As recomendacoes sao baseadas na melhor evidencia cientifica disponivel no momento e no consenso de especialistas com grande experiencia na area. Resumo A pandemia da doença causada pelo novo coronavírus (COVID-19) trouxe grandes desafios para o sistema de saúde devido ao aumento exponencial do número de pacientes acometidos. A racionalização de recursos e a indicação correta e criteriosa de exames de imagem e procedimentos intervencionistas tornaram-se necessárias, priorizando a segurança do paciente, do ambiente e dos profissionais da saúde. Esta revisão visa auxiliar e orientar os profissionais envolvidos na realização desses exames e procedimentos a fazê-los de forma eficaz e segura.

In due time, the Brazilian Society of Cardiology decided to create a guideline on telemedicine applied to cardiology, also known as telecardiology. According to the Pan American Health Organization (PAHO) and the World Health Organization (WHO), telemedicine is “The delivery of health care services, where distance is a critical factor, by all health care professionals using information and communication technologies for the exchange of valid information for diagnosis, treatment, and prevention of disease and injuries, research and evaluation, and for the continuing education of health care providers, all in the interests of advancing the health of individuals and their communities.” Such a seemingly simple and altruistic definition carries a wide range of potential implications at various levels, from an ethical point of view to a potential impact on clinical practice and outcomes. Hence, the importance of guidelines, organized by the medical community through scientific societies, in offering to all of those involved in the process a reference based, as much as possible, on expert opinion, current scientific evidence, and on respect for medical ethical and deontological values. Oportunamente, a Sociedade Brasileira de Cardiologia decidiu criar uma diretriz sobre a telemedicina aplicada à cardiologia, também conhecida como telecardiologia. Segundo a Organização Pan-Americana da Saúde (OPAS) e a Organização Mundial da Saúde (OMS), a telemedicina é “A prestação de serviços de saúde, onde a distância é um fator crítico, por todos os profissionais de saúde que utilizam tecnologias de informação e comunicação para a troca de informações válidas para diagnóstico, tratamento e prevenção de doenças e lesões, pesquisa e avaliação, e para a educação continuada dos profissionais de saúde, tudo no interesse do avanço da saúde dos indivíduos e de suas comunidades”. Tal definição aparentemente simples e altruísta carrega uma ampla gama de implicações potenciais em vários níveis, desde um ponto de vista ético até um potencial impacto na prática clínica e nos resultados. Daí a importância das diretrizes, organizadas pela comunidade médica por meio das sociedades científicas, em oferecer a todos os envolvidos no processo uma referência baseada, tanto quanto possível, na opinião de especialistas, nas evidências científicas atuais e no respeito à ética médica e valores deontológicos.

In face of the pandemic of the novel coronavirus disease 2019 (COVID-19), the management of patients with cardiovascular risk factors and/or disease is challenging. The cardiovascular complications evidenced in patients with COVID-19 derive from several mechanisms, ranging from direct viral injury to complications secondary to the inflammatory and thrombotic responses to the infection. The proper care of patients with COVID-19 requires special attention to the cardiovascular system aimed at better outcomes.Frente à pandemia da doença causada pelo novo coronavírus (COVID-19), o manejo do paciente com fator de risco e/ou doença cardiovascular é desafiador nos dias de hoje. As complicações cardiovasculares evidenciadas nos pacientes com COVID-19 resultam de vários mecanismos, que vão desde lesão direta pelo vírus até complicações secundárias à resposta inflamatória e trombótica desencadeada pela infecção. O cuidado adequado do paciente com COVID-19 exige atenção ao sistema cardiovascular em busca de melhores desfechos.

Background Right ventricular function is a crucial factor of the prognosis of systemic lupus erythematosus (SLE). Objectives To evaluate the right ventricular function in SLE patients with different degrees of pulmonary hypertension (PH) by strain and strain rate imaging. Methods A total of 102 SLE patients and 30 healthy volunteers were studied between October 2015 and May 2016. Patients were divided into three groups according to pulmonary artery systolic pressure (PASP) estimated by echocardiography: group control (A); PASP ≤ 30 mmHg (group B, n = 37); PASP 30-50 mmHg (mild PH; group C, n = 34); and PASP ≥ 50 mmHg (moderate-to-severe PH; group D, n = 31). Longitudinal peak systolic strain (ε) and strain rate (SR), including systolic strain rate (SRs), early diastolic strain rate (SRe) and late diastolic strain rate (SRa) were measured in the basal, middle and apical segments of the right ventricular free wall in participants by two-dimensional speckle tracking echocardiography (2D-STE) from the apical four-chamber view. A p < 0.05 was set for statistical significance. Results The parameters of ε, SRs, SRe, and SRa were significantly decreased in groups C and D compared with groups A and B. The ε of each segments was significantly lower in group D than in group C, while there were no differences in SRs, SRe and SRa between groups C and D. Conclusions Strain and strain rate imaging could early detect the right ventricular dysfunction in SLE patients with PH, and provide important value for clinical therapy and prognosis of these patients.

