Background: Left ventricular mass (LVM) is an independent risk factor for the prediction of cardiac events. Its assessment is a clinically important diagnostic procedure in cardiology and may be performed by Computed Tomography (CT). The aim of this study was to assess the correlation between the cardiac left ventricular shell volume (LVShV) determined by postmortem Computed Tomography (PMCT) and the anatomic LVM obtained at autopsy and to calculate the myocardial tissue density., Methods: A total of 109 deceased individuals were examined with a 64-slice CT scanner and LVShV was determined. At autopsy, the left ventricle was dissected and weighted. The correlation between LVShV and the anatomic LVM was analysed. Asymmetric left ventricular (LV) hypertrophy was recorded. Inter-observer variability was evaluated, and a density value for myocardial tissue was calculated., Results: The mean age of the deceased was 55 ± 16 years, and 58% was men. We found 30 cases of asymmetric LV hypertrophy. A highly positive correlation existed between LVShV and anatomic LVM (r = 0.857; p < 0.0001), regardless of hypertrophy, asymmetric hypertrophy and gender. The mean difference in the inter-observer variability for LVShV assessment was - 4.4 ml (95% CI: -26.4; 17.6). A linear regression analysis was performed, resulting in a value of 1.265 g/ml for myocardial tissue density. Applying the hitherto used myocardial tissue density of 1.055 g/ml underestimated the anatomic LVM by 18.1% (p < 0.0001)., Conclusion: PMCT is a helpful tool for the assessment of LVM, and LVShV is highly correlated with LVM as assessed by subsequent autopsy. The correlation between the two was independent of gender, hypertrophy and LV asymmetric hypertrophy. We found a higher myocardial tissue density of 1.265 g/ml compared to previous studies. We show that PMCT combined with autopsy may contribute not only to anatomical but also clinical knowledge.

Current autopsy practice in forensic pathology is to a large extent based on experience and individual customary practices as opposed to evidence and consensus based practices. As a result there is the potential for substantial variation in how knowledge is applied in each case. In the present case series, we describe the variation observed in autopsy reports by five different pathologists of eight victims who died simultaneously from traumatic asphyxia due to compression during a human stampede. We observed that there was no mention of the availability of medical charts in five of the reports, of potentially confounding resuscitation efforts in three reports, of cardinal signs in seven reports and of associated injuries to a various degree in all reports. Further, there was mention of supplemental histological examination in two reports and of pre-autopsy radiograph in six reports. We inferred that reliance on experience and individual customary practices led to disparities between the autopsy reports as well as omissions of important information such as cardinal signs, and conclude that such reliance increases the potential for error in autopsy practice. We suggest that pre-autopsy data-gathering and the use of check lists specific to certain injury causes are likely to result in less deviation from evidence-based practices in forensic pathology. Pre-autopsy data-gathering and check lists will help ensure a higher degree of standardization in autopsy reports thus enhancing the quality and accuracy of the report as a legal document as well as rendering it more useful for data-gathering efforts., (Copyright © 2013 Elsevier Ltd and Faculty of Forensic and Legal Medicine. In this pilot study, the authors tested whether photogrammetry can replace or supplement physical measurements made during autopsies and, based on such measurements, whether virtual computer models may be applicable in forensic reconstructions. Photogrammetric and physical measurements of markers denoting wounds on five volunteers were compared. Virtual models of the volunteers were made, and the precision of the markers' locations on the models was tested. Twelve of 13 mean differences between photogrammetric and physical measurements were below 1 cm, which indicates that the photogrammetric method has a high accuracy. The precision of the markers' location on the models was somewhat less, although the method is still promising and potentially superior to the current procedures used for reconstructions. The possibility to measure any distance on a body, even after the autopsy is concluded and the corpse is no longer available, is one of the biggest benefits of photogrammetry., (© 2013 American Academy of Forensic Sciences.)

