Introduction: The aims of this article are to analyse the neuropsychological changes in the medium–term in children subjected to aortic arch surgery using selective cerebral perfusion (SCP), as well as to detect any modifiable factors in the surgical technique that may contribute to minimising the subsequent neurological involvement. Material and methods: Inclusion criteria were established as: aortic arch disease operated on using SCP during the first year of life, between 10 August 2004 and 24 May 2016, biventricular physiology, and gestational age greater than 31 weeks. In the absence of a chromosomal disease, they were classified, from a neurological point of view, using the Rankin score. Children over 4-years of age were subjected to intelligence studies, including attention level, development, and psycho-lingual skills. Results: The study included a total of 82 patients with a mean age of 1.8 months. The mean SCP flow was 32 ml/kg/min. The mean time of SCP was 31 min. The overall mortality of the series was 14.8%. Neurological dysfunction was observed in 35.9% of patients, and the following were detected as risk factors: surgery in patients less than 10-days-old, duration of SCP greater than 40 min, and the time required for the cooling down and/or warming-up. Attention deficit was diagnosed in 35.2% of patients greater than 5-years-old. Conclusions: Patients operated on using SCP in in the first year of life required a neuropsychological follow-up, and there are modifiable surgical factors that may have an influence on neurological development. Resumen: Introducción: Los objetivos son analizar las alteraciones neuropsicológicas a medio plazo de los niños intervenidos del arco aórtico mediante perfusión cerebral selectiva (PCS) y detectar posibles factores modificables en la técnica quirúrgica que pueden ayudar a minimizar la afectación neurológica posterior. Material y métodos: Se establecieron como criterios de inclusión: patología del arco aórtico intervenida mediante PCS durante el primer año de vida, entre el 10 de Agosto de 2004 y el 24 de Mayo de 2016, fisiología biventricular y edad gestacional mayor de 31 semanas. En ausencia de cromosomopatía se clasificaron desde el punto de vista neurológico siguiendo el modified Rankin score. En mayores de 4 años se realizaron estudios de inteligencia, nivel de atención, maduración y aptitudes piscolingüisticas. Resultados: 82 pacientes se incluyeron, cuya edad media en el momento de la cirugía fue de 1,8 meses. El Flujo medio en PCS fue de 32 ml/kg/min. El tiempo medio de PCS fue de 31 minutos. La mortalidad global de la serie fue de 14,8 %. Con disfunción neurológica se observaron un 35,9 % y se detectaron como factores de riesgo: cirugía en menores de 10 días de edad, duración de PCS mayor de 40 minutos y la duración del enfriamiento y/o calentamiento. Un 35,2 % de los pacientes mayor de 5 años fueron diagnosticados de déficit de atención. Conclusiones: Los pacientes intervenidos en el primer año de vida con PCS precisan un seguimiento neuropsicológico y hay factores modificables quirúrgicos que pueden influir en el desarrollo neurológico.

Objetivo: Determinar si la canulación arterial axilar para perfusión selectiva anterograda cerebral (PSAC) con hipotermia moderada reduce la morbilidad, mortalidad y el pronóstico neurológico en pacientes llevados a cirugía del arco aórtico. Pacientes y métodos: Se hace una revisión retrospectiva de 2 cohortes de pacientes a quienes se les realizo cirugía del arco aórtico urgente o electiva, para comparar dos técnicas de canulación arterial diferente; la canulación axilar para perfusión selectiva anterograda cerebral con hipotermia moderada y la canulación femoral con hipotermia profunda, en un periodo de tiempo comprendido entre Diciembre del 2002 y Agosto del 2008 en el servicio de cirugía cardiovascular de la Fundación Cardioinfantil Instituto de Cardiología. Resultados: 68 pacientes, 50 hombres y 18 mujeres fueron llevados a cirugía de arco aórtico 19 pacientes (27%) con diagnóstico de aneurisma de aorta ascendente y arco aórtico y 49 pacientes (72%) con disección aortica tipo A aguda o crónica. A 55 pacientes (80.9%) se les hizo canulación axilar y a 13 pacientes (19,1%) se les hizo canulación femoral. No se encontraron diferencias en las variables preoperatorias entre los dos grupos. La mortalidad global fue de 13,2% (9 pacientes), todas las muertes se presentaron en pacientes con diagnóstico de disección aortica tipo A. El 55% de los pacientes (38/68) se operaron de urgencia. No se presentaron diferencias estadísticamente significativas en la mortalidad entre el grupo de canulación axilar, 8 muertes (14,5%) y el grupo de canulación femoral 1 muerte (7,7%) con (p=0.5). No se encontraron diferencias en la incidencia de déficit neurológico permanente entre los dos grupos, presentándose solamente dos pacientes con accidente cerebrovascular ambos en el grupo de canulación axilar (P=0.48). Conclusiones: la canulación axilar con perfusión cerebral selectiva anterograda en hipotermia moderada representa un método seguro de protección cerebral, con una mortalidad aceptable y una incidencia baja de complicaciones neurológicas permanentes, no encontramos diferencias en la mortalidad o la aparición de eventos neurológicos, cuando se comparo con la técnica de canulación arterial femoral. No se presentaron complicaciones asociadas al sitio de canulación en ninguno de los dos grupos. Objective: Determine if axilary cannulation for selective antegrade cerebral perfusion (SACP) with moderate hypothermia has contributed to improve both morbility mortality and neurological prognosis in patients undergoing aortic arch surgery. Patients and methods: This revision is retrospective using two cohorts of patients who underwent to urgent or elective arch surgery to compare the two methods of different arterial cannulation: axilary cannulation for selective antegrade cerebral perfusion with moderate hypothermia and femoral cannulation with deep hypothermia in a period of time from December 2002 to August 2008 in the Department of Cardiovascular Surgery in The Fundacion Cardioinfantil in Bogota, Colombia. Results: 68 patients 50 men and 18 women were operated for aortic arch surgery. 19 patients (27%) with ascending aortic arch aneurysm and 49 patients (72%) from December 2002 to August 2008 with acute or chronic aortic dissection type A. 55 patients (80.9%) underwent axilary cannulation and 13 patients (19, 1%) femoral cannulation. There were no differences found in the preoperative variables between the two groups. The global mortality was of 13,2% (9 patients). All of the deaths were observed in the group with the diagnosis of aortic dissection type A. 55% (38/68) of the patients were operated as urgent surgery. There was no statistically significant difference in mortality in the axilary cannulation group 8 deaths (14,5%) and in the femoral cannulation group1 death (7,7%) (p=0.5). There was no difference found in the incidence of permanent neurological deficit between the two groups. Presenting itself only in two patients with stroke both in the axilary cannulation group (P=0.48). . Conclusions: Axilary cannulation for selective antegrade cerebral perfusion with moderate hypothermia represents a safe method of cerebral protection, with an acceptable mortality rate and low incidence of permanent neurological complications, there were no differences in the mortality or appearance of adverse neurological events, when compared to the method of arterial femoral cannulation. No complications were found associated to the site of cannulation in either of the two groups.
