Background The development of direct-acting antivirals (DAA) for HCV has revolutionized the treatment of HCV, including its treatment in patients with HIV coinfection. The aim of this study was to compare the changes in liver function between coinfected and monoinfected patients with cirrhosis who achieved HCV eradication by DAA. Methods Patients with pre-treatment diagnosis of HCV liver cirrhosis, consecutively enrolled in the multicenter PITER cohort, who achieved a sustained virological response 12 weeks after treatment cessation (SVR12) were analysed. Changes in Child-Pugh (C-P) class and the occurrence of a decompensating event was prospectively evaluated after the end of DAA treatment. Cox regression analysis was used to evaluate factors independently associated with changes in liver function following viral eradication. Results We evaluated 1350 patients, of whom 1242 HCV monoinfected (median follow-up 24.7, range 6.8–47.5 months after viral eradication) and 108 (8%) HCV/HIV coinfected (median follow-up 27.1, range 6.0–44.6). After adjusting for age, sex, HCV-genotype, HBsAg positivity and alcohol use, HIV was independently associated with a more advanced liver disease before treatment (C-P class B/C vs A) (OR: 3.73, 95% CI:2.00–6.98). Following HCV eradication, C-P class improved in 17/20 (85%) coinfected patients (from B to A and from C to B) and in 53/82 (64.6%) monoinfected patients (from B to A) (p = 0.08). C-P class worsened in 3/56 coinfected (5.3%) (from A to B) and in 84/1024 (8.2%) monoinfected patients (p = 0.45) (from A to B or C and from B to C). Baseline factors independently associated with C-P class worsening were male sex (HR = 2.00; 95% CI = 1.18–3.36), platelet count p = 0.83). Conclusions Improvement of liver function was observed following HCV eradication in the majority of patients with cirrhosis; however viral eradication did not always mean cure of liver disease in both monoinfected and coinfected patients with advanced liver disease.

Deep brain stimulation is a promising treatment for neuropsychiatric conditions such as major depression. It could be optimized by identifying neural biomarkers that trigger therapy selectively when symptom severity is elevated. We developed an approach that first used multi-day intracranial electrophysiology and focal electrical stimulation to identify a personalized symptom-specific biomarker and a treatment location where stimulation improved symptoms. We then implanted a chronic deep brain sensing and stimulation device and implemented a biomarker-driven closed-loop therapy in an individual with depression. Closed-loop therapy resulted in a rapid and sustained improvement in depression. Future work is required to determine if the results and approach of this n-of-1 study generalize to a broader population. This case report describes a biomarker-driven closed-loop therapy for depression using implanted electrodes to continually sense brain activity and automatically trigger direct brain electrical stimulation in high-depression states.

The realization of hybrid superconductor–semiconductor quantum devices, in particular a topological qubit, calls for advanced techniques to readily and reproducibly engineer induced superconductivity in semiconductor nanowires. Here, we introduce an on-chip fabrication paradigm based on shadow walls that offers substantial advances in device quality and reproducibility. It allows for the implementation of hybrid quantum devices and ultimately topological qubits while eliminating fabrication steps such as lithography and etching. This is critical to preserve the integrity and homogeneity of the fragile hybrid interfaces. The approach simplifies the reproducible fabrication of devices with a hard induced superconducting gap and ballistic normal-/superconductor junctions. Large gate-tunable supercurrents and high-order multiple Andreev reflections manifest the exceptional coherence of the resulting nanowire Josephson junctions. Our approach enables the realization of 3-terminal devices, where zero-bias conductance peaks emerge in a magnetic field concurrently at both boundaries of the one-dimensional hybrids., Advanced fabrication techniques enable a wide range of quantum devices, such as the realization of a topological qubit. Here, the authors introduce an on-chip fabrication technique based on shadow walls to implement topological qubits in an InSb nanowire without fabrication steps such as lithography and etching.

