Motion reconstruction using wearable sensors enables broad opportunities for gait analysis outside laboratory environments. Inertial Measurement Unit (IMU)-based foot trajectory reconstruction is an essential component of estimating the foot motion and user position required for any related biomechanics metrics. However, limitations remain in the reconstruction quality due to well-known sensor noise and drift issues, and in some cases, limited sensor bandwidth and range. In this work, to reduce drift in the height direction and handle the impulsive velocity error at heel strike, we enhanced the integration reconstruction with a novel kinematic model that partitions integration velocity errors into estimates of acceleration bias and heel strike vertical velocity error. Using this model, we achieve reduced height drift in reconstruction and simultaneously accomplish reliable terrain determination among level ground, ramps, and stairs. The reconstruction performance of the proposed method is compared against the widely used Error State Kalman Filter-based Pedestrian Dead Reckoning and integration-based foot-IMU motion reconstruction method with 15 trials from six subjects, including one prosthesis user. The mean height errors per stride are 0.03 ± 0.08   c m on level ground, 0.95 ± 0.37   c m on ramps, and 1.27 ± 1.22   c m on stairs. The proposed method can determine the terrain types accurately by thresholding on the model output and demonstrates great reconstruction improvement in level-ground walking and moderate improvement on ramps and stairs. Lower limb amputation has been associated with secondary impairments such as knee osteoarthritis in the uninvolved limb. Greater knee loading in the frontal plane has been related to severity and rate of progression in knee osteoarthritis. Reduced push-off work from the involved limb can increase uninvolved limb knee loading. However, little is known about specific effects that prosthetic foot damping may have on uninvolved limb loading. We hypothesized that uninvolved limb peak knee internal abduction moment (IAM) and loading rates would be greater when using a high-damping foot compared with a low-damping foot, across walking speeds. Eight healthy, young subjects walked in a prosthesis simulator boot using the experimental feet. Greater uninvolved limb first peak IAM (+16% in fast speed, P = .002; +11% in slow speed, P = .001) and loading rates (+11% in fast speed, P = .003) were observed when using the high-damping foot compared with low-damping foot. Within each foot, uninvolved limb first peak IAM and loading rates had a trend to increase with increased walking speed. These findings suggest that damping properties of prosthetic feet are related to uninvolved limb peak knee IAM and loading rates. [ABSTRACT FROM AUTHOR]

The human ankle produces a large burst of 'push-off' mechanical power late in the stance phase of walking, reduction of which leads to considerably poorer energy economy. It is, however, uncertain whether the energetic penalty results from poorer efficiency when the other leg joints substitute for the ankle's push-off work, or from a higher overall demand for work due to some fundamental feature of push-off. Here, we show that greater metabolic energy expenditure is indeed explained by a greater demand for work. This is predicted by a simple model of walking on pendulum-like legs, because proper push-off reduces collision losses from the leading leg. We tested this by experimentally restricting ankle push-off bilaterally in healthy adults (N=8) walking on a treadmill at 1.4 m s-1, using ankle-foot orthoses with steel cables limiting motion. These produced up to (~50% reduction in ankle push off power and work, resulting in up to ~50%greater net metabolic power expenditure to walk at the same speed. For each 1 J reduction in ankle work, we observed 0.6 J more dissipative collision work by the other leg, 1.3 J more positive work from the leg joints overall, and 3.94 J more metabolic energy expended. Loss of ankle push-off required more positive work elsewhere to maintain walking speed; this additional work was performed by the knee, apparently at reasonably high efficiency. During human walking, the center of pressure under the foot progresses forward smoothly during each step, creating a wheeMike motion between the leg and the ground. This rolling motion might appear to aid walking economy, but the mechanisms that may lead to such a benefit are unclear, as the leg is not literally a wheel. We propose that there is indeed a benefit, but less from rolling than from smoother transitions between pendulum-like stance legs. The velocity of the body center of mass (COM) must be redirected in that transition, and a longer foot reduces the work required for the redirection. Here we develop a dynamic walking model that predicts different effects from altering foot length as opposed to foot radius, and test it by attaching rigid, arc-like foot bottoms to humans walking with fixed ankles. The model suggests that smooth rolling is relatively insensitive to arc radius, whereas work for the step-to-step transition decreases approximately quadratically with foot length. We measured the separate effects of arc-foot length and radius on COM velocity