Purpose: The purpose of this study was to investigate the incidence of hip arthroscopy in patients with labral pathology in the United States from 2011-2018 using a large national database., Methods: Patients who underwent hip arthroscopy from 2011-2018 were identified using Current Procedural Terminology (CPT) codes in the PearlDiver Patients Records Database (Colorado Springs, CO), which is a national database of orthopedic insurance records, including Medicare patients. The incidence of surgeries, age, and gender were all recorded. Groups were analyzed using SPSS version 24 (IBM, Armonk NY). CPT codes 29914/15/16 were introduced January 1, 2011 which is when we began our analysis. CPT-29999 and laterality were not assessed., Results: In total, 35,966 arthroscopies were identified between 2011 and 2018 from a randomly selected sample of 30 million orthopaedic patients from the PearlDiver Mariner dataset. The incidence increased by 85% from 2011 to 2018 (7.31 cases vs 13.54 cases per 100,000 patients). The distribution of the age of patients undergoing hip arthroscopy was bimodal with the mode of each peak at 18 years old and 42 years old, respectively. Females underwent surgery more frequently (67.9%) than males (32.1%). The most common CPT code for hip arthroscopy was 29914 (43.9) which corresponds to a hip arthroscopy with femoroplasty., Conclusions: Our findings corroborate those of previous studies and support the increase in incidence of hip arthroscopy in the United States. We indicate an overall increase of 85% from 2011 to 2018 and support previous trends, such as higher incidence of hip arthroscopy in women. Background: There is a paucity of literature on asymptomatic gluteus medius pathology. Moreover, no studies have examined the prevalence of asymptomatic gluteus medius pathology., Purpose: To describe the prevalence of asymptomatic gluteus medius pathology in patients undergoing hip arthroscopy for femoroacetabular impingement., Study Design: Cross-sectional study; Level of evidence, 3., Methods: A database search of our institution was performed for patients undergoing hip arthroscopy for labral treatment between February 2008 and January 2019. Patients were included if they had gluteus medius pathology identified through magnetic resonance imaging (MRI). Patients were deemed to be asymptomatic if they lacked greater trochanteric hip tenderness, abductor weakness, a positive Trendelenburg sign, or a positive Trendelenburg gait on physical examination. Patients were excluded if they were unwilling to participate or did not have a documented physical examination or MRI read in the database., Results: A total of 2851 hips (2452 patients) met the inclusion/exclusion criteria. Gluteus medius pathology was found in 871 hips (30.6%) on MRI. Symptomatic gluteus medius pathology was observed in 414 (14.5%) hips, of which 305 (10.7%) had tendinosis, 99 (3.5%) had partial-thickness tears, and 10 (0.4%) had full-thickness tears. Asymptomatic gluteus medius pathology was observed in 457 (16.0%) hips, of which 408 (14.3%) had tendinosis and 49 (1.7%) had partial-thickness tears. No hips with full-thickness tears on MRI were asymptomatic. Patients with asymptomatic partial-thickness tears were significantly older than those with only tendinosis (45.3 vs 39.4 years, respectively; P = .001). Patients aged 40 years or older had a 2.11 (1.80-2.50) ( P < .001) relative risk of asymptomatic pathology compared with patients younger than 40 years., Conclusion: Although there is a meaningful prevalence of asymptomatic gluteus medius tendinosis, the prevalence of asymptomatic gluteus medius tears is low. Treatment of gluteus medius tendinosis should therefore be based not solely on MRI findings but rather on a complete clinical evaluation. In contrast, MRI findings of partial or full-thickness gluteus medius tears may be more likely to have clinical significance.

