Abstract

Data regarding transthoracic echocardiography (TTE) application to obstetric patients are scant. Often, anaesthetists preparing for caesarean section are unaware of the proportion of obstetric patients who have relevant cardiac disease. This audit aimed at undertaking a retrospective analysis of TTE performed in intensive care unit patients after caesarean section. Over five years, 56 women were eligible. Echocardiographic abnormalities were deemed relevant if graded as of moderate severity. The most common reason was dyspnoea (41%). Echocardiography demonstrated structural abnormality in 29% or functional abnormality in 38%. It may be appropriate to undertake preoperative echocardiography more commonly in high-risk obstetric patients.

Abstract

Purpose To investigate the natural history and genetic associations of drusenoid pigment epithelial detachment (DPED) associated with age-related macular degeneration (AMD). Design Retrospective analysis of a prospective cohort study. Participants Of the 4203 Age-Related Eye Disease Study 2 (AREDS2) participants, 391 eyes (325 participants) had DPED without late AMD at the time of DPED detection. Genetic analyses included 120 white AREDS2 participants and 145 Age-Related Eye Disease Study (AREDS) participants with DPED. Methods Baseline and annual stereoscopic fundus photographs were graded centrally to detect DPED, a well-defined yellow elevated mound of confluent drusen ≥433 μm in diameter, and to evaluate progression rates to late AMD: geographic atrophy (GA) and neovascular (NV)-AMD. Five single nucleotide polymorphisms (CFH [rs10611670], C3 [rs2230199], CFI [rs10033900], C2/CFB [rs114254831], ARMS2 [rs10490924]) and genetic risk score (GRS) group were investigated for association with DPED development. Kaplan–Meier analyses and multivariable proportional hazard regressions were performed. Main Outcome Measures Progression rates to late AMD and decrease of ≥3 lines in visual acuity (VA) from the time of DPED detection; association of rate of DPED development with genotype. Results Mean (standard deviation [SD]) follow-up time from DPED detection was 4.7 (0.9) years. DPED was associated with increased risk of progression to late AMD (hazard ratio [HR], 2.36; 95% confidence interval [CI], 1.98–2.82; P Conclusions This study replicates the results of previous natural history studies of eyes with DPED including the high rates of progression to late AMD and vision loss (regardless of progression to late AMD). The genetic associations are consistent with genes associated with AMD progression.

Abstract

Purpose To analyze the prevalence, incidence, and clinical characteristics of eyes with geographic atrophy (GA) in age-related macular degeneration (AMD), including clinical and genetic factors affecting enlargement. Design Prospective cohort study within a controlled clinical trial. Participants Age-Related Eye Disease Study 2 (AREDS2) participants, aged 50–85 years. Methods Baseline and annual stereoscopic color fundus photographs were evaluated for GA presence and area. Analyses included GA prevalence and incidence rates, Kaplan-Meier rates, mixed-model regression, and multivariable analysis of the square root of GA, area adjusted for covariates, including clinical/imaging characteristics and genotype. Main Outcome Measures (1) Presence or development of GA; (2) change in the square root of GA area over time. Results At baseline, 517 eyes (6.2%) of 411 participants (9.8%) had pre-existing GA (without neovascular AMD), with the following characteristics: 33% central, 67% noncentral; and the following configurations: 36% small, 26% solid/unifocal, 24% multifocal, 9% horseshoe/ring, and 6% indeterminate. Of the remaining 6530 eyes at risk, 1099 eyes (17.3%) of 883 participants developed incident GA without prior neovascular disease during mean follow-up of 4.4 years. The Kaplan-Meier rate of incident GA was 19% of eyes at 5 years. In eyes with incident GA, 4-year risk of subsequent neovascular AMD was 29%. In eyes with incident noncentral GA, 4-year risk of central involvement was 57%. GA enlargement rate (following square root transformation) was similar in eyes with pre-existing GA (0.29 mm/year; 95% confidence interval 0.27–0.30) and incident GA (0.28 mm/year; 0.27–0.30). In the combined group, GA enlargement was significantly faster with noncentrality, multifocality, intermediate baseline size, and bilateral GA (P Conclusions Analyses of AREDS2 data on natural history of GA provide representative data on GA evolution and enlargement. GA enlargement, which was influenced by lesion features, was relentless, resulting in rapid central vision loss. The genetic variants associated with faster enlargement were partially distinct from those associated with risk of incident GA. These findings are relevant to further investigations of GA pathogenesis and clinical trial planning.

