Background: Screws are the most frequently inserted orthopaedic implants. Biomechanical, laboratory-based studies are used to provide a controlled environment to investigate revolutionary and evolutionary improvements in orthopaedic techniques. Predominantly, biomechanical trained, non-surgically practicing researchers perform these studies, whilst it will be orthopaedic surgeons who will put these procedures into practice on patients. Limited data exist on the comparative performance of surgically and non-surgically trained biomechanical researchers when inserting screws. Furthermore, any variation in performance by surgeons and/or biomechanical researchers may create an underappreciated confounder to biomechanical research findings. This study aimed to identify the differences between surgically and non-surgically trained biomechanical researchers' achieved screw tightness and stripping rates with different fixation methods. Methods: Ten orthopaedic surgeons and 10 researchers inserted 60 cortical screws each into artificial bone, for three different screw diameters (2.7, 3.5 and 4.5 mm), with 50% of screws inserted through plates and 50% through washers. Screw tightness, screw hole stripping rates and confidence in screw purchase were recorded. Three members of each group also inserted 30 screws using an augmented screwdriver, which indicated when optimum tightness was achieved. Results: Unstripped screw tightness for orthopaedic surgeons and researchers was 82% (n = 928, 95% CI 81–83) and 76% (n = 1470, 95% CI 75–76) respectively (p < 0.001); surgeons stripped 48% (872/1800) of inserted screws and researchers 18% (330/1800). Using washers was associated with increased tightness [80% (95% CI 80–81), n = 1196] compared to screws inserted through plates [76% (95% CI 75–77), n = 1204] (p < 0.001). Researchers were more accurate in their overall assessment of good screw insertion (86% vs. 62%). No learning effect occurred when comparing screw tightness for the first 10 insertions against the last 10 insertions for any condition (p = 0.058–0.821). Augmented screwdrivers, indicating optimum tightness, reduced stripping rates from 34 to 21% (p < 0.001). Experience was not associated with improved performance in screw tightness or stripping rates for either group (p = 0.385–0.965). Conclusions: Surgeons and researchers showed different screw tightness under the same in vitro conditions, with greater rates of screw hole stripping by surgeons. Additive manufacturing, aka three dimensional (3D) printing, is increasingly being used for personalized orthopedic implants. Additively manufactured components normally undergo further processing, in particular 3D printed locking osteosynthesis plates require post‐printing screw thread creation. The aim of this study was to compare 3D printed threads with machined and hand‐tapped threads for a locking plate application. Pushout tests were performed on 115 additively manufactured specimens with tapered screw holes; additive manufacture was performed at 0°, 20°, 45°, or 90° build orientations. The screw holes were either machined, hand‐tapped or 3D printed. The 3D printed screw holes were left as printed, or run through with a tap lubricated with water or with thread cutting oil. Printed threads run through using oil, with a build orientation of 90°, had comparable pushout force (median: 6377 N 95% confidence interval [CI]: 5616‐7739 N) to machined (median: 6757 N; 95% CI: 6682‐7303 N) and hand‐tapped (median: 7805 N; 95% CI: 7154‐7850 N) threads. As printed threads and those run through using water had significantly lower pushout forces. This study shows for the first time that 3D printed screw threads for a locking osteosynthesis plate application have comparable strength to traditionally produced screw threads. Background: A simple suture technique in transosseous meniscal root repair can provide equivalent resistance to cyclic load and is less technically demanding to perform compared with more complex suture configurations, yet maximum yield loads are lower. Various suture materials have been investigated for repair, but it is currently not clear which material is optimal in terms of repair strength. Meniscal root anatomy is also complex; consisting of the ligamentous mid-substance (root ligament), the transition zone between the meniscal body and root ligament; the relationship between suture location and maximum failure load has not been investigated in a simulated surgical repair. Hypotheses: (A) Using a knottable, 2-mm-wide, ultra-high-molecular-weight polyethylene (UHMWPE) braided tape for transosseous meniscal root repair with a simple suture technique will give rise to a higher maximum failure load than a repair made using No. 2 UHMWPE standard suture material for simple suture repair. (B) Suture position is an important factor in determining the maximum failure load. Study Design: Controlled laboratory study. Methods: In part A, the posterior root attachment of the medial meniscus was divided in 19 porcine knees. The tibias were potted, and repair of the medial meniscus posterior root was performed. A suture-passing device was used to place 2 simple sutures into the posterior root of the medial meniscus during a repair procedure that closely replicated single-tunnel, transosseous surgical repair commonly used in clinical practice. Ten tibias were randomized to repair with No. 2 suture (Suture group) and 9 tibias to repair with 2-mm-wide