Abstract

Deutschland hat eines der besten Gesundheitssysteme der Welt; dies gilt auch fur die medizinische Versorgung von Kindern und Jugendlichen. Aber wie lange noch? Eine uberalterte Arzteschaft bei gleichzeitig stark abnehmender Lebensarbeitszeit droht in wenigen Jahren das Versorgungssystem vollends aus der Balance zu bringen. Mehr Pravention, aber auch die Zunahme chronischer Erkrankungen und soziogener Entwicklungsstorungen erfordern zukunftig mehr denn je mehr Padiater. Daher mussen wir in Deutschland sofort mehr Arzte praxisnah aus- und weiterbilden und anders ausbilden. Dazu brauchen wir einen Ausbildungsabschnitt in der ambulanten Versorgung, denn dort werden die meisten Kinder und Jugendlichen behandelt. Denn ein Student, der heute sein Medizinstudium aufnimmt, wird fruhestens in 11 Jahren als Facharzt zur Verfugung stehen. Bis dahin sind die leistungsstarken Padiater der Babyboomergeneration allerdings langst im Ruhestand. Hochste Zeit also, dass gehandelt wird.

Abstract

This pilot study explored the international female (IF) students’ (n = 17) lived experiences of health care accessibility while studying in a small town in Canada. Analysis guided by a phenomenological method resulted in three major themes – (1) after arriving to attend university, IF students experienced challenges in staying healthy, such as learning how Canadian medical insurance works and how to access medical help, (2) IF students developed a support system over time, consisting of their university’s student advisors, host families and friends and (3) as IF students became familiar with living in Canada, they developed self-directed care strategies such as taking care of each other if they became sick. They also became comfortable reaching out through social media to ask questions and connect with others who were also new to Canada. Abstract

The muographic imaging of volcanoes relies on the measured transmittance of the atmospheric muon flux through the target. An important bias affecting the result comes from background contamination mimicking a higher transmittance. The MU-RAY and TOMUVOL collaborations measured independently in 2013 the atmospheric muon flux transmitted through the Puy de Dome volcano using their early prototype detectors, based on plastic scintillators and on Glass Resistive Plate Chambers, respectively. These detectors had three (MU-RAY) or four (TOMUVOL) detection layers of 1 m2 each, tens (MU-RAY) or hundreds (TOMUVOL) of nanosecond time resolution, a few millimeter position resolution, an energy threshold of few hundreds MeV, and no particle identification capabilities. The prototypes were deployed about 1.3 km away from the summit, where they measured, behind rock depths larger than 1000 m, remnant fluxes of 1.83±0.50(syst)±0.07(stat) m−2 d−1 deg−2 (MU-RAY) and 1.95±0.16(syst)±0.05(stat) m−2 d−1 deg−2 (TOMUVOL), that roughly correspond to the expected flux of high-energy atmospheric muons crossing 600 meters water equivalent (mwe) at 18° elevation. This implies that imaging depths larger than 500 mwe from 1 km away using such prototype detectors suffer from an overwhelming background. These measurements confirm that a new generation of detectors with higher momentum threshold, time-of-flight measurement, and/or particle identification is needed. The MU-RAY and TOMUVOL collaborations expect shortly to operate improved detectors, suitable for a robust muographic imaging of kilometer-scale volcanoes.

Abstract

ANTARES is a high-energy neutrino telescope installed in the Mediterranean Sea at a depth of 2475 m. It consists of a three-dimensional array of optical modules, each containing a large photomultiplier tube. A total of 2700 front-end ASICs named Analogue Ring Samplers (ARS) process the phototube signals, measure their arrival time, amplitude and shape as well as perform monitoring and calibration tasks. The ARS chip processes the analogue signals from the optical modules and converts information into digital data. All the information is transmitted to shore through further multiplexing electronics and an optical link. Abstract

full-scale mechanical prototype line was deployed to a depth of 2500 m to test the leak tightness of the electronics containers and the pressure-resistant properties of an electromechanical cable under evaluation for use in the ANTARES deep-sea neutrino telescope. During a month-long immersion study, line parameter data were taken using miniature autonomous data loggers and shore-based optical time domain reflectometry. Details of the mechanical prototype line, the electromechanical cable and data acquisition are presented. Data taken during the immersion study revealed deficiencies in the pressure resistance of the electromechanical cable terminations at the entry points to the electronics containers. The improvements to the termination, which have been integrated into subsequent detection lines, are discussed. The line also allowed deep-sea acoustic measurements with a prototype hydrophone system. The technical setup of this system is described, and the first results of the data analysis are presented. Abstract

