Abstract

Background: Limited data are currently available on the performance of everolimus eluting bioresorbable vascular scaffold (BVS) for treatment of complex coronary lesions representative of daily practice., Methods: This is a prospective, mono-center, single-arm study, reporting data after BVS implantation in patients presenting with stable, unstable angina, or non-ST segment elevation myocardial infarction caused by de novo stenotic lesions in native coronary arteries. No restrictions were applied to lesion complexity. Procedural results and 12-month clinical outcomes were reported., Results: A total of 180 patients have been evaluated in the present study, with 249 treated coronary lesions. Device Success per lesion was 99.2%. A total of 119 calcified lesions were treated. Comparable results were observed among severe, moderate and noncalcified lesions in term of %diameter stenosis (%DS) (20.3 ± 10.5%, 17.8 ± 7.7%, 16.8 ± 8.6%; P = 0.112) and acute gain (1.36 ± 0.41 mm, 1.48 ± 0.44 mm, 1.56 ± 0.54 mm; P = 0.109). In bifurcations (54 lesions), side-branch ballooning after main vessel treatment was often performed (33.3%) with low rate of side-branch impairment (9.3%). A total of 29 cases with coronary total occlusions were treated. After BVS implantation %DS was not different from other lesion types (17.2 ± 9.4%, vs. 17.7 ± 8.6%; P = 0.780). At one year, all-cause mortality was reported in three cases. The rate of target lesion revascularization and target vessel revascularization was 3.3%. The rate of definite scaffold thrombosis was 2.6%., Conclusions: The implantation of the everolimus eluting bioresorbable vascular scaffold in an expanded range of coronary lesion types and clinical presentations was observed to be feasible with promising angiographic results and mid-term clinical outcomes. © 2016 Wiley Periodicals, Inc., (© 2016 Wiley Periodicals, Inc.)

Abstract

Aims: Our aim was to test the feasibility and safety of the transfemoral PulseCath iVAC 2L (PulseCath, Amsterdam, The Netherlands)., Methods and Results: Circulatory support devices are helpful adjunctive tools to perform high-risk percutaneous coronary interventions (PCI). The PulseCath iVAC 2L is a novel pulsatile circulatory support system capable of generating output of up to 2 L/min. We performed a prospective clinical pilot study enrolling 14 patients who underwent high-risk PCI under protection with the iVAC 2L. Median age was 74 (56-84) years. Implantation of the iVAC 2L was successful in 13 (93%) patients. Median device flow was 1.4 (1.1-2.0) L/min. Total support time was 67 (23-149) minutes. The use of iVAC 2L support was associated with a better mean arterial pressure and cardiac output during the procedure. Angiographic success was 100%. There was one major procedural complication related to the 19 Fr access sheath. There were no major adverse events at three-month follow-up., Conclusions: Circulatory support with the iVAC 2L device is feasible and safe in patients undergoing high-risk PCI.

Abstract

Aims: We aimed to asses the generalizability of two 'all-comers' randomized clinical trials (AC-RCTs) in patients undergoing percutaneous coronary intervention (PCI)., Methods and Results: Recently two large AC-RCT's comparing drug-eluting stents were performed in our institution (LEADERS and RESOLUTE-III). During the inclusion period of these trials 1242 consecutive PCI patients were treated of whom 579 (48%) were actually included. The most important reasons for non-participation were inability to provide informed consent (33.5%), refused to participate (19%), or patient met one of the other exclusion criteria (26.9%). Trial participants more frequently had stable angina (42.5 vs. 34.4%) and less frequently acute myocardial infarction as indication for PCI (31.4 vs. 42.4%) than non-participants. Hypertension (52.8 vs. 49.1%) and hypercholesterolaemia (56.3 vs. 49.1%) were seen more frequently in trial participants; heart failure was less common (2.1 vs. 4.4%). A significant difference in 30-day mortality was observed between AC-RCT participants and non-participants [0.7 vs. 4.5% events; adjusted hazard ratio (aHR) 0.18 and 95% confidence interval (CI) 0.06-0.52]. One-year mortality was also lower (3.1 vs. 6.9% events; aHR: 0.51 and 95% CI: 0.29-0.91, but 1-year mortality in 48 h survivors was similar (3.1 vs. 4.2% events; aHR: 0.74 and 95% CI: 0.41-1.34)., Conclusion: Applying the all-comers design did not result in inclusion of all consecutive patients, as only half of the target population was enrolled. It should be noted, however, that this design included more patients than observed in classical RCTs. AC-RCT participants and non-participants were different in terms of baseline characteristics and outcome.