Introduction: At 2.7% in 1970, the Danish medico-legal autopsy frequency was lower than recent frequencies observed in the Nordic countries (4-24%). The aim of this study was to analyse trends in the number and frequency of Danish medico-legal autopsies., Material and Methods: Data were collected from the computerized archives in the Danish departments of forensic medicine, Statistics Denmark and the Danish National Board of Health., Results: During the 1996-2005 study period, a total of 14,990 medico-legal autopsies were performed. This yielded a medico-legal autopsy frequency of between 2.4% and 2.8%. Our results revealed a certain constancy in the number of medico-legal autopsies and frequency of autopsy with regard to manner of death, sex and number, but we also observed changes in the age composition which involved a decrease in the proportion of young (20-39 years) and an increase in the proportion of middle-aged persons (40-59 years)., Conclusion: Denmark has a lower autopsy frequency than other Nordic countries and this may constitute a problem for quality of death-statistics and research based on autopsy results. If the patterns in medico-legal autopsy frequencies continue, a decrease in the number of medico-legal autopsies is to be expected. As a consequence, education and training of forensic pathologists may be impaired.

Since the beginning of this century, the use of postmortem computed tomography (PMCT) in forensic autopsies has increased. In this study we examined how similar dental charts created using PMCT as a solitary examination mode were to dental charts created using the conventional method of a clinical inspection including intraoral radiographs. A total of 100 previously performed dental identification cases were retrospectively included in the study. For each case, a dental chart was created solely based upon PMCT. The PMCT based dental chart was subsequently compared with the chart created from the previous conventional identification examination. Based upon the accuracy, sensitivity and specificity values PMCT performed very well compared to the conventional method in the identification concerning presence or absence of teeth, the presence of crowns, bridges and endodontic treatments as well as the presence and types of fillings. PMCT performed poorly concerning the extension of fillings and identification of small, tooth-colored fillings. The use of PMCT is a valuable supplement to the conventional methods available for forensic odontologists and may be of great value for initial screening in mass fatalities. [ABSTRACT FROM AUTHOR]

