Objective: The aim of this study was to disclose the differences of surgical outcomes between Marfan syndrome with mutations in fibrillin-1 gene and Loeys-Dietz syndrome with mutations in transforming growth factor-beta receptor 1 and 2., Methods: We reviewed 368 patients aged less than 50 years who underwent surgery for thoracic aortic diseases between 1988 and 2019, and enrolled 99 patients with Marfan syndrome (26.9%; 57 men, 33 ± 7.5 years) and 24 patients with Loeys-Dietz syndrome (6.5%; 13 men, 28 ± 11 years)., Results: Freedom from all causes of mortality was similar between the 2 groups (P = .40, log-rank). The cumulative incidence of reintervention was significantly lower in the Marfan syndrome group than in the Loeys-Dietz syndrome group (P = .016, Gray). The cumulative incidence of first aortic arch reoperation for aortic arch aneurysm was significantly lower in the Marfan syndrome group than in the Loeys-Dietz syndrome group (P < .001, Gray). The cumulative incidence of first aortic root reoperation for aortic root aneurysm (P = .57, Gray) and first descending aorta reoperation for descending aortic aneurysm (P = .76, Gray) was similar between the 2 groups. The cumulative incidence of aortic dissection after initial surgery was significantly lower in Marfan syndrome than in Loeys-Dietz syndrome (P = .0059, Gray)., Conclusions: Loeys-Dietz syndrome with mutations in transforming growth factor-beta receptor 1 and 2 revealed higher rates of reoperation, and more specifically the arch reoperation was higher in those with Loeys-Dietz syndrome than those with Marfan syndrome. Aggressive arch surgery in the initial operation on the proximal aorta is recommendable in Loeys-Dietz syndrome to avoid additional aortic events. Objectives: The surgical indications for acute type A aortic dissection (AAAD) in patients in cardiopulmonary arrest remain controversial. Outcomes of AAAD for patients who underwent cardiopulmonary resuscitation (CPR) were evaluated., Methods: Between 2004 and 2018, of the 519 patients who underwent AAAD repair, 34 (6.6%) required CPR before or on starting AAAD repair. The patients were divided into 2 groups, survivors (n = 13) and nonsurvivors (n = 21), to compare the early operative outcomes, including mortality and neurological events., Results: The major cause of cardiovascular collapse requiring CPR was aortic rupture/cardiac tamponade (n = 21 [61.8%]), followed by coronary malperfusion (n = 12 [35.3%]) and acute aortic valve regurgitation (n = 3 [8.8%]). There were 3 (23.1%) patients in the survivors group and 11 (52.4%) in the nonsurvivors group who required ongoing CPR at the beginning of AAAD repair (P = .039). Of these patients, 1 survivor and 6 nonsurvivors could not achieve return of spontaneous circulation after pericardiotomy (P = .045). Although the duration from onset or arrival to the operating room was similar (P = .35 and P = .49, respectively), overall duration of CPR was shorter in survivors (10 minutes [range, 7.5-16 minutes] vs 16.5 minutes [range, 15-20 minutes]; P = .044). All survivors without any neurological deficits showed return of spontaneous circulation after pericardiotomy. Multivariate regression modeling showed that CPR duration >15 minutes was a significant risk factor for in-hospital mortality (P = .0040)., Conclusions: CPR duration beyond 15 minutes may be a contraindication for AAAD repair. Objective: The purpose of this study was to determine the clinical outcomes of thoracic endovascular aortic repair in 8 patients with acute type A aortic dissection with an entry tear in the descending aorta., Subjects and Methods: From January 2016 to December 2018, eight patients (mean age 76 years; range 54-92 years) were treated by thoracic endovascular aortic repair due to high operative risk for conventional open repair. All patients had significant comorbidities, and two had critical organ malperfusion due to aortic dissection. Surgical outcomes were retrospectively reviewed., Results: All procedures were technically successful with complete coverage of the entry tear. The proximal landing zone was Zone 1 in 2, Zone 2 in 1, Zone 3 in 4, and Zone 4 in 1 patient. Patients requiring Zone 1 and 2 thoracic endovascular aortic repair underwent aortic arch bypass simultaneously. Mean operation time was 132 min. There were no hospital deaths and no serious complications, including stroke and spinal cord ischemic injury. All patients had complete thrombosis and shrinkage of the false lumen in the ascending aorta before discharge. During up to 36-month follow-up (mean 20 ± 12 months), there were no adverse aortic events except one who died due to ischemic colitis 4 months after the procedure., Conclusion: Thoracic endovascular aortic repair could be a useful alternative surgical option for patients with retrograde acute type A aortic dissection with an entry in the descending aorta who are not suitable for conventional open surgery. Careful follow-up of such patients is mandatory.

