The neuromodulatory subcortical system (NSS) nuclei are critical hubs for survival, hedonic tone, and homeostasis. Tau-associated NSS degeneration occurs early in Alzheimer's disease (AD) pathogenesis, long before the emergence of pathognomonic memory dysfunction and cortical lesions. Accumulating evidence supports the role of NSS dysfunction and degeneration in the behavioral and neuropsychiatric manifestations featured early in AD. Experimental studies even suggest that AD-associated NSS degeneration drives brain neuroinflammatory status and contributes to disease progression, including the exacerbation of cortical lesions. Given the important pathophysiologic and etiologic roles that involve the NSS in early AD stages, there is an urgent need to expand our understanding of the mechanisms underlying NSS vulnerability and more precisely detail the clinical progression of NSS changes in AD. Here, the NSS Professional Interest Area of the International Society to Advance Alzheimer's Research and Treatment highlights knowledge gaps about NSS within AD and provides recommendations for priorities specific to clinical research, biomarker development, modeling, and intervention. HIGHLIGHTS: Neuromodulatory nuclei degenerate in early Alzheimer's disease pathological stages. Alzheimer's pathophysiology is exacerbated by neuromodulatory nuclei degeneration. Neuromodulatory nuclei degeneration drives neuropsychiatric symptoms in dementia. Biomarkers of neuromodulatory integrity would be value-creating for dementia care. Neuromodulatory nuclei present strategic prospects for disease-modifying therapies.

The purpose of the present study was to explore whether Computer-Based Cognitive Training (C-BCT) versus Paper-Pencil Cognitive Training (P-PCT) is more beneficial in improving cognitive and language deficits in Greek patients living with Alzheimer’s disease (pwAD). Twenty pwAD were assigned to two groups: (a) the C-BCT group, receiving a computer-based cognitive training program using the RehaCom software, and (b) the P-PCT group, which received cognitive training using paper and pencil. The cognitive training programs lasted 15 weeks and were administered twice a week for approximately one hour per session. The analyses of each group’s baseline versus endpoint performance demonstrated that the P-PCT group improved on delayed memory, verbal fluency, attention, processing speed, executive function, general cognitive ability, and activities of daily living. In contrast, the C-BCT group improved on memory (delayed and working), naming, and processing speed. Comparisons between the two groups (C-BCT vs. P-PCT) revealed that both methods had significant effects on patients’ cognition, with the P-PCT method transferring the primary cognitive benefits to real-life activities. Our findings indicate that both methods are beneficial in attenuating cognitive and language deficits in pwAD. The need for large-scale neurobehavioral interventions to further clarify this issue, however, remains a priority.

