The article presents the results of domestic and international studies of the use of the fourth-generation fluoroquinolone moxifloxacin in the treatment of commu-nity-acquired pneumonia and pneumonia caused by SARS-CoV-2. Analysis of the clinical use of moxifloxacin in the treatment of complications of the respiratory tract infec-tions indicates its high efficiency and bioavailability, a low risk of resistance, which makes it possible to recommend it for widespread use in the work of a practicing doctor. Ge-neric drug Maxicin (Yuria-Pharm), available as a 20 mg/ml concentrate in a 20 ml vial, can significantly reduce the cost of treatment and optimize infusion therapy by choosing both volume and solvent.

Исследование проведено у 34 детей после выполнения оперативных вмешательств на органах брюшной полости. В зависимости от методики обезболивания пациенты были разделены на 2 группы: в 1-й группе использовали блокаду квадратной мышцы поясницы (quadratus lumborum block), во 2-й группе назначали только внутривенную непрерывную инфузию фентанила. Проведение многокомпонентной анальгезии (с использованием блокады квадратной мышцы поясницы) значительно уменьшает негативные последствия недостаточного обезболивания у детей и способствует эффективной анальгезии после травматических операций, уменьшает проявления синдрома интраабдоминальной гипертензии и улучшает кровоток в сосудах брюшной полости.

Актуальность. Блокада квадратной мышцы поясницы (qudratus lumborum block) находит широкое применение как компонент периоперационного обезболивания в абдоминальной хирургии у взрослых и мало используется в детской практике. Целью нашего исследования были изучение анатомических особенностей пространства квадратной мышцы поясницы и распределение местного анальгетика в пространстве в зависимости от введенной дозы, что позволит найти наиболее эффективную и безопасную дозу препаратов для обезболивания. Материалы и методы. Исследование проведено на 12 трупах лиц разных полов и возраста (20–73 года). Методом препарирования изучали топографоанатомические особенности мышц, фасциально-клетчаточных пространств, сосудов и нервов поясничной области. Выполнено послойное препарирование тканей поясничной области. Затем после визуализации целевых анатомических структур с помощью ультразвука в технике in-plane иглой 22 G выполнялось введение в передне-медиальном направлении в боковом положении по заднему краю квадратной мышцы поясницы (qudratus lumborum block) красителя метиленового синего для изу­чения распространения анестетика. Статистическую обработку полученных данных проводили с применением методов вариационной статистики с помощью программы Stаtistica 5.5 (принадлежит ЦНИТ ВНМУ им. Н.И. Пирогова, лицензионный № AXXR910A374605FA). Результаты. Исследовали площадь поверхности поперечной мышцы спины, которая колебалась в пределах 10,4–14,3 см2 и в среднем составляла 12,01 ± 2,30 см2. Исследование распределения раствора в пространстве квадратной мышцы поясницы в зависимости от дозы показало, что при применении дозы 0,2 мл/кг массы тела распределение раствора составляет в среднем всего 6,2 ± 2,1 см2, что не позволяет заполнить все пространство раствором, а при использовании дозы 0,3 мл/кг массы тела — 12,1 ± 1,4 см2, что вполне соответствует средней площади исследуемого пространства. Выводы. Изучение площади распределения раствора в пространстве квадратной мышцы поясницы позволяет предположить, что для обезболивания будет достаточно местного анестетика в дозе 0,3 мл/кг массы тела. Эта доза безопасна для использования как у взрослых, так и у детей. Клиническая эффективность требует дальнейшего изучения.

While writing the article, we analyzed all the studies related to ropivacaine for the first half of 2021, which were published in PubMed. Previous review articles on ropivacaine were also analyzed. Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12���g���dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (���week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g���dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g���dl-1 in week 2 and 8.0 [7.3 to 9.0] g���dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml���kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g���dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. One of the most complex problems of modern surgery is the treatment and prevention of anaerobic infection. Main place in overcoming of this infectious complication is devoted to urgent surgical intervention and correct early empiric antibacterial therapy. Special place in the combined empiric antibacterial treatment is devoted to 5-nitroimidazoles. Modern data about clinical efficacy of ornidazole in the treatment of surgical infectious complications is presented in the article. We performed a comparison of therapeutic activity and physical-chemical properties, pharmacokinetic and pharmacodynamic features of ornidazole with the first representative of 5-nitroimidazole class – metronidazole. Taking into account antiprotozoal and antianaerobic activity of ornidazole, it is recommended to be used in the schemes of combined therapy of severe generalized anaerobic infections and purulent processes of different localization; treatment and prevention of surgical infections after intraabdominal, thoracic, proctological and dental surgical interventions.

