AimThe study aimed to examine the optimal anterior-posterior depth which will reduce the time to return of spontaneous circulation and improve survival during chest compressions. Asphyxiated neonatal piglets receiving chest compression resuscitated with a 40% anterior-posterior chest depth compared with 33%, 25% or 12.5% will have reduced time to return of spontaneous circulation and improved survival.MethodsNewborn piglets (n=8 per group) were anaesthetised, intubated, instrumented and exposed to 45 min normocapnic hypoxia followed by asphyxia and cardiac arrest. Piglets were randomly allocated to four intervention groups (‘anterior-posterior 12.5% depth’, ‘anterior-posterior 25% depth’, ‘anterior-posterior 33% depth’ or ‘anterior-posterior 40% depth’). Chest compressions were performed using an automated chest compression machine with a rate of 90 per minute. Haemodynamic and respiratory parameters, applied compression force, and chest compression depth were continuously measured.ResultsThe median (IQR) time to return of spontaneous circulation was 600 (600–600) s, 135 (90–589) s, 85 (71–158)* s and 116 (63–173)* s for the 12.5%, 25%, 33% and 40% depth groups, respectively (*pConclusionsTime to return of spontaneous circulation and survival were similar between 25%, 33% and 40% anterior-posterior depths, while 12.5% anterior-posterior depth did not result in return of spontaneous circulation or survival. Haemodynamic and respiratory parameters improved with increasing anterior-posterior depth, suggesting improved organ perfusion and oxygen delivery with 33%–40% anterior-posterior depth.Trial registration numberPTCE0000193.

Background Chest compression (CC) during sustained inflations (CC+SI) compared with CC with asynchronized ventilation (CCaV) during cardiopulmonary resuscitation in asphyxiated pediatric piglets will reduce time to return of spontaneous circulation (ROSC). Methods and Results Piglets (20–23 days of age, weighing 6.2–10.2 kg) were anesthetized, intubated, instrumented, and exposed to asphyxia. Cardiac arrest was defined as mean arterial blood pressure P =0.0292). There was a 100% higher rate of ROSC in the CC+SI group versus CCaV group, with 10 (83%) versus 5 (42%) achieving ROSC ( P =0.089), respectively. Piglets in the CC+SI and CCaV groups received intravenous epinephrine boluses to achieve ROSC (8/12 versus 10/12 P =0.639). There was a significantly higher minute ventilation in the CC+SI group, which was secondary to a 5‐fold increase in the number of inflations per minute and a 1.5‐fold increase in tidal volume. Conclusions CC+SI reduced time to ROSC and improved survival compared with using CCaV. CC+SI allowed passive ventilation of the lung while providing chest compressions. This technique warrants further studies to examine the potential to improve outcomes in pediatric patients with cardiac arrest. Background: Male newborns have a greater risk of poor cardiovascular and respiratory outcomes compared to females. The mechanisms associated with the “male disadvantage” remains unclear. We have previously shown no difference between male and female newborn piglets during hypoxia, asphyxia, resuscitation, and post-resuscitation recovery. However, it is unknown if there are differences in resuscitation outcomes between males and females during different cardiopulmonary resuscitation techniques.Intervention and Measurements: Secondary analysis of 184 term newborn mixed breed duroc piglets (1–3 days of age, weighing 2.0 (0.2) kg) from seven different studies, which were exposed to 30–50 min of normocapnic hypoxia followed by asphyxia until asystole. This was followed by cardiopulmonary resuscitation. For the analysis, piglets were divided into male and female groups, as well as resuscitation technique groups (sustained inflation, 3:1 compression-to-ventilation ratio, or asynchronous ventilations during chest compressions). Cardiac function, carotid blood flow, and cerebral oxygenation were continuously recorded throughout the experiment.Main results: Regardless of resuscitation technique, there was no significant difference between males and females in the number achieving return of spontaneous circulation (ROSC) [95/123 (77%) vs. 48/61 (79%)], the time to achieve ROSC [112 (80–185) s vs. 110 (77–186) s], and the 4-h survival rate [81/95 (85%) vs. 40/48 (83%)]. Levels of the injury markers interleukin (IL)−1ß, IL-6, IL-8, and tumor necrosis factor-α in frontoparietal cortex tissue homogenates were similar between males and females.Conclusions: Regardless of resuscitation technique, there was no significant effect of sex on resuscitation outcome, survival, and hemodynamic recovery in asphyxiated newborn piglets.

