Abstract: OBJECTIVE Sulfonylureas, the first available drugs for the management of type 2 diabetes, remain widely prescribed today. However, there exists significant variability in glycemic response to treatment. We aimed to establish heritability of sulfonylurea response and identify genetic variants and interacting treatments associated with HbA1c reduction. RESEARCH DESIGN AND METHODS As an initiative of the Metformin Genetics Plus Consortium (MetGen Plus) and the DIabetes REsearCh on patient straTification (DIRECT) consortium, 5,485 White Europeans with type 2 diabetes treated with sulfonylureas were recruited from six referral centers in Europe and North America. We first estimated heritability using the generalized restricted maximum likelihood approach and then undertook genome-wide association studies of glycemic response to sulfonylureas measured as HbA1c reduction after 12 months of therapy followed by meta-analysis. These results were supported by acute glipizide challenge in humans who were naïve to type 2 diabetes medications, cis expression quantitative trait loci (eQTL), and functional validation in cellular models. Finally, we examined for possible drug-drug-gene interactions. RESULTS After establishing that sulfonylurea response is heritable (mean ± SEM 37 ± 11%), we identified two independent loci near the GXYLT1 and SLCO1B1 genes associated with HbA1c reduction at a genome-wide scale (P < 5 × 10−8). The C allele at rs1234032, near GXYLT1, was associated with 0.14% (1.5 mmol/mol), P = 2.39 × 10−8), lower reduction in HbA1c. Similarly, the C allele was associated with higher glucose trough levels (β = 1.61, P = 0.005) in healthy volunteers in the SUGAR-MGH given glipizide (N = 857). In 3,029 human whole blood samples, the C allele is a cis eQTL for increased expression of GXYLT1 (β = 0.21, P = 2.04 × 10−58). The C allele of rs10770791, in an intronic region of SLCO1B1, was associated with 0.11% (1.2 mmol/mol) greater reduction in HbA1c (P = 4.80 × 10−8). In 1,183 human liver samples, the C allele at rs10770791 is a cis eQTL for reduced SLCO1B1 expression (P = 1.61 × 10−7), which, together with functional studies in cells expressing SLCO1B1, supports a key role for hepatic SLCO1B1 (encoding OATP1B1) in regulation of sulfonylurea transport. Further, a significant interaction between statin use and SLCO1B1 genotype was observed (P = 0.001). In statin nonusers, C allele homozygotes at rs10770791 had a large absolute reduction in HbA1c (0.48 ± 0.12% [5.2 ± 1.26 mmol/mol]), equivalent to that associated with initiation of a dipeptidyl peptidase 4 inhibitor. CONCLUSIONS We have identified clinically important genetic effects at genome-wide levels of significance, and important drug-drug-gene interactions, which include commonly prescribed statins. With increasing availability of genetic data embedded in clinical records these findings will be important in prescribing glucose-lowering drugs.

Abstract: Journal of synchrotron radiation 28(3), 688 - 706 (2021). doi:10.1107/S1600577521002551, The high-precision X-ray diffraction setup for work with diamond anvil cells (DACs) in interaction chamber 2 (IC2) of the High Energy Density instrument of the European X-ray Free-Electron Laser is described. This includes beamline optics, sample positioning and detector systems located in the multipurpose vacuum chamber. Concepts for pump–probe X-ray diffraction experiments in the DAC are described and their implementation demonstrated during the First User Community Assisted Commissioning experiment. X-ray heating and diffraction of Bi under pressure, obtained using 20 fs X-ray pulses at 17.8 keV and 2.2 MHz repetition, is illustrated through splitting of diffraction peaks, and interpreted employing finite element modeling of the sample chamber in the DAC., Published by Wiley-Blackwell, [S.l.]

Abstract: To date, 18 living recommendations for the clinical care of pregnant and postpartum women with COVID-19 have been issued by the National COVID-19 Clinical Evidence Taskforce. This includes recommendations on mode of birth, delayed umbilical cord clamping, skin-to-skin contact, breastfeeding, rooming-in, antenatal corticosteroids, angiotensin-converting enzyme inhibitors, disease-modifying treatments (including dexamethasone, remdesivir and hydroxychloroquine), venous thromboembolism prophylaxis and advanced respiratory support interventions (prone positioning and extracorporeal membrane oxygenation). Through continuous evidence surveillance, these living recommendations are updated in near real-time to ensure clinicians in Australia have reliable, evidence-based guidelines for clinical decision-making. Abstract: Assessing patient goals is crucial in understanding patient centered outcomes and satisfaction. However, patient goals may change throughout treatment. Our objective is to identify the changes in patient-selected goals of Parkinson's disease (PD) patients undergoing bilateral subthalamic nucleus deep brain stimulation (STN-DBS) and examine the relationship among patient-selected goal achievement, standard DBS outcome measures, and overall patient satisfaction. Seventy-five patients undergoing bilateral STN-DBS listed three patient-selected goals before surgery. After six months, patients were asked to restate the three goals and to rate the degree of goal achievement and the overall satisfaction of surgery. The three most frequently selected goals were "dyskinesia", "gait disorder", and "medication off duration". After six months, 80.0% of patients could not accurately recall their pre-DBS goals. "Dyskinesia" was the most consistently selected goal, more patients selected "tremor" and "less medication" at post-DBS compared to pre-DBS, and less patients selected "gait disorder" at post-DBS compared to pre-DBS. 74.7% of patients reported overall satisfaction by stating they were "very much" or "much better after surgery". Patient satisfaction significantly correlated with goal achievement (r = 0.640; p 0.001). Interestingly, change in UPDRS motor scores did not correlate with patient satisfaction (r = 0.100; p = 0.395). Although recalled goals do not accurately represent the pre-surgical goals, the achievement score for recalled goals significantly correlated with patient satisfaction. Patient goals change due to many reasons. Therefore, follow-up patient counseling to discuss goals and outcomes is important in improving patient satisfaction after STN-DBS.

