Pressure waves from explosions or other traumatic events can damage the neurons of the eye and visual centers of the brain, leading to functional loss of vision. There are currently few treatments for such injuries that can be deployed rapidly to mitigate damage. Brain-derived neurotrophic factor (BDNF) and activation of its receptor tropomycin-related kinase B (TrkB) have neuroprotective effects in a number of degeneration models. Small molecule activators of TrkB, such as N-[2-(5-hydroxy-1H-indol-3-yl)ethyl]-2-oxopiperidine-3-carboxamide (HIOC), cross the blood-brain and blood-retina barriers after systemic administration. We characterize the effects of blast-induced ocular trauma on retinal and visual function. We show that systemic administration of HIOC, a potent small molecule activator of the BDNF/TrkB receptor, preserves visual function in mice exposed to ocular blast injury. The HIOC treatment for one week preserves visual function for at least four months. The HIOC treatment effectively protected vision when the initial dose was administered up to 3 h after blast, but not if the initial treatment was delayed for 24 h. We provide evidence that the therapeutic effect of HIOC is mediated by activation of BDNF/TrkB receptors. The results indicate that HIOC may be useful for managing ocular blast injury and other forms of traumatic optic neuropathy.

Traumatic optic neuropathy is sinister sequelae of craniofacial trauma leading to vision loss. The decision between early medical or surgical intervention is usually individualised. Visual evoked potentials may guide the treatment plan. We describe a young male presenting 5 days after a road traffic accident with no perception of light vision in the right eye. He was managed medically with high dose of intravenous steroids. At the 3-month follow-up, he reported a reversal of vision loss with return of visual acuity to 3/60, which improved to 6/36 at 5 months and remained stable at 8 months., Competing Interests: Competing interests: None declared., (© BMJ Publishing Group Limited 2020. There are a number of surgical options for treatment of idiopathic intracranial hypertension (IIH) when it is refractory to medical treatment and weight loss. Optic nerve sheath fenestration (ONSF) is one of these options. Use of this procedure varies among centers due to experience with the procedure and concern for associated complications that can result in severe loss of vision. This review summarizes the literature concerning post-surgical complications of ONSF for IIH.

Septoplasty is a frequently performed procedure with mostly minor complications. Nevertheless, it may lead to serious complications.The authors present a 29-year-old patient admitted to their hospital 10 days after septoplasty in another institution. He suffered a total loss of vision immediately after the surgery.High-resolution computed tomography and magnetic resonance imaging revealed a direct trauma of the optic canal and an inflamed and edematous optic nerve. Even though a successful endoscopic optic nerve was performed, the patient's loss of vision did not improve after the surgery.Blindness after septoplasty is an extremely rare but a devastating complication. There are only few patients that indicates the optic nerve injury caused by a direct trauma.

Purpose: To report a novel intraocular complication of endoscopic sinus surgery., Methods: Retrospective case report., Results: A 44-year-old otherwise-healthy man underwent endoscopic right ethmoid sinus surgery and immediately developed loss of vision in his right eye. Postoperatively, patient had ipsilateral light perception vision, proptosis, periorbital ecchymosis, and large angle exotropia with virtually no adduction. Orbital computed tomography confirmed violation of the right medial rectus muscle and optic nerve. Dilated fundus examination showed a macular fold with edema and hemorrhages, blockage on angiography of the macula, and subretinal hyperfluorescence in the periphery. After 6 months of observation, his central vision had improved to 20/300 with macular atrophy and choroidal atrophy in the periphery., Conclusion: Macular edema, nonperfusion and ischemia, choroidal nonperfusion (Amalric sign), and atrophy can be iatrogenic sequelae of endoscopic ethmoid sinus surgery.