One of the leading cause of the decrease of treatment efficacy in patient with community-acquired infections in the intensive care department is a spread of antibiotic resistance in main causative agents. Among Gram-negative microorganisms Pseudomonas aeruginosa is one of the leading causative agents, that is related to health service. A review of foreign and domestic literature concerning tobramycin – systemic form use in the current medical practice in the conditions of increasing resistance of microorganisms to the most broad-spectrum antibiotics is presented in the article. Pharmacokinetic and pharmacodynamic characteristics of tobramycin are described, results of clinical trials, that demonstrate an efficacy of its use in the combination with other antibiotics in the life-threatening infections caused by Gram-negative microorganisms are presented in the article.

Background: ultrasound (US)-guided Quadratus Lumborum Block (QLB) is an effective technique in providing analgesia for abdominal surgery. This study was designed to evaluate the efficacy of a US-guided Quadratus Lumborum Block (QLB) compare it with a caudal block in children after acute appendicitis. Patients and methods. Forty eight (48) ASA 1–2, 1–5-year-old children scheduled for elective acute appendicites were studied. All patients received general anesthesia; sevoflurane was used for induction and maintenance of anesthesia and laryngeal mask airway (LMA) was used to secure the airway. After securing an intravenous cannula, patients were randomized to a US-guided Quadratus Lumborum Block (QLB) (n = 24) (group Q) using 0.5 ml/kg of 0.25 % bupivacaine, injected on the same side of surgery, and group C received a caudal block using 1 ml/kg of 0.2 % bupivacaine (n = 24). Surgery was allowed 15 min after administration of the block. Block failure was considered in case of gross movement or more than 20 % change in heart rate and/or ABP persisting more than 1 min after skin incision. Any adverse events were recorded. After surgery, patients remained for 4 h in the recovery room. Postoperative analgesia was evaluated using Children and Infants Postoperative Pain Scale (CHIPPS). An anesthesiologist, who was not part of the study team, evaluated the need for rescue analgesia in the intraoperative and postoperative period and a recovery nurse collected the data. If the CHIPPS score was greater than 4, a rescue analgesia of 20 mg/kg acetaminophen was administered. Results. No difference was found in hemodynamics in both groups. Also, intraoperative fentanyl consumption was not different and no rescue analgesia was required in the postanesthesia care unit. Conclusion. US-guided Quadratus Lumborum Block (QLB) is as effective as caudal block in providing immediate postoperative analgesia in acute appendicitis., Предпосылки. Dexmedetomidine is a fairly new and promising drug for use in intensive care and surgery. Due to the fact that it is an agonist of alpha-2-adrenoceptors, dexmedetomidine has an analgesic, sedative effect and affects hemodynamic parameters. Due to the fact that dexmedetomedin does not have the ability to suppress respiration, it is used in patients with respiratory disorders. Dexmedetomidine has minimal side effects and a wide range of uses.

To date, much research has focused on finding the ideal adjuvant for local anesthetics. Each of them can have different effects. In anesthesiology and pain medicine, depending on the purpose, one or another effect may be desirable. For example, in some situations it is desirable to accelerate the onset of action or prolong the duration of the block, in others the desired effect may be a “response to intravascular administration”, it should also be borne in mind that not all adjuvants are allowed for intrathecal administration., На сегодня большое количество исследований направлено на поиски идеальной добавки к местным анестетикам. Каждый из них может обладать различными эффектами. В анестезиологии и медицине боли в зависимости от целей желанным может быть тот или иной эффект. К примеру, в одних ситуациях желательно ускоренное начало действия или удлиненная продолжительность блока, в других – желательной может быть “реакция на внутрисосудистое введение”; также следует помнить о том, что не все адъюванты разрешены для интратекального введения., На сьогодні велика кількість досліджень спрямована на пошуки ідеального ад’юванта місцевих анестетиків. Кожен з них може володіти різними ефектами. В анестезіології та медицині болю залежно від цілей бажаним може бути той чи інший ефект. До прикладу, в одних ситуаціях бажаним є прискорений початок дії чи подовжена тривалість блоку, в інших – бажаною може бути “реакція на внутрішньосудинне введення”; також слід пам’ятати про те, що не всі ад’юванти дозволені для інтратекального введення.

