1. Impact of marketing mix elements on the brand equity of online food delivery services: an emerging economy perspective

Abstract: The online food delivery (OFD) industry has experienced significant growth and transformation, enabling businesses to operate in a dual-channel manner and diversify their revenue sources. The paper aims to investigate the impact of service marketing mixes on brand equity of Online Food Delivery Applications in an emerging economy, Bangladesh. Through the application of the positivist research philosophy, the study employs regression analysis to ascertain the influence of the 7 Ps of the service marketing mix on brand equity. The results empirically demonstrate the important contribution of the three 3 Ps to brand equity. Place, Promotion and Physical evidence have been found to significantly impact the brand equity of OFD platforms. The influences of other predictors (product, price, people and process) are weak and are not making a significant contribution to the prediction of brand loyalty towards online food delivery service. By fully embracing the potential of the services marketing mix, managers and enterprises can maximize the advantages offered by this expanding industry, foster a competitive advantage, and contribute to the development of a sustainable OFD market, which in turn benefits the broader business, society, and economy. This study is one of the first studies measuring the impact of service marketing mixes on brand equity in the growing food delivery marketplace.

Abstract: AbstractThis study addresses Bangladesh’s unemployment problem and its youth’s limited entrepreneurial aspiration focusing on critical factors that influence entrepreneurial intentions among university students in Bangladesh. Recognizing universities’ role in fostering entrepreneurship, the study aims to identify these determinants, bridging a gap in literature in the context of Bangladesh. Built upon an extended theory of planned behavior, the study incorporates ‘access to finance’ and ‘entrepreneurship program’ as mediators. This framework examines the relationships of ‘attitude’, ‘subjective norms’, ‘perceived behavioral control’, ‘entrepreneurship program’, ‘access to finance’, and their collective impact on entrepreneurial intentions. Utilizing stratified random sampling, the researchers collected data from 394 students across 23 selected Bangladeshi universities offering entrepreneurship programs encompassing Faculty of Business & Entrepreneurship and Faculty of Science & Engineering. The study employed Structural equation modeling (SEM) for analyzing the data. The findings revealed that there were positive relationships between ‘entrepreneurial intention’ and ‘attitude’, ‘subjective norms’, ‘perceived behavioral control’, ‘entrepreneurship program’, and ‘access to finance’. The findings have practical implications for Bangladesh’s government and educational institutes when it comes to promoting entrepreneurship programs extensively. The empirical results clearly demonstrate that the integration of access to finance and practical entrepreneurship programs significantly boosts the entrepreneurial aspirations of university students, encouraging them to pursue entrepreneurship as a career path. This, in turn, contributes to the advancement of economic sustainability in Bangladesh. These outcomes are consistent with the overarching aims of Sustainable Development Goal 8, which strives to foster continuous economic growth, generate employment prospects, and ensure decent work opportunities, particularly for the youth.

Abstract: Rubber and articles thereof commodities are one of Indonesia's leading exports, and natural rubber dominates among those commodities. However, in the last decade, the export performance of natural rubber commodities has not been optimal, especially in the last three years as seen from the declining export value. The purpose of the study was to determine the effect of production, international prices, and also the exchange rate on Indonesia's natural rubber exports, as well as to see which factors had a dominant influence. The analytical method used is multiple regression using secondary data for a period of 30 years (1991-2020), the export value of natural rubber commodities, production, international prices, and exchange rates. Result of this study, production, international prices, and exchange rates have a positive and significant impact on the export value of Indonesia's natural rubber commodities. The production, international prices, and exchange rates together have a significant effect on the export value of Indonesia's natural rubber commodities. The international price factor is the most dominating factor in influencing the export value of Indonesia's natural rubber commodities.

Abstract: We try to analyze the attitudes of taxpayers regarding tax system. We propose an approach which combines between theory and practice. In the first step, we present a normative approach; based on a theoretical study. In order to produce an empirical knowledge of an aspect of reality we present a descriptive approach; based on a survey of different categories of taxpayers. We have released some conditions of the model of expected utility and we have introduced a new parameter reflecting the efficiency of tax control. Previous studies are based on a maximal efficiency tax control (100%) which is a particular case of our model. We found that for Moroccan case, the fraud is related to the size of companies; it’s more important in the big companies than small and medium enterprises. The findings will help tax authorities to achieve their goals in fighting tax fraud.