Background: Pediatric resuscitation guidelines recommend continuous chest compressions (CC) with asynchronized ventilation (CCaV) during cardiopulmonary resuscitation (CPR) once an airway has been secured. However, the optimal CC technique remains unknown. Aim: To determine if CCs during sustained inflations (SI) improves return of spontaneous circulation (ROSC) of asphyxiated pediatric piglets compared to CCaV. Intervention and measurements: Piglets (age 20-23 days) were anesthetized, intubated, instrumented, and asphyxiated. Protocolized resuscitation was initiated when mean arterial blood pressure was 2 O for 30sec, CCs were provided at a rate of 100/min, and SI was interrupted after 30 sec for one second before a further 30sec SI was provided. CC and SI were continued until ROSC. Respiratory parameters, carotid blood flow, systemic artery pressures, and cerebral oxygenation were measured. Main results: Median (IQR) time to ROSC was significantly decreased in the CC+SI group compared to CCaV with 248 (41-346)sec vs. 720 (167-720)sec (p=0.049), respectively. CC+SI had a 100% higher rate of ROSC compared to CCaV with 10(83%) vs. 5(42%) (p=0.089), respectively. Piglets in the CC+SI group received a median (IQR) 3.5 (0-5.8) intravenous epinephrine boluses compared to 8.0 (1.8-11) in the CCaV group (p=0.123). Conclusions: Combining CC and SI significantly improved time to ROSC and survival in asphyxiated pediatric piglets when compared to the current standard of CCs with CCaV during pediatric CPR.

In low-power wide area networks (LPWAN), a considerable number of end devices (EDs) communicate with the gateway in a certain area, whereas for transmitted data, a low data rate and high latency are allowed. Long-range (LoRa), as one of the LPWAN technologies, considers pure ALOHA and chirp spread spectrum (CSS) in the media access control (MAC) and physical (PHY) layers such that it can improve the energy efficiency while mitigating inter-cell interference (ICI). This paper investigates the system throughput of LoRa networks under the assumption that the interferences between EDs for exclusive regions are ignored using CSS. In order to establish an analytical model for the performance of LoRa, we introduce the pure ALOHA capture model, which is the power threshold model. For this model, we assume that the interfering power is proportional to the length of the time overlapped. In addition, we discuss LoRa gain by comparing the total throughput of LoRa with that of non-CSS.

Peroxisomal D-bifunctional protein (DBP), encoded by the HSD17B4 gene, catalyzes β-oxidation of very long chain fatty acids (VLCFAs). The deficiency of this peroxisomal enzyme leads to the accumulation of VLCFAs, causing multisystemic manifestations including the brain, retina, adrenal gland, hearing, and skeletal system. Herein, we report the first Korean neonatal case of peroxisomal DBP deficiency and the clinical prognosis over 2 years. This patient showed craniofacial dysmorphism, club foot, and seizures with cyanosis one day after birth. Elevated VLCFAs levels were indicative of a peroxisomal disorder. Targeted exome sequencing was performed and two missense mutations p.Asp117Val and p.Phe279Ser in the HSD17B4 gene were identified. The patient had type III DBP deficiency; therefore, docosahexaenoic acid and non-soluble vitamins were administered. However, progressive nystagmus, optic nerve atrophy, and bilateral hearing defects were observed and follow-up brain imaging revealed leukodystrophy and brain atrophy. Multiple anti-epileptic drugs were required to control the seizures. Over two years, the patient achieved normal growth with home ventilation and tube feeding. Hereby, the subject's parents had support during the second pregnancy from the proven molecular information. Moreover, targeted exome sequencing is an effective diagnostic approach, considering genetic heterogeneity of Zellweger spectrum disorders., Graphical Abstract

As the significance of increased power consumption in modern society is emphasized, the need for technology to enhance energy efficiency is also increasing. The power consumption of buildings comprises a large proportion of the total energy consumption and systematic methods are needed in order to manage it effectively. This paper proposes a multiple power-based building energy management system (MPBEMS) for the efficient management of building energy. MPBEMS means a system that integrates and manages multiple power produced in one or more ways. The analysis of the big-data based power usage measured in different types of buildings utilizes the power distribution method between multiplepower sources. The paper also suggests and verifies an energy prediction model for efficient building energy management, called the Adaptive Energy Consumption Prediction (AECP) algorithm. This paper proposes a building energy management method using an energy prediction model and analyzes its efficiency by applying it to actual buildings. The result of the efficiency analysis using the proposed system shows an annual electricity rate reduction efficiency of 5%.

Background: Open excision of the olecranon bursa has been performed traditionally. However, surgical complications such as wound healing problems and recurrence may occur after the surgery. The purpose of this study was to report on the clinical outcomes of endoscopic olecranon bursal resection performed in both septic and aseptic olecranon bursitis.Methods: We retrospectively reviewed 30 patients who underwent endoscopic olecranon bursal resection from June 2007 to January 2012. There were 20 males and 10 females. The ages ranged from 22 to 80 years, with an average age of 57.4 years and the average follow-up was 21.1 months (6–61.5 months). There were 15 cases in the septic group. The treatment outcome was measured according to the following; the rate of recurrence, range of motion, complications associated with surgery, VAS and QuickDASH.Results: There were no complications such as postoperative infection or neurovascular injuries. In the septic group, the VAS and QuickDASH scores were significantly improved from 5.6 to 0.1 and from 28 to 1.3, respectively. In the aseptic group, the VAS and QuickDASH scores were improved from 0.6 to 0.1 and from 25.7 to 0.5, respectively. In all cases, there were no recurrences and no limitations of joint motion until the final follow-up.Conclusions: We were able to obtain excellent outcomes without recurrence by performing endoscopic olecranon bursal resection in both septic and aseptic olecranon bursitis.

