BACKGROUND: Immediate initiation of antiretroviral therapy (ART) regardless of CD4 cell count reduces risk for AIDS and non-AIDS-related events in asymptomatic, HIV-positive persons and is the standard of care. However, most HIV-positive persons initiate ART when their CD4 count decreases below 500 × 10 9 cells/L. Consequences of delayed ART on risk for non-AIDS-defining and AIDS-defining cancer, one of the most common reasons for death in HIV, are unclear. OBJECTIVE: To estimate the long-term risk difference for cancer with the immediate ART strategy.DESIGN: Multinational prospective cohort study.SETTING: The D:A:D (Data collection on Adverse events of anti-HIV Drugs) study, which included HIV-positive persons from Europe, Australia, and the United States.PARTICIPANTS: 8318 HIV-positive persons with at least 1 measurement each of CD4 cell count and viral load while ART-naive (study period, 2006 to 2016).MEASUREMENTS: The parametric g-formula was used, with adjustment for baseline and time-dependent confounders (CD4 cell count and viral load), to assess the 10-year risk for non-AIDS-defining and AIDS-defining cancer of immediate versus deferred (at CD4 counts RESULTS: During 64 021 person-years of follow-up, 231 cases of non-AIDS-defining cancer and 272 of AIDS-defining cancer occurred among HIV-positive persons with a median age of 36 years (interquartile range, 29 to 43 years). With immediate ART, the 10-year risk for non-AIDS-defining cancer was 2.97% (95% CI, 2.37% to 3.50%) and that for AIDS-defining cancer was 2.50% (CI, 2.37% to 3.38%). Compared with immediate ART initiation, the 10-year absolute risk differences when deferring ART to CD4 counts less than 500 × 10 9 cells/L and less than 350 × 10 9 cells/L were 0.12 percentage point (CI, -0.01 to 0.26 percentage point) and 0.29 percentage point (CI, -0.03 to 0.73 percentage point), respectively, for non-AIDS-defining cancer and 0.32 percentage point (CI, 0.21 to 0.44 percentage point) and 1.00 percentage point (CI, 0.67 to 1.44 percentage points), respectively, for AIDS-defining cancer. LIMITATION: Potential residual confounding due to observational study design.CONCLUSION: In this young cohort, effects of immediate ART on 10-year risk for cancer were small, and further supportive data are needed for non-AIDS-defining cancer.PRIMARY FUNDING SOURCE: Highly Active Antiretroviral Therapy Oversight Committee.

Growth in the livestock sector is associated with heightened risk for epidemic diseases. The increasing spillover of new diseases from wildlife is being driven by wide-scale anthropogenic changes allowing for more frequent and closer wildlife-human and wildlife-livestock contacts. An increasing number of epidemics in livestock are associated with rapid transition of livestock systems from extensive to intensive, and local to global movement of livestock and their products through value chain networks with weak biosecurity. Major livestock epidemics in the past two decades have had substantial economic impacts, and the COVID-19 pandemic highlights the devastating socio-economic consequences that spillovers can have when not identified and controlled early in the process of emergence. This highlights the importance of Veterinary Services to integrated, whole-of-society efforts to control infectious diseases in animals. Emphasis within Veterinary Services must be placed on prevention and preparedness. The authors suggest four areas for continued improvement in Veterinary Services to meet this challenge. These are a) continued development of staff capacity for risk assessment and value chain analysis, together with improved policies and communication, b) appropriate adaptation of approaches to prevention and control in resource-poor settings, c) improved multi-sectoral and transboundary cooperation, which enables the sharing of resources and expertise, and d) systematic approaches that enable Veterinary Services to influence decisionmaking for trade, markets, business, public health, and livelihood development at the national and regional levels.

The study was undertaken to explore the nutritive potential of cassava leaves and Azolla as supplementary source of crude protein to cattle fed on either mixed range grass hay or Bracharia Mulato II. Two experiments were carried out, the first was the determination of the biomass yield of Azolla, its relative growth rate and doubling time while the second was a feeding trial to determine the performance of small East African Shorthorned Zebu heifers of similar age and weight. The heifers (four each) were randomly assigned to six treatments in the feeding trial that ran for 16 weeks. The doubling time for Azolla was 5 days while the relative growth rate was 0.15 g/g/day. The chemical analysis indicated that Azolla is rich in crude protein (18.8%) and minerals (3.29% calcium, 2.66% potassium and 0.15% phosphorus and other minerals in trace levels). All the supplemented groups showed more weekly weight gains ranging from 0.75 to 1.11 kg per day more than the control. Overall, the treatment with Azolla or cassava showed slightly higher but insignificant increases in weight gains. Azolla and cassava leaves can therefore be considered as potential feed for livestock. Key words: Arid and range lands, cattle, doubling time, relative growth rate.

On the last week of May of 2018, a community-based syndromic surveillance system detected mass abortions and deaths of young livestock in northeastern Kenya. Two weeks later, Rift Valley fever (RVF) was confirmed in humans presenting with febrile illness and hemorrhagic syndrome in the same region. A joint animal and human response team carried out an investigation to characterize the outbreak and identify drivers of disease transmission. Here, we describe the outbreak investigation and findings. A total of 106 human cases were identified in the months of May and June 2018: 92% (98) and 8% (8) of these cases occurring in the northern and western regions of Kenya, respectively. Seventy-six (72%) were probable cases, and 30 (28%) were laboratory confirmed by ELISA and/or PCR. Among the confirmed cases, the median age was 27.5 years (interquartile range = 20), and 60% (18) were males. Overall, the case fatality rate was 7% (n = 8). The majority of the confirmed cases, 19 (63%), reported contact with livestock during slaughter and consumption of meat from sick animals. All confirmed cases had fever, 40% (12) presented with hemorrhagic syndrome, and 23% (7) presented with jaundice. Forty-three livestock herds with at least one suspect and/or confirmed animal case were identified. Death of young animals was reported in 93% (40) and abortions in 84% (36) of livestock herds. The outbreak is indicative of the emergence potential of RVF in traditionally high- and low-risk areas and the risk posed by zoonosis to livestock keepers.