Abstract: Purpose: Services for Persons with Disabilities (PWD) not only involve physical needs, but educational services, particularly in the area of religious education. This includes the study of the Quran and fardhu ain, which are crucial for PWD who practice Islam. However, special needs and specific methods must be considered to ensure that Islamic education can be effectively conveyed, whether by individuals or volunteer organizations. Since outreach efforts for PWD require a lot of manpower and specific skills, this study takes the approach that there is a need for youth volunteers who can provide services to PWD. While there are various types of volunteer organizations available, there are not many that focus on outreach efforts for PWD in Malaysia. This has led to a lack of understanding and appreciation of Islam among the PWD community. Therefore, the objective of this study is to foster sustainable services for the PWD community through youth volunteers by analyzing the key characteristics of volunteers using Fuzzy Delphi. Design/methodology/approach: This study uses a design method and a developmental research approach that has three phases. In this study, the researchers focused on the second phase of Fuzzy Delphi. 13 experts were involved in this study to rank the elements in developing a model for volunteer outreach efforts for PWD. Findings: The findings showed that all elements such as thinking, attitude, and skills, exceeded 70%, which is considered suitable, applicable and a must to the volunteers for PWD. Research limitations/implications: This research involves volunteers among typical person who service PWD. For future, it hopes research might be conducted among PWD who involve in volunteering service. Practical implications: These finding can be a reference to any development of training module for volunteers service especially which focusing on PWD. It is hoped that this findings could be as a guide for youth volunteers in teaching religious education for the PWD community. Abstract: The increasing fatality report of road accident in Malaysia is alarming over the last 10 years. The significant increase of private transportation possession especially in Selangor and Kuala Lumpur proven the significant relationship with the increase of road accidents between 2009 and 2018. Many programs have been carried out to address concerns on increasing traffic accidents under massive national plans throughout the nation. National Key Economic Area under a program called National Transport Policy, MRT project was introduced to strengthen the continuous effort for public transport sustainability. Nevertheless, since the launch of MRT until today, average ridership has not reached its target. Lack of research on the behavioral intention particularly towards the utilization of MRT in Malaysia provide an opportunity to empirically explore this in Malaysian context. The focus of this research was to determine the relationship between independent variable (timeliness) towards dependent variable (intention to use). Five hundred (500) questionnaires were distributed at selected MRT station around Kuala Lumpur and Selangor. Respondent was frequent user of MRT service and those commute on daily basis. Data were analyzed using SPSS version. Findings indicated that result for direct effect revealed significant in relationship between timeliness with intention to use thus hypothesis is accepted. The results proofed MRT service play important role as part of the transportation channel towards the target group with the direct impact contribution of timeliness as determinant factors of preference. The evolution of social media plays an important role in the overall decision-making process of the consumer as it has revolutionized the way people think, feel about, and consume things. Social media interaction is the most important method of shaping the decision of people and knowing what they feel and want from the business. The social media channels involve the content creations by the audience apart from that generated by the publishing houses or the media companies and even the social media channel such as Facebook, Twitter, YouTube, LinkedIn, Instagram, etc. This research explored how social media interactions play a significant role in consumer decision-making with a scope of Saudi Arabia user. With a total population of 35.08mil (GMI,2022), 27.08 million, or 79.25% are active social media users, either for personal or commercial usage through its' posting on their favorite types of content. Businesses can use this research paper to learn how the population of Saudi Arabia interacts on social media and how businesses can create strategies that appealing to customers' habits. A quantitative approach research questionnaires is developed to explore the relationship between social media interaction and consumer decision-making by using different dimensions of social media interactions. The purpose of this preliminary study is to determine the reliability and validity of the instrument used and feasible for full-scale analysis of the studied phenomenon. PLS-SEM is employed in this preliminary study with seventy-five respondents from a few regions has been selected to response on the questionnaires through simple random sampling method. The result of PLS-SEM analysis on the measurement model had showed a robust analysis of internal reliability and validity. Assessment of items and constructs included in the study exhibits a good internal consistency and valid, thus, reflects an acceptable research model that are feasible and ready a full-scale analysis. Abstract: Competition among private universities in Malaysia seems to be getting fierce and tough. The number of service providers increased, which provides more advantages to the students. Students have more bargaining power due to ample choices and options before deciding. This study measures three independent factors: brand awareness, brand image and perceived brand quality of higher education institutes toward student choices for enrolment. Three hypotheses were developed to measure a direct relationship between the variables toward student intentions to register. Data were collected among the new students from the private universities within Klang valley. 5 Likert scales were used to measure respondents' feedback. Abstract: Middle East countries have experienced a decline in female labor force participation over the last three decades. Indeed, male labor force participation rates remain higher than that of female labor force, making the gender gap in labor force participation is high. This study empirically investigates the possible determinants of female labor force participation in the Middle East countries. The determinants examined in this study included, fertility rate, female primary education, female secondary education, female tertiary education, urban population, female unemployment, and GDP per capita. For this purpose, the study utilizes the three alternative models of static panel data comprising of the pooled ordinary least squares, random effects, and fixed effects. After obtaining a convenient model, the analysis proceeds with the diagnostic tests to check for multicollinearity, heteroskedasticity, and serial correlation, which, together, make the method reliable. The study comprises the annual panel data for ten Middle East countries covering the period from 1996 to 2018. The data were taken from the World Bank online database, World Development Indicators. The empirical results of this study indicate that female tertiary education has positive and significant impact on female labor force participation rate, while fertility rate, female secondary education, and urbanization rate have negative and significant impact on female labor force participation rate. These results reflect the need for the government to provide tertiary education facilities to female students, and encourage them to pursue their tertiary education in order to increase participation of female in labor force. This is particularly significant as the importance of tertiary education is undeniable in terms of generating highly skilled workers and the declining fertility rate. Moreover, policies in the region should remove barriers to women's labor force participation and at the same time, encourage women's active engagement in the labor market for the achievement of economic benefits. Abstract: This empirical research aims to look at the factors that mark the financial performance of Islamic and traditional banking sector in Pakistan. The results of the data signify that gross domestic product and inflation is connected to the financial performance of Islamic banks in all aspects of profitability ratios and gross domestic product has insignificant relation with the return on equity and assets of conventional banks. Inflation has a weighty effect on return of assets of conventional banks. Further results indicate a negative impact of bank size on return of assets Islamic and positive impact on conventional banks. Abstract: This work proposes an optimal pricing scheme for provisioning sensors-as-a-service (Se-aaS) for catering to applications with multi-tenancy requirements in a sensor-cloud platform. The scheme orchestrates a trade-off analysis between communication range and price in a sensor-cloud platform with range-reconfigurable nodes. The proposed scheme consists of two phases -- (a) selection of a neighbor node of a source node, and determination of optimal price for the selected neighbor node. In the first phase, a source node adjusts its communication range and selects its best possible neighbor node using selectivity factor of all the neighbor nodes. In the second phase, we design a utility function to determine the optimal price of the selected neighbor node. We use the Lagrangian function to model the proposed problem as a mixed-integer linear program (MILP) and obtain the optimal solution using the Karush-Kuhn-Tucker (KKT) conditions. The existing works on pricing in sensor-cloud are deficient in considering the presence of reconfigurable communication range of sensor nodes. Extensive experimental results show that the proposed scheme performs better compared to the existing pricing schemes for sensor-cloud. In particular, the proposed scheme is capable of reducing the charged price by 10.55%, as compared to the existing pricing scheme, DOPH.