Approximately 0.1% for term and 10–15% of preterm infants receive chest compression (CC) in the delivery room, with high incidence of mortality and neurologic impairment. The poor prognosis associated with receiving CC in the delivery room has raised concerns as to whether specifically-tailored cardiopulmonary resuscitation methods are needed. The current neonatal resuscitation guidelines recommend a 3:1 compression:ventilation ratio; however, the most effective approach to deliver chest compression is unknown. We recently demonstrated that providing continuous chest compression superimposed with a high distending pressure or sustained inflation significantly reduced time to return of spontaneous circulation and mortality while improving respiratory and cardiovascular parameters in asphyxiated piglet and newborn infants. This review summarizes the current available evidence of continuous chest compression superimposed with a sustained inflation.

We present a highly efficient double plasma mirror (DPM) that provides ultrahigh-contrast multi-petawatt (PW) laser pulses with a temporal contrast ratio reaching 1 0 17 up to 160 ps and 1 0 12 up to 2 ps before the main pulse. The high reflectivity of 70%, along with the high-contrast enhancement factor of 700,000, was achieved from the DPM installed after the final stage of a 4 PW Ti:sapphire laser. The 4 PW laser was equipped with cross-polarized wave generation and optical parametric chirped-pulse amplification stages for initial high-contrast operation. The DPM operation was undertaken with conditions that did not modify the spatiotemporal profiles of incident multi-PW laser pulses. This highly efficient DPM with the high-contrast enhancement promises the utilization of multiple PMs as a practical rear end for upcoming tens of petawatt lasers to achieve ultrahigh temporal contrast.

Advances in medicine and dietary standards over recent decades have remarkably increased human life expectancy. Unfortunately, the chance of developing age-related diseases, including neurodegenerative diseases (NDDs), increases with increased life expectancy. High metabolic demands of neurons are met by mitochondria, damage of which is thought to contribute to the development of many NDDs including Parkinson’s disease (PD). Mitochondrial damage is closely associated with the abnormal production of reactive oxygen species (ROS), which are widely known to be toxic in various cellular environments, including NDD contexts. Thus, ways to prevent or slow mitochondrial dysfunction are needed for the treatment of these NDDs. In this review, we first detail how ROS are associated with mitochondrial dysfunction and review the cellular mechanisms, such as the mitochondrial quality control (MQC) system, by which neurons defend against both abnormal production of ROS and the subsequent accumulation of damaged mitochondria. We next highlight previous studies that link mitochondrial dysfunction with PD and how dietary antioxidants might provide reinforcement of the MQC system. Finally, we discuss how aging plays a role in mitochondrial dysfunction and PD before considering how healthy aging through proper diet and exercise may be salutary.

OBJECTIVES: Drug–drug interactions (DDIs) can result in patient harm. DDI alerts are intended to help prevent harm; when the majority of alerts presented to providers are being overridden, their value is diminished. Our objective was to evaluate the overall rates of DDI alert overrides and how rates varied by specialty, clinician type, and patient complexity. METHODS: A retrospective study of DDI alert overrides that occurred during 2012 and 2013 within the inpatient setting described at the medication-, hospital-, provider-, and patient encounter–specific levels was performed at an urban, quaternary-care, pediatric hospital. RESULTS: There were >41 000 DDI alerts presented to clinicians; ∼90% were overridden. The 5 DDI pairs that were most frequently presented and overridden included the following: potassium chloride–spironolactone, methadone–ondansetron, ketorolac–ibuprofen, cyclosporine–fluconazole, and potassium chloride–enalapril, each with an alert override rate of ≥0.89. Override rates across provider groups ranged between 0.84 and 0.97. In general, patients with high complexity had a higher frequency of alert overrides, but the rates of alert overrides for each DDI pairing did not differ significantly. CONCLUSIONS: High rates of DDI alert overrides occur across medications, provider groups, and patient encounters. Methods to decrease DDI alerts which are likely to be overridden exist, but it is also clear that more robust and intelligent tools are needed. Characteristics exist at the medication, hospital, provider, and patient levels that can be used to help specialize and enhance information transmission.

