Purpose To report long-term results of stent placement and medical therapy for symptomatic and asymptomatic patients, respectively, with carotid artery near-occlusion with full collapse. Materials and Methods Between January 2008 and December 2010, 204 carotid arteries diagnosed by duplex scanning as exhibiting complete occlusion were re-examined with CT angiography; 46 arteries in 46 patients were patent with threadlike lumens and were reclassified as exhibiting near-occlusion with full collapse. Asymptomatic patients (n = 22) received best medical therapy (BMT) alone, and symptomatic patients (n = 24) were referred for carotid artery stent (CAS) placement plus BMT. Patients underwent clinical follow-up for 63.9 months ± 23.6 and duplex surveillance. Results None of the 22 asymptomatic patients treated with BMT alone experienced neurologic events during the follow-up interval. Four died of unrelated causes, resulting in a cumulative survival rate of 81.8%. Technical failure occurred in 5 of 24 symptomatic patients, but none had perioperative complications related to inability to cross the near-occlusion. Of the 19 patients with procedural success, 1 developed immediate upper limb monoparesis; none had periprocedural myocardial infarction, and none died. At 60-month follow-up, patients who underwent successful CAS placement had neurologic event–free and cumulative survival rates of 89.4% and 89.4%; patients with failed recanalization had neurologic event–free and cumulative survival rates of 0% and 40.0% (P = .01). Conclusions Asymptomatic patients with carotid near-occlusion with full collapse experienced good outcomes with BMT alone. Symptomatic patients who underwent CAS placement demonstrated long-term survival and freedom from neurologic event rates comparable to those of asymptomatic patients.

This article has the general objective to verify the accessibility of academics with a disability or reduced mobility in a teaching center of a Federal Public University of Santa Catarina. To meet this goal, there are this specific objectives: Check for people with disabilities or reduced mobility; Verify the sustainability of accessibility with a disabled and propose a summary management plan - 5W2H for the deficit issues. The 5W2H is an objective management tool and consists of seven questions to answer to the deficit issues. It is noteworthy that can be used for decision making about issues found after the calculation of sustainability. The methodology of the aims is considered descriptive and exploratory. With regard to the technical procedures this is a case study. The approach to the problem is characterized as qualitative. In the end it was concluded that the overall sustainability of the accessibility obtained the 48.8% rate, considered "regular", "only serves legislation". Thus, it is understood that people according to the law in Brazil have the right to study and all institutions need to provide access and use of environments for people with disabilities or reduced mobility. As a process of continuous improvement the 5W2H suggests as one of the actions: increasing the number of restrooms for accessibility within what the law provides. The study covers analysis of the ten dimensions for evaluation of courses by INEP.

Neurocysticercosis is the most common parasitic infection affecting the central nervous system, usually involving the brain parenchyma, intracranial subarachnoid space, or ventricular system. In rare cases, there is involvement of the spine (vertebral, epidural, subdural, arachnoid, or intramedullary). Even in endemic regions, this variant is rare, with an incidence below 5% of all patients. The diagnosis is made based on the symptoms, which can be very unspecific, imaging and CSF analysis, with biopsy as a possibility. Treatment is usually curative, but important deficits can develop, due to compression of the spinal cord or nerve roots, arachnoiditis, or meningitis. We present the case of a patient who developed this entity, with poor clinical scenario, and review the literature on the topic.

RESUMO Objetivo: analisar a influência da carga trabalho, estresse, Burnout, satisfação e percepção do ambiente de cuidado, pela equipe de enfermagem com a presença de eventos adversos em Unidade de Terapia Intensiva de Trauma. Método: estudo observacional realizado em Unidade de Terapia Intensiva de Trauma. Para a coleta de incidentes foram acompanhados 195 pacientes, prospectivamente, sendo utilizados instrumentos para medir a carga de trabalho de enfermagem, estresse, Burnout, satisfação no trabalho e trabalho de avaliação do ambiente laboral pela equipe de enfermagem. Os dados foram analisados por meio de análise estatística. Resultados: ocorreram 1.586 incidentes, predominantemente incidentes sem dano (78,44%). Entre a equipe de enfermagem, 77,40% tinham níveis médios de estresse; 17,00% apresentaram Burnout; 56,6% estavam insatisfeitos e consideraram as características ambientais inadequadas. A carga de trabalho de enfermagem foi alta (73,24%). Houve associação entre incidentes e tempo de permanência. Os incidentes sem dano tiveram associação com a carga de trabalho de enfermagem. Conclusão: a identificação de fatores associados pode prevenir a ocorrência de incidentes. Método: estudio observacional en la Unidad de Cuidados Intensivos de Trauma. Para la recolección de los incidentes fueron seguidos prospectivamente 195 pacientes siendo utilizados instrumentos para medir la carga de trabajo de enfermería, el estrés, el Burnout, la satisfacción laboral y el trabajo de evaluación del entorno de trabajo por parte del personal de enfermería. Los datos se analizaron usando el estudio estadístico apropiado. Resultados: ocurrieron 1.586 incidentes predominantemente incidentes sin daño (78,44%). Entre el personal de enfermería, 77.40% tenían niveles medios de estrés; 17,00% mostró Burnout; y el 56,6% estaban insatisfechos y consideraron inadecuadas las características ambientales. La carga de trabajo de enfermería fue alta (73,24%). Se observó una asociación entre la incidencia y la duración de la estancia. Incidentes sin lesiones se asociaron con la carga de trabajo de enfermería. Conclusión: La identificación de factores asociados puede prevenir la aparición de incidentes. Among the nursing staff, 77.40% had average levels of stress; 17.00% presented Burnout; 56.6% were dissatisfied and considered the environmental characteristics to be inadequate. Nursing workload was high (73.24%). An association between incident and length of stay was found. No harm incidents were associated with nursing workload. Conclusion: identifying associated factors can prevent the occurrence of incidents.