Abstract: OBJECTIVES: Given the rising prevalence of cisplatin-induced peripheral neuropathy (CisIPN), investigations to alleviate its adverse effects are required. Oxidative stress and free radical development are essential pathways of CisIPN. Specifically, dexamethasone and citicoline are characterized by anti-inflammatory and antioxidant activities that might reduce CisIPN incidence and severity. The current study assessed the possible impacts of novel interventions, dexamethasone, and, citicoline on CisIPN. MATERIALS AND METHODS: Seventy-two male mice were randomly allocated into nine groups (n: 8/each group). Different doses of dexamethasone (7.5, 15, 30 mg/kg, i.p.), citicoline (10, 20, 40 mg/kg, i.p.), and the combination (dexamethasone 7.5 mg/kg + citicoline 10 mg/kg, i.p.) were injected in the first three days and one day before receiving cisplatin (2 mg/kg, i.p.). The tail-flick method was used for assessing nociception. Abstract: BackgroundTo date, there is no valid and reliable instrument to specifically evaluate the married adolescent women's SRH needs. Hence, the aim of this study was to develop and evaluate the psychometric properties of a questionnaire for assessing Iranian married adolescent women's SRH needs. MethodsThe current exploratory sequential mixed method study was performed in two phases. In the first phase, a preliminary questionnaire was developed based on in-depth interviews with 34 married adolescent women and 4 key informants as well as a comprehensive literature review. In the second phase, validity of the questionnaire was assessed using face, content, and construct validity and reliability of the questionnaire was assessed using internal consistency and test-retest reliability.ResultsBased on qualitative content analysis and literature review, 137 items were extracted. After several modifications of the generated items, a 108-item questionnaire was prepared for the psychometric process. After checking face and content validity, 85 items remained in the study. In the exploratory factor analysis, eleven items were removed and the remaining 74 items were categorized into nine factors. The Cronbach's alpha coefficient and the Intraclass correlation coefficient were found to be 0.878 and 0.99 for whole scale, respectively.Conclusions The 74-item questionnaire has acceptable validity and reliability. Therefore, it can be used as an appropriate instrument for assessing married adolescent women's SRH needs.

The research process begins with design, needs analysis, machine rebuilding and electrical assembly, machine function testing, analysis of filament, and filament testing in a 3D printing machine. The categories of shredded plastic material were plastic cups (polypropylene, PP) and a mixture of plastic bottles (polyethylene terepththalate, PET) and plastic cups (polypropylene, PP). The analysis of the research was the capacity of the extrusion machine, the best temperature in producing filaments based on shapes and sizes, and testing of 3D printing filaments of plastic waste which was applied to the 3D printing machine. The result showed that 190°C was the greater temperature to heat the barrel, the machine capacity of each plastic waste category, and the characteristic plastic waste was almost similar compared to the market filament of polylactic acid (PLA) in terms of filament size and 3D printing machine parameter.