Severe mental illness (SMI) is associated with a reduced life expectancy of up to 20 years. One possible contributor to this fact is dysregulation of the hypothalamus-pituitary-adrenal (HPA)-axis. Looking at the morphology of effector organs, such as the adrenal glands themselves, could reveal insights into organ function and response to possible HPA-dysregulation. This forensic autopsy-based study investigated if there were any morphological changes in adrenal glands between decedents who had previously been submitted to a psychiatric hospital with a diagnosis of schizophrenia (n = 34), bipolar (n = 5), or depressive disorder (n = 20), any other psychiatric diagnosis (n = 36) compared with decedents who had no previous psychiatric admission (n = 40). Length of admissions to psychiatric wards and admission in the 180 days preceding death was included in regression as proxy variables for severity of illness. On the macroscopic level, we found no difference in gland weight or volume. On the microscopic level, we found a 25% increase in cross-sectional area of the zona fasciculata (ZF) in decedents who had a diagnosis of schizophrenia compared with controls (p = 0.033). Other diagnosis groups did not differ from controls. Total admission length was positively correlated with area of the ZF. Lay Summary People with a severe mental disorder may be in a constant state of increased stress, which is harmful. This study looked at the adrenal gland, which produces stress hormones, to see if they were different in deceased persons who had suffered from a severe mental illness. We found that the part of the adrenal gland that produces stress hormones is larger in deceased patients who suffered from schizophrenia, but not other types of psychiatric illnesses, compared to deceased persons with no history of psychiatric illness. Purpose: This paper presents a proposed guideline for the use of post-mortem computed tomography (PMCT) during forensic dental identification. Currently, whole-body PMCT is widely used prior to autopsies for the diagnosis of fractures, organ changes, hemorrhages, and for the localization of foreign bodies, but it may also facilitate the odontological identification process in single cases and in cases involving multiple fatalities. Several studies have described the use of PMCT in forensic odontological work, but we have not found any comprehensive set of guidelines on how to perform a forensic odontological examination using PMCT. The aim was to develop guidelines for creating post-mortem dental charts during forensic odontological identification examinations using the standard functions of PMCT. Methods: A proposed guideline was developed from 15 selected cases examined at the Section of Forensic Pathology, Department of Forensic Medicine at the University of Copenhagen in Denmark from October 2011 to May 2012. Using the functionalities and three-dimensional (3D) reconstructions of OsiriX DICOM-viewer software (Pixmeo Sarl, Bernex, Geneva, Switzerland) we adjusted the contrast and brightness settings and developed a proposed guideline for creating PMCT-based dental charts. A four-step guideline was produced. Conclusion: In our casework, we are currently using the guidelines proposed herein. The use of PMCT has allowed us to target our clinical examinations, greatly improving their efficiency. Furthermore, PMCT allows the storage of data for later documentation and research. The Agatston score (AS) is the gold standard CT calcium scoring method in clinical practice. However, the AS is an indirect method of determining calcium amount, whereas atomic absorption spectroscopy (AAS) can directly measure the calcium amount. Our primary aim was to investigate the association between the AS and the coronary calcium amount measured by AAS. Furthermore, we compared our outcome to the macroscopic and histological coronary calcification and stenosis assessment, thus allowing us to infer a clinical coronary artery status based on post-mortem findings. Deceased individuals were examined with a 64-slice multidetector CT scanner, and the AS was determined. At autopsy, the degree of CAC and stenosis was determined, and the coronary arteries were excised and weighed. The coronary arteries were decalcified in a solution that was examined using AAS to measure the calcium amount. The degree of CAC and stenosis was also assessed by a histological examination. One hundred thirty-two coronary arteries were examined, and AS was highly correlated to the coronary calcium amount, measured by AAS, (r2 = 0.72, Pearson 0.85, p < 0.0001). In cases with AS 0, AAS measurements showed zero or very low calcium amounts. AS was also correlated to macroscopic and histological calcification assessments (Spearman's rho 0.68, p < 0.0001, Spearman's rho 0.82, p < 0.0001). Background:An introduction to a forensic medical homicide database established at the Department of Forensic Medicine in Copenhagen. Content: The database contains substantial clinical and demographic data obtained in conjunction with medico-legal autopsies of victims and forensic clinical examinations of perpetrators in homicide cases in eastern Denmark. Validity and coverage: The database contains information on all homicide cases investigated at the Department of Forensic Medicine in Copenhagen since 1971. Coverage for the catchment area of the department is assumed to be very good because of a medico-legal homicide autopsy rate close to 100%. Regional differences might exist however, due to the fact that the catchment area of the department is dominated by the city of Copenhagen. Conclusions: The strength of the database includes a long running time, near complete regional coverage and an exhaustive list of registered variables. Carbon monoxide (CO) inhalation is a well-known method of committing suicide. There has been a drastic increase in suicide by inhalation of CO, produced from burning charcoal, in some parts of Asia, and a few studies have reported an increased number of these deaths in Europe. CO-related deaths caused by charcoal burning have, to our knowledge, not been recorded in the Danish population before. In this retrospective study we present all autopsied cases of CO poisoning caused by charcoal burning in the period 2008-2012. 19 autopsied cases were identified, comprising 11 suicides, 4 accidents, and 2 cases of maternal/paternal filicide-suicide. The mean age of decedents was 38.2 years and the majority of the decedents were men. In 16 cases carboxyhemoglobin levels were above 50 % and in 14 cases we found distinctive cherry red livor mortis. Various concentrations of ethanol and drugs were found in 9 cases. Data suggest that this method of death has increased significantly in Denmark. Therefore, it is highly relevant to draw attention to the subject, to increase awareness as well as prevent future escalation. [ABSTRACT FROM AUTHOR]

The purpose of this study was to evaluate the lateral angle method for the internal acoustic canal for accurately determining the sex of human skeletal remains using measurements taken from computed tomography ( CT) scans. Previous reports have observed that the lateral angle size in females is significantly larger than in males. The method was applied to an independent series of 77 postmortem CT scans (42 males, 35 females) to validate its accuracy and reliability. The mean lateral angle of the internal acoustic canal was found to be larger in females (46.5°) than in males (43.4°). However, the difference was not statistically significant and the sex differences reported in previous studies were not substantiated. Concealment of pregnancy and newborn infant abandonment are closely associated with neonaticide, the killing of an infant within the first 24 h of life or less than 28-30 days depending on the jurisdiction. Abandonment of newborn infants occurs throughout the world and often the outcome for the infant is death. Together with neonaticide it is felt to be one of the least preventable crimes. In this retrospective study we present all forensically known Danish cases of abandoned newborn infant corpses, covering the period from 1997 to 2008. Eleven newborn infant corpses were found; we registered characteristics of the newborn infants and the circumstances of the cases based on autopsy reports. One further newborn infant was included, dating back to 1992, as it was found to be connected with one of the later cases. The mean age of the women who abandoned their newborn infants was 22 years, and five of the autopsied newborn infants were probably alive when abandoned. In two cases the newborn infants were half siblings and abandoned by the same mother. The time span from abandonment to when the newborn infant was found ranged from hours to 7 years. Two-thirds of the newborn infants were girls (66.6%). The most common means of disposal was in a plastic bag (~60%); only one newborn infant was wearing clothes when found. Causes of death were usually given as asphyxia, brain injury or simply undetermined. Two-thirds of the newborn infants showed signs of violence. Abstract: Computed tomography (CT) has been used routinely at the Department of Forensic Medicine, University of Copenhagen since 2002. A retrospective study was performed in order to correlate CT-scan based diagnoses of cranial and cerebral lesions with macroscopic autopsy diagnoses in 56 cases. The CT-scans were performed by a forensic pathologist. They were obtained by using two different CT-scan protocols. The results showed correct skull fracture diagnoses in 34/56 cases. Fractures were diagnosed partially (9) or missed totally (13) on CT-images in 22 cases. The agreement for fracture diagnoses of the anterior, medial and posterior cranial fossae was 20%, 52% and 60%, respectively. Fractures involving bilateral bones were diagnosed correctly more frequently. The diagnostic agreement regarding brain injuries varied from 0% to 79%. Both the autopsy-reports and CT-scan descriptions need to be standardized in order to secure more exact comparisons in the future. [Copyright &y& Elsevier]