OBJECTIVE: To evaluate the decision of orthopedics surgeons regarding which cases they would indicate surgery or non-surgical treatment. METHODS: 20 images of radiographs with fracture in the middle third of the collar bone (AO/OTA 15-B) in anteroposterior view were analyzed, and divided into four groups: group 1 - fracture type AO/OTA 15-B1 without displacement; group 2 - fracture type AO/OTA 15-B1 with displacement; group 3 - fracture type AO/OTA 15-B2; group 4 - fracture type AO/OTA 15-B3. The evaluator was requested to indicate the choice of treatment, surgical or non-surgical. RESULTS: There was no strong correlation between the amount of surgical indications and the working experience or age of the medical evaluator. It was observed that the average of surgical indications in the total sample was 52%. When indications were studied in different areas of Brazil, there was no significant difference among them. No pattern for the Brazilian regions studied was observed in the case analysis. Even within a group (cases of the same complexity), no specific pattern of surgical indication was observed. CONCLUSION: No association between surgical indication and the length of professional experience was found. The Southern and Southeastern regions were those that most recommended surgeries in groups 2, 3, and 4. In no region the same level of surgical indication for cases of the same complexity rate was kept. RESUMO OBJETIVO: Avaliar a decisão de cirurgiões ortopédicos sobre em que casos indicariam a cirurgia ou tratariam não cirurgicamente. MÉTODOS: Foram analisadas 20 imagens de radiografias com fratura do terço médio da clavícula (AO/OTA 15-B) em incidência anteroposterior, que foram divididas em quatro grupos: 1 - fratura do tipo AO/OTA 15-B1 sem desvio; 2 - fratura do tipo AO/OTA 15-B1 com desvio; 3 - fratura do tipo AO/OTA 15-B2; 4 - fratura do tipo AO/OTA 15-B3. Ao avaliador, foi solicitado que indicasse o tipo de tratamento: cirúrgico ou não cirúrgico. RESULTADOS: Não houve correlação forte entre a quantidade de indicações cirúrgicas e o tempo de atuação do médico avaliador ou sua idade. Verificou-se que a média de indicação de cirurgias no total da amostra foi de 52%. Quando estudadas as indicações por diferentes regiões do Brasil, não houve diferença significativa. Não foi verificado qualquer padrão para as regiões brasileiras na análise por caso. Mesmo dentro de um grupo (casos de mesma complexidade), não foi verificado um padrão específico de indicação cirúrgica. CONCLUSÃO: Não foi verificada associação entre a indicação cirúrgica e o tempo de atuação do profissional. As regiões Sul e Sudeste são as que mais recomendam a cirurgia dos grupos 2, 3 e 4. Verificou-se que em nenhuma região foi mantido o mesmo nível de indicação de cirurgias para casos do mesmo grau de complexidade.

Aortoesophageal fistula induced by atherosclerotic thoracic aortic aneurysm is rare, but is usually a fatal disorder, with few survivors reported. We report the case of a 72-year-old man with aortoesophageal fistula successfully treated in a two-stage operation. In the first stage, we performed resection and replacement of the aortic aneurysm with a prosthetic graft in situ, esophagectomy, cervical esophagostomy, and jejunostomy. After the patient recovered well postoperatively, a transmediastinal retrosternal interposition of the stomach was performed, with esophagogastroanastomosis in the cervical area, to re-establish the gastrointestinal tract. We include a discussion of the causes, diagnostic approach, management of the aorta and esophagus, and review of the literature. (J Vasc Surg 1999;30:1150-7.)