The experience of using dexmedetomedine for sedation with respiratory disorders of varying severity has been known for a long time. This article examined two clinical cases in which dexmedetomedine was used for connection and adaptation of patients to mechanical ventilation of lungs. The purpose of the work is evaluation of the effectiveness and depth of sedation with varying degrees of respiratory failure, as well as the effect of dexmedetomedine on respiration.

Pediatric Intensive Care Units (PICUs) provide multidisciplinary care to critically ill children and their families. Grief is present throughout the trajectory of illness and can peak around the time of death or non-death losses. The objective of this study was to assess how PICUs around the world implement grief and bereavement care (GBC) as part of an integrated model of care. This is a multicenter cross-sectional, prospective survey study. Questionnaires with multiple-choice and open-ended questions focusing on unit infrastructure, personnel, policies, limited patient data, and practices related to GBC for families and health care professionals (HCPs) were completed by on-site researchers, who were HCPs on the direct care of patients. PICU fulfillment of GBC goals was evaluated using a custom scoring based on indicators developed by the Initiative for Pediatric Palliative Care (IPPC). We compared average total and individual items fulfillment scores according to the respective country's World Bank income. Patient characteristics and details of unit infrastructure were also evaluated as potential predictors of total GBC fulfillment scores. Statistical analysis included multilevel generalized linear models (GLM) with a Gaussian distribution adjusted by child age/gender and clustering by center, using high income countries (HICs) as the comparative reference. Additionally, we applied principals of content analysis to analyze and summarize open-ended answers to contextualize qualitative data. The study included 34 PICUs from 18 countries: high-income countries (HICs): 32.4%, upper middle-income countries (UMICs): 44.1%, low middle-income and low-income countries (LMI/LICs): 23.5%. All groups reported some compliance with GBC goals; no group reported perfect fulfillment. We found statistically significant differences in GBC fulfillment scores between HICs and UMICs (specifically, HCP grief support), and between HICs and LMICs (specifically, family grief support and HCP grief support). PICUs world-wide provide some GBC, independent of income, but barriers include lack of financial support, time, and training, overall unit culture, presence of a palliative care consultation service, and varying cultural perceptions of child death. Disparities in GBC for families and HCPs exist and were related to the native countries' income level. Identifying barriers to support families and HCPs, can lead to opportunities of improving GBC in PICUs world-wide.

The adequate assessment and management of pain remains a challenging task in the Pediatric Intensive Care Unit (PICU). Our goal is to describe how pain is assessed and managed in PICUs around the world and to examine how human and material resources impact achievement of this goal. An international multicenter cross-sectional observational study was designed with the participation of 34 PICUs located in urban, suburban, and rural areas of 18 countries. We evaluated how PICUs around the world assessed and managed pain according to the Initiative for Pediatric Palliative Care recommendations, and how human and material resources impacted achievement of this goal. Data was collected for this study from 2016 to 2018 using questionnaires completed by medical doctors and nurses. In this paper, we focus on the indicators related to how pain is managed and assessed. The average achievement of the goal of pain relief across all centers was 72.2% (SD: 21.1). We found a statistically significant trend of more effective pain management scores, routine assessment, proper documentation, and involvement of pain management experts by increasing country income. While there are efforts being made worldwide to improve the knowledge in pain assessment and management, there is a lack of resources to do so appropriately in low-middle-income countries. There is a mismatch between the existing guidelines and policies, which are mainly designed in high income countries, and the resources available in lower resourced environments.

OBJECTIVE The aim: To perform microbiological investigation and analytic mathematic prediction of clinical isolates of S. aureus to aminoglycosides in patients with severe burns. PATIENTS AND METHODS Materials and methods: We analyzed resistance of 199 S. aureus strains to aminoglycosides (gentamicin, tobramycin and amikacin) and doxycycline from 435 patients treated in the regional hospital due to burns for the period from 2011-2017. RESULTS Results: We created predictive curves for the prediction of susceptibility of S. aureus strains to aminoglycosides and doxycycline based on the changes in S. aureus resistance during the years of observation and expressed in mathematic equations. Susceptibility of S. aureus to gentamicin was 42.86 % at the end of observation and will decline in future. Despite tobramycin was efficient against 72.86 % of strains in 2017, mathematic modeling indicates rapid decline in its efficacy in future. Efficacy of amikacin was dropping during the last years, but according to the equation it efficacy will increase over 60 % in 2018. S. aureus susceptibility to doxycycline was 65.38 % in 2017 and mathematic modeling indicates its gradual decline in the nearest future. CONCLUSION Conclusions: Predicitive values of S. aureus susceptibility indicates not sufficient efficacy of these drugs in patients with infectious complications of burns. Tendency of the slight decline of S. aureus susceptibility to doxycycline still indicates sufficient levels of its efficacy in the nearest future. This justify its use as a second-line therapy with the causative agent in patients with burns.

