Abstract

Background Kidney failure with replacement therapy and hemodialysis are associated with a decrease in quality of life (QOL). Self-reported QOL symptoms are not always prioritized by the medical team, potentially leading to conflicting priorities with patients. Electronic patient-reported outcome measures (ePROMs) allow physicians to better identify these symptoms. The objective was to describe the prevalence of symptoms self-reported by hemodialysis (HD) patients. Methods A multicenter cross-sectional study was conducted in three HD centers. Patients were included if they were 18 years old or over treated with HD for at least 3 months in a center. Data were collected by the patient via a self-administered ePROMs questionnaire. Data included patient characteristics, post-dialysis fatigue and intensity, recovery time after a session, perceived stress, impaired sleep the day before the dialysis session, current state of health and the change from the past year. A multivariate analysis was conducted to identify relations between symptoms. Results In total, we included 173 patients with a mean age of 66.2 years, a mean ± SD hemodialysis duration of 48.9 ± 58.02 months. The prevalence of fatigue was 72%. 66% had a high level of stress (level B or C). Recovery time was more than 6 h after a HD session for 25% of patients and 78% declared they had a better or unchanged health status than the previous year. Sleep disturbance was associated with cardiovascular comorbidities (OR 5.08 [95% CI, 1.56 to 16.59], p = 0.007). Conclusions Fatigue and stress were the main symptoms reported by HD patients. The patient’s care teams should better consider these symptoms.

Abstract

Background and Aims The health crisis linked to the COVID-19 epidemic has required lockdown measures in France and changes in practices in dialysis centers. The objective was to assess the depressive and anxiety symptoms during lockdown in hemodialysis patients and their caregivers, to assess their coping strategies during this period and to assess the symptoms of depression, anxiety and post traumatic stress beyond confinement. Method We sent, during lockdown period, between April and May 2020, self-questionnaires to voluntary subjects (patients and caregivers), treated by hemodialysis or who worked in hemodialysis in one of the 14 participating centers in France. We analyzed their perception of dialysis sessions (beneficial or worrying), their stress level (VAS rated from 0 to 10), their anxiety and depressive symptoms (HADS). Factors associated with stress, anxiety and depression were analyzed with multiple logistic regression models. We will look for associations between coping strategies, participant characteristics and symptoms of stress, anxiety and depression using chi-square tests. A second questionnaire was sent out in October to collect symptoms of depression, anxiety and post-traumatic stress beyond confinement. Symptoms will be described and factors associated with stress, anxiety and depression will be analyzed with multiple logistic regression models. Results 669 patients and 325 caregivers agreed to participate. 70% of participants found it beneficial to come to dialysis during confinement. The proportions of subjects with a stress level ≥ 6 linked to the epidemic, confinement, fear of contracting COVID-19 and fear of infecting a loved one were respectively 23.9%, 26.2%, 33.4% and 42%. 39.2% presented with certain (13.7%) or doubtful (19.2%) anxious symptoms. 21.2% presented a certain (7.9%) or doubtful (13.3%) depressive symptomatology. Age, gender, history of psychological disorders and perception of dialysis sessions were associated with levels of stress, anxiety and depression. 685 subjects participated in the second part of the study (68.9% of the participants of the first part). Analyzes of this data are in progress. Conclusion During the lockdown period, in France, the majority of hemodialysis patients and caregivers found it beneficial to come to dialysis. One in 3 subjects had anxiety symptoms and one in 5 subjects had depressive symptoms. It will be interesting to investigate if there was an association between the coping strategies implemented by the participants and their level of stress, anxiety and depression during confinement and to analyze the evolution of the anxiety-depressive symptoms over time.

