level implants may present better long-term results in terms of peri-implant bone maintenance, as compared with Bone Level implants, when placed in reconstructed areas. Aim The aim of this study was to verify if the three-dimensional implant position in fresh alveolar socket may influence the maintenance of stable gingival margins around single premolar restorations with immediate implant placement. Methods After 16 weeks of healing, implants were loaded with single crowns. Documentation consisted in peri-apical radiographs taken: before treatment; at the time of implant placement; at the time of prosthetic rehabilitation and 12 months after the start of prosthetic function. Parameters assessed were: the presence/absence of inter-proximal papilla; the inter-implant-tooth distance (ITD); the distance from the base of the crown contact-point to the inter-dental bone crest (CPB); and the buccal-gingival tissue modifications. ITD, CPB values and the modification of buccal-gingival tissue were determined using an image processing software. Results Twenty-one healthy patients with thick gingival biotype, with a total of 21 maxillary premolars scheduled for extractions were enrolled. Following extraction, implants were immediately positioned. A 100% of implant survival rate was observed after 12 months of function. The presence of the mesial papilla was significantly correlated with the mean mesial CPB value (P = 0.0519 by means of two-tailed Mann-Whitney U-test). No significant correlation was found between the mesial papilla presence and the mean mesial ITD score (P = 0.7722 by means of two-tailed Mann-Whitney U-test). Furthermore, the presence of the distal papilla was not significantly correlated with the mean distal ITD and CPB values: scores of 0.7195 and 0.8225 were calculated respectively. Conclusion In the premolar area, the relationship between the presence of a complete inter-proximal papilla and ITD parameter was not significant (P > 0.05). The relationship between the presence of a complete inter-proximal papilla and CPB parameter still remain unclear. Nevertheless, a more large sample size is required to confirm this trend. Objectives: The purposes of this study were to compare: (a) autogenous bone grafts (ABG) and distraction osteogenesis (DO) for their ability in correcting vertically deficient mandibular ridges and their capability in maintaining over time the vertical bone gain obtained before and after implant placement; and (b) the survival and success rates of implants placed in the reconstructed or distracted areas. Material and methods: In a 2-year period (2001–2002), 17 patients presenting with vertically atrophied partially edentulous mandibles requiring implant-supported prosthetic rehabilitation, were included in this study. Patients were randomly assigned to two groups. Eight patients (group 1) were treated with ABG harvested from the mandibular ramus, while nine patients (group 2) were treated by means of DO. In group 1, patients received implants 4–5 months after the reconstructive procedure, while in group 2 implants were placed at the time of distraction device removal (approximately 3 months after the completion of distraction). A total of 19 endosseous implants were placed in group 1, and 21 implants were placed in group 2 patients. For both groups, after an additional 3–5-month period, prosthetic rehabilitation was started. Results: Bone resorption before implant placement was significantly higher in group 1 ( P=0.01), while no statistically significant differences were found between the two groups as far as survival and success rates of implants and peri-implant bone resorption after the start of prosthetic loading were concerned. Conclusion: The results suggested that: (a) both techniques may effectively improve the deficit of vertically resorbed edentulous ridges; (b) survival and success rates of implants placed in the reconstructed/distracted areas are consistent with those of implants placed in native bone. Aim: The purpose of this clinical follow-up study was to report the clinical outcome of osseointegrated implants placed in extremely atrophied edentulous maxillae after Le Fort I osteotomy and interpositional autogenous iliac bone grafts. Patients and methods: In a 10-year period (1995–2004), 39 patients, 18 males and 21 females, aged from 32 to 76 years, presenting with severely atrophied edentulous maxillae were treated with Le Fort I osteotomy and interpositional iliac bone grafts. Four to 8 months after the reconstructive procedure, 281 osseointegrated implants were placed in the reconstructed maxillae. Four to 8 months afterwards, abutments were connected and the prosthetic rehabilitation started. The mean follow-up period of implants after the start of prosthetic loading was 45.9 months (range: 12–108 months). Results: The reconstructive procedure was successful in 38 of 39 patients. In one patient, partial loss of the inlay graft occurred before implant placement. Six patients (42 implants) dropped out of the study. Fifteen implants were removed during the follow-up period, due to loss of integration. Thirty-two implants, although integrated, presented with peri-implant bone-level changes higher than those proposed for successful implants. Cumulative survival and success rates of implants were 94.5% and 82.9%, respectively. Conclusion: Results from this study showed that Le Fort I osteotomy with interpositional bone grafts followed by delayed implant placement is an acceptable means to rehabilitate edentulous patients affected by extremely atrophied edentulous maxillae. Survival rates of implants are consistent with those related to implants placed in native, non-reconstructed bone. Objectives: To analyze publications related to augmentation procedures and to evaluate the success of different surgical techniques for ridge reconstruction and the survival/success rates of implants placed in the augmented areas. Material and methods: Clinical investigations published in English involving at least 5 patients and with a minimum follow-up of 6 months were included. The following procedures were considered: a) Guided bone regeneration (GBR); 2) Onlay bone grafts; 3) Inlay grafts; 4) Bone splitting for ridge expansion (RE); 5) Distraction osteogenesis (DO); and 6) Revascularized flaps. Success rates of augmentation procedures and related morbidity, as well as survival and success rates of implants placed in the augmented sites were analyzed. Results: Success rates of surgical procedures ranged from 60% to 100% for GBR, from 92% to 100% for onlay bone grafts, from 98% to 100% for ridge expansion techniques, from 96,7% to 100% for DO, and was 87.5% for revascularized flaps, whereas survival rates of implants ranged from 92% to 100% for GBR, from 60% to 100% for onlay bone grafts, from 91% to 97.3% for RE, from 90.4% to 100% for DO, and, finally, was 88.2% for revascularized flaps. Conclusion: On the basis of available data it was shown that it was difficult to demonstrate that a particular surgical procedure offered better outcome as compared to another. The purpose of this prospective study was to compare vertical guided bone regeneration (GBR) and vertical distraction osteogenesis (DO) for their ability in correcting vertically deficient alveolar ridges and their ability in maintaining over time the vertical bone gain obtained before and after implant placement. Eleven patients (group 1) were treated by means of vertical GBR with autogenous bone and e-PTFE membranes, while 10 patients (group 2) were treated by means of DO. In group 1, six patients received implants at the time of GBR (subgroup 1A), while five patients had implants placed at the time of membrane removal (subgroup 1B). In group 2, implants were placed at the time of distraction device removal. A total of 25 implants were placed in group 1 and 34 implants were placed in group 2 patients. Three to 5 months after implant placement, patients were rehabilitated with implant-borne dental prostheses. The following parameters were evaluated: (a) bone resorption of the regenerated ridges before and after implant placement; (b) peri-implant clinical parameters 1, 2, and 3 years after prosthetic loading of implants; (c) survival and success rates of implants. Bone resorption values before and after implant placement were significantly higher in group 1. The results suggested that both techniques may improve the deficit of vertically resorbed edentulous ridges, although distraction osteogenesis seems to be more predictable as far as the long-term prognosis of vertical bone gain is concerned. Implant survival rates as well as peri-implant clinical parameters do not differ significantly between the two groups, whereas the success rate of implants placed in group 2 patients was higher than that obtained in group 1 patients.
