Supplemental Digital Content is available in the text., IMPORTANCE: Even with its proclivity for older age, coronavirus disease 2019 has been shown to affect all age groups. However, there remains a lack of research focused primarily on the young adult population. OBJECTIVES: To describe the epidemiology and outcomes of coronavirus disease 2019 and identify the risk factors associated with critical illness and mortality in hospitalized young adults. DESIGN, SETTINGS, AND PARTICIPANTS: A retrospective cohort study of the Society of Critical Care Medicine’s Viral Infection and Respiratory Illness Universal Study registry. Patients 18–40 years old, hospitalized from coronavirus disease 2019 from March 2020 to April 2021, were included in the analysis. MAIN OUTCOMES AND MEASURES: Critical illness was defined as a composite of mortality and 21 predefined interventions and complications. Multivariable logistic regression was used to assess associations with critical illness and mortality. RESULTS: Data from 4,005 patients (152 centers, 19 countries, 18.6% non-U.S. patients) were analyzed. The median age was 32 years (interquartile range, 27–37 yr); 51% were female, 29.4% Hispanic, and 42.9% had obesity. Most patients (63.2%) had comorbidities, the most common being hypertension (14.5%) and diabetes (13.7%). Hospital and ICU mortality were 3.2% (129/4,005) and 8.3% (109/1,313), respectively. Critical illness occurred in 25% (n = 996), and 34.3% (n = 1,376) were admitted to the ICU. Older age (p = 0.03), male sex (adjusted odds ratio, 1.83 [95% CI, 1.2–2.6]), and obesity (adjusted odds ratio, 1.6 [95% CI, 1.1–2.4]) were associated with hospital mortality. In addition to the above factors, the presence of any comorbidity was associated with critical illness from coronavirus disease 2019. Multiple sensitivity analyses, including analysis with U.S. patients only and patients admitted to high-volume sites, showed similar risk factors. CONCLUSIONS: Among hospitalized young adults, obese males with comorbidities are at higher risk of developing critical illness or dying from coronavirus disease 2019.

Supplemental Digital Content is available in the text., IMPORTANCE: At the start of the coronavirus disease 2019 pandemic, medications repurposed for management of coronavirus disease 2019 were used in the absence of clinical trial evidence. OBJECTIVES: To describe the variation and evolution in use of repurposed medications for coronavirus disease 2019. DESIGN, SETTING, AND PARTICIPANTS: Observational cohort study of adults hospitalized with coronavirus disease 2019 between February 15, 2020, and April 12, 2021, across 76 United States and international hospitals within the Society of Critical Care Medicine’s Discovery Viral Infection and Respiratory Illness Universal Study coronavirus disease 2019 registry. MAIN OUTCOMES AND MEASURES: Hospital variation was quantified using multivariable adjusted random effects logistic regression models and unsupervised clustering. Repurposed medications included antivirals, corticosteroids, hydroxychloroquine, immunomodulators, and therapeutic dose anticoagulants. RESULTS: Among 7,069 adults hospitalized with coronavirus disease 2019, 1,979 (28%) received antivirals, 2,876 (41%) received corticosteroids, 1,779 (25%) received hydroxychloroquine, 620 (9%) received immunomodulators, and 2,154 (31%) received therapeutic dose anticoagulants. Contribution of hospital site to risk-adjusted variation was 46% for antivirals, 30% for corticosteroids, 48% for hydroxychloroquine, 46% for immunomodulators, and 52% for therapeutic dose anticoagulants. Compared with the early pandemic, the later pandemic practice phenotypes converged with increased use of antivirals (odds ratio, 3.14; 95% CI, 2.40–4.10) and corticosteroids (odds ratio, 5.43; 95% CI, 4.23–6.97), with decreased use of hydroxychloroquine (odds ratio, 0.02; 95% CI, 0.01–0.04) and immunomodulators (odds ratio, 0.49; 95% CI, 0.34–0.70). There was no clinically significant change in the use of therapeutic dose anticoagulants (odds ratio, 1.01; 95% CI, 1.01–1.02). There were no differences in risk-adjusted mortality between hospitals with high rates of repurposed medication use compared with hospitals with low rates of use. CONCLUSIONS AND RELEVANCE: Hospital variation in the use of repurposed medications varied widely across hospitals early in the pandemic and later converged with the emergence of randomized clinical trials. Platforms developed for rapid activation and enrollment in clinical trials of repurposed medications are needed prior to the next pandemic to expedite effective, evidence-based practice.