Abstract: As wearable microelectronics become more ubiquitous, and the size and power requirements of such devices decrease, there is increasing research interest in harnessing power from ambient environmental sources through embedded systems as alternative to battery replacements. Accordingly, a multimodal hybrid piezo-electromagnetic insole energy harvester (PEM-IEH) has been presented in this paper as a means to reclaim the biomechanical energy wasted in the surroundings during daily walking. The hybrid device consists of two piezo-ceramic wafer plates, two magnets, and two wound coils. The designed harvester has been simulated, fabricated and experimentally validated. From the frequency response, the hybrid harvester exhibits four resonant frequencies concentrated around 8, 25, 50, and 51 Hz. A maximum combined power of 1400 µW is generated across the optimal load resistances of upper and lower electromagnetic generators at first resonant frequency (8 Hz) under 0.5 g base acceleration, and a 269 µW peak power is obtained across the optimum load resistances of the upper and lower piezoelectric generators at 4th resonance (51 Hz) under 0.5 g. The miniature (46.8 cm3) and lightweight (43.3 g) harvester was incorporated into the sole of an adult-sized commercial shoe, and has been shown to be able to charge a 100 µF capacitor, up to 2.4 Volt within approximately 10 min of slow jogging. The capacitor charging performance presents a remarkable potential application of the harvester in development of micro-power monitoring sensors and wearable microelectronic gadgets where batteries pose a practical bottleneck.

Abstract: Reading is one of the activities known to give a lot of benefits to readers. In the context of university students, reading can improve their academic performance. However, literature indicated that students read-only to pass their examination and not for pleasure and acquiring knowledge. The study reported in this paper investigated the reading habit and reading attitudes among students in the research university library in Malaysia. It is believed that positive reading attitudes towards education materials or non-education materials lead to positive reading experiences, which also lead to higher academic performance. A descriptive study used questionnaires as an instrument of a quantitative method. Convenience sampling was employed and 400 respondents from four research university status libraries were selected. Moderate levels of reading attitude were obtained from the result of the study, in which several variables produced a significant relationship in the reading attitude (education background and favorite activities during leisure time). The findings of this study have shown the behavior of academic library users in terms of reading habits, reading attitudes, and selection of reading materials. Several suggestions such as providing a variety of reading material (sense of humor and humanities, public figures and tv personalities) and reading encouragement activities have been suggested. This study provides useful inputs to the Ministry of Higher Education (MOHE) in constructing effective strategies to cultivate reading habits among reluctant readers in public universities.

Abstract: BACKGROUND The neutrophil to lymphocyte ratio (NLR) has been reported to be associated with clinical outcomes of patients with severe traumatic brain injury (TBI). This study aimed to evaluate the correlation between the dynamics of NLR and clinical outcomes of pediatric patients with moderate to severe TBI. Abstract: Diffuse optical tomography (DOT) leverages near-infrared light propagation through tissue to assess its optical properties and identify abnormalities. DOT image reconstruction is an ill-posed problem due to the highly scattered photons in the medium and the smaller number of measurements compared to the number of unknowns. Limited-angle DOT reduces probe complexity at the cost of increased reconstruction complexity. Reconstructions are thus commonly marred by artifacts and, as a result, it is difficult to obtain an accurate reconstruction of target objects, e.g., malignant lesions. Reconstruction does not always ensure good localization of small lesions. Furthermore, conventional optimization-based reconstruction methods are computationally expensive, rendering them too slow for real-time imaging applications. Our goal is to develop a fast and accurate image reconstruction method using deep learning, where multitask learning ensures accurate lesion localization in addition to improved reconstruction. We apply spatial-wise attention and a distance transform based loss function in a novel multitask learning formulation to improve localization and reconstruction compared to single-task optimized methods. Given the scarcity of real-world sensor-image pairs required for training supervised deep learning models, we leverage physics-based simulation to generate synthetic datasets and use a transfer learning module to align the sensor domain distribution between in silico and real-world data, while taking advantage of cross-domain learning. Applying our method, we find that we can reconstruct and localize lesions faithfully while allowing real-time reconstruction. We also demonstrate that the present algorithm can reconstruct multiple cancer lesions. The results demonstrate that multitask learning provides sharper and more accurate reconstruction.

