1. Pharmaceutical pricing and reimbursement policies in Algeria, Morocco, and Tunisia: comparative analysis

Abstract: ABSTRACTObjectives In this paper, we outline and compare pharmaceutical pricing and reimbursement policies for in-patent prescription medicines in three Maghreb countries, Algeria, Morocco, and Tunisia, and explore possible improvements in their pricing and reimbursement systems.Methods The evidence informing this study comes from both an extensive literature review and a primary data collection from experts in the three studied countries.Key findings Twenty-six local experts where interviewed Intervieweesincluded ministry officials, representatives of national regulatory authorities, health insurance organizations, pharmaceutical procurement departments and agencies, academics, private pharmaceutical-sector actors, and associations. Results show that External Reference Pricing (ERP) is the dominant pricing method for in-patent medicines in the studied countries. Value-based pricing through Health Technology Assessment (HTA) is a new concept, recently used in Tunisia to help the reimbursement decision of some in-patent medicines but not yet used in the pricing of innovative medicines in the studied countries. Reimbursement decision is mainly based on negotiations set on Internal Reference Pricing (IRP).Conclusion Whereas each country has its specific regulations, there are many similarities in the pricing and reimbursement policies of in-patent medicines in Algeria, Morocco, and Tunisia. The ERP was found to be the dominant method to inform pricing and reimbursement decisions of in-patent medicines. Countries in the region can focus on the development of explicit value assessment systems and minimize their dependence on ERP over the longer-term. In this context, HTA will rely on local assessment of the evidence.

2. ON THE ESTIMATION OF TAX-SPEND DEBATE IN ALGERIA: EVIDENCE FROM NARDL MODELLING AND ASYMMETRIC CAUSALITY TEST

Abstract: Since its independence, the Algerian economy has dealt with many crises, especially those related to the global oil markets, which are witnessing severe fluctuations, and since Algeria is a rentier economy these crises have threaten its fiscal policy in the last four decades. In such cases, policy-makers have to think wisely about any possible negative effects that may impact the economy through fixing the issue of the adjustment to both spending and revenues. Previous literature assumes a symmetric linkage between government spending and revenue. However, a variation on public revenue does not necessarily have the same impact on public expenditure, or vice-versa. Considering this possible asymmetry between the variables, the main purpose of this study is to examine the asymmetric relationship between government expenditures and revenues in Algeria over the period 1970-2018, and to confirm one of the four hypotheses linked to the tax-spend debate. To characterize asymmetry, this study uses the nonlinear autoregressive distributed lags (NARDL) approach of cointegration developed by Shin et al (2014) between these two variables in the adjustment process of the budgetary. Furthermore, we applied the asymmetric causality approach test using bootstrap simulations with leverage adjustments popularized by Hatemi-J, 2012; Hacker and Hatemi-J, 2012 to determine the causal relationship between variables. The findings provide evidence that suggests that there is a long-run relationship between government expenditures and revenues. To put it differently, in the long run, positives changes in revenue dominate the response of government expenditure. The same applies to the response of spending since positive changes in government expenditure are greater than negative changes. Asymmetric causality results supported a bidirectional causality pattern between public expenditures and revenues in Algeria, thus it supports the fiscal synchronization hypothesis in Algeria. The results of the research are important to increase interest in fiscal policy in Algeria, where under this hypothesis, the fiscal authorities of Algeria must try to raise revenues and work to rationalize public spending simultaneously to control the budget deficit. 3. La résilience du personnel soignant à l'épreuve de la pandémie de COVID-19: une étude dans un hôpital public en Tunisie

Abstract: Ce travail cherche à appréhender les déterminants de la résilience du personnel soignant dans le contexte de la crise sanitaire Covid-19. Son but est d'accéder à une compréhension profonde des motifs, des forces et des processus à l'oeuvre dans la dynamique complexe de la résilience. Les résultats de l'étude menée auprès du personnel soignant d'un hôpital public en Tunisie ont révélé que la résilience résulte de l'activation de prédispositions, de facteurs de protection et de ressources autant personnelles qu'interpersonnelles et socioculturelles. L'étude souligne que la résilience génère une réelle valeur ajoutée pour les patients, les soignants et la pratique des soins aussi bien sur le plan humain, sanitaire, éthique qu'économique. Plus particulièrement, dans le contexte pandémique actuel, le personnel soignant semble plus que jamais exposer aux risques psychosociaux et à un quotidien professionnel inédit et jonché de tensions. Les résultats de l'étude menée auprès du personnel soignant d'un hôpital public en Tunisie ont révélé que la résilience résulte de l'activation de prédispositions, de facteurs de protection et de ressources autant personnelles qu'interpersonnelles et socioculturelles. Cette recherche a révélé que la résilience n'est pas une réaction spontanée dans un contexte professionnel marqué par l'adversité et l'incertitude. Elle est le fruit d'un mix mettant en jeu les dispositions personnelles, la dynamique de groupe, le soutien social, la stabilité familiale, le style de leadership et les contingences situationnelles. De même, l'étude souligne que la résilience génère une réelle valeur ajoutée pour les patients, les soignants et la pratique des soins aussi bien sur le plan humain, sanitaire, éthique qu'économique. elle montre que le développement de la résilience constitue une responsabilité partagée entre le personnel soignant, le système hospitalier, les autorités sanitaires, l'encadrement et les dirigeants des établissements de santé. Through this study, we sought to understand the determinants of the resilience of healthcare staff during the COVID-19 health crisis. Its goal was to gain a deep understanding of the motives, forces and processes at work in the complex dynamics of resilience. It seems all the more crucial to gain such understanding, given that there seem to be lessons to be learned for a rethinking of working conditions in public hospitals. in particular, during the current pandemic, the healthcare staff seem more than ever exposed to psychosocial risks and to a working life that has never been so stressful. The study was carried out among the healthcare staff at a public hospital in Uinisia, and its findings show that resilience results from the activation of predispositions, protective factors and resources that are as much personal as they are interpersonal and sociocultural. resilience is not a spontaneous response in a work context marked by adversity and uncertainty. it results from a mix involving personal dispositions, group dynamics, social support, family stability, leadership style and situational contingencies. Our findings further show that resilience generates real added value for patients, for caregivers and for healthcare practice as much in human, healthcare and ethical terms as in economic terms. Developing resilience is a responsibility to be shared between healthcare staff, the hospital system, health authorities, managers and leaders of healthcare institutions. 4. Le downsizing en temps de COVID-19: la difficile conciliation entre le social et l'économique. Le cas des survivants d'un établissement hôtelier en Tunisie