Riparian forests ecosystems play significant role in biodiversity conservation and provision of ecosystem goods and services which support local livelihoods. However, riparian ecosystems are threatened by degradation attributed to anthropogenic activities. Understanding the interaction between anthropogenic activities and socio-economic factors, and their associated impact on riparian degradation is essential for designing appropriate management strategies for these ecosystems. This study assessed the socio-economic factors that drive degradation and their implication on conservation of River Lumi riparian ecosystem. Semi-structured questionnaires, Focus Group Discussion (FGDs) and key informant interviews were used to collect the data. The findings indicate that crop farming and livestock production were the main sources of livelihood practiced by 84% and 70% of the households respectively. Consequently, agricultural intensification resulting from increased demand for agricultural commodities (33%), over-grazing coupled with influx of livestock from private ranches and neighbouring pastoralist Maasai community (20%), and deforestation fuelled by population increase (17%) were the main drivers of degradation in River Lumi riparian ecosystem. The findings indicate that about 91% of the adjacent communities are dependent on River Lumi for their sustenance and their livelihoods will be affected by continued degradation of the riparian ecosystem; thus there is need for development of appropriate management strategies including sustainable livelihood systems to conserve River Lumi riparian ecosystem.

Background We implemented a longitudinal study to determine the incidence of Brucella infection in cattle, camels, sheep and goats that were being raised in a pastoral area in Isiolo County, Kenya. An initial cross-sectional survey was implemented to identify unexposed animals for follow up; that survey used 141 camels, 216 cattle, 208 sheep and 161 goats. Sera from these animals were screened for Brucella spp. using the Rose Bengal Plate test (RBPT), a modified RBPT, and an indirect multispecies Enzyme Linked Immunosorbent Assay (iELISA). Results of RBPT and iELISA were interpreted in parallel to determine seroprevalence. A total of 30 camels, 31 cattle, 22 sheep and 32 goats that were seronegative by all the above tests were recruited in a subsequent longitudinal study for follow up. These animals were followed for 12 months and tested for anti-Brucella antibodies using iELISA. Seroconversion among these animals was defined by a positive iELISA test following a negative iELISA result in the previous sampling period. All seropositive samples were further tested using real-time PCR-based assays to identify Brucella species. These analyses targeted the alkB and BMEI1162 genes for B. abortus, and B. melitensis, respectively. Data from the longitudinal study were analysed using Cox proportional hazards model that accounted for within-herds clustering of Brucella infections. Results The overall incidence rate of Brucella infection was 0.024 (95% confidence interval [CI]: 0.014–0.037) cases per animal-months at risk. Brucella infection incidence in camels, cattle, goats and sheep were 0.053 (0.022–0.104), 0.028 (0.010–0.061), 0.013 (0.003–0.036) and 0.006 (0.0002–0.034) cases per animal-months at risk, respectively. The incidence rate of Brucella infection among females and males were 0.020 (0.009–0.036) and 0.016 (0.004–0.091), respectively. Real-time PCR analyses showed that B. abortus was more prevalent than B. melitensis in the area. Results of multivariable Cox regression analysis identified species (camels and cattle) as an important predictor of Brucella spp. exposure in animals. Conclusions This study estimated an overall brucellosis incidence of 0.024 cases per animal-months at risk with camels and cattle having higher incidence than sheep and goats. These results will inform surveillance studies in the area.

In the light of the urgency raised by the COVID-19 pandemic, global investment in wildlife virology is likely to increase, and new surveillance programmes will identify hundreds of novel viruses that might someday pose a threat to humans. To support the extensive task of laboratory characterization, scientists may increasingly rely on data-driven rubrics or machine learning models that learn from known zoonoses to identify which animal pathogens could someday pose a threat to global health. We synthesize the findings of an interdisciplinary workshop on zoonotic risk technologies to answer the following questions. What are the prerequisites, in terms of open data, equity and interdisciplinary collaboration, to the development and application of those tools? What effect could the technology have on global health? Who would control that technology, who would have access to it and who would benefit from it? Would it improve pandemic prevention? Triple-Negative Breast Cancer (TNBC) is the most aggressive breast cancer subtype, characterized by limited treatment options and higher relapse rates than hormone-receptor-positive breast cancers. Chemotherapy remains the mainstay treatment for TNBC, and platinum salts have been explored as a therapeutic alternative in neo-adjuvant and metastatic settings. However, primary and acquired resistance to chemotherapy in general and platinum-based regimens specifically strongly hampers TNBC management. In this study, we used carboplatin-resistant in vivo patient-derived xenograft and isogenic TNBC cell-line models and detected enhanced Wnt/β-catenin activity correlating with an induced expression of stem cell markers in both resistant models. In accordance, the activation of canonical Wnt signaling in parental TNBC cell lines increases stem cell markers' expression, formation of tumorspheres and promotes carboplatin resistance. Finally, we prove that Wnt signaling inhibition resensitizes resistant models to carboplatin both in vitro and in vivo, suggesting the synergistic use of Wnt inhibitors and carboplatin as a therapeutic option in TNBC. A cross-sectional study was conducted to determine the seroprevalence of Brucella spp. and Leptospira spp. and risk factors of exposure in cattle in three zones with varying land use types and wildlife–livestock interactions. Five villages were selected purposively; two in areas with intensive livestock–wildlife interactions (zone 1), another two in areas with moderate livestock–wildlife interactions (zone 2) and one in areas where wildlife–livestock interactions are rarer (zone 3). Sera samples were collected from 1170 cattle belonging to 390 herds in all the zones and tested for antibodies against Brucella abortus and Leptospira interrogans serovar hardjo using ELISA kits. Data on putative risk factors for seropositivity of these pathogens in cattle were collected using a questionnaire. The overall apparent animal-level seroprevalence of brucellosis and leptospirosis was, respectively, 36.9% (95% CI 34.1–39.8) and 23.5% (95% CI 21.1–26.0). Brucella spp. seroprevalence was higher in zone 1 than in zones 2 and 3 (χ2 = 25.1, df = 2, P Leptospira spp. seroprevalence than zone 3 (χ2 = 7.0, df = 2, P = 0.029). Results of multivariable analyses identified animal sex (female) and zones (high interface area) as significant predictors (P Brucella spp. For Leptospira spp., important predictors of animal-level seropositivity were animal sex (female), zones (moderate interface area) and herds utilizing a communal grazing reserve. The seroprevalences of Brucella spp. and Leptospira spp. in cattle were higher in areas with moderate to high wildlife–livestock interactions than those with rare interactions.