BACKGROUND: In individuals with concurrent chronic kidney disease (CKD) and cardiovascular disease (CVD), the association between left atrial volume (LAV) and serum levels of C-reactive protein (CRP) is shown. OBJECTIVE: Verify the presence of associations between systemic inflammation and LA dilation in patients on hemodialysis (HD) without clinically evident CVD. METHODS: This was an observational cross-sectional study of a population on HD (> 3 months), which excluded patients with acute or chronic inflammatory diseases (infections, malignancies, autoimmune diseases) hemodynamic instability, use of anti-inflammatory drugs, hyperparathyroidism, arrhythmias, mitral valve disease and prior cardiovascular (CV) events. CRP and interleukin-6 (IL-6) measurements as well as Doppler echocardiography were obtained. Correlation coefficients were determined to evaluate the associations between variables. RESULTS: A total of 58 patients were included (28 men, aged 55 ± 15 years), on HD for 24 ± 16 months, 45% were hypertensive, 26% diabetic, with median CRP of 5.1 mg/dL and IL-6 of 6.1 pg/dL. CRP significantly correlated with LAV (p = 0.040), LAV index (LAVi, p = 0.02) and mitral inflow E wave (p = 0.014). IL-6, despite the strong association with CRP levels (r = 0.75, p < 0.001), did not correlate with echocardiographic indices. Individuals in the top quartile of CRP had significantly higher LAVi than the others (42 ± 17 versus 32 ± 11 mL/m2, p = 0.015). CONCLUSIONS: In subjects on HD with no prior CV event, there was an association between elevated CRP levels and LA enlargement. The findings suggest an association between physiopathological processes related to left atrial dilation and systemic inflammatory state of patients on HD.

Background Myocardial infarction is a public health problem. Functional food is an alternative treatment for cardiovascular diseases. Objective The objective was to analyze the functional and anatomopathological post-myocardial-infarction effects of soybean extract (SE) and isoflavone (IF). Methods Myocardial infarction was induced in adult Wistar rats. After 5 days, an echocardiogram was performed to determine heart rate (HR), ejection fraction (EF), systolic volume (LVESV) and diastolic volume (LVEDV). Animals with ventricular dysfunction (EF n =14), SE ( n =15) and IF ( n =12). The IF group received 120 mg/kg/day isolated IF, and the SE group received 12.52 g/day. After 30 days, a new echocardiogram was performed. A histological exam was carried out to determine the collagen. Activity of biochemical markers [arginase, lactate dehydrogenase (LDH) and malate dehydrogenase] was measured. Results The animals of the control, IF and SE groups showed a reduction in EF after the infarction ( P =.432, P =.017 and P =.320, respectively). An increase of LVESV and LVEDV was observed in all groups ( P =.009, P =.001 and P =.140; and P =.003, P =.008 and P =.205, respectively). A reduction of HR was found in the SE group ( P =.020). There was a greater activity of LDH in the SE group. A smaller quantity of mature collagen was found in the region proximal to the myocardial infarction in the SE group. Conclusion A protective effect in the SE group was observed 30 days after the myocardial infarction.

Background: Sudden cardiac death (SCD) is the leading cause of death in maintenance hemodialysis (HD) patients, but there is little information about underlying risk factors. Objectives: Evaluate the association between clinical and echocardiographic variables with SCD on HD patients. Methods: Retrospective nested case-control study on chronic HD patients who were prospectively followed. The primary endpoint was SCD. Variables were compared by Student t test, Mann-Whitney or Chi-Square, and independent predictors of SCD were evidenced by multivariate logistic regression. Results: We followed 153 patients (50 ± 15 years, 58% men) for 23 ± 14 months and observed 35 deaths, 17 of which were SCD events. When compared to the control group (matched for gender, age, and body mass index) there were no differences regarding time on dialysis, traditional biochemical parameters, blood pressure, smoking, use of cardiovascular protective drugs, ejection fraction, left ventricular dimensions, and diastolic function indices. On the other hand, in the SCD group, we found a higher prevalence of previous heart failure, acute myocardial infarction and diabetes, greater left ventricular mass index, greater left atrial size and lower global myocardial performance. After multivariate logistic regression analysis, diabetes (OR = 2.6; CI = 1.3-7.5; p = 0.023) and left ventricular mass index ≥ 101 g/m2.7 (OR = 1.04; CI = 1.01-1.08; p = 0.028) showed independent association with SCD events. Conclusions: HD patients with diabetes mellitus and left ventricular hypertrophy appear to have the highest risk of SCD. Preventive and therapeutic strategies should be encouraged in addressing these risk factors to minimize the occurrence of SCD in HD patients.

