Introduction and objective: To state the severity shown by this pathology, updating its more appropriate diagnosis and therapeutic methods. Despite its typical bad prognosis, we can show that favorable cases exist. Description: 43-year-old male suffering nasal obstruction and epistaxis, whose endoscopy reveals an erosive, bleeding and voluminous mass. Results: 3-year-survival, up to now. Discussion: Rare entity in western countries, but more frequent in Asia, that has been treated in different ways all along the years. However, except in unusual cases, prognosis is fateful. Serious complications, even as a result of treatment, can come up and lead to death. Conclusions: Unusual and of a poor prognosis entity, without a standardized treatment. However, there are chances of a good response to it, with satisfactory prospects of the survival rate., Introducción y objetivo: Poner de manifiesto la severidad de la patología presentada, actualizando los métodos diagnósticos y terapéuticos más apropiados, así como demostrar que, a pesar del mal pronóstico conocido, existen casos favorables. Descripción: Varón de 43 años con dificultad respiratoria nasal y epistaxis, cuya endoscopia nasal muestra lesión erosiva, sangrante y voluminosa. Resultados: Supervivencia de al menos 3 años, hasta este momento. Discusión: enfermedad poco frecuente en el mundo occidental, aunque de mayor incidencia en Asia, cuyas opciones de tratamiento han ido variando. No obstante, salvo excepciones, el pronóstico es poco favorable, padeciendo severas complicaciones que pueden desembocar en el fallecimiento. Conclusiones: Patología poco frecuente con mal pronóstico, sin tratamiento estandarizado. Sin embargo, se han descrito casos de buena respuesta con buenas perspectivas de supervivencia.

Objective To inspect and describe the results of tracheotomies carried out at our ENT Department and to assess changes in indications over the last few decades. Material and methods Retrospective study of 13 patients, aged 3 months to 14 years, admitted by the ENT Department to the Paediatric ICU at Puerta del Mar University Hospital for elective tracheotomy over a 92-month period. Results The main indication is shared by obstructive respiratory insufficiency, post-surgery extubation failure and as a means of assisting ventilation in children requiring prolonged intubation. Conclusions In the last 30 years the indications for paediatric tracheotomy have changed and the number of children needing intubation for more than 20–30 days has increased. It is no longer an emergency procedure to overcome a blockage in the upper airways but has instead become a technique for maintaining assisted ventilation for the medium to long term.

Pneumoparotiditis is related to the cause of an infrequent increase in the size of the parotid gland due to the entrance of air through the Stenon duct following an increase in the pressure in the oral cavity. It is observed in musicians who play wind instruments, glass blowers, patients undergoing diagnostic or therapeutic techniques, in adolescents that provoke it intentionally, and in mentally handicapped people as found in the clinical case reported here. The symptoms are usually non-specific and the diagnosis is confirmed with an axial tomography. The interest in this clinical case lies in the peculiar form of presentation and its diagnostic confirmation through the use of CT imaging.

La neumoparotiditis supone una causa infrecuente de aumento de tamano de la glandula parotida debido a la entrada de aire por el conducto de Stenon secundaria al incremento de presion en la cavidad oral. Se observa en musicos de instrumentos de viento, sopladores de vidrio, pacientes sometidos a tecnicas diagnosticas y/o terapeuticas y, de forma intencionada, en adolescentes y disminuidos psiquicos, como en el caso clinico que presentamos. La clinica suele ser inespecifica y el diagnostico se confirma mediante tomografia computarizada. El interes del caso clinico radica en la particular forma de presentacion y la confirmacion diagnostica mediante la imagen de la tomografia.