Introduction: With the rapid rise in arthroscopy rates for the management of Femoroacetabular Impingement (FAI) Syndrome, it is important to understand current surgical rates and the impact of these surgeries within the Military Health System (MHS). The purpose of this study was to provide an epidemiological descriptive summary of hip arthroscopy for FAI Syndrome in the MHS and describe perioperative healthcare utilization variables., Methods: Eligible beneficiaries ages 18-50, undergoing hip arthroscopy with 2-year follow-up after surgery were included. Healthcare utilization data were abstracted from the MHS Data Repository (MDR) from June 2003 to July 2015, and included all visits, costs, procedures, and prescriptions taking place in both military and civilian hospitals worldwide., Results: 1870 patients were included (mean age 32.2 years; 55.5% male). 51.7% of the procedures took place in military versus 48.3% in civilian hospitals. Mean hip-related healthcare costs in the 2-year following surgery were $15,434 per patient. Patients had a median of 3 opioid prescriptions and 72% had a comorbidity present after surgery. Generally, rates of surgery grew annually from 66 cases in 2004 to 422 cases in 2013. Overall complication rates were comparable to other published reports. Procedures in both military and civilian hospitals had the same rates of femoroplasty and labral repairs, however acetabuloplasty procedures occurred at a higher rate in military (18.9%) vs civilian (14.7%) hospitals. Only 58.8% of patients had physical therapy in the year prior to surgery, while 82.7% had it after surgery. Additionally, 50% of patients had received opioid prescriptions in the 1 year prior to surgery, while 38.9% had 3 or greater opioid prescriptions beyond the initial perioperative fill within the 2-year follow-up., Conclusion: Rates of arthroscopy have grown in the MHS over the last decade. Complication rates are similar to those reported in other populations and settings. Utilization of physical therapy was much more likely after surgery than prior to it. Purpose: Predictors of outcome after femoroacetabular impingement (FAI) surgery are not well-documented. This study presents data from the Danish Hip Arthroscopy Registry (DHAR) for such analyses. The purpose of this study was to identify predictors of poor outcome after FAI surgery in a Danish FAI population. The primary hypothesis was that older patients, patients with severe cartilage damage and female patients might have inferior outcome results compared with younger patients, patients with minor cartilage damage and male patients., Methods: Radiological and surgical data as well as patient-reported outcome measures (PROM) from FAI patients in DHAR between January 2012 and May 2015 were collected. PROMs consisting of Copenhagen Hip and Groin Outcome Score (HAGOS), quality of life (EQ-5D), Hip Sports Activity Scale (HSAS) and Numeric Rating Scale (NRS) pain scores were assessed. The patients were divided into three age groups (< 25, 25-39 and ≥ 40 years). Cartilage injuries were classified according to International Cartilage Repair Society (ICRS) (femoral side) and modified Becks (acetabular side) classifications. A non-parametric statistic method was used to analyze the differences between the preoperative and postoperative PROM values., Results: Data from 2054 FAI procedures in DHAR was collected. 53% of the procedures were done in female patients. All HAGOS sub-scales, EQ-5D, HSAS and NRS pain (rest and walk) demonstrated significant improvements in all age groups at follow-up. Comparison between age groups demonstrated poorer outcomes in both older age groups when compared with the < 25 years age group at 1- and 2-year follow-ups. Higher degrees of femoral and acetabular cartilage injury did have a negative influence on outcome at follow-up. Comparison between genders demonstrated lower preoperative outcomes in females and lower outcome score (HSAS) 1 and 2 years after FAI surgery., Conclusions: Age above 25 and major cartilage injury might negatively affect the outcome of surgery, however, gender could not be identified as a negative predictor of clinical outcome after FAI surgery, but might negatively affect sports participation in females., Level of Evidence: III, prospective cohort study.

Objective: The International Cartilage Repair Society classification is the one mainly used to define chondral defects. However, this classification does not include delamination. The objective of the study is to describe the characteristics of this lesion to better explain its classification in the context of chondral lesions of the hip., Design: We performed a retrospective analysis of 613 patients who underwent hip arthroscopy. In this group, the incidence, localization, histological characteristics, and association to femoroacetabular impingement as well as to other intraarticular lesions of acetabular delamination (AD) were analyzed. Preoperative magnetic resonance imaging accuracy and the different treatment options were also evaluated., Results: In our series, the incidence of the AD was 37% (226 patients over 613). The average age of this group was significantly lower (39.3 years) than the entire group of patients. Isolated cam ( P < 0.01) and pincer morphologies ( P < 0.05) had a significant statistical association with the AD. This lesion was primarily localized at the acetabular chondrolabral junction, mainly on the anterosuperior quadrant. The intraarticular lesions more frequently associated to AD were labral lesions (94.25%, P < 0.01), ligamentum teres lesions (28.32%, P < 0.05), and femoral head chondral lesions (19.9%, P < 0.01). The histological examination of the AD was characterized by hypocellularity and structural disorder of the matrix, with fissures. Treatment remains controversial., Conclusion: AD represents an intermediate stage in chondral damage and can be classified as a "2a" grade lesion. Histological examination confirms the intermediate and progressive character of this injury.