Abstract: To evaluate the difference in oncologic outcome between vulvar cancer patients with uni- and bilateral inguino-femoral lymph nodal involvement and to identify factors affecting their oncologic outcome Patients who underwent inguino-femoral lymphadenectomy for vulvar cancer were classified into three groups according to their lymph nodal status at the histology analysis (negative, positive one side, positive bilaterally). PFS and OS survival were calculated using the Kaplan–Meier method. Univariate and multivariate analyses were performed to analyze factors predicting overall survival and progression-free survival. Multivariable models were used for variables reporting a p value ≤ 0.1 at the univariate analysis. p values ≤ 0.05 were considered statistically significant. One hundred and forty-six patients were considered for the analysis. Patients with bilaterally negative lymph nodes had significantly longer PFS and OS as compared to patients with unilateral and bilateral involvement. Patients with unilateral lymph nodal involvement had better PFS than patients with bilateral lymph nodal involvement. Among these patients, the difference in the OS approached but did not reach statistical significance. At the multivariate analysis, the tumor size affected PFS and lymph nodal involvement affected OS. Vulvar cancer patients with bilateral positive lymph nodes have worse oncologic outcome as compared to patients with unilateral lymph nodal involvement; similarly, patients with unilateral lymph nodal involvement have worse oncological outcome as compared to patients with bilateral negative lymph nodes. Furthermore, tumor size and lymph nodal status are independent factors predicting recurrence rate and overall survival, respectively.

Abstract: Quantum computing promises to overcome computational limitations with better and faster solutions for optimization, simulation, and machine learning problems. Europe and Germany are in the process of successfully establishing research and funding programs with the objective to advance the technology’s ecosystem and industrialization, thereby ensuring digital sovereignty, security, and competitiveness. Such an ecosystem comprises hardware/software solution providers, system integrators, and users from research institutions, start-ups, and industry. The vision of the Quantum Technology and Application Consortium (QUTAC) is to establish and advance the quantum computing ecosystem, supporting the ambitious goals of the German government and various research programs. QUTAC is comprised of ten members representing different industries, in particular automotive manufacturing, chemical and pharmaceutical production, insurance, and technology. In this paper, we survey the current state of quantum computing in these sectors as well as the aerospace industry and identify the contributions of QUTAC to the ecosystem. We propose an application-centric approach for the industrialization of the technology based on proven business impact. This paper identifies 24 different use cases. By formalizing high-value use cases into well-described reference problems and benchmarks, we will guide technological progress and eventually commercialization. Our results will be beneficial to all ecosystem participants, including suppliers, system integrators, software developers, users, policymakers, funding program managers, and investors.

Abstract: Objectives To determine the prevalence of Melanocortin-4 Receptor (MC4R) mutations in a cohort of children and adolescents with overweight or obesity and to determine whether treatment responses differed between carriers and noncarriers. Methods Using target region capture sequencing, an MC4R mutation screen was performed in 1261 Danish children and adolescents enrolled at a tertiary multidisciplinary childhood obesity treatment center. Measurements of anthropometrics, blood pressure, fasting blood biochemistry including lipid and hormone levels, and dual-energy X-ray absorptiometry were performed at baseline and throughout treatment. Results Of 1209 children and adolescents that met all criteria to be included in the described analyses, 30 (2.5%) carried damaging or unresolved MC4R mutations. At baseline, mutation carriers exhibited higher concentrations of plasma thyroid-stimulating hormone (p = 0.003), and lower concentrations of plasma thyroxine (p = 0.010) compared to noncarriers. After a median of 1 year of treatment (range 0.5–4.0 years), body mass index (BMI) standard deviation score (SDS) was reduced in noncarriers but not in carriers, and this difference in treatment response was statistically significant (p = 0.005). Furthermore, HDL cholesterol was reduced in carriers, a response significantly different from that of noncarriers (p = 0.017). Conclusion Among Danish children and adolescents with overweight or obesity entering a tertiary lifestyle intervention, 2.5% carried damaging or unresolved MC4R mutations. In contrast to noncarriers, carriers of damaging or unresolved MC4R mutations failed to reduce their BMI SDS during obesity treatment, indicating a need for personalized treatment based on the MC4R genotype.