Abstract

Maturation of powder-bed additive manufacturing (AM) is essential for the business benefit the rapid adoption of AM offers to industry. One of the principal challenges in powder-bed AM is the mitigation of distortion due to material shrinkage and residual stresses induced during the build process. In order to address this, a new methodology for distortion compensation is developed and presented in this paper. The novelty of the methodology lies in the use of a mathematical model for pre-distorting the design geometry based on 3D optical scanning measurement data. The methodology has been applied to two industrial Inconel 718 components (a turbine blade and an impeller). It was experimentally demonstrated that distortion compensation is achievable using the proposed methodology. The results showed the compensation methodology reduced distortion from approximately ±300 μm to approximately ±65 μm for both components. In summary, the novel methodology can be used to deliver near-zero distorted parts for industry using powder-bed AM processes.

Examining two characters from different cultures whose ancestors fought for opposing sides during World War II, this story wonders if their marriage was doomed by history and the silences of intergenerational shame that emerged in Germany particularly during the post-war occupation period. Using the methodology of the witnessing imagination this essay also argues that creative writing gives shape to traumatic historical events and allows remarkable access to complex processes of recovery. e19327 Background: Micro-Satellite Instability (MSI) or DNA Mismatch Repair (MMR) is an established prognostic test associated with response to chemotherapies in Lynch Syndrome, colorectal cancer and other indications. Recently, MSI/MMR status has been shown to predict response to immune checkpoint inhibitors (CPI). Pembrolizumab has been approved in the USA for MSI-high or mismatch repair deficient patients for all indications. A similar approval has not been obtained in the EU. This data research project examined MSI/MMR testing in Europe and its possible association with use of CPI therapies. Methods: This study was conducted using Oncology Dynamics, an IQVIA oncology cross-sectional survey, collecting anonymized real-world patient-level data. 97,807 and 96,111 drug treated patients for solid tumors in 2018 & 2019, respectively, were collected across 5 European countries (France, Germany, Great Britain, Italy and Spain). Data included number of patients tested for MSI/MMR, results of the test, site and funding type, and the drug(s) prescribed. Results: European MSI/MMR test rates ranged from 3% in Italy in 2018 to 12% in France in 2019. Testing rates increased modestly in all countries over the period. Testing rates were determined in 25 specific indications plus all other indications. As expected, rates were highest in colorectal and stomach cancer. Testing was observed in other indications including Esophageal, Brain and Melanoma all with rates of 10% or higher. The percentage of MSI-High patients ranged from a low of 13% in Italy in 2019 to 26% in France in 2019. Finally, 17% of MSI-High patients are treated with CPIs compared to 5% of MSI-Low patients across all countries in 2019. Conclusions: The results over two years and almost 200,000 patients present insights into the use of MSI testing in 5 European countries. MSI testing is performed in many indications in addition to those typically associated with the biomarker. The increase in CPI use in MSI-High patients suggest that test results may impact use of checkpoint inhibitors although causation may not be inferred from the current analysis. However, the correlation suggests that further analysis is warranted.

Abstract

We report a retrospective review of our institutional data using once daily intravenous (IV) busulfan (Bu) and cyclophosphamide (Cy) and total body irradiation (TBI)/Cy in patients who received allogeneic hematopoietic stem cell transplant (HCT) from January 2000 to December 2006. Bu 3.2 mg/kg IV once daily × 4 days followed by Cy 60 mg/kg IV daily × 2 days was given to 42 patients. Cy 60 mg/kg IV daily for 2 days and fractionated TBI 1200 cGy delivered over 3 days was given to 60 patients. Veno-occlusive disease developed in two patients in the once daily BuCy group and no patients in the TBI/Cy group. The once daily BuCy group had significantly less transplant-related mortality (TRM) than the TBI/Cy group at 100 days ( p = 0.047) and better overall survival at 1 year ( p = 0.01). This review demonstrates once daily IV BuCy and allogeneic HCT is well tolerated with no unexpected TRM or differences in clinical outcomes.