Livestock-wildlife interactions promote the transmission of a wide range of infectious diseases that constraint livestock production. We used a participatory appraisal approach to find out and rank infectious diseases of concern to pastoralists in a zone of intense wildlife-livestock interaction and another zone with limited interactions. Four villages were selected purposively in areas with intensive cattle-wildlife interactions (zone 1), and another two in areas with low to moderate cattle-wildlife interactions (zone 2). Data were collected in focus group discussions (FGDs) using participatory epidemiological methods (PE); each group had 8–13 participants. Results of impact matrix scoring from all sites indicated that malignant catarrhal fever (MCF), anthrax, foot and mouth disease (FMD), contagious bovine pleuropneumonia (CBPP), east coast fever (ECF) and African animal trypanosomiasis (ATT), in decreasing order, had the highest impact on livestock production. A Kruskal–Wallis test revealed a significant difference in FMD annual prevalence between cattle age groups (p 4 years (median score of 32.5, range, 10–50). FMD had the highest impact on milk production, but based on veterinary costs (treatment costs), it was ranked second to CBPP. The study provides information on disease priorities that occur in the target zones in Mara ecosystem and which the local pastoralists must consider when accessing key ecosystem services such as water and pasture. Brucellosis, caused by several species of the genus Brucella, is a zoonotic disease that affects humans and animal species worldwide. Information on the Brucella species circulating in different hosts in Kenya is largely unknown, thus limiting the adoption of targeted control strategies. This study was conducted in multi-host livestock populations in Kenya to detect the circulating Brucella species and assess evidence of host–pathogen associations. Serum samples were collected from 228 cattle, 162 goats, 158 sheep, 49 camels, and 257 humans from Narok and Marsabit counties in Kenya. Information on age, location and history of abortion or retained placenta were obtained for sampled livestock. Data on age, gender and location of residence were also collected for human participants. All samples were tested using genus level real-time PCR assays with primers specific for IS711 and bcsp31 targets for the detection of Brucella. All genus positive samples (positive for both targets) were further tested with a speciation assay for AlkB and BMEI1162 targets, specific for B. abortus and B. melitensis, respectively. Samples with adequate quantities aggregating to 577 were also tested with the Rose Bengal Test (RBT). A total of 199 (33.3%) livestock and 99 (38.5%) human samples tested positive for genus Brucella. Animal Brucella PCR positive status was positively predicted by RBT positive results (OR = 8.3, 95% CI 4.0–17.1). Humans aged 21–40 years had higher odds (OR = 2.8, 95% CI 1.2–6.6) of being Brucella PCR positive compared to the other age categories. The data on detection of different Brucella species indicates that B. abortus was detected more often in cattle (OR = 2.3, 95% CI 1.1–4.6) and camels (OR = 2.9, 95% CI 1.3–6.3), while B. melitensis was detected more in sheep (OR = 3.6, 95% CI 2.0–6.7) and goats (OR = 1.7, 95% CI 1.0–3.1). Both B. abortus and B. melitensis DNA were detected in humans and in multiple livestock host species, suggesting cross-transmission of these species among the different hosts. The detection of these two zoonotic Brucella species in humans further underpins the importance of One Health prevention strategies that target multiple host species, especially in the multi-host livestock populations.

Background Anthrax is an important zoonotic disease in Kenya associated with high animal and public health burden and widespread socio-economic impacts. The disease occurs in sporadic outbreaks that involve livestock, wildlife, and humans, but knowledge on factors that affect the geographic distribution of these outbreaks is limited, challenging public health intervention planning. Methods Anthrax surveillance data reported in southern Kenya from 2011 to 2017 were modeled using a boosted regression trees (BRT) framework. An ensemble of 100 BRT experiments was developed using a variable set of 18 environmental covariates and 69 unique anthrax locations. Model performance was evaluated using AUC (area under the curve) ROC (receiver operating characteristics) curves. Results Cattle density, rainfall of wettest month, soil clay content, soil pH, soil organic carbon, length of longest dry season, vegetation index, temperature seasonality, in order, were identified as key variables for predicting environmental suitability for anthrax in the region. BRTs performed well with a mean AUC of 0.8. Areas highly suitable for anthrax were predicted predominantly in the southwestern region around the shared Kenya-Tanzania border and a belt through the regions and highlands in central Kenya. These suitable regions extend westwards to cover large areas in western highlands and the western regions around Lake Victoria and bordering Uganda. The entire eastern and lower-eastern regions towards the coastal region were predicted to have lower suitability for anthrax. Conclusion These modeling efforts identified areas of anthrax suitability across southern Kenya, including high and medium agricultural potential regions and wildlife parks, important for tourism and foreign exchange. These predictions are useful for policy makers in designing targeted surveillance and/or control interventions in Kenya. To identify factors that influence the spatial distribution of the disease in Kenya, we analyzed surveillance on available anthrax outbreaks recorded in the southern half of the country. Areas predicted to be highly suitable for the disease were predominantly in the southwestern region around the shared Kenya-Tanzania border running as a belt through central regions and central highlands of Kenya. These suitability regions extend westwards to cover large areas in western highlands and the western regions around Lake Victoria and bordering Uganda. The entire eastern and lower-eastern regions towards the coastal region were predicted to have lower suitability for anthrax. Cattle density, rainfall of wettest month, soil clay content, soil pH, soil organic carbon, length of longest dry season, vegetation index and temperature seasonality were key variables predicting the distribution of anthrax in the region. The study generated a suitability map depicting geographical areas that can be targeted for risk-based surveillance and or control measures for the disease.