Abstract: The spread of Internet-of-Things (IoT) in domains like health monitoring and the success of cloud services have pushed the horizon of new computing paradigms, such as wearable computing, with health monitoring systems being found in smart homes, hospitals or sports devices. The deployment of off-the-shelf sensors to support monitoring of health functionality, physiology, and activity, can yield to identify health risks. Still, there are several technological challenges to be addressed before biosensor systems with smart healthcare services can be widely adopted. This work addresses latest developments in Smart Health applications, focusing on technical challenges from both system and service perspectives in the context of wearable and medical IoT. We classify relevant publications from the last five years by parameters in the IoT paradigm, including the wearable technology, vital signs, service identification, and IoT computing models such as edge, fog, and cloud. Our findings show that the development faces a lack of system standardization and that data processing moves from servers or cloud locations toward the devices. Second, we examine the challenges present in this domain, the main ones identified are security, privacy, and energy efficiency.

This guideline also considers additional issues prioritised by the Cochrane Institute and the Institute for Research in Operative Medicine (IFOM). The evaluation of evidence was based on the system developed by the Scottish Intercollegiate Guidelines Network (SIGN). A multi-part nominal group process moderated by the AWMF was used to compile this S3-level guideline. Recommendations Recommendations on consultations, indications and the process of performing a caesarean section as well as the care provided to the mother and neonate were drawn up.

Abstract: Recently, several biologics have been introduced as add-on treatment for severe asthma [1]. The biologics target the underlying mechanism driving the disease and are recommended for specific phenotypes such as mepolizumab (anti-interleukin (IL)-5) for severe eosinophilic asthma [2] and omalizumab (anti-immunoglobulin (Ig)E) for severe allergic asthma [3]; i.e. specific endotypes of type 2 inflammation. Dupilumab (anti-IL-4Rα) is a newer biologic recommended for severe asthma with type 2 inflammation characterised by elevated blood eosinophils (≥150 per μL) and/or exhaled nitric oxide fraction (FeNO ≥25 ppb) [4]. Abstract: OBJECTIVEFor patients with highly asymmetrical Parkinson’s disease (PD), unilateral subthalamic nucleus (STN) deep brain stimulation (DBS) has been suggested as a reasonable treatment. However, the results of a previous 2-year follow-up study involving patients with prominently asymmetrical PD who had unilateral STN DBS suggested that simultaneous bilateral surgery should be performed. In the present study, the authors analyze 7-year follow-up data from the same patient group to examine changes in motor benefit from unilateral STN DBS over time and the interval between initial unilateral surgery and a second (contralateral) STN DBS surgery.METHODSEight patients with highly asymmetrical parkinsonism who underwent unilateral STN DBS were evaluated. The factors measured were scores on the motor part of the Unified Parkinson’s Disease Rating Scale (UPDRS III), Hoehn and Yahr (HY) stage, and levodopa equivalent daily dose (LEDD). Evaluations occurred at 3, 6, and 12 months after the initial surgery and annually thereafter.RESULTSThe mean follow-up period was 91.5 months (range 36–105 months). Three years after the initial unilateral surgery, motor benefits on the contralateral side continued; however, an aggravation of the ipsilateral parkinsonism attenuated the improvement in total UPDRS III scores, which reverted to baseline. Axial motor score, LEDD, and HY stage did not differ from the baseline. Seven of 8 patients (87.5%) were considered candidates for a second surgery to offer additional motor benefits. Of the 7 candidates, 4 patients (50% of total patients) underwent the second surgery at 58.5 ± 11.6 (mean ± SD) months after the initial surgery. Three patients were not able to have the second surgery: one patient died of gastric cancer, one patient was severely immobilized by an accident, and one patient could not afford the second surgery. One patient remained content with the initial unilateral surgery throughout the follow-up period.CONCLUSIONSSeven of 8 patients with unilateral STN DBS became candidates for second surgery before battery replacement surgery of the first implanted device. Baseline asymmetry alone may not predict appropriate candidates for unilateral STN DBS. This study provides further evidence that, from a long-term perspective, initial simultaneous bilateral STN DBS should be considered for PD patients with prominently asymmetrical motor symptoms.