Purpose: (1) To estimate the frequency of subspine impingement (SSI) morphology in patients with a diagnosis of femoroacetabular impingement (FAI) and (2) to describe the performance of the alpha angle, range of motion, and femoral and acetabular anteversion for the identification of cases with and without SSI morphology., Methods: We performed a retrospective observational study of patients with symptomatic FAI evaluated by computed tomography between February 2015 and June 2017. SSI morphology was identified using a 3-dimensional dynamic study with Move Forward software. A case was considered positive if a contact area of the anterior inferior iliac spine with the femoral neck was evidenced. Measurements of acetabular anteversion, femoral anteversion, the lateral center-edge angle, the alpha angle, and the neck-shaft angle, as well as range-of-mobility data, were collected., Results: The study included 135 patients (194 hips), with a mean age of 39.1 ± 13.9 years; 65.2% were women. SSI morphology was found in 23.7% of hips (46 hips) (95% confidence interval, 18.3%-30.2%). Of the hips identified with SSI, 52.2% had a type I anterior inferior iliac spine, 41.3% had type II, and 6.5% had type III. In hips with SSI, median femoral anteversion was 5.6° (interquartile range, 2.1°-7.5°) and values of less than 8° would increase the suspected SSI morphology (81.8% sensitivity, 70.5% specificity)., Conclusions: SSI morphology is a frequent finding in patients with symptomatic FAI through a 3-dimensional dynamic study. Background: Femoroacetabular impingement syndrome (FAI), a hip disorder affecting active young adults, is believed to be a leading cause of hip osteoarthritis (OA). Current management approaches for FAI include arthroscopic hip surgery and physiotherapy-led non-surgical care; however, there is a paucity of clinical trial evidence comparing these approaches. In particular, it is unknown whether these management approaches modify the future risk of developing hip OA. The primary objective of this randomised controlled trial is to determine if participants with FAI who undergo hip arthroscopy have greater improvements in hip cartilage health, as demonstrated by changes in delayed gadolinium-enhanced magnetic resonance imaging (MRI) of cartilage (dGEMRIC) index between baseline and 12 months, compared to those who undergo physiotherapy-led non-surgical management., Methods: This is a pragmatic, multi-centre, two-arm superiority randomised controlled trial comparing hip arthroscopy to physiotherapy-led management for FAI. A total of 140 participants with FAI will be recruited from the clinics of participating orthopaedic surgeons, and randomly allocated to receive either surgery or physiotherapy-led non-surgical care. The surgical intervention involves arthroscopic FAI surgery from one of eight orthopaedic surgeons specialising in this field, located in three different Australian cities. The physiotherapy-led non-surgical management is an individualised physiotherapy program, named Personalised Hip Therapy (PHT), developed by a panel to represent the best non-operative care for FAI. It entails at least six individual physiotherapy sessions over 12 weeks, and up to ten sessions over six months, provided by experienced musculoskeletal physiotherapists trained to deliver the PHT program. The primary outcome measure is the change in dGEMRIC score of a ROI containing both acetabular and femoral head cartilages at the chondrolabral transitional zone of the mid-sagittal plane between baseline and 12 months. Secondary outcomes include patient-reported outcomes and several structural and biomechanical measures relevant to the pathogenesis of FAI and development of hip OA. Interventions will be compared by intention-to-treat analysis., Discussion: The findings will help determine whether hip arthroscopy or an individualised physiotherapy program is superior for the management of FAI, including for the prevention of hip OA., Trial Registration: Australia New Zealand Clinical Trials Registry reference: ACTRN12615001177549 . Femoroacetabular impingement may be particularly disabling to the high-demand athlete, especially those with significant cutting and pivoting requirements. If nonoperative treatment fails to adequately alleviate symptoms or sufficiently restore function in the athlete, hip arthroscopy can lead to improved pain, improved range of motion, and high rates of return to play with proper postoperative rehabilitation. The rate of return to previous level of competition is also high with accurate diagnosis and well-executed correction of deformity. A clear understanding of the etiology, diagnosis, management, and outcomes is essential for clinicians to optimally help patients to return to play., (Copyright © 2016 Elsevier Inc. Background: Hip arthroscopy has allowed the diagnosis and treatment of chondral injuries., Methods: We retrospectively analysed the intraoperative data of 359 patients treated with hip arthroscopy from January 2012 to December 2013. We estimated the frequency, location and extension of acetabular cartilage (AC) injuries and their correlation to femoroacetabular impingement (FAI)., Results: Grade 1 and 2 acetabular chondral lesions were not statistically significant in incidence, location and extension. Chondral lesions were absent on the acetabulum in 3.9% of cases, significantly lower compared to the femoral head (32.0%). 101 (28.1%) were affected by an acetabular chondral lesion. In 244 (68.0%) the chondral lesion was located on both acetabulum and femoral head. On the acetabulum, peripheral superior and superior-posterior area were frequently involved. Grade 4 acetabular chondral defects showed a significant high location on the superior-anterior, superior and superior-posterior area, involving both the peripheral and central surface. Delamination was present in 113 (31.5%) patients. Patients affected by acetabular delamination showed a reduced injury extension if compared to grade 3 or 4. 81 patients (25%) revealed acetabular chondral lesions grade 2a, 3 and 4 without any radiological, clinical or arthroscopic sign of FAI., Conclusions: Location, extension and degree of hip chondral lesions show a progression toward a progressive degeneration. The high percentage of chondral delamination in the hip must be taken in consideration. The presence of symptomatic chondral lesion in cases with no radiological evidence of FAI, suggests that their aetiopathogenesis could be related to biomechanical alterations.