Background: Computed Tomography (CT) has become a widely used supplement to medico legal autopsies at several forensic institutes. Amongst other things, it has proven to be very valuable in visualising fractures of the cranium. Also CT scan data are being used to create head models for biochemicall trauma analysis by Finite Element Analysis. If CT scan data are to be used for creating individual head models for retrograde trauma analysis in the future we need to ascertain how well cranial fractures are captured by CT scan. The purpose of this study was to compare the diagnostic agreement between CT and autopsy regarding cranial fractures and especially the precision with which cranial fractures are recorded. Methods: The autopsy fracture diagnosis was compared to the diagnosis of two CT readings (reconstructed with Multiplanar and Maximum Intensity Projection reconstructions) by registering the fractures on schematic drawings. The extent of the fractures was quantified by merging 3 dimensional datasets from both the autopsy as input by 3D digitizer tracing and CT scan. Results: The results showed a good diagnostic agreement regarding fractures localised in the posterior fossa, while the fracture diagnosis in the medial and anterior fossa was difficult at the first CT scan reading. The fracture diagnosis improved during the second CT scan reading. Thus using two different CT reconstructions improved diagnosis in the medial fossa and at the impact points in the cranial vault- However, fracture diagnosis in the anterior and medial fossa and of hairline fractures in general still remained difficult. Conclusion: The study showed that the forensically important fracture systems to a large extent were diagnosed on CT images using Mutiplanar and Maximum Intensity Projection reconstructions. Difficulties remained in the minute diagnosis of hairline fractures. These inconsistencies need to be resolved in order to use CT scan data of victims for individual head modelling and trauma analysis. [ABSTRACT FROM AUTHOR]

The article presents a study which explored mummification and gave a presentation of various key mummy finds. The different ways of natural mummification include dessication, freezing and bog bodies, among other others. While artificial indicates that mummification is the result of intentional human activity. Other topics discussed include a brief history of mummy studies, methods in mummy research that include mummy autopsy and endoscopy, and relationship between mummies and paleopathology.