Introduction: Research indicate that construction site accidents are a global concern, and rates are rapidly increasing. In developing countries such as Saudi Arabia, safety issues are frequently ignored, and little is known about their causes. Objectives: This study aimed to shed light on factors causing accidents in Saudi Arabian construction companies. Methods: An online detailed survey, using Google Form, of accident features was distributed randomly to potential employees in 35 construction companies in Saudi Arabia, where one of the top administrators or safety officers were required to respond to the survey. It was conducted from 1st June to 31st August, 2013. The safety practices and perceptions of accident causes were assessed. Results: The response rate was 63%. Over half of the surveyed organizations encountered all of the selected accident types. While 19 (86%) of the construction companies maintained the equipment regularly, 15 (68%) had regular maintenance staff and 13 (59%) inspected the equipment before use. Although 18 (82%) of the workers were supplied with personal protective equipment (PPE), only 12 (55%) emphasized its use and offered site orientation for new employees. In the last part of the survey, respondents were requested to rate 25 factors affecting safety performance at the construction sites on a scale of 1 to 5, with 5 being the most important. The three most important factors of poor safety performance were the firm's top leaders, a lack of training, and the reckless operation of equipment. Conclusion: Changing attitudes of surrounding safety culture have the potential to significantly improve safety outcomes in the Saudi Arabian construction industry. Two Saudi Arabian corporations, Saudi Aramco and Saudi Chevron Petrochemical provide a positive model for increasing construction safety in the country, but there is a paucity of industry-level data. Further scholarly attention is strongly indicated.

Metamaterial-based double-slit complementary split rectangular resonator sensor is proposed for the characterisation of dielectric properties of the materials under test (MUTs). The proposed sensor is designed and simulated on the CST microwave studio software using a low-cost substrate FR4. An array of three identical resonators is etched in the ground plane of the sensor to achieve a single and deep notch of −58.7 dB in the transmission coefficient (S 21) at the resonant frequency of 7.01 GHz, which is the novelty of the proposed sensor. A deep and single resonant frequency band has a significant role in the precise measurement of the dielectric properties of the MUTs. The effective constitutive parameters are extracted from the S-parameters. An equivalent circuit model is suggested that describes the overall behaviour of the sensor. The sensor is fabricated on the FR4 substrate and measured through the vector network analyser (N5224B) by placing the standard materials. The parabolic equation for the proposed sensor is formulated to approximate the permittivity of the MUTs. A very small percentage of error, 0.77, is found which shows high accuracy of the sensor. This methodology is efficient, simple in fabrication, and reduces cost and computational time also.

Rationale and Objectives An increasing number of neurological complications and corresponding radiological findings have been reported in patients with COVID-19 infection. The purpose of this study is to systematically review the current literature on COVID-19-associated neuroradiological findings and examine the prevalence of different findings in patients with both severe and mild COVID-19 infection. Materials and Methods A comprehensive literature search of the PubMed and Embase databases was performed. Any studies reporting CT or MRI neuroimaging findings in patients with confirmed COVID-19 infection were included. Patient demographics, main radiological findings, neurological symptoms, and severity of COVID-19 infection were tabulated and quantified according to infection severity. Results Sixty-one studies published between 2019 and 2020 comprising 711 patients were analyzed according to severity of respiratory symptoms. The main neuroradiological findings for patients with mild classification were cranial nerve abnormalities, ischemic infarction, and white matter abnormalities, while the main findings in patients with severe classification were white matter abnormalities, ischemic infarction, and hemorrhagic events. Conclusion Neuroradiological manifestations in COVID-19 infection are highly heterogeneous and differ based on the severity of COVID-19 infection. Cranial nerve abnormalities appear exclusive to mild infection, with a high degree of olfactory tract involvement, while hemorrhagic events are more common in severe infection. Notably, ischemic infarction was equally prevalent in both mild and severe COVID-19 infection. Healthcare providers treating COVID-19 patients should be aware of these potential complications and consider neurological assessment and neuroimaging studies when indicated.