Objective: The midterm outcomes and aortic remodeling after thoracic endovascular aortic repair (TEVAR) for uncomplicated type B aortic dissection (TBAD) were evaluated., Methods: Forty-seven patients (mean age 66 ± 12 years) who underwent TEVAR for uncomplicated TBAD with double-barrel type from January 2012 to December 2017 were retrospectively analyzed. The indication for TEVAR for entry closure was a maximum aortic diameter > 40 mm with a patent false lumen. Twenty-six patients (55.3%) had TEVAR in chronic phase, over 6 months after the onset of aortic dissection., Results: There was no hospital death or serious complication. During follow-up (mean 35 ± 16 months), overall 3-year survival was 95.6 ± 3.1%. A significant trend was observed with a higher rate of shrinkage of overall aortic diameter, expansion of the true lumen, and shrinkage of the false lumen more proximally from the stent graft-covered site. Rate of aortic shrinkage in chronic with aortic diameter more than 50 mm was lower compared with the other (proximal: 33.3% vs. 80-100%, distal 0-16.7% vs. 50-52.9%). Rate of aortic dilation distally to the stent graft-covered site was 28% in chronic compared with 5% in non-chronic. Adverse events were mainly due to distal aortic dilation, and 3-year freedom from all adverse events was 79.8 ± 6.5%., Conclusions: Favorable aortic remodeling of the proximal stent graft-covered site could be expected even in the chronic phase if preoperative aortic dilation over 50 mm is unaccompanied. Careful follow-up focusing on dilation of the distal aortic segment is mandatory especially in patients who underwent TEVAR in chronic phase.

Objectives: In patients with acute type A aortic dissection, the use of the frozen elephant trunk (FET) procedure with total arch replacement (TAR) has been indicated for emergency operations to obtain thrombosis of the distal false lumen (FL). However, data comparing the FET and the classical elephant trunk (CET) procedures, including the incidences of mortality, morbidity, spinal cord injury and aortic remodelling, have not yet been reported. The goal of this study was to compare the early outcomes of TAR with the FET and the CET procedures., Methods: The past 7 years of medical records of 323 patients with type A aortic dissection who underwent emergency surgery were reviewed retrospectively, and 148 patients who underwent TAR were shortlisted for the study. First, the patients were divided into 2 groups, the CET group (n = 115; age 65 ± 12 years) and FET group (n = 33; 67 ± 11 years), to compare the early operative outcomes, including mortality and morbidity. Second, 86 patients (CET 56; FET 30) fulfilling the inclusive criteria (inserted length of elephant trunk ≥5 cm, involvement of dissection at the descending aorta and sufficient computed tomographic evaluation at ≥6 months after the operation) were compared to evaluate the patency and diameter of the FL at each segment of the downstream aorta., Results: The in-hospital mortality rate was 8.1% (12/148), without significant differences between the 2 groups (CET 8.7% vs FET 6.1%; P = 1). No spinal cord ischaemia was encountered in either group. The incidence of postoperative FL patency at the level of the left lower pulmonary vein was 30% in the FET group, which was significantly lower than that in the preoperative state (73%) and in the CET group (77%). The downsizing of the aortic diameter at the distal edge of the CET or the FET, the left lower pulmonary vein and the coeliac axis was significant in the FET group 6 months after TAR., Conclusions: According to our initial experience, the FET compared to the CET procedure showed comparable early complications and an advantage concerning FL thrombosis and aortic remodelling at early follow-up examinations., (© The Author(s) 2019. An 81-year-old woman was referred to our centre for emergency surgery with a suspected diagnosis of acute aortic dissection. Laboratory tests showed marked elevation of serum creatinine and blood urea nitrogen. Enhanced computed tomography (CT) demonstrated Type A aortic dissection with a thrombosed false lumen in