Abstract

Background Approximately 2 million Americans develop a diabetic foot ulcer (DFU) annually; >50% die and 5% lose a limb within 5 years. IDSA guidelines recommend multidisciplinary team care for these patients (moderate evidence). Little is known about who should compose the team or how the team should function (low evidence). We conducted a systematic review following PRISMA guidelines to evaluate the effect of multidisciplinary team care on major amputation in patients with DFU’s and describe team composition and function. Methods A medical reference librarian searched databases without date limits through May 26, 2017. Two independent reviewers screened abstracts and then full text using the following inclusion criteria: original article; reported the effect of multidisciplinary teams (≥2 specialties) on major amputation; included a control group; >50% of study patients had diabetes; in English. Abstracted data included study design, patient characteristics, team composition and function, and major amputation rates. Results We included 33 studies (Figure 1). Five (15%) were in the United States, and 27 (82%) were historically controlled trials. Thirty-two (97%) documented lower major amputation rates among patients cared for by a multidisciplinary team (Figure 2). Relative reductions ranged from 11 to 90%. A 12% relative increase was observed in the single study documenting increased rates of major amputation following multidisciplinary care. Thirty-six different specialties were represented in the 26 studies reporting team composition, including: endocrinology (85%), vascular surgery (73%), orthopedic surgery (65%), podiatry (54%), and infectious disease (50%). Teams functioned in the following settings: inpatient (30%), outpatient (15%), or both (55%). Among 12 studies reporting team function, the following topics were addressed: surgical debridement/offloading (66%), vascular disease (63%), infection (59%), and glycemic control (41%). Conclusion Care by multidisciplinary teams may help prevent major amputation for patients with DFUs. Team composition and function, and reductions in major amputation rates, varied considerably. The average American teenager spends nearly eleven hours a day engaged in social media and accessing digital music multimedia, which has negative effects on health and educational attainment. Curricula that contain youthfriendly media could improve learning, reduce risky and problem behavior and improve health outcomes, but may be controversial given some of the content. This study was designed to assess stakeholder’s perceptions of a curriculum built around Popular Youth Music Media (PYMM) called Musics Energy: The Message in the Music™ (ME: MIM). This intervention consists of six modules implemented over 24 sessions. ME-MIM uses PYMM to teach students how to deconstruct music by collaborating, communicating, reading closely, thinking critically, conducting research and taking useful notes by listening. In-depth interviews and focus groups were conducted with 11 middleschool students and 6 teachers across multiple school districts. The constant comparative method was employed to explore emerging and recurring themes. Major themes were that ME: MIM was acceptable to most students and conditionally acceptable to most teachers. Stakeholders indicated that ME:MIM may be used as a component of a culturally informed, carefully monitored curriculum. Given the findings of this study, ME:MIM appears to be an acceptable and feasible approach to addressing health risk behavior, education disparities and educational engagement, especially among those who may be at higher risk for school disengagement and those who are exposed to more programmed media messages.

Nuclear medicine technologists (NMTs) have the highest effective doses of radiation among medical workers. With increase in the use of lightweight materials in diagnostic radiography, the aim was to compare the effectiveness of lead and lightweight aprons in shielding from 99m-Technetium ((99m)Tc) gamma rays. The doses received from a scattering phantom to the entrance, 9cm depth and exit of a phantom were measured with LiF:Mg, Cu, P thermoluminescent dosemeters (TLDs). Doses and spectra were assessed without no shielding, with 0.5-mm lead and lightweight aprons. The lead and lightweight aprons decreased entrance surface doses by 76 and 59%, respectively. Abstract