Abstract: Background : Invasive fungal infection (IFI) remains a common complication after lung transplantation, causing significant morbidity and mortality. We have attempted to quantify systematically risk factors of IFI in lung transplant recipients. Methods : Studies were retrieved from Ovid MEDLINE, Ovid Embase, Cochrane database of systematic reviews and Cochrane central register of controlled trials. All case-control and cohort studies evaluating the risk factors of IFI in adult lung transplant recipients were screened. Two researchers reviewed and assessed all studies independently. We pooled the estimated effect of each factor associated with IFI by using a random effect model. Results : Eight studies were included in the systematic review and five studies were eligible for the meta-analysis. Rates of IFI range from 8-33% in lung transplant recipients. Independent risk factors for invasive aspergillosis (IA) in lung transplantation include previous fungal colonization (OR 2.44; 95% CI 0.08-0.47), cytomegalovirus infection (OR 1.96; 95% CI 1.08-3.56) and single lung transplantation (OR 1.77; 95% CI 1.08-2.91). Pre-emptive antifungal therapy (OR 0.20; 95% CI 0.09-0.48) is a protective factor for IA in lung transplant (OR 0.2; 95% CI 0.08-0.47). Conclusion : Cytomegalovirus infection, previous fungal colonization and single lung transplantation independently increase the risk of IA in lung transplant recipients. Pre-emptive antifungal therapy is a protective factor for IA in the lung transplant population.

Abstract: The mineral matter contained in feedstocks has a generally limiting impact on the process design of high-temperature conversion processes. In the present study, the composition, mineral phases, and process-relevant properties of ashes are investigated by different experimental and modeling methods, which were reviewed in the literature regarding the frequently applied methods. Various analyses are exemplarily performed for the ashes of a high-sodium coal from China, generated at temperatures of 150–950 °C. X-ray fluorescence (XRF) analysis, microwave-assisted inductively-coupled plasma optical emission spectrometry (MW-ICP-OES), and the same technique with electrothermal vaporization (ETV-ICP-OES) are applied to analyze the chemical composition of the bulk material. The chemical composition of the near-surface region is studied by scanning electron microscopy with energy-dispersive X-ray spectroscopy (SEM-EDX). Mineral phases are analyzed by X-ray diffraction (XRD) and thermochemical calculations. The process-relevant ash fusion behavior is studied by a common ash fusion test (AFT) and thermomechanical analysis (TMA) and supported by thermochemical calculations. The different ashing temperatures have a recognizable impact on the composition, formation and transformation of mineral phases, and resulting ash fusion behavior, while each property is monitored by at least two different methods. For this purpose, a detailed analysis of the results achieved by the individual methods is performed. Finally, the results obtained by different methods for the same ash property are compared for monitoring the validity of the results and, for example, extracting about the gas phase transfer of selected ash components.

Abstract: Objectives Low back pain is a major health issue in most industrialized countries. Lumbar fascia is supported as a potential source of pain in the lumbar region. Myofascial release is a manual therapeutic approach that focuses on restoring altered soft tissue function. On the other hand, one of the most commonly used physical therapy methods for low back pain is electrotherapy. The purpose of this study was to compare the effect of lumbar Myofascial release and electrotherapy on clinical outcomes of Non-specific low back pain and elastic modulus of lumbar myofascial tissue. Design Randomized, clinical trial. Setting Outpatient Low back pain clinic. Subjects 32 subjects with low back pain. Interventions Subjects were randomized into the myofascial release group (n = 16) and electrotherapy group(n = 16). Subjects in the myofascial release group received 4 sessions of myofascial release in the lumbar region, and the electrotherapy group received 10 sessions of electrotherapy. Main measures Low back pain severity, and elastic modulus of the lumbar myofascial tissue were assessed before and after treatment. Results An independent sample T-test was used to compare baseline variables in both groups (p > 0.05) (effect size≥0.83), Paired T-test was used to compare within-group changes after performing myofascial release and electrotherapy (p ≤ 0.023) (effect size≥0.56), and the GLM Anova test was used to Comparison of Changes in the Elastic Modulus of the Lumbar Spine and Low Back Pain between-group (F (10,21) = 12.10, P Conclusion The improvements in the outcome measures suggest that lumbar myofascial release may be effective in subjects with non-specific low back pain. Data suggest that the elastic modulus of lumbar fascia and the severity of low back pain are directly linked. Decreasing the elastic modulus after myofascial release can directly affect reducing low back pain.

Abstract: Rare genetic neurological disorders (RND; ORPHA:71859) are a heterogeneous group of disorders comprising >1700 distinct genetic disease entities. However, genetic discoveries have not yet translated into dramatic increases of diagnostic yield and indeed rates of molecular genetic diagnoses have been stuck at about 30–50% across NGS modalities and RND phenotypes [1, 2]. Existence of yet unknown disease genes as well as shortcomings of commonly employed NGS technologies and analysis pipelines in detecting certain variant types are typically cited to explain the low diagnosis rates. To increase the diagnostic yield in RNDs - one of the four focus disease groups in Solve-RD - we follow two major approaches, that we will here present and exemplify: (i) systematic state-of the art re-analysis of large cohorts of unsolved whole-exome/genome sequencing (WES/WGS) RND datasets; and (ii) novel-omics approaches. Based on the way Solve-RD systematically organizes researchers’ expertise to channel this approach [3], the European Reference Network for Rare Neurological Diseases (ERN-RND) has established its own Data Interpretation Task Force (DITF) within SOLVE-RD, which is currently composed of clinical and genetic experts from 29 sites in 15 European countries. The Solve-RD project has received funding from the European Union’s Horizon 2020 research and innovation programme under Grant Agreement No. 779257. 36. Factors Affecting Generalization of Ocular Myasthenia Gravis in Palembang