fluctuations, work performed by the legs and metabolic cost. Experimental data (A/=8) show that foot length indeed has much greater effect on both the mechanical work of the step-to-step transition (23% variation, P=0.04) and the overall energetic cost of walking (6%, P=0.03) than foot radius (no significant effect, P>0.05). We found the minimum metabolic energy cost for an arc foot length of approximately 29% of leg length, roughly comparable to human foot length. Lower-limb amputees expend more energy to walk than non-amputees and have an elevated risk of secondary disabilities. Insufficient push-off by the prosthetic foot may be a contributing factor. We aimed to systematically study the effect of prosthetic foot mechanics on gait, to gain insight into fundamental prosthetic design principles. We varied a single parameter in isolation, the energy-storing spring in a prototype prosthetic foot, the controlled energy storage and return (CESR) foot, and observed the effect on gait. Subjects walked on the CESR foot with three different springs. We performed parallel studies on amputees and on non-amputees wearing prosthetic simulators. In both groups, spring characteristics similarly affected ankle and body center-of-mass (COM) mechanics and metabolic cost. Softer springs led to greater energy storage, energy return, and prosthetic limb COM push-off work. But metabolic energy expenditure was lowest with a spring of intermediate stiffness, suggesting biomechanical disadvantages to the softest spring despite its greater push-off. Disadvantages of the softest spring may include excessive heel displacements and COM collision losses. We also observed some differences in joint kinetics between amputees and non-amputees walking on the prototype foot. During prosthetic push-off, amputees exhibited reduced energy transfer from the prosthesis to the COM along with increased hip work, perhaps due to greater energy dissipation at the knee. Nevertheless, the results indicate that spring compliance can contribute to push-off, but with biomechanical trade-offs that limit the degree to which greater push-off might improve walking economy. Simple dynamic walking models based on the inverted pendulum predict that the human body's center of mass (COM) moves along an arc during each step, with substantial work performed to redirect the COM velocity in the step-to-step transition between arcs. But humans do not keep the stance leg perfectly straight and need not redirect their COM velocity precisely as predicted. We therefore tested a pendulum-based model against a wide range of human walking data. We examined COM velocity and work data from normal human subjects (N=10) walking at 24 combinations of speed (0.75 to 2.0 m s-1) and step length. These were compared against model predictions for the angular redirection of COM velocity and the work performed on the COM during redirection. We found that the COM is redirected through angular changes increasing approximately linearly with step length (R2=0.68), with COM work increasing with the squared product of walking speed and step length (R2=0.82), roughly in accordance with a simple dynamic walking model. This model cannot, however, predict the duration of COM redirection, which we quantified with two empirical measures, one based on angular COM redirection and the other on work. Both indicate that the step-to-step transition begins before and ends after double support and lasts about twice as long - approximately 20-27% of a stride. Although a rigid leg model can predict trends in COM velocity and work, the non-rigid human leg performs the step-to-step transition over a duration considerably exceeding that of double support. The plantigrade human foot rolls over the ground during each walking step, roughly analogous to a wheel. The center of pressure progresses on the ground like a wheel of radius 0.3 L (leg length). We examined the effect of varying foot curvature on the mechanics and energetics of walking. We controlled curvature by attaching rigid arc shapes of various radii to the bottoms of rigid boots restricting ankle motion. We measured mechanical work performed on the center of mass (COM), and net metabolic rate, in human subjects (N=10) walking with seven arc radii from 0.02-0.40 m. Simple models of dynamic walking predict that redirection of COM velocity requires step-to-step transition work, decreasing quadratically with arc radius. Metabolic cost would be expected to change in proportion to mechanical work. We measured the average rate of negative work performed on the COM, and found that it followed the trend well (r²=0.95), with 2.37 times as much work for small radii as for large. Net metabolic rate (subtracting quiet standing) also decreased with increasing arc radius to a minimum at 0.3 L, with a slight increase thereafter. Maximum net metabolic rate was 6.25 W kg-1 (for small-radius arc feet), about 59% greater than the minimum rate of 3.93 W kg-1, which in turn was about 45% greater than the rate in normal walking. Metabolic rate was fit reasonably well (r²=0.86) by a quadratic curve, but exceeded that expected from COM work for extreme arc sizes. Other factors appear to increase metabolic cost for walking on very small and very large arc feet. These factors