Vitamin D (25-hydroxyvitamin D, 25(OH)D) deficiency, hypovitaminosis D, is highly prevalent in chronic kidney disease patients and is potentially involved with complications in the hemodialysis (HD) population. The aim of this study was to evaluate the impact of cholecalciferol supplementation on biomarkers of mineral metabolism, inflammation, and cardiac function in a group of HD patients presenting with hypovitaminosis D and low intact parathyroid hormone (iPTH) levels.HD patients with iPTH levels of300 pg/mL, not receiving vitamin D therapy, and presenting with 25(OH)D levels of30 ng/mL were enrolled in this prospective study. Oral cholecalciferol was prescribed once a week in the first 12 weeks (50,000 IU) and in the last 12 weeks (20,000 IU) of the study. High-sensitivity C-reactive protein, interleukin-6, and serum albumin were used as inflammatory markers. Echocardiograms were performed on a midweek interdialytic day at baseline and after 6 months of cholecalciferol supplementation.In all, 30 patients were included in the final analysis. We observed a significant increase in serum 25(OH)D levels after 3 months (46.2 ± 14.4 ng/mL vs. 18.1 ± 6.6 ng/mL; P.001) and after 6 months (40.4 ± 10.4 ng/mL vs. 18.1 ± 6.6 ng/mL; P.001) of cholecalciferol supplementation. There were no significant changes in alkaline phosphatase, iPTH, phosphorus, and serum albumin levels, but there was a slight but significant increase in calcium levels after 6 months of cholecalciferol supplementation (9.4 ± 0.6 mg/dL vs. 9.0 ± 0.6 mg/dL; P = .02). Additionally, we observed a significant reduction in high-sensitivity C-reactive protein levels after 3 months (median: 0.62 [0.05 to 29.6] mg/L vs. 0.32 [0.02 to 3.13] mg/L; P = .02) and after 6 months (median: 0.62 [0.05 to 29.6] mg/L vs. 0.50 [0.02 to 5.66] mg/L; P = .04) of cholecalciferol supplementation, as well as a significant reduction in interleukin-6 levels (median: 6.44 pg/mL vs. 3.83 pg/mL; P = .018) after 6 months of supplementation. Left ventricular mass index was significantly reduced at the end of supplementation (159 ± 55 g/m(2) vs. 175 ± 63 g/m(2); P = .03).Cholecalciferol supplementation in HD patients was found to be safe and efficient to correct hypovitaminosis D and established little impact on mineral metabolism markers. Additionally, we observed a reduction in important surrogate markers of cardiovascular risk, namely systemic inflammation and left ventricular hypertrophy, suggesting an anti-inflammatory action and possibly an improvement of cardiac dysfunction.

Heart failure (HF) is very common in the general population, and risk factors for HF, such as coronary artery disease, diabetes, obesity, and hypertension, are frequently present in patients with CKD. Therefore, HF is also an important cause of morbidity and mortality in this population. Diastolic heart failure (DHF), also called HF with preserved ejection fraction, refers to a clinical syndrome in which patients have symptoms and signs of HF, normal or near normal left ventricular (LV) systolic function, and evidence of diastolic dysfunction (e.g., abnormal LV filling and elevated filling pressure). Recent data suggest that HF with normal ejection fraction is even more common in patients than HF with low ejection fraction, including those on hemodialysis. Not surprisingly, DHF is a strong predictor of death in CKD patients. In this article, we review the information available on the mechanisms, clinical presentation, impact, and potential interventions in DHF based on evidence from CKD patients, as well as evidence from the general population potentially applicable to the CKD population.

Most of the recent advances in the understanding of chronic kidney disease (CKD)-related cardiovascular disease have focused on atherosclerosis and arteriosclerosis, and much less effort has been dedicated to unveil and evaluate the mechanisms and impact of interventions related to myocardial dysfunction. Hence, echocardiographic evaluation plays a pivotal role in establishing the diagnosis of myocardiopathy as well as in stratifying risk and defining the impact of interventions. The aim of this review is to examine the profile of myocardiopathy in CKD, and to identify how the echocardiogram can be useful in diagnostic and prognostic clinical approaches.

Chronic kidney disease (CKD) and ESRD, treated with conventional hemo- or peritoneal dialysis are both associated with a high prevalence of an increase in left ventricular mass (left ventricular hypertrophy [LVH]), intermyocardial cell fibrosis, and capillary loss. Cardiac magnetic resonance imaging is the best way to detect and quantify these abnormalities, but M-Mode and 2-D echocardiography can also be used if one recognizes their pitfalls. The mechanisms underlying these abnormalities in CKD and ESRD are diverse but involve afterload (arterial pressure and compliance), preload (intravascular volume and anemia), and a wide variety of afterload/preload independent factors. The hemodynamic, metabolic, cellular, and molecular mediators of myocardial hypertrophy, fibrosis, apoptosis, and capillary degeneration are increasingly well understood. These abnormalities predispose to sudden cardiac death, most likely by promotion of electrical instability and re-entry arrhythmias and congestive heart failure. Current treatment modalities for CKD and ESRD, including thrice weekly conventional hemodialysis and peritoneal dialysis and metabolic and anemia management regimens, do not adequately prevent or correct these abnormalities. A new paradigm of therapy for CKD and ESRD that places prevention and reversal of LVH and cardiac fibrosis as a high priority is needed. This will require novel approaches to management and controlled interventional trials to provide evidence to fuel the transition from old to new treatment strategies. In the meantime, key management principles designed to ameliorate LVH and its complications should become a routine part of the care of the patients with CKD and ESRD.

