Purpose: The purpose of this study was to investigate the impact of a new arterial intravascular pump on the hemodynamic surroundings within the aorta. Methods: A new arterial intravascular pump was placed in the descending aorta, and the effects of three positions within the aorta, as well as the number (n = 1 to 3) of pumps, on arterial flow features, organ perfusion, and blood trauma were investigated using a computational fluid dynamics (CFD) method. Results: It was found that as the pump position was moved backward, the perfusion in the three bifurcated vessels of the aorta arch increased and the pump suction flow decreased, resulting in a reduced high shear stress and decreased residence time in the three branches of the aortic arch. The further posterior the location of the pump, the better the blood flow perfusion to the kidneys, while the perfusion at the bifurcation of the abdominal aorta was reduced, due to the pump suction effect. Compared to the condition with single pump support, the multi-pump assist model can significantly reduce the pump rotating speed, while keeping the same flow patterns, leading to a decreased volume of high shear stress and flow loss. When increasing the number of pumps, the perfusion to the three branches of the aortic arch increased, accompanied by a diminished residence time, and the perfusion to the other aortic branches was decreased. However, the perfusion to the other aortic branches, especially for the renal arteries and even under a three-pump condition, was close to that without pump assistance. Conclusion: The placement of an intravascular pump near the beginning of the suprarenal abdominal aorta was considered the optimal location, in order to improve the hemodynamic surroundings. Increasing the number of pumps can significantly reduce the rotational speed, while maintaining the same flowrate, with a decreased fluid energy loss and a reduced high shear stress. In accordance with "the response to injury" theory, the entry of monocytes into the intima guided by inflammation signals, taking up cholesterol and transforming into foam cells, and egress from plaques determines the progression of atherosclerosis. Multiple cytokines and receptors have been reported to be involved in monocyte recruitment such as CCL2/CCR2, CCL5/CCR5, and CX3CL1/CX3CR1, and the egress of macrophages from the plaque like CCR7/CCL19/CCL21. Interestingly, some neural guidance molecules such as Netrin-1 and Semaphorin 3E have been demonstrated to show an inhibitory effect on monocyte migration. During the processes of monocytes recruitment and migration, factors affecting the biomechanical properties (e.g., the membrane fluidity, the deformability, and stiffness) of the monocytes, like cholesterol, amyloid-β peptide (Aβ), and lipopolysaccharides (LPS), as well as the biomechanical environment that the monocytes are exposed, like the extracellular matrix stiffness, mechanical stretch, blood flow, and hypertension, were discussed in the latter section. Till now, several small interfering RNAs (siRNAs), monoclonal antibodies, and antagonists for CCR2 have been designed and shown promising efficiency on atherosclerosis therapy. Seeking more possible biochemical factors that are chemotactic or can affect the biomechanical properties of monocytes, and uncovering the underlying mechanism, will be helpful in future studies. Local hemodynamic environment is a determinant factor in drug delivery from a drug eluting stent (DES) to the target arterial tissue. By using a simplified model of a DES, we demonstrated that if a DES had a drug release mode of uniform rate the drug released from the stent will distribute non-uniformly along the stent due to the flowing blood, with a significantly higher drug concentration at the distal part of the stent than that at the proximal one. This may explain why a DES could retard neointimal formation and vascular remodeling in downstream coronary segments. To solve this problem, we thereafter optimized the drug release mode of the DES as an exponential function. The simulation results showed that the optimized drug release mode could lead to a fairly uniform drug concentration distribution along the stent. Therefore, the present study suggested that to achieve a more effective result, optimization of drug eluting strategy (drug release mode) for the DES would be essential. [ABSTRACT FROM AUTHOR]

To elucidate the difference between Newtonian and shear thinning non-Newtonian assumptions of blood in the analysis of DES drug delivery, we numerically simulated the local flow pattern and the concentration distribution of the drug at the lumen-tissue interface for a structurally simplified DES deployed in a curved segment of an artery under pulsatile blood flow conditions. The numerical results showed that when compared with the Newtonian model, the Carreau (shear thinning) model could lead to some differences in the luminal surface drug concentration in certain areas along the outer wall of the curved vessel. In most areas of the vessel, however, there were no significant differences between the 2 models. Particularly, no significant difference between the two models was found in terms of the area-averaged luminal surface drug concentration. Therefore, we believe that the shear thinning property of blood may play little roles in DES drug delivery. Nevertheless, before we draw the conclusion that Newtonian assumption of blood can be used to replace its non-Newtonian one for the numerical simulation of drug transport in the DES implanted coronary artery, other more complex mechanical properties of blood such as its thixotropic behavior should be tested. Background: Blood flow condition may have influence upon the hydraulic conductivity of venous graft (Lp,vein) in an arterial bypass, then affecting the accumulation of low-density lipoproteins (LDLs) within the graft wall. To probe this possibility, we first measured in vitro the filtration rates of swine lateral saphenous vein segments under different flow rates, and the correlation of Lp,vein with wall shear stress (WSS) was then obtained.Results: The experimental results showed that when WSS was very low, Lp,vein would increase drastically with WSS from 1.16 ± 0.15 × 10-11 m/s Pa at 0 dyn/cm2 to 2.17 ± 0.20 × 10-11 m/s Pa at 0.7 dyn/cm2, then became constant of approximately 2.33 × 10-11 m/s Pa as the WSS increased further. Based on the experimental results, we assumed three different cases of Lp,vein and numerically simulated the LDLs transport in an arterial bypass model with venous graft. Case A: Lp,vein = 2.33 × 10-11 m/s Pa; Case B: Lp,vein = 1.16 × 10-11 m/s Pa (static condition with WSS of 0); Case C: Lp,vein was shear dependent. The simulation showed that the deposition/accumulation of LDLs within the venous graft wall in Case A was greatly enhanced when compared with that in Case B. However, the LDL accumulation in the graft wall was similar for Case A and Case C.Conclusions: Our study, therefore, indicates that when the venous graft was implanted as a bypass graft, the Lp,vein might remain nearly constant along its whole length except for very few areas where the value of WSS was extremely low (less than 0.7 dyn/cm2) and the effects of Lp,vein modulated by blood flow on LDL transport may be neglected. Background and Aims: The crossed limbs stent graft technique is regularly employed to treat abdominal aortic aneurysm patients with unfavorable aneurysm necks or widely splayed common iliac arteries. This article numerically evaluates the hemodynamic performance of the crossed limbs strategy by analyzing numerical simulations and conducting experiments using two series of idealized bifurcated stent grafts with different cross angles and cross positions.Results: Results demonstrated that the absolute helicity at outlets decreased with increased cross angles and increased with decreased cross positions. The time-averaged wall shear stress remained approximately unchanged, whereas the oscillating shear index and relative resident time decreased slightly when the cross angle increased and cross position decreased in iliac grafts. Additionally, both numerical and in vitro experimental results indicate the displacement force acting on the stent graft gradually increased as cross angles increased and cross positions decreased. Results further indicated that strip areas with a high oscillating shear index and high relative resident time, which may be vulnerable to thrombosis formation, exist along the outer surface of the iliac artery grafts.Conclusion: Given this information, the optimal crossed limbs configuration may contain a small cross angle and low cross position; however, low cross positions may increase the risk of migration.