Purpose We evaluated three blood glucose self-monitoring for measuring whole blood glucose levels in preterm and low-birth-weight infants. Methods Between December 1, 2012 and March 31, 2013, 230 blood samples were collected from 50 newborns, who weighed, ≤2,300 g or were ≤36 weeks old, in the the neonatal intensive care unit of Eulji University Hospital. Three blood glucose self-monitoring (A: Precision Pcx, Abbott; B: One-Touch Verio, Johnson & Johnson; C: LifeScan SureStep Flexx, Johnson & Johnson) were used for the blood glucose measurements. The results were compared to those obtained using laboratory equipment (D: Advia chemical analyzer, Siemens Healthcare Diagnostics Inc.). Results The correlation coefficients between laboratory equipment and the three blood glucose self-monitoring (A, B, and C) were found to be 0.888, 0.884, and 0.900, respectively. For glucose levels≤60 mg/dL, the correlation coefficients were 0.674, 0.687, and 0.679, respectively. For glucose levels>60 mg/dL, the correlation coefficients were 0.822, 0.819, and 0.839, respectively. All correlation coefficients were statistically significant. And the values from the blood glucose self-monitoring were not significantly different from the value of the laboratory equipment , after correcting for each device's average value (P>0.05). When using laboratory equipment (blood glucose ≤60 mg/dL), each device had a sensitivity of 0.458, 0.604, and 0.688 and a specificity of 0.995, 0.989, and 0.989, respectively. Conclusion Significant difference is not found between three blood glucose self-monitoring and laboratory equipment. But correlation between the measured values from blood glucose self-monitoring and laboratory equipment is lower in preterm or low-birth-weight infants than adults.

This paper focuses on evaluating the performance of a cellular network using power control based frequency reuse partitioning (FRP) in downlink (DL). In our work, in order to have the realistic environment, the spectral efficiency of the system is evaluated through traffic analysis, which most of the previous works did not consider. To further decrease the cell edge user’s outage, the concept of power ratio is introduced and applied to the DL FRP based cellular network. In considering network topology, we first divide the cell coverage area into two regions, the inner and outer regions. We then allocate different sub-bands in the inner and outer regions of each cell. In the analysis, for each zone ratio, the performance of FRP system is evaluated for the given number of power ratios. We consider performance metrics such as call blocking probability, channel utilization, outage probability and effective throughput. The simulation results show that there is a significant improvement in the outage experienced by outer UEs with power control scheme compared to that with no power control scheme and an increase in overall system throughput.

This article presents low-power injection-locked frequency dividers (ILFD) operating in the Ka band. To achieve low-power operation, we use two circuit techniques to reduce the effects of the capacitance and resistance. To achieve a high oscillation frequency by reducing the effective capacitance, a distributed LC tank with a split auxiliary negative-gm cell is used. To achieve a high loop gain for a given power, we reduce the gate resistance by modifying the foundry-provided transistor layout. Two versions of the divider, which have different inductor implementations, are fabricated in 1-poly 9-metal 65-nm low-power RFCMOS. When biased at 1 V and 1 mA, the ILFD shows successful injection locking at 34 GHz with −35 dBm input power. When biased at 1.2 V and 3 mA, the input frequency ranges from 32.3 to 34.6 GHz at 5 dBm input power. Good phase noise spectral density of −126.6 and −132.0 dBc/Hz at a 1-MHz offset is achieved. © 2015 Wiley Periodicals, Inc. Microwave Opt Technol Lett 57:1379–1383, 2015

To improve millimeter-wave amplifier performance, we propose a transistor layout that reduces the gate access resistance. The proposed layout trades overlap capacitance for reduced gate access resistance, as shown by transistor parameter extraction. The proposed transistor layout improves the maximum available power gain by 1.5 dB at 67 GHz. The extrapolated maximum frequency of oscillation (f max) also increases by 23 % from 91.3 to 112.5 GHz. Using the improved transistor layout, we design a V-band power amplifier that has enhanced performance. The power amplifier fabricated using the proposed transistor achieves 15.1 dB of small-signal gain, 16.5 GHz of 3-dB bandwidth, and an output power of 7.8 dBm at 61.5 GHz. Compared to an amplifier using conventional layout, the small-signal gain is improved by 3.7 dB and the output power is increased by 3 dBm at 61.5 GHz.

Coordinated multi-point (CoMP) transmission and reception with joint transmission (JT) allows some neighbouring base stations (BSs) to transmit a user equipment (UE) in the cell edge region its downlink traffic simultaneously such that it can improve the cell edge throughput while mitigating inter-cell interference (ICI). This letter investigates the outage probability of CoMP-JT based on Alamouti scheme, assuming that the signals transmitted from BSs to a UE in the cell edge region undergo path-loss and Rayleigh fading while the UE is randomly located in that region. In addition, we discuss a CoMP-JT gain by comparing the outage probability of CoMP-JT with that of non-CoMP.

This paper describes a 28GHz CMOS direct conversion transceiver with packaged 2×4 patch antenna arrays for 5G communication. Test results show good RF performances of Rx NF 6.7dB, Maximum Tx EIRP 31.5dBm (1PA P out_sat =10.5dBm), LO integrated phase noise −37.8dBc (0.67°), Rx/Tx EVM around 2.2% (−33.1dB) at mid RF power, and well-fitted beam control capability.