Abstract: As of January 12, 2021, Australia has reported 28,634 COVID-19 cases. Most (20,411) cases are from the state of Victoria. In response to rising infections and community transmission in July 2020, on August 2nd, several restrictions were imposed for the following 111 days, including an 8pm curfew, a travel restriction to 5 km from home, and closure of nonessential services. It is unknown how this affected people living with HIV (PLHIV), who already experience disproportionate levels of mental health issues, comorbidity, and stigma. An online survey was designed with HIV community-based organizations to investigate the impact of the pandemic on Victorian PLHIV. Participants were recruited voluntarily both through social media and Infectious Diseases clinics at participating hospitals. There were 153 respondents. Most were male (77%), aged between 30 and 60 years (77%), and Australian-born (63%). Forty-three percent, 31%, and 25% reported negative impacts upon personal relationships, employment, and income, respectively. HIV care continued with 95% and 98% being able to access their HIV provider and antiretroviral therapy (ART), respectively. Telehealth was used by 92% and was largely well received. PLHIV reported worry about physical health (68%), mental health (66%), finances (50%),z and accommodation (25%). Fifty percent of participants reported weight gain and 27% increased alcohol intake. This study demonstrated the widespread negative effects of the COVID-19 pandemic on PLHIV in Victoria, although provision of HIV care and ART continued uninterrupted. This highlighted the importance of mental health support and social welfare programs during times of health care and societal strain.

Abstract: Background: Left ventricular ejection fraction (EF) is an indicator of cardiac function, usually assessed in individuals with heart failure and other cardiac conditions. Although family studies indicate that EF has an important genetic component with heritability estimates up to 0.61, to date only 6 EF-associated loci have been reported. Methods: Here, we conducted a genome-wide association study (GWAS) of EF in 26 638 adults from the Genetic Epidemiology Research on Adult Health and Aging and the UK Biobank cohorts. Results: A meta-analysis combining results from Genetic Epidemiology Research on Adult Health and Aging and UK Biobank identified a novel locus: TMEM40 on chromosome 3p25 (rs11719526; β=0.47 and P =3.10×10 −8 ) that replicated in Biobank Japan and confirmed recent findings implicating the BAG3 locus on chromosome 10q26 in EF variation, with the strongest association observed for rs17617337 (β=−0.83 and P =8.24×10 −17 ). Although the minor allele frequencies of TMEM40 rs11719526 were generally common (between 0.13 and 0.44) in different ethnic groups, BAG3 rs17617337 was rare (minor allele frequenciesTMEM40 or BAG3 on EF are largely independent of these conditions. Conclusions: In this large and multiethnic study, we identified 2 loci, TMEM40 and BAG3 , associated with EF at a genome-wide significance level. Identifying and understanding the genetic determinants of EF is important to better understand the pathophysiology of this strong correlate of cardiac outcomes and to help target the development of future therapies.

A large ontology (the Digital Reference) that describes semiconductor supply chains and supply chains containing semiconductors is needed so as to leverage the highly distributed and decentralized data sources. The Digital Reference Platform Application (DRPA) is a tool designed to explore the power of the Digital Reference Ontology, offering features for ontology exploration and data creation along with Arrowhead Framework integration. With this, the DRPA offers interoperability between arbitrary software applications and industrial cyber-physical systems - integrated into Arrowhead Local Clouds. This approach applied for a voting wristband prototype shows the potential of the DPRA. The final target is to extend the potential of DRPA beyond Ontology exploration.

Abstract: A printable indicator ink for detecting photocatalysis was formulated on the base of the INTEC indicator ink concept. The ink was reformulated to be printable with a HP C6602A bubblejet printing cartridge, mounted on a 2D CNC system, on different building materials. In addition, an automated evaluation system including a measurement chamber and a control- and evaluation software was developed to provide easy to interpret mappings of the photocatalytic surface activity as a control instrument for quality assurance in the industrial production. Test measurements with samples of industrial products revealed a possible spatial resolution within millimetre range.

Abstract: Introduction Previous studies have reported improvement of impulse control disorders (ICDs) after subthalamic nucleus (STN) deep brain stimulation (DBS) as well as some de novo ICDs. However, it is not clear how STN DBS changes ICDs in the long-term. Methods materials Eighty-nine patients with Parkinson's disease (PD) who had received a bilateral STN DBS from 2005 to 2009 and were included in our previous study were followed for 7 years with the modified Minnesota Impulsive Disorders Interview (mMIDI). Their mMIDI scores, medication, and frontal function tests measured preoperatively and at 1 and 7 years postoperatively were compared. Results A total of 61 patients were analyzed after excluding 10 and 18 patients due to death and lost to follow-up, respectively. The numbers of the patients with an ICD at each point were 8, 10, and 7, respectively. All preoperative ICDs disappeared after DBS. De novo ICDs within 1 year after DBS disappeared except for 1 patient. Six of the seven patients, who reported ICDs 7 years after the DBS developed that ICD between 1 and 7 years. Their total levodopa equivalent daily dose (LEDD) and dopamine agonist dose were not higher compared to the other 54 patients without ICDs. There was no correlation with the frontal lobe dysfunction and the electrode position in the subthalamus. Conclusion STN DBS improves baseline ICDs and results in the development of “transient” de novo ICDs in the short-term. In addition, there is a unique group of the patients who develop ICDs a long time after DBS.