Background: Kidney transplant (Tx) patients present a reduced cardiovascular (CV) mortality in comparison to the dialysis population, but in comparison to the general population, it is still several-fold higher. Methods: We studied risk factors for CV disease in a group of 38 patients (50% males, median age 36 years) who underwent a living donor Tx at the baseline and after 3 ± 1 and 9 ± 2 months. Results: The prevalence of overweight increased from 26 to 54% after Tx (p < 0.001). The mean systolic blood pressure decreased significantly after the Tx (148 ± 27.6 vs. 126 ± 12.7 mm Hg). There was a significant increase in LDL (97 ± 30 vs. 114 ± 35) and hematocrit (33.8 ± 4.4 to 42 ± 5.7%) levels and a significant reduction in fibrinogen levels (394 ± 91 vs. 366 ± 100 mg/dl) after 9 months as compared to the baseline. Obesity and dislipidemia were significantly correlated with inflammation. Significant changes in left ventricle mass index (293 ± 116 vs. 241 ± 96) were observed after the Tx. Patients with a low glomerular filtration rate (GFR) in the follow-up evaluation presented higher LDL (128 ± 7 vs. 99 ± 7 mg/dl; p < 0.05) and higher fibrinogen levels (399 ± 21 vs. 332 ± 22 mg/dl; p < 0.05) compared to patients with a high GFR. Conclusion: Most of the risk factors analyzed (particularly the uremia-related) improved after the renal Tx, which could justify the positive impact of Tx on the development of CV disease. Inflammation and dyslipidemia were related to renal dysfunction after the Tx, suggesting that complete restoration of renal function may have an impact on reducing CV mortality in CKD patients treated with renal Tx.

OBJETIVO: Avaliar os efeitos da redução da pré-carga induzida por uma sessão de hemodiálise sobre o índice de desempenho miocárdico (Tei) e outros parâmetros ecocardiográficos de função cardíaca. MÉTODOS: Estimamos o índice de Tei e parâmetros de função sistólica e diastólica do ventrículo esquerdo (VE), antes e depois de uma sessão de hemodiálise. Incluímos no estudo indivíduos em ritmo sinusal e sem antecedentes de insuficiência coronariana ou evidências de valvopatia e derrame pericárdico. RESULTADOS: 15 pacientes (8 homens, idade 53 ± 14 anos) completaram o estudo. Após ultrafiltração de 2,2 ± 1,1 litros, a onda E diminuiu (p < 0,05) e a onda A permaneceu inalterada (p = ns), resultando em decréscimo de E/A (p < 0,01). O índice de Tei aumentou (0,57 ± 0,07 para 0,65 ± 0,09, p < 0,01) através do prolongamento do TRIV (101 ± 14 para 113 ± 17 ms, p < 0,01) e encurtamento do TEJ (271 ± 22 para 252 ± 22 ms, p < 0,05). O TCIV não variou (p = ns). Os parâmetros diastólicos ao Doppler tecidual não mudaram (p = ns), enquanto a velocidade sistólica (S) aumentou (p < 0,05). CONCLUSÃO: O índice de Tei foi afetado pela alteração da pré-carga induzida pela hemodiálise, assim como outros parâmetros derivados do Doppler transvalvar mitral. Os parâmetros diastólicos do Doppler tecidual do anel mitral foram independentes da pré-carga, enquanto a velocidade sistólica sugeriu melhora na função sistólica do VE após o procedimento. OBJECTIVE: To assess the influence of preload reduction by hemodialysis on Doppler Tei Index of myocardial performance and other parameters of cardiac function. METHODS: The Tei index and left ventricular (LV) systolic and diastolic function parameters were estimated, before and after a single hemodialysis session. Only subjects who were in sinus rhythm, without history of coronary artery disease, and no evidence of cardiac valve disease and pericardial effusion were included in the study. RESULTS: Fifteen patients (8 men, mean age 53 ± 14 years) completed the study. After an ultrafiltration of 2,2 ± 1,1 liters, peak mitral E velocity decreased (p< 0,05) and A velocity remained unchanged (p = ns), resulting in reduction of E/A ratio (p< 0,01). The Tei index increased (from 0,57 ± 0,07 to 0,65 ± 0,09, p< 0,01) because of significant prolongations in isovolumetric relaxation time (from 101 ± 14 to 113 ± 17 ms, p< 0,01) and ejection time (from 271 ± 22 to 252 ± 22, p< 0,05). The isovolumetric contraction time did not vary (p = ns). There was no change in diastolic tissue Doppler parameters, while systolic velocities increased (p< 0,05). CONCLUSION: The Tei index was affected by hemodialysis-induced preload alterations, as well as other mitral inflow Doppler-derived parameters. The diastolic parameters of mitral annulus Doppler tissue were independent of preload, while systolic velocities suggested improved systolic function.

During the past few years, it has become increasingly evident that residual renal function (RRF) is an important and independent predictor of poor outcome in patients with chronic kidney disease (CKD). Although the causes of this observation are not fully understood, it appears that the loss of RRF impairs both fluid removal and clearance of solutes, which in turn leads to uremic toxicity and increased morbidity and mortality. There is increasing evidence that patients with CKD develop signs of fluid overload already in the early phases of the disease, and this may be a stimulus for inflammatory activation. Recently, an inflammatory component was identified in uremic atherosclerotic and non-atherosclerotic cardiovascular disease (CVD), which have been consistently associated with poor clinical outcome in patients with CKD. Signs of systemic inflammation occur in parallel to the impairment in renal function, and the pathophysiology is most likely multifactorial, including a decrease in cytokine clearance, advanced glycation end-product accumulation, oxidative stress, and principal fluid overload. Additionally, inflammation seems to be a predictor of accelerated loss of renal function. In this article, we discuss the evidence showing that patients with CKD generally have fluid overload, the mechanisms by which impaired renal function may lead to a chronic inflammatory state, and the available information linking fluid overload to accelerated loss of renal function and CVD through inflammation. Inflammation may lead to the development of complications of CKD, in particular CVD, but on the other hand may also lead to a faster progression of renal disease. Strategies aiming to reduce fluid overload may be useful to reduce cardiovascular morbidity and mortality, but also preserve RRF.