Detecting the transmission phase of a quantum dot via interferometry can reveal the symmetry of the orbitals and details of electron transport. Crucially, interferometry will enable the read-out of topological qubits based on one-dimensional nanowires. However, measuring the transmission phase of a quantum dot in a nanowire has not yet been established. Here, we exploit recent breakthroughs in the growth of one-dimensional networks and demonstrate interferometric read-out in a nanowire-based architecture. In our two-path interferometer, we define a quantum dot in one branch and use the other path as a reference arm. We observe Fano resonances stemming from the interference between electrons that travel through the reference arm and undergo resonant tunnelling in the quantum dot. Between consecutive Fano peaks, the transmission phase exhibits phase lapses that are affected by the presence of multiple trajectories in the interferometer. PURPOSE: To assess how well a brain MRI lesion segmentation algorithm trained at one institution performed at another institution, and to assess the effect of multi-institutional training datasets for mitigating performance loss. MATERIALS AND METHODS: In this retrospective study, a three-dimensional U-Net for brain MRI abnormality segmentation was trained on data from 293 patients from one institution (IN1) (median age, 54 years; 165 women; patients treated between 2008 and 2018) and tested on data from 51 patients from a second institution (IN2) (median age, 46 years; 27 women; patients treated between 2003 and 2019). The model was then trained on additional data from various sources: (a) 285 multi-institution brain tumor segmentations, (b) 198 IN2 brain tumor segmentations, and (c) 34 IN2 lesion segmentations from various brain pathologic conditions. All trained models were tested on IN1 and external IN2 test datasets, assessing segmentation performance using Dice coefficients. RESULTS: The U-Net accurately segmented brain MRI lesions across various pathologic conditions. Performance was lower when tested at an external institution (median Dice score, 0.70 [IN2] vs 0.76 [IN1]). Addition of 483 training cases of a single pathologic condition, including from IN2, did not raise performance (median Dice score, 0.72; P = .10). Addition of IN2 training data with heterogeneous pathologic features, representing only 10% (34 of 329) of total training data, increased performance to baseline (Dice score, 0.77; P < .001). This final model produced total lesion volumes with a high correlation to the reference standard (Spearman r = 0.98). CONCLUSION: For brain MRI lesion segmentation, adding a modest amount of relevant training data from an external institution to a previously trained model supported successful application of the model to this external institution. Background Anti-NMDA receptor encephalitis is an immune-mediated disorder characterized by antibodies against the GluN1 subunit of the NMDA receptor that is increasingly recognized as a treatable cause of childhood epileptic encephalopathy. In adults, the disorder has been associated with reversible changes in brain volume over the course of treatment and recovery, but in children, little is known about its time course and associated imaging manifestations. Case presentation A previously healthy 20-month-old boy presented with first-time unprovoked seizures, dysautonomia, and dyskinesia. Paraneoplastic workup was negative, but CSF was positive for anti-NMDAR antibodies. The patient’s clinical condition waxed and waned over a 14-month course of treatment with first- and second-line immunotherapies (including steroids, IVIG, rituximab, and cyclophosphamide). Serial brain MRIs scans obtained at 5 time points spanning this same period showed no abnormal signal or enhancement but were remarkable for cycles of reversible regional cortical volume loss. All scans included identical 1-mm resolution 3D T1-weighted sequences obtained on the same 3 T scanner. Using a novel longitudinal processing stream in FreeSurfer6 (Reuter M, et. al, Neuroimage 61:1402–18, 2012) we quantified the rate of change in cortical volume at each vertex (% volume change per month) between consecutive scans and correlated these changes with the time course of the patient’s treatment and clinical response. We found regionally specific changes in cortical volume (up to 7% per month) that preferentially affected the frontal and occipital lobes and paralleled the patient’s clinical course, with clinical decline associated with volume loss and clinical improvement associated with volume gain. Conclusions Our results suggest that reversible cortical volume loss in anti-NMDA encephalitis has a regional specificity that mirrors many of the clinical symptoms associated with the disorder and tracks the dynamics of disease severity over time. This case illustrates how quantitative morphometric techniques can be applied to clinical imaging data to reveal patterns of brain change that may provide insight into disease pathophysiology. More widespread application of this approach might reveal regional and temporal patterns specific to different types of autoimmune encephalitis, providing a tool for diagnosis and a surrogate marker for monitoring treatment response.