Abstract: Some of patients with Parkinson's disease (PD) have abnormal tandem gait (TG) performance without any symptoms and signs of cerebellar dysfunction. Clinical difference between patients with good and poor TG performance has not yet been studied. We report the relationship between tandem gait performance and clinical characteristics including 37 patients with PD who had no evidence of cerebellar dysfunction. Using tandem gait test, the patients were divided into two groups (good-TG and poor-TG). We evaluated the two groups with Montreal Cognitive Assessment (MoCA), Mini Mental Status Examination (MMSE), Frontal Assessment Batter (FAB), Beck Depression Index (BDI-II), REM Sleep Behavior Disorder Single-Question Screen (RBD1Q), MDS-UPDRS items related to axial disability and freezing. Fifteen participants were classified as good-TG group and 22 were as poor-TG group. Participants in good-TG group had higher MoCA score and lower BDI-II score. The proportion of participants who answered "yes" to RBD1Q was lower in good-TG group (27%, 4 of 15) than that in poor-TG group (55%, 12 of 22). All participants in good-TG group marked "0" for the MDS-UPDRS item 2.13 which addresses freezing event over the past week, whereas 18% (4 of 22) of participants in poor-TG group marked "1". In conclusion, tandem gait performance may be related to various clinical characteristics including cognitive function, mood, RBD, and freezing in patients with PD.

Abstract: Simulation is capable to cope with the uncertain and dynamic nature of industrial value chains. However, in-depth system expertise is inevitable for mapping objects and constraints from the real world to a virtual model. This knowledge-intensity leads to long development times of respective projects, which contradicts the need for timely decision support. Since more and more companies use industrial knowledge graphs and ontologies to foster their knowledge management, this paper proposes a framework on how to efficiently derive a simulation model from such semantic knowledge bases. As part of the approach, a novel Simulation Ontology provides a standardized meta-model for hybrid simulations. Its instantiation enables the user to come up with a fully parameterized formal simulation model. Newly developed Mapping Rules facilitate this process by providing guidance on how to turn knowledge from existing ontologies, which describe the system to be simulated, into instances of the Simulation Ontology. The framework is completed by a parsing procedure for an automated transformation of this conceptual model into an executable one. This novel modeling approach makes model development more efficient by reducing its complexity. It is validated in a use case implementation from semiconductor manufacturing, where cross-domain knowledge was required in order to model and simulate the impacts of the COVID-19 pandemic on a global supply chain network.

Abstract: In this paper, a supply chain simulation testbed for the semiconductor industry is proposed. We start by identifying requirements for such reference datasets, and then we identify the main building blocks. The nodes of the supply chain that represent semiconductor wafer fabrication facilities (wafer fabs) are built on a simulation model from the measurement and improvement of manufacturing capacity project. We present two techniques to reduce the modeling and computational burden that are able to deal with load-dependent cycle times in single front-end and back-end facilities and in the overall network. The first method models in detail only the bottlenecks in the nodes of the supply chain, while the second one uses empirical distributions for cycle time and throughput. The quality of these reduction techniques is assessed by comparing the detailed model and the models with a reduced level of detail. We present an application scenario for the testbed by simulating a semiconductor supply network. In addition, the usage of the testbed is discussed.

Abstract: With diverse product mixes in fabs, high demand volatility, and numerous manufacturing steps spread across different facilities, it is impossible to analyze the combined impacts of multiple operations in semiconductor supply chains without a modeling tool like simulation. This paper explains how ontologies can be used to develop and deploy simulation applications, with interoperability and knowledge sharing at the semantic level. This paper proposes a concept to automatically build simulations using ontologies and its preliminary results. The proposed approach seeks to save time and effort expended in recreating the information for different use cases that already exists elsewhere. The use case provides first indications that with an enhancement of a so-called Digital Reference with Semantic Web Technologies, modeling and simulation of semiconductor supply chains will not only become much faster but also require less modeling efforts because of the reusability property.

Abstract: BackgroundFirst-pass effect (FPE), restoring complete or near complete reperfusion (modified Thrombolysis in Cerebral Infarction (mTICI) 2c-3) in a single pass, is an independent predictor for good functional outcomes in the endovascular treatment of acute ischemic stroke. The economic implications of achieving FPE have not been assessed.ObjectiveTo assess the economic impact of achieving complete or near complete reperfusion after the first pass.MethodsPost hoc analyses were conducted using ARISE II study data. The target population consisted of patients in whom mTICI 2c–3 was achieved, stratified into two groups: (1) mTICI 2c–3 achieved after the first pass (FPE group) or (2) after multiple passes (non-FPE group). Baseline characteristics, clinical outcomes, and healthcare resource use were compared between groups. Costs from peer-reviewed literature were applied to assess cost consequences from the perspectives of the United States (USA), France, Germany, Italy, Spain, Sweden, and United Kingdom (UK).ResultsAmong patients who achieved mTICI 2c–3 (n=172), FPE was achieved in 53% (n=91). A higher proportion of patients in the FPE group reached good functional outcomes (90-day modified Rankin Scale score 0–2 80.46% vs 61.04%, pConclusionsFPE resulted in improved clinical outcomes, translating into lower healthcare resource use and lower estimated costs.

