The importance of accurate structural discrimination of the human grey matter regions has motivated the development of observer-independent reproducible methods that account for inter-individual architectonic variations. We introduce a non-invasive statistical residual analysis framework, employing unique tissue-specific magnetic resonance fingerprinting (MRF) signals after adjusting for the effect of T1 and T2* MR relaxometry parameters (here termed MRF residuals). A 7 T Siemens MR scanner was used to acquire MRF signals, quantitative transmit magnetic field (B1+) maps and T1-weighted anatomical images of eleven cortical areas (5L, 5M, 5Ci, 7A, 7P, 7PC, hIP3, BA2, BA4a, BA4p and BA6) from six female participants. MRF residual signal for each voxel was calculated as the difference between the actual and best matching MRF signal evolutions from a precomputed MRF dictionary covering a range of T1, T2* and B1+ values. To compare MRF residuals between regions of interest, normalised autocorrelation was used as a shape-based statistical signal characterisation method and the Euclidean distance between autocorrelation profiles of residuals was used to measure the interareal dissimilarity. In the eleven cortical areas in both cerebral hemispheres of six participants, the proposed MRF residual analysis consistently showed interareal dissimilarity profiles that concorded with histological studies, indicating that MRF residuals potentially contain tissue microstructural information. MRF residual signals provide additional area-specific information that is complementary to the MR relaxometry-based (T1, T2*) information used previously for distinguishing microstructural differences between human cerebral cortex regions in vivo. The proposed approach led to more accurate identification of structural variations across cortical areas of interest.

PURPOSE: Quantitative susceptibility mapping (QSM) estimates the spatial distribution of tissue magnetic susceptibilities from the phase of a gradient-echo signal. QSM algorithms require a signal mask to delineate regions with reliable phase for subsequent susceptibility estimation. Existing masking techniques used in QSM have limitations that introduce artifacts, exclude anatomical detail, and rely on parameter tuning and anatomical priors that narrow their application. Here, a robust masking and reconstruction procedure is presented to overcome these limitations and enable automated QSM processing. Moreover, this method is integrated within an open-source software framework: QSMxT. METHODS: A robust masking technique that automatically separates reliable from less reliable phase regions was developed and combined with a two-pass reconstruction procedure that operates on the separated sources before combination, extracting more information and suppressing streaking artifacts. RESULTS: Compared with standard masking and reconstruction procedures, the two-pass inversion reduces streaking artifacts caused by unreliable phase and high dynamic ranges of susceptibility sources. It is also robust across a range of acquisitions at 3 T in volunteers and phantoms, at 7 T in tumor patients, and in an in silico head phantom, with significant artifact and error reductions, greater anatomical detail, and minimal parameter tuning. CONCLUSION: The two-pass masking and reconstruction procedure separates reliable from less reliable phase regions, enabling a more accurate QSM reconstruction that mitigates artifacts, operates without anatomical priors, and requires minimal parameter tuning. The technique and its integration within QSMxT makes QSM processing more accessible and robust to streaking artifacts.

Objective: Ultra-high-field B0 ≥ 7 tesla (7T) cardiovascular magnetic resonance (CMR) offers increased resolution. However, electrocardiogram (ECG) gating is impacted by the magneto-hydrodynamic effect distorting the ECG trace. We explored the technical feasibility of a 7T magnetic resonance scanner using an ECG trigger learning algorithm to quantitatively assess cardiac volumes and vascular flow. Methods: 7T scans were performed on 10 healthy volunteers on a whole-body research MRI MR scanner (Siemens Healthineers, Erlangen, Germany) with 8 channel Tx/32 channels Rx cardiac coils (MRI Tools GmbH, Berlin, Germany). Vectorcardiogram ECG was performed using a learning phase outside of the magnetic field, with a trigger algorithm overcoming severe ECG signal distortions. Vectorcardiograms were quantitatively analyzed for false negative and false positive events. Cine CMR was performed after 3rd-order B0 shimming using a high-resolution breath-held ECG-retro-gated segmented spoiled gradient echo, and 2D phase contrast flow imaging. Artefacts were assessed using a semi-quantitative scale. Results: 7T CMR scans were acquired in all patients (100%) using the vectorcardiogram learning method. 3,142 R-waves were quantitatively analyzed, yielding sensitivity of 97.6% and specificity of 98.7%. Mean image quality score was 0.9, sufficient to quantitate both cardiac volumes, ejection fraction, and aortic and pulmonary blood flow. Mean left ventricular ejection fraction was 56.4%, right ventricular ejection fraction was 51.4%. Conclusion: Reliable cardiac ECG triggering is feasible in healthy volunteers at 7T utilizing a state-of-the-art three-lead trigger device despite signal distortion from the magnetohydrodynamic effect. This provides sufficient image quality for quantitative analysis. Other ultra-high-field imaging applications such as human brain functional MRI with physiologic noise correction may benefit from this method of ECG triggering.

Purpose The purpose of this study is to demonstrate a method for specific absorption rate (SAR) reduction for 2D T2 -FLAIR MRI sequences at 7 T by predicting the required adiabatic radiofrequency (RF) pulse power and scaling the RF amplitude in a slice-wise fashion. Methods We used a time-resampled frequency-offset corrected inversion (TR-FOCI) adiabatic pulse for spin inversion in a T2 -FLAIR sequence to improve B 1 + homogeneity and calculated the pulse power required for adiabaticity slice-by-slice to minimize the SAR. Drawing on the implicit B 1 + inhomogeneity in a standard localizer scan, we acquired 3D AutoAlign localizers and SA2RAGE B 1 + maps in 28 volunteers. Then, we trained a convolutional neural network (CNN) to estimate the B 1 + profile from the localizers and calculated pulse scale factors for each slice. We assessed the predicted B 1 + profiles and the effect of scaled pulse amplitudes on the FLAIR inversion efficiency in oblique transverse, sagittal, and coronal orientations. Results The predicted B 1 + amplitude maps matched the measured ones with a mean difference of 9.5% across all slices and participants. The slice-by-slice scaling of the TR-FOCI inversion pulse was most effective in oblique transverse orientation and resulted in a 1 min and 30 s reduction in SAR induced delay time while delivering identical image quality. Conclusion We propose a SAR reduction technique based on the estimation of B 1 + profiles from standard localizer scans using a CNN and show that scaling the inversion pulse power slice-by-slice for FLAIR sequences at 7T reduces SAR and scan time without compromising image quality.