Abstract: Introduction. Ocular myasthenia gravis (OMG) is an autoimmune disease which is characterized by weakness of extraocular muscles, levator palpebrae and orbicularis oculi, resulting in ptosis and binocular diplopia. Nearly all patients present with eyelid and extra ocular muscles involvement. Approximately 30% to 80% of patients with OMG experience a conversion to generalized myasthenia gravis (GMG) within 2 years. There are not only have ptosis and diplopia but also limb weakness,bulbar symptoms, or even respiratory failure. This study was aimed to observe the clinical features of OMG to GMG and risk factors and median time to conversion of OMG to GMG of myasthenia gravis patients in Mohammad Hoesin General Hospital Palembang. Method. This study is a cohort retrospective study and the data were collected from the medical records of 91 patients who were registered as myasthenia gravis patients during September 2018 to March 2020. Sosiodemographic and clinical characteristics, including onset of OMG to GMG, history of smoking, presence of thymic abnormalities, and medications received were reviewed retrospectively. Result. A total of 91 OMG patients were observed in this study with 32 (35,2%) patients converted from ocular myastenia gravis to general myastenia gravis. Median conversion time to GMG was 34 months. Risk factor for convertion cases of OMG to MGG was receiving immunosupressive agents (Risk: 14.7, 95% CI 4.83, 44.7), thymus hyperplasia (Risk: 3.36, CI 95% 0.33, 33.6), Female (Risk: 2.41, 95% CI 0.94, 6.17), Smoking (Risk: 1.56, 95% CI 0.31, 7.81). Conclusion. Ptosis was the definitive sign for OMG in this study, with all patients had ptosis, thus it needs the colaboration from neuroophthalmologist and neurologist to diagnose and manage this case. Most of converted case was female and those who receive an immunosupressive agent therapy.

Abstract: Introduction: Knee Osteoarthritis (OA) is a weight-bearing joint disease and is more common in overweight and obese persons. The objective of the study was to assess the feasibility and acceptability of Instructions of Daily Care (IDC) on pain, mobility, and Body Mass Index (BMI) among knee OA participants who are overweight or obese. Material and Method: The study was an open-label randomized controlled trial of six weeks. Forty overweight and obese participants with knee OA were randomly divided into two groups by a computer-generated number. The participants in the Instruction Group (IG) were provided with leaflets explaining IDC for the duration of six weeks. Both groups were instructed to take low doses of the non-steroid anti-inflammatory drug (NSAIDs) on alternate days. The outcome measures were pain, mobility and BMI. The feasibility and acceptability of knee pain and mobility were assessed using a questionnaire designed by experts in rehabilitation. Results: Participants in the IG reported more statistically significant pain relief as assessed by the Western Ontario and McMaster Universities Osteoarthritis Index score (p=0.001) and improvement in mobility (p=0.000) assessed by the Timed Up and Go test score after six weeks compared to the Control Group (CG). Both groups did not demonstrate any significant change in BMI (p-value > 0.05). The results of descriptive statistics showed a significantly higher satisfaction score for participants who received a combination of IDC and NSAIDs, indicating an acceptable intervention. Conclusion: The IDC is effective and acceptable in terms of improving pain and mobility and should be recommended as the usual care of treatment.

38. Omega-3 as an Adjunctive Therapy of Sertraline and Venlafaxine Substantially Improves Symptoms of Major Depressive Disorder: A Double-blind, Placebo-controlled Study

Abstract: Background: Due to the possible effect of omega-3 fatty acids on reducing depressive symptoms, in this study, we investigated these effects in combination with other antidepressants. Methods: The study was a double-blind clinical trial on 100 patients with major depressive disorder who were divided into four groups of 25 each and treated with 50 mg daily sertraline plus placebo, 50 mg daily sertraline plus two grams Omega 3 daily, 75 mg daily venlafaxine plus placebo, and 75 mg daily venlafaxine plus 2 g Omega 3 daily for 6 weeks. Results: The mean Hamilton depression rating score of sertraline and venlafaxine plus omega-3 after treatment were 4.42 and 4.23 respectively versus sertraline and venlafaxine plus placebo 14.4 and 14.2 respectively (P value=0.0001). Conclusion: Omega-3 enhanced the clinical function of sertraline and venlafaxine to reduce the severity of depression. Adding omega-3 to either sertraline or venlafaxine does not have a comparative advantage over each other in terms of the improvement of severity of depressive symptoms.