Simple, portable, and low-cost paper-based sensors are alternative devices that have the potential to replace high-cost sensing technologies. The compatibility of the paper base biosensors for both chemical and biochemical accentuates its feasibility for application in clinical diagnosis, environmental monitoring, and food quality monitoring. High concentration of copper in blood serum and urine is associated with diseases like liver diseases, carcinomas, acute and chronic infections, rheumatoid arthritis, etc. Detection of copper concentration can give an early sign of Alzheimer disease. Apart from that genetic Wilson's disease can be detected by evaluating the concentration of copper in the urine. In view of the above advantages, a novel and the highly sensitive paper-based sensor has been designed for the selective detection of Cu2+ ions. The fast and highly sensitive chemiresistive multi-dye system sensor can detect Cu2+ ions selectively in as low as 2.23 ppm concentration. Least interference has been observed for counter ion in the detection of Cu2+. Copper chloride, nitrate, and acetate were used to validate the detection process. This assay provides a very high selectivity of Cu2+ ion over other metal cations such as Na+, Mg2+, Ca2+, etc. The easy preparation and high stability of dye solutions, easy functionalization of the paper-based sensors, high selectivity over other cations, low interference of counter anion, and significantly low detection limit of 2.23 ppm make it an effective Cu2+ ion sensor for real-time application in near future. Abstract Aβ42 is one of the most extensively studied blood and Cerebrospinal fluid (CSF) biomarkers for the diagnosis of symptomatic and prodromal Alzheimer’s disease (AD). Because of the heterogeneity and transient nature of Aβ42 oligomers (Aβ42Os), the development of technologies for dynamically detecting changes in the blood or CSF levels of Aβ42 monomers (Aβ42Ms) and Aβ42Os is essential for the accurate diagnosis of AD. The currently commonly used Aβ42 ELISA test kits usually mis-detected the elevated Aβ42Os, leading to incomplete analysis and underestimation of soluble Aβ42, resulting in a comprised performance in AD diagnosis. Herein, we developed a dual-target lateral flow immunoassay (dLFI) using anti-Aβ42 monoclonal antibodies 1F12 and 2C6 for the rapid and point-of-care detection of Aβ42Ms and Aβ42Os in blood samples within 30 min for AD diagnosis. By naked eye observation, the visual detection limit of Aβ42Ms or/and Aβ42Os in dLFI was 154 pg/mL. The test results for dLFI were similar to those observed in the enzyme-linked immunosorbent assay (ELISA). Therefore, this paper-based dLFI provides a practical and rapid method for the on-site detection of two biomarkers in blood or CSF samples without the need for additional expertise or equipment. BackgroundAlzheimer’s disease (AD) is one of the common neurodegenerative diseases, which often coexists with epilepsy. It is very significant to study the treatment options and the relationship between AD and epilepsy.AimsThe purpose of this study was to analyze the top 100 cited papers about AD and epilepsy using bibliometrics, and to describe the current situation and predict research hot spots.MethodsTop 100 papers were obtained from the Web of Science Core Collection (WoSCC). The WoSCC was used to analyze the author, institution, country, title, keywords, abstract, citation, subject category, publication year, impact factor (IF), and other functions. SPSS25 software was used for statistical analysis and CiteSpace V.5.7.R2 was used to visualize the information through collaborative networks.ResultsThe number of publications gradually increased from 2000 to 2021. The total citation count for the top 100 papers ranged from 15 to 433(mean = 67.43). The largest number of papers were published in 2016 (n = 11). Meanwhile, USA (centrality: 0.93) and Columbia University (centrality: 0.06) were the most influential research country and institutions, respectively. The top contributing journals was Journal of Alzheimer’s Disease (8%). The IF for journals ranged from 1.819 to 53.44. A network analysis of the author’s keywords showed that “beta” (centrality: 0.39), “amyloid beta” (centrality: 0.29), “hyperexcitability” (centrality: 0.29) and “disease” (centrality: 0.29) had a high degree of centrality.ConclusionAD and epilepsy have been intensively studied in the past few years. The relationships, mechanisms and treatment of AD and epilepsy will be subjects of active research hotpots in future. These findings provide valuable information for clinicians and scientists to identify new perspectives with potential collaborators and cooperative countries.

