Implant loosening remains a primary cause of failure of total hip arthroplasty 1 (THA) and is often detected late, when pain occurs. Acoustic emission 2 (AE) analysis is a promising method for early loosening detection, on the supposition that relative movements at the bone-implant interface induce detectable AE signals. To distinguish loosening-induced AE signals from those of stable THA components in vitro investigations are necessary. Substituting human with animal bone for such testing could enable simplified and cost-effective sample preparation. The aim of this study was to investigate whether AE signals differ between bone tissues of different species. AE signals generated by relative movements between TiAl 6 V 4 and human, bovine, and porcine cortical bone were investigated. Per species, 125 movements were analyzed, with 26 AE features identified for each movement. The most important time and frequency features of AE signals from human bone differed significantly from those of both animal species. Signals of human origin were longer and exhibited higher rise time. The main frequency components of human AE signals were in a lower frequency range, with a centroid frequency of 113.7 kHz. Loosening and infection are the main reasons for revision surgery in total hip arthroplasty (THA). Removing partially detached cemented implant components during revision surgery remains challenging and poses the risk of periprosthetic bone damage. A promising approach for a gentler removal of partially detached prostheses involves softening the PMMA-based bone cement by heating it above its glass transition temperature (T G ), thus loosening the implant-cement bond. It is assumed that the T G of PMMA-based bone cement decreases in-vivo due to the gradual absorption of body fluid. Reliable data on T G are essential to develop a heat-based method for removing cemented implant components during revision surgery. The effect of water absorption was investigated in-vitro by ageing PMMA-based bone cement samples for different periods up to 56 days in both Ringer's solution (37 °C) and air (37 °C and 30% humidity). Subsequently, the T G and Vicat softening temperatures of the samples were determined by differential scanning calorimetry and Vicat tests, respectively, according to prescribed methods. Over the entire ageing period, i.e. comparing one day of ageing in air and 56 days in Ringer's solution, the Vicat softening temperature dropped by 16 °C, while the T G dropped by 10 °C for Palacos® R PMMA-based bone cement. Water absorption over time correlated significantly with the Vicat softening temperature until saturation of the PMMA-based bone cement was reached. Based on the T G and Vicat softening temperature measurements, it can be assumed that in body-aged bone cement, an optimal softening can be achieved within a temperature range of 85 °C-93 °C to loosen the bond between the PMMA-based bone cement mantle and the prosthesis stem. In cases where mobility and joint function are impaired after implantation of a THA, weakening of hip movement in both extension/flexion and adduction/abduction may play a role due to shortening of the physiological lever arm of the hip muscles. Mechanical factors of influence include the lateral femoral offset, which affects the lever arm, and the antetorsion angle of the hip prosthesis, which affects the anterior femoral offset. This study aimed to investigate the effect of an altered antetorsion angle of the implant on the hip moments and gait patterns of the patient. For this study, 13 patients with a conventional stem on one side and a calcar-guided short stem implanted on the contralateral side were included. To determine the maximum hip moment, tests were performed on a dynamometer in extension/flexion and adduction/abduction in addition to gait analysis. As a control, a comparison was made with data from a reference group of 30 healthy subjects. Both implants showed similar symmetry indices. There was a significant difference between the implants for adduction moments (p < 0.001). The ratios between the directions of moments showed no significant differences. The joint function measured by isokinetic measurements and gait analysis remains comparable to the healthy control group after short stem arthroplasty, but shows slight changes after conventional stem arthroplasty., (© 2023. Purpose: Femoral defects often make it difficult to achieve sufficient fixation of the stem during revision surgery. No clinical studies comparing modular stems are available and differentiated recommendations are rare. The aim of this study was to compare the fixation of different revision stems in an experimental and standardised manner., Methods: Segmental AAOS type I and III defects were reproduced in four femur pairs and two modular stems of different shape (cylindrical and conical) were implanted. Interfacial stem-bone movements were measured under axial torque application to analyze the stem fixation depending on defect extension., Results: Both stems showed adequate fixation in AAOS type I defect. The defect extension significantly reduced the fixation of both implants. The fixation pattern changed significantly for the cylindrical-shaped stem but was maintained for the conical-shaped stem., Conclusions: Shape as well as the extension of femoral defect have an impact on primary fixation. A type I defect seems to be bridgeable for both stems albeit in a different way. In contrast, stem-demanding activities during the healing phase have to be avoided for the conical-shaped stem in a type III defect, whereas the cylindrical-shaped stem has already ceased to be sufficiently stable in this case.