Abstract: Four 3D printable Portland cement-based mix designs were developed. A method for determination of compressive and flexural strength of 3D printed prisms (taking into account the effects of 3D printing on the surface of the object) and a comparison to molded samples are presented. Prisms prepared by 3D printing show interfilament voids but have nevertheless mean values of compressive strength which are comparable to molded prisms. Flexural strength was strongly affected by surface irregularities introduced by manufacturing by 3D printing and is in every case lower as for molded specimens. In most cases, the variation of strength values of 3D printed test specimens was considerably higher than for molded equivalents. The presented Portland cement-based mix designs include a mix with the use of an ordinary Portland cement (OPC) clinker mixed with a calcium sulfate addition which is lower than in standard OPC which was also successfully utilized for 3D printing. This mix performed best in compressive and flexural strength. The developed materials were further examined by slump test and ultrasonic wave velocity.

Abstract: Background Omalizumab is approved for treating severe allergic asthma from age 6, but the definition of severe asthma including a systematic assessment to rule out difficult-to-treat asthma has changed since the drug was approved in 2003. Methods We conducted a systematic review and meta-analysis of two critical (exacerbation rate, oral corticosteroid (OCS) treatment) and eight important clinical outcomes in children, adolescents and adults, and specifically searched papers for systematic assessment of severe asthma. Results Adults: seven studies (n = 2159) ascertaining exacerbation rate showing a 37% (95% CI 21–50) reduction in favor of omalizumab, larger than the pre-specified minimal clinically important difference (MCID) of 25%. Only one open-label study (n = 82) was identified assessing the percentage of patients experiencing reduction of OCS-maintenance treatment showing a significantly greater decrease in the omalizumab group (− 45% vs. + 18.3%, p = 0.002). Children and adolescents: four studies (n = 1551) reported data on exacerbations (no meta-analysis conducted), showed overall improvements in exacerbation rate and some passed MCID. No OCS studies were identified. No included studies provided systematic assessment of severe asthma according to current guidelines. Conclusions Omalizumab provides clinically relevant improvements in exacerbation rate among children, adolescents, and adults and in OCS-reduction among adults. Abstract: Moore's Law indicates that the amount of transistors which can be accommodated on integrated circuit (IC) double every two years in semiconductor manufacturing industry. As a capital intensive and competitive industry, supply chains in the semiconductor industry feature high structural complexity and high demand uncertainty. In order to enhance the effectiveness of the enterprise, total resource management (TRM) for semiconductor industry is increasingly crucial. Past studies focused on measuring the effectiveness of either demand fulfillment or capacity utilization, but little researches have done for investigating the inter-coordination on both aspects. This paper proposes overall supply chain effectiveness (OSCE) for the incorporation of demand planning and capacity portfolio based on PDCCCR framework to evaluate the effectiveness of semiconductor supply chain industry. The proposed index is implemented in an empirical study based on a simulation model for semiconductor backend production. Finally, the paper concludes with the discussion of future research directions.

Abstract: The increasing customisation of products, which leads to higher numbers of product variants with smaller lot sizes, requires a high flexibility of manufacturing systems. These systems are subject to dynamic influences and need increasing effort for the generation of the production schedules and for the control of the processes. This paper presents an approach that addresses these challenges. First, scheduling is done by coupling an optimisation heuristic with a simulation model to handle complex and stochastic manufacturing systems. Second, the simulation model is continuously adapted by real-time data from the shop floor. If, e.g., a machine breakdown or a rush order appears, the simulation model and consequently the scheduling model is updated and the optimisation heuristic adjusts an existing schedule or generates a new one. This approach uses real-time data provided by future cyber-physical systems to integrate scheduling and control and to manage the dynamics of highly flexible manufacturing systems.