knottable braided tape (Tape group). The repair strength was assessed by maximum failure load measured by use of a materials testing machine. Micro–computed tomography (CT) scans were obtained to assess suture positions within the meniscus. The wide range of maximum failure load appeared related to suture position. In part B, 10 additional porcine knees were prepared. Five knees were randomized to the Suture group and 5 to the Tape group. All repairs were standardized for location, and the repair was placed in the body of the meniscus. A custom image registration routine was created to coregister all 29 menisci, which allowed the distribution of maximum failure load versus repair location to be visualized with a heat map. Results: In part A, higher maximum failure load was found for the Tape group (mean, 86.7 N; 95% CI, 63.9-109.6 N) compared with the Suture group (mean, 57.2 N; 95% CI, 30.5-83.9 N). The 3D micro-CT analysis of suture position showed that the mean maximum failure load for repairs placed in the meniscus body (mean, 104 N; 95% CI, 81.2-128.0 N) was higher than for those placed in the root ligament (mean, 35.1 N; 95% CI, 15.7-54.5 N). In part B, the mean maximum failure load was significantly greater for the Tape group, 298.5 N (P = .016, Mann-Whitney U; 95% CI, 183.9-413.1 N), compared with that for the Suture group, 146.8 N (95% CI, 82.4-211.6 N). Visualization with the heat map revealed that small variations in repair location on the meniscus were associated with large differences in maximum failure load; moving the repair entry point by 3 mm could reduce the failure load by 50%. Conclusion: The use of 2-mm braided tape provided higher maximum failure load than the use of a No. 2 suture. The position of the repair in the meniscus was also a highly significant factor in the properties of the constructs. Thorough pre-testing is critical in assessing the likely in vivo performance of spinal devices prior to clinical use. However, there is a lack of data available concerning the dynamic testing of lumbar (porcine model) total disc replacements in all six axes under preload conditions. The aim of this study was to provide new data comparing porcine lumbar spinal specimen stiffness between the intact state and after the implantation of an unconstrained total disc replacement, in 6 degrees of freedom. The dynamic, stiffness matrix testing of six porcine lumbar isolated disc specimens was completed using triangle waves at a test frequency of 0.1 Hz. An axial preload of 500 N was applied during all testing. Specimens were tested both in the intact condition and after the implantation of the total disc replacement. Sixteen key stiffness terms were identified for the comparison of the intact and total disc replacement specimens, comprising the 6 principal stiffness terms and 10 key off-axis stiffness terms. The total disc replacement specimens were significantly different to the intact specimens in 12 of these key terms including all six principal stiffness terms. The implantation of the total disc replacement resulted in a mean reduction in the principal stiffness terms of 100%, 91%, and 98% in lateral bending, flexion-extension, and axial rotation, respectively. The novel findings of this study have demonstrated that the unconstrained, low-friction total disc replacement does not replicate the stiffness of the intact specimens. It is likely that other low-friction total disc replacements would produce similar results due to stiffness being actively minimised as part of the design of low-friction devices, without the introduction of stiffening elements or mechanisms to more accurately replicate the mechanical properties of the natural intervertebral disc. This study has demonstrated, for the first time, a method for the quantitative comparative mechanical function testing of total disc replacements and provides baseline data for the development of future devices. Background context Complex testing protocols are required to fully understand the biomechanics of the spine. There remains limited data concerning the mechanical properties of spinal specimens under dynamic loading conditions in six axes. Purpose To provide new data on the mechanical properties of functional spinal unit (FSU) and isolated disc (ISD) spinal specimens in 6 df. Study design Dynamic, six-axis stiffness matrix testing of porcine lumbar spinal specimens. Methods The stiffness matrix testing of lumbar porcine FSU (n=6) and ISD (n=6) specimens was completed in a custom six-axis spine simulator using triangle wave cycles at a frequency of 0.1 Hz. Specimens were first tested without an axial preload, then with an axial preload of 500 N, with equilibration times of both 30 and 60 minutes. Results The stiffness matrices were not symmetrical about the principal stiffness terms. The facets increased all the principal stiffness terms with the exception of axial compression-extension. Significant differences were detected in 15 stiffness terms because of the application of an axial preload in the ISD specimens, including an increase in all principal stiffness terms. There were limited differences in stiffness because of equilibration time of 30 and 60 minutes. Conclusions The assumption of stiffness matrix symmetry used in many previous studies is not valid. The biomechanical testing of spinal specimens should be completed in 6 df, at physiologic loading rates, and incorporate the application of an axial preload. The