Background The purpose of this study was to suggest a strategic approach to the management of anteromedial coronoid facet (AMCF) fracture that is related to posteromedial rotational instability of the elbow through investigation of the surgical outcome of diverse combinations, including internal fixation of AMCF fractures and repair of collateral ligament injury. Methods The study enrolled 18 patients. On the basis of the size of the coronoid fracture and the degree of the soft tissue injuries that were evaluated with computed tomography, magnetic resonance imaging, and varus stress test under anesthesia, these fractures were managed differently. Functional outcomes were evaluated with the visual analog scale score, modified Mayo Elbow Performance Score, and Disabilities of the Arm, Shoulder and Hand score. Plain radiographs were used to evaluate the degree of arthrosis. Results There were 2 cases of O’Driscoll type 2, subtype 1 fractures; 14 cases of type 2, subtype 2 fractures; and 2 cases of type 2, subtype 3 fractures. Seven cases were managed with only AMCF fracture fixation, 4 cases with only lateral ulnar collateral ligament (LUCL) repair, 6 cases with concomitant repair of the LUCL and AMCF fracture, and 1 case with a conservative method. There were no significant differences among O’Driscoll types and among the subgroups of type 2-2 ( P > .05). When the elbow showed instability on the varus stress test after AMCF fracture fixation, the soft tissue injuries of the lateral elbow were more severe ( P =. 015). Arthrosis was not correlated with the patient's symptoms ( P > .05). Conclusion AMCF fracture can be treated by only AMCF fracture fixation, only LUCL repair, or a combination of these techniques, depending on the size of the AMCF fracture fragment and the degree of the lateral soft tissue injuries.

Coordinated multi-point transmission (CoMP) is considered to be a promising technique to improve the throughput for LTE-Advanced systems. One important approach for CoMP is Joint Transmission (JT). However, the analytical model of JT has not been fully studied, as user equipments (UEs) receiving the desired signals from an adjacent base station (BS) as well as serving BS, or only serving BS were not distinguished. We derive a new analytical model to describe the call admission control in JT based systems. The performance measures of interest are the call blocking probability, and resource utilization. Furthermore, we compare the performance of JT-based systems and non-JT- based systems. The analytical results are in reasonable agreement with the simulation results.

In heterogeneous networks, the almost blank subframes (ABS) for inter-cell interference coordination (ICIC), which can be protected from the CCI due to unutilized subframes (i.e., ABS) is proposed. However, the analytical model for ABS-based systems has not been fully studied yet. In this paper, we derive a new analytical model to evaluate the performance of ABS-based systems. In an analytic model, we assume that each carrier in multicarrier systems, such as in OFDMA, is subject to large-scale fading, which is independent of other carriers. As a performance measure, we present the cumulative distribution function (CDF) for the effective SINR. We show the accuracy of the analytical model via simulation results.

For applications of 5G (5th generation mobile networks) communication systems, dual-polarized patch array antenna operating at 28.5 GHz is designed on the package substrate. To verify the radiation performance of designed antenna itself, a test package including two patch antennas is also design and its scattering parameters were measured. Using a large height of dielectric materials, 1.5 ∼ 2.0 GHz of antenna bandwidth is achieved which is wide enough. Besides, the dielectric constants are reduced to reflect variances of material properties in the higher frequency region. Measured results of the test package show a good performance at the operating frequency, indicating that the fabricated antenna package will perform well, either. In the future work, manufacturing variances will be investigated further.

The low-power, low-cost femtocell network has been proposed not only to alleviate traffic load to the macro base station (eNB) but also to cover the indoor coverage hole problem. However, in the dense femtocell environment where many femtocells are deployed to cover the whole large office building, performance of such femtocell environment can be deteriorated due to severe co-channel interference problem between the eNB and femtocells and among neighboring femtocells. In particular, a macro UE(mUE) located within femtocell coverage may experience severe co-channel interference from surrounding femtocells. Therefore, In this paper, we propose a novel power control schemes to mitigate interference to a mUE under such dense LTE femtocell environment. With proposed femtocell power control schemes, performance of the mUE can be greatly improved in terms of the outage probability and the SINR while maintaining satisfying femtocell performance. Simulation based performance study shows that the proposed power control scheme is able to enhance mUE performance more than 30% than the conventional dense femtocell in terms of the two performance metrics.

In the presence of a high power cellular network, picocells are added to a Macro-cell layout aiming to enhance total system throughput from cell-splitting. While because of the different transmission power between macrocell and picocell, and co-channel interference challenges between the existing macrocell and the new low power node-picocell, these problems result in no substantive improvement to total system effective throughput. Some works have investigated on these problems. Pico Cell Range Expansion (CRE) technique tries to employ some methods (such as adding a bias for Pico cell RSRP) to drive to offload some UEs to camp on picocells. In this work, we propose two solution schemes (including cell selection method, channel allocation and serving process) and combine new adaptive frequency partitioning reuse scheme to improve the total system throughput. In the simulation, we evaluate the performances of heterogeneous networks for downlink transmission in terms of channel utilization per cell (pico and macro), call blocking probability, outage probability and effective throughput. The simulation results show that the call blocking probability and outage probability are reduced remarkably and the throughput is increased effectively.