In this study, we report a case of a healthy, 25‑year‑old male patient with presyncope during a football match divergent blood pressure, and a grade 3/6 aortic diastolic murmur. The echocardiogram showed a rounded structure in the basal area of the interventricular septum adjacent to the aortic valve, which causes inadequate commissural support, communicating with the left ventricle (LV). There was moderate secondary aortic insufficiency and dilated LV with preserved contractility. MRI revealed myocardial density in the structure without delayed enhancement. After follow-up, the LV increased further in size and syncope on exertion. The patient underwent surgery and the anatomic pathologist diagnosed left ventricular diverticulum. The diverticulum, a congenital protrusion in the myocardial wall of the LV, differs from an aneurysm by its close connection to the chamber and presence of myocardial fibers in the composition of the wall1. It most often affects the LV or right ventricular apex and is rare in the septum. Serial foetal echocardiography showed the development of severe left ventricular systolic dysfunction and thrombosis in a previously healthy foetus. Normal cardiac findings in a mid-trimester foetus do not exclude subsequent dilated cardiomyopathy.

Os aneurismas de átrio direito são entidades raramente relatadas na prática cardiológica, especialmente em vida intra-uterina, e podendo ser confundidos com derrame pericárdico e anomalia de Ebstein da valva tricúspide. Apresentamos revisão da literatura e ilustramos com caso de diagnóstico pré-natal de aneurisma de átrio direito cursando com sinais de hidropisia. Right atrium aneurysms are entities which are rarely reported in cardiologic practice, especially in intrauterine life, and may be mistaken with pericardial effusion and Ebstein's anomaly. We show a review of the literature and illustrate with a case of prenatal diagnosis of right atrium aneurysm running through with hydropsy signs.

Hypertension is highly prevalent in patients with chronic kidney disease and hypervolemia is one of the principal causes.To evaluate the influence of the reduction of volemia on blood pressure as well as on echocardiographic parameters in patients on continuous ambulatory peritoneal dialysis.Twelve patients with no clinical evidence of hypervolemia were submitted to an increase in the rate of the dialysis with the purpose of reducing body weight by 5%. The volemia was evaluated by electrical bioimpedance and by ultrasound of the inferior cava vena (ICV). Blood pressure was measured by ambulatory blood pressure monitoring and cardiac function was evaluated by echocardiography both at baseline and 5 weeks after the intervention period.After the increase in the ultrafiltration, body weight, extracellular water and the inspiratory diameter of the ICV decreased significantly in parallel with a non-significant increase in the collapsing ICV index. Despite the reduction of anti-hypertensive drugs, systolic blood pressure during the sleep period decreased from 138.4 ± 18.6 to 126.7 ± 18.0 mmHg, the nocturnal blood pressure drop increased and the final systolic left ventricular diameter decreased significantly.Reduction of the volemia of patients on peritoneal dialysis, with no signs of hypervolemia, was associated with a better blood pressure control and with a decrease of the final systolic left ventricular diameter.

FUNDAMENTO: A dilatacao do ventriculo esquerdo (VE) apos infarto agudo do miocardio (IAM) e um importante determinante do prognostico. A razao entre a velocidade diastolica E do fluxo mitral e a velocidade diastolica e' do anel mitral (relacao E/e') e o melhor indice nao invasivo para detectar elevacao aguda da pressao de enchimento do VE. A hipotese deste estudo e que a E/e' possa predizer remodelacao do VE apos IAM tratado. OBJETIVO: Avaliar se a E/e' prediz remodelacao ventricular apos IAM, em comparacao aos dados clinicos, laboratoriais e ecocardiograficos tradicionais. METODO: Ecocardiogramas foram realizados em pacientes consecutivos com primeiro IAM, apos angioplastia transluminal coronariana (ATC) seguida de recanalizacao efetiva, 48 horas e 60 dias apos o evento. A E/e' foi calculada pela media de quatro sitios do anel mitral. Remodelacao do VE foi definida como aumento ≥ 15% do volume sistolico final ao metodo de Simpson. Analises estatisticas incluiram teste t de Student, curvas receptor-operador (ROC) e regressao logistica multivariada, com p significante < 0,05. RESULTADOS: Estudados 55 pacientes, com idade 58 ± 11 anos, 43 homens, observou-se E/e' maior (13 ± 4 versus 8,5 ± 2; p < 0,001) no grupo com remodelacao (n = 13) em relacao ao grupo sem remodelacao (n = 42). A curva ROC indicou E/e' como preditor de remodelacao (area sob a curva = 0,81, p = 0,001). Analises de regressao contendo variaveis clinicas, laboratoriais e Doppler-ecocardiograficas confirmaram E/e' como preditor independente da remodelacao (odds ratio 1,42; p = 0,01). CONCLUSAO: A relacao E/e' e um preditor util de remodelacao do VE apos IAM, indicando pacientes com maior risco cardiovascular.