may include effort expended to stabilize the joints (especially the knee) or to maintain balance. Purpose: Understanding muscle-tendon forces (e.g., triceps surae and Achilles tendon) during locomotion may aid in the assessment of human performance, injury risk, and rehabilitation progress. Shear wave tensiometry is a noninvasive technique for assessing in vivo tendon forces that has been recently adapted to a wearable technology. However, previous laboratory-based and outdoor tensiometry studies have not evaluated running. This study was undertaken to assess the capacity for shear wave tensiometry to produce valid measures of Achilles tendon loading during running at a range of speeds. Methods: Participants walked (1.34 m⋅s-1) and ran (2.68, 3.35, and 4.47 m⋅s-1) on an instrumented treadmill while shear wave tensiometers recorded Achilles tendon wave speeds simultaneously with whole-body kinematic and ground reaction force data. A simple isometric task allowed for the participant-specific conversion of Achilles tendon wave speeds to forces. Achilles tendon forces were compared with ankle torque measures obtained independently via inverse dynamics analyses. Differences in Achilles tendon wave speed, Achilles tendon force, and ankle torque across walking and running speeds were analyzed with linear mixed-effects models. Results: Achilles tendon wave speed,Achilles tendon force, and ankle torque exhibited similar temporal patterns across the stance phase of walking and running. Significant monotonic increases in peak Achilles tendon wave speed (56.0-83.8 m⋅s-1), Achilles tendon force (44.0-98.7 N⋅kg-1), and ankle torque (1.72-3.68 N⋅m⋅(kg-1)) were observed with increasing locomotion speed (1.34-4.47 m⋅s-1). Tensiometry estimates of peak Achilles tendon force during running (8.2-10.1 body weights) were within the range of those estimated previously via indirect methods. Conclusions: These results set the stage for using tensiometry to evaluate Achilles tendon loading during unobstructed athletic movements, such as running, performed in the field. (1) Ultimate frisbee involves frequent cutting motions, which have a high risk of anterior cruciate ligament (ACL) injury, especially for female players. This study investigated the in-game cutting maneuvers performed by female ultimate frisbee athletes to understand the movements that could put them at risk of ACL injury. (2) Lower-body kinematics and movement around the field were reconstructed from wearable lower-body inertial sensors worn by 12 female players during 16 league-sanctioned ultimate frisbee games. (3) 422 cuts were identified from speed and direction change criteria. The mean cut had approach speed of 3.4 m/s, approach acceleration of 3.1 m/s2, cut angle of 94 degrees, and ground-contact knee flexion of 34 degrees. Shallow cuts from 30 to 90 degrees were most common. Speed and acceleration did not change based on cut angle. Players on more competitive teams had higher speed and acceleration and reduced knee flexion during cutting. (4) This study demonstrates that a lower-body set of wearable inertial sensors can successfully track an athlete's motion during real games, producing detailed biomechanical metrics of behavior and performance. These in-game measurements can be used to specify controlled cutting movements in future laboratory studies. Inertial measurement units (IMUs) are small wearable sensors that have tremendous potential to be applied to clinical gait analysis. They allow objective evaluation of gait and movement disorders outside the clinic and research laboratory, and permit evaluation on large numbers of steps. However, repeatability and validity data of these systems are sparse for gait metrics. The purpose of this study was to determine the validity and between-day repeatability of spatiotemporal metrics (gait speed, stance percent, swing percent, gait cycle time, stride length, cadence, and step duration) as measured with the APDM Opal IMUs and Mobility Lab system. We collected data on 39 healthy subjects. Subjects were tested over two days while walking on a standard treadmill, split-belt treadmill, or overground, with IMUs placed in two locations: both feet and both ankles. The spatiotemporal measurements taken with the IMU system were validated against data from an instrumented treadmill, or using standard clinical procedures. Repeatability and minimally detectable change (MDC) of the system was calculated between days. IMUs displayed high to moderate validity when measuring most of the gait metrics tested. Additionally, these measurements appear to be repeatable when used on the treadmill and overground. The foot configuration of the IMUs appeared to better measure gait parameters; however, both the foot and ankle configurations demonstrated good repeatability. In conclusion, the IMU system in this study appears to be both accurate and repeatable for measuring spatiotemporal gait parameters in healthy young adults. [ABSTRACT FROM AUTHOR]