Abstract: Lobjectif de cette recherche est de comprendre comment une entreprise peut mettre en place une stratégie de downsizing socialement responsable en temps de COVID-19. La visée de cette étude est de proposer une analyse fine des formes du downsizing en mettant l'accent sur son caractère distinctif dans les pays du Sud. Les résultats de l'étude qualitative, de nature exploratoire, menée au sein d'un établissement hôtelier en Tunisie mettent en exergue une série de mesures socialement responsables. Parmi ces pratiques à forte valeur ajoutée, figurent notamment la responsabilisation, la participation, l'accompagnement de proximité, la communication, la formation professionnelle, l'implication du syndicat et le respect de la législation sociale. Les conclusions soulignent que la mise en oeuvre d'un ensemble de préalables et de dispositifs avant, au cours et après le downsizing permet de limiter ses dégâts psychologiques et ses effets pervers. Aussi, elles révèlent que le déploiement de pratiques managériales bienveillantes, équitables et respectueuses de la dignité humaine, ainsi que l'adoption d'un agir éthique rendent le downsizing socialement toléré. Il est clair qu'il n'y a pas un modèle typique ou un << one best way >> en matière de downsizing. Le recours à une approche ancrée dans les particularismes locaux est plus pertinent qu'un raisonnement en termes de meilleures pratiques universellement applicables. Le downsizing socialement responsable permet de concilier la rationalité psychosociale et la rationalité économique. Toutefois, il exige des concessions, des compromis parfois difficiles et s'appuie sur des équilibres précaires et des arrangements négociés. L'étude souligne qu'il est difficile d'enrayer les conséquences négatives du downsizing sur le plan humain, mais qu'il est nécessaire d'agir adéquatement et justement en faisant preuve de bienveillance et de responsabilité organisationnelle. L'objectif de cette recherche est de comprendre comment l'entreprise peut mettre en place une stratégie de downsizing socialement responsable en temps de COVID-19. Les résultats de l'étude qualitative, de nature exploratoire, menée au sein d'un établissement hôtelier en Tunisie révèlent une série de pratiques socialement responsables qui confère un caractère distinctif à la stratégie de downsizing déployée. Les conclusions soulignent que la mise en oeuvre d'un ensemble de mesures avant, au cours et après le downsizing permet de limiter ses dégâts psychologiques et ses effets pervers. Aussi, elles révèlent que la mobilisation de pratiques managériales bienveillantes, équitables et respectueuses de la dignité humaine, ainsi que l'adoption d'un agir éthique et juste rendent le downsizing socialement toléré. The aim of this study is to provide a detailed analysis of downsizing forms, emphasizing its distinctive character in southern countries. The results of our qualitative protocol, of an exploratory nature, carried out in a Tunisian hotel, revealed a mix of socially responsible measures. These high added-value practices include accountability, participation, local support, communication, professional training, union involvement and compliance with social legislation. The conclusions underlined that the implementation of a set of prerequisites and mechanisms before, during and after downsizing makes it possible to limit its psychological damage and its perverse effects. Also, they revealed that the deployment of benevolent managerial practices, fair and respectful of human dignity, as well as the adoption of an ethical guidelines have made downsizing socially tolerated. Clearly there is not a typical model or 'one best way' when it comes to downsizing. The deployment of an approach rooted in local particularities is more relevant than reasoning in terms of universally applicable best practices. Socially responsible downsizing makes it possible to reconcile psychosocial rationality and economic rationality. However, it requires concessions, difficult compromises, and is based on precarious balances and negotiated arrangements. The study underscored that it is difficult to prevent the negative human consequences of downsizing, but that it is necessary to do things properly and fairly with kindness and organizational responsibility. Abstract: Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). Abstract: Aims: Opioid-reduced anesthesia (ORA) was suggested to decrease morphine consumption after adolescent idiopathic scoliosis (AIS) surgery and incidence of chronic pain. Materials & methods: A prospective analysis using the ORA in AIS surgery was performed. Two cohorts were compared: a control group (opioid-based anesthesia) and the ORA group. The main outcome was morphine consumption at day 1. Results: 33 patients operated for AIS using ORA were compared with 36 with opioid-based anesthesia. Morphine consumption was decreased in the ORA group (1.1 mg.kg-1 [0.2–2] vs 0.8 mg.kg-1 [0.3–2]; p = 0.02) at day 1. Persistent neuropathic pain at 1 year was decreased in the ORA group (p = 0.02). Conclusion: The ORA protocol is efficient to reduce postoperative morphine consumption in AIS surgery and preventing neuropathic pain.

