Background: The INTERCEPT Blood System pathogen reduction technology (PRT), which uses amotosalen and ultraviolet A light treatment (amotosalen/UV-PRT), inactivates pathogens in plasma and platelet components (PCs). This review summarizes data describing the inactivation efficacy of amotosalen/UVA-PRT for a broad spectrum of viruses and parasites., Methods: Twenty-five enveloped viruses, six nonenveloped viruses (NEVs), and four parasites species were evaluated for sensitivity to amotosalen/UVA-PRT. Pathogens were spiked into plasma and PC at high titers. Samples were collected before and after PRT and assessed for infectivity with cell cultures or animal models. Log reduction factors (LRFs) were defined as the difference in infectious titers before and after amotosalen/UV-PRT., Results: LRFs of ≥4.0 log were reported for 19 pathogens in plasma (range, ≥4.0 to ≥7.6), 28 pathogens in PC in platelet additive solution (PC-PAS; ≥4.1-≥7.8), and 14 pathogens in PC in 100% plasma (PC-100%; (≥4.3->8.4). Twenty-five enveloped viruses and two NEVs were sensitive to amotosalen/UV-PRT; LRF ranged from >2.9 to ≥7.6 in plasma, 2.4 or greater to greater than 6.9 in PC-PAS and >3.5 to >6.5 in PC-100%. Infectious titers for four parasites were reduced by >4.0 log in all PC and plasma (≥4.9 to >8.4)., Conclusion: Amotosalen/UVA-PRT demonstrated effective infectious titer reduction for a broad spectrum of viruses and parasites. This confirms the capacity of this system to reduce the risk of viral and parasitic transfusion-transmitted infections by plasma and PCs in various geographies., (© 2020 The Authors. Background: Blood platelets (PLTs) are critical for hemostasis, and they contain biologically active constituents with the potential to modulate inflammatory responses. This study examined the effects of photochemical pathogen inactivation treatment (PCT) on the release of cytokines and/or chemokines from PLT components., Study Design and Methods: Double-dose apheresis PLT components were suspended in plasma-PLT additive solution mixtures and divided into paired therapeutic units. One unit served as an untreated control and the other unit was treated with PCT. PLT concentrations, pH, and levels of cytokines and/or chemokines (CD62p, platelet-derived growth factor-AB, interleukin [IL]-8, soluble CD40 ligand [sCD40L], IL-1beta, and tumor necrosis factor alpha) were measured during 7 days of storage in PLT component supernatants and PLT lysates., Results: PLT content, pH, and cytokine and/or chemokine content and release from PLT component prepared with PCT were not different (p > 0.05) from paired control components during storage. Levels of sCD40L, however, increased significantly during storage while decreasing in parallel within PLT lysates, although no differences were detected between paired PCT and control PLT component., Conclusion: PCT did not increase the release or secretion of PLT chemokines and/or cytokines over a 7-day period compared to conventional PLT component.

Background: A triple storage (TS) set allows for pathogen inactivation (PI) treatment of triple‐dose apheresis platelet products with amotosalen + UVA. We evaluated the quality and metabolic parameters of platelet concentrates (PCs) pathogen inactivated and stored for 7 days. Materials and methods: Twelve triple‐dose products collected with two different apheresis platforms were treated with amotosalen+UVA. Products were split into three single‐dose units. Testing was made pretreatment, after splitting, at days 5 and 7 of storage. Results: Single‐dose PI PCs had a mean platelet content of 2.89 ± 0.35 x 1011. From baseline to day 7, pH remained stable (7.1 ± 0.1 vs. 7.0 ± 0.1), pO2 increased (11.3 ± 2.4 vs. 18.3 ± 3.5 kPa) as did LDH (201 ± 119 vs. 324 ± 203 U/L) and lactate (3.6 ± 1.7 vs. 12.1 ± 1.5 mmol/L) (all p < 0.01); pCO2 decreased (4.1 ± 0.8 vs. 1.5 ± 0.7 mmHg; p < 0.01) and so did bicarbonate (6.6 ± 1.1 vs. 2.5 ± 1.4 mmol/L), glucose (5.6 ± 1.2 vs. 0.4 ± 0.4 mmol/L) and ATP (3.4 ± 0.9 vs. 2.5 ± 1.4 nmol/108 platelets) (all p < 0.05). Conclusion: Triple‐dose PCs processed with the TS sets fulfilled the quality requirements and displayed metabolic changes of expected extent during 7‐day storage. [ABSTRACT FROM AUTHOR]

Suite à l'adoption des concentrés de plaquettes (CP) atténués pour les agents pathogènes par le système INTERCEPT™ (CP-IA), l'AP–HM a évalué l'impact du changement sur les besoins en transfusion et sur la sécurité des patients en fonctionnement de routine. Comparer l'utilisation de CP et de concentré de globules rouges (CGR) et les événements indésirables receveurs (EIR) sur 2 populations de patients adultes et pédiatriques en hématologie-oncologie au cours de 2 périodes avant et après la mise en place des CP-IA. Les données démographiques, les transfusions de CP, les besoins concomitants en CGR et les EIR ont été analysés rétrospectivement sur 2 périodes : le premier trimestre 2017 (pré-IA) et le premier trimestre 2018 (post-IA). Les CP sont produits à partir de mélanges (MCP-IA) ou d'aphérèse (CPA-IA) avec une durée de conservation maximale de 5 jours. Voir Tableau 1 malgré l'analyse démographique semblable pour les populations de patients, la répartition des âges et des indications était différente (p = 0,007 et p < 0,001) analysée par transfusion. Les consommations de CP et de CGR n'ont pas augmenté et la durée du support plaquettaire a été réduite pour la période post-IA. Les EIR enregistrés ont été rares. La fréquence des décès était plus élevée dans la période pré-IA. Cette étude rétrospective sur 2 périodes pré et post-IA n'a pas montré d'augmentation de l'utilisation de CP ou de CGR (abaissée dans l'étude), ni d'augmentation des EIR. Les différences de répartition d'âge et des pathologies et la mortalité plus élevée dans le groupe pré-IA appellent à une étude sur une période de surveillance plus longue. [ABSTRACT FROM AUTHOR]