Abstract: Objective Forty-nine known single nucleotide polymorphisms (SNPs) associating with body mass index (BMI)-adjusted waist-hip-ratio (WHR) (WHRadjBMI) were recently suggested to cluster into three groups with different associations to cardiometabolic traits. Genetic risk scores of the clusters on the risk of incident diabetes and associations with detailed cardiometabolic phenotypes were tested. Methods In a prospective study of 6,121 Inter99 individuals, the risk of incident diabetes using Cox proportional hazards regression was evaluated. Using linear regession, the associations between genetic risk scores and anthropometry and blood samples at fasting and during an oral glucose tolerance test were tested. Analyses were adjusted for age, sex, and BMI. Results Cluster 1 associated with an increased risk of diabetes (HR = 1.05, P = 2.74 × 10−4) and with a poor metabolic profile, including fasting serum triglyceride (β = 0.98% mmol/L, P = 3.33 × 10−8) and Matsuda index (β = −0.74%, P = 1.29 × 10−4). No similar associations for Clusters 2 and 3 were found. The three clusters showed different patterns of association with waist circumference, hip circumference, and height. Conclusions Our results suggest that the 49 WHRadjBMI-associated SNPs affect metabolic health differently depending on the cluster of SNPs. The clusters further associate differently with anthropometric measures.

Abstract: Objective Subthalamic nucleus deep brain stimulation (STN DBS) has a positive effect on sleep quality, but its effect on wake functions is controversial. This study evaluated the longitudinal changes of the quality of sleep and excessive daytime sleepiness (EDS) in Parkinson’s disease (PD) patients undergoing STN DBS and identify which factors are associated with the presence of EDS before and after STN DBS. Patient and methods A total of 33 PD patients who underwent bilateral STN DBS between July 2011 and October 2015 were recruited. We evaluated subjective sleep quality assessed by Parkinson’s Disease Sleep Scale (PDSS) and EDS using Epworth Sleepiness Scale (ESS) preoperatively and 6 months, 1 year, and 3 years postoperatively. Results There is a significant improvement in PDSS, and a noticeable change occurs immediately after the surgery. After DBS, the number of patients with persistent EDS gradually decreased, but patients with newly developed EDS were added. Baseline ESS score was highly correlated with EDS at 6 months and 1 year postoperatively, and older age of PD onset was highly associated with EDS at 1 year after DBS. At 3 years after DBS, the total PDSS score is a main contributing factor for EDS. There was no significant difference in dopamine agonist dose (agonist LED) and levodopa equivalent daily dose (LEDD) between groups with and without EDS at any time points. Conclusion Bilateral STN DBS improves the subjective sleep quality, but EDS may improve or worsen. The risk factors for EDS change over time after STN DBS. Interestingly, dopaminergic medication did not affect EDS in DBS-treated PD patients.

Abstract: Objective: This study aimed to describe the change in functional status following bilateral subthalamic nucleus stimulation (STN-DBS) in Parkinson’s disease (PD) and to identify predictors of postoperative functional dependence. Methods: We included PD patients with bilateral STN-DBS who had complete Schwab & England Activities of Daily Living (S&E ADL) Scale data at baseline and 6 months after surgery from our prospective registry. Functional dependence was defined as an S&E ADL score of less than 80%. All data were collected from the on-medication state and on-stimulation state (after surgery). Logistic regression analyses were performed to determine the factors predictive of functional dependence after surgery. Results: A total of 196 patients were included. At baseline, 41 patients were functionally dependent and the other 155 were functionally independent. Among the patients with preoperative dependence, 32 (78%) became functionally independent after surgery, and this conversion was associated with a lower baseline axial score (p = 0.012). Among the patients with preoperative independence, 21 (14%) developed postoperative dependence, and this conversion was associated with a higher baseline axial score (p = 0.013) and its smaller improvement (p < 0.001). Female sex (odds ratio [OR] 3.214; 95% confidence interval [CI] 1.210–8.542; p = 0.019) and a higher baseline axial score (OR 1.184; 95% CI 1.056–1.327; p = 0.004) significantly predicted the risk of postoperative functional dependence. Conclusions: We found that functional status following bilateral STN-DBS is closely related to preoperative axial symptoms. When loss of independence is a potential target for STN-DBS, clinicians should take into consideration the severity of axial impairment before surgery.

Abstract: This paper elaborates on a study on the implementation of a pedagogical agent. The agent is meant to improve and enhance the learnability of the serious game Lost Earth 2307. The game is analyzed in order to find the aspects that need to be improved (i.e. problems players might have with learning how to use and handle the game). Based on these findings the implementation of the pedagogical agent and the methods used are presented and elaborated. Furthermore, the question whether a pedagogical agent can improve the learnability of game mechanics in serious games compared to the “Standard form contract”-style tutorials used in Lost Earth 2307, is discussed. A study about the evaluation of the implemented pedagogical agent, its research question and the metrics used, is presented. The paper depicts the setup of the user study using SUS and NASA TLX questionnaires and report in detail about the results and discusses the outcomes.

Abstract: Government relationships can be complex and difficult to understand. The relationships between members of a legislature, bills, votes and lobbyists who promote various causes are important to understand in representative democracies, but difficult to retrieve using current methods. In this paper, we propose a 3D visualization system to explore such legislative relationships for users. We use real data from California state legislature obtained from the Digital Democracy project. We also conduct a 20 person user study to gauge the differences between traditional ways of looking up information versus our graph based methods. While not being as comprehensive, most users found our interactive visualizations more intuitive than regular web-based information retrieval.