Left atrial (LA) volume has been proposed as a less preload-dependent parameter of diastolic function than Doppler mitral inflow. We hypothesize that in the absence of mitral regurgitation and atrial fibrilation, LA enlargement could be a more practical (and relatively preload-independent) method for the evaluation of left ventricular diastolic function. The aim of the present study was to determine the effects of preload reduction by hemodialysis on LA volume.

FUNDAMENTO: As consequências e os riscos do exercício físico contínuo por períodos prolongados não estão esclarecidos. OBJETIVO: Avaliar os efeitos do exercício prolongado em participantes de uma ultramaratona de 24 horas. MÉTODOS: Vinte corredores foram selecionados para avaliação, um dia antes e imediatamente após a prova em que os corredores devem percorrer a maior distância em 24 horas. Foram obtidos dados clínicos, laboratoriais e ecocardiográficos. RESULTADOS: A distância média percorrida foi de 140,3 ± 18,7 km. Os corredores apresentaram redução do peso corpóreo (p < 0,001) e da pressão arterial sistólica (p < 0,001) e diastólica (p = 0,004). As alterações hematológicas foram compatíveis com o estresse fisiológico. A concentração plasmática de creatinofosfoquinase (CPK) aumentou significativamente (163,4 ± 56,8 versus 2978,4 ± 1921,9 U/L; p < 0,001) e esteve inversamente correlacionada com a distância percorrida: os que correram maiores distâncias apresentaram níveis mais baixos de CPK (Pearson r = 0,69; p = 0,02). Após a corrida, dois corredores apresentaram discreta elevação de troponina T. Em um deles, houve queda concomitante na fração de ejeção (coronariopatia foi excluída subsequentemente). O ecocardiograma na avaliação basal mostrou hipertrofia de ventrículo esquerdo em um e aumento do volume atrial esquerdo em cinco corredores. Após a prova, houve redução na relação E/A (p < 0,01). CONCLUSÃO: O exercício físico prolongado está associado a alterações cardiovasculares e metabólicas. As alterações cardiológicas encontradas sugerem que o fenômeno de fadiga cardíaca pode ocorrer nessa modalidade de corrida. O efeito a longo prazo dessas alterações, com a manutenção da prática desse tipo de atividade, ainda é desconhecido. BACKGROUND: The consequences and risks of prolonged physical exercise are not well established. OBJECTIVE: To evaluate the effects of prolonged physical exercise on the participants of a 24-hour ultramarathon race. METHODS: Twenty male runners were selected for evaluation a day before and immediately after the race, where the athletes had to cover the most distance in 24 hours. Clinical, laboratory and echocardiographic data were obtained at both evaluations. RESULTS: Mean distance covered was 140.3 ± 18.7 km. Runners showed weight loss (p < 0.001) and decrease in systolic (p < 0.001) and diastolic (p = 0.004) blood pressure. Hematological changes were compatible with the physiological stress. Plasma levels of creatine phosphokinase strikingly increased post-race (163.4 ± 56.8 vs. 2978.4 ± 1921.9 U/L; p < 0.001) and was inversely correlated with distance covered: those who covered the longest distances showed the lowest CPK levels (Pearson r = 0.69, p = 0.02). After the race, 2 runners showed a slight increase in Troponin levels. One of them also had simultaneous decrease in left ventricular ejection fraction (coronary artery disease was subsequently ruled out). Basal echocardiography assessment had shown LV hypertrophy in one and increased left atrial volume in five runners. After the race, there was a decrease in E/A ratio (p < 0.01). CONCLUSION: Prolonged physical exercise is associated with metabolic and cardiovascular alterations. Cardiac abnormalities found in our study suggest that cardiac fatigue may occur in this specific race modality. The long-term effect of these alterations, while maintaining the routine practice of prolonged strenuous physical activity, is still unknown.

Atrial aneurysms involving the free wall or atrial appendage are rare entities in cardiology practice and may be associated with atrial arrhythmias or embolic phenomena. We review the literature and report a case of aneurysm of the right atrial appendage in a young adult, whose diagnosis was established with echocardiography after an episode of paroxysmal atrial flutter.

Background: Vitamin D [25(OH)D] deficiency is a cardiovascular risk factor in the hemodialysis (HD) population. The aim of this study was to identify hypovitaminosis D in HD patients without signs of hyperparathyroidism and to analyze its association to inflammation and echocardiographic alterations. Methods: Patients on HD with iPTH Results: Sixty-one patients (mean age of 56 ± 15 years, 52% males, 93% Caucasians, 31% diabetic) were included, and 75% presented hypovitaminosis D. Inflammation was more prevalent among those with hypovitaminosis D, and these patients presented higher relative wall thickness (0.48 ± 0.11 vs. 0.42 ± 0.10 mm; p = 0.05) and lower left ventricular diastolic (49.8 ± 6.2 vs. 54.7 ± 5.8 mm; p = 0.013) and systolic (31.9 ± 5.7 vs. 36.8 ± 7.2 mm; p = 0.012) diameters. Conclusions: Hypovitaminosis D is associated with inflammation and concentric geometric pattern of the left ventricle, even in the absence of high iPTH levels. Vitamin D repletion (aiming to reduce cardiovascular complications) should also be considered in HD patients with normal or low iPTH levels.