In this paper, we propose the cooperative transmission scheme (CTS) for system throughput maximization in OFDMA based enterprise femtocell networks. In our scheme, the user equipment (UE) can receive the desired signal from serving femtocell BS (fBS) as well as an adjacent fBS. Thus, UE achieves an improved signal to interference plus and noise ratio (SINR) by the synchronized two signals. The performances of this strategy consider not only the call-level quality of service (QoS) but also the packet-level QoS. We first measure the call blocking probability and utilization for the downlink resources for various offered load in femtocell. Based on that, the outage probability and effective throughput of the system are simulated. Simulation results show that the proposed scheme can reduce the outage probability for enterprise femtocell compared with conventional systems.

One of the most fundamental functions required for an intelligent agent is to estimate its current position based upon uncertain sensor data. In this paper, we explain the implementation of a robot localization system using Particle filters, which are the most effective one of the probabilistic localization methods, and then present the result of experiments for evaluating the performance of our system. Through conducting experiments to compare the effect of the noise-free model with that of the noisy state transition model considering inherent errors of robot actions, we show that it can help improve the performance of the Particle filter localization to apply a state transition model closely approximating the uncertainty of real robot actions.

PURPOSE Response to neo-adjuvant chemotherapy is an important prognostic factor for osteosarcoma (OS) and the Ewing sarcoma family of tumors (ESFT). [F-18]-fluorodeoxy-D-glucose (FDG)-positron emission tomography (PET) is a non-invasive imaging modality that predicts histologic response to chemotherapy of various malignancies; however, limited data exist about the usefulness of FDG-PET in predicting the histologic response of pediatric bone tumors to chemotherapy. We analyzed the FDG-PET imaging characteristics of pediatric bone tumors and determined the association with response to chemotherapy. MATERIALS AND METHODS Pediatric patients with OS (n=19) or ESFT (n=17) were evaluated for FDG-PET standard uptake values before (SUV1) and after (SUV2) chemotherapy. The relationship to the chemotherapy response was assessed by histopathology in surgically-excised tumors. A complete data set (SUV1, SUV2, and histologic response) was available in 23 patients. RESULTS While the mean SUV1s were not different between patients with OSs and ESFTs (9.44 vs. 6.07, p=0.24), the SUV2s were greater in the patients with OSs than ESFTs (4.55 vs. 1.66, p=0.01). The ratios of SUV2-to-SUV1 (SUV2 : SUV1) were 0.65 and 0.35 for OS and ESFT, respectively (p=0.08). All of the patients with ESFTs and 47% of the patients with OS had a favorable histologic response to chemotherapy. The SUV(2 : 1) [(SUV1-SUV2)/SUV1]≥0.5 and SUV2≤2.5 were related to favorable histologic responses to chemotherapy; the sensitivity and specificity of SUV(2 : 1) at 0.5 and SUV2 at 2.5 were 93% and 88%, and 88% and 78%, respectively. CONCLUSION FDG-PET can be used as a non-invasive surrogate to predict response to chemotherapy in children with bone tumors.

In this paper, we analyze and propose the Joint Call Admission Control(JCAC) scheme to combine network selection scheme and radio resource reservation based Call Admission Control(CAC) in LTE-WLAN heterogeneous networks. First, We propose the JCAC system that uses network decision rate to select a network for terminal and radio resource reservation scheme in overlaying LTE-WLAN network environment. And we analyze the performance of a proposed system using markov chain model. The performance is presented in terms of the new call blocking probability, handoff call dropping probability, and channel utilization of each network. As a performance result of the our research, the system using JCAC is better than the system using non-JCAC. We found a suitable resource reservation rate that is 10% in the system using JCAC. Our work may be useful as a guideline of resource reservation rate to introduce JCAC system using resource reservation scheme in overlaying LTE-WLAN network environment.

High-dose methotrexate (MTX) chemotherapy extends the duration of hospitalization and introduces the risks of serious complications related to delayed MTX excretion. The treatment of delayed MTX excretion is largely dependent on invasive measures such as hemodialysis because the clinical data regarding the efficacy or safety of carboxypetidase G(2) is limited. We report here on the cases of two pediatric osteosarcoma patients with delayed MTX excretion and who were successfully managed using supportive measures. Potential life-threatening complications were prevented by administering high doses of leucovorin.

In this paper, we evaluate the downlink performance of Transparent mode (T-mode) and Non-Transparent mode (NT-mode) in a two-hop cellular system based on IEEE 802.16j. In particular, we evaluate the performance in terms of the system capacity, optimal resource allocation, and outage probability using Monte Carlo simulation with various system parameters such as different Frequency Reuse Factors (FRFs) and the distance between Base Station (BS) and Relay Station (RS). To analyze the Signal to Interference and Noise Ratio (SINR) of the access and relay links, an SINR model is introduced for cellular multihop systems considering intra- and inter-cell interferences. Then, we present a method of optimal resource allocation for the Access Zone (AZ) and Relay Zone (RZ) to maximize the system capacity. Consequently, the simulation results provide an insight into choosing the appropriate RS position and optimal resource allocation. Through numerical examples, it is found that the FRFs of two and three are good choices to achieve the highest capacity with low outage in T- and NT-modes, respectively.