Objective The aim: Investigate the effect of 0.9% NaCl, HES 130, HAES-LX-5% and mannitol 15% on cerebral hemodynamics in patients with AIS. Patients and methods Materials and methods: The study included 100 patients with AIS. As the investigated solutions were used: isosmolar 0.9% NaCl, hyperosmolar mannitol 15%, colloid-isoosmolar HES 130, colloid-hyperosmolar HAES-LX-5%. The control group received only 0.9% NaCl compared: 0.9% NaCl+HES 130, 0.9% NaCl+HAES-LX-5%, 0.9% NaCl+mannitol 15%. Evaluation of cerebral hemodynamic (indexes of cerebral blood flow) was performed using doppler ultrasound of cerebral arteries. Results Results: The dynamics of specific volume velocity of blood flow per 100 grams of brain substance indicates that in the group of 0.9% NaCl and 0.9% NaCl+mannitol is the tendency to decrease the blood flow of the brain during 7 days of treatment, respectively: 2.8% and 7.5%. In patients with HES 130 solution cerebral blood flow increases by 14.2%, whereas when applied HAES-LX-5% during 7 days, it increases by 43.2% (p=0.004). Conclusion Conclusions: The analysis of the data of treatment the patients with AIS showed the best effect (p=0.004) of improvement of the cerebral circulation in the use of the polyfunctional infusion solution HAES-LX-5% unlike the 0.9% NaCl group and group of 0.9% NaCl+mannitol where was a decrease of the dynamics of cerebral blood flow, which could lead to hypoperfusion of the brain.

Objective The aim of the study was to find the most rational approach to postoperative pain relief in the complex of intensive care for the acute and early periods of traumatic diseasein elderly patients. Patients and methods Materials and methods: 132 gerontology patients with multiple trauma were included in the study. Respiratory and cardiovascular systems parameters were assessed, coagulogram and inflammatory response markers, pain severity and analgesia quality were assed. Results Results: According to the data obtained from the evaluation of VAS and Likerts scale, as well as other results, it was found that the use of NSAIDS and mu-opioid antagonists in the acute and early period of traumatic disease reduces significantly the severity of pain at a minimum daily dose (0.15 mg / kg), decreased the period of anaesthesia on demand, minimized negative respiratory and cardiovascular effects. Presence of sedative effect contributes to the improvement of emotional state of these patients. Conclusion Conclusions: Age-related changes and concomitant pathology in geriatric patients define the need for the safe pain relief in this patients, in order to decrease possible adverse events. Optimal combination in geriatric patients with perioperative pain syndrome due to polytrauma is NSAIDs with antagonists-agonists. This combination reduce the prevalence and severity of negative respiratory and cardiovascular.

Neuropathic pain is a pain caused by a disease or focal damage to the somatosensory nervous system. The prevalence of chronic pain with neuropathic features in different countries is estimated at 7–10 %. Damages to the nervous system can occur at the level of peripheral nerves, plexus and dorsal roots (peripheral neuropathic pain) or spinal cord and brain (central neuropathic pain). Neuropathic pain is based on pathological activation of pain pathways. Neuropathic pain occurs with diabetic polyneuropathy more often than with all polyneuropathies of another etiology. Hyperglycemia is the major cause of chronic diabetes mellitus and its progression. Since the cause of pain can rarely be cured, treatment is usually symptomatic. Pain is an inevitable consequence of surgical interventions in children, resulting in great stress and discomfort not only for patients but also for their parents. The intensity of the pain depends not only on the level of injury after the operation, but also on the localization and the nature of the procedure. Management of pain in children is best done through a multimodal approach: opioids, auxiliary drugs such as nonsteroidal anti-inflammatory drugs (NSAIDs) and acetaminophen, anti-neuroleptics such as gabapentin, and regional anesthetic methods. Postoperative anesthesia in abdominal surgery at present is a topical problem in anesthetic practice. In this clinical case, we would like to demonstrate the experience of applying post-operative anesthesia using subnormal dosages of ketamine. The patient was given anesthesia with prolonged infusion of a ketamine solution in a submorbid dose of 0.2 mg/kg/h IV. An assessment of the quality of anesthesia by assessing the level of stress markers, such as blood glucose, cortisol levels, and the assessment of the pain level on the NIPS scale was performed. Conclusion: The use of a ketamine solution in a dose of 0.2 mg/kg/h has a positive effect on treating postoperative pain in patients after severe abdominal surgical interventions. Висновок: Використання розчину кетаміну в дозі 0,2 мг/кг/год має позитивний ефект лікування післяопераційного болю у пацієнтів після важких абдомінальних оперативних втручань. Застосування розчину кетаміну в дозі 0,2 мг/кг/год зменшує толерантність пацієнта до опіоїдних анальгетиків і розвитку гіпералгезії та алодинії.