Dopamine, the main catecholamine neurotransmitter plays an important role in renal, cardiovascular, central nervous systems, and pathophysiological processes. The abnormal dopamine levels can result in neurological disorders such as Parkinson's, Alzheimer's, schizophrenia, acute anxiety, neuroblastoma and also contribute to cognitive dysfunctions. Given the widespread importance of dopamine concentration levels, it is imperative to develop sensors that are able to monitor dopamine. Herein, we have developed pre-anodized disposable paper electrode modified with 1-pyrenebutyric acid, for the selective and sensitive determination of dopamine. The sensor was characterized with Fourier transform infrared spectroscopy, Energy dispersive X-ray spectroscopy, X-ray photoelectron spectroscopy, and electrochemical techniques for addressing the robust formation and electrochemical activity. The modified electrode exhibited excellent electrocatalytic activity towards dopamine without the common interference from ascorbic acid. The calibration plot for the dopamine sensor resulted linear range from 0.003 μM to 0.5 μM with a detection limit of 0.11 nM. The sensor's potential utility was tested by monitoring dopamine concentration changes in rat brain homogenates when subjected to neurotoxicity. The developed sensor was validated with gold-standard UV–Vis spectroscopy studies and computational studies were performed to understand the interaction between 1-pyrenebutyric acid and dopamine. The Alzheimer's Disease Neuroimaging Initiative (ADNI) is an ongoing, longitudinal, multicenter study designed to develop clinical, imaging, genetic, and biochemical biomarkers for the early detection and tracking of Alzheimer's disease (AD). The initial study, ADNI-1, enrolled 400 subjects with early mild cognitive impairment (MCI), 200 with early AD, and 200 cognitively normal elderly controls. ADNI-1 was extended by a 2-year Grand Opportunities grant in 2009 and by a competitive renewal, ADNI-2, which enrolled an additional 550 participants and will run until 2015. This article reviews all papers published since the inception of the initiative and summarizes the results to the end of 2013. The major accomplishments of ADNI have been as follows: (1) the development of standardized methods for clinical tests, magnetic resonance imaging (MRI), positron emission tomography (PET), and cerebrospinal fluid (CSF) biomarkers in a multicenter setting; (2) elucidation of the patterns and rates of change of imaging and CSF biomarker measurements in control subjects, MCI patients, and AD patients. CSF biomarkers are largely consistent with disease trajectories predicted by β-amyloid cascade (Hardy, J Alzheimer's Dis 2006;9(Suppl 3):151-3) and tau-mediated neurodegeneration hypotheses for AD, whereas brain atrophy and hypometabolism levels show predicted patterns but exhibit differing rates of change depending on region and disease severity; (3) the assessment of alternative methods of diagnostic categorization. Currently, the best classifiers select and combine optimum features from multiple modalities, including MRI, [(18)F]-fluorodeoxyglucose-PET, amyloid PET, CSF biomarkers, and clinical tests; (4) the development of blood biomarkers for AD as potentially noninvasive and low-cost alternatives to CSF biomarkers for AD diagnosis and the assessment of α-syn as an additional biomarker; (5) the development of methods for the early detection of AD. CSF biomarkers, β-amyloid 42 and tau, as well as amyloid PET may reflect the earliest steps in AD pathology in mildly symptomatic or even nonsymptomatic subjects and are leading candidates for the detection of AD in its preclinical stages; (6) the improvement of clinical trial efficiency through the identification of subjects most likely to undergo imminent future clinical decline and the use of more sensitive outcome measures to reduce sample sizes. Multimodal methods incorporating APOE status and longitudinal MRI proved most highly predictive of future decline. Refinements of clinical tests used as outcome measures such as clinical dementia rating-sum of boxes further reduced sample sizes; (7) the pioneering of genome-wide association studies that leverage quantitative imaging and biomarker phenotypes, including longitudinal data, to confirm recently identified loci, CR1, CLU, and PICALM and to identify novel AD risk loci; (8) worldwide impact through the establishment of ADNI-like programs in Japan, Australia, Argentina, Taiwan, China, Korea, Europe, and Italy; (9) understanding the biology and pathobiology of normal aging, MCI, and AD through integration of ADNI biomarker and clinical data to stimulate research that will resolve controversies about competing hypotheses on the etiopathogenesis of AD, thereby advancing efforts to find disease-modifying drugs for AD; and (10) the establishment of infrastructure to allow sharing of all raw and processed data without embargo to interested scientific investigators throughout the world.

The diagnosis of Alzheimer’s disease (AD) is frequently missed or delayed in clinical practice. To remedy this situation, we developed a screening, paper-based (P-ELISA) platform to detect β-amyloid peptide 1–42 (Aβ42) and provide rapid results using a small volume, easily accessible plasma sample instead of cerebrospinal fluid. The protocol outlined herein only requires 3 μL of sample per well and a short operating time (i.e., only 90 min). The detection limit of Aβ42 is 63.04 pg/mL in a buffer system. This P-ELISA-based approach can be used for early, preclinical stage AD screening, including screening for amnestic mild cognitive impairment (MCI) due to AD. It may also be used for treatment and stage monitoring purposes. The implementation of this approach may provide tremendous impact for an afflicted population and may well prompt additional and expanded efforts in both academic and commercial communities.

The advancement of Alzheimer's disease (AD) is directly correlated to the phosphorylation damage of Tau protein (Tau-441), which is considered the most reliable indicator for early detection of AD. Nevertheless, the concentration of Tau-441 in human serum remains considerably low. To address this issue, a paper-based fluorescence immunosensor based on internal filter effect (IFE) was developed for the detection Tau-441. The microfluidic paper-based channel was prepared and modified with a gold layer to attach primary antibodies and increase the sensitivity of the sensor. All at once, Au@CeO 2 nanorods was employed as a fluorescence probe to hydrolyze p-nitrophenyl phosphate (pNPP), which generates hydrolyzed product p-nitrophenol (PNP) can reduce or quench the fluorescence intensity of N-doped graphene quantum dots (NGQDs). The immunosensor was designed to detect an increase in the amount of Tau-441 collected as the fluorescence intensity of NGQDs decreased. The developed sensor demonstrated satisfactory results in clinical serum samples, offering a promising concept for an immunoassay tagged with nanozymes for early detection of AD. Traumatic brain injury (TBI) and post-traumatic stress disorder (PTSD) are signature injuries of the wars in Iraq and Afghanistan and have been linked to an increased risk of Alzheimer's disease (AD) and other dementias. A meeting hosted by the Alzheimer's Association and the Veterans' Health Research Institute (NCIRE) in May 2012 brought together experts from the U.S. military and academic medical centers around the world to discuss current evidence and hypotheses regarding the pathophysiological mechanisms linking TBI, PTSD, and AD. Studies underway in civilian and military populations were highlighted, along with new research initiatives such as a study to extend the Alzheimer's Disease Neuroimaging Initiative (ADNI) to a population of veterans exposed to TBI and PTSD. Greater collaboration and data sharing among diverse research groups is needed to advance an understanding and appropriate interventions in this continuum of military injuries and neurodegenerative disease in the aging veteran.