the ascending aorta. The primary entry tear was located 2 cm distal to the left subclavian artery. Malperfusion of bilateral renal arteries was also evident due to compression by the false lumen. Considering her poor preoperative condition, thoracic endovascular repair of the entry was performed. The primary entry tear was covered using a covered Zenith TX-D stent graft, and a supplemental non-covered Zenith TX-D stent was deployed from the distal edge of the stent graft to the infrarenal aorta. After 5 days of dialysis and additional renal angioplasty for the stenotic left renal artery, her renal function recovered to normal. Her postoperative course was uneventful. Enhanced CT 1 month after surgery showed obliteration of the false lumen of the ascending aorta and patent renal arteries bilaterally.

Objectives: Aggressive total arch replacement (TAR) to obtain thrombosis of the distal false lumen (FL) in patients with Stanford Type A acute aortic dissection, particularly with a patent FL at the descending aorta, is discussed. The aim of this study was to examine the efficacy of our strategy., Methods: In the last 20 years, we retrospectively reviewed the records of 518 patients with Type A acute aortic dissection who underwent an emergent surgery. Among them, 290 patients with a preoperative patent FL at the descending aorta were enrolled in this study. Patients were divided in 2 groups: the non-TAR group (n = 124; 68 ± 14 years) and the TAR group (n = 166; 61 ± 13 years)., Results: In-hospital mortality was 11% (32/290) without significant difference between the 2 groups (the non-TAR group 13% vs the TAR group 10%, P = 0.45). The rates of FL thrombosis of the entire descending aorta were detected at 32% in the non-TAR group and 41% in the TAR group (P = 0.16). Freedom from distal aortic dilatation ≥50 mm was significantly higher in the TAR group (P = 0.03) than in the non-TAR group. Independent predictors of distal aortic dilatation >50 mm were patients in the non-TAR group (P = 0.01; hazard ratio 3.1, 95% confidence interval 1.28-8.05) and unachieved primary entry tear resection (P = 0.002; hazard ratio 6.2, 95% confidence interval 1.38-8.66)., Conclusions: Our surgical strategy with an aggressive entry resection with higher rate of TAR was acceptable. In patients with a patent FL at the descending aorta, TAR should be considered to prevent the future growth of the distal aorta.

Objectives: Arch repair using the frozen elephant trunk (FET) technique has been utilized to treat Type A acute aortic dissection. In contrast, the long-term outcomes of the non-FET technique focus on the thrombosed false lumen (FL), and distal aortic reoperation rates remain unclear. The goal of our study was to investigate the efficacy and long-term outcomes of the non-FET technique as the benchmark., Methods: We reviewed 518 patients with Type A acute aortic dissection in the last 20 years. Among them, 139 hospital survivors (61 ± 12 years) who had undergone total arch replacement with the non-FET technique were enrolled. A total of 86% (120/139) of patients had a patent FL at the descending aorta preoperatively. The median follow-up period was 41 (1-219) months., Results: No spinal cord ischaemia and new entry tear formation were observed. Postoperative FL thrombosis of the entire descending aorta was obtained in 47% (66/139) and in 39% (47/120) of patients who had a patent FL preoperatively. Freedom from dissection-related distal reoperation rates at 1, 3, 5 and 10 years were 87%, 83%, 81% and 78%, respectively. The results of multivariate analysis indicated that the predictors of a dissection-related reoperation were connective tissue disease [hazard ratio (HR) 4.6, P = 0.006], re-entry at the superior mesenteric artery (HR 2.9, P = 0.04), unachieved primary entry resection (HR 5.3, P = 0.001) and preoperative maximum descending aortic diameter ≥38 mm (HR 11.6, P < 0.001)., Conclusions: Total arch replacement with the non-FET technique was safe and reliable from the viewpoint of spinal cord ischaemia. Further comparative studies between the FET and the non-FET techniques are required.