Background: Reversibility is usually used for the differentiation between asthma and COPD, but actually can it be used for the prediction of treatment response? Materials and methods: 81 patients was included into the study. We collected medical history, body composition, mMRC, CAT, SGRQ, PFT. Patients were divided into 2 groups – low-reversibility group (LR) with ≤ 6 % FEV1 improvement, and high-reversibility group (HR) - > 6 %. Re-assessment was performed in 3 months. Results: 39 (48.1 %) patients were in LR, and 42 patients (51.9 %) in HR. Average age: 63.7±5.6 in LR; 65.6±5.9 in HR. Smoking history: 26.7±3.9 in LR; 18.6±3.8 in HR (p 0.05). The article presents modern data on the use of N-acetylcysteine in the treatment of infectious diseases of the respiratory tract in the practice of a physician. Its antioxidant, mucolytic, anti-inflammatory, pulmoprotective and antitoxic properties are described. The emphasis is placed on the antioxidant properties of N-acetylcysteine, which are expressed when used in high doses of 1200 mg/day or more. There is a review of the literature on the use of high doses of N-acetylcysteine in the treatment of pneumonia, cystic fibrosis, COPD, ARDS. The effect of N-acetylcysteine on individual risk factors for prolonged pneumonia is analyzed. Given the epidemiological situation around the world, the antioxidant properties of N-acetylcysteine in the treatment of pneumonia, including complicated COVID-19, are relevant and require further study.

The article provides information on the possibilities of using postnatal ultrasound screening of the uropoietic tract illustrated by the results of using this method in the Slovak Republic. Its main task is to identify malformations of the uropoietic system that were not diagnosed during prenatal diagnosis. The most clinically significant malformations of the kidneys are associated with impaired urine flow (obstructive uropathy). Patients with these malformations often require surgical intervention in the neonatal period or in early childhood. Early diagnosis and timely correction of obstructive uropathy allows maintaining the normal functioning and morphological integrity of the kidneys, preventing the development of chronic kidney disease, improving the quality and life expectancy of patients. Thus, the costs of screening research are incommensurate with the consequences of an untimely diagnosed pathology and the treatment of complications such as chronic kidney disease, kidney failure, recurrent urinary tract infections (antibiotic therapy, hemodialysis, etc.)., У статті наводиться інформація про можливості використання постнатального ультразвукового скринінгу уропоетичного тракту на прикладі результатів застосування цього методу в Словацькій Республіці. Lower respiratory tract infections are the third mortality cause in the world and the first mortality cause in low-income countries. Community-acquired pneumonia (CAP) is a dangerous infectious pathology, especially in children younger than 5 years, elderly people and patients with compromised immunity. Antibiotic therapy is a standard treatment method for CAP. But an increase in antibiotics use caused a development of resistance in bacteria and onset of adverse events in humans. A new class of fluorchinolones is widely used nowadays in clinical practice. They have good activity against Gram-negative bacteria, as early chinolones, as well as against Gram-positive bacteria, such as pneumococci and atypical bacteria. An increasing amount of penicillin-resistant streptococcal pneumonia leads to more intensive studies of the use of new chinolones. In this review the data of 750 mg levofloxacin is presented. It is used in short courses for the treatment of severe and moderate CAP. Taking into account the broad spectrum of activity of levofloxacin ad bactericidal activity of this antibiotic, it can be a potential alternative therapy for the treatment of children and adults with CAP, especially with a tendency of an increase in antibiotic resistance of pneumococcus.