Simultaneous [18 F]-fluorodeoxyglucose positron emission tomography and functional magnetic resonance imaging (FDG-PET/fMRI) provides the capability to image two sources of energetic dynamics in the brain – cerebral glucose uptake and the cerebrovascular haemodynamic response. Resting-state fMRI connectivity has been enormously useful for characterising interactions between distributed brain regions in humans. Metabolic connectivity has recently emerged as a complementary measure to investigate brain network dynamics. Functional PET (fPET) is a new approach for measuring FDG uptake with high temporal resolution and has recently shown promise for assessing the dynamics of neural metabolism. Simultaneous fMRI/fPET is a relatively new hybrid imaging modality, with only a few biomedical imaging research facilities able to acquire FDG PET and BOLD fMRI data simultaneously. We present data for n = 27 healthy young adults (18–20 yrs) who underwent a 95-min simultaneous fMRI/fPET scan while resting with their eyes open. KIDSpec, the Kinetic Inductance Detector Spectrograph, is a concept for a highly sensitive, medium-spectral-resolution optical through near-IR spectrograph. It uses the intrinsic-energy-resolving capability of an array of optical/IR-sensitive MKIDs to distinguish multiple orders from a low-resolution grating. By acting as an ‘order resolver,’ the MKID array replaces the cross-disperser in an echelle spectrograph. This greatly simplifies the optical layout of the spectrograph and enables longer slits than are possible with cross-dispersed instruments (to improve sky subtraction). KIDSpec would have similar capabilities to ESO’s highly successful X-shooter instrument. It would provide an R = 4000–10,000 spectrum covering the optical and near-IR spectral range (0.4–1.5 µm). As well as a ‘long-slit’ mode, the IFU would provide a small ($$\sim 50\hbox { spaxel}$$∼50spaxel) field of view for spatially resolved sources. In addition, the photon-counting operation of MKIDs and their photon-energy-resolving ability enable a read-noise-free spectrum with perfect cosmic ray removal. The spectral resolution would be sufficient to remove the bright night-sky lines without the additional pixel noise, making the instrument more sensitive than an equivalent semiconductor-based instrument. KIDSpec would enhance many existing high-profile science cases, including transient (GRB, SNe, etc.) follow-up, redshift determination of faint objects and transit spectroscopy of exoplanets. In addition, it will enable unique science cases, such as dynamical mass estimates of the compact objects in ultra-compact binaries.

This paper presents an analysis of the predicted performance of Microwave Kinetic Inductance Detectors (MKIDS) in interferometric instruments. MKIDS are energy sensitive photon counting detectors which can be designed to operate over a wide range of photon energies, and have excellent timing capabilities. Several Optical and near-IR (OIR) MKID instruments have already been used on-sky, and there are ongoing developments in the US and Europe to improve their capabilities for a wide range of science cases. For interferometric instruments the benefits are clear, with no read out noise or dark current, and time resolution of a microsecond, they can be used for both fringe tracking and science detectors. The inherent energy resolution allows for a range of potential improvements to instruments, from rejection of stray light and cosmic rays, to improved wavelength coverage, to simplified optical designs with few, or no, moving elements.

IntroductionThe majority of blindness worldwide could be prevented or reversed with early diagnosis and treatment, yet identifying at-risk and prevalent cases of eye disease and linking them with care remain important obstacles to addressing this burden. Leading causes of blindness like glaucoma, diabetic retinopathy and age-related macular degeneration have detectable early asymptomatic phases and can cause irreversible vision loss. Mass screening for such diseases could reduce visual impairment at the population level.Methods and analysisThis protocol describes a parallel-group cluster-randomised trial designed to determine whether community-based screening for glaucoma, diabetic retinopathy and age-related macular degeneration reduces population-level visual impairment in Nepal. A door-to-door population census is conducted in all study communities. All adults aged ≥60 years have visual acuity tested at the census visit, and those meeting referral criteria are referred to a local eye care facility for further diagnosis and management. Communities are subsequently randomised to a community-based screening programme or to no additional intervention. The intervention consists of a single round of screening including intraocular pressure and optical coherence tomography assessment of all adults ≥60 years old with enhanced linkage to care for participants meeting referral criteria. Four years after implementation of the intervention, masked outcome assessors conduct a repeat census to collect data on the primary outcome, visual acuity. Individuals with incident visual impairment receive a comprehensive ophthalmological examination to determine the cause of visual impairment. Outcomes are compared by treatment arm according to the originally assigned intervention.Ethics and disseminationThe trial has received ethical approval from the University of California San Francisco Institutional Review Board, Nepal Netra Jyoti Sangh and the Nepal Health Research Council. Results of this trial will be disseminated through publication in peer-reviewed journals and presentation at local and international meetings.Trial registration numberNCT03752840

White-matter lesion count and volume estimation are key to the diagnosis and monitoring of multiple sclerosis (MS). Automated MS lesion segmentation methods that have been proposed in the past 20 years reach their limits when applied to patients in early disease stages characterized by low lesion load and small lesions. We propose an algorithm to automatically assess MS lesion load (number and volume) while taking into account the mixing of healthy and lesional tissue in the image voxels due to partial volume effects. The proposed method works on 3D MPRAGE and 3D FLAIR images as obtained from current routine MS clinical protocols. The method was evaluated and compared with manual segmentation on a cohort of 39 early-stage MS patients with low disability, and showed higher Dice similarity coefficients (median DSC = 0.55) and higher detection rate (median DR = 61%) than two widely used methods (median DSC = 0.50, median DR, Highlights • Modeling the partial volume improves lesion volumetric measurements. • Higher detection of small lesions inherently prone to partial volume effects. • Partial volume effects should be taken into account in early stages of MS.

Several diffusion-weighted MRI techniques have been developed and validated during the past 2 decades. While offering various neuroanatomical inferences, these techniques differ in their proposed optimal acquisition design, preventing clinicians and researchers benefiting from all potential inference methods, particularly when limited time is available. This study reports an optimal design that enables for a time-efficient diffusion-weighted MRI acquisition scheme at 7 Tesla. HARMONI (High Angular Resolution MOnolithic Integral field spectrograph) 1 is a planned first-light integral field spectrograph for the Extremely Large Telescope. The spectrograph sub-system is being designed, developed, and built by the University of Oxford. The project has just completed the Preliminary Design Review (PDR), with all major systems having nearly reached a final conceptual design. As part of the overall prototyping and assembly, integration, and testing (AIT) of the HARMONI spectrograph, we will be building a full-scale engineering model of the spectrograph. This will include all of the moving and mechanical systems, but without optics. Its main purpose is to confirm the AIT tasks before the availability of the optics, and the system will be tested at HARMONI cryogenic temperatures. By the time of the construction of the engineering model, all of the individual modules and mechanisms of the spectrograph will have been prototyped and cryogenically tested. The lessons learned from the engineering model will then be fed back into the overall design of the spectrograph modules ahead of their development.