An aerosolizing reducing technique for the extraction of impacted mandibular third molars with the use of a bi-bevel chisel and mallet will be presented. Every oral and maxillofacial surgeon (OMS) should have this technique in their armamentarium, especially with the current global pandemic with the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), the cause of COVID-19. In this era, we must all consider ways to reduce the number of aerosols produced during the extraction of third molars. Prioritizing hand instrumentation with the use of chisels and mallets will reduce aerosolization as compared with the more traditional technique of using a surgical handpiece.

Brucella is a highly pathogenic bacteria endemic in Kenya, and in spite of its severity in humans, the highly inadequate Febrile Antigen Brucella Agglutination Test (FBAT) remains a primary tool for its diagnosis. Blood samples were collected from febrile patients in Kinna health center and screened by the local routine. Milk samples were purchased from local milk hawkers and analyzed for Brucella antibodies using the milk ring test (MRT). The MRT-positive milk was traced to farms, and lactating cattle were sampled for milk and blood. Milk was MRT-tested and the serum was analyzed using the Rose Bengal test (RBT) and iELISA. Available patient and farm samples were stored on FTA cards for qPCR analyses. Despite a limited sample size, our study, in line with previous reports, shows a low diagnostic sensitivity (67%) and specificity (40%) of FBAT when compared to qPCR. As many as 48% of the raw bulk cattle milk samples were MRT-positive for Brucella antibodies and 60% of cattle on three visited farms were IS711 qPCR-positive. This case-based One Health investigation confirms the suspected Brucella presence, suggesting a targeted vaccination at high-prevalence farms, urgent interventions on milk safety, and a re-evaluation of the diagnostic and treatment regimen.

Milk production is an important livelihood source for smallholder dairy farmers in low-to-middle-income countries (LMICs) such as Kenya. However, milk quality and safety are a challenge due to unhygienic handling and non-adherence to food safety standards. The objective of this study was to investigate the knowledge, attitudes and adoption of milk quality and food safety practices by smallholder farmers in Kenya. Ten Focus Group Discussions (FGDs), involving 71 smallholder farmers, were held to collect qualitative data on knowledge, attitudes and practices (KAPs) of smallholder dairy farmers in Laikipia, Nakuru, and Nyandarua counties. Additionally, data were collected through a cross-sectional administered to 652 smallholder farming households. The results of the study revealed low knowledge level and negative attitudes towards respecting antibiotics treatment withdrawal periods, milk quality standards and food safety regulations. Farmers stated they had received low levels of training on milk quality and safety standards. The majority of farmers adopted animal health measures and hygienic measures such as hand washing and udder cleaning. However, unhygienic milking environments, the use of plastic containers, the use of untreated water, and lack of teat dipping compromised milk quality and safety. Currently, milk production, handling and consumption could expose actors along the dairy value chain to health risks. The adoption of milk quality and food safety practices was influenced by farmers’ knowledge, socioeconomic characteristics, and choice of marketing channel. There is a need to improve farmers’ knowledge and attitudes and implement hygienic control, disease control and antibiotic residue control practices in the milk production process to meet required milk quality and food safety standards. Awareness campaigns and training programmes for smallholder dairy farmers could foster behavioural change and lead to an improvement in milk quality in Kenya.

Poor milk safety constitutes a persistent public health risk in Kenya. Poor milk composition, microbial contamination and adulteration is a constraint to dairy sector development. We hypothesise that variation in milk quality and safety depends on variation between farming systems. We argue that this variation between farming systems is associated with spatial location which affects the agro-ecological conditions and the availability of labour and land. We used a spatial framework based on the distance to urban markets to distinguish the following farming systems: relatively intensive dairy systems in urban locations (UL), semi-intensive dairy systems in mid-rural locations (MRL) and extensive dairy systems in extreme rural locations (ERL). We aimed to investigate the variation in the quality of raw milk in these dairy farming systems and associated value chains in central Kenya. For this reason, we combined several methods such as participatory rural appraisal, participant observation, and milk physicochemical and microbiological analyses to collect data. Milk samples were collected at the informal and informal value chain nodes - farms, informal collection centres, informal retailing centres including milk vending machines, and formal bulking centres - where milk changes hands between value chain actors. Milk quality was compared to standards recommended by the Kenya Bureau of Standards (KeBS). There were no differences in the quality of raw milk between locations or between nodes. The overall milk physicochemical composition means (standard error) of the milk were within KeBS standards: fat 3.61 (0.05), protein 3.46 (0.06), solid-not fats 9.18 (0.04), density 1.031 (0.0002) and freezing point −0.597 (0.019). The protein percentage was below KeBS standards at all value chain nodes, except at the formal bulking node. There was significant contamination of milk samples: 16.7% of samples had added water, 8.8% had somatic cell count SCC above 300,000, 42.4% had E. coli, 47.9% had Pseudomonas spp., 3.3% had Staphylococcus spp. and 2.9% tested positive for brucellosis antibodies. Unsanitary milk handling practices were observed at farms and all value chains nodes. Milk physicochemical composition except for protein content meets the KeBS Standard. High levels of microbial contamination of milk pose a public health risk to consumers and show that urgent action is needed to improve milk quality.

An evaluation of emerging issues in One Health (OH) in Sub-Saharan Africa was undertaken to map the existing OH initiatives in Sub-Saharan African (SSA) countries. Desk review, expert opinions survey, limited interviews and wider consultations with selected OH stakeholders were conducted. The strengths, weaknesses, opportunities and threats to OH initiatives were identified. OH influence, interest and impacts were evaluated. One Health is transiting from multidisciplinary to transdisciplinary concepts and OH viewpoint should move from ‘proxy for zoonoses’, to include issues of climate change, nutrition and food safety, social sciences, geography, policy and planning, economics, welfare and well-being, antimicrobial resistance (AMR), vector-borne diseases, toxicosis and pesticides issues. While the identified major strengths should be boosted, the weaknesses should be addressed.OH Networks in SSA were spatially and temporally spread across SSA and stakeholders were classified as key, latent, marginal and OH defenders. Imbalance in stakeholders’ representation led to hesitation in buying-in from stakeholders who are outside the main networks. Theory of change, monitoring and evaluation frameworks, and tools to standardized evaluation of OH policies is needed for sustained future of OH and the future OH engagement should be outputs and outcomes-driven and not activity-driven.National roadmap for OH implementation and institutionalization is necessary and proofs of concepts in OH should be verified and scaled-up. Dependence on external funding is unsustainable and must be addressed. Necessary policy and legal instrument to support OH nationally and sub-nationally should be implemented taking cognizance of contemporary issues like urbanization, endemic poverty and other emerging issues. Utilizing current technologies and OH approach to address ongoing pandemic of COVID-19 and other emerging diseases is desirable. Finally, OH implementation should be anticipatory and not reactive to significantly benefit budgeting and contain disease outbreaks in animal sources before the risk of spillover to human can be envisaged.