Abstract

Background: Health status has become increasingly important as an outcome measure in patients with cardiovascular disease. Poor patient-rated health status has been shown to predict mortality in patients with coronary artery disease and heart failure. In patients treated with percutaneous coronary intervention (PCI), we examined whether poor health status predicts 6-year mortality and whether a decline in health status is associated with adverse clinical outcome., Methods: Consecutive patients (N = 872) treated with PCI as part of the RESEARCH registry, completed the 36-item Short-Form Health Survey (SF-36) at 1 and 12 months post-PCI., Results: The SF-36 domains physical functioning (hazard ratio [HR] 2.59, 95% CI 1.61-4.16), social functioning (HR 2.76, 95% CI 1.74-4.37), role limitations due to physical functioning (HR 2.45, CI 1.52-3.92), mental health (HR 2.12, 95% CI 1.35-3.31), vitality (HR 1.73, 95% CI 1.09-2.74), bodily pain (HR 2.25, 95% CI 1.43-3.54), and general health (HR 2.46, 95% CI 1.57-3.87) were associated with 6-year mortality. A decline in health status was not related with higher 6-year mortality., Conclusions: Health status domains as measured with the SF-36 predicted death at 6-year follow-up in PCI patients treated with drug-eluting stenting, independent of demographic and clinical characteristics. In contrast, a decline in health status between 1 and 12 months post index procedure, as measured with the SF-36, was not associated with 6-year mortality in PCI patients treated with drug-eluting stenting.

Abstract

Introduction: Preparatory information before an invasive procedure has positive effects (e.g. on recovery, well-being and anxiety). However, preparation of patients for a repeat procedure is hardly investigated. The question is whether these patients benefit from the same preparatory information., Aims: To determine whether there are differences in terms of anxiety and well-being between patients undergoing their first percutaneous transluminal coronary angioplasty (PTCA) and those undergoing a repeat PTCA., Design: Descriptive correlational study with a quantitative and qualitative research component., Method: First-time PTCA patients (n=46) and re-PTCA patients (n=40) were asked to complete three psychological self-report questionnaires (HADS, HPPQ and VAS) before the procedure. Five re-PTCA patients were interviewed the day after the procedure., Results: We were not able to show statistically significant differences between the two groups. On the anxiety scales re-PTCA patients scored 6.5 (HADS) and 4.0 (VAS) vs. 5.0 (P=0.25) and 2.6 (P=0.30) for first-time PTCA patients. On well-being (HPPQ) these differences were 18.0 vs. 19.0 (P=0.40). Once the interview data were coded, four themes were apparent, namely: recurrent symptoms, information, experience and future prospects., Conclusion: A trend is seen towards a worse condition in the re-PTCA group with respect to anxiety and well-being. However, the differences are not statistically significant, they seem to be clinically relevant. The interviews point out that the return of symptoms and future prospects, rather than the procedure itself, are an important part in preparing for a repeat intervention. This aspect is currently no part of the provided preparatory information. Future research will have to determine the most beneficial method of preparing these patients (e.g. tailored preparation, emotional support, coaching).

Abstract

AIMS: The purpose of the Euro Heart Survey Programme of the European Society of Cardiology is to evaluate to which extent clinical practice endorses existing guidelines as well as to identify differences in population profiles, patient management, and outcome across Europe. The current survey focuses on the invasive diagnosis and treatment of patients with established coronary artery disease (CAD). METHODS AND RESULTS: Between November 2001 and March 2002, 7769 consecutive patients undergoing invasive evaluation at 130 hospitals (31 countries) were screened for the presence of one or more coronary stenosis >50% in diameter. Patient demographics and comorbidity, clinical presentation, invasive parameters, treatment options, and procedural techniques were prospectively entered in an electronic database (550 variables+29 per diseased coronary segment). Major adverse cardiac events (MACE) were evaluated at 30 days and 1 year. Out of 5619 patients with angiographically proven coronary stenosis (72% of screened population), 53% presented with stable angina while ST elevation myocardial infarction (STEMI) was the indication for coronary angiography in 16% and non-ST segment elevation myocardial infarction or unstable angina in 30%. Only medical therapy was continued in 21%, whereas mechanical revascularization was performed in the remainder [percutaneous coronary intervention (PCI) in 58% and coronary artery bypass grafting (CABG) in 21%]. Patients referred for PCI were younger, were more active, had a lower risk profile, and had less comorbid conditions. CABG was performed mostly in patients with left main lesions (21%), two- (25%), or three-vessel disease (67%) with 4.1 diseased segments, on average. Single-vessel PCI was performed in 82% of patients with either single- (45%), two- (33%), or three-vessel disease (21%). Stents were used in 75% of attempted lesions, with a large variation between sites. Direct PCI for STEMI was performed in 410 cases, representing 7% of the entire workload in the participating catheterization laboratories. Time delay was within 90 min in 76% of direct PCI cases. In keeping with the recommendations of practice guidelines, the survey identified under-use of adjunctive medication (GP IIb/IIIa receptor blockers, statins, and angiotensin-converting enzyme-inhibitors). Mortality rates at 30 days and 1 year were low in all subgroups. MACE primarily consisted of repeat PCI (12%). CONCLUSION: The current Euro Heart Survey on coronary revascularization was performed in the era of bare metal stenting and provides a global European picture of the invasive approach to patients with CAD. These data will serve as a benchmark for the future evaluation of the impact of drug-eluting stents on the practice of interventional cardiology and bypass surgery.