Background: Traumatic optic neuropathy is characterized by sudden loss of vision following facial trauma leading to variable visual deficits. The purpose of this study was to evaluate recent institutional trends in the treatment of traumatic optic neuropathy, evaluate the outcomes of different treatment strategies, and identify factors associated with improved vision., Methods: Institutional review board approval was obtained to retrospectively review patients diagnosed with traumatic optic neuropathy at a high-volume trauma center from 2004 to 2012. Pretreatment and posttreatment visual acuity was compared using quantitative analysis of standard ophthalmologic conversion., Results: A total of 109 patients met inclusion criteria (74.3 percent male patients), with a mean age of 38.0 ± 17.5 years (range, 8 to 82 years). Management of traumatic optic neuropathy involved intravenous corticosteroids alone in 8.3 percent of patients (n = 9), 56.9 percent (n = 62) underwent observation, 28.4 percent (n = 31) had surgical intervention, and 6.4 percent (n = 7) underwent surgery and corticosteroid administration. Only 19.3 percent of patients returned for follow-up. Vision improved in 47.6 percent of patients, with a mean follow-up of 12.9 weeks. Patients younger than 50 years had a trend toward higher rates of visual improvement, 60 percent versus 16.7 percent (p = 0.15)., Conclusions: The majority of traumatic optic neuropathy patients are unlikely to return for a follow-up examination. Optic nerve decompression has fallen out of favor in the authors' institution, and observation is the most common management strategy. Outcomes following corticosteroid administration and observation are comparable., Clinical Question/level of Evidence: Therapeutic, IV.

Objective: To report a unique case of unilateral blindness secondary to transnasal endoscopic surgery and stenting for right choanal atresia., Case Report: A 24-year-old man was referred with right eye blindness and acute headache, occurring immediately after transnasal endoscopic surgical repair of unilateral right choanal atresia with placement of an endonasal stent. Maxillo-facial computed tomography with three-dimensional reconstruction showed the endonasal stent entering the right nostril, passing through the lamina papyracea into the orbit and running anterior to the optic foramen towards the superior orbital fissure. Despite stent removal and medical treatment (ceftriaxone and dexamethasone), permanent right eye blindness secondary to an irreversible lesion of the optic nerve was diagnosed. At three-month follow up, an uncommon, complete fibrous obliteration of the right nasal fossa was noticed., Conclusion: To the best of our knowledge, this is the only published report of unilateral blindness following transnasal endoscopic stenting for right choanal atresia. Causes of this complication, and ways of avoiding it, are discussed.

Background: The optic nerve and the globes are resistant to mild to moderate trauma and bilateral avulsion of the eyes is rare with only a few cases reported in the literature., Case: We report two cases of traumatic bilateral eye avulsion. The first case was secondary to a bear bite and was managed successfully, although the eyes were not salvageable whereas the second case which was due to physical assault expired due to associated severe head injury., Conclusion: Traumatic bilateral globe avulsion/loss is a rare complication of trauma. In developing countries like Nepal, poverty, forest encroachment activities, extensive deforestation, frequent domestic disturbances and lack of education are some of the circumstances that increase the probability of such visual injuries., (© NEPjOPH.)

Unlabelled: The optic nerve injury is produced by damaging the optic nerve within a craniocerebral trauma. The anatomo-pathology and the pathology of posttraumatic visual impairments are fundamental data for the therapeutic indications. The microscopic examination reveals only the particular cases of very serious or severe extensive lesions. Damage to the vessels of the optic nerve, damage and break of the optical fibers are responsible for immediate visual impairments and secondary hematomas and edemas involving. The factors for the late appearance of visual impairments are numerous and often confounding., Material and Methods: In the present research, we have a female patient D.A., aged 29, diagnosed with closed fracture in the right frontal lobe, irradiated to the optic canal; blindness in the right eye., Results: Break of the optical fibers is a result of bone fragments or of traction and torsion. Damage to the vessels of the optic nerve represents the main cause of blindness for most authors. Traction and torsion of the vessels at the moment of injury may cause thrombosis and small hemorrhages which lead to severe ischemia of the, optical fibers., Conclusions: The optic nerve injury is produced by damaging the optic nerve within a craniocerebral trauma, through an indirect mechanism most of the times.