Walking is not always a free and unencumbered task. Everyday activities such as walking in pairs, in groups, or on structured walkways can limit the acceptable gait patterns, leading to motor behavior that differs from that observed in more self-selected gait. Such different contexts may lead to gait performance different than observed in typical laboratory experiments, for example, during treadmill walking. We sought to systematically measure the impact of such task constraints by comparing gait parameters and their variability during walking in different conditions over-ground, and on a treadmill. We reconstructed foot motion from foot-mounted inertial sensors, and characterized forward, lateral and angular foot placement while subjects walked over-ground in a straight hallway and on a treadmill. Over-ground walking was performed in three variations: with no constraints (self-selected, SS); while deliberately varying walking speed (self-varied, SV); and while following a toy pace car programmed to vary speed (externally-varied, EV). We expected that these conditions would exhibit a statistically similar relationship between stride length and speed, and between stride length and stride period. We also expected treadmill walking (TM) would differ in two ways: first, that variability in stride length and stride period would conform to a constant-speed constraint opposite in slope from the normal relationship; and second, that stride length would decrease, leading to combinations of stride length and speed not observed in over-ground conditions. Results showed that all over-ground conditions used similar stride length-speed relationships, and that variability in treadmill walking conformed to a constant-speed constraint line, as expected. Decreased stride length was observed in both TM and EV conditions, suggesting adaptations due to heightened awareness or to prepare for unexpected changes or problems. We also evaluated stride variability in constrained and unconstrained tasks. We observed that in treadmill walking, lateral variability decreased while forward variability increased, and the normally-observed correlation between wider foot placement and external foot rotation was eliminated. Abstract: The lack of functional ankle musculature in lower limb amputees contributes to the reduced prosthetic ankle push-off, compensations at other joints and more energetically costly gait commonly observed in comparison to non-amputees. A variety of energy storing and return prosthetic feet have been developed to address these issues but have not been shown to sufficiently improve amputee biomechanics and energetic cost, perhaps because the timing and magnitude of energy return is not controlled. The goal of this study was to examine how a prototype microprocessor-controlled prosthetic foot designed to store some of the energy during loading and return it during push-off affects amputee gait. Unilateral transtibial amputees wore the Controlled Energy Storage and Return prosthetic foot (CESR), a conventional foot (CONV), and their previously prescribed foot (PRES) in random order. Three-dimensional gait analysis and net oxygen consumption were collected as participants walked at constant speed. The CESR foot demonstrated increased energy storage during early stance, increased prosthetic foot peak push-off power and work, increased prosthetic limb center of mass (COM) push-off work and decreased intact limb COM collision work compared to CONV and PRES. The biological contribution of the positive COM work for CESR was reduced compared to CONV and PRES. However, the net metabolic cost for CESR did not change compared to CONV and increased compared to PRES, which may partially reflect the greater weight, lack of individualized size and stiffness and relatively less familiarity for CESR and CONV. Abstract: Lower extremity amputation not only limits mobility, but also increases the risk of knee osteoarthritis of the intact limb. Dynamic walking models of non-amputees suggest that pushing-off from the trailing limb can reduce collision forces on the leading limb. These collision forces may determine the peak knee external adduction moment (EAM), which has been linked to the development of knee OA in the general population. We therefore hypothesized that greater prosthetic push-off would lead to reduced loading and knee EAM of the intact limb in unilateral transtibial amputees. Seven unilateral transtibial amputees were studied during gait under three prosthetic foot conditions that were intended to vary push-off. Prosthetic foot-ankle push-off work, intact limb knee EAM and ground reaction impulses for both limbs during step-to-step transition were measured. Overall, trailing limb prosthetic push-off work was negatively correlated with leading intact limb 1st peak knee EAM (slope=−.72±.22; p =.011). Prosthetic push-off work and 1st peak intact knee EAM varied significantly with foot type. The prosthetic foot condition with the least push-off demonstrated the largest knee EAM, which was reduced by 26% with the prosthetic foot producing the most push-off. Trailing prosthetic limb push-off impulse was negatively correlated with leading intact limb loading impulse (slope=−.34±.14; p =.001), which may help explain how prosthetic limb push-off can affect intact limb loading. Prosthetic feet that perform more prosthetic push-off appear to be associated with a reduction in 1st peak intact knee EAM, and their use could potentially reduce the risk and burden of knee osteoarthritis in this population.