This activity aims to provide educative training on how to carry out reforestation in narrow areas and an effort to increase public awareness in dealing with plastic waste that continues to grow. The method used is to educate the orphanage children, explain and understand waste, impacts, 3R materials, and practice processing plastic bottles into "POTS." As a result of the essential nature of the training, the results achieved are pretty good, considering that making pots from used plastic bottles requires skill and perseverance from the participants

Abstract: Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. Abstract: Background and study aims: Chronic hepatitis C virus (HCV) infection has always been identified as a major health threat and a potential cause of liver cirrhosis , portal hypertension , and other associated problems. The introduction of direct-acting antiviral agents (DAAs) has represented a paradigm shift in HCV management. In this study, we aim to observe the rate of sustained virologic response (SVR12) in a large scale of patients at a single center as well as record the post-treatment changes in the hematologic, hepatic, and renal biochemical profiles. Patients and methods In total, 1933 chronic HCV genotype 4 mono-infected non-HCC patients who completed the treatment with six different DAA regimens in the Faculty of Medicine, Ain Shams University Research Institute (MASRI), were retrospectively enrolled in this study. The rate of sustained virologic response after 12 weeks off-therapy (SVR12) was assessed. The baseline characteristics to predict the SVR12 were then analyzed. The post-treatment changes in many profiles were recorded and analyzed. Results The overall SVR12 rate was 96.2% (after excluding 84 cases who were lost to follow-up). It was achieved in 346/375 patients (92.3%), 466/477 patients (97.7%), 60/62 patients (96.8%), 11/11 patients (100%), 532/545 patients (97.6%), and 445/463 patients (96.1%) who received sofosbuvir/daclatasvir (SOF/DCV), sofosbuvir/daclatasvir/ribavirin (SOF/DCV/RBV), sofosbuvir/ledipasvir (SOF/LDV), sofosbuvir/ledipasvir/ribavirin (SOF/LDV/RBV), sofosbuvir/simeprevir (SOF/SMV), and ombitasvir/paritaprevir/ritonavir/ribavirin (OBV/PTV/r + RBV), respectively. In total, 73 patients (3.8%) failed to achieve SVR12. The baseline aspartate aminotransferase (AST), cirrhotic status, and treatment regimen were determined to have a significant impact on SVR12. In the overall treated population, the levels of serum AST, alanine aminotransferase , albumin , creatinine, bilirubin , and hemoglobin and platelet count improved significantly after treatment. Furthermore, sustained virologic response was strongly related to cirrhosis and its degree. Conclusion The interferon-free DAA regimens offered high SVR12 rates in Egyptian patients with chronic HCV infection. They were associated with a significant improvement in the hematologic, hepatic, and renal biochemical profiles. The baseline AST, liver cirrhosis, and treatment regimen might have an impact on achieving SVR.

Abstract: We present a quantitative performance analysis of a wide range of state-of-the-art object detection models, such as Mask R-CNN He et al.(2017)[8], RetinaNet Lin et al.(2017)[17] and EfficinetDet Tan et al.(2019)[28] in haze affected environments. This work uses two key performance metrics (Mean Average Precision and Localised Recall Precision) to provide a nuanced view of real world performance of these models in an on-road driving application. Our findings show that the presence of haze further exacerbates the performance differences between single-stage and multi-stage detection models. In addition, not all aspects of the model performance are affected equally. The inclusion of Local Recall Precision (LRP) Oksuz et al.(2018)[21] suggests that more recent models have much improved localisation performance even with similar false negative and false positive results. We also highlight some of the inherent limitations of Neural Network based approaches that could be addressed by Bayesian Neural Networks in the future.

Abstract: Mixed-signal machine-learning classification has recently been demonstrated as an efficient alternative for classification with power expensive digital circuits. In this paper, a single-MOSFET analog multiplier is proposed for classifying high-dimensional input data into multi-class output space with less power and higher accuracy than state-of-the-art mixed-signal linear classifiers. A high-resolution (i.e., multi-bit) multiplication is facilitated within a single-MOSFET by feeding the features and feature weights into, respectively, the body and gate inputs. High-resolution classifier that considers the decisions of the individual predictors is designed at 180nm technology node and operates at 100MHz in near/subthreshold region. To evaluate the performance of the classifier, a reduced MNIST dataset is generated by downsampling the MNIST digit images from 784 features to 48 features. The system is simulated across a wide range of PVT variations, exhibiting average accuracy of 92% (2% improvement over state-of-the-art), energy consumption of 67.3 pJ per classification (over 8 times lower than state-of-the-art classifiers), area of 27,570 μm2 per binary classifier, and a stable response under PVT variations. Finally, to provide ground for future work on ultra-low-power deep and convolutional networks, scalability and robustness of the proposed multiplier is evaluated with a convolutional neural network on CIFAR-10. Similar classification accuracy with digital and SMART hardware has been observed. Abstract: PURPOSE Genomic tests improve accuracy of risk prediction for early breast cancers but these are expensive. This study evaluated the clinical utility of EndoPredict®, in terms of impact on adjuvant therapy recommendations and identification of parameters to guide selective application. METHODS Patients with ER-positive, HER2-negative, and early-stage invasive breast cancer were tested with EndoPredict®. Two cohorts were recruited: one consecutively and another at clinical team discretion. Abstract: With the volume of data increasing exponentially, there is a growing interest in helping people to benefit from their data regardless of its poor quality. For instance, data with an imbalanced distribution has a negative effect on the performance achieved by most traditional classification techniques. This paper proposes TGT (Train Generate Test), a novel oversampling technique for handling imbalanced datasets problem. Using different learning strategies, TGT guarantees that the generated synthetic samples reside in minority regions. Abstract: International audience; This paper aims to establish an intelligent procedure that combines the observational method with the existing deep learning technique for updating deformation of braced excavation in clay. The gated recurrent unit (GRU) neural network is adopted to formulate the forecast model and learn the potential rules in the field observations using the Nesterov-accelerated Adam (Nadam) algorithm. In the proposed procedure, the GRU-based forecast model is first trained based on the field data of previous and current stages. Then, the field data of the current stage are used as input to predict the deformation response of the next stage via the previously trained GRU-based forecast model. This updating process will loop up till the end of the excavation. This procedure has the advantage of directly predicting the deformation response of unexcavated stages based on the monitoring data. The proposed intelligent procedure is verified on two well-documented cases in terms of accuracy and reliability. The results indicate that both wall deflection and ground settlement are accurately predicted as the excavation proceeds. Furthermore, the advantages of the proposed intelligent procedure compared with the Bayesian/optimization updating are illustrated.