Abstract: Isoniazid (INH), being the first-line drug used as an anti-tuberculosis drug, is known to be associated with physiological deteriorations including hepatic and neurologic disturbances. This study was aimed at biochemical and behavioral characterization of toxic manifestations of isoniazid treatment in Wistar rats. Experimental animals were divided into four groups. Each group consists of six animals including the control group (saline solution), I25 group (25 mg/kg of INH), I50 group (50 mg/kg of INH), and I100 group (100 mg/kg of INH). Animals received daily INH for 30 days. Isoniazid is known to be associated with hepatotoxicity; it's among the most common causes of drug-induced toxicities. For this reason assays for aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), and lactate dehydrogenase (LDH) were performed to assess liver toxicity. Moreover, behavioral study, renal, and lipid parameters were also assessed in addition to histological features of the liver and brain. Significant differences in all studied parameters were seen especially in the I100 group and a marked increase in liver enzymes activities, such as AST and ALT was observed. In another hand, there were no major clinical signs in treated animals, except fatigue and anxiety in the I100 group. On the other hand, the histological findings showed potential liver and brain injury which was evidenced by degenerative changes, infiltration, and hepatocyte necrosis, in addition to the appearance of many pyramidales cells in the gyrus. The current study findings suggest that INH interacts with multiple biochemical pathways in the body what comes up by behavioral changes and liver disturbances in animals caused by INH toxicity.

Abstract: International audience; Due to the multi-path propagation and extreme sensitivity to minor changes in the propagation medium, the coda waves open new fascinating possibilities in the non-destructive evaluation and acoustic imaging. However, their noise-like structure and high spurious sensitivity for ambient conditions (temperature, humidity, and others) make it challenging to perform localized inspection in the overall coda wave evolution. While existing deterministic solutions reach their limit in handling complex data, emerging techniques such as deep learning-based algorithms have shown a promising potential to overcome these limitations. This paper proposes a deep neural network that paves the way to make the complex features of coda waves more handleable to reliably exploit coda waves in several applications even in a changing or unstable environment. 10. Determinants of the Digital Divide among French Higher Education Teachers

Abstract: The widespread use of technology in daily life, and particularly in education in higher education institutions has devoted growing attention to the nature of ICT usages by Higher Education Teachers which has seen as an increasingly important factor for the successful integration of these technologies. This study aims to analyze the determining factors of the various uses of ICT by teachers in the university environment and to characterize their variety and intensity. For this end, we conducted a survey of a sample of 2,079 teachers from public universities in France. Our approach consisted in measuring the intensity of use of ICT in academia in order to appreciate the resulting digital divides between different groups of teachers. Multinomial logistic regression shows that the differences in the use of ICT are linked to the differences in initial digital skills between teachers. Furthermore, the training in ICT, age, gender and social context appear to have a manifold influence on ICT use. Our results clearly confirm the existence of digital divides, it prompts us to analyze more precisely the role of innovative users and that of first-time adopters when they appear to be actors involved in the diffusion of ICT within universities.

Abstract: Immunoglobulin G4-related disease (IgG4-RD) is a fibroinflammatory condition of unknown etiology, with presumed autoimmune mechanisms. It is characterized by high levels of IgG4 and variable clinical manifestations. It can involve one or multiple organs. Herein, we reported the case of a 62-year-old man with three organs involvement. He initially presented with recurrent jaundice. Laboratory analysis revealed cholestasis, high gamma-globulin levels, renal failure, and proteinuria. Abdominal Magnetic Resonance Imaging (MRI) showed segmental strictures of the left intrahepatic bile ducts and the wirsung duct with an increased volume of the pancreas and diffuse bilateral enlargement of the kidneys. Laboratory tests revealed high IgG4 levels (770 mg/dL). Based on the biological and radiological findings, we have suggested the diagnosis of systemic IgG4-related disease involving bile ducts, the pancreas, and probably the kidneys. Renal biopsy revealed lymphoplasmacytic infiltrate and fibrosis, but no IgG4-positive cell. The patient received corticosteroid therapy with a complete resolution of all symptoms and a rapid normalization of all blood tests. The present case underlines the complexity of IgG4-RD because of its variable clinical presentation. The diagnosis is challenging and should be carefully assessed for possible multi-organ involvement.

Abstract: The development of future wireless access networks often results in very high energy consumption. To reduce this consumption, decision-makers (DM) minimize the number of base stations ( $$\hbox {BS}_s$$ ) installed while using a dynamic BS on/off strategy. However, reducing the number of base stations leads to insufficient network coverage. Indeed, for better coverage, the decision-maker (DM) should install enough base stations. We can therefore see that we have two contradictory objectives. On the other hand, we can easily notice that the information of the network traffic evolves over time. Therefore and in order to make a realistic study, we will consider the traffic information as an imprecise and uncertain value instead of a constant value. For the reasons aforementioned, we introduce in this paper, a fuzzy multi-objective mathematical model in which each traffic is a fuzzy variable, and then, we present a decision-making model based on possibility theory. To solve this problem, we used two meta-heuristic algorithms. The obtained results proved the efficiency of our model compared to previous studies. Indeed, the proposed methodology results not only in a reduction of $$\hbox {CO}_2$$ emissions (between 18.15 and 24.18%) but also guarantees good network coverage.

Abstract: International audience; The study of coverage problem in uncertain WSN environment requires the consideration of this uncertainty by taking the best possible decisions, since it is impossible to explicitly represent all the combinatorics to produce a conditional active/passive state nodes' planning in the area of interest, and allow reasoning on various environmental states of the partially known physical world. This paper addresses the problem of area coverage based on the Dempster-Shafer theory. The authors aim to ensure the full area coverage while using a subset of connected nodes, with minimal costs using a minimal number of dominant nodes regardless of the type of used deployment (random or deterministic). This is ensured by activating a single node in each subset of each geographic sub-area, thus extending the lifetime of the wireless sensor network to its maximum. The comparison of the proposed model denoted evidential approach for area coverage (EAAC) with two well-known protocols and with a recent one showed a better performance and a slight improvement in the covered area.