Rift Valley fever (RVF) is a vector-borne disease transmitted by a broad spectrum of mosquito species, especially Aedes and Culex genus, to animals (domestic and wild ruminants and camels) and humans. Rift Valley fever is endemic in sub-Saharan Africa and in the Arabian Peninsula, with periodic epidemics characterised by 5–15 years of inter-epizootic periods. In the last two decades, RVF was notified in new African regions (e.g. Sahel), RVF epidemics occurred more frequently and low-level enzootic virus circulation has been demonstrated in livestock in various areas. Recent outbreaks in a French overseas department and some seropositive cases detected in Turkey, Tunisia and Libya raised the attention of the EU for a possible incursion into neighbouring countries. The movement of live animals is the most important pathway for RVF spread from the African endemic areas to North Africa and the Middle East. The movement of infected animals and infected vectors when shipped by flights, containers or road transport is considered as other plausible pathways of introduction into Europe. The overall risk of introduction of RVF into EU through the movement of infected animals is very low in all the EU regions and in all MSs (less than one epidemic every 500 years), given the strict EU animal import policy. The same level of risk of introduction in all the EU regions was estimated also considering the movement of infected vectors, with the highest level for Belgium, Greece, Malta, the Netherlands (one epidemic every 228–700 years), mainly linked to the number of connections by air and sea transports with African RVF infected countries. Background The seroprevalence of brucellosis among nomadic pastoralists and their livestock in arid lands is reported to be over10-fold higher than non-pastoralists farmers and their livestock in Kenya. Here, we compared the seroprevalence of nomadic pastoralists and mixed farming with their knowledge of the disease and high-risk practices associated with brucellosis infection. Methods Across-sectional study was conducted in two counties - Kiambu County where farmers primarily practice smallholder livestock production and crop farming, and Marsabit County where farmers practice nomadic pastoral livestock production. Stratified random sampling was applied, in which sublocations were initially selected based on predominant livestock production system, before selecting households using randomly generated geographical coordinates. In each household, up to three persons aged 5 years and above were randomly selected, consented, and tested for Brucella spp IgG antibodies. A structured questionnaire was administered to the household head and selected individuals on disease knowledge and risky practices among the pastoralists and mixed farmers compared. Multivariable mixed effects logistic regression model was used to assess independent practices associated with human Brucella spp. IgG seropositivity. Results While the majority (74%) of pastoralist households had little to no formal education when compared to mixed (8%), over 70% of all households (pastoralists and mixed farmers) had heard of brucellosis and mentioned its clinical presentation in humans. However, fewer than 30% of all participants (pastoralists and mixed farmers) knew how brucellosis is transmitted between animals and humans or how its transmission can be prevented. Despite their comparable knowledge, significantly more seropositive pastoralists compared to mixed farmers engaged in risky practices including consuming unboiled milk (79.5% vs 1.7%, p p < 0.001), assisting in animal birth (43.0% vs 9.3%, p < 0.001), and handling raw hides (30.6% vs 5.5%, p < 0.001)., Conclusion Nomadic pastoralists are more likely to engage in risky practices that promote Brucella Infection, probably because of their occupation and culture, despite having significant knowledge of the disease.

The effects of COVID-19 have gone undocumented in nomadic pastoralist communities across Africa, which are largely invisible to health surveillance systems despite the fact that they are of key significance in the setting of emerging infectious disease. We expose these landscapes as a “blind spot” in global health surveillance, elaborate on the ways in which current health surveillance infrastructure is ill-equipped to capture pastoralist populations and the animals with which they coexist, and highlight the consequential risks of inadequate surveillance among pastoralists and their livestock to global health. As a platform for further dialogue, we present concrete solutions to address this gap.

Elevated blood pressure remains the single biggest risk factor contributing to the global burden of disease and mortality. May Measurement Month is an annual global screening campaign aiming to improve awareness of blood pressure at the individual and population level. Adults ({greater than or equal to}18 years) recruited through opportunistic sampling were screened at sites in 92 countries during May 2019. Ideally three blood pressure readings were measured for each participant, and data on lifestyle factors and co-morbidities were collected. Hypertension was defined as a systolic BP {greater than or equal to} 140 mmHg, and/or a diastolic BP {greater than or equal to} 90 mmHg (mean of the second and third readings) or taking antihypertensive medication. When necessary, multiple imputation was used to estimate participants' mean blood pressure. Mixed-effects models were used to evaluate associations between blood pressure and participant characteristics. Of 1,508,130 screenees 482,273 (32.0%) had never had a blood pressure measurement before and 513,337 (34.0%) had hypertension, of whom 58.7% were aware and 54.7% were on antihypertensive medication. Rift Valley fever (RVF) virus is associated with recurring disease epidemics among livestock and humans in sub-Saharan Africa and Middle East, where the virus is endemic in certain ecosystems. Since the mechanisms of its maintenance during the long (4–10 year) inter-epidemic periods (IEPs) are poorly understood, we reviewed the latest findings that shed light on this, and identify ecological factors unique to the endemic areas. Recent studies reported acute RVF cases in humans and livestock, and significant disease prevalence during IEPs, indicating low-level cycling of the virus in wildlife, livestock, and humans. Niche modeling identified ecological factors that seem important in supporting the mosquito vector and virus maintenance, and the occurrence of RVF epidemics. The virus is primarily maintained by circulating at low levels among wildlife, livestock, and humans, transmitted by mosquito vectors in ecosystems characterized by low annual rainfall during non-El Nino climatic periods and certain soil types. The findings indicate that prolonged survival of the virus in mosquito eggs, suggested in previous studies, may not be required for its maintenance.