Aim: To evaluate the surgical outcomes and midterm results of patients under 50 years old with acute type A aortic dissection (AAAD)., Methods: The study population included 51 patients who underwent AAAD repair between 2003 and 2016. Of these 51 patients, 46 (90.1%) were males and 14 (27.5%) had connective tissue disorders. Twenty-five patients (49.0%) had a body mass index >25.0 kg/m 2 and two patients were pregnant. Total arch replacement was performed in 39 patients (76.5%) and hemiarch replacement in 12 patients (23.5%). Twenty patients (39.2%) underwent concomitant surgeries, including root replacement, lower limb bypass, or coronary artery bypass grafting., Results: There was one in-hospital death (2.0%). Median follow-up was 55 months and overall survival rates were 87.7% at 5 years and 81.9% at 10 years. Rates of freedom from reoperation were 60.2% at 5 years and 50.2% at 10 years. Eighteen patients (35.3%) required reoperation, due to progressive aortic dilatation, new dissection in the aortic root, infection, and hemolysis. A patent false lumen represented a significant risk factor for reoperation (P < 0.001). Four patients (7.8%) underwent reoperation within 3 months after the initial repair., Conclusions: Surgical outcomes after AAAD repair for young patients were satisfactory. A patent false lumen significantly increased the need for reoperation (P = 0.002), but did not affect long-term survival. Objectives: Actin, alpha-2, smooth muscle, aorta (ACTA2) mutations are one of the major causes of familial thoracic aortic aneurysms and dissections. The aim of this study was to review our clinical results of young adult patients with aortic disease caused by ACTA2 mutations., Methods: We reviewed the medical records of 251 patients (<50 years old) who underwent surgery for thoracic aortic diseases between 2004 and 2014. Among them, nine patients (3.5%) had ACTA2 mutations. Their average age was 35 years (range 22-47) and two patients (22.2%) were males. No patients fulfilled the diagnostic criteria for Marfan syndrome. Preoperative diagnoses included annulo-aortic ectasia (n = 2), localized dissection of the sinus of Valsalva (n = 2), acute type B aortic dissection (n = 1), and chronic type B (n = 4). Eight patients (88.9%) had hypertension., Results: A thoracoabdominal aortic replacement was required in three patients who had descending replacement for residual chronic type B aortic dissection. A patient who had thoracic endovascular aortic repair for complicated acute type B aortic dissection showed no aortic dilatation for 7 years after TEVAR. Histological results revealed cystic medial necrosis (CMN) in most cases (7/8; 87.5%)., Conclusion: Surgical outcomes for patients with ACTA2 mutations were satisfactory. CMN was a major histological finding and family history of aortic event was detected in only half of the patients with ACTA2 mutations. Despite no characteristic physical findings besides hypertension, connective tissue disease including ACTA2 mutations should be considered for aortic dissection in young adult patients.