Background: Currently, short cementless femoral stems are alternative implants to treat osteoarthritis in young patients with sufficient bone quality in order to get a more proximal load transfer. The purpose of the present study was to biomechanically compare joint reconstructions resulting from implantations of a short-stemmed and a conventional hip replacement., Methods: 100 patients (50 short-stemmed and 50 conventional) were retrospectively examined. For evaluation of biomechanical parameters standardised pre- and postoperative X-rays (pelvic AP views) were used and digitally analysed., Results: The horizontal femoral off-set increased within both groups (short: 2.0 mm and conventional: 3.3 mm), with a significant increase in the conventional group. The hip centre of rotation was significantly medialised after both procedures (short: 6.0 mm and conventional: 4.2 mm). Limb length was shorter preoperatively at the arthroplasty side, but increased thereafter (short: 2.1 mm and conventional: 2.6 mm). Compared to the native contralateral side the limb length was almost equal after THA (short: 0.7 mm and conventional: 0.8 mm)., Conclusions: Our study shows that an almost anatomic reconstruction of hip joint biomechanics is possible with a short-stemmed prosthesis compared to a conventional stem, regarding limb length, centre of rotation and offset., (Georg Thieme Verlag KG Stuttgart · New York.)

Purpose: The purpose of this study was to examine the concept of proximal load initiation of a total short-stemmed hip arthroplasty (Metha BBraun, Aesculap, Tuttlingen, Germany) on the basis of bone variations by means of osteodensitometric dual energy X-ray absorptiometry and radiologic measurements., Methods: After power analysis 40 patients were included in this study. DXA examination, radiological and clinical follow-up was performed pre-operatively and postoperatively. Socio-demographic relevancies of bone progression and radiological changes were raised epidemiologically and clinically., Results: Improvement of the Harris hip score from 54.7 points preoperative to 96.7 points postoperative was detected (p < 0.01). Loss of summarized overall net average bone mass density (netavg BMD) could only be manifested after six months compared to the netavg BMD of the postoperative measurement (p < 0.01). After six and 12 months BMD atrophy was shown mainly in regions of interest (ROI) 1, 4 and 7. Positive correlations between changes of BMD and age, male sex and BMI were detectable. No stem had to be revised., Conclusions: The Metha implant shows excellent osseointegration at the coated area of the stem without factors of aseptic loosening in the short term. This study shows that parameters like age, sex and BMI influence BMD progression and stress shielding. Metha implant shows excellent results especially in young patients with good bone stock.

Purpose: In total hip arthroplasty fixation of revision stems can be demanding due to femoral bone loss. Strut grafts are often used for bone augmentation and stabilization of the newly inserted prosthesis. The aim of this study was to assess the effect of strut grafts on primary stability under various stem fixation conditions., Methods: Two different revision stems (cylindrical and conical shape) were implanted into synthetic femora. Following a semicircular transfemoral osteotomy, three deficient femoral bearings were simulated (bony lid reattached with cable wires; weakened lid reattached with cable wires; strut grafts placed to the weakened lid with cable wires). Relative micro-movements were measured between prostheses and bones due to an axial moment applied to the stems., Results: Relative movements correlated to the stem shape. The cylindrical stem showed higher movements increasing significantly with a weakened bony lid and portrayed a slight decrease of movements with strut graft application. No unequivocal influence of the weakened lid could be detected for the conical implant. Strut graft application did not show an additional stabilizing effect., Conclusions: The primary stability of the cylindrical fixation concept decreases with impaired fixation conditions of the femur. A clear restabilizing effect with strut grafts could not be proven. A decrease of primary stability due to the impaired bone could not be observed for the conical stem shape. Additionally, strut grafts do not enhance fixation for this stem shape. We conclude that surgeons should not rely on a stabilizing effect of strut grafts in revision hip surgery.

Modular neck implants are an attractive treatment tool in total hip replacement. Concerns remain about the mechanical stability and metal ion release caused by the modular connection. Five different implant designs were investigated in an experimental set-up. In vivo conditions were simulated and the long-term titanium release was measured. Finally, the modular connections were inspected for corrosion processes and signs of fretting. No mechanical failure or excessive corrosion could be identified for the implants tested. The titanium releases measured were extremely low compared to in vivo and in vitro studies and were not in a critical range.