Abstract

The aim of this study was to estimate the incidence of COVID-19 disease in the French national population of dialysis patients, their course of illness and to identify the risk factors associated with mortality. Our study included all patients on dialysis recorded in the French REIN Registry in April 2020. Clinical characteristics at last follow-up and the evolution of COVID-19 illness severity over time were recorded for diagnosed cases (either suspicious clinical symptoms, characteristic signs on the chest scan or a positive reverse transcription polymerase chain reaction) for SARS-CoV-2. A total of 1,621 infected patients were reported on the REIN registry from March 16th, 2020 to May 4th, 2020. Of these, 344 died. The prevalence of COVID-19 patients varied from less than 1% to 10% between regions. The probability of being a case was higher in males, patients with diabetes, those in need of assistance for transfer or treated at a self-care unit. Dialysis at home was associated with a lower probability of being infected as was being a smoker, a former smoker, having an active malignancy, or peripheral vascular disease. Mortality in diagnosed cases (21%) was associated with the same causes as in the general population. Higher age, hypoalbuminemia and the presence of an ischemic heart disease were statistically independently associated with a higher risk of death. Being treated at a selfcare unit was associated with a lower risk. Thus, our study showed a relatively low frequency of COVID-19 among dialysis patients contrary to what might have been assumed., Graphical abstract

Abstract

ObjectivesCardiorenal syndrome (CRS) is the combination of acute heart failure syndrome (AHF) and renal dysfunction (creatinine clearance (CrCl) ≤60 mL/min). Real-life data were used to compare the management and outcome of AHF with and without renal dysfunction.DesignProspective, multicentre.SettingTwenty-six academic, community and regional hospitals in France.Participants507 patients with AHF were assessed in two groups according to renal function: group 1 (patients with CRS (CrCl ≤60 mL/min): n=335) and group 2 (patients with AHF with normal renal function (CrCl >60 mL/min): n=172).ResultsDifferences were observed (group 1 vs group 2) at admission for the incidence of chronic heart failure (56.42% vs 47.67%), use of furosemide (60.9% vs 52.91%), insulin (15.52% vs 9.3%) and amiodarone (14.33% vs 4.65%); additionally, more patients in group 1 carried a defibrillator (4.78% vs 0%), had ≥2 hospitalisations in the last year (15.52% vs 5.81%) and were under the care of a cardiologist (72.24% vs 61.63%). Clinical signs were broadly similar in each group. Brain-type natriuretic peptide (BNP) and BNP prohormone were higher in group 1 than group 2 (1157.5 vs 534 ng/L and 5120 vs 2513 ng/mL), and more patients in group 1 were positive for troponin (58.2% vs 44.19%), had cardiomegaly (51.04% vs 37.21%) and interstitial opacities (60.3% vs 47.67%). The only difference in emergency treatment was the use of nitrates, (higher in group 1 (21.9% vs 12.21%)). In-hospital mortality and the percentage of patients still hospitalised after 30 days were similar between groups, but the median stay was longer in group 1 (8 days vs 6 days).ConclusionsRenal impairment in AHF should not limit the use of loop diuretics and/or vasodilators, but early assessment of pulmonary congestion and close monitoring of the efficacy of conventional therapies is encouraged to allow rapid and appropriate implementation of alternative therapies if necessary.

Abstract

Background Tunnelled dialysis catheter (TC) infections are a major health complication and are associated with increased antibiotic consumption, hospital stays, health costs and mortality. Experimental data provide evidence that Ethenox, a mixture of enoxaparine 1000 U/mL in 40% v/v ethanol, could be a promising lock solution. The aim of the study is to compare an interdialytic lock solution of Ethenox with reference lock solutions, unfractionated heparin (UFH) or citrate 4% for the prevention of TCI in hemodialysis patients. Method This study will monitor a multicentre, prospective, single blind, randomized, controlled, parallel group trial. The main inclusion criteria are patients > 18 years old with end-stage renal disease, treated with chronic hemodialysis/hemodiafiltration three times a week, with incident or prevalent non-impregnated internal jugular TCs inserted for at least 2 weeks and able to give informed consent. Exclusion criteria are TCI in the previous 4 weeks and anti-infective treatment for TCI in the previous 2 weeks. Patients will be randomized to receive either study treatment Ethenox in the intervention group or reference solutions in the control group, unfractionated heparin (UFH) or citrate 4% w/v according to usual practice. The primary outcome measure will be time to first TCIs assessed by an endpoint adjudication committee blinded to the study arm according to predefined criteria. Patients will receive the study treatment for up to 12 months. Intention-to-treat analysis of the primary endpoint will be performed with a marginal Cox proportional hazard model. Prospective power calculations indicate that the study will have 90% statistical power to detect a clinical significant two-fold increase in median infection-free survival if 200 patients are recruited into each arm over a period of 24 months. Discussion Firm evidence of the efficacy of the Ethenox lock in preventing TCI could be of major clinical benefit for patients. The results of this study will allow the development of new guidelines based on a high level of evidence. Abstract