OBJECTIVE:\ud We estimated the cost consequence of Italian National Health System (NHS) investment in direct-acting antiviral (DAA) therapy according to hepatitis C virus (HCV) treatment access policies in Italy.\ud \ud METHODS:\ud A multistate, 20-year time horizon Markov model of HCV liver disease progression was developed. Fibrosis stage, age and genotype distributions were derived from the Italian Platform for the Study of Viral Hepatitis Therapies (PITER) cohort. The treatment efficacy, disease progression probabilities and direct costs in each health state were obtained from the literature. The break-even point in time (BPT) was defined as the period of time required for the cumulative costs saved to recover the Italian NHS investment in DAA treatment. Three different PITER enrolment periods, which covered the full DAA access evolution in Italy, were considered.\ud \ud RESULTS:\ud The disease stages of 2657 patients who consecutively underwent DAA therapy from January 2015 to December 2017 at 30 PITER clinical centres were standardized for 1000 patients. The investment in DAAs was considered to equal €25 million, €15 million, and €9 million in 2015, 2016, and 2017, respectively. For patients treated in 2015, the BPT was not achieved, because of the disease severity of the treated patients and high DAA prices. For 2016 and 2017, the estimated BPTs were 6.6 and 6.2 years, respectively. The total cost savings after 20 years were €50.13 and €55.50 million for 1000 patients treated in 2016 and 2017, respectively.\ud \ud CONCLUSIONS:\ud This study may be a useful tool for public decision makers to understand how HCV clinical and epidemiological profiles influence the economic burden of HCV.

Purpose fMRI is increasingly used for presurgical language mapping, but lack of standard methodology has made it difficult to combine/compare data across institutions or determine the relative efficacy of different approaches. Here, we describe a quantitative analytic framework for determining language laterality in clinical fMRI that addresses these concerns. Methods We retrospectively analyzed fMRI data from 59 patients who underwent presurgical language mapping at our institution with identical imaging and behavioral protocols. First, we compared the efficacy of different regional masks in capturing language activations. Then, we systematically explored how laterality indices (LIs) computed from these masks vary as a function of task and activation threshold. Finally, we determined the percentile threshold that maximized the correlation between the results of our LI approach and the laterality assessments from the original clinical radiology reports. Results First, we found that a regional mask derived from a meta-analysis of the fMRI literature better captured language task activations than masks based on anatomically defined language areas. Then, we showed that an LI approach based on this functional mask and percentile thresholding of subject activation can quantify the relative ability of different language tasks to lateralize language function at the population level. Finally, we determined that the 92nd percentile of subject-level activation provides the optimal LI threshold with which to reproduce the original clinical reports. Conclusion A quantitative framework for determining language laterality that uses a functionally-derived language mask and percentile thresholding of subject activation can combine/compare results across tasks and patients and reproduce clinical assessments of language laterality.

Hereditary hemochromatosis (HH) is an autosomal recessive genetic disorder that can lead to iron overload, causing oxidative damage to affected organs. HH type 1 is predominantly associated with homozygosity for the mutation p.C282Y. Previous case studies have reported tentative links between HH and movement disorders, e.g., Parkinson’s disease, and basal ganglia abnormalities on magnetic resonance imaging. We investigated the impact of p.C282Y homozygosity: on whole brain T2 intensity differences, a measure of iron deposition, and; on measures of movement abnormalities and disorders within UK Biobank. The neuroimaging analysis (154 p.C282Y homozygotes, 595 matched controls) showed that p.C282Y homozygosity was associated with decreased T2 signal intensity in motor circuits (basal ganglia, thalamus, red nucleus, and cerebellum; Cohen’s d > 1) consistent with substantial iron deposition. Across the whole UK Biobank (2,889 p.C282Y homozygotes, 496,968 controls), we found a significant enrichment for movement abnormalities in male homozygotes (OR (95% CI) = 1.82 (1.27-2.61), p=0.001), but not females (OR (95% CI) = 1.10 (0.69-1.78), p=0.71). Among the 31 p.C282Y homozygote males with a movement disorder only 7 had a concurrent HH diagnosis. These findings indicate susceptibility to iron overload in subcortical structures in p.C282Y homozygotes, and confirmed an increased risk of movement abnormalities and disorders in males. Given the effectiveness of early treatment in HH, screening for p.C282Y homozygosity in high risk individuals may offer a potential avenue to reduce iron accumulation in the brain and limit additional risk for the development of movement disorders among males.

Radiogenomics, defined as the integrated analysis of radiologic imaging and genetic data, is a well-established tool shown to augment neuroimaging in the clinical diagnosis, prognostication, and scientific study of late-onset Alzheimer disease (LOAD). Early work using candidate single nucleotide polymorphisms (SNPs) identified genetic variation in APOE, BIN1, CLU, and CR1 as key modifiers of brain structure and function using magnetic resonance imaging (MRI). More recently, polygenic risk scores used in conjunction with MRI and positron emission tomography have shown great promise as a risk-stratification tool for clinical trials and care-management decisions. In addition, recent work using multimodal MRI and positron emission tomography as proxies of LOAD progression has identified novel risk variants that are enhancing our understanding of LOAD pathophysiology and progression. Herein, we highlight key studies and trends in the radiogenomics of LOAD over the past two decades and their implications for clinical practice and scientific research.