Today it is known that the use of opioid analgesics can further lead to the development of hyperalgesia. Opioid-induced hyperalgesia is an adaptive response of the body in response to exogenous administration of opioids, the mechanisms of development of which are associated with the activation of the central glutamatergic system and the release of spinal dinorphins. In contrast, gabapentin, NSAIDs, and ketamine have opioid-preserving properties, reducing the number of opioid-associated side effects. Hyperalgesia is a condition that underlies the formation of chronic pain and develops regardless of the degree of postoperative wound repair. For the treatment of pain in the postoperative period, the main group of treatment agents are opioid analgesics, which are prescribed to 60% of patients. However, with severe pain, there is a need for opioids in doses that exceed the standard recommended. Failed block could be one of the adverse results of the conductive anaesthesia. There are situations, when in seemingly perfect technique of the block, properly chosen drug, dosage, adjuvant – the block is not achieved (it does not occur or is incomplete or so-called “mosaic block” occurs). It is necessary to remember about atypical or abnormal nerves branching, possible interneural anastomoses, when looking for the possible cause of it. Information about most common types of interneural anastomosis of arm’s and leg’s long nerves is presented in this article. Knowledge of possible abnormal neural innervation can provide an understanding of inefficacy of some simple blocks, decrease amount of mistakes in data interpretation, that was obtained during electroneuromyographic (ENMG) investigations, improve understanding of clinical pattern in the peripheral nerve injuries, that form anastomosis., Неудачная блокада может быть одним из побочных явлений проводниковой анестезии. Background: In obese patients, high closing capacity and low functional residual capacity increase the risk for expiratory alveolar collapse. We hypothesized that lung aeration and respiratory mechanics improve in obese patients during FCV. Methods: We compared FCV and volume-controlled (VCV) ventilation in 18 obese patients in a randomized crossover setting. Starting with baseline measurements, ventilation settings were kept identical except for the ventilation mode related differences (VCV: inspiration to expiration ratio 1:2 with passive expiration, FCV: inspiration to expiration ratio 1:1 with active, linearized expiration). Primary endpoint of the study was the change of end-expiratory lung volume compared to baseline ventilation. Secondary endpoints were the change of mean lung volume, respiratory mechanics and hemodynamic variables. Results: The loss of end-expiratory lung volume and mean lung volume compared to baseline was lower during FCV compared to VCV (end-expiratory lung volume: FCV, - 144 ± 177 ml; VCV, - 323 ± 232 ml; p Conclusions: This study demonstrates that, compared to VCV, FCV improves regional ventilation distribution of the lung at comparable PEEP, tidal volume, Plat and ventilation frequency.

In the structure of postoperative anesthesia in children, as well as in other age groups, the main place is occupied by narcotic analgesics; the route of administration is traditional– intramuscular. The purpose of the study was to in-crease the effectiveness of postoperative analgesia with opioid analgesics in children. The study was conducted in 37 children with a mean age of 3.2 ± 0.8 years operated on tumors in the peritoneal space (27 children, 73%), kidney tumors (8chil-dren, 22%) and ovarian tumors (2 children, 5%). Comprehensive study of the reactions of pain behavior, physiological parameters and laboratory stress tests showed that the use of the scheme of analgesia based on acetaminophen 30 mg/kg and the method of continuous infusion of drugs (fentanyl in a dose of 5 μg/kg/h) for postoperative anesthesia leads to effective analgesia after traumatic operations in the neonatal period. The use of the method of continuous intravenous infusion of opioid analgesics leads to a pronounced, to varying degrees, sedation and respiratory depression, which enables children to adapt well to the apparatus of mechanical ventilation after various surgical interventions.

OBJECTIVE The aim was to examine intraocular pressure (IOP) during lumbar spine surgery in PP under general vs spinal anesthesia and to compare it with volunteers in PP. PATIENTS AND METHODS Materials and methods: We performed randomized prospective single institutional trial. Patients were operated in PP with fixation of 1-2 spinal segments. Patients of group I (n = 30) were operated under SA, group 2A (n = 25) - under TIVA (total intravenous anesthesia) with 45° head rotation (left eye was located lower, than right eye), group 2B (n=25) - under TIVA with no head rotation (both eyes were located on the same level). IOP was measured with Maklakov method before and after surgery. Volunteers (n = 20) were examined before and 90 minutes after lying in PP with 45° head rotation. RESULTS Results: In all patients and volunteers after lying in PP, we found that IOP have increased. In SA patients and in TIVA patients with no head rotation there was no difference between eyes. The most significant raise of IOP was found in the dependent eye of IIA group patients: it was higher than in volunteers and I group patients (p < 0.01), and IIB group patients (p < 0.05). In SA patients there was no difference in IOP comparing to volunteers. CONCLUSION Conclusions: IOP increased in PP in healthy people and patients under anesthesia (SA and TIVA). IOP in SA patients did not differ from volunteers. IOP increased superiorly in the dependent eye in TIVA patients.