Background: To address the lack of information about the size of ruptures associated with chronic dissection in the descending and thoracoabdominal aorta, we evaluated the natural history of this pathology., Methods and Results: We analyzed data from 571 patients (mean age, 69.4±11.6 years) with unrepaired chronic aortic dissection in the descending or thoracoabdominal aorta with a maximal aortic diameter of ≥3.5 cm from 2007 to 2014. This was a cross-sectional study. Data on the timing of computed tomographic scan were as follows: for ruptured cases: at the time of rupture; for nonruptured cases: the initial aortic diameter. Patients with connective tissue disorders were excluded. The primary end point was evidence of aortic rupture on computed tomographic images. The median maximal diameter was 4.3 cm (limits, 3.5-9.0 cm) for all aortas and 5.6 cm (n=31; limits, 3.6-8.0 cm) for ruptured aortas. For aortic diameters of 4.0 to 4.4, 4.5 to 4.9, 5.0 to 5.4, 5.5 to 5.9, and 6.0 to 6.4 cm, the incidence of rupture was 0%, 3.3%, 15.3%, 18.8%, and 28.6%, respectively. The risk factors for rupture were absence of hypertension, chronic heart failure, chronic-phase dissection, and Yale index., Conclusions: The risk of aortic rupture increased with an aortic diameter of ≥5.0 cm in patients with chronic aortic dissection in the descending or thoracoabdominal aorta. We would recommend 5.0 cm as an acceptable size for elective resection of subacute or chronic aortic dissection in the descending or thoracoabdominal aorta., (© 2017 American Heart Association, Inc.)

Objectives: Spiral incision of the thoracic wall towards the tip of a scapula and approach through the sixth intercostal space has been a standard method for the replacement of thoracoabdominal and descending aortic aneurysms. However, the exposure of the proximal lesion of the aorta with the spiral incision is not always sufficient for patients with a lesion extending into the aortic arch. Patients with Marfan syndrome tend to have a flat chest, and exposure using left thoracotomy generally causes difficulty to operate on the aortic arch., Methods: Since May 2012, 47 patients (mean age 51.2 ± 16.1, range 9-79, 33 males) have received a novel incision for better exposure of the extended descending and thoracoabdominal aneurysm. A straight incision instead of the traditional spiral one was made from the axilla to the umbilical region and the fourth to sixth ribs were transected. The latissimus dorsi muscle and thoracodorsal artery were preserved, which could be a source for collateral circulation to the Adamkiewicz artery. There were two emergent operations for acute aortic dissection. Twenty-four patients (51%) had undergone previous proximal aortic operation, and 2 patients undergone debranched thoracic endovascular aneurysm repair of the aortic arch. Connective tissue disorders were diagnosed in 16 (34.0%) patients (Marfan syndrome 13, Loeys-Dietz syndrome 3). All surgeries were performed under profound hypothermia., Results: Seven patients underwent total descending aortic replacement, and the others had Type II thoracoabdominal aortic replacements. Three had partial aortic arch replacement, 5 had total aortic arch replacement and 3 had Y-grafting for the abdominal aorta concomitantly. Operation time was 567 ± 141 min and cardiopulmonary bypass time was 259 ± 60 min. Three patients had a major stroke (6.4%), and 1 had a minor stroke. There was no spinal cord complication among survivors. Hospital mortality rate was 4.3% (2/47). These 2 patients underwent thoracoabdominal aortic replacement, and had a major stroke., Conclusions: This new exposure with straight incision with rib-cross thoracotomy provided excellent exposures for the long segment of the thoracoabdominal aorta, and it enabled extended replacement from the ascending aorta to the abdominal aorta., (© The Author 2016. Objectives: An entry located at aortic arch in acute type A aortic dissection (AAAD) is uncommon. It remains controversial whether or not aggressive primary entry resection should be routinely performed in such patients. We have adopted an aggressive strategy of entry site resection, including total arch replacement (TAR) in patients with arch tears. The purpose of this study was to investigate the efficacy of our surgical management approach, using aggressive