Sleep deprivation is a common complaint in modern societies. Insufficient sleep has increased the risk of catching neurodegenerative diseases such as Alzheimer Several studies have indicated that restricted sleep increases the level of deposition of β-amyloid and formation of neurofibrillary tangles, the major brain microstructural hallmarks for Alzheimer disease. The mechanisms by which sleep deprivation affects the pathology of Alzheimer disease has not yet been fully and definitively identified. However, risk factors like apolipoprotein E risk alleles, kinases and phosphatases dysregulation, reactive oxygen species, endoplasmic reticulum damages, glymphatic system dysfunctions and orexinergic system inefficacy have been identified as the most important factors which mediates between the two conditions. In this review, these factors are briefly discussed.

Joint analysis of varied biomarkers in blood sample will provide more accurate results for the monitoring and management of Alzheimer's disease (AD). In addition to the known β-amyloid protein (Aβ) used as a reliable clinical marker for AD diagnosis, Fetuin B also demonstrates a strong correlation with AD progression. Especially, for medical diagnostics, on-site detection of multi-analyte is quite important and therefore, it is desirable to develop a rapid and portable, multiplex electrochemical assay for simultaneous detection of Aβ and Fetuin B, which has been seldomly reported before. To achieve this goal, a multiplex paper-based electrode was prepared via vacuum filtration, by which, single-walled carbon nanotubes (SWCNTs) and AuNPs were sequentially filtered through cellulose paper with a five-electrode pattern to form a conductive underlay and electrical connections. A nanocomposite of AuNPs-loaded dendritic mesoporous silica nanoparticles (dmSiO 2 -Au) was firstly synthesized and further deposited thionine (Thi) for signal amplification. An immunosensor was correspondingly fabricated, which involved a combination of multiplex electrodes and dmSiO 2 -Au-Thi nanocomposite to facilitate simultaneous monitoring of Aβo and Fetuin B. Additionally, ferrocene (F c) was applied onto a separate working electrode to perform a ratiometric electrochemistry for enhancing accuracy. The outcomes demonstrated high specificity, sensitivity, and stability, and no obvious cross-talking between three working electrodes. Application of this sensor resulted in a successful determination of Aβo and Fetuin B levels in serum and brain tissue of transgenic AD mice. The early diagnosis of Alzheimer’s disease (AD) remains a challenge for medical scientists worldwide, leading to a number of research efforts that focus on biosensor development for AD biomarkers. However, the application of these complicated biosensors is limited in medical diagnosis, due to the difficulties in robust sensing platform development, high costs, and the necessity for technical professionals. We successfully developed a robust straightforward manufacturing process for the fabrication of multi-chamber paper devices using the wax printing method and exploited it to detect amyloid beta 42 oligomers (AβO42, a significant biomarker of AD) using copper-enhanced gold nanoprobe colorimetric immunoblotting. Small hydrophilic reaction chambers could concentrate the target sample to the desired size to improve the sensing performance. The copper-enhanced gold nanoprobe immunoblot using the designed multi-chamber platform exhibited a highly sensitive performance with a limit of detection of 320 pg/mL by the naked eye and 23.7 pg/mL by a smartphone camera. This process from sensing manufacture to sensing conduction is simple to perform whenever medical technicians require time- and cost-savings, without complicated instruments or the need for technical professionals, making it feasible to serve as a diagnostic tool worldwide for the early monitoring of AD and scalable devices for the sensing application of various biomarkers in clinical settings.