Aim. The assessment and choice of analgesia method relative to hyperalgesia development in the experiment. Methods. Antiflogogenic effect of preparations was studied on the model of caragenin edema in 147 white non-linear rats with the mass 120–150 g. As a hyperalgesia model was used a caragenin edema. It was reproduced by the subplantar administration of 0,05 ml of 1 % caragenin solution in the right back extremity of the animal. Results. The combined use of bupivacaine regional analgesia manifested the moderate antiflogogenic effect (AFA was 25,3, 20,9 and 11,6 %, respectively), whereas fentanyl administration is, on the contrary, associated with the growth of prostaglandin edema. The conducting anesthesia with bupivacaine led to the more expressed analgesic effect that was proved by the level of corticosterone in blood. Neuropathic pain occurs with diabetic polyneuropathy more often than with all polyneuropathies of another etiology. Because the cause of pain can rarely be cured, treatment is usually symptomatic. Neuropathic pain is usually poorly controlled by analgesics. Management of neuropathic pain is started with conservative pharmacotherapy before invasive pain management is applied. Although there are many drugs that can be used in patients with diabetic pain syndrome, pain syndrome can not be surely stoped with monotherapy. In addition, the patient may not tolerate the full therapeutic dose of the drug. All this dictates the need for combination therapy. It is believed that regional anesthesia as an independent type of analgesia or a component of combined anesthesia is the method of choice for the elderly and senile patients. The main reasons for this choice, when comparing regional anesthesia with narcosis, are less stressful response of the organism, absence of depression of the central nervous system, stable reliable analgesia with complete blockade of nociceptive reflexes with the provision of adequate muscle relaxation, prevention of neurovegetative reactions, which ultimately helps to reduce the incidence of postoperative complications and mortality. According to British scientists, about 300 million operations are performed around the world annually. They cause acute postoperative pain, the management of which is crucial for improving patient outcomes and reducing healthcare costs. Local anesthetic infiltration before closing the surgical incision is a commonly used technique in the operating room. This review focuses on the use of local anesthetic infiltration, 0.25% bupivacaine, into surgical incisions to reduce postoperative pain, as confirmed by an estimate of a reduction in the use of postoperative opioids and a visual analogue scale (VAS). The presented clinical cases and the combined analgesia scheme with infiltration of a local anesthetic into the postoperative wound were used to make it possible to argue about the effectiveness of anesthesia because on the peripheral mechanism of pain. Infiltration analgesia reduced the need for opioids and the time of stay in hospitals. Зроблено висновки, що є потреба в подальшому дослідженні методик доставки анестетиків у післяопераційні рани для управління болем та покращення якості лікування.

Objectives To evaluate the analgesic effect of qudratus lumborum block (QLB) in pediatric patients administered before appendicitis. Methods This study was completed at the Faculty of Medicine, Vinnitsa national medical university, Vinnitsa, Ukraine, between September 2018 and Febrary 2019. In a prospective, double blind, randomized, placebo controlled clinical study, 38 pediatric patients scheduled for appendicitis operations were randomized to group Q (treatment group) or group C (controls). After receiving general anesthesia, group T received a QLB block using 0,3 mL per kg 0.25% bupivacaine on the right side, where as group C received a control block using 0,3 mL per kg 0.9% Sodium chloride. During the first 24 hours after surgery, the patient pain was evaluated using the visual analogue scale (VAS) at rest and while coughing. Postoperative patient controlled analgesia morphine consumption, VAS scores, and side effects were recorded. Results Of 24 patients, Group Q (n=14) had significantly lower VAS pain scores than Group C (n=10) both at rest and while coughing. The total morphine consumed was lower (5.2 ± 2.4) versus 17.3 ± 3.4 mg, p Conclusion As part of a multimodal analgesic regime after appendicitis, morphine consumption and VAS pain scores were significantly lower among those receiving 0,3 mL per kg 0.25% bupivacaine administered for a QL block than among controls.

Purpose Fluid overload is frequently found in critically ill pediatric patients with severe polytrauma (SP) and is associated with adverse outcomes. Lungultrasonography (LU) and bioimpedance spectroscopy (BIS) are potentially useful tools for the noninvasive volume assessment. We evaluated the utility of these measures, alone or in combination, in estimating the PaO2/FiO2 ratio in critical pediatric patients with SP . Methods In a prospective pilot observational study we included 23 patients who presented on admission or developed at any time during intensivecare unit stay SP. Patients were studied at baseline and after 48 h with LU, BIS and arterial blood gas. Results In the univariable analysis, the PaO2/FiO2 ratio was negatively correlated with the B-lines score, and this association was maintained even after adjustments. A cutoff value of 17 for the B-lines score has a sensibility of 73,91% and a specificity of 69, 23% in identifying patients with PaO2/FiO2 Conclusion LU can be used for functional lung evaluation and identification of pediatric patients with increase pulmonary water content and decrease PaO2/FiO2 ratio.

