Background: Given the increasing lifespan of the elderly and the higher proportion of older people in the global population, the incidence rate of neurodegenerative diseases is increasing. The aim of this study is to evaluate, by means of computer simulations, developments in the costs of treating and caring for people suffering from Alzheimer’s disease (AD) in the EU 28 by 2080, while assuming the introduction of drug administrations at various disease stages. Methods: Impact analysis leverages a mathematical model that compares five different population development scenarios when introducing different types of drugs to the scenarios but without changing the treatment. Changes in the economic burden are considered as of 2023, when new drugs are expected to enter the market. Findings: The results of the simulations show that by prolonging the length of a person’s ‘stay’ in the Mild, Moderate, or Severe stage, the total cost of care for all persons with AD will increase by 2080. For individual scenarios, the percentage of patients and costs increased as follows: Mild by one year, by 10.61%; Mild by two years, by 17.73%; Moderate by one year, by 16.79%; Moderate by two years, by 34.88%; and Severe by one year, by 23.79%. The change in cost development when prolonging the stay in the Mild cognitive impairment stage (by lowering the incidence by 10%, 30%, or 50%) reduced the cost (by 4.88%, 16.78% and 32.48%, respectively). Interpretation: The results unambiguously show that any intervention prolonging a patient’s stay in any stage will incur additional care costs and an increase in the number of persons with AD. Therefore, extending lifespan is important in terms of improving the quality of life of patients, and the introduction of new drugs must consider the additional costs imposed upon society. The purpose of this study is to analyze the economic burden of persons with Alzheimer's disease (AD) and Parkinson's disease (PD) in Europe. On the basis of available data about the number of persons with dementia, their prevalence, and treatment and care costs, a mean cost burden is estimated for the year of 2030 and for the year of 2050 in Europe. The method of retrospective analysis of available sources was used; furthermore, analysis of database data such as WHO and Eurostat, which provide information about the number of older people and people with dementia; and specification of direct and indirect medical and nonmedical costs of patients with AD and PD from current studies was also used. The findings of this study confirm that the number of patients affected with AD and PD, as well as annual costs of the treatment and care of these patients, in the selected European countries are rapidly growing. The cost burden of both AD and PD in the selected European countries rises year by year, and by 2050, the cost burden of both diseases in fact will be almost two times higher in comparison with the year of 2010. In 2050, the overall mean cost burden is estimated to reach 357 billion Euros. As the World Health Organization reports, mental illnesses have a serious impact on more than 25% of all population people worldwide at some time during their lives. Mental illnesses are universal; they affect people of any age, both women and men, the rich and the poor, no matter from which urban and rural environment they come from. Mental illnesses have an enormous economic effect on societies and on the quality of people’s life, including their families. The purpose of this study is to describe social and economic aspects of Alzheimer’s disease (AD) with respect to the early diagnosis. The authors provide an analysis of costs of treatment and care in the selected countries where the data from the available studies are recalculated into comparable quantities. Furthermore, the authors analyse aspects and possibilities of care for patients with AD in the informal (home) environment in compliance with individual phases of this disease. In the article, the method of research of available sources focusing on social and economic issues of AD is used. In order to compare costs of treatment and care of the AD patients, the Qualitative Comparative Analysis Method is exploited. This paper deals with the analysis of the costs, applied, for example, when treating specific diseases - an important aid in prioritizing the process of resource allocation. In our review, the specific disease is dementia caused by Alzheimer's disease. This paper aims to provide more information on the partial costs per patient that are calculated according to the aggregated data from publicly available sources as well as from the results of authors' own investigation. The University Hospital in Hradec Králové and the General Health Insurance Company of the Czech Republic participated in this research. The elementary research objective was to compare the costs per patient diagnosed early onset, to those of the patient diagnosed later. The Czech Republic lacks information regarding dementia. Therefore, these issues require attention. The methods used in this paper included time series analyses, methods of direct questioning, interviews with experts, and analyses of medical documentation. These methods were combined to exploit their particular advantages and to ensure the issues discussed, were covered. The investigation showed that the underpinning of patients with Alzheimer's disease at early onset is advantageous from an economic perspective, because the cost of outpatient care is much lower compared with that of inpatient care. The international comparisons of the volume of care provided should be approached with great caution. These are based solely on the facts of various expert estimates and are not usually supported by hard data. Yet, they still illustrate the overall view of our ability to take care of people with dementia. According to experts, care in the Czech Republic significantly lags behind the rest of developed Europe. While services are provided to 26% of people with dementia in Germany and 50% in France, the experts estimate that services are provided to only 10% of the population in the Czech Republic. If we were to offer a similar volume of services in the Czech Republic, to the same extent as the average in Europe, we would have to triple the existing capacities. [ABSTRACT FROM AUTHOR]