Background: \ud Spontaneous coronary artery dissection (SCAD) is an increasingly recognized cause of acute coronary syndromes (ACS) afflicting predominantly younger to middle-aged women. Observational studies have reported a high prevalence of extracoronary vascular anomalies, especially fibromuscular dysplasia (FMD) and a low prevalence of coincidental cases of atherosclerosis. PHACTR1/EDN1 is a genetic risk locus for several vascular diseases, including FMD and coronary artery disease, with the putative causal noncoding variant at the rs9349379 locus acting as a potential enhancer for the endothelin-1 (EDN1) gene.\ud \ud Objectives: \ud This study sought to test the association between the rs9349379 genotype and SCAD.\ud \ud Methods: \ud Results from case control studies from France, United Kingdom, United States, and Australia were analyzed to test the association with SCAD risk, including age at first event, pregnancy-associated SCAD (P-SCAD), and recurrent SCAD.\ud \ud Results: \ud The previously reported risk allele for FMD (rs9349379-A) was associated with a higher risk of SCAD in all studies. In a meta-analysis of 1,055 SCAD patients and 7,190 controls, the odds ratio (OR) was 1.67 (95% confidence interval [CI]: 1.50 to 1.86) per copy of rs9349379-A. In a subset of 491 SCAD patients, the OR estimate was found to be higher for the association with SCAD in patients without FMD (OR: 1.89; 95% CI: 1.53 to 2.33) than in SCAD cases with FMD (OR: 1.60; 95% CI: 1.28 to 1.99). There was no effect of genotype on age at first event, P-SCAD, or recurrence.\ud \ud Conclusions: \ud The first genetic risk factor for SCAD was identified in the largest study conducted to date for this condition. This genetic link may contribute to the clinical overlap between SCAD and FMD.

An improvement of the wide-angular scan-loss compensation (SLC) and sidelobe level (SLL) in a small linear array with 23 elements is discussed. The array integrates small subarrays with an optimized pattern for SLC, while the SLL suppression is obtained by using a combination of elements with phase steering only and elements with amplitude/phase control in the center. This combination leads to a less number of T/R module. When the antenna is scanned to ±60°, a maximum first and peak SLL (FSLL and PSLL) of -15.1 dB are obtained. In addition, a -4.6 scan-loss is obtained, it means that the SLC is 2.4 dB with a Cavity-backed U-slotted Patch (CUP) antenna. These performance results are very attractive in particular for a small linear array with wide angular scanning capability.

Highlights • A U-Net incorporating spatial prior information can successfully segment 6 brain tissue types. • The U-Net was able to segment gray and white matter in the presence of lesions. • The U-Net surpassed the performance of its source algorithm in an external dataset. • Segmentations were produced in a hundredth of the time of its predecessor algorithm., Delineation and quantification of normal and abnormal brain tissues on Magnetic Resonance Images is fundamental to the diagnosis and longitudinal assessment of neurological diseases. Here we sought to develop a convolutional neural network for automated multiclass tissue segmentation of brain MRIs that was robust at typical clinical resolutions and in the presence of a variety of lesions. We trained a 3D U-Net for full brain multiclass tissue segmentation from a prior atlas-based segmentation method on an internal dataset that consisted of 558 clinical T1-weighted brain MRIs (453/52/53; training/validation/test) of patients with one of 50 different diagnostic entities (n = 362) or with a normal brain MRI (n = 196). We then used transfer learning to refine our model on an external dataset that consisted of 7 patients with hand-labeled tissue types. We evaluated the tissue-wise and intra-lesion performance with different loss functions and spatial prior information in the validation set and applied the best performing model to the internal and external test sets. The network achieved an average overall Dice score of 0.87 and volume similarity of 0.97 in the internal test set. Further, the network achieved a median intra-lesion tissue segmentation accuracy of 0.85 inside lesions within white matter and 0.61 inside lesions within gray matter. After transfer learning, the network achieved an average overall Dice score of 0.77 and volume similarity of 0.96 in the external dataset compared to human raters. The network had equivalent or better performance than the original atlas-based method on which it was trained across all metrics and produced segmentations in a hundredth of the time. We anticipate that this pipeline will be a useful tool for clinical decision support and quantitative analysis of clinical brain MRIs in the presence of lesions.