Abstract: As social media and virtual communities increase in popularity, the spread of word of mouth becomes easier, challenging firms to measure and manage the success of marketing initiatives in online community environments. This research examines how consumers react to firms' active participation in consumer-to-consumer conversations in an online community setting. The authors develop a tailored community-matched measure of consumer reaction (consumer sentiment) and analyze more than 115,000 consumer posts from ten online forums with active firm participation. The results indicate that consumers show diminishing returns to active firm engagement, which, at very high levels, can undermine consumer sentiment. Further subgroup analyses by conversation type indicate that these relationships hold for conversations that address consumers' functional needs but do not hold for conversations that address social needs. Finally, the results show diminishing returns to firm engagement for consumers primarily interested in product-related support but show no relationship for consumers primarily interested in inspiration and entertainment. These findings provide insights for marketing performance measurement and resource allocation in online communities.

Abstract: BACKGROUND: In Europeans, 45 genetic risk variants for coronary artery disease (CAD) have been identified in genome-wide association studies. We constructed a genetic risk score (GRS) of these variants to estimate the effect on incidence and clinical predictability of myocardial infarction (MI) and CAD.METHODS: Genotype was available from 6041 Danes. An unweighted GRS was constructed by making a summated score of the 45 known genetic CAD risk variants. Registries provided information (mean follow-up = 11.6 years) on CAD (n = 374) and MI (n = 124) events. Cox proportional hazard estimates with age as time scale was adjusted for sex, BMI, type 2 diabetes mellitus and smoking status. Analyses were also stratified either by sex or median age (below or above 45 years of age). We estimated GRS contribution to MI prediction by assessing net reclassification index (NRI) and integrated discrimination improvement (IDI) added to the European SCORE for 10-year MI risk prediction.RESULTS: The GRS associated significantly with risk of incident MI (allele-dependent hazard ratio (95%CI): 1.06 (1.02-1.11), p = 0.01) but not with CAD (p = 0.39). Stratification revealed association of GRS with MI in men (1.06 (1.01-1.12), p = 0.02) and in individuals above the median of 45.11 years of age (1.06 (1.00-1.12), p = 0.03). There was no interaction between GRS and gender (p = 0.90) or age (p = 0.83). The GRS improved neither NRI nor IDI.CONCLUSION: The GRS of 45 GWAS identified risk variants increase the risk of MI in a Danish cohort. The GRS did not improve NRI or IDI beyond the performance of conventional European SCORE risk factors.

Abstract: Global manufacturing supply chains may link subsequent production facilities by intermodal transport operations, which are characterised by long transport times, scarce transport capacities and a given time table. Consequently only an integrated scheduling of production and intermodal transport operations may be able to materialise the competitive advantage of such a supply chain in terms of total cost and on time delivery reliability. The execution of this planning task is challenging for both supply chain professionals and scientists, since the underlying planning problem is NP-hard. This paper details a new methodological approach for solving integrated production and transport scheduling problems based on a graph, which allows a reformulation of the scheduling problem as a shortest path problem for each job, which can be solved in polynomial time. The proposed method is applied to a supply chain scenario, which contains a manufacturing facility in Brazil and shipments to customers in Germany. The obtained results show that the approach is suitable for the scheduling of large-scale problems and can be flexibly adapted to different real-world scenarios.

Abstract: Background Little is known about the incidence of severe critical events in children undergoing general anaesthesia in Europe. We aimed to identify the incidence, nature, and outcome of severe critical events in children undergoing anaesthesia, and the associated potential risk factors. Methods The APRICOT study was a prospective observational multicentre cohort study of children from birth to 15 years of age undergoing elective or urgent anaesthesia for diagnostic or surgical procedures. Children were eligible for inclusion during a 2-week period determined prospectively by each centre. There were 261 participating centres across 33 European countries. The primary endpoint was the occurence of perioperative severe critical events requiring immediate intervention. A severe critical event was defined as the occurrence of respiratory, cardiac, allergic, or neurological complications requiring immediate intervention and that led (or could have led) to major disability or death. This study is registered with ClinicalTrials.gov, number NCT01878760. Findings Between April 1, 2014, and Jan 31, 2015, 31â127 anaesthetic procedures in 30â874 children with a mean age of 6Â·35 years (SD 4Â·50) were included. The incidence of perioperative severe critical events was 5Â·2% (95% CI 5Â·0â5Â·5) with an incidence of respiratory critical events of 3Â·1% (2Â·9â3Â·3). Cardiovascular instability occurred in 1Â·9% (1Â·7â2Â·1), with an immediate poor outcome in 5Â·4% (3Â·7â7Â·5) of these cases. The all-cause 30-day in-hospital mortality rate was 10 in 10â000.