Communication is very important for people to be successfully integrated into social environment and make and maintain relationship. Particularly, language difficulties lead to social exclusion of the people affected with Alzheimer's disease (AD) and contribute to a significant decrease in the quality of their life and also have a big impact on their family members who in most cases become their caregivers who need to communicate with their loved ones in order to meet their needs. Therefore, the goal of this study is to describe language impairments in the individual phases of AD and discuss their improvement with respect to AD on the basis of literature review. The authors of this article use traditional research methods in order to achieve the goal set mentioned earlier. First, a method of literature review of available sources describing language impairments in the individual phases of AD is exploited. Second, to show how informal caregivers and relevant drugs can successfully intervene in the improvement of these language impairments, a method of comparison of different research studies exploring such social intervention and medical treatment is used. The world report about the Alzheimer's disease (World Alzheimer's Report 2013) points out that there is a lack of carers (social workers) and also lack of dependants support. The report shows, that with an aging world population the traditional informal care provided by family, friends and community ceases to be sustainable. The world report expects that in approximately 40 years the number of dependent older people will be almost three times higher - from 101 million in 2010 to 277 million in 2050. The World Health Organisation (2013) divided old age into three stages. From early old age, 60 to 74, from 74 to 89 years of age and 90 years or more is denoted like longevity.As the biggest health insurance company in the Czech Republic, the General Health Insurance Company (VZP) issues nearly 600 million Czech crowns for the treatment of patients with the Alzheimer's disease. Every year there are more and more patients, last year VZP recorded more than 27 thousand patients. While not in all cases is disease diagnosed in time. Experts estimated that Alzheimer's disease which is in most cases the most common cause of dementia is suffered by more than 130 thousand of citizens of Czech Republic. Every state creates its own program which focuses on old age and aging in the Czech Republic it is the “National program of preparation for aging of the population”. The aim of this program is to create conditions for the solutions of aging issues and to achieve changes in attitudes and approaches at every level. Therefore, it is necessary provide support from state and other non-state institutions which can improve the quality of life for senior citizens. The support means some kind of handover or support in various forms of assistance in diverse situations (material, economical, informational etc.).The paper describes the latest national and regional trends for people with dementia in need of care and represents an economic and managerial analysis of the long-term care systems, in the Czech Republic. Increasing life expectancy in modern society is undoubtedly due to improved healthcare, scientific advances in medicine, and the overall healthy lifestyle of the general population. However, this positive trend has led to an increase in the number of older people with a growing need for a sustainable system for the long-term care of this part of the population, which includes social and health services that are essential for a high quality of life. Longevity also brings challenges in the form of a polymorbid geriatric population that places financial pressure on healthcare systems. Regardless, one disease dominates the debate about financial sustainability due to the increasing numbers of people diagnosed, and that is Alzheimer's disease (AD). The presented paper aims to demonstrate the economic burden of social and healthcare services. Data from two regions in the Czech Republic were selected to demonstrate the potential scope of the problem. The future costs connected with AD are calculated by a prediction model, which is based on a population model for predicting the number of people with AD between 2020 and 2070. Based on the presented data from the two regions in the Czech Republic and the prediction model, several trends emerged. There appears to be a significant difference in the annual direct costs per person diagnosed with AD depending on the region in which they reside. This may lead to a significant inequality of the services a person can acquire followed by subsequent social issues that can manifest as a lower quality of life. Furthermore, given the prediction of the growing AD population, the costs expressed in constant prices based on the year 2020 will increase almost threefold during the period 2020–2070. The predicted threefold increase will place additional financial pressure on all stakeholders responsible for social and healthcare services, as the current situation is already challenging. Summary: Determining the cost of healthcare and social care for patients is a crucial issue for many parties; therefore, both public and private payments play a decisive role in patient care. The article deals with the analysis of the possibilities of the simulation of costs related to Alzheimer's disease. This disease is highly variable, and the cost items vary considerably. Therefore, it is necessary to use simulation methods. The results of simulation models can then be implemented in sophisticated methods working with activity costs. Findings: Models for health and social care are specific. No significant re-engineering is expected in this area, so the models must be unambiguous and easy to understand for all representatives involved. Modeling of business processes is thus a suitable means for analysis in this area. The process cost calculation is built on two simulation models. The first model is the Business Process Model and Notation (BPMN), the results of which are verified in the system dynamics model created in the stock and flow diagram. The simulations results give us appropriate values for calculating the total activity-based costs of AD. Applications: The practical part of this article deals with the identification of costs related to Alzheimer's disease and their allocation concerning remedies. Based on this information, models for Alzheimer's disease process cost management are developed. Results: The BPMN simulation was performed for 100 passes through a fixed-length phase process. The duration of each phase was set at 48, 108, and 40 months. Five simulations were set up, each Mini-Mental State Examination (MMSE) score set, which affected the passage through the process. Software Stella Professional was used for simulations for constant MMSE score values for the home and facility options. The values are as follows: MMSE 5 = 1331. 26   € ( 1627. 72   € ); MMSE 15 = 1202. 72   € ( 1102. 02   € ); MMSE 25 = 1051. 24   € ( 848. 91   € ). Conclusions: Both AD models created in this article produce comparable results, although each of them works on a different principle. The results are based on the selection of original data, and at the same time, these models point to the processes and subprocesses that lead to the costs. The description, specifications, and possible values of partial costs are crucial knowledge for persons in the management functions of public administration and self-government in the area of finance and management not in the field of specific diseases.