Large cohort studies suggest that high convective volumes associated with online hemodiafiltration may reduce the risk of mortality/morbidity compared to optimal high-flux hemodialysis. By contrast, intradialytic tolerance is not well studied. The aim of the FRENCHIE (French Convective versus Hemodialysis in Elderly) study was to compare high-flux hemodialysis and online hemodiafiltration in terms of intradialytic tolerance. In this prospective, open-label randomized controlled trial, 381 elderly chronic hemodialysis patients (over age 65) were randomly assigned in a one-to-one ratio to either high-flux hemodialysis or online hemodiafiltration. The primary outcome was intradialytic tolerance (day 30-day 120). Secondary outcomes included health-related quality of life, cardiovascular risk biomarkers, morbidity, and mortality. During the observational period for intradialytic tolerance, 85% and 84% of patients in high-flux hemodialysis and online hemodiafiltration arms, respectively, experienced at least one adverse event without significant difference between groups. As exploratory analysis, intradialytic tolerance was also studied, considering the sessions as a statistical unit according to treatment actually received. Over a total of 11,981 sessions, 2,935 were complicated by the occurrence of at least one adverse event, with a significantly lower occurrence in online hemodiafiltration with fewer episodes of intradialytic symptomatic hypotension and muscle cramps. By contrast, health-related quality of life, morbidity, and mortality were not different in both groups. An improvement in the control of metabolic bone disease biomarkers and β2-microglobulin level without change in serum albumin concentration was observed with online hemodiafiltration. Thus, overall outcomes favor online hemodiafiltration over high-flux hemodialysis in the elderly.

Abstract

Correction of anemia in type 2 diabetes (T2DM) patients with chronic kidney disease stages 3-4 may slow the decline of kidney function but may increase cardiovascular risk through higher hematocrit. The NEPHRODIAB2 study was designed to assess efficacy and safety of complete hemoglobin (Hb) normalization in these patients.We randomly assigned 89 T2DM patients with an estimated glomerular filtration rate (eGFR; abbreviated 175 Modification of Diet in Renal Disease formula) of 25 to 60 ml/min per 1.73 m(2) and moderate anemia (Hb, 100-129 g/l) to a target Hb value in subnormal range (110-129g/l, group 1, n=43) or normal range (130-149 g/l, group 2, n=46). The primary end point was eGFR decline after 2 years of follow-up. Secondary end points included iron and erythropoietin dosage, quality of life (Medical Outcomes Study 36-item Short-Form Health Survey scores) and adverse events.Six months after randomization, the mean Hb levels were120 g/l in group 1 and130 g/l in group 2 (P.05 at 6, 12, 18 and 24 months). Blood pressure, 24-h proteinuria and HbA1c did not differ during follow-up (P.05). Two-year declines in eGFR were -8.7±12.2 in group 1 and -5.1±7.8 ml/min per 1.73 m(2) in group 2 (P=.29). Mean weekly use of erythropoietin was 7.8±11.6 μg in group 1 and 30.1±33.6 μg in group 2 (P.0001). There was no significant difference regarding Medical Outcomes Study 36-item Short-Form Health Survey score change or adverse event occurrence.In this trial, normalization of Hb level in T2DM patients with chronic kidney disease was safe but did not significantly slow renal function decline and increased treatment cost due to erythropoietin use.