Quantitative susceptibility mapping (QSM) reveals pathological changes in widespread diseases such as Parkinson’s disease, Multiple Sclerosis, or hepatic iron overload. QSM requires multiple processing steps after the acquisition of magnetic resonance imaging (MRI) phase measurements such as unwrapping, background field removal and the solution of an ill-posed field-to-source-inversion. Current techniques utilize iterative optimization procedures to solve the inversion and background field correction, which are computationally expensive and lead to suboptimal or over-regularized solutions requiring a careful choice of parameters that make a clinical application of QSM challenging. We have previously demonstrated that a deep convolutional neural network can invert the magnetic dipole kernel with a very efficient feed forward multiplication not requiring iterative optimization or the choice of regularization parameters. In this work, we extended this approach to remove background fields in QSM. The prototype method, called SHARQnet, was trained on simulated background fields and tested on 3T and 7T brain datasets. We show that SHARQnet outperforms current background field removal procedures and generalizes to a wide range of input data without requiring any parameter adjustments. In summary, we demonstrate that the solution of ill-posed problems in QSM can be achieved by learning the underlying physics causing the artifacts and removing them in an efficient and reliable manner and thereby will help to bring QSM towards clinical applications.

The High Angular Resolution Monolithic Optical and Near-infrared Integral field spectrograph (HARMONI) will be one of the instruments installed on ESO's 39-meter Extremely Large Telescope (ELT) at first light. The instrument will operate from 0.47 - 2.45 μm with Δλ/λ = 3,000 - 17,000. On-sky spatial pixels (spaxels) are divided between four spectrographs, each equipped with 11 transmission diffraction gratings to cover the ranges of wavelengths and spectral resolutions. These spectrographs will be cooled to ~140 K to decrease thermal radiation at longer wavelengths. In all configurations, the diffraction grating will lose a greater fraction of scientific light than any other single optic in the instrument. Additionally, manufacturers are often unable to measure the fraction of transmitted light at HARMONI's longest wavelengths. For these reasons, we have developed a setup to measure the efficiencies of transmission diffraction gratings across HARMONI's bandpass. The setup uses modulated signals, a single detector, and a lock-in amplifier to minimize sources of systematic errors. A modified version of this setup may be used to measure stray light. These setups and initial results are presented.

Previously, 7T CMR was constrained to using pulse oximetry for triggering, We explored the technical feasbility of a 7 T research MR scanner using of-the-art ECG trigger algorithm to assess left and right ventricular volumes, aortic and pulmonary valve flow. Methods: 7T CMR scans were performed on 10 healthy volunteers on whole-body research MRI scanner (Siemens Healthcare, Erlangen, Germany) with 8 channel Tx/32 channel Rx cardiac coil (MRI Tools GmbH, Berlin, Germany) under institutional review board approval. Vectorcardiogram ECG was successfully performed using a learning phase outside of the magnetic field, with a trigger algorithm with sufficient accuracy for CMR despite severe ECG signal distortions from the 7T field. Cine CMR was performed after 3rd-order B0 shimming using a high-resolution breath-held ECG-retro-gated segmented two-dimensional spoiled gradient echo sequence, and 2-dimensional phase contrast flow imaging. Analysis was performed using Cmr42 software (Circle CVi, Calgary). Results: Successful 7T CMR scans were acquired in all patients (100%) using the Vectorcardiogram 3-lead ECG method. Image quality was sufficient to quantitate both left and right ventricular volumes, ejection fraction, aortic and pulmonary blood flow and regurgitant fractions in 9/10 (90%) of volunteers (figure 1), with one volunteer having difficulty with breath-holding and ventricular ectopy causing gating artefacts.

HARMONI is an Integral Field Spectrograph (IFS) for ESO’s ELT. It has been selected as the first light spec- trograph and will provide the workhorse spectroscopic capabilities for the ELT for many years. HARMONI is currently at the PDR-level and the current design for the HARMONI IFS consists of a number of spaxel scales sampling down to the diffraction limit of the telescope. It uses a field splitter and image slicer to divide the field into 4 sub-units, each providing an input slit to one of four nearly identical spectrographs. All spectrographs will operate at near infrared wavelengths (0.81-2.45 micrometers), sampling different parts of the spectrum with a range of spectral resolving powers (3300, 7000, 18000). In addition, two of the four spectrographs will have a Visible capability (0.5-0.83 micrometers) operating with seeing-limited observations. This proceeding presents an overview of the opto-mechanical design and specifications of the spectrograph units for HARMONI.

Microwave Kinetic Inductance Detectors, or MKIDS, have the ability to simultaneous resolve the wavelength of individual photons and time tag photons with microsecond precision. This opens up a number of exciting new possibilities and efficiency gains for optical/IR astronomy. In this paper we describe a plan to take the MKID technology, which we have demonstrated on the Palomar, Lick, and Subaru Telescopes, out of the realm of private instruments usable only by experts. Our goal is to incorporate MKIDs into a facility-class instrument at the Keck 1 Telescope that can be used by a large part of the astronomical community. This new instrument, the Keck Radiometer Array using KID ENergy Sensors (KRAKENS), will be a 30 kpix integral field spectrograph (IFS) with a 42.5” x 45” field of view, extraordinarily wide wavelength coverage from 380-1350 nm, and a spectral resolution R=λ/uλ > 20 at 400 nm. Future add on modules could enable polarimetry and higher spectral resolution. KRAKENS will be built using the same style MKID arrays, cryostat, and similar readout electronics to those used in the successful 10 kpix DARKNESS instrument at Palomar and 20 kpix MEC instrument at Subaru, significantly reducing the technical risk.

Microwave Kinetic Inductance Detector (MKID) devices offer inherent spectral resolution, simultaneous read out of thousands of pixels, and photon-limited sensitivity at optical wavelengths. Before taking observations the readout power and frequency of each pixel must be individually tuned, and if the equilibrium state of the pixels change, then the readout must be retuned. This process has previously been performed through manual inspection, and typically takes one hour per 500 resonators (20 h for a ten-kilo-pixel array). We present an algorithm based on a deep convolution neural network (CNN) architecture to determine the optimal bias power for each resonator. The bias point classifications from this CNN model, and those from alternative automated methods, are compared to those from human decisions, and the accuracy of each method is assessed. On a test feed-line dataset, the CNN achieves an accuracy of 90% within 1 dB of the designated optimal value, which is equivalent accuracy to a randomly selected human operator, and superior to the highest scoring alternative automated method by 10%. On a full ten-kilopixel array, the CNN performs the characterization in a matter of minutes — paving the way for future mega-pixel MKID arrays.