FUNDAMENTO: Disfunção diastólica é frequente em pacientes de hemodiálise, mas seu impacto na evolução clínica é incerto. OBJETIVO: Avaliar a prevalência e o impacto prognóstico da disfunção diastólica (DD) avançada (DDA) do ventrículo esquerdo (VE) em pacientes de hemodiálise. MÉTODOS: Ecocardiogramas foram realizados em pacientes no primeiro ano de hemodiálise, em ritmo sinusal, sem doença cardiovascular manifestada, excluindo-se aqueles com valvopatia significativa ou derrame pericárdico. Pela avaliação integrada dos dados ecodopplercardiográficos, a função diastólica foi classificada como: 1) normal, 2) DD discreta (alteração do relaxamento) e 3) DDA (pseudonormalização e fluxo restritivo). Os desfechos pesquisados foram mortalidade geral e eventos cardiovasculares. RESULTADOS: Foram incluídos 129 pacientes (78 homens), com idade 52 ± 16 anos e prevalência de DD de 73% (50% com DD discreta e 23% com DDA). No grupo com DDA, demonstrou-se maior idade (p < 0,01), pressão arterial sistólica (p < 0,01) e diastólica (p = 0,043), massa do VE (p < 0,01), índice do volume do átrio esquerdo (p < 0,01) e proporção de diabéticos (p = 0,019), além de menor fração de ejeção (p < 0,01). Após 17 ± 7 meses, a mortalidade geral foi significativamente maior naqueles com DDA, em comparação aos normais e com DD discreta (p = 0,012, log rank test). Na análise multivariada de Cox, a DDA foi preditiva de eventos cardiovasculares (hazard ratio 2,2, intervalo de confiança 1,1-4,3, p = 0,021) após ajuste para idade, gênero, diabete, massa do VE e fração de ejeção. CONCLUSÃO: A DDA subclínica foi encontrada em aproximadamente um quarto dos pacientes de hemodiálise e acarretou impacto prognóstico, independente de outros dados clínicos e ecocardiográficos. OBJECTIVE: To evaluate the prevalence and prognostic impact of left ventricular (LV) advanced diastolic dysfunction (ADD) in patients on hemodialysis. METHODS: The echocardiograms were performed during the first year of HD therapy, in patients with sinus rhythm, with no evidence of cardiovascular disease, excluding those with significant valvopathy or pericardial effusion. The combined assessment of the Doppler echocardiographic data classified the diastolic dysfunction as: 1) normal diastolic function; 2) mild DD (relaxation alteration) and 3) ADD (pseudonormalization and restrictive flow pattern). The assessed outcomes were general mortality and cardiovascular events. RESULTS: A total of 129 patients (78 males), aged 52 ± 16 years, with a DD prevalence of 73% (50% with mild DD and 23% with ADD) were included in the study. The group with ADD was older (p < 0.01) and presented higher systolic (p < 0.01) and diastolic BP (p = 0.043), LV mass (p < 0.01), left atrial volume index (p < 0.01) and number of diabetic patients (p = 0.019), as well as lower ejection fraction (EF) (p < 0.01). After 17 ± 7 months, the general mortality was significantly higher in individuals with ADD, when compared to those with normal function and mild DD (p = 0.012, log rank test). At Cox multivariate analysis, ADD was predictive of cardiovascular events (hazard ratio 2.2; confidence interval: 1.1-4.3; p = 0.021) after adjusted for age, gender, diabetes, LV mass and EF. CONCLUSION: The subclinical ADD was identified in approximately 25% of the patients undergoing hemodialysis and had a prognostic impact, regardless of other clinical and echocardiographic data. OBJETIVO: Evaluar la prevalencia y el impacto pronóstico de la disfunción diastólica (DD) avanzada (DDA) del ventrículo izquierdo (VI) en pacientes de hemodiálisis. MÉTODOS: Se realizaron ecocardiogramas en pacientes en el primer año de hemodiálisis, en ritmo sinusal, sin enfermedad cardiovascular manifestada, excluyéndose aquellos con valvulopatía significativa o derrame pericárdico. Por la evaluación integrada de los datos del ecocardiograma Doppler, la función diastólica fue clasificada como: 1) normal, 2) DD discreta (alteración de la relajación) y 3) DDA (seudonormalización y flujo restrictivo). Los desenlaces investigados fueron mortalidad general y eventos cardiovasculares. RESULTADOS: Fueron incluidos 129 pacientes (78 hombres), con edad 52 ± 16 años y prevalencia de DD del 73% (50% con DD discreta y 23% con DDA). En el grupo con DDA, se presentó mayor edad (p < 0,01), presión arterial sistólica (p < 0,01) y diastólica (p = 0,043), masa del VI (p < 0,01), índice del volumen del atrio izquierdo (p < 0,01) y proporción de diabéticos (p = 0,019), además de menor fracción de eyección (p < 0,01). Después de 17 ± 7 meses, la mortalidad general fue significativamente mayor en aquellos con DDa, en comparación a los normales y con DD discreta (p = 0,012, log rank test). Alterações de estrutura e função cardíacas detectadas pela ecocardiografia são comuns em pacientes com doença renal crônica em hemodiálise e predizem um pior prognóstico. Esta revisão aborda recentes evidências da utilidade do método na detecção da disfunção cardíaca clínica e subclínica, estratificação do risco cardiovascular e avaliação das estratégias de intervenção terapêutica. Changes in cardiac structure and function detected by echocardiography are common in patients with chronic kidney disease undergoing hemodialysis, and have been recognized as key outcome predictors. This review attempts to summarize recent evidence pointing to the usefulness of the method in the detection of clinical and subclinical cardiac dysfunction, stratification of cardiovascular risk and assessment of intervention strategies. Alteraciones de estructura y función cardíacas detectadas por ecocardiografía son comunes en pacientes con enfermedad renal crónica en hemodiálisis y predicen un peor pronóstico. Esta revisión aborda recientes evidencias de la utilidad del método en la detección de la disfunción cardíaca clínica y subclínica, estratificación del riesgo cardiovascular y evaluación de las estrategias de intervención terapéutica.