The IEEE802.16j standard uses non-transparent relay stations to extend coverage. There are two types of non-transparent relay modes, that is, the time-division transmit and receive (TTR) relay mode which can operate with one of two types of frame structures, a single-frame and multiframe structure, and the simultaneous transmit and receive (STR) relay mode. In this paper, we analyze the relay performance of TTR and STR relay modes in IEEE 802.16j MMR system. We also propose a fair resource allocation scheme for the downlink relay frame. Numerical results show that relay performance of the TTR with a single-frame or a multiframe structure and that of the STR relay modes are almost the same in a two-hop system. However, in a three-hop system, the TTR mode with a single-frame structure outperforms other relay modes.

The spectral efficiency of cellular networks can be improved when proximate users engage in device-to-device (D2D) communications to communicate directly without going through a base station. However, D2D communications that are not properly designed may generate interference with existing cellular networks. In this paper, we study resource allocation and power control to minimize the probability of an outage and maximize the overall network throughput. We investigate three power control-based schemes: the Partial Co-channel based Overlap Resource Power Control (PC.OVER), Fractional Frequency Reuse based Overlap Resource Power Control (FFR.OVER) and Fractional Frequency Reuse based Adaptive Power Control (FFR.APC) and also compare their performance. In PC.OVER, a certain portion of the total bandwidth is dedicated to the D2D. The FFR.OVER and FFR.APC schemes combine the FFR techniques and the power control mechanism. In FFR, the entire frequency band is partitioned into two parts, including a central and edge sub-bands. Macrocell users (mUEs) transmit using uniform power in the inner and outer regions of the cell, and in all three schemes, the D2D receivers (D2DRs) transmit with low power when more than one D2DRs share a resource block (RB) with the macrocells. For PC.OVER and FFR.OVER, the power of the D2DRs is reduced to its minimum, and for the FFR.APC scheme, the transmission power of the D2DRs is iteratively adjusted to satisfy the signal to interference ratio (SIR) threshold. The three schemes exhibit a significant improvement in the overall system capacity as well as in the probability of a user outage when compared to a conventional scheme.

In this paper, we propose a frequency planning and transmission (Tx) power management scheme, called Fractional Frequency Reuse based Adaptive Power Control (FFR.APC) Scheme for orthogonal frequency division multiple access (OFDMA) and time division duplex (TDD) based on LTE-Advanced device-to-device (D2D) network . In the proposed scheme , the macro base station (mBS) and D2D senders (D2DSs) service macro user equipments (mUEs) and D2D receivers (D2DRs) use different frequency bands and optimal Tx power chosen as D2D links’ locations in inner and outer zones in different periods to reduce interference substantially. Simulations show the proposed scheme outperforms D2D networks with soft frequency reuse (SFR) and Fractional Frequency Reuse (FFR) schemes in terms of the system throughput and outage probability for mUEs and D2DRs.

Motivation: The solvent accessibility of amino acid residues plays an important role in tertiary structure prediction, especially in the absence of significant sequence similarity of a query protein to those with known structures. The prediction of solvent accessibility is less accurate than secondary structure prediction in spite of improvements in recent researches. The k-nearest neighbor method, a simple but powerful classification algorithm, has never been applied to the prediction of solvent accessibility, although it has been used frequently for the classification of biological and medical data. Results: We applied the fuzzy k-nearest neighbor method to the solvent accessibility prediction, using PSI-BLAST profiles as feature vectors, and achieved high prediction accuracies. With leave-one-out cross-validation on the ASTRAL SCOP reference dataset constructed by sequence clustering, our method achieved 64.1% accuracy for a 3-state (buried/intermediate/exposed) prediction (thresholds of 9% for buried/intermediate and 36% for intermediate/exposed) and 86.7, 82.0, 79.0 and 78.5% accuracies for 2-state (buried/exposed) predictions (thresholds of each 0, 5, 16 and 25% for buried/exposed), respectively. Our method also showed slightly better accuracies than other methods by about 2--5% on the RS126 dataset and a benchmarking dataset with 229 proteins. Device-to-Device (D2D) communication is a technology component for long-term evolution-advanced (LTE-A). In D2D communication, users in close proximity to each other can communicate directly without going through a base station; such direct communication can improve spectral efficiency. Although D2D communication brings improvement in spectral efficiency, it also causes interference to the cellular network as a result of spectrum sharing. In particularly, D2D communication can generate interference for each D2D pair when the common wireless medium in a co-located limited area is accessed. Even though the interference management for between the D2D pair and cellular networks has been proposed, the interference reducing methods have still not been fully studied for the D2D pairs. In this paper, we investigate the problem of D2D pair coexistence in which interference is considered between D2D pairs. Using a signal to interference model for a target D2D pair, we provide an analysis of the aggregated throughput of a dense D2D network. For a target D2D pair, we assume that the desired signal and interference signals obey multipath fading and shadow fading. Through analysis, we demonstrate the effect of cluster size such as the number of D2D pairs and the size of the considered area on the network performance. The analytical results are compared with computer simulations. Our work can be used for a rough guideline for controlling the system throughput in a dense D2D network environment.