Hip arthroscopy is an increasingly common procedure, particularly for the treatment of femoroacetabular impingement. Various complications have been previously reported, and the authors sought to further evaluate the safety of this procedure. This study was conducted to identify the incidence of femoral neck fracture as well as treatment and outcomes after arthroscopic femoroplasty. In April 2013, a survey was administered to 28 established hip arthroscopists regarding the breadth of their experience, including the total number of hip arthroscopies and proximal femoroplasties performed and the number of postoperative femoral neck fractures. Fracture type, patient age, patient sex, time to fracture, comorbidities, treatments, and outcomes were queried. The study identified 27,200 total arthroscopies and 14,945 proximal femoroplasties performed by the surgeons, with 11 postoperative proximal femur fractures. The incidence of proximal femur fracture after arthroscopic femoroplasty was 0.07%, based on combined data of high-volume hip arthroscopists at multiple medical centers. Mean time to fracture after arthroscopic femoroplasty was 40.5 (±26.6) days postoperatively. The male-to-female ratio was approximately 1:3 for those with fracture, and mean patient age was 52 (±13) years. More than half of the fractures were caused by violation of weight-bearing precautions. All patients had improvement in symptoms after treatment. Femoral neck fracture after arthroscopic femoroplasty is a rare complication for established hip arthroscopists. It is most common in women and patients older than 50 years. Background: Patients with symptomatic femoroacetabular impingement (FAI) frequently have bilateral deformity and inquire about the prognosis of their contralateral, asymptomatic hip. Idiopathic coxarthrosis has been established as an independent risk factor for joint failure on the other side., Purpose: To determine the prevalence of bilateral arthroscopic treatment for FAI and to identify predictive patient demographics and radiographic findings for bilateral, symptomatic disease., Study Design: Case control study; Level of evidence, 3., Methods: Over a 2-year period, patients receiving single-sided FAI surgery for pain and radiographic deformity were identified as unilateral. In the same period, patients undergoing their second side were labeled bilateral, regardless of when the first surgery was performed. Proximal femoral alpha angle; lateral center edge angle; sagittal center edge angle; acetabular version at 1, 2, and 3 o'clock; and femoral torsion were measured on preoperative computed tomography scans., Results: The unilateral group included 514 patients, and the bilateral group included 132 patients. Women composed 48% of the unilateral group but only 35% of the bilateral group (P = .006). The mean age of unilateral patients was 30.3 (±10.7) years and 27.6 (±9.7) years for the first side of bilateral patients (P = .010). The bilateral hips had higher alpha angles (63.8° vs 59.8°, P = .004), less acetabular anteversion at the 3-o'clock position (13.0° vs 15.9°, P < .001), and similar femoral torsion (15.1° vs 15.5°, P = .793) compared with unilateral hips. A multivariable logistic regression model found that for every 5 years of younger age, 5° higher alpha angle, and 5° decrease in 3-o'clock acetabular version, patients were more likely to undergo bilateral treatment for FAI, by 13.5%, 14.5%, and 25.5%, respectively. In a side-to-side comparison of both hips in bilateral patients, alpha angle (r = 0.72) and acetabular version at 1 o'clock (r = 0.73) showed high correlation., Conclusion: Male sex, younger age, higher alpha angles, and reduced acetabular anteversion at initial presentation are significant risk factors for identifying patients who may ultimately require bilateral surgery for symptomatic FAI. Among bilaterally treated patients, no radiographic parameters were predictive of which side would require treatment first. Patients with FAI requiring surgery should be closely monitored for contralateral hip disease.