Semiconducting–superconducting hybrids are vital components for the realization of high-performance nanoscale devices. In particular, semiconducting–superconducting nanowires attract widespread interest owing to the possible presence of non-abelian Majorana zero modes, which are quasiparticles that hold promise for topological quantum computing. However, systematic search for Majoranas signatures is challenging because it requires reproducible hybrid devices and reliable fabrication methods. This work introduces a fabrication concept based on shadow walls that enables the in situ, selective, and consecutive depositions of superconductors and normal metals to form normal-superconducting junctions. Crucially, this method allows to realize devices in a single shot, eliminating fabrication steps after the synthesis of the fragile semiconductor/superconductor interface. At the atomic level, all investigated devices reveal a sharp and defect-free semiconducting–superconducting interface and, correspondingly, a hard induced superconducting gap resilient up to 2 T is measured electrically. While the cleanliness of the technique enables systematic studies of topological superconductivity in nanowires, it also allows for the synthesis of advanced nano-devices based on a wide range of material combinations and geometries while maintaining an exceptionally high interface quality.

Circuit quantum electrodynamics, where photons are coherently coupled to artificial atoms built with superconducting circuits, has enabled the investigation and control of macroscopic quantum-mechanical phenomena in superconductors. Recently, hybrid circuits incorporating semiconducting nanowires and other electrostatically-gateable elements have provided new insights into mesoscopic superconductivity. Extending the capabilities of hybrid flux-based circuits to work in magnetic fields would be especially useful both as a probe of spin-polarized Andreev bound states and as a possible platform for topological qubits. The fluxonium is particularly suitable as a readout circuit for topological qubits due to its unique persistent-current based eigenstates. In this Letter, we present a magnetic-field compatible hybrid fluxonium with an electrostatically-tuned semiconducting nanowire as its non-linear element. We operate the fluxonium in magnetic fields up to 1T and use it to observe the $\varphi_0$-Josephson effect. This combination of gate-tunability and field-compatibility opens avenues for the exploration and control of spin-polarized phenomena using superconducting circuits and enables the use of the fluxonium as a readout device for topological qubits.

A quasi-optical antenna system is designed for terahertz active standoff personnel screening in this paper. The design principle of the quasi-optical antenna is described in detail according to actual needs. The results from GRASP software shows that the system can achieve 10 mm imaging resolution over a 28 × 24 cm field of view at 2 m with a center frequency 180 GHz. it has the potential to be used in active terahertz standoff personnel screening.

The spatial resolution of terahertz (THz) image is often degraded by many factors during the imaging process. Therefore, a “Zero-Shot” super-resolution CNN framework, which is trained only by the input image itself, is proposed to cope with those degradation factors. With this unsupervised framework, the restoration level of the CNN can be flexibly adjusted based on the input THz image in order to achieve the best restoration effect. Experimental results on both the simulated data and real tested THz data have proved the effectiveness of our method.

Strong spin-orbit semiconductor nanowires coupled to a superconductor are predicted to host Majorana zero modes. Exchange (braiding) operations of Majorana modes form the logical gates of a topological quantum computer and require a network of nanowires. Here, we develop an in-plane selective-area growth technique for InSb-Al semiconductor-superconductor nanowire networks with excellent quantum transport properties. Defect-free transport channels in InSb nanowire networks are realized on insulating, but heavily mismatched InP substrates by 1) full relaxation of the lattice mismatch at the nanowire/substrate interface on a (111)B substrate orientation, 2) nucleation of a complete network from a single nucleation site, which is accomplished by optimizing the surface diffusion length of the adatoms. Meditation has shown to benefit a wide range of conditions and symptoms, but the neural mechanisms underlying the practice remain unclear. Magnetic resonance imaging (MRI) studies have investigated the structural brain changes due to the practice by examining volume, density, or cortical thickness changes. However, these studies have focused on adults; meditation’s structural effects on the adolescent brain remain understudied. In this study, we investigated how meditation training affects the structure of the adolescent brain by scanning a group of 38 adolescents (16.48 ± 1.29 years) before and after participating in a 12-week meditation training. Subjects underwent Training for Awareness, Resilience, and Action (TARA), a program that mainly incorporates elements from mindfulness meditation and yoga-based practices. A subset of the adolescents also received an additional control scan 12 weeks before TARA. We conducted voxel-based morphometry (VBM) to assess gray matter volume changes pre- to post-training and during the control period. Subjects showed significant gray matter (GM) volume decreases in the left posterior insula and to a lesser extent in the left thalamus and left putamen after meditation training. There were no significant changes during the control period. Our results support previous findings that meditation affects regions associated with physical and emotional awareness. However, our results are different from previous morphometric studies in which meditation was associated with structural increases. We posit that this discrepancy may be due to the differences between the adolescent brain and the adult brain.