Quantitative susceptibility mapping (QSM) is based on magnetic resonance imaging (MRI) phase measurements and has gained broad interest because it yields relevant information on biological tissue properties, predominantly myelin, iron and calcium in vivo. Thereby, QSM can also reveal pathological changes of these key components in widespread diseases such as Parkinson's disease, Multiple Sclerosis, or hepatic iron overload. While the ill-posed field-to-source-inversion problem underlying QSM is conventionally assessed by the means of regularization techniques, we trained a fully convolutional deep neural network - DeepQSM - to directly invert the magnetic dipole kernel convolution. DeepQSM learned the physical forward problem using purely synthetic data and is capable of solving the ill-posed field-to-source inversion on in vivo MRI phase data. The magnetic susceptibility maps reconstructed by DeepQSM enable identification of deep brain substructures and provide information on their respective magnetic tissue properties. In summary, DeepQSM can invert the magnetic dipole kernel convolution and delivers robust solutions to this ill-posed problem.

Quantitative susceptibility mapping (QSM) aims to extract the magnetic susceptibility of tissue from magnetic resonance imaging (MRI) phase measurements. The mapping of magnetic susceptibility in vivo has gained broad interest in several fields of science and medicine because it yields relevant information on biological tissue properties, predominantly myelin, iron and calcium. Thereby, QSM can also reveal pathological changes of these key components in devastating diseases such as Parkinson’s disease, Multiple Sclerosis, or hepatic iron overload. As QSM requires the solution of an ill-posed field-to-source-inversion, current techniques utilize manual optimization of regularization parameters to balance between smoothing, artifacts and quantification accuracy. We trained a fully convolutional deep neural network - DeepQSM - to invert the magnetic dipole kernel convolution. This network is capable of solving the ill-posed field-to-source inversion on real-world in vivo MRI phase data without the need for manual parameter tuning, which proves that this network has generalized the underlying mathematical principle of the dipole inversion. We demonstrate that DeepQSM’s susceptibility maps enable identification of deep brain substructures that are not visible in MRI phase data and provide information on their respective magnetic tissue properties. We illustrate DeepQSM’s clinical relevance in a patient with multiple sclerosis showing its sensitivity to white matter lesions. In summary, DeepQSM can be used to determine the composition of myelin sheets of nerve fibers in the brain, and to assess quantitative information on iron homeostasis and its dysregulation, and will subsequently contribute to a better understanding of these biological processes in health and disease.

Purpose: Signal magnitude can robustly be combined using the sum-of-squares approach. Methods have been developed to combine complex images. However, techniques based only on signal phase have not been developed and evaluated. Theory and Methods: We performed simulations to demonstrate the effect of noise on coil combination. 32-channel 7 Tesla human gradient echo MRI brain data were collected. We combined phase images based on phase noise leading to spatially selective and coil selective combination of phase images. We compared our selective combination approach to optimal noise distribution and adaptive combination methods. Results: We found that selective combination of signal phases leads to improved phase signal-to-noise ratio. Furthermore, a phase shift can be present in combined phase images introduced by the method used to combine multiple channel phases. Conclusion: Mapping of signal phase from ultra-high field MRI data undoubtedly provides a wealth of information about the ageing brain and the effects of neurodegenerative disorders. Measurement of signal phase is essential in frequency shift mapping and in quantitative susceptibility mapping. The method used to combine signal phase should be informed by an understanding of the noise distribution in signal phase at the individual channel level. Magn Reson Med 76:1469–1477, 2016.

INTRODUCTION: Intravoxel incoherent motion (IVIM) imaging is an MRI perfusion technique that uses a diffusion-weighted sequence with multiple b values and a bi-compartmental signal model to measure the so-called pseudo-diffusion of blood caused by its passage through the microvascular network. The goal of the current study was to assess the feasibility of IVIM perfusion fraction imaging in patients with acute stroke. METHODS: Images were collected in 17 patients with acute stroke. Exclusion criteria were onset of symptoms to imaging >5 days, hemorrhagic transformation, infratentorial lesions, small lesions

Purpose To evaluate the combination of low-B1+ adiabatic pulses and high permittivity (er ≈ 165) dielectric pads effectiveness to reproducibly improve the inversion efficiency for whole-brain MP2RAGE scans, at ultra-high field. Materials and Methods Two low-B1+ adiabatic pulses, HS8 and TR-FOCI, were compared with the conventional HS1 adiabatic pulse in MP2RAGE acquisitions of four subjects at 7 Tesla. The uniform MP2RAGE images were qualitatively assessed for poor inversion artifacts by trained observers. Each subject was rescanned using dielectric pads. Eight further subjects underwent two MP2RAGE scan sessions using dielectric pads and the TR-FOCI adiabatic pulse. Results The HS8 and TR-FOCI pulses improved inversion coverage in all subjects compared with the HS1 pulse. However, in subjects whose head lengths are large (≥136 mm) relative to the coil's z-coverage, the B1+ field over the cerebellum was insufficient to cause inversion. Dielectric pads increase the B1+ field, by ∼50%, over the cerebellum, which in conjunction with the TR-FOCI pulse, reproducibly improves the inversion efficiency over the whole brain for subjects with head lengths ≤155 mm. Minor residual inversion artifacts were present in three of eight subjects (head lengths ≥155 mm). Conclusion The complementary techniques of low-B1+ adiabatic RF pulses and high permittivity dielectric pads allow whole-brain structural T1w images to be reliably acquired at ultra-high field.