Objectives To compare outcomes of children receiving noninvasive ventilation with those receiving invasive ventilation as first-line mode of mechanical ventilation following unplanned intensive care admission. Design Propensity score-matched cohort study analyzing data prospectively collected by the Pediatric Intensive Care Vinnitsa Regional Children Hospital 15 years (2003–2018). Setting PICU in the Vinnitsa Regional Children Hospital of whom submitted Pediatric Critical Care Minimum Dataset data for the entire study period. Patients Children consecutively admitted to study PICUs. Planned admissions following surgery, unplanned admissions from other hospitals, those on chronic ventilation, and those who did not receive mechanical ventilation on the day of PICU admission were excluded. Interventions Use of noninvasive ventilation, rather than invasive ventilation, as the first-line mode of mechanical ventilation. Measurements and Main Results PICU mortality, length of ventilation, length of PICU stay, and ventilator-free days at day 28. During the study period, there were 11,112 PICU admissions. A total of 1,114 admissions (10, 1%) were eligible for analysis once predefined exclusion criteria were applied: 504 (45.24%) received ‘noninvasive ventilation first,’ whereas 221 (19.83%) received ‘invasive ventilation first’; 24 (2.15%) admissions could not be classified. Admitting PICU site explained 6.5% of the variation in first-line mechanical ventilation group (95% CI, 4.0–18.0%). In propensity score-matched analyses, receiving noninvasive ventilation first was associated with a significant reduction in mortality by 4.2% (95% CI, 1.9–5.6%), length of ventilation by 1.8 days (95% CI, 1.0–3.3), and length of PICU stay by 2.4 days (95% CI, 1.6–3.7), as well as an increase in ventilator-free days at day 28 by 4.1 days (95% CI, 3.1–5.4). Conclusions Use of noninvasive ventilation as first-line mode of mechanical ventilation in critically ill children admitted to PICU in an unplanned fashion may be associated with significant clinical benefits.

The objective of this study was to evaluate the implementation of a focused cardiac ultrasound (FoCUS) protocol in a pediatric emergency department (PED). Methods We conducted a cross-sectional, observational, quality improvement project in a PED of an urban tertiary care Vinnitsa region children hospital. A FoCUS protocol was collaboratively developed by pediatric intensive care and pediatric emergency medicine. This included a reference document with definitions, indications, image acquisition guidelines, and interpretation expectations. We measured physician-emergency pediatric doctors performance against pediatric cardiologist interpretation of stored cine clips as our reference standard. Focused cardiac ultrasound interpretation was dichotomized for the presence or absence of pericardial effusion, depressed left ventricular function, and chamber size abnormalities. Results 243 FoCUSs were performed by 5 different emergency doctors from January 218 to December 2019. The prevalence of FoCUS abnormalities was 19.4%. For pericardial effusion, sensitivity was 99,2% (95% confidence interval [CI], 48%–100%) and specificity was 94% (95% CI, 90–100%). For depressed function, sensitivity was 99,4% (95% CI, 56%–100%) and specificity was 98% (95% CI, 94%–100%). For chamber size abnormalities, sensitivity was 97% (95% CI, 50%–100%) and specificity was 95% (95% CI, 88%–98%). The median number of monthly FoCUS increased from 3 (preprotocol) to 7 (postprotocol), and the median rate of adequate studies increased from 0% to 55%. Conclusions We report the collaborative development and successful implementation of a PED FoCUS protocol. Physician-emergency doctors interpretation of FoCUS yielded acceptable results.

The main remit of the European Society for Paediatric Anaesthesiology (ESPA) Pain Committee is to improve the quality of pain management in children. The ESPA Pain Management Ladder is a clinical practice advisory based upon expert consensus to help to ensure a basic standard of perioperative pain management for all children. Further steps are suggested to improve pain management once a basic standard has been achieved. The guidance is grouped by the type of surgical procedure and layered to suggest basic, intermediate, and advanced pain management methods. The committee members are aware that there are marked differences in financial and personal resources in different institutions and countries and also considerable variations in the availability of analgesic drugs across Europe. We recommend that the guidance should be used as a framework to guide best practice.