Purpose of Review: To examine the proposed mechanisms of vision-threatening injuries occurring secondary to orbital and facial trauma: traumatic optic neuropathy (TON), retrobulbar haemorrhage (RBH) and penetrating eye injury. To evaluate the evidence supporting different management options for traumatic vision-threatening injury., Recent Findings: Despite considerable debate over the roles of surgical decompression and systemic steroid therapy for TON, these interventions have not been proved to be more effective than conservative management and there is limited evidence that the use of steroids may be associated with an adverse outcome. Lateral canthotomy and inferior cantholysis have been proven to be effective treatments for RBH. Orbital exploration and surgical evacuation of haematoma remains a second line intervention. Open globe injuries require immediate primary surgical exploration and repair. Irretrievable devastating globe injuries require either enucleation or evisceration. There is no consensus as to which is the best treatment with recent surveys indicating that enucleation is preferred in the USA and evisceration in the United Kingdom., Summary: Conservative management is the first line treatment for TON. The evidence strongly supports lateral canthotomy and inferior cantholysis as best treatment for RBH. There is no consensus as to whether enucleation or evisceration is the best treatment for irretrievable devastating globe injury. The choice of management is currently determined by surgeon preference.

Few cases of ophthalmic complications have been reported following sinus endoscopic surgery with current techniques. Irreversible damage to orbital structures was found in a 21-year-old patient referred to our department for evaluation after sinus endoscopic surgery and ipsilateral amaurosis.

A 59-year-old Chinese man presented with no light perception in the left eye 1 day following functional endoscopic sinus surgery. The patient underwent endoscopic optic nerve decompression with topical and systemic application of nerve growth factor and steroids after a failed trial of high-dose intravenous corticosteroids. Visual acuity improved to 20/25 at 3 weeks and remained stable at 9 months. Reports of full visual recovery are exceedingly rare following this functional endoscopic sinus surgery complication.

Purpose: Traumatic optic neuropathy (TON) is a dangerous complication of head trauma but the mechanism of the symptoms still remains obscure. Moreover, there is no uniformly accepted treatment protocol for this condition. Endoscopic, minimally invasive decompression of the optic nerve in its bony canal is an alternative to conservative approach., Material and Methods: Endoscopic optic nerve decompression was performed in 6 patients, whom head trauma caused blindness or severe impairment of vision. In 5 of them direct optic nerve injury might have been suspected due to presence of bony fractures in the region of the optic canal and the orbital apex. The time from the trauma to the surgical intervention varied from 8 hours to 30 days. All the patients before the attempted surgery were treated with steroids, however the doses and time of this treatment varied significantly., Results: There were no complications of the surgery; all the patients were mobilized on the day of operation and reported no problems with nasal breathing. Vision improved in 2 of 6 patients (33.3%), interestingly both of them had experienced rather gradually developing deterioration of vision during several hours after the trauma., Conclusions: Endoscopic optic nerve decompression is a minimally invasive and safe procedure to be employed for optic nerve decompression in patients with posttraumatic impairment of vision.

Delay in diagnosis and treatment of an elderly woman with a malar fracture that caused retrobulbar haemorrhage resulted in complete loss of vision in the right eye.

Aim: To report severe visual loss caused by optic nerve avulsion (ONA) in children with door-handle trauma., Methods: Clinical records at a tertiary eye care hospital, of 14 children who sustained severe visual loss as a result of door-handle injuries, were reviewed. The data were analysed for location, presenting symptoms and signs, diagnostic studies, intervention, and the cause of visual loss., Results: There were 11 males and three females with an average age of 8 years and an average height of 125 cm. The place of trauma was home in 11 and school in three children. Presenting visual acuity (VA) was light perception (LP) in five patients and no light perception (NLP) in nine. All the 14 children had evidence of ONA and four patients had ruptured eye globes that required initial repair. The diagnosis of ONA was made clinically or by imaging studies and confirmed histopathologically in eyes that were enucleated. Average follow up was 28.8 months (range 4 months to 8 years). Final VA was LP in one patient and NLP in 13 patients, eight eyes required enucleation for painful blind eye or to achieve optimal cosmesis., Conclusion: ONA was the common cause of visual loss in children who sustained ocular trauma caused by door-handles.

Introduction: Visual loss after Le Fort I osteotomy is a devastating complication the mechanism of which is not always clear., Methods: A case report of blindness following Le Fort I osteotomy is presented. The literature on the various skull base complications associated with Le Fort I osteotomies is reviewed and the mechanisms of these complications discussed., Conclusion: The radiological findings in this case are similar to those previously reported. They strongly support the hypothesis that an adverse transmission of forces associated with pterygomaxillary separation via the sphenoid bone to the intra- and extracranial portions of the skull base is the main reason for injury to the optic and other cranial nerves as well as to the branches of the carotid artery.