present study has provided new data on the mechanical properties of spinal specimens and demonstrates that the dynamic stiffness matrix method provides a means to more fully understand the natural spine and quantitatively assess spinal instrumentation. Flexion following arthroplasty of the hip is important for activities of daily living. Studies have highlighted a possible reduction in flexion following Metal-on-Metal Hip Resurfacing Arthroplasty (MoMHRA) but failed to account for inter-subject variability and the possible etiology for this reduction. This in vivo study aims to determine whether flexion is restored following MoMHRA and identify factors that influence it. Charnley Class A patients (n=112) that underwent MoMHRA were reviewed in a dedicated clinic assessing flexion (resurfaced and contra-lateral hips) and outcome. The difference in flexion between both hips was defined as flexion deficit (δflexion). Various patient (age, gender, BMI) and surgical (component orientation, size, head-neck-ratio, offset) factors were examined in terms of their effect on δflexion. MoMHRA-hips had significantly reduced flexion as compared to the native hips. This flexion-deficit correlated with contra-lateral maximum flexion, component size, head-neck-ratio and component orientation. The findings demonstrate that flexion following MoMHRA is strongly correlated to but is reduced in comparison to the native, disease-free, hip flexion. Abstract: Background context: Although a great deal of research has been completed to characterize the stiffness of spinal specimens, there remains a limited understanding of the spine in 6 df and there is a lack of data from dynamic testing in six axes. Purpose: This study details the development and validation of a dynamic six-axis spine simulator. Study design: Biomechanical study. Methods: A synthetic spinal specimen was used for the purpose of tuning the simulator, completing positional accuracy tests, and measuring frequency response under physiological conditions. The spine simulator was used to complete stiffness matrix tests of an L3–L4 lumbar porcine functional spinal unit. Five testing frequencies were used, ranging from quasistatic (0.00575 Hz) to dynamic (0.5 Hz). Tests were performed without an axial preload and with an axial preload of 500 N. Results: The validation tests demonstrated that the simulator is capable of producing accurate positioning under loading at frequencies up to 0.5 Hz using both sine and triangle waveforms. The porcine stiffness matrix tests demonstrated that the stiffness matrix is not symmetrical about the principal stiffness diagonal. It was also shown that while an increase in test frequency generally increased the principal stiffness terms, axial preload had a much greater effect. Conclusions: The spine simulator is capable of characterizing the dynamic biomechanics of the spine in six axes and provides a means to better understand the complex behavior of the spine under physiological conditions. Metal-on-metal hip resurfacing is becoming increasingly popular, and a number of new devices have been recently introduced that, in the short term, appear to have satisfactory outcome but many questions are still open on the biomechanics of the resurfaced femur This could be investigated by means of finite element analysis, but, in order to be effective in discerning potential critical conditions, the accuracy of the models' predictions should be assessed. The major goal of this study was to validate, through a combined experimental-numerical study, a finite element modeling procedure for the simulation of resurfaced femurs. In addition, a preliminary biomechanical analysis of the changes induced in the femoral neck biomechanics by the presence of the device was performed, under a physiologic range of hip joint reaction directions. For this purpose, in vitro tests and a finite element model based on the same specimen were developed using a cadaver femur The study focused on the Conserve Plus, one of the most common contemporary resurfacing designs. Five loading configurations were identified to correspond to the extremes of physiological directions for the hip joint. The agreement between experimental measurements and numerical predictions was good both in the prediction of the femoral strains (R² >0.9), and in the prosthesis micromotions (error <20 μm), giving confidence in the model predictions. The preliminary biomechanical analysis indicated that the strains in the femoral neck are moderately affected by the presence of the prosthesis, apart from localized strain increments that can be considerable, always predicted near the stem. Low micromotions and contact pressure were predicted, suggesting a good stability of the prosthesis. The model accuracy was good in the prediction of the femoral strains and moderately good in the prediction of the bone-prosthesis micromovements. Although the investigated loading conditions were not completely physiological, the preliminary biomechanical analysis showed relatively small changes for the proximal femur after implantation. This validated model can support realistic simulations to examine physiological load configurations and the effects of variations in prosthesis design and implantation technique. Background context High-energy impacts are commonly encountered during sports such as rugby union. Although catastrophic injuries resulting from such impacts are rare, the consequences