During the real-aperture-scanning imaging process, terahertz (THz) images are often plagued with the problem of low spatial resolution. Therefore, an accommodative super-resolution framework for THz images is proposed. Specifically, the 3D degradation model for the imaging system is firstly proposed by incorporating the focused THz beam distribution, which determines the relationship between the imaging range and the corresponding image restoration level. Secondly, an adjustable CNN is introduced to cope with this range dependent super-resolution problem. By simply tuning an interpolation parameter, the network can be adjusted to produce arbitrary restoration levels between the trained fixed levels without extra training. Finally, by selecting the appropriate interpolation coefficient according to the measured imaging range, each THz image can be coped with its matched network and reach the outstanding super-resolution effect. Both the simulated and real tested data, acquired by a 160 ∼ 220 GHz imager, have been used to demonstrate the superiority of our method.

Deep brain stimulation is a promising treatment for severe depression, but lack of efficacy in randomized trials raises questions regarding anatomical targeting. We implanted multi-site intracranial electrodes in a severely depressed patient and systematically assessed the acute response to focal electrical neuromodulation. We found an elaborate repertoire of distinctive emotional responses that were rapid in onset, reproducible, and context and state dependent. Results provide proof of concept for personalized, circuit-specific medicine in psychiatry. Acute mapping of responses to focal electrical neuromodulation in a patient with treatment-resistant depression reveals distinct mood and symptom changes that are highly context dependent.

An advanced design, adding a significant first sidelobes level (FSLL) improvement to a previously introduced wide-angular-scanning, linear array prototype with demonstrated scan-loss compensation (SLC) and sidelobes suppression features is discussed. The linear array makes use of in-line subarrays for SLC and an additional amplitude taper in its central, uniform region for lowering the FSLL with as much as $4\mathrm{d}\mathrm{B}$ at $\pm 60^{\mathrm{o}}$ scanning ($13\mathrm{d}\mathrm{B}$ at $\pm 20^{\mathrm{o}}$ scanning). Several Taylor-type amplitudes tapers are compared, the best overall performance improvement being observed for a -18dB prototype taper. The advocated solution is highly suitable to high-sensitivity radars requiring a fast-scanning, fan-beam.

Effective October 1, 2018, the Member States of the European Union had to bring into force the laws, regulations and administrative provisions necessary to comply with Directive (EU) 2016/97 of the European Parliament and of the Council of January 20, 2016 on insurance distribution (IDD). The IDD arose out of a desire to give insurance customers equal protection regardless of the type of distributor from which they obtained insurance. Essentially, the IDD seeks to level the playing field of protections for insurance customers by simplifying, consolidating, and expanding customer protections when needed. The IDD has the stated goal of focusing on “the area of the disclosure of information” to customers. The directive is intentionally broad and applies “to persons whose activity consists of providing insurance or reinsurance distribution services to third parties.” Although it is much too early to predict the course of the IDD within the European Union, a comparison can be drawn with the Model Acts promulgated by the National Association of Insurance Commissioners (NAIC) in the United States to glean an inkling as to where the IDD might be headed. Parts of the Model Acts have been in place for a number of years and, while the legal regimes they cover are modestly different, there are nonetheless broad lessons that can be drawn in the comparison of the two. Whether the path of the IDD follows the arc of the Model Acts, or not, will perhaps be attributed to three instrumental aspects: 1. The IDD is unquestionably focused on customer protection. The NAIC is more nearly concerned with uniformity. It may be that the IDD’s focus will contribute to better traction among the EU Member States then Model Acts have experienced in the United States. 2. Unlike the IDD, the NAIC Model Acts are not comprehensive with respect to customer protection. 3. The NAIC Model Acts have seen inconsistent adoption by the states, a factor that has contributed to a lack of uniformity and constancy across any number of insurance products. While the IDD should not suffer from spotty adoption, the relative flexibility of the EU Member States in adopting more stringent rules may lead to a lack of uniformity and consistency similar to that of the Model Acts. Thus, the IDD may very well face the same headwinds faced by the Model Acts in the United States. Accurate predictions are always elusive when dealing with the implementation of regulation and legislation. Accordingly, we will watch with curiosity whether the IDD, which takes a much more global approach in customer protection, will see more success.