Background: Earlier studies have addressed the human total cranial vault thickness and generally found no correlation with sex, age or body weight. However, the thickness of the diploe has not been investigated. Our study has determined the diploeic thickness of the human cranial vault using modern autopsy material. Methods: The diploeic bone thickness was measured in 64 individuals (43 males, 21 females) autopsied at our institute. The thickness was measured by X-raying biopsies trephined at four specific locations on the skull. Complete medical records and pathologic autopsy results were available. Results: There was a statistically significant difference in diploeic thickness between males and females in the frontal region only. Diploeic thickness was highly correlated with total cranial vault bone thickness, except for the left euryon in females. Subsequent analyses failed to reveal any correlations between the diploeic thickness and age and height and weight of the individual. Conclusion: Males overall have a thicker diploe, albeit this difference is statistically significant only in the frontal region. We could not discern any trends as pertains to diploeic thickness versus age, height or weight. Since the thickness of the diploe may be an important parameter in biomechanical modelling of the cranial vault, this means that the diploe can be built into such models based on the total cranial thickness, except for the frontal region where the sexual dimorphism must be taken into account. Our findings are consistent with previous studies relating the total cranial thickness to the same parameters, in that we found a high correlation between diploeic and total cranial thickness (except at the left euryon for females). Finally, we recommend that future studies try to incorporate CT or MR scan imaging, rather than point sampling, in order to achieve a total assessment of the dimensionalities of the diploe. Abstract Elevated systemic oxidative stress levels of 8-oxoGuo and 8-oxodG have been reported in individuals with severe mental illness (SMI). As no previous studies have addressed the link between local levels of 8-oxoGuo and 8-oxodG in the central nervous system (CNS), measured in cerebrospinal fluid (CSF), and urinary systemic levels, we employed autopsy-based material to elucidate this aspect. Additionally, we investigated the impact of 8-oxoGuo and 8-oxodG levels on the prevalence of somatic co-morbidities. Based on post mortem samples from deceased individuals with SMI (N = 107), we found significantly elevated urinary levels of both 8-oxoGuo and 8-oxodG compared to mentally healthy living controls. While we found an association between urinary and CSF 8-oxodG levels (r = 0.50, P < 0.001), a similar correlation was not evident for 8-oxoGuo (r = 0.15, P = 0.16). Additionally, the two r -values we r e significantly different (P < 0.001). Neither marker in urine or CSF was associated with obesity-related variables, metabolic syndrome or type 2 diabetes. The post mortem interval did not affect the results, but the agonal phase seemingly introduced bias. This study provided novel insights into the cellular oxidative stress levels in individuals with SMI. We demonstrated that increased oxidative stress locally and systemically is correlated and is a clear phenomenon in SMI. Although post mortem measurements contain some weaknesses, our study indicates DNA as the main site of oxidative stress modifications in the CNS in SMI. This may provide novel opportunities for treatment modalities. Additionally, our study demonstrated the applicability of post mortem material investigating systemic and local 8-oxoGuo and 8-oxodG levels. Objectives: As forensic age estimations in the living are performed without medical indication, there is a need for the development of non-ionizing methods. This study investigates the use of 1.0 T MRI to visualize the ossification status of the medial end of the clavicle. Material and methods: T2 weighted 3D images were collected from a 1.0 T MR system. We prospectively scanned 102 subjects, 47 autopsy cases and 55 living volunteers (12-33 years). Images were scored in blind trials by three observers using a 4-stage system. Observers differed by level of training and radiological expertise. Results: Motion artefacts reduced image resolution in living subjects. However, mean age at stage 4 was significantly different from mean age at stage 2 and 3. The minimum age at stage 4 was 19.8 years. Interobserver agreement was moderate for autopsy cases and slight for living subjects. By contrasting subjects of ''incomplete fusion'' (≤ stage 3 on both sides) against ''fused'' (at least one stage 4), agreement rate rose to moderate (K = 0.414) for living subjects. Despite the low agreement rate, no subject younger than 18 years was assessed as having ''fused'' clavicles. Conclusion: At lower image resolution, a 2-stage system increases agreements rates among observers. To further increase accuracy, clavicle staging needs to be performed by trained observers. If available, a 1.0 T MR system may be used for age estimation in the living. However, further studies are needed to ensure that the ability to discriminate adults from minors, i.e. 18 years, is maintained. [ABSTRACT FROM AUTHOR]

Abstract: The aim of the study was to evaluate the reliability of methods used for forensic dental age estimation. We analysed all cases over the last 21 years (1984–2004) of unidentified bodies that were examined for identification purposes (including age assessment), and of which secure identification was subsequently achieved. In total, the study included 51 cases and 7 different methods had been used for dental age estimation, with the Bang/Ramm and the Gustafson/Johanson methods being the most frequently applied. The age estimates had usually been recorded as 10-year intervals. Factual ages at death were in the range of 6–76 years, with the largest concentration of cases being in the age interval of 25–55 years (34 cases). There was good agreement between estimated age interval and factual age at death in 37/51 (72%) of the cases. In eight cases the factual age at death deviated up to ±5 years from the estimated age, and in six cases by more than 6 years. The average difference between factual age at death and estimated age was 4.5 years. The four subadults in the material were all correctly estimated within an age range of ±3 years. Our study showed that forensic odontological age estimates are reliable. However, the implementation of the specific methods may need to be adjusted concerning age ranges. In the future we recommend to register anamnestic information and the different steps in the methods used. Clinical evaluation should contain more details about attrition, colour, number and presumed age of the restorations and periodontal status. [Copyright &y& Elsevier]