can be devastating for all those involved. A greater level of understanding of cervical spine injury mechanisms is required, with the ultimate aim of minimizing such injuries. Purpose The present study aimed to provide a greater understanding of cervical spine injury mechanisms, by subjecting porcine spinal specimens to impact conditions based on those measured in vivo. The impacts were investigated using high-speed digital image correlation (DIC), a method not previously adopted for spinal impact research. Study design This was an in vitro biomechanical study. Methods Eight porcine specimens were impacted using a custom-made rig. The cranial and caudal axial loads were measured at 1 MHz. Video data were captured with two cameras at 4 kHz, providing measurements of the three-dimensional deformation and surface strain field of the specimens using DIC. Results The injuries induced on the specimens were similar to those observed clinically. The mean±standard deviation peak caudal load was 6.0±2.1 kN, which occurred 5.6±1.1 ms after impact. Damage observable with the video data occurred in six specimens, 5.4±1.1 ms after impact, and the peak surface strain at fracture initiation was 4.6±0.5%. Conclusions This study has provided an unprecedented insight into the injury mechanisms of the cervical spine during impact loading. Screws are the most commonly inserted orthopaedic implants. However, several variables related to screw insertion and tightening have not been evaluated. This study aimed firstly to assess the effect of insertion variables on screw tightness, secondly to improve methodologies used by researchers when testing screw insertion techniques and thirdly to assess for any learning or fatigue effects when inserting screws. Two surgeons tightened a total of 2280 non-locking, 3.5 mm cortical screws, with 120 screws inserted to what they felt to be optimum tightness whilst varying each of the following factors: different screwdrivers for measuring torque, screwdriver orientation, gloves usage, dominant/non-dominant hand usage, awareness to the applied torque (blinded, unblinded and re-blinded), four bone densities and seven cortical thicknesses. Screws were tightened to failure to determine stripping torque, which was used to calculate screw tightness – ratio between stopping and stripping torque. Screw tightness increased with glove usage, being blinded to the applied torque and with denser artificial bone and with thinner cortices. Considering all the insertions performed, the two surgeons stopped tightening screws at difference values of tightness ((77% versus 66% (p < 0.001)). A learning effect was observed with some parameters including sterile gloves usage and non-dominant hand application. Different insertion conditions frequently changed screw tightness for both surgeons. Given the influence of screw tightness on fixation stability, the variables investigated within this study should be carefully reported and controlled when performing biomechanical testing alongside practicing screw insertion under different conditions during surgical training. • Common variables related to screw insertion impact on tightness and stripping rates. • Awareness of applied torque improves screw insertion and reduces stripping rates. • The tested variables need to be controlled for and reported during biomechanical testing. Finite element (FE) models of long bones are widely used to analyze implant designs. Experimental validation has been used to examine the accuracy of FE models of cadaveric femurs; however, although convergence tests have been carried out, no FE models of an intact and implanted human cadaveric tibia have been validated using a range of experimental loading conditions. The aim of the current study was to create FE models of a human cadaveric tibia, both intact and implanted with a unicompartmental knee replacement, and to validate the models against results obtained from a comprehensive set of experiments. Seventeen strain rosettes were attached to a human cadaveric tibia. Surface strains and displacements were measured under 17 loading conditions, which consisted of axial, torsional, and bending loads. The tibia was tested both before and after implantation of the knee replacement. FE models were created based on computed tomography (CT) scans of the cadaveric tibia. The models consisted of ten-node tetrahedral elements and used 600 material properties derived from the CT scans. The experiments were simulated on the models and the results compared to experimental results. Experimental strain measurements were highly repeatable and the measured stiffnesses compared well to published results. For the intact tibia under axial loading, the regression line through a plot of strains predicted by the FE model versus experimentally measured strains had a slope of 1.15, an intercept of 5.5 microstrain, and an R2 value of 0.98. For the implanted tibia, the comparable regression line had a slope of 1.25, an intercept of 12.3 microstrain, and an R2 value of 0.97. The root mean square errors were 6.0% and 8.8% for the intact and implanted models under axial loads, respectively. The model produced by the current study provides a tool for simulating mechanical test conditions on a human tibia. This has considerable value in reducing the costs of physical testing by pre-selecting the most appropriate test conditions or most favorable prosthetic designs for final mechanical testing.