Dual split ring resonator based Metamaterial sensor materials is presented in this paper. This complementary split ring resonator (CSRR) is etched in the ground plane on back of the Microstrip line. Full wave electromagnetic simulation is done to verify the resonance phenomenon. Presented metamaterial sensor shows good result around 6GHz. Simulation results show S21 with −31 dB notch depth at resonant frequency. Electromagnetic properties like effective electric permittivity ( $\varepsilon$ ) and magnetic permeability ( $\mu$ ) of the sensor are derived from S-parameters simulation. Standard photolithography technique is used in fabrication of this metamaterial sensor using low cost FR4 substrate. S-parameters are measured by using Keysight technologies Vector Network Analyzer N5224B. S-parameters simulated and measured results show good agreement. By measurement of only resonant frequency (GHz) and the magnitude (dB) response of the sensor with material under test (MUT) the dielectric constant of the material can be calculated numerically. This nondestructive measurement technique has many applications in material testing for security and bio-sensing.

PURPOSE: To evaluate whether patients with neurofibromatosis type 1 (NF1)—a multisystem neurodevelopmental disorder with myriad imaging manifestations, including focal transient myelin vacuolization within the deep gray nuclei, brainstem, and cerebellum—exhibit differences in cortical and subcortical structures, particularly in subcortical regions where these abnormalities manifest. MATERIALS AND METHODS: In this retrospective study, by using clinically obtained three-dimensional T1-weighted MR images and established image analysis methods, 10 intracranial volume–corrected subcortical and 34 cortical regions of interest (ROIs) were quantitatively assessed in 32 patients with NF1 and 245 age- and sex-matched healthy control subjects. By using linear models, ROI cortical thicknesses and volumes were compared between patients with NF1 and control subjects, as a function of age. With hierarchic cluster analysis and partial correlations, differences in the pattern of association between cortical and subcortical ROI volumes in patients with NF1 and control subjects were also evaluated. RESULTS: Patients with NF1 exhibited larger subcortical volumes and thicker cortices of select regions, particularly the hippocampi, amygdalae, cerebellar white matter, ventral diencephalon, thalami, and occipital cortices. For the thalami and pallida and 22 cortical ROIs in patients with NF1, a significant inverse association between volume and age was found, suggesting that volumes decrease with increasing age. Moreover, compared with those in control subjects, ROIs in patients with NF1 exhibited a distinct pattern of clustering and partial correlations. DISCUSSION: Neurofibromatosis type 1 is characterized by larger subcortical volumes and thicker cortices of select structures. Most apparent within the hippocampi, amygdalae, cerebellar white matter, ventral diencephalon, thalami and occipital cortices, these neurofibromatosis type 1–associated volumetric changes may, in part, be age dependent. © RSNA, 2018 Online supplemental material is available for this article.

Hypothalamic hamartomas (HHs) are benign lesions that cause medically refractory seizures, behavioral disturbances, and endocrine dysfunction. Open resection of HHs does not guarantee seizure freedom and carries a relatively high risk of morbidity. Minimally invasive stereotactic laser ablation has recently been described as an effective and safe alternative for HH treatment. Prior studies have not, however, assessed HH lesion size and morphology, 2 factors that may influence treatment results and, ultimately, the generalizability of their findings. In this paper, the authors describe seizure outcomes for 5 pediatric patients who underwent laser ablation of sessile HHs. Lesions were treated using a frameless, interventional MRI-guided approach, which facilitated laser targeting to specific components of these complex lesions. The authors’ experiences in these cases substantiate prior work demonstrating the effectiveness of laser therapy for HHs, while elucidating HH complexity as a potentially important factor in laser treatment planning, and in the interpretation of early studies describing this treatment method.