primary entry resection., Methods: Between January 2000 and December 2014, we retrospectively reviewed the records of 334 patients with AAAD who underwent emergent surgery. The mean age was 67 ± 13 years (range, 20-95 years). Ninety-five patients (28%) presented with shock vital status, and 84 patients (25%) manifested malperfusion of branched arteries. Primary entry resection was achieved in 95% of patients under an aggressive surgical strategy [hemiarch replacement for 173 (52%) patients and TAR for 161 (48%) patients] concomitant with 22 coronary artery bypass grafts and 38 root replacements. Ninety-six percent of hospital survivors (298/311) were followed for a median of 39 months (range, 0-179 months)., Results: Operation, cardiopulmonary bypass, cardiac arrest, antegrade cerebral perfusion and lower body circulatory arrest times were 447 ± 170, 236 ± 93, 112 ± 74, 115 ± 81 and 54 ± 18 min, respectively. The 30-day mortality rate was 5.4%. The in-hospital mortality rate was 8.4% (6.9% at our hospital). Incidences of postoperative permanent neurological dysfunction, tracheotomy and newly permanent haemodialysis were 6.9, 8 and 2%, respectively, with no spinal cord injuries observed. Complete false lumen thrombosis was achieved in 57% of patients as visualized by postoperative computed tomography angiography. After 3, 5 and 10 years, overall survival rates were 81, 74 and 65%, respectively, and the percentages of patients free from downstream dissection-related reoperation were 89, 86 and 80%, respectively. Multivariable analysis demonstrated that the risk factors for downstream aortic reoperation were patent false lumen, residual primary entry tear and connective tissue disorder., Conclusions: The surgical outcomes following aggressive treatment of AAAD are satisfactory. False lumen thrombosis can be achieved in a relatively high proportion of patients using this technique, resulting in a low rate of subsequent downstream aortic reoperations., (© The Author 2016. Objective: Anastomosis in dissected aorta management remains challenging. The patent false lumen of remnant aorta influences the growth of the residual aorta after hemiarch replacement. We evaluated the beneficial effect of adventitial inversion technique for distal anastomotic reinforcement of hemiarch replacement in acute type A aortic dissection., Methods: From 2006 to 2014, 90 patients with DeBakey type I aortic dissection who underwent hemiarch replacement for acute type A aortic dissection management at the National Cerebral and Cardiovascular Center were retrospectively analyzed. Patients were divided according to the technique used: the adventitial inversion technique in group A and the original Sandwich method with Teflon felt in group S. Surgical variables and aortic morphology from distal aortic anastomosis were evaluated by computed tomography after surgery., Results: The mean follow-up time was 2.2 ± 2.1 years with a follow-up rate of 91.1%. Cardiopulmonary bypass time was 208.2 ± 93.9 minutes in group A and 220.6 ± 93.9 minutes in group S; lower body circulatory arrest time was 51.6 ± 10.2 minutes in group A and 54.5 ± 17.8 minutes in group S. No significant differences were observed between groups. The overall hospital mortality rate was 10.0%. Postoperative false lumen thrombosis rate at proximal descending aorta on enhanced delayed phase computed tomography was significantly higher in group A than in group S., Conclusions: The adventitial inversion technique may facilitate thrombotic closure of the distal false lumen in acute type A aortic dissection management by hemiarch replacement., (Copyright © 2016 The American Association for Thoracic Surgery. Proximal anastomosis is an important operative procedure in type A acute aortic dissection. We report our experience with the proximal stepwise technique, which is widely used during distal anastomosis in total arch replacement, in a series of 53 patients. We treated 53 patients for aortic dissection using this technique in our center. There were no bleeding adverse events during the operations and no early death caused by bleeding. Loeys-Dietz syndrome (LDS) is a recently identified rare connective tissue disorder caused by mutations of the transforming growth factor-β receptors and first described in 2005. It is an autosomal dominant syndrome with 2 different