ANTARES is a neutrino telescope being deployed in the Mediterranean Sea. It consists of a three dimensional array of photomultiplier tubes that can detect the Cherenkov light induced by charged particles produced in the interactions of neutrinos with the surrounding medium. High angular resolution can be achieved, in particular when a muon is produced, provided that the Cherenkov photons are detected with sufficient timing precision. Considerations of the intrinsic time uncertainties stemming from the transit time spread in the photomultiplier tubes and the mechanism of transmission of light in sea water lead to the conclusion that a relative time accuracy of the order of 0.5 ns is desirable. Accordingly, different time calibration systems have been developed for the ANTARES telescope. In this article, a system based on Optical Beacons, a set of external and well-controlled pulsed light sources located throughout the detector, is described. This calibration system takes into account the optical properties of sea water, which is used as the detection volume of the ANTARES telescope. Abstract

Application Specific Integrated Circuits, ASICs, similar to those envisaged for the readout electronics of the central calorimeters of detectors for a future lepton collider have been exposed to high-energy electromagnetic showers. A salient feature of these calorimeters is that the readout electronics will be embedded into the calorimeter layers. In this article it is shown that interactions of shower particles in the volume of the readout electronics do not alter the noise pattern of the ASICs. No signal at or above the MIP level has been observed during the exposure. The upper limit at the 95% confidence level on the frequency of fake signals is smaller than 1 × 10 − 5 for a noise threshold of about 60% of a MIP. For ASICs with similar design to those which were tested, it can thus be largely excluded that the embedding of the electronics into the calorimeter layers compromises the performance of the calorimeters.

Abstract

The CALICE collaboration is studying the design of high performance electromagnetic and hadronic calorimeters for future International Linear Collider detectors. For the hadronic calorimeter, one option is a highly granular sampling calorimeter with steel as absorber and scintillator layers as active material. High granularity is obtained by segmenting the scintillator into small tiles individually read out via silicon photo-multipliers (SiPM). A prototype has been built, consisting of thirty-eight sensitive layers, segmented into about eight thousand channels. In 2007 the prototype was exposed to positrons and hadrons using the CERN SPS beam, covering a wide range of beam energies and incidence angles. The challenge of cell equalization and calibration of such a large number of channels is best validated using electromagnetic processes. The response of the prototype steel-scintillator calorimeter, including linearity and uniformity, to electrons is investigated and described.

Objective: To investigate the pharmcokinetics and toxicity of high-dose topical colonic 5-aminosalicylic acid (5-ASA, mesalazine) therapy in patients with severely active total ulcerative colitis. Design: During colonoscopy, a tube was placed through the instrumentation channel into the caecum; 4 g 5-ASA (enema) was instilled twice daily through the tube for 3 weeks. Patients: Eight patients with severely active total ulcerative colitis were investigated. Methods: 5-ASA and acetylated 5-ASA were monitored in serum (2 days) and urine (5 days). Colonic transport of Tc 99m -labelled 5-ASA enema was measured by the nuclear imaging technique on the fifth day. 5-ASA toxicity was clinically and biochemically monitored

