Hypertension (HTN) is among the leading global preventable risk factors for cardiovascular disease and premature mortality. Early detection and effective management of HTN have demonstrated significant reductions in mortality morbidity rate, and health care costs. Furthermore, screening for HTN by nonphysician health care providers improves detection rates and medical management. As physical therapist practice advances to a more independent care model, physical therapists may serve as the first point of contact into the health care system, thereby necessitating a need for routine blood pressure (BP) monitoring. This is especially relevant in the outpatient physical therapist practice setting, where there is evidence for elevated BP measures among patients, yet omission of routine screening in this setting is well documented. Leading physical therapy professional organizations include statements in their guidelines that suggest that physical therapists have a duty to provide a standard of care that protects the safety and optimizes the overall health of patients under their care. Therefore, it is imperative not only that physical therapists include BP examination into routine practice protocols but that the knowledge and skills to accurately measure and interpret BP at rest and during exercise be integrated into the standard of care. Hypertension in patients on hemodialysis (HD) contributes significantly to their morbidity and mortality. This study examined whether a supportive nursing intervention incorporating monitoring, goal setting, and reinforcement can improve blood pressure (BP) control in a chronic HD population. A randomized controlled design was used, and 118 participants were recruited from six HD units in the Detroit metro area. The intervention consisted of 1) BP education sessions; 2) a 12-week intervention, including monitoring, goal setting, and reinforcement; and 3) a 30-day post-intervention follow-up period. Participants in the treatment were asked to monitor their BP, sodium, and fluid intake weekly for 12 weeks in weekly logs. BP, fluid, and sodium logs were reviewed weekly with the researcher to determine if goals were met or not met. Reinforcement was given for goals met and problem solving offered when goals were not met. The control group received standard care. Both systolic and diastolic BPs were significantly decreased in the treatment group. DESIGN: Population-based cohort study. SETTING: County of Vasterbotten, Sweden. PARTICIPANTS: Half of all subjects aged 85 and all of those aged 90 and 95 and older (N=348) in one urban and five rural municipalities in the north of Sweden. MEASUREMENTS: Among others, supine blood pressure, Mini-Mental State Examination, Barthel Index of activities of daily living, Mini Nutritional Assessment, and body mass index. Information on diagnoses, medications, and 4-year mortality was collected. Associations between blood pressure and mortality were investigated using Cox regression analyses, controlling for a number of diagnoses and health factors. RESULTS: Baseline systolic blood pressure (SBP), diastolic blood pressure, and pulse pressure were all inversely associated with mortality within 4 years according to univariate analysis. SBP was the strongest predictor. In Cox regression analyses, low SBP ([less than or equal to]120 mmHg) correlated with greater 4-year all-cause mortality alone and when controlling for health status. This connection persisted after exclusion of deaths within the first year. There was a tendency toward a U-shaped mortality curve for the adjusted model, with SBP of 164.2 mmHg (95% confidence interval=154.1-183.8 mmHg) being associated with the lowest mortality. CONCLUSION: Lower SBP seems to be associated with greater mortality in people aged 85 and older, irrespective of health status. There are indications of a U-shaped correlation between SBP and mortality, and the optimal SBP for this age group could be above 140 mmHg. Background: The relative importance of blood pressure components (systolic blood pressure, diastolic blood pressure, and pulse pressure) on cardiovascular risk is currently being debated. Many studies, however, are limited by inadequate statistical methods to separate these effects. Objective: To evaluate the joint effect of blood pressure components on all-cause and cardiovascular mortality by using non-parametric and change point models. Design: Prospective cohort study. Setting: 15-year mortality follow-up of participants in the Second National Health and Nutrition Examination Survey. Participants: 7830 white and African-American men and women 30 to 74 years of age, apparently free of cardiovascular disease at baseline. Measurements: Baseline blood pressure, corrected for measurement error. Results: Of the 1588 patients who died, 582 died of cardiovascular disease. Systolic blood pressure was linearly related to all-cause and cardiovascular mortality in younger and elderly participants. The association of diastolic blood pressure with all-cause and cardiovascular mortality was hockey stick-shaped (flat then increasing) in younger participants and J-shaped in elderly participants. Increased pulse pressure was associated with increased risk, decreased risk, or no change in risk depending on age and systolic and diastolic blood pressure. Conclusions: On the basis of these and previous data, the evidence for a monotonic association of systolic blood pressure with all-cause and cardiovascular mortality is compelling, but a J-shaped association for diastolic blood pressure may develop at older age. The complexity of the association of pulse pressure with mortality discourages its use for prognostic or therapeutic decisions.