This article aims to clarify the influence of design- and manufacturing-related parameters on wear of metal-on-metal (MoM) joint bearings. A database search for publications on wear simulator studies of MoM bearings was performed. The results of published studies were normalized; groups with individual parameters were defined and analyzed statistically. Fifty-six investigations studying a total of 200 implants were included in the analysis. Clearance, head size, carbon content, and manufacturing method were analyzed as parameters influencing MoM wear. This meta-analysis revealed a strong influence of clearance on running-in wear for implants of 36-mm diameter and an increase in steady-state wear of heat treated components., ((c) 2009 Orthopaedic Research Society. Implants with surfaces of various porosities and pore sizes are in clinical use. This article demonstrates how macroscopic porous metal fragments can detach from the implant surface in total hip arthroplasty (THA) and cause significant third-body damage such as deep scratches and indentations in implants' bearing surfaces. Radiographs prior to revision surgery due to aseptic loosening of the acetabular component revealed the presence of numerous small metal fragments approximately 1 to 2 mm in size in the periarticular area. The size, shape, and material of the metal fragments (cobalt-chromium-molybdenum [CoCrMo]) indicated that they originated from the porous metal surface. In this case, the acetabular liner composite material consisted of two-thirds polyurethane and one-third aluminium oxide ceramic. The femoral head was made of aluminium oxide ceramic. The aluminium oxide femoral head, which had been in situ for 21 years, showed no signs of macroscopic indentations or scratches, suggesting that an aluminium oxide bearing surface, which is significantly harder than the CoCrMo debris, is not significantly affected by metal debris embedment in the counterface material. The polyurethane-aluminium oxide composite material used for the acetabular liner is not comparable to a traditional ceramic bearing surface material. Debris damaged the surface of and became embedded in the liner, causing accelerated wear of the femoral head. In porous metal surface THA, ceramic-on-ceramic bearing couples should, due to their superior hardness, be considered to prevent excessive wear, including debris embedment and scratching of the bearing surfaces, especially in revision cases.

Background: Metal-on-metal total hip resurfacing arthroplasty is increasingly being performed in young and active patients. Preclinical in vitro testing of implants is usually performed with use of hip simulators, and the serum metal ion concentration is determined for the purpose of monitoring the patients. The goal of this study was to characterize the early running-in period in vivo and in vitro by characterizing metal ion levels., Methods: A well-functioning total hip resurfacing prosthesis was implanted in fifteen consecutive patients, and the serum metal ion concentrations in these patients were then determined preoperatively and at intervals during the first postoperative year (at one, six, twelve, twenty-four, and fifty-two weeks). The number of walking cycles was measured with use of a computerized accelerometer in order to compare walking cycles to hip simulator cycles. In vitro, five similar components were investigated for 3 million cycles with use of a hip simulator. Serum samples were obtained at different time points, and wear was measured by quantifying wear particles and ions in the samples. All patient and simulation serum samples were analyzed with use of inductively coupled plasma-mass spectrometry. One simulator implant was investigated with use of scanning electron microscopy., Results: The serum chromium and cobalt levels of the patients continuously increased during the first six months and showed an insignificant decrease thereafter. The molybdenum concentration was unchanged compared with preoperative values. In contrast, the simulator measurements showed a different wear pattern with a high-wear running-in period and a low-wear steady-state phase. The running-in period was delayed by 300,000 cycles and lasted up to 1 million cycles. Scanning electron microscopic analysis showed a carbon-rich protein film predominantly in the early phases of simulation. Scratches were detected originating from pits filled with aluminum oxide and silicon oxide and from pulled-out carbides that were causing third-body wear., Conclusions: The simulator study allowed an exact characterization of the running-in period and showed a delayed onset of running-in wear. In contrast, the clinical data showed a slow increase in measured ion concentrations. These different wear patterns are probably due to the effects of distribution, accumulation, and excretion of particles and ions in vivo.

This cadaveric study examined fracture loads in cemented and uncemented hip stems. Additionally, individual data and bone quality were analyzed and correlated to fracture patterns and fracture load. Cemented or uncemented hip stems were implanted in a randomized fashion in 10 matched paired fresh-frozen femora (donor median age, 78 years, and donor median weight, 74.2 kg). Bone density was measured before the femurs were fractured under load (maximum load of 10,000 N), and fracture patterns were analyzed according to the Vancouver and Johansson classification systems. In the uncemented group, all of the femurs fractured with a median load of 2625 N (range, 1725-7647 N). In the cemented group, 5 femurs fractured with a median maximum load of 9127 N (range, 2845-10,000 N) and 5 femurs did not fracture with a maximum load of 10,000 N. Fracture load corresponded to 4 times and 8.8 times body weight in the uncemented and cemented groups, respectively. Fracture patterns corresponded to Vancouver type A fractures in uncemented stems and Vancouver type C fractures in cemented hip stems. Analysis showed a significant correlation between fracture load and bone density in the uncemented group, whereas there was no correlation in the cemented group. Patients with poor bone quality treated with an uncemented hip stem are at higher risk for periprosthetic fractures; therefore, we recommend cemented stems in this group of patients. Cementation appears to protect against periprosthetic fractures, probably from internal stiffening of the femoral cavity.