Abstract: International audience; In 2017, the Food and Drug Administration published a safety recommendation to limit the exposure to general anesthesia as much as possible below the age of three. Indeed, several preclinical and clinical studies have questioned the possible toxicity of general anesthesia on the developing brain. Since then, recent clinical studies tried to mitigate this alarming issue. What is true, what is false? Contrary to some perceptions, the debate is not over yet. Only stronger translational research will allow scientists to provide concrete answers to this public health issue. In this review, we will provide and discuss the more recent data in this field, including the point of view of preclinical researchers, neuropsychologists and pediatric anesthesiologists. Through translational research, preclinical researchers have more than ever a role to play to better understand and identify long-term effects of general anesthesia for pediatric surgery on brain development in order to minimize it.

Abstract: BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12��g��dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (��week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g��dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g��dl-1 in week 2 and 8.0 [7.3 to 9.0] g��dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml��kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g��dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. Abstract: This paper proposes an efficient high‐order finite impulse response (FIR) filter structure for field programmable gate array (FPGA)‐based applications with simultaneous digital signal processing (DSP) and look‐up‐table (LUT) reduced utilization. The real‐time updating of the filter coefficients is also put into perspective. In order to perform these objectives, both the speed and the structure of FPGA are efficiently exploited. The gap between the required input sampling frequency and the FPGA allowed maximum frequency is managed to achieve additional computing sequences. Furthermore, the special structures of the FPGA Look‐up‐table Shift‐Register (LUT‐SR) and their internal connections are fully employed for pipelining and selecting the input samples. The FPGA Block RAMs (BRAMs) are employed for handling the reconfigurable filter coefficients, and the FPGA DSP slices are associated for computing the output data of the BRAMs and the multiplexers. To synchronize the BRAM unit addressing with the LUT multiplexer selection, a single unit is used for simultaneous control. The obtained results show that the proposed reconfigurable 16‐tap FIR filter offers reductions of 79.3% and 74.4% of slice utilization over the hybrid variable size partitioning (VP‐Hybrid) based structure and the Radix‐2r based structure, respectively when implemented on a Xilinx Spartan‐6 XC6SLX45 FPGA. Moreover, an improvement of efficiency is achieved compared to all reputed FPGA‐based architectures.

Abstract: Background During inhalational induction of anaesthesia for children, severe respiratory events can occur but can be rapidly treated once intravenous access is in place. Reducing the time to successful cannulation during inhalational induction for children with poor vein visibility would improve safety. Objective To study the effectiveness of a near-infrared (NIR) vascular imaging device (Veinviewer) to facilitate intravenous cannulation. Design A prospective, multicentre, randomised, open clinical trial. Setting The operating rooms of three paediatric hospitals in Paris, France, from 1 October 2012 to 31 March 2016. Patients Children up to the age of 7 years, with poor vein visibility requiring general anaesthesia. Intervention Inhalational anaesthesia was initiated and intravenous cannulation was performed with the standard approach or with the Veinviewer Vision. Main outcome measures The primary outcome was the time to successful intravenous cannulation. A secondary outcome was the proportion of successful first attempts. Results The mean time to successful intravenous cannulation was 200 (95% CI, 143 to 295) seconds in the Veinviewer and 252 (95% CI, 194 to 328) seconds for the control group: hazard ratio 1.28 (1.02 to 1.60) (P = 0.03). The adjusted hazard ratio for known predictive factors was 1.25 (0.99 to 1.56) (P = 0.06). Success at the first attempt was 64.6% (102/158) in the 'Veinviewer' group vs. 55.6% (85/153) in the 'control' group (P = 0.10). Conclusion The Veinviewer has limited value in reducing the time to successful intravenous cannulation during inhalational anaesthesia for young children with poor vein visibility. However, there is a strong trend to reducing the delay in some cases and, given its absence of side effects, it could be part of a rescue option for a difficult venous-access strategy. Abstract: Lean manufacturing accomplishments have led researchers and practitioners to consider extending lean to different engineering enterprise parts, including product and process development. However, in the early phases of product development, it is important to both reduce costs and improve a product's sustainability performance. This paper develops a framework to guide designers and manufacturing managers to design products that better meet customer needs and requirements for sustainable products and services. To achieve this, a critical review of the existing literature in the field of lean design, and eco-design was conducted to fill the research gap and then develop the proposed methodological framework. The integration aimed to create synergies to extend the integration benefits and promote rational use of resources and time to create value for customers by eliminating waste and unnecessary actions in the product development process. The main objective is to ensure the development of products with minimal negative impacts on the natural environment. To our knowledge, this is the first time in the literature that an integrated framework is bringing together lean design and eco-design principles, practices, guidelines, drivers, barriers, and success factors. The framework can potentially serve product designers, manufacturing engineers, sales/marketing personnel, in fact, anyone who has a vested interest in designing environmentally friendly products.