Congenital pulmonary lymphangiectasia (CPL) is a rare lymphatic pulmonary abnormality. CPL with respiratory distress has a poor prognosis, and is frequently fatal in neonates. We report a case of pneumonectomy for CPL in a newborn. An infant girl, born at 39 weeks' after an uncomplicated pregnancy, exhibited respiratory distress 1 hr after birth, which necessitated intubation and aggressive ventilator care. Right pneumonectomy was performed after her symptoms worsened. Histologic examination indicated CPL. She is currently 12 months old and developing normally. Pneumonectomy can be considered for treating respiratory symptoms for improving chances of survival in cases with unilateral CPL. Graphical Abstract Keywords: Lymphangiectasia, Pulmonary, Congenital; Infant, Newborn; Pneumonectomy INTRODUCTION Congenital pulmonary lymphangiectasia (CPL) is a rare primary developmental defect of the lung characterized by subpleural, interlobular, perivascular, and peribronchial lymphatic dilatation (1). The condition was originally described by Virchow (2) in 1856. Although the exact incidence of CPL is unknown, autopsy studies suggest that approximately 0.5%-1% of infants who are stillborn or die in the neonatal period have CPL (3). Patients with CPL usually present in the first few hours of life with intractable respiratory failure, cyanosis, and hypoxia associated with bilateral chylothorax. In an 18-yr retrospective study, Mettauer et al. (4) reported that 6 of 7 patients with pulmonary lymphangiectasia died. They concluded that although lymphangiectasia was not the direct cause of death in 2 of these patients, the prognosis of this disease was poor (4). Another study reported a mortality rate in cases of neonatal pulmonary lymphangiectasia of nearly 100% (5). However, recent advances in neonatal care have significantly improved the survival in cases of CPL with neonatal-onset (1, 6, 7). In the present report, we describe the case of a newborn with CPL who survived after pneumonectomy of the affected lung.

The purpose of our study was to correlate the morphology of displaced radial head and neck (DRHN) fractures with associated ligament injuries and evaluate the outcomes of management of the torn ulnar collateral ligament (UCL).Twenty-nine surgically treated patients presenting with a DRHN fracture were classified using the Charalambous classification by 3-dimensional computed tomography. Accordingly, the collateral ligament and overlying muscle injuries and bony contusions were investigated with magnetic resonance imaging. The valgus stress test was performed intraoperatively on 20 patients with magnetic resonance-confirmed complete UCL rupture. If there was no firm end point, the UCL was repaired (group B). The others were treated conservatively (group A). The Minnesota Elbow Performance Score and Disabilities of the Arm, Shoulder and Hand scores were used to measure patient outcome.There was a trend toward a higher incidence of complete UCL rupture in Charalambous type 3D and 4D fractures (P = .110) and complete lateral UCL rupture in Charalambous type 1D and 2D fractures (P = .126), although this was not statistically significant. Rupture of the overlying muscles was more common in group B than in group A (P.05). Functional outcome between the 2 groups was not significantly different.DRHN fracture is always a complex fracture accompanied by collateral ligament injuries. Type 3D and 4D tended to have a higher association with UCL rupture compared with type 1D and 2D, types commonly associated with lateral UCL rupture. The magnetic resonance imaging-confirmed torn UCL could be managed conservatively if there was a firm end point on valgus stress test.

In this paper, we propose a hybrid division cooperative transmission (HDCT) scheme for system throughput enhancement in enterprise femtocell systems. In HDCT scheme, a cooperative transmission (CT) scheme is applied to user equipments (UEs) located at the edge of a serving femtocell base station (fBS), whereas conventional scheme is applied to UEs located near the fBS. In the zone where CT is applied, a UE is capable of receiving desired signal from serving fBS as well as from an adjacent fBS. Thus, the UE achieves an improved signal to interference plus noise ratio (SINR) by using two time synchronized signals from two adjacent fBSs. Performance of the proposed scheme is evaluated in terms of call level quality of service (QoS) (i.e., call blocking probability) and packet level QoS (i.e., outage probability). We first measure the resource utilization and call blocking probability for the downlink channel for various offered load. Based on that, we obtain the outage probability and effective throughput of the system by computer simulations. Finally, the results of the simulation yield the optimum hybrid zone for achieving the maximum system throughput.

Simulation results show that compared to the conventional scheme, the proposed scheme can reduce the UE's outage probability in the enterprise femtocell environment. In particular, a significant performance improvement can be achieved with the proposed CTS for UEs located at the cell edge.