Purpose To evaluate the feasibility of intravoxel incoherent motion (IVIM) perfusion measurements in the brain with currently available imaging systems. Materials and Methods We acquired high in-plane resolution (1.2 × 1.2 mm) diffusion-weighted images with 16 different values of b ranging from 0 to 900 s/mm, in three orthogonal directions, on 3T systems with a 32-multichannel receiver head coil. IVIM perfusion maps were extracted by fitting a double exponential model of signal amplitude decay. Regions of interest were drawn in pathological and control regions, where IVIM perfusion parameters were compared to the corresponding dynamic susceptibility contrast (DSC) parameters. Results Hyperperfusion was found in the nonnecrotic or cystic part of two histologically proven glioblastoma multiforme and in two histologically proven glioma WHO grade III, as well as in a brain metastasis of lung adenocarcinoma, in a large meningioma, and in a case of ictal hyperperfusion. A monoexponential decay was found in a territory of acute ischemia, as well as in the necrotic part of a glioblastoma. The IVIM perfusion fraction f correlated well with DSC CBV. Conclusion Our initial report suggests that high-resolution brain perfusion imaging is feasible with IVIM in the current clinical setting. J. Magn. Reson. AIMS: Bicuspid aortic valve (BAV) causes complex flow patterns in the ascending aorta (AAo), which may compromise the accuracy of flow measurement by phase-contrast magnetic resonance (PC-MR). Therefore, we aimed to assess and compare the accuracy of forward flow measurement in the AAo, where complex flow is more dominant in BAV patients, with flow quantification in the left ventricular outflow tract (LVOT) and the aortic valve orifice (AV), where complex flow is less important, in BAV patients and controls. METHODS AND RESULTS: Flow was measured by PC-MR in 22 BAV patients and 20 controls at the following positions: (i) LVOT, (ii) AV, and (iii) AAo, and compared with the left ventricular stroke volume (LVSV). The correlation between the LVSV and the forward flow in the LVOT, the AV, and the AAo was good in BAV patients (r = 0.97/0.96/0.93; P < 0.01) and controls (r = 0.96/0.93/0.93; P < 0.01). However, in relation with the LVSV, the forward flow in the AAo was mildly underestimated in controls and much more in BAV patients [median (inter-quartile range): 9% (4%/15%) vs. 22% (8%/30%); P < 0.01]. This was not the case in the LVOT and the AV. The severity of flow underestimation in the AAo was associated with flow eccentricity. CONCLUSION: Flow measurement in the AAo leads to an underestimation of the forward flow in BAV patients. Measurement in the LVOT or the AV, where complex flow is less prominent, is an alternative means for quantifying the systolic forward flow in BAV patients.

PURPOSE: At high magnetic field strengths (B0 >/= 3 T), the shorter radiofrequency wavelength produces an inhomogeneous distribution of the transmit magnetic field. This can lead to variable contrast across the brain which is particularly pronounced in T2 -weighted imaging that requires multiple radiofrequency pulses. To obtain T2 -weighted images with uniform contrast throughout the whole brain at 7 T, short (2-3 ms) 3D tailored radiofrequency pulses (kT -points) were integrated into a 3D variable flip angle turbo spin echo sequence. METHODS: The excitation and refocusing "hard" pulses of a variable flip angle turbo spin echo sequence were replaced with kT -point pulses. Spatially resolved extended phase graph simulations and in vivo acquisitions at 7 T, utilizing both single channel and parallel-transmit systems, were used to test different kT -point configurations. RESULTS: Simulations indicated that an extended optimized k-space trajectory ensured a more homogeneous signal throughout images. In vivo experiments showed that high quality T2 -weighted brain images with uniform signal and contrast were obtained at 7 T by using the proposed methodology. CONCLUSION: This work demonstrates that T2 -weighted images devoid of artifacts resulting from B1 + inhomogeneity can be obtained at high field through the optimization of extended kT -point pulses.

Objectives: The aim of this study was to study focal cerebellar pathology in early stages of multiple sclerosis (MS) using ultra-high-field magnetization-prepared 2 inversion-contrast rapid gradient-echo (7T MP2RAGE). Materials and Methods: Twenty early-stage relapsing-remitting MS patients underwent an MP2RAGE acquisition at 7 T magnetic resonance imaging (MRI) (images acquired at 2 different resolutions: 0.58 x 0.58 x 0.58 mm(3), 7T_0.58, and 0.75 x 0.75 x 0.90 mm(3), 7T_0.75) and 3 T MRI (1.0 x 1.0 x 1.2 mm(3), 3T_1.0). Total cerebellar lesion load and volume andmean cerebellar lesion volume were compared across images using a Wilcoxon signed-rank test. Mean T1 relaxation times in lesions and normal-appearing tissue as well as contrast-to-noise ratio (CNR) measurements were also compared using a Wilcoxon signed-rank test. A multivariate analysis was applied to assess the contribution of MRI metrics to clinical performance in MS patients. Results: Both 7T_0.58 and 7T_0.75 MP2RAGE showed significantly higher lesion load compared with 3T_1.0 MP2RAGE (P < 0.001). Plaques that were judged as leukocortical in 7T_0.75 and 3T_1.0 MP2RAGEs were instead identified as WM lesions in 7T_0.58 MP2RAGE. Cortical lesion CNR was significantly higher in MP2RAGEs at 7 T than at 3 T. Total lesion load as well as total and mean lesion volume obtained at both 7 T and 3 T MP2RAGE significantly predicted attention (P < 0.05, adjusted R-2 = 0.5), verbal fluency (P < 0.01, adjusted R-2 = 0.6), and motor performance (P = 0.01, adjusted R-2 = 0.7). Conclusions: This study demonstrates the value of 7 T MP2RAGE to study the cerebellum in early MS patients. 7T_0.58 MP2RAGE provides a more accurate anatomical description of white and gray matter pathology compared with 7T_0.75 and 3T_1.0 MP2RAGE, likely due to the improved spatial resolution, lower partial volume effects, and higher CNR.

Two different cryogenic lens mounts have been developed and tested for potential use in the HARMONI spectrograph cameras. Problems were encountered during initial tests whereby the lenses were cracking where they were adhered to the mount. This was found to be caused by the choice of adhesive and solved by changing to a silicone RTV glue. The cryogenic performance of the two lens mount designs was tested, with the baseline design seeing the lens move by 18 μm radially from warm to cold, which is just within the tightest tolerances from the optical design, as long as any misalignments in the mounting procedure can be removed when aligning the lenses in the camera barrel. The alternative design was found have much worse performance with the lens moving by 40 μm due to fragile flexures and so is no longer being considered. A mounting procedure for spherical lenses has also been developed which is capable of peak to valley alignments of 10 μm axially, and 5 μm radially.