Abstract: Background Little evidence is available regarding work-related quality of life (WRQoL) for anesthesiologists. We aimed to explore factors associated with WRQoL among French anesthesiologists. Methods The study surveyed French anesthesiologists qualified for more than 2 years. The primary objective was the determination of factors associated with WRQoL. Factors analyzed included demographic characteristics, lifestyle, financial status, personality traits, professional relations, management and organization, and occupational tasks when at work. Statistical analyses were performed using a multivariable quantile regression model. Results Overall, 2040 anesthesiologists responded to the survey and 1922 responses were analyzed. The latter represents 19% of practicing French anesthesiologists. The following factors were independently associated with increased WRQoL: family income, long-term employment, organizational and managerial factors (lesser weekly workload, the belief of providing high quality, safe health care services, team management, and operating theatre organization), human relations (satisfaction with workplace ambiance and relations with hospital management and colleagues), and occupational tasks (participation in team activities). Three personality traits were found to be significantly associated with increased WRQoL: extraversion, conscientiousness, and openness. Neuroticism was associated with reduced WRQoL. Conclusions The current study demonstrates exogenous and endogenous factors associated with increased WRQoL in anesthesiologists. Results should be considered as explorative and provide hypotheses for further research in this domain.

Abstract: The main aim of this paper is to investigate the effect of the informal economy (IE) on foreign direct investment (FDI) in a sample of petroleum producing countries (Algeria, Norway, the Russian Federation, Saudi Arabia and United States) based on data covering the period of 1991–2018 and using the Non-linear Autoregressive Distribution Lag (NARDL) model. The NARDL model was built separately for each country in the study sample. The main finding of this study is the impact of IE size on FDI inflows in all of the countries in the study sample, even if they are all producing and exporting countries. The empirical results lead to distinguish between two sub-groups. The first sub-group consists of countries whose FDI inflows have been positively affected by positive and negative shocks in the IE. These countries are characterised by a high share of natural resources in their GDP. The second sub-group consists of countries whose inward FDI has been positively affected by negative shocks in the IE and negatively affected by the positive ones. The most common feature of this subgroup is the relative independence of economics from natural resources.

Abstract: Groundwater is often the preferred source for human consumption, irrigation and industry. In Algeria, many people count on well water for their drinking water supply and irrigation of agricultural lands; especially in rural areas, which is a strategic reserve of which quality must be assured. Groundwater protection requires the detection of origins of pollution and its consequences on the ecological system. The region of Bouirahas seen water-borne diseases occurrence caused by groundwater pollution, which has led us to look for the origin of this problem. It is considered like a high-risk area because it has recorded a large number of Toxi-food collective, Viral Hepatitis A, typhoid fever and Choléra cases in 2018. The study carried out based on the potential risks to human health and the GIS for a period of eight years (from 2008 to 2015), has allowed us to the elaboration of water-borne disease mapping of the whole region and to find that the most affected municipalitie sare: Bouira, Sour El Ghozlene, Lakhdaria and Ain Bessem .The correlation between these diseases and the irrigated perimeters by wadis is very strong in this municipalities.

We employ a conditional mixed-process approach and the results of a residential survey in Kosovo to investigate whether e-skills boost e-services in hospitality. Our findings suggest that e-skills are important to explain different behaviors related to use of e-services. The facilitating conditions, expected performance, social influences and perceived value of the services have a positive impact while perceived risks to privacy have a negative impact on behavioral intention. We also find that gender and income have a positive effect on both behavioral use and behavioral intention while age has no effect on either dimension.

Abstract: International audience; Introduction: Paediatric anaesthesia requires specific theoretical knowledge and practical training. Non-technical skills and psychological factors might influence learning and practice. The aim of this study was to assess personality type and decision-making styles of paediatric anaesthesiology residents during the management of simulated intraoperative life-threatening cases.Method: Residents in anaesthesiology (between 4 and 5 years of training) participated in a simulated hypoxic cardiac arrest in the operating theatre. Their performance was evaluated using a score derived from international recommended management algorithm. They were asked to answer self-assessment questionnaires regarding both their personality (the five personality factors) and their decision-making style. Correlations between performance and personality were investigated.Results: Thirty-eight residents participated in the simulation session and 36 accepted to answer the questionnaires. Good management scoring was positively correlated with agreeableness and conscientiousness personality traits but was negatively correlated with avoidance and spontaneous decision-making styles.Discussion: The current study identified personality traits and decision-making styles that might influence the management of critical situations during paediatric anaesthesia. The proper identification of these factors might allow targeted personalised training to improve knowledge mobilisation and translation in the clinical context.

Abstract: International audience; This paper covers the study of high-frequency (~ 1 GHz) ZnO piezoelectric transducer integrated on a silicon substrate able to generate both compressional and shear acoustic waves. First, to promote the longitudinal mode, an electrical matching of the transducer in this high-frequency range is effectuated. Second, to promote shear waves, new deposition conditions were applied, giving thin zinc oxide films of inclined c-axis. The RF microprobe was used to validate the transducer design and to conduct the viscosity measurements. Thus, the shear and the volume viscosity of a water droplet were measured.