Inhibition of the mammalian target of rapamycin (mTOR) signaling pathway is a potentially useful therapeutic strategy in the treatment of advanced prostate cancer. However mTOR antagonists used as single agents are not likely to result in dramatic clinical responses, so that it is useful to identify prospective agents that might be useful in combination. We treated CWR22Rv1 and LNCaP prostate cancer cells with an mTOR inhibitor, rapamycin, alone, or in combination with either of two receptor protein kinase (RTK) inhibitors. We assessed the effects of these treatments on cell survival and activation of down-stream mTOR target proteins. Treatment with either PD16839, an EGFr antagonist, or imatinib mesylate (Gleevec), a PDGFr, c-kit and bcr/abl antagonist, enhanced the anti-proliferative effects of rapamycin. We therefore assessed the effects of treatment with the RTK antagonist alone and in combination with rapamycin on mTOR targeted proteins. RTK antagonists alone had no effect or paradoxically increased phosphorylation of the mTOR targeted proteins, p70 S6 kinase and ribosomal S6. In contrast, when these cells were treated with either RTK antagonist in the presence of rapamycin, there was a dramatic decrease in phosphorylation of these two mTOR-targeted proteins. These effects were not mediated through phospho-AKT. Since two separate RTK antagonists had additive antiproliferative effects in combination with an mTOR antagonist and were associated with a dramatic decrease in mTOR targeted proteins in cells with or without PTEN expression, the strategy deserves further evaluation for the treatment of prostate cancer.

Improving symptom management for palliative care patients has obvious benefits for patients and advantages for the clinicians, as workload demands and work-related stress can be reduced when the emergent symptoms of patients are managed in a timely manner. The use of emergency medication kits (EMKs) can provide such timely symptom relief. The purpose of this study was to conduct a survey of a local service to examine views on medication management before and after the implementation of an EMK and to conduct a nationwide prevalence survey examining the use of EMKs in Australia. Most respondents from community palliative care services indicated that EMKs were not being supplied to palliative care patients but believed such an intervention could improve patient care.

Abstract

In the past 30 years, the rates of incarceration and recidivism for women in the United States have increased dramatically. Choice Theory® Connections (CTC) is a gender-tailored pre-release intervention program based on Choice Theory® (Glasser, 1999), and designed to achieve meaningful and sustainable cognitive and behavioral change. This evaluation examines CTC among 96 female participants in a California state prison enrolled in an introductory ( n = 58) or advanced ( n = 38) course. CTC significantly improved perceived stress, mindfulness, emotion regulation, impulsivity, and well-being on completion; effects were stronger for the introductory cohort, but significant effects also emerged for the advanced cohort. In addition, participants in the advanced cohort reported better scores at baseline, demonstrating the effects of prolonged engagement with the intervention. Results suggest that CTC can improve incarcerated women’s well-being pre-release, a strong predictor of recidivism post-release. Further study and wider use of CTC are encouraged.

There are many challenges in developing research projects in research-naive clinical settings, especially palliative care where resistance to participate in research has been identified. These challenges to the implementation of research are common in nursing practice and are associated with attitudes towards research participation, and some lack of understanding of research as a process to improve clinical practice. This is despite the professional nursing requirement to conduct research into issues that influence palliative care practice. The purpose of this paper is to describe the process of implementing a clinical research project in collaboration with the clinicians of a palliative care community team and to reflect on the strategies implemented to overcome the challenges involved. The challenges presented here demonstrate the importance of proactively implementing engagement strategies from the inception of a research project in a clinical setting.

Abstract

Circulating inflammatory markers may play an important role in cognitive impairment at older ages. Mice deficient for the chemokine (C-C motif) receptor 2 (CCR2) develop an accelerated Alzheimer-like pathology. CCR2 is also important in neurogenesis. To identify human gene transcripts most closely associated with Mini-Mental State Examination (MMSE) scores, we undertook a genome-wide and inflammation specific transcriptome screen in circulating leukocytes from a population-based sample.We measured in vivo transcript levels by microarray analysis in 691 subjects (mean age 72.6 years) in the InCHIANTI study (Invecchiare in Chianti, aging in the Chianti area). We assessed expression associations with MMSE performance at RNA collection and prior 9-year change in MMSE score in linear regression models.In genome-wide analysis, raised CCR2 expression was cross-sectionally the most strongly associated transcript with lower MMSE score (beta=-0.16, p=5.1×10(-6), false discovery rate (FDR; q=0.077). Amongst inflammatory transcripts, only CCR2 expression was associated with both MMSE score and accelerated decline in score over the preceding 9 years (beta=-0.16, p=5.1×10(-6), q=0.003; and beta=-0.13, p=5.5×10(-5), q=0.03, respectively). CCR2 expression was also positively associated with apolipoprotein E (ApoE) e4 Alzheimer disease risk haplotype.We show for the first time that CCR2 expression is associated with lower MMSE scores in an older human population. Laboratory models of Ccr2-mediated β-amyloid removal and regulation of neurogenesis affecting cognitive function may be applicable in humans. CCR2-mediated pathways may provide a possible focus for intervention to potentiate protective reactions to Alzheimer pathology in older people, including for people with an adverse ApoE haplotype.