The imaging of very fast flow with phase contrast magnetic resonance is limited by how much the echo time (TE) can be shortened. The long TE of conventional Cartesian-based sequences results in an increased signal loss from intravoxel dephasing and therefore unreliable velocity estimates within a voxel. A through-plane phase contrast (PC) ultrashort echo time (UTE) sequence, incorporating the velocity encoding into the slice-selection gradients, can achieve a TE that is around 25% of that of the conventional sequence. The shorter TE provides a faster snapshot of the flow and ensures less signal loss and fewer phase errors from large accelerations and spin mixing. The utilization of phase contrast ultrashort echo time (PC-UTE) techniques provide a means to investigate very fast flow regimes, in this case, jets in aortic stenosis in vivo. Microwave Kinetic Inductance Detector (MKID) arrays, recently demonstrated at the Palomar 200 inch telescope, are a new superconducting detector technology that has great potential for astrophysics. We propose a new type of instrument, a Superconducting Multi-Object Spectrograph (SuperMOS), that uses MKIDs in conjunction with a focal plane mask. We present the design and science goals of a SuperMOS designed for LSST follow-up named Giga-z. Housing a 100,000 pixel MKID array with energy resolution R=50{100 and a 0.35-1.35 μm bandwidth, it will be capable of measuring 2 billion spectra and determining redshifts over 20,000 square degrees in 3 years down to mi ≈ 24.5 on a dedicated 4-meter telescope. Compared to LSST alone, Giga-z should improve the redshift precision by a factor of three with a much lower catastrophic failure rate.

Thinned CCD detectors display fringing which arises from the interference of multiply reflected light in the layers of the CCD. If the layer construction - the thicknesses and refractive indexes of the layers - is known, then the observed fringing can be accurately modelled and used to correct imaging and spectroscopic data for its effects. In practice the specifications on the actual deposited layer thicknesses may not be known to sufficient accuracy to predict the fringe behaviour. Thus calibration data, in the form of monochromatic flat fields, is required and can be modelled using the technique outlined by Malamuth et al. 2003, which has been applied to ACS CCDs. Initial tests to model the observed fringing of the FORS2 MIT CCD are described. A set of six monochromatic flat fields was generated by shining light from a tunable monochromator into the FORS2 instrument calibration unit. Peak-to-peak fringe amplitude of 7-8% was measured at a wavelength of 970 nm. In order to model the CCD structure, a series of monochromatic flats at closely spaced wavelengths is required, which would have been very time-consuming to acquire with the monochromator. An alternative strategy is outlined. The fringing characteristics of CCDs should be systematically calibrated while still in the test laboratory. © 2008 Springer-Verlag Berlin Heidelberg.

Microwave Kinetic Inductance Detectors (MKIDs) are superconducting detectors capable of counting single photons and measuring their energy in the UV, optical, and near-IR. MKIDs feature intrinsic frequency domain multiplexing (FDM) at microwave frequencies, allowing the construction and readout of large arrays. Due to the microwave FDM, MKIDs do not require the complex cryogenic multiplexing electronics used for similar detectors, such as Transition Edge Sensors (TESs), but instead transfer this complexity to room temperature electronics where they present a formidable signal processing challenge. In this paper we describe the first successful effort to build a readout for a photon counting optical/near-IR astronomical instrument, the ARray Camera for Optical to Near-infrared Spectrophotometry (ARCONS). This readout is based on open source hardware developed by the Collaboration for Astronomy Signal Processing and Electronics Research (CASPER). Designed principally for radio telescope backends, it is flexible enough to be used for a variety of signal processing applications.

To evaluate the sensitivity of the perfusion parameters derived from Intravoxel Incoherent Motion (IVIM) MR imaging to hypercapnia-induced vasodilatation and hyperoxygenation-induced vasoconstriction in the human brain.This study was approved by the local ethics committee and informed consent was obtained from all participants. Images were acquired with a standard pulsed-gradient spin-echo sequence (Stejskal-Tanner) in a clinical 3-T system by using 16 b values ranging from 0 to 900 sec/mm(2). Seven healthy volunteers were examined while they inhaled four different gas mixtures known to modify brain perfusion (pure oxygen, ambient air, 5% CO(2) in ambient air, and 8% CO(2) in ambient air). Diffusion coefficient (D), pseudodiffusion coefficient (D*), perfusion fraction (f), and blood flow-related parameter (fD*) maps were calculated on the basis of the IVIM biexponential model, and the parametric maps were compared among the four different gas mixtures. Paired, one-tailed Student t tests were performed to assess for statistically significant differences.Signal decay curves were biexponential in the brain parenchyma of all volunteers. When compared with inhaled ambient air, the IVIM perfusion parameters D*, f, and fD* increased as the concentration of inhaled CO(2) was increased (for the entire brain, P = .01 for f, D*, and fD* for CO(2) 5%; P = .02 for f, and P = .01 for D* and fD* for CO(2) 8%), and a trend toward a reduction was observed when participants inhaled pure oxygen (although P.05). D remained globally stable.The IVIM perfusion parameters were reactive to hyperoxygenation-induced vasoconstriction and hypercapnia-induced vasodilatation. Accordingly, IVIM imaging was found to be a valid and promising method to quantify brain perfusion in humans.

Purpose: To clarify the use of MRI phase contrast (PC), as an alternative to Doppler echocardiography, when measuring high-velocity turbulent jets associated with stenotic valvular disease. Materials and Methods: In vivo PC aortic stroke volume (SV) was compared with ventricular SV in 31 patients with moderate to severe aortic stenosis (AS). Two in vitro pipe experiments were conducted to evaluate errors in steady stenotic and nonstenotic turbulent flows. Results: The average in vivo error in SV was -24% in the left-ventricular (LV) outflow tract (LVOT) and -41% in the aortic root. Errors were most prominent in patients with the highest Doppler peak velocities. In vitro nonstenotic flow experiments showed accurate flow measurement with an average error of 1.8%. Significant errors were found in the in vitro stenotic flow, which reduced with shorter echo times (TE): average error -166/-67/-25/-13/-8.8% for TEs of 4.8/4.0/3.3/2.2/2.0 msec. In both the in vivo and in vitro stenotic experiments the errors were associated with signal loss in the flow-compensated magnitude image. Conclusion: Signal loss is associated with flow errors in stenotic jets. Current clinically available PC pulse sequences with TE >2 msec may not accurately quantify flow for severe lesions.

Trabajo presentado en SPIE Astronomical Telescopes, celebrado en San Diego (California), del 26 de junio al 1 de julio de 2016, HARMONI is the E-ELT's first light visible and near-infrared integral field spectrograph. It will provide four different spatial scales, ranging from coarse spaxels of 60 × 30 mas best suited for seeing limited observations, to 4 mas spaxels that Nyquist sample the diffraction limited point spread function of the E-ELT at near-infrared wavelengths. Each spaxel scale may be combined with eleven spectral settings, that provide a range of spectral resolving powers (R 3500, 7500 and 20000) and instantaneous wavelength coverage spanning the 0.5 - 2.4 ¿m wavelength range of the instrument. In autumn 2015, the HARMONI project started the Preliminary Design Phase, following signature of the contract to design, build, test and commission the instrument, signed between the European Southern Observatory and the UK Science and Technology Facilities Council. Crucially, the contract also includes the preliminary design of the HARMONI Laser Tomographic Adaptive Optics system. The instrument's technical specifications were finalized in the period leading up to contract signature. In this paper, we report on the first activity carried out during preliminary design, defining the baseline architecture for the system, and the trade-off studies leading up to the choice of baseline.