Abstract: Objective Current understanding of the neuromodulatory effects of deep brain stimulation (DBS) on large-scale brain networks remains elusive, largely due to the lack of techniques that can reveal DBS-induced activity at the whole-brain level. Using a novel 3T magnetic resonance imaging (MRI)-compatible stimulator, we investigated whole-brain effects of subthalamic nucleus (STN) stimulation in patients with Parkinson disease. Methods Fourteen patients received STN-DBS treatment and participated in a block-design functional MRI (fMRI) experiment, wherein stimulations were delivered during "ON" blocks interleaved with "OFF" blocks. fMRI responses to low-frequency (60Hz) and high-frequency(130Hz) STN-DBS were measured 1, 3, 6, and 12 months postsurgery. To ensure reliability, multiple runs (48 minutes) of fMRI data were acquired at each postsurgical visit. Presurgical resting-state fMRI (30 minutes) data were also acquired. Results Two neurocircuits showed highly replicable, but distinct responses to STN-DBS. A circuit involving the globus pallidus internus (GPi), thalamus, and deep cerebellar nuclei was significantly activated, whereas another circuit involving the primary motor cortex (M1), putamen, and cerebellum showed DBS-induced deactivation. These 2 circuits were dissociable in terms of their DBS-induced responses and resting-state functional connectivity. The GPi circuit was frequency-dependent, selectively responding to high-frequency stimulation, whereas the M1 circuit was responsive in a time-dependent manner, showing enhanced deactivation over time. Finally, activation of the GPi circuit was associated with overall motor improvement, whereas M1 circuit deactivation was related to reduced bradykinesia. Interpretation Concurrent DBS-fMRI using 3T revealed 2 distinct circuits that responded differentially to STN-DBS and were related to divergent symptoms, a finding that may provide novel insights into the neural mechanisms underlying DBS. Abstract: Signal loss in blood oxygen level-dependent (BOLD) functional neuroimaging is common and can lead to misinterpretation of findings. Here, we reconstructed compromised fMRI signal using deep machine learning. We trained a model to learn principles governing BOLD activity in one dataset and reconstruct artificially compromised regions in an independent dataset, frame by frame. Intriguingly, BOLD time series extracted from reconstructed frames are correlated with the original time series, even though the frames do not independently carry any temporal information. Moreover, reconstructed functional connectivity maps exhibit good correspondence with the original connectivity maps, indicating that the model recovers functional relationships among brain regions. We replicated this result in two healthy datasets and in patients whose scans suffered signal loss due to intracortical electrodes. Critically, the reconstructions capture individual-specific information. Deep machine learning thus presents a unique opportunity to reconstruct compromised BOLD signal while capturing features of an individual’s own functional brain organization., Signal loss in blood oxygen level‐dependent (BOLD) fMRI can lead to misinterpretation of findings. The authors trained a deep learning model to reconstruct compromised BOLD signal in datasets from healthy participants and in patients whose scans suffered signal loss due to intracortical electrodes.

Abstract: Purpose [18F]-2-Fluoro-2-deoxy-D-glucose PET/CT (FDG PET/CT) is a sensitive and quantitative technic for detecting inflammatory process. Glucose uptake is correlated with an increased anaerobic glycolysis seen in activated inflammatory cells such as monocytes, lymphocytes, and granulocytes. The aim of the study was to assess the inflammatory status at the presumed peak of the inflammatory phase in non-critically ill patients requiring admission for COVID-19. Methods Patients admitted with COVID-19 were prospectively enrolled. FDG PET/CT was performed from day 6 to day 14 of the onset of symptoms. Depending on FDG PET/CT findings, patients’ profiles were classified as “inflammatory” or “low inflammatory.” FDG PET/CT data were compared with chest CT evolution and short-term clinical outcome. All inflammatory sites were reported to screen potential extra-pulmonary tropism. Results Thirteen patients were included. Maximum standardized uptake values ranged from 4.7 to 16.3 in lungs. All patients demonstrated increased mediastinal lymph nodes glucose uptake. Three patients (23%) presented mild nasopharyngeal, two patients (15%) bone marrow, and five patients (38%) splenic mild increase in glucose uptake. No patient had significant digestive focal or segmental glucose uptake. There was no significant physiological myocardial glucose uptake in all patients except one. There was no correlation between PET lung inflammatory status and chest CT evolution or short-term clinical outcome. Conclusion Inflammatory process at the presumed peak of the inflammatory phase in COVID-19 patients is obvious in FDG PET/CT scans. Glucose uptake is heterogeneous and typically focused on lungs. Abstract: Stomach cancer is a major global health problem; it is one of the ten most common cancers with poor survival, and its incidence is characterized by wide variation. The aim of this work is to carry out a retrospective epidemiological study on gastric cancer in the wilaya of Tlemcen (West Algeria) over a period of 5 years (2011–2015). The data set was provided by the cancer registry of the Tlemcen wilaya. The statistical analysis was performed using software SPSS.21. During this period, 394 cases of gastric cancers were collected, including 199 patients with gastric adenocarcinoma (50.50%); gastric cancer was ranked in 5th rank of the ten most answered cancers in the wilaya. The average age of the patients was 60.5 ± 14.208 years (60.618 ± 13.556 men, 56.654 ± 14.8761 women) with an extremity ranging from 18 to 91 years. A predominance of men has been observed with 60.4% against 39.6% of women, with a sex ratio of 1.5 and a significant difference between the two sexes (p = 0.08). Patients over 50 years of age represented the predominant age group (73.4%). The tumor topography was antropyloric in 6.09% of the cases, and the vast majority of the diagnosed cases was in the local stage (17.01%). Stomach cancer remains one of the top ten cancers in the Tlemcen willaya, so extensive research on the risk factors for gastric cancer remains important to plan effective preventive and curative strategies to reduce this burden.

Abstract: Objectives The world is currently facing an unprecedented healthcare crisis caused by the COVID-19 pandemic. The objective of these guidelines is to produce a framework to facilitate the partial and gradual resumption of intervention activity in the context of the COVID-19 pandemic. Methods The group has endeavoured to produce a minimum number of recommendations to highlight the strengths to be retained in the 7 predefined areas: (1) protection of staff and patients; (2) benefit/risk and patient information; (3) preoperative assessment and decision on intervention; (4) modalities of the preanaesthesia consultation; (5) specificity of anaesthesia and analgesia; (6) dedicated circuits and (7) containment exit type of interventions. Results The SFAR Guideline panel provides 51 statements on anaesthesia management in the context of COVID-19 pandemic. After one round of discussion and various amendments, a strong agreement was reached for 100% of the recommendations and algorithms. Conclusion We present suggestions for how the risk of transmission by and to anaesthetists can be minimised and how personal protective equipment policies relate to COVID-19 pandemic context.