Purpose: To report a patient whose self-inflicted blindness was not clinically apparent by history or external signs of trauma., Design: Observational case report., Methods: A 12-year-old girl with a history of social and behavioral problems was noted to have visual loss in her right eye. Examination revealed no light perception, optic nerve atrophy, partial upper lid ptosis, exotropia, and hypoesthesia of the cheek, all on her right side., Results: After undergoing extensive examinations which were unrevealing for a diagnosis, the patient admitted to a recurrent maneuver, which she secretly used to relieve anxiety and stress. This maneuver consisted of inserting her index finger under the right supraorbital rim and forcibly subluxating her globe out of the orbital space., Conclusions: Self-inflicted visual loss can occur in nonpsychotic and nonviolent patients. Accurate diagnosis is important, as there is a risk of similar involvement to the fellow eye, and referral for psychiatric counseling is mandatory.

Purpose: We conducted a 19-year review of patients with facial fractures who were treated in the Iranian Maxillofacial Unit at the Mobasher Emergency Hospital, Hamedan Province, Iran, to specifically consider those fractures that resulted in blindness or severe visual impairment., Materials and Methods: During the period of February 16, 1984, to March 20, 2003, a total of 2,503 patients with facial fractures were operatively treated. Of these, 550 (22%) patients had orbital region fractures and were specifically studied., Results: From our facial fractures database, 83 (3.31%) patients were identified as having ocular or extraocular injuries. Of these, 39 patients (1.56%) had severe visual impairment or blindness., Conclusions: Laterally directed forces are implied as major causative factors in blindness or visual impairment. Males (83.3%), left eye (63.3%), third and fourth age decades (53.3%), and motor vehicle accidents (63.3%) were the most commonly involved gender, site, age, and cause of monocular blindness, respectively.

A 4-year-old boy presented with complete blindness in his left eye after accidentally walking into a merchandise display hook while shopping in a department store. An ophthalmologic examination revealed no light perception, an amaurotic pupil, and nasal conjunctival injection in the eye. Intravenous steroids did not improve his vision.

A 30-year-old paranoid schizophrenic man suffered a psychotic episode while flying on an airplane, locked himself in the bathroom, detached the temples of his sunglasses, and stabbed them deeply into both medial orbits. He then secured one temple into the door hinge and rammed the back of his neck repeatedly against it. The injuries caused no light perception from optic nerve trauma and a Brown-Séquard hemitransection of the spinal cord.

This chapter discusses the cases of postoperative blindness reported in the literature and the theories that attempt to explain the mechanisms involved. Although uncommon, alterations in vision and blindness after anesthesia for major surgical procedures, particularly cardiopulmonary bypass or spine surgery, are well documented, with an incidence varying between 0.05% and 1%. Accurate incidence data are unavailable because it is not known what percentages are reported. However, the large number of case reports over many years has provided some significant information. Although sustained compression of the eye is an important cause, postoperative visual loss may also occur, in an unrelated manner, because of ischemic optic neuropathy, central retinal artery or vein occlusion, or cortical blindness.

Purpose: To raise the awareness of the rare complication of monocular blindness resulting from radiofrequency rhizotomy for trigeminal neuralgia., Methods: Case series from tertiary referral centers. Patients were referred after complaint of loss of vision., Results: We report three patients who had trigeminal neuralgia and underwent treatment by radiofrequency rhizotomy. Each developed complete loss of vision in one eye immediately after the procedure. One patient died of an unrelated cause, and pathologic analysis revealed changes compatible with acute trauma to the right optic nerve., Conclusion: Our third case confirms the hypothesis that traumatic optic neuropathy after trigeminal radiofrequency rhizotomy results from malpositioning of the rhizotomy needle through the inferior orbital fissure into the orbital apex rather than the foramen ovale.

A 16-year-old female presented with unilateral blindness in her right eye 2 months after blunt head trauma. Optic nerve edema was demonstrated by funduscopic examination, ultrasonography, and magnetic resonance imaging and failed to respond to medical treatment. Delayed post-traumatic blindness may be a severe complication of head trauma. The late appearance leads to delayed diagnosis and resulting unresponsiveness to treatment. Patients who experience head trauma that could involve the optic nerves should undergo ultrasonography of the optic nerves. An abnormal finding should be followed by an intensive evaluation to determine possible damage.