Epidemiologic data indicate a global distribution of anthrax outbreaks associated with certain ecosystems that promote survival and viability of Bacillus anthracis spores. Here, we characterized three anthrax outbreaks involving humans, livestock, and wildlife that occurred in the same locality in Kenya between 2014 and 2017. Clinical and epidemiologic data on the outbreaks were collected using active case finding and review of human, livestock, and wildlife health records. Information on temporal and spatial distribution of prior outbreaks in the area was collected using participatory epidemiology. The 2014–2017 outbreaks in Nakuru West subcounty affected 15 of 71 people who had contact with infected cattle (attack rate = 21.1%), including seven with gastrointestinal, six with cutaneous, and two with oropharyngeal forms of the disease. Two (13.3%) gastrointestinal human anthrax cases died. No human cases were associated with infected wildlife. Of the 54 cattle owned in 11 households affected, 20 died (attack rate = 37%). The 2015 outbreak resulted in death of 10.5% of the affected herbivorous wildlife at Lake Nakuru National Park, including 745 of 4,500 African buffaloes (species-specific mortality rate = 17%) and three of 18 endangered white rhinos (species-specific mortality rate = 16%). The species mortality rate ranged from 1% to 5% for the other affected wildlife species. Participatory epidemiology identified prior outbreaks between 1973 and 2011 in the same area. The frequency and severity of outbreaks in this area suggests that it is an anthrax hotspot ideal for investigating risk factors associated with long-term survival of anthrax spores and outbreak occurrence.

Today's sales force play crucial roles and are considered essential and strategic for enhancing competitiveness and sales growth. Thus, effective management of the sales force has assumed top priority for the marketing function in organizations, particularly in emerging markets to capture the millions of newly affluent consumers. This paper assesses how sales force management leverages the dynamics of personal branding and technology-enabled sales tools in the emerging market context to improve sales force productivity and performance. Using structural equation modelling with SmartPLS, responses from 209 sales executives in the insurance and fast moving consumer goods sector in the Greater Accra region of Ghana were analysed. Results highlight the relevance of appropriate training, motivation and recruitment processes on sales force ability to develop personal branding and use of technology to facilitate sales.

Zoonotic diseases, transmitted from animals to humans, are a public health challenge in developing countries. Livestock value chain actors have an important role to play as the first line of defence in safeguarding public health. However, although the livelihood and economic impacts of zoonoses are widely known, adoption of biosecurity measures aimed at preventing zoonoses is low, particularly among actors in informal livestock value chains in low and middle-income countries. The main objective of this study was to investigate knowledge of zoonoses and adoption of biosecurity measures by livestock and milk value chain actors in Bura, Tana River County, in Kenya, where cattle, camels, sheep and goats are the main livestock kept. The study utilised a mixed methods approach, with a questionnaire survey administered to 154 value chain actors. Additional information was elicited through key informant interviews and participatory methods with relevant stakeholders outside the value chain. Our results found low levels of knowledge of zoonoses and low levels of adherence to food safety standards, with only 37% of milk traders knowing about brucellosis, in spite of a sero-prevalence of 9% in the small ruminants tested in this study, and no slaughterhouse worker knew about Q fever. Actors had little formal education (between 0 and 10%) and lacked training in food safety and biosecurity measures. Adoption of biosecurity measures by value chain actors was very low or non-existent, with only 11% of butchers wearing gloves. There was a gendered dimension, evidenced by markedly different participation in value chains and lower adoption rates and knowledge levels among female actors. Finally, cultural and religious practices were shown to play an important role in exposure and transmission of diseases, influencing perceptions and attitudes to risks and adoption of biosecurity measures.

We designed and piloted a livestock identification and traceability system (LITS) along the Northern Tanzania–Narok–Nairobi beef value chain. Animals were randomly selected and identified at the primary markets using uniquely coded ear tags. Data on identification, ownership, source (village), and the site of recruitment (primary market) were collected and posted to an online database. Similar data were collected in all the markets where tagged animals passed through until they got to defined slaughterhouses. Meat samples were collected during slaughter and later analyzed for tetracycline and diminazene residues using high-performance liquid chromatography (HPLC). Follow up surveys were done to assess the pilot system. The database captured a total of 4260 records from 741 cattle. Cattle recruited in the primary markets in Narok (n = 1698) either came from farms (43.8%), local markets (37.7%), or from markets in Tanzania (18.5%). Soit Sambu market was the main source of animals entering the market from Tanzania (54%; n = 370). Most tagged cattle (72%, n = 197) were slaughtered at the Ewaso Ng’iro slaughterhouse in Narok. Lesions observed (5%; n = 192) were related to either hydatidosis or fascioliasis. The mean diminazene aceturate residue level was 320.78 ± 193.48 ppb. We used the traceability system to identify sources of animals with observable high drug residue levels in tissues. Based on the findings from this study, we discuss opportunities for LITS—as a tool for surveillance for both animal health and food safety, and outline challenges of its deployment in a local beef value chain—such as limited incentives for uptake.

This study explores how technology transfer can help firms to build capability through market orientation and outsourcing to impact their performance in Ghana. Quantitative data were collected from Ghana’s Club 100 companies with questionnaires and analysed using various statistical tools, including the partial least squares approach and structural equations modelling. It was found that technology transfer, outsourcing and market orientation were key sources of organisational capability in Ghana’s leading firms. Although the overall effect of technology transfer, outsourcing and market orientation in building capability to impact performance is significant and positive for domestic and foreign firms, the impact is relatively stronger amongst domestic firms. The paper concludes with highlights of the specific contribution it makes to existing knowledge and managerial implications of the findings as well the study’s limitations and directions for future research.