Abstract

In a 28-day non-blinded study of 1071 patients with functional dyspeptic symptoms in a general practice setting, 666 presented with mainly typical symptoms of functional dyspepsia and received 5 mg cisapride three times daily, while 405 with predominating symptoms indicative of gastroesophageal reflux received 10 mg cisapride three times daily. On the basis of an anamnestic risk factor analysis for organic lesions, 'low-risk' patients were to be treated directly with cisapride, while for 'high-risk' patients a more thorough gastrointestinal examination was recommended before starting cisapride. Of patients in the dyspepsia group 75% reported a good or excellent response; the corresponding rate was 80% in the reflux group. Low-risk patients in both groups tended to respond better than high-risk patients (mean difference, 11%). Patients and investigators reached identical assessments of response. Concomitant antacids, calcium antagonists, beta-blockers and sedatives did not affect the results, but concomitant NSAIDs reduced the mean improvement rate by 14% (p < 0.01). Abstract

Risk assessment of recurrent variceal bleeding is essential for therapeutic decisions and is usually performed by endoscopy of the upper gastrointestinal tract. We studied the value of portal venous duplex sonography in predicting subsequent variceal bleeding in patients with cirrhosis.Thirty patients with cirrhosis who received sclerotherapy because of acute variceal hemorrhage for the first time (hemorrhage group), 30 patients with cirrhosis who had no previous hemorrhage (nonhemorrhage group), and 30 control subjects were examined prospectively. With the use of portal duplex and color Doppler sonography, flow direction, flow velocity, vein diameter, and response to respiration of portal vein vessels were measured and portosystemic collaterals and thrombosis of portal vessels were visualized. The results of these measurements and imaging findings were combined into a Doppler sonoscore. At entry into the study, all patients were classified on the basis of a sonoscore as having a low (sonoscore,4) or a high (sonoscore,or = 4) risk for subsequent hemorrhage. During a mean follow-up period of 2 years (range, 15-36 months), the predictive value of this Doppler sonoscore was studied.In the hemorrhage group, the prevalence of recurrent hemorrhage was 40%, despite sclerotherapy, and the mortality rate was 60%. In patients with a Doppler sonoscore of 4 or more, the prevalence of recurrent hemorrhage was 67%, whereas in patients with a score less than 4, the prevalence was only 22% (p.02). After sclerotherapy, endoscopic criteria showed no significant correlation with the prevalence of bleeding. In the nonhemorrhage group, the prevalence of variceal hemorrhage occurring was 13%, and Doppler sonographic criteria showed no significant correlation with the prevalence of subsequent hemorrhage.We conclude that Doppler sonography, performed after the first occurrence of variceal hemorrhage, provides useful prognostic information regarding the risk of recurrent hemorrhage. If these results are confirmed, Doppler sonography may be used to select the best method of treatment.

Abstract

A precise detector alignment and absolute pointing is crucial for point-source searches. The ANTARES neutrino telescope utilises an array of hydrophones, tiltmeters and compasses for the relative positioning of the optical sensors. The absolute calibration is accomplished by long-baseline low-frequency triangulation of the acoustic reference devices in the deep-sea with a differential GPS system at the sea surface. The absolute pointing can be independently verified by detecting the shadow of the Moon in cosmic rays.

Abstract

“To be more specific, I am opposed to the introduction into the secondary school of the calculus in any form, of projective geometry, of analytic geometry as such, or of any special stress on the concept of function.” Thus Ralph Dennison Beetle1 of Dartmouth College rejects in part the recommendations of the National Committee, which say that “The one great idea which is best adapted to unify the course is that of functional relation” and which also include calculus in every plan of arrangement of material for the twelfth year. Having taught analytics and calculus as such, for the past four years to twelfth year students and having stressed the function concept in the eleventh and twelfth years, I am in direct opposition to Professor Beetle's opinion. In this article are presented some of the advantages that have resulted from this teaching which seem to make it desirable as a required part of the advanced mathematics in the secondary school.

The object of the study was to discover the relationship between mercury levels in maternal and cord blood. Paired whole blood samples, maternal and cord blood were analysed for total mercury content; 45 paired samples from non-fish-eating women in southern Saskatchewan demonstrated no significant differences in mercury levels between maternal and cord blood; 43 paired samples from fish-eating women in northern Saskatchewan demonstrated a significant difference, cord blood levels being higher. The significance is discussed.