Abstract: Resume Problematique L’analyse de la fonction ventriculaire droite au cours de la phase aigue de l’embolie pulmonaire (EP) a ete largement etudiee dans la litterature. Toutefois, l’etude des proprietes fonctionnelles ventriculaires droites a distance de cet episode n’a ete qu’exceptionnellement rapportee et ceci malgre l’importance de la morbi-mortalite de l’EP. But du travail Evaluer la fonction ventriculaire droite apres un premier episode d’EP. Methodes Dans notre etude, nous avons compare les parametres echocardiographiques de la fonction ventriculaire droite (VD) chez 25 patients ayant eu un premier episode d’EP non grave datant de plus de 6 mois avec ceux de 25 sujets temoins sains. Resultats Dans l’etude de la fonction VD, nous avons note que les valeurs moyennes des parametres standards etaient significativement plus basses dans le groupe EP par rapport au groupe temoin mais leurs valeurs sont restees dans les limites de la normale. La valeur moyenne du strain global VD etait significativement plus basse chez les patients par rapport aux temoins (−21 ± 4,8 vs −25 ± 2,4 % ; p Conclusion Notre etude a suggere l’existence d’une dysfonction systolique VD a distance d’un premier episode d’EP et ceci malgre l’absence de symptomes fonctionnels et d’HTAP.

Abstract: Global Positioning System (GPS) navigation devices and applications have become ubiquitous over the last decade. However, it is unclear whether using GPS affects our own internal navigation system, or spatial memory, which critically relies on the hippocampus. We assessed the lifetime GPS experience of 50 regular drivers as well as various facets of spatial memory, including spatial memory strategy use, cognitive mapping, and landmark encoding using virtual navigation tasks. We first present cross-sectional results that show that people with greater lifetime GPS experience have worse spatial memory during self-guided navigation, i.e. when they are required to navigate without GPS. In a follow-up session, 13 participants were retested three years after initial testing. Although the longitudinal sample was small, we observed an important effect of GPS use over time, whereby greater GPS use since initial testing was associated with a steeper decline in hippocampal-dependent spatial memory. Importantly, we found that those who used GPS more did not do so because they felt they had a poor sense of direction, suggesting that extensive GPS use led to a decline in spatial memory rather than the other way around. These findings are significant in the context of society’s increasing reliance on GPS.

Abstract: The use of mechanically representative phantoms is important for experimental validation in ultrasound (US) imaging, elastography, and image registration. This article proposes a model to predict the elastic modulus of a soft tissue-mimicking phantom based on two very easily controllable parameters: gelatin concentration and refrigeration duration. The model has been validated on small- and large-scale phantoms; it provides a good prediction of the elastic modulus in both cases (error < 16.2%). The tissue-mimicking phantom is made following a low-cost and simple fabrication procedure using commercial household gelatin with psyllium hydrophilic mucilloid fiber to obtain echogenicity. A large range of elastic properties was obtained (15–100kPa) by adjusting the gelatin concentration between 5% and 20% (g/mL) and the refrigeration time of the sample between 2 and 168 h, allowing to mimic normal and pathological human soft tissues. The phantom’s acoustic properties (velocity, attenuation, and acoustic impedance) are also assessed using the American Institute of Ultrasound in Medicine (AIUM) standard.

On page 1174, in the second paragraph of the Statistical methods section, the second sentence should read as follows: The incidence of difficult intubation was determined including those whose tracheas were already intubated and is reported as a percentage with a 95% exact binomial CI. On page 1175, in the third paragraph of the Statistical methods section ‘mean standardised difference (MSD)’ should read ‘standardised mean difference (SMD)’ The authors would like to apologise for any inconvenience caused.

Abstract: Background: Neonates and infants requiring anaesthesia are at risk of physiological instability and complications, but triggers for peri-anaesthetic interventions and associations with subsequent outcome are unknown. Methods: This prospective, observational study recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. The primary aim was to identify thresholds of pre-determined physiological variables that triggered a medical intervention. The secondary aims were to evaluate morbidities, mortality at 30 and 90 days, or both, and associations with critical events. Results: Infants (n=5609) born at mean (standard deviation [SD]) 36.2 (4.4) weeks postmenstrual age (35.7% preterm) underwent 6542 procedures within 63 (48) days of birth. Abstract: International audience; The study examined the impact of different factors on greenhouse gas (GHG) emissions, by applying the extended STIRPAT model and decoupling analysis for Tunisia for the period 1990–2018. Furthermore, the study utilizes Tapio decoupling model, and the Auto-Regressive Distributed Lag (ARDL) bounds test approach to examine the relationship between the variables of greenhouse gas (GHG) emissions, economic growth, energy consumption, urbanization, innovation, and trade openness. The findings validated an inverted U-shape relationship between GDP and GHG emissions. In addition, we find that the consumption of renewable energy contributes to the reduction of GHG emissions in the long run. The findings call authority for the adaption of the regulatory framework relating to energy management, energy efficiency and the development of renewable energies, as well as to initiate energy market reforms, implement mitigation strategies and encourage investments in clean energies.

Abstract: This empirical study examines the asymmetric effects of oil price shocks on economic growth in Algeria. The dynamic models of Non-linear Autoregressive Distribution Lags (NARDL) are used to analyze this relationship throughout 1970-2018. The results of this study reveal that there is a non-linear connection among the variables in the long run. As the empirical results of the NARDL model estimation shows that the response of real GDP to positive oil shocks is greater than the negative shocks. It is also evident that positive shocks have a low effect on economic growth in Algeria. In the short term, the results show that the effect of oil shocks on economic growth is symmetric and very weak. While the results showed that government revenue has a greater impact on economic growth than capital expenditure. This reflects the urgent need to diversify sources of income to ensure its sustainability as a first stage before focusing on the government spending aspect.