phenotypic expressions-LDS I and II. LDS is characterized by the triad of arterial tortuosity and aneurysm, hypertelorism, and a bifid uvula or cleft palate. We present a case of a 9-year-old boy diagnosed with LDS who underwent urgent thoracoabdominal aortic aneurysm repair followed by total arch replacement and aortic valve-sparing root replacement (AVSRR)., (Copyright © 2016 The Society of Thoracic Surgeons. Background: The aim of this study was to evaluate the early and long-term results of a composite valve graft root replacement for various aortic root diseases., Methods: Between 1978 and 2005, 273 patients with various disorders of the aortic root underwent a composite valve graft root replacement. The mean age of the patients was 47.5 +/- 13.2 years. There were 93 patients with Marfan syndrome, 56 aortitis, and 63 type A aortic dissections. Thirty-nine emergency operations and 55 redo operations were included. For the proximal anastomosis, a skirted technique was used in 157 patients. For the coronary reconstruction, Bentall's original inclusion technique was utilized in 36 patients, a direct button technique in 159, and a graft interposition technique in 63. The mean follow-up was 106 months., Results: The in-hospital mortality was 9.5%. An emergency operation emerged as a significant predictor of early death. The actuarial survival rate was 87.0% and 72.9% at 5 and 15 years, respectively. The age at the operation, aortitis, Marfan syndrome, and use of a standard proximal anastomosis emerged as independent determinants of late death. The actuarial reoperation free rate was 96.3% and 89.7% at 5 and 15 years, respectively. In the patients who underwent the skirted technique the incidence of late graft detachment was less frequent than that of the standard technique., Conclusions: A composite valve graft root replacement is a safe and reliable procedure for various aortic root diseases with stable early- and long-term results. The skirted technique seems to be attractive to avoid late graft detachment even in cases with a fragile inflammatory pathology.

Open in new tab Download slide Open in new tab Download slide OBJECTIVES Resection of a primary entry tear is essential for the treatment of Stanford type A acute aortic dissection (AAAD). In DeBakey type III retrograde AAAD (DBIII-RAAAD), resection of the primary entry tear in the descending aorta is sometimes difficult. The frozen elephant trunk technique and thoracic endovascular aortic repair (TEVAR) enable the closure of the primary entry in the descending aorta. The aim of this study was to investigate the efficacy of resection or closure of primary entry, i.e. entry exclusion, in patients with DeBakey type III retrograde-AAAD. METHODS The medical records of 654 patients with AAAD who underwent emergency surgery between January 2000 and March 2019 were retrospectively reviewed, and 80 eligible patients with DeBakey type III retrograde-AAAD were divided into the excluded (n  = 50; age, 62 ± 12 years) and residual (n  = 30; age, 66 ± 14 years) groups according to postoperative computed tomography angiographic data of the false lumen around the primary entry. The excluded group was defined as having a postoperative false lumen at the level of the elephant trunk or thrombosis of the endograft including primary entry. Patients with early false lumen enhancement around the elephant trunk or an unresected primary entry tear after isolated hemiarch replacement were included in the residual group. The early and long-term surgical outcomes were compared between the groups. RESULTS The in-hospital mortality rate was 8% (6/80), with no significant difference observed between the excluded and the residual groups (10% and 7%, respectively; P  >   0.99). Ninety-five percentage of the patients (20/21) achieved entry exclusion with stent grafts including the frozen elephant trunk procedure and TEVAR. Spinal cord ischaemia was not observed in either group. The cumulative overall survival at 5 years was comparable between the 2 groups (76% and 81% in the excluded and residual groups, respectively; P  =   0.93). The 5-year freedom from distal aortic reoperation rate was significantly higher in the excluded group (97%) than in the residual group (97% vs 66%; P  =   0.008). CONCLUSIONS Not only resection but also closure using the entry exclusion approach