The planet's mean air and ocean temperatures have been rising over the last century because of increasing greenhouse gas (GHG) emissions. These changes have substantial effects on the epidemiology of infectious diseases. We describe direct and indirect processes linking climate change and infectious diseases in livestock with reference to specific case studies. Some of the studies are used to show a positive association between temperature and expansion of the geographical ranges of arthropod vectors (e.g. Culicoides imicola, which transmits bluetongue virus) while others are used to illustrate an opposite trend (e.g. tsetse flies that transmit a range of trypanosome parasites in sub-Saharan Africa). We further describe a positive association between extreme events: droughts and El Niño/southern oscillation (ENSO) weather patterns and Rift Valley fever outbreaks in East Africa and some adaptation practices used to mitigate the impacts of climate change that may increase risk of exposure to infectious pathogens. We conclude by outlining mitigation and adaptation measures that can be used specifically in the livestock sector to minimize the impacts of climate change-associated livestock diseases.

Rift Valley fever (RVF) is a vector-borne zoonotic disease caused by phlebovirus in the family Bunyaviridae. In Kenya, major outbreaks occurred in 1997-1998 and 2006-2007 leading to human deaths, huge economic losses because of livestock morbidity, mortality, and restrictions on livestock trade.This study was conducted to determine RVF seroprevalence in cattle, sheep, and goats during an interepidemic period in Garissa County in Kenya.In July 2013, we performed a cross-sectional survey and sampled 370 ruminants from eight RVF-prone areas of Garissa County. Rift Valley fever virus (RVFV) antibodies were detected using a multispecies competitive enzyme-linked immunosorbent assay. Mixed effect logistic regression models were used to determine the association between RVF seropositivity and species, sex, age, and location of the animals.A total of 271 goats, 87 sheep, and 12 cattle were sampled and the overall immunoglobulin G seroprevalence was 27.6% (95% CI [23-32.1]). Sheep, cattle, and goats had seroprevalences of 32.2% (95% CI [20.6-31]), 33.3% (95% CI [6.7-60]), and 25.8% (95% CI [22.4-42]), respectively. Seropositivity in males was 31.8% (95% CI [22.2-31.8]), whereas that of females was 27% (95% CI [18.1-45.6]).The high seroprevalence suggests RVFV circulation in domestic ruminants in Garissa and may be indicative of a subclinal infection. These findings provide evidence of RVF disease status that will assist decision-makers to flag areas of high risk of RVF outbreaks and prioritize the implementation of timely and cost-effective vaccination programs.

Lake Turkana, the world’s largest desert lake, is the second largest producer of freshwater fishery in Kenya. The lake is co-managed by stakeholders’ groups called Beach Management Units (BMUs) in a co-management arrangement with the government. Despite the enactment of co-management policy, management related challenges including illegal, unreported and unregulated fishing, overexploitation and resource use conflicts still bedevil the lake leading to plummeting fisheries production. This study identified gaps in stakeholders’ inclusion and mobilization methods used by BMUs. Data was collected using questionnaires from 693 respondents in eight beaches. Chi-square was used to test for statistical associations between variables (p?0.05). Results showed that 13 stakeholders grouped into county government (24%), national government (35%) and donors (42%) were included in the lake’s fisheries management. Rift Valley fever (RVF) is a zoonotic disease of great public health and economic importance transmitted by mosquitoes. The main method of preventing the disease is vaccination of susceptible livestock before outbreaks occur. Studies on RVF vaccines have focused on the production processes, safety, and efficacy standards but those on uptake and adoption levels are rare. This study sought to understand the barriers faced by men and women farmers in the uptake of livestock vaccines to inform strategies for optimizing the use of vaccines against RVF in East Africa. The cross-sectional qualitative study utilized the pairwise ranking technique in sex disaggregated focus group discussions to identify and rank these barriers. Results indicate that men and women farmers experience barriers to vaccine uptake differentially. The barriers include the direct and indirect cost of vaccines, distances to vaccination points, availability of vaccination crushes, intra-household decision making processes and availability of information on vaccination campaigns. The study concludes that vaccine provision does not guarantee uptake at the community level. Hence, these barriers should be considered while designing vaccination strategies to enhance community uptake because vaccine uptake is a complex process which requires buy-in from men and women farmers, veterinary departments, county/district and national governments, and vaccine producers.

BackgroundCystic hydatidosis (CH), a neglected parasitic zoonosis, is endemic in many parts of Kenya and could be spread along livestock marketing chains. Poor knowledge, attitude and practices (KAP) enables this spread in remote areas with inadequate public health services. We estimated prevalence, identified possible origin of CH to Busia, Kenya and assessed KAP among cattle owners and abattoir workers.Methods and Principal FindingsWe conducted a cross-sectional study on slaughtered livestock and interviewed their owners and abattoir workers in Busia in May–June 2018. We used visual observation, palpation and incision to identify cysts. Polymerase chain reaction (PCR) was used for confirmatory diagnosis. Epi Info 7 was used to calculate descriptive and associative statistics. Of 302 carcasses inspected, cysts were visualized in nine (2.98%, 95% Confidence Interval (CI): 1.46–5.78). Fourteen samples were collected and 13 (92.86%) were positive on PCR (sensitivity=92%, specificity=95%). All carcasses with cysts were from West Pokot County, which borders Busia to the north. We interviewed 310 participants: 260 were males (83.87%, 95% CI: 79.19 – 87.69); median age was 41 years (range=21-69). Dogs were kept by 221 (71.99%, 95% CI: 66.55 – 76.87), of which 83 (37.56%, 95% CI: 28.33 – 48.52) improperly disposed of dog faeces. Home slaughtering was practiced by 196 (63.23%, 95% CI: 58.78-69.80), of which 115 (58.67%, 95% CI: 51.44-65.64) were not inspected and 85 (43.37%, 95% CI: 36.32-50.62) fed raw organs to dogs. Adequate knowledge was associated with butcher ownership (P-value = 0.002), age ≥35 years (P-value = 0.002) and higher literacy level (P-value Conclusions and SignificanceThere is non-negligible risk of CH in Busia communities which might worsen with time given that the county is connected to endemic areas through livestock trade. Busia County, in Western Kenya is part of a livestock marketing chain between Kenya and Uganda. Animals from high endemic regions in Uganda and Kenya can easily spread the parasite to Busia through improper disposal of their infested organs. Non-pastoral communities like Busia may not have much cumulative experience about the disease though their practices may contribute to the perpetuation of the parasite in their environment. The parasite is gradually spreading to new areas and it is very important to the public health players in Kenya to take action so as to prevent further spread of this disease. Findings from this study show that the disease is no longer limited to pastoral communities only. There is need for the implementation of information, education and communication campaigns to improve the knowledge, attitude and practices of Busia community and other non-endemic regions on the disease.