Abstract

Crom (Core Rhythm Operating Machine) was designed to provide a flexible, extensible platform on which to develop our next generation of visual effects tools. At its heart were three key design decisions: utilize a strongly-typed dependency graph where the functional pieces are responsible for producing/manipulating a single value type, and the connective tissue handles necessary type conversions; separate functionality and value storage so a single functional piece can operate on disparate tasks without losing the advantage of a persistent cache; and make the application extensible in both functionality and interface through all levels of production development.

Abstract

Our purpose was to compare systolic and diastolic blood pressures obtained with the Spacelabs 90207 (Spacelabs Medical Products, Dee Why, Australia) or Accutracker II (Suntech Medical Instruments, Melbourne, Australia) ambulatory blood pressure monitoring devices with intraarterial blood pressures in pregnant women.Direct (intraarterial) and resting blood pressures with the ambulatory blood pressure monitoring device were compared in 39 pregnant women (14 Accutracker II and 25 Spacelabs 90207).The Accutracker II device underestimated direct systolic pressure by -9 (-13, -3) mm Hg (median, interquartile range) (p = 0.028) but gave similar diastolic pressure. The Spacelabs 90207 device gave similar systolic pressures but overestimated direct diastolic pressure by 7 (2, 12) mm Hg (p0.001). Variability for systolic and diastolic blood pressures within subjects was similar with the two devices. Both received poor gradings by standards of the British Hypertension Society and did not meet criteria of the Association for the Advancement of Medical Instrumentation, with intraarterial recordings used as the reference.The Accutracker II device significantly underestimated resting direct systolic pressure, whereas the Spacelabs 90207 device significantly overestimated resting direct diastolic pressure in pregnant women. Although poor gradings were achieved for both devices when intraarterial pressures were used as the reference, this is similar to comparisons of routine mercury sphygmomanometry with intraarterial recordings and does not mean these devices are unsuitable for use in pregnancy.

Abstract

Our goals were to determine (1) whether Korotkoff phase IV or V sound was a more accurate measure of diastolic blood pressure in pregnancy and (2) interobserver variability of mercury sphygmomanometry of pregnant women.Direct (intraarterial) and indirect (mercury sphygmomanometry) blood pressures were compared in 28 pregnant women. Interobserver variability was assessed in a separate study of 86 pregnant women using four highly trained observers.(1) Routine sphygmomanometry underestimated direct systolic pressure by 11 (3, 18) mm Hg, p0.001 (median, interquartile range of differences). Phase IV Korotkoff sound overestimated direct diastolic pressure by 9 (2, 12) mm Hg (p0.001) and phase V by 4 (2, 7) mm Hg (p = 0.04). Phase V-recorded diastolic pressure was closer to direct diastolic pressure significantly more often (75%) than was phase IV-recorded diastolic pressure (21%) (p = 0.003). Mean arterial pressures did not differ significantly according to the method used. (2) Median blood pressures did not differ among the four observers for systolic, diastolic phase IV, or phase V recordings. Maximum difference for blood pressure recording among observers was 4 (2, 6) mm Hg.Auscultatory sphygmomanometry in pregnant women underestimates systolic and overestimates diastolic blood pressure, but the phase V Korotkoff sound is more likely to represent the true diastolic pressure than is the phase IV sound.