Abstract

Background and objective Catheter-related bacteraemia (CRB) is associated with an increased risk of morbidity and mortality in haemodialysis patients and accounts for 50 to 70% of catheter removals. Antibiotic lock therapy can prevent CRB in these patients but may have side-effects such as loss of hearing or acquisition of antibiotic resistance. The aim of this study was to evaluate CRB incidence over three consecutive periods using different antibiotic locks. Programme Setting and participants All adults treated by haemodialysis through a tunnelled catheter (March 1, 1999–December 31, 2008) (102 patients, 144 tunnelled catheters, 31 536 catheter-days). Quality improvement plan Universal hygiene rules, setting up of a follow-up registry, antimicrobial locks: heparin plus gentamicin (Period 1, March 1999—June 2000); heparin alone (Period 2, July 2000—December 2004); citrate (Period 3, January 2005—December 2008). Measures Repeated observations of CRB, catheter colonisation and orifice infection, analysis using simple descriptive statistics, χ 2 tests. Results Period 1: CRB incidence was 1.96 per 1000 catheters-days in 1999 and 0.29 in 2000 (p Period 2: CRB incidence increased from 0.39 per 1000 catheter-days in 2001 to 2.03 in 2002. In 2003, an audit of hygiene practices revealed non compliance with universal hygiene guidelines by the youngest nurses. After intensive nurse training, CRB incidence decreased to 0.76 per 1000 catheter-days in 2003 and to 0.63 in 2004. Period 3: CRB incidence decreased to 0.28, 0.37, 0.63, and 0 per 1000 catheter-days in 2005, 2006, 2007 and 2008, respectively. No adverse events and no catheter thrombosis were observed. Discussion and conclusions A gentamicin lock was associated with a highly significant reduction in CRB incidence but also with a highly significant emergence of bacterial resistance. Heparin alone was associated with a decrease in resistance but a higher CRB incidence despite reinforced hygiene rules. In units such as dialysis centres, gestures are repetitive and drifts in practices are quick to occur. Regular assessment is therefore essential. Since January 2003, a hygiene nurse carries out 6-monthly audits of our quality procedures in clinical and hygiene practice. The follow-up registry we have set up enables periodical re-evaluation of our procedures and the taking of corrective actions. Contexte et objectif L9infection reste la complication la plus frequente des catheters (KT) d9hemodialyse. Elle represente 50 a 70% des motifs d9ablations des KT. Le risque relatif de bacteriemie est de 7,64 (fistule, RR =1). En 1999 nous avons constate un taux eleve d9infection secondaire aux KT. Programme Un audit initial a ete realise pendant 6 mois (de mars 1999 a septembre 1999) qui a confirme un taux hautement eleve (L9incidence de bacteriemie est de 1.01 episodes pour 1000 jours/catheters pour les catheters tunnelises et 5.91 episodes pour 1000 jours/catheters pour les catheters non tunnelises). Vu les resultats de l9audit toute l9equipe medicale et paramedicale est mobilisee. Une nouvelle organisation de travail et des procedures decrivant la methode de branchement et debranchement des catheters sont redigees et validees par le CLIN. Nous avons instaure depuis 1999 une surveillance continue des infections des KT de dialyse, ce qui a permis d’evaluer nos pratiques professionnelles durant 3 periodes. Un audit est realise a chaque augmentation des taux d9infections KT. Resultats er periode (fev. 1999- juin 2000) tous les KT sont fermes par un verrou mixte (gentamicine+heparine). Des l9annee 2000 on constate une dimunition significative des bacteriemies (1,96 pour 1000 jours catheters en 1999 et 0,29 en 2000). Devant l’emergence de staphylocoque epidermidis multi resistant, nous avons arrete les verrous gentamicine en juin 2000. Il a fallut 18 mois avant de retrouver une ecologie bacterienne de 1999. eme periode (juillet 2002- dec 2004) tous les KT sont fermes par un verrou heparine seul. Nous avons constate une augmentation des infections des KT de 0.26 pour 1000 jours catheters en 2000, 0.39 en 2001 et 2.03 en 2002. Un audit realise par le CLIN montre une derive des pratiques chez les jeunes IDE. Une formation intensive des IDE pour le respect des protocoles de branchement debranchement est mise en place. L9incidence des infections des KT est a 0.76 en 2003 et 0.63 en 2004. eme periode (janv. 2005- dec. 2008) tous les KT sont fermes par un verrou citrate (46.7%). L9incidence des infections des KT est a 0.28 en 2005, 0.30 en 2006, 0,63 en 2007 et 0 en 2008. Conclusion Ce travail nous a permis de diminuer le risque d9infection secondaire aux KT. L9utilisation des verrous avec de la gentamicine a ete efficace au depend de l9augmentation de staphyloccoques multiresistants. Nous avons alerte la communaute nephrologique des 2004. Dans les services comme les centres d9hemodialyse les gestes sont repetetifs, les derives peuvent rapidement s9installer. Une evaluation reguliere nous parait indispensable. Depuis 2003 tous les 6 mois nous pratiquons un audit par l9infirmiere du CLIN avec un compte rendu et intervention.