Abstract: Background: Major depressive disorder (MDD) is a heterogeneous syndrome and can be conceptualized as a mixture of dimensional abnormalities across several specific brain circuits. The neural underpinnings of different symptom dimensions in MDD remain elusive. Here, we aimed to identify robust, generalizable brain-based biomarkers for different symptom dimensions in MDD, using individualized functional connectomes. Methods: Patterns of functional connectivity (FC) associated with symptom severity were identified using a novel individualized functional network parcellation analysis in conjunction with hierarchical clustering. Dimension-specific prediction models were trained to estimate symptom severity in first-episode medication-naive patients (Discovery dataset, N=95) and replicated in an independent Validation dataset (N=94). In a cohort of subjects who underwent antidepressant treatment (N=55), correlation between FC changes and symptom changes was further explored. Results: Two distinct symptom clusters previously identified in MDD patients, namely dysphoric and anxiosomatic symptoms, were robustly replicated in our data. A connectivity biomarker associated with dysphoric symptoms was identified, which mainly involved the default, dorsal attention, and limbic networks. Critically, this brain-symptom association was confirmed in the Validation dataset. Moreover, the marker also tracked with dysphoric symptom improvement following a two-week antidepressant treatment. For comparison, we repeated our analyses using a non-individualized approach and failed to identify replicable brain-symptom biomarkers. Further quantitative analysis indicated that generalizability of connectivity-symptom association was hampered when functional regions were not localized in individuals. Conclusions: This work reveals robust, replicable FC biomarkers for dysphoric symptoms in MDD, demonstrates the advantage of individual-oriented approach, and emphasizes the importance of generalizability in psychiatric neuroimaging analysis. Abstract: ObjectiveA meta-analysis of proton magnetic resonance spectroscopy studies to investigate alterations in brain metabolites in people with HIV (PWH), the relationship between metabolite alterations and combination antiretroviral therapy (cART), and the relationship between metabolite alterations and cognitive impairment.MethodsThe PubMed database was searched for studies published from 1997 to 2020. Twenty-seven studies were identified, which included 1255 PWH and 633 controls. Four metabolites (N-acetyl aspartate [NAA], myo-inositol [mI], choline [Cho], and glutamatergic metabolites [Glx]) from 5 brain regions (basal ganglia [BG], frontal gray and white matter [FGM and FWM], and parietal gray and white matter [PGM and PWM]) were pooled separately using random-effects meta-analysis.ResultsDuring early HIV infection, metabolite alterations were largely limited to the BG, including Cho elevation, a marker of inflammation. cART led to global mI and Cho normalization (i.e., less elevations), but improvement in NAA was negligible. In chronic PWH on cART, there were consistent NAA reductions across brain regions, along with Cho and mI elevations in the FWM and BG, and Glx elevations in the FWM. Cognitive impairment was associated with NAA reduction and to a lesser degree mI elevation.ConclusionsThe BG are the primary region affected during early infection. cART is successful in partially controlling neuroinflammation (global mI and Cho normalization). However, neuronal dysfunction (NAA reductions) and neuroinflammation (mI and Cho elevations) persist and contribute to cognitive impairment in chronic PWH. Novel compounds targeting NAA signal pathways, along with better neuroinflammation control, may help to reduce cognitive impairment in PWH.

Abstract: Malignant adenomyoepithelioma (AME) of the breast is an exceptionally rare form of breast cancer, with a significant metastatic potential. Chemotherapy has been used in the management of advanced AME patients, however the majority of treatments are not effective. Recent studies report recurrent mutations in the HRAS Q61 hotspot in small series of AMEs, but there are no preclinical or clinical data showing H-Ras protein as a potential therapeutic target in malignant AMEs. We performed targeted sequencing of tumours’ samples from new series of 13 AMEs, including 9 benign and 4 malignant forms. Samples from the breast tumour and the matched axillary metastasis of one malignant HRAS mutated AME were engrafted and two patient-derived xenografts (PDX) were established that reproduced the typical AME morphology. The metastasis-derived PDX was treated in vivo by different chemotherapies and a combination of MEK and BRAF inhibitors (trametinib and dabrafenib). All malignant AMEs presented a recurrent mutation in the HRAS G13R or G12S hotspot. Mutation of PIK3CA were found in both benign and malignant AMEs, while AKT1 mutations were restricted to benign AMEs. Treatment of the PDX by the MEK inhibitor trametinib, resulted in a marked anti-tumor activity, in contrast to the BRAF inhibitor and the different chemotherapies that were ineffective. Overall, these findings further expand on the genetic features of AMEs and suggest that patients carrying advanced HRAS-mutated AMEs could potentially be treated with MEK inhibitors.

Abstract: In this paper, we are interested in the modelling and simulation of imperfect systems in the DEVS context. We want to hybrid the DEVS formalism with artificial neural networks and to propose a new modelling and simulation approach which makes it possible to represent the behavior of imperfect systems. The problematic of our work is the integration into the DEVS formalism of tools from artificial intelligence allowing the representation, manipulation, and processing of imperfect data (imprecise, uncertain). NN-DEVS is a new hybrid approach which allows to extend the classic DEVS formalism. This new approach is effective in uncertain systems where the behavior of the system is stochastic. To validate the proposed NN-DEVS approach, we apply this approach to a complex reactive navigation system of a mobile robot.