Functional imaging with intravoxel incoherent motion (IVIM) magnetic resonance imaging (MRI) is demonstrated. Images were acquired at 3 Tesla using a standard Stejskal-Tanner diffusion-weighted echo-planar imaging sequence with multiple b-values. Cerebro-spinal fluid signal, which is highly incoherent, was suppressed with an inversion recovery preparation pulse. IVIM microvascular perfusion parameters were calculated according to a two-compartment (vascular and non-vascular) diffusion model. The results obtained in 8 healthy human volunteers during visual stimulation are presented. The IVIM blood flow related parameter fD* increased 170% during stimulation in the visual cortex, and 70% in the underlying white matter.

In the context of various policy initiatives concerning widening access to and strengthening recruitment and retention in the health services, cadet schemes – predominantly in nursing – have proliferated over the last few years. As part of a larger national evaluation of National Health Service (NHS) cadet schemes, this paper reports on a survey of senior cadet students across 62 cadet schemes in England and examines their experience of being a cadet on such a scheme. Cadets forming the most senior cohort from each of the 62 schemes (n=596) were surveyed using a questionnaire. The questionnaire included self-rated measures of job satisfaction, job stress and commitment. A 5% sample of these cadets participated in follow-up telephone interviews. Cadets reported high satisfaction with their courses. One of the most positive aspects of the schemes was the first-hand experience of working in the NHS they provided, whilst also giving cadets the opportunity to gain recognisable skills and qualifications. Cadets scored highly on the job satisfaction scale and, on the job stress scale, showed low stress overall. A significant positive correlation was found between satisfaction and stress, indicating that the cadets who are most satisfied are also more highly stressed. A negative correlation was found between stress and the dimensions of commitment indicating that those cadets who are stressed are less committed to the NHS. A negative correlation was also found between satisfaction and the dimensions of commitment, suggesting that commitment to the NHS is not contingent on high satisfaction. The implications for the findings of the survey are discussed.

Artificial neural networks (ANNs) were used for voxel-wise parameter estimation with the combined intravoxel incoherent motion (IVIM) and kurtosis model facilitating robust diffusion parameter mapping in the human brain. The proposed ANN approach was compared with conventional least-squares regression (LSR) and state-of-the-art multi-step fitting (LSR-MS) in Monte-Carlo simulations and in vivo in terms of estimation accuracy and precision, number of outliers and sensitivity in the distinction between grey (GM) and white (WM) matter. Both the proposed ANN approach and LSR-MS yielded visually increased parameter map quality. Estimations of all parameters (perfusion fraction f, diffusion coefficient D, pseudo-diffusion coefficient D*, kurtosis K) were in good agreement with the literature using ANN, whereas LSR-MS resulted in D* overestimation and LSR yielded increased values for f and D*, as well as decreased values for K. Using ANN, outliers were reduced for the parameters f (ANN, 1%; LSR-MS, 19%; LSR, 8%), D* (ANN, 21%; LSR-MS, 25%; LSR, 23%) and K (ANN, 0%; LSR-MS, 0%; LSR, 15%). Moreover, ANN enabled significant distinction between GM and WM based on all parameters, whereas LSR facilitated this distinction only based on D and LSR-MS on f, D and K. Overall, the proposed ANN approach was found to be superior to conventional LSR, posing a powerful alternative to the state-of-the-art method LSR-MS with several advantages in the estimation of IVIM-kurtosis parameters, which might facilitate increased applicability of enhanced diffusion models at clinical scan times.

The feasibility to measure brain perfusion using intravoxel incoherent motion (IVIM) MRI has been reported recently with currently clinically available technology. The method is intrinsically local and quantitative, but is contaminated by partial volume effects with cerebrospinal fluid (CSF). Signal from CSF can be suppressed by a 180° inversion recovery (180°-IR) magnetization preparation, but this also leads to strong suppression of blood and brain tissue signal. Here, we take advantage of the different T relaxations of blood and brain relative to CSF, and implement a T-prepared IVIM (T2prep IVIM) inversion recovery acquisition, which permits a recovery of between 43% and 57% of arterial and venous blood magnetization at excitation time compared with the theoretical recovery of between 27% and 30% with a standard 180°-IR. We acquired standard IVIM (IVIM), T2prep IVIM and dynamic susceptibility contrast (DSC) images at 3 T using a 32-multichannel receiver head coil in eight patients with known large high-grade brain tumors. We compared the contrast and contrast-to-noise ratio obtained in the corresponding cerebral blood volume images quantitatively, as well as subjectively by two neuroradiologists. Our findings suggest that quantitative cerebral blood volume contrast and contrast-to-noise ratio, as well as subjective lesion detection, contrast quality and diagnostic confidence, are increased with T2prep IVIM relative to IVIM and DSC.

ULTRASPEC is a high-speed imaging photometer mounted permanently at one of the Nasmyth focii of the 2.4-m Thai National Telescope (TNT) on Doi Inthanon, Thailand's highest mountain. ULTRASPEC employs a 1024x1024 pixel frame-transfer, electron-multiplying CCD (EMCCD) in conjunction with re-imaging optics to image a field of 7.7'x7.7' at (windowed) frame rates of up to ~200 Hz. The EMCCD has two outputs - a normal output that provides a readout noise of 2.3 e- and an avalanche output that can provide essentially zero readout noise. A six-position filter wheel enables narrow-band and broad-band imaging over the wavelength range 330-1000 nm. The instrument saw first light on the TNT in November 2013 and will be used to study rapid variability in the Universe. In this paper we describe the scientific motivation behind ULTRASPEC, present an outline of its design and report on its measured performance on the TNT., 14 pages, 10 figures, 3 tables, accepted for publication in MNRAS

HARMONI is a visible and near-infrared (0.47 to 2.45 μm) integral field spectrometer, providing the E-ELT's core spectroscopic capability, over a range of resolving powers from R (≡λ/Δλ)~500 to R~20000. The instrument provides simultaneous spectra of ~32000 spaxels at visible and near-IR wavelengths, arranged in a √2:1 aspect ratio contiguous field. HARMONI is conceived as a workhorse instrument, addressing many of the E-ELT’s key science cases, and will exploit the E-ELT's scientific potential in its early years, starting at first light. HARMONI provides a range of spatial pixel (spaxel) scales and spectral resolving powers, which permit the user to optimally configure the instrument for a wide range of science programs; from ultra-sensitive to diffraction limited, spatially resolved, physical (via morphology), chemical (via abundances and line ratios) and kinematic (via line-of-sight velocities) studies of astrophysical sources. Recently, the HARMONI design has undergone substantial changes due to significant modifications to the interface with the telescope and the architecture of the E-ELT Nasmyth platform. We present an overview of the capabilities of HARMONI, and of its design from a functional and performance viewpoint.