Abstract: Background Despite the lack of official COVID-19 statistics, various workplaces and occupations have been at the center of COVID-19 outbreaks. We aimed to compare legal measures and governance established for managing COVID-19 infection risks at workplaces in nine Asia and Pacific countries and to recommend key administrative measures. Methods We collected information on legal measures and governance from both general citizens and workers regarding infection risks such as COVID-19 from industrial hygiene professionals in nine countries (Indonesia, India, Japan, Malaysia, New Zealand, Republic of the Philippines, Republic of Korea, Taiwan, and Thailand) using a structured questionnaire. Results A governmental body overseeing public health and welfare was in charge of containing the spread and occurrence of infectious diseases under an infectious disease control and prevention act or another special act, although the name of the pertinent organizations and legislation vary among countries. Unlike in the case of other traditional hazards, there have been no specific articles or clauses describing the means of mitigating virus risk in the workplace that are legally required of employers, making it difficult to define the responsibilities of the employer. Each country maintains own legal systems regarding access to the duration, administration, and financing of paid sick leave. Many workers may not have access to paid sick leave even if it is legally guaranteed. Conclusion Specific legal measures to manage infectious disease risks, such as providing proper personal protective equipment, education, engineering control measures, and paid sick leave are recommended to be stipulated in Industrial safety and health-related acts.

Abstract: Critical illness could result in altered pharmacokinetic profiles of many commonly administered medications in ICU settings. Pharmacokinetic, which consists of a multistage process containing absorption, distribution, metabolism, and elimination, could be significantly altered in critically ill patients that emphasize the importance of individualized pharmacotherapy. The most important pharmacokinetic parameters that would be altered in critically ill patients are volume of distribution, clearance, trough concentration, peak concentration, and area under the curve. In this review, the most commonly administered drugs in critically ill patients have been discussed in detail based on the pharmacokinetic aspects of drugs and patients. The contents of this paper have been designed as a brief guideline for professionals who are practicing in ICU settings. Clinical data regarding precise drug dosing, renal dose adjustments, and dialyzability considerations have been included. The major goal of this review was to summarize the commonly prescribed drugs in ICU to facilitate the accessibility of various data need during the management of ICU admitted patients by the professional in this unit to minimize drug dosing errors.

Abstract: OBJECTIVES Data regarding enhanced recovery after thoracic surgery (ERATS) are sparse and inconsistent. This study aims to evaluate the effects of implementing an enhanced ERATS programme on postoperative outcomes, patient experience and quality of life (QOL). METHODS We conducted a prospective, longitudinal study evaluating 9 months before (pre-ERATS) and 9 months after (post-ERATS) a 3-month implementation of an ERATS programme in a single academic tertiary care centre. All patients undergoing major thoracic surgeries were included. The primary outcomes included length of stay (LOS), adverse events (AEs), 6-min walk test scores at 4 weeks, 30-day emergency room visits (without admission) and 30-day readmissions. The process-of-care outcomes included time to ‘out-of-bed’, independent ambulation, successful fluid intake, last chest tube removal and removal of urinary catheter. Perioperative anaesthesia-related outcomes were examined as well as patient experience and QOL scores. RESULTS The pre-ERATS group (n = 352 patients) and post-ERATS group (n = 352) demonstrated no differences in demographics. Post-ERATS patients had improved LOS (4.7 vs 6.2 days, P CONCLUSIONS ERATS implementation was associated with improved LOS, expedited feeding, ambulation and chest tube removal, without increasing AEs or readmissions, while maintaining a high level of patient satisfaction and QOL.