for DeBakey type III retrograde-AAAD utilizing new technologies including the frozen elephant trunk technique and TEVAR might mitigate dissection-related reoperations. Background: Loeys-Dietz syndrome (LDS) is a recently recognized connective tissue disorder (CTD) caused by mutations in transforming growth factor-beta receptor (TGFBR)1 and TGFBR2. Surgical outcomes of aortic repair in patients with LDS are poorly known. Methods: We enrolled 16 patients with TGFBR mutations identified by gene analysis in this study. Between 1993 and 2011, they underwent 41 aortic surgical procedures. Ten patients (group D: dissection group) underwent aortic repair for acute or chronic aortic dissection as a first surgical intervention, and 6 patients (group N: nondissection group) underwent surgical treatment for aortic root dilatation. The mean follow-up period was 103.7 ± 92.3 months (range, 2– 276 months). Results: There were no in-hospital deaths. In group N, valve-sparing root replacement (VSRR) was performed in all patients. The residual aorta in 9 patients (90%) from group D required further repairs, 3 times on average. Moreover, in 4 patients (40%), the aorta was entirely replaced in serial procedures. In group N, aortic dissection occurred in only 1 patient (17%). The aortic event-free rates at 5 years were 40% in group D and 80% in group N, respectively (p = 0.819). One late death due to arrhythmia occurred 1 month after VSRR. The cumulative survival rates at 5 years were 100% in group D and 83% in group N, respectively (p = 0.197). Conclusions: Surgical outcomes for patients with LDS were satisfactory. Once aortic dissection occurred, the aorta expanded rapidly, requiring further operations. Objective: Although the surgical result of conventional aortic arch replacement has been improved with sophisticated techniques, it is still a deeply invasive procedure. On the other hand, advanced age has been reported as a factor of increased morbidity and mortality in patients undergoing cardiovascular surgery. The number of octogenarians, however, is steadily increasing. The aim of this study is to analyze the outcome of aortic arch surgery in octogenarians. Methods: From January 1995 to September 2007, 113 octogenarians and 1 nonagenarian underwent aortic arch replacement (mean age, 83.0 ± 2.5; 60 men) in our institute. All patients underwent surgery under hypothermic circulatory arrest. The lowest body temperature during circulatory arrest was below 22°C until 2001 and 28°C after 2002. Antegrade selective cerebral perfusion or retrograde cerebral perfusion was used as an additional brain protection technique. Emergency operations were performed in 37 (32.5%) patients; of them, 28 (75.7%) were for acute aortic dissection. Seventy-four (64.9%) patients underwent total arch aortic replacement and the other 40 (35.1%), hemiarch aortic replacement. Concomitant operations consisted of aortic root replacement in 1 patient, aortic valve replacement in 4, and coronary artery bypass grafting in 25. Results: The average duration of circulatory arrest, myocardial ischemic time, and pump time was 57 ± 21, 123 ± 45, and 224 ± 80 minutes, respectively. The total hospital mortality was 7.9% (9/114), 5.2% (4/77) for elective operations and 13.5% (5/37) for emergency operations (P = .12). The hospital mortality was 19.2% (5/26) until 2001 and decreased to 4.5% (4/88) after 2002 (P = .015). Eleven (9.6%) of the 114 patients had a perioperative stroke, and 8 (7.0%) had transient neurologic dysfunction. Other complications were respiratory failure in 17 (14.9%) patients, bleeding in 6 (5.3%), gastrointestinal tract problems in 3 (2.6%), and mediastinitis in 1 (8.8%) patient. Chronic obstructive pulmonary disease was a multivariate predictor (P < .05) of hospital death and emergency operation was a predictor of perioperative stroke. The postoperative 1-year survival was 84.8%, the 3-year survival was 68.5%, and the 5-year survival was 58.1%. Conclusions: The outcome of conventional aortic arch surgery in octogenarians is improving. The operations were performed with an acceptable operative risk even under emergency situations, including acute aortic dissection. The conventional surgical option for aortic arch diseases should not be abandoned only because of the high chronologic age of the patient. [Copyright &y& Elsevier]