Abstract

Objective: To investigate whether antidrug antibodies and/or drug non-trough levels predict the long-term treatment response in a large cohort of patients with rheumatoid arthritis (RA) treated with adalimumab or etanercept and to identify factors influencing antidrug antibody and drug levels to optimize future treatment decisions. Methods: A total of 331 patients from an observational prospective cohort were selected (160 patients treated with adalimumab and 171 treated with etanercept). Antidrug antibody levels were measured by radioimmunoassay, and drug levels were measured by enzyme-linked immunosorbent assay in 835 serial serum samples obtained 3, 6, and 12 months after initiation of therapy. The association between antidrug antibodies and drug non-trough levels and the treatment response (change in the Disease Activity Score in 28 joints) was evaluated. Results: Among patients who completed 12 months of followup, antidrug antibodies were detected in 24.8% of those receiving adalimumab (31 of 125) and in none of those receiving etanercept. At 3 months, antidrug antibody formation and low adalimumab levels were significant predictors of no response according to the European League Against Rheumatism (EULAR) criteria at 12 months (area under the receiver operating characteristic curve 0.71 [95% confidence interval (95% CI) 0.57, 0.85]). Antidrug antibody–positive patients received lower median dosages of methotrexate compared with antidrug antibody–negative patients (15 mg/week versus 20 mg/week; P = 0.01) and had a longer disease duration (14.0 versus 7.7 years; P = 0.03). The adalimumab level was the best predictor of change in the DAS28 at 12 months, after adjustment for confounders (regression coefficient 0.060 [95% CI 0.015, 0.10], P = 0.009). Etanercept levels were associated with the EULAR response at 12 months (regression coefficient 0.088 [95% CI 0.019, 0.16], P = 0.012); however, this difference was not significant after adjustment. A body mass index of ≥30 kg/m2 and poor adherence were associated with lower drug levels. Conclusion: Pharmacologic testing in anti–tumor necrosis factor–treated patients is clinically useful even in the absence of trough levels. At 3 months, antidrug antibodies and low adalimumab levels are significant predictors of no response according to the EULAR criteria at 12 months.

Abstract

Introduction: Gender estimation from dismembered human body parts and skeletal remains in cases of mass disasters, explosions, and assaults cases is an imperative element of any medico-legal investigations and has been a major challenge for forensic scientists. Aim: The aim of the present study was to estimate the gender by using index and ring finger length ratio, hand and foot index along with the correlation of both the hand and foot index to determine the vital role of all the indices in establishing gender identity. Materials and Methods: A descriptive cross-sectional study was done on 300 subjects (150 males and 150 females). Various anthropometric measurements like hand length, hand breadth and hand index, Index Finger Length (IFL), Ring Finger Length (RFL) and IFL/RFL ratio as well as foot length, foot breadth and foot index were estimated in millimeters (mm) with the help of a sliding-anthropometric caliper. The data was analysed using independent t-test and Pearson correlation coefficient test. A probability value (p) of < 0.05 was considered statistically significant. Results: The index and ring finger ratio was found to be higher in females as compared to males. The hand and foot index was more in males than in females. The index and ring finger length ratio, hand and foot index between males and females was found to be statistically significant for both hands and feet. A statistically significant correlation was determined between hand indexes versus foot index. Conclusion: This study can be useful to establish the gender of a dismembered hand or foot when subjected for medicolegal examination.

Abstract

We report magnetic resonance results on (0.3-8 [micro]m) films of c-axis oriented barium ferrite grown by pulsed laser deposition on sapphire substrates. By optimizing the growth and annealing parameters, we have observed numerous (up to 25) spin wave resonance modes as well as nearly intrinsic ferromagnetic resonance linewidth of 23 Oe at 58 GHz, when the applied field is parallel to the c axis. On rotation of the field away from the film normal, the linewidth increases rapidly. This is taken in to account by using a small (0.3 [degrees]) spread in the film orientation combined with the conventional formula for shape anisotropy. Atomic force microscopy surface roughness measurements support these considerations. Index Terms--Ferrite films, magnetic materials, magnetic resonance, thin films.

Abstract

We present the results of an investigation of the magnetic properties [A.sub.2]FeRe[O.sub.6] (A = Ca, Sr, Ba). We have studied the magnetization, high-temperature susceptibility and the ferromagnetic resonance response. The Ba and Sr systems are conducting while the Ca material is an insulator. The ordering temperature of the Ca materials is highest (~ 540 K) and decreases to 315 K for the Ba system. The high-temperature susceptibility suggests a large moment, and the Curie-Weiss temperature is very close to the ordering temperature, as expected for a simple ferromagnet, although the ferromagnetic moment is somewhat less than expected. The spin dynamics are rather peculiar: the ferromagnetic resonance results suggest a magnetically inhomogeneous material. We discuss these results in terms of cation disorder. Abstract

Dens invaginatus (DI or dens in dente) and Talons cusp or dens evaginatus are relatively rare developmental anomalies which affect the shape of teeth. DI is a deep surface invagination of the crown or root which is commonly detected on routine radiographic examination. Talon cusp is an additional cusp that projects predominately from the lingual surface of anterior teeth. These anomalies most commonly may be observed in the maxillary lateral tooth. Concurrently, the coexistence of these two anomalies in a single tooth is considered extremely rare. However, still in literature, there are a few reports cases available. This article is a case report of Talons cusp with DI involving maxillary lateral incisors in a 16-year-old patient.

Abstract

The effect on shareholder wealth of removing preemptive rights from corporate charters is examined. Samples of 303 preemptive rights amendments from 1962 through 1981 are examined. A decrease in shareholder wealth is observed. Rights offerings are more beneficial to shareholders than underwritten offerings and less costly. The amendment causes share prices to fall on mailing date.

Abstract

Amongst the varieties of procedures which have been adopted for controlling birth rate, the intra-uterine contraceptive device (IUCD) has a very prominent place. The IUCD has been found to be effective as a contraceptive device in several studies (1 — 4) and several theories have been proposed to explain its mechanism of action. Recently it has been suggested that the presence of IUCD in the rat uterus leads to an increase in the local mast cell population, and this may be an important factor in its mechanism of contraceptive action (5). It was further proposed that increase in number of mast cells might be directly responsible for prevention of nidation, as injection of comp. 48/80 a compound which is known to disrupt mast cells, at times allowed conception to take place even in the presence of the device. In view of the importance of the problem, the mast cell population in human uterus after insertion of the IUCD was studied.