The spectrograph sub-system is responsible for dispersing the light from the slicer with the required spectral resolving power and imaging the spectra on to a detector. Each image slicer creates a single exit slit feeding a single spectrograph unit containing visible (VIS) and infrared (IR) cameras. The four HARMONI slicers in total create four exit slits, feeding four spectrograph units comprising of collimators, dispersers, and cameras. The focal plane of each camera contains a mosaic of two 4Kx4K detectors, leading to 8K pixels along the length of the slit. The HARMONI wavelength range (0.43 μm to 2.45 μm) splits into a visible and a near-infrared wavelength range with a transition wavelength at 0.8 μm. The optical design of HARMONI up to the dispersers is fully reflective and therefore the pre-optics and IFU subsystems, as well the spectrograph collimator, can be used for both the visible and near-infrared wavelength range. Only the dispersers and the spectrograph cameras are different for the visible and near infrared spectral ranges. To not duplicate sub-systems unnecessarily the wavelength split in the spectrograph is realised by inserting a dichroic in the collimated beam before the disperser to either direct the light towards the visible disperser and camera, or let it pass toward the near-infrared disperser and camera. In contrast to the Phase A study all of HARMONI spectrograph unit will have both visible and near infrared disperser and cameras.

We present a novel concept for a highly sensitive, medium spectral resolution optical through near-IR spectrograph. KIDSpec, the Kinetic Inductance Detector Spectrograph, uses the intrinsic energy resolving capability of an array of optical/IR-sensitive MKIDs to distinguish multiple orders from a low line-density (echelle) grating. MKID arrays have a wide bandpass (0.1-2.5um) and good quantum efficiency, making them strong candidates for replacing CCDs in many astronomical instruments. By acting as an ‘order resolver’, the MKID array replaces the cross-disperser in an echelle spectrograph. This greatly simplifies the optical layout of the spectrograph and enables longer slits than are possible with cross-dispersed instruments. KIDSpec would have similar capabilities to ESO’s X-shooter instrument. It would provide an R=4000-10,000 spectrum covering the entire optical and near-IR spectral range. In addition to a ‘long-slit’ mode, the IFU would provide a small (~50 spaxel) field-of-view for spatially resolved sources. In addition, the photon-counting operation of MKIDs and their photon-energy resolving ability enable a read-noise free spectrum with perfect cosmic ray removal. The spectral resolution would be sufficient to remove the bright night-sky lines without the additional pixel noise, making the instrument more sensitive than an equivalent semiconductor-based instrument. KIDSpec would enhance many existing high-profile science cases, including transient (GRB, SNe, etc.) follow-up, redshift determination of faint objects and transit spectroscopy of exoplanets. In addition it will enable unique science cases, such as dynamical mass estimates of the compact objects in ultra-compact binaries.

PURPOSE: To suppress the noise, by sacrificing some of the signal homogeneity for numerical stability, in uniform T1 weighted (T1w) images obtained with the magnetization prepared 2 rapid gradient echoes sequence (MP2RAGE) and to compare the clinical utility of these robust T1w images against the uniform T1w images. MATERIALS AND METHODS: 8 healthy subjects (29.0 ± 4.1 years; 6 Male), who provided written consent, underwent two scan sessions within a 24 hour period on a 7T head-only scanner. The uniform and robust T1w image volumes were calculated inline on the scanner. Two experienced radiologists qualitatively rated the images for: general image quality; 7T specific artefacts; and, local structure definition. Voxel-based and volume-based morphometry packages were used to compare the segmentation quality between the uniform and robust images. Statistical differences were evaluated by using a positive sided Wilcoxon rank test. RESULTS: The robust image suppresses background noise inside and outside the skull. The inhomogeneity introduced was ranked as mild. The robust image was significantly ranked higher than the uniform image for both observers (observer 1/2, p-value = 0.0006/0.0004). In particular, an improved delineation of the pituitary gland, cerebellar lobes was observed in the robust versus uniform T1w image. The reproducibility of the segmentation results between repeat scans improved (p-value = 0.0004) from an average volumetric difference across structures of ≈ 6.6% to ≈ 2.4% for the uniform image and robust T1w image respectively. CONCLUSIONS: The robust T1w image enables MP2RAGE to produce, clinically familiar T1w images, in addition to T1 maps, which can be readily used in uniform morphometry packages.

BACKGROUND AND PURPOSE: Intravoxel incoherent motion MRI has been proposed as an alternative method to measure brain perfusion. Our aim was to evaluate the utility of intravoxel incoherent motion perfusion parameters (the perfusion fraction, the pseudodiffusion coefficient, and the flow-related parameter) to differentiate high- and low-grade brain gliomas. MATERIALS AND METHODS: The intravoxel incoherent motion perfusion parameters were assessed in 21 brain gliomas (16 high-grade, 5 low-grade). Images were acquired by using a Stejskal-Tanner diffusion pulse sequence, with 16 values of b (0–900 s/mm(2)) in 3 orthogonal directions on 3T systems equipped with 32 multichannel receiver head coils. The intravoxel incoherent motion perfusion parameters were derived by fitting the intravoxel incoherent motion biexponential model. Regions of interest were drawn in regions of maximum intravoxel incoherent motion perfusion fraction and contralateral control regions. Statistical significance was assessed by using the Student t test. In addition, regions of interest were drawn around all whole tumors and were evaluated with the help of histograms. RESULTS: In the regions of maximum perfusion fraction, perfusion fraction was significantly higher in the high-grade group (0.127 ± 0.031) than in the low-grade group (0.084 ± 0.016, P < .001) and in the contralateral control region (0.061 ± 0.011, P < .001). No statistically significant difference was observed for the pseudodiffusion coefficient. The perfusion fraction correlated moderately with dynamic susceptibility contrast relative CBV (r = 0.59). The histograms of the perfusion fraction showed a “heavy-tailed” distribution for high-grade but not low-grade gliomas. CONCLUSIONS: The intravoxel incoherent motion perfusion fraction is helpful for differentiating high- from low-grade brain gliomas.