Introduction Brucellosis is an important zoonotic disease in Kenya, and identifying the bacteria in milk is important in assessing the risk of exposure in people. Methods A cross-sectional study that involved 175 households was implemented in the pastoral counties of Marsabit and Isiolo in Kenya. Pooled milk samples (n = 164) were collected at the household level, and another 372 were collected from domesticated lactating animals (312 goats, 7 sheep, 50 cattle and 3 camels). Real-time polymerase chain reaction (qPCR) testing of the milk samples was performed to identify Brucella species. Brucella anti-LPS IgG antibodies were also detected in bovine milk samples using an indirect enzyme-linked immunosorbent assay (ELISA). Results Based on the qPCR, the prevalence of the pathogen at the animal level (considering samples from individual animals) was 2.4% (95% confidence interval (CI) 1.1–4.5) and 3.0% (CI: 1.0–7.0) in pooled samples. All 14 samples found positive by qPCR were from goats, with 10 contaminated with B. abortus and 4 with B. melitensis. The Brucella spp. antibody prevalence in bovine milk using the milk ELISA was 26.0% (95% CI: 14.6–40.3) in individual animal samples and 46.3% (95% CI: 30.7–62.6) in pooled samples. Conclusion The study is the first in Kenya to test for Brucella spp. directly from milk using qPCR without culturing for the bacteria. It also detected B. abortus in goats, suggesting transmission of brucellosis between cattle and goats. The high prevalence of Brucella spp. is a significant public health risk, and there is a need for intervention strategies necessary in the study area.

Climate change influences the occurrence and transmission of a wide range of livestock diseases through multiple pathways. Diseases caused by pathogens that spent part of their life cycle outside the host (e.g. in vectors or the environment) are more sensitive in this regard, compared to those caused by obligate pathogens. In this chapter, we use two well-studied vector-borne diseases—Rift Valley fever (RVF) and tick-borne diseases (TBDs)—as case studies to describe direct pathways through which climate change influences infectious disease-risk in East and southern Africa. The first case study demonstrates that changes in the distribution and frequency of above-normal precipitation increases the frequency of RVF epidemics. The second case study suggests that an increase in temperature would cause shifts in the spatial distribution of TBDs, with cooler and wetter areas expected to experience heightened risk with climate change. These diseases already cause severe losses in agricultural productivity, food security and socio-economic development wherever they occur, and an increase in their incidence or geographical coverage would intensify these losses. We further illustrate some of the control measures that can be used to manage these diseases and recommend that more research should be done to better understand the impacts of climate change on livestock diseases as well as on the effectiveness of the available intervention measures.

Owing to intense competition in today's business environment, organisations that aspire to excel in their respective sectors need to develop the appropriate capabilities in order to gain and sustain a competitive edge. Outsourcing and technology transfer provide firms with the platform for developing capability to achieve superior performance in the marketplace. However, in the setting of a developing economy, the relative effects of outsourcing and technology transfer on corporate performance among foreign and local companies when investigated together in an amalgamated model is yet to be examined empirically. To obtain a deeper understanding, two pathways through which outsourcing and technology transfer enhance corporate performance of companies in Ghana are presented. Results of this research indicate differences in the extent to which outsourcing and technology transfer influence capability and subsequently affect corporate performance. Limitations of this study as well as implications of the findings for managers and researchers are ultimately highlighted.

Dromedaries in Africa and elsewhere carry the Middle East respiratory syndrome coronavirus (MERS-CoV). To search for evidence of autochthonous MERS-CoV infection in humans, we tested archived serum from livestock handlers in Kenya for MERS-CoV antibodies. Serologic evidence of infection was confirmed for 2 persons sampled in 2013 and 2014.

We examine two widely used treatment strategies for African animal trypanosomosis in West Africa: preventive drug control ex-ante trypanosomosis infection and curative drug control ex-post trypanosomosis infection. We investigate which combination of these alternative strategies is economically optimal for cattle farmers. We apply a dynamic optimisation framework to consider both the negative externality of drug resistance development and human behaviour. We develop a bio-economic model to simulate the economic consequences of treatment strategies in a dynamic scenario that takes into account the interactions among the vector, host and livestock farmers. This model allows for the evolution of drug-resistant trypanosomes through trypanocide misuse and simulates the observed behaviours of cattle farmers based on the elicited risk and time preferences of a sample of 202 cattle farmers in Mali and Burkina Faso. The results show that the private optimal mix of treatment strategies for a risk averse and patient farmer involves preventive treatment for susceptible cattle, supported by a small number of curative treatments for infected cattle. Compared with the treatment strategies observed in the field, this optimal mix of treatment strategies would save approximately 5% of the annual income of a livestock farmer in the study area and would reduce the prevalence of trypanosomosis. In addition, we demonstrate that a reduction in a farmer's risk aversion is associated with higher treatment rates that can avoid additional losses. By contrast, a decrease in a farmer's patience is related to lower treatment rates that thwart additional benefits. Our results suggest that individual risk and time preferences need to be considered in the development process of disease control interventions.

A detailed quantitative comparison of fully nonlinear computations with the measurements of unidirectional wave groups is presented. Computational results on evolving wave groups were compared with previous available experiments. The local surface elevation variation, the evolution of envelope shapes, the velocity of propagation of the steepest crests in the group and their relation to the height of the crests were obtained numerically and experimentally. Conditions corresponding to incipient wave breaking were investigated in greater detail. The results shed additional light on mechanisms leading to the breaking of steep waves, as well as on the crucial importance of exact matching between initial conditions in computations and experiments.