Descrevemos dois casos de protrusão localizada do ventrículo esquerdo (VE), entidade que tem sido descrita na literatura como aneurisma ou divertículo. Em ambos os casos, observou-se uma evolução distinta da anteriormente relatada. A incidência e história natural dessas raras anomalias são pouco conhecidas, podendo evoluir de forma assintomática ou gerar graves complicações e até o óbito no período pré-natal. A abordagem terapêutica deve ser individualizada. We report two cases of localized left ventricular (LV) protrusion, an entity that has been described in the literature as aneurysm or diverticulum. Both cases had different outcomes from those previously reported. Little is known on the incidence and natural history of these rare anomalies, whose progression may be asymptomatic or lead to severe complications and death in the prenatal period. The therapeutic approach should be customized. Describimos dos casos de protrusión localizada del ventrículo izquierdo (VI), entidad que se describe en la bibliografía como aneurisma o divertículo. En ambos casos se observó una evolución distinta a la anteriormente relatada. La incidencia e historia natural de estas raras anomalías son poco conocidas, pudiendo evolucionar de forma asintomática o generar graves complicaciones e incluso el óbito en el período prenatal. El abordaje terapéutico debe ser individualizado.

Symptomatic intradialytic hypotension (IH) continues to be an important complication of hemodialysis treatment. It has been suggested that patients with left ventricular (LV) diastolic dysfunction may be more sensitive to the effects of reduced cardiac filling. Left atrial volume index (LAVi) reflects the chronicity of exposure to elevated LV filling pressures. The aim of this study was to identify the association between echocardiographic disease, in particular left atrium enlargement, and IH. Echocardiograms obtained in 172 patients undergoing hemodialysis in sinus rhythm and with no significant valvular or pericardial disease were analyzed. The independent association between LAVi and IH was assessed using multivariate logistic regression. IH was identified in 27 patients (16%). The patients who experienced the hypotensive episodes had a greater prevalence of previous heart failure (59% vs 22%, P0.001), systolic dysfunction (33% vs 14%, P = 0.003), and LAVi35 mL/m(2) (59% vs 32%, P = 0.008). No differences were noted for age, gender, body mass index, duration of dialysis, blood pressure, use of drugs, and proportions of arterial hypertension, diabetes, LV hypertrophy and diastolic dysfunction by Doppler. After multivariate analysis, only heart failure and LAVi35 mL/m(2) were seen to be independent predictors of IH. The finding of left atrium enlargement in patients undergoing hemodialysis may be useful in the clinical prediction of IH.

OBJETIVO: Avaliar o valor prognóstico do índice do volume do átrio esquerdo (IVAE) no curso clínico dos pacientes de hemodiálise (HD), comparando com parâmetros clínicos e ecocardiográficos previamente estabelecidos. MÉTODOS: Foram realizadas ecocardiografias em 118 pacientes de HD, que foram acompanhados por 19 ± 8 meses. O desfecho pesquisado foi composto de mortalidade geral e eventos cardiovasculares não-fatais. O valor prognóstico independente do IVAE foi testado pela análise multivariada de Cox. RESULTADOS: O IVAE e outros parâmetros clínicos e ecocardiográficos foram preditores do prognóstico na análise univariada. Na análise multivariada, o IVAE foi preditor independente do prognóstico (hazard ratio, 1,03 por ml/ m²; intervalo de confiança de 95%, 1,01 a 1,05; p=0,014) e adicionou informação incremental ao modelo contendo preditores tradicionais de risco cardiovascular, como massa ventricular esquerda, fração de ejeção e variáveis clínicas (p=0,02). CONCLUSÃO: O IVAE é preditor independente do prognóstico em pacientes de HD, provendo informação incremental aos dados clínicos e Doppler ecocardiográficos tradicionais. OBJECTIVE: To evaluate the prognostic value of left atrial volume index (LAVi) in the clinical course of hemodialysis (HD) patients, compared with previously established echocardiographic and clinical parameters. METHODS: Echocardiograms were obtained from 118 hemodialysis patients, who were then followed for 19 ± 8 months. Study endpoint was a composite of all-cause mortality and nonfatal cardiovascular events. Cox multivariate analysis was used do assess the independent prognostic value of LAVi. RESULTS: On univariate analysis, LAVi and other clinical and echocardiographic parameters were predictive of prognosis. Multivariate analyses showed that LAVi was an independent predictor of prognosis (hazard ratio 1.03 per ml/m², 95% confidence interval: 1.01 to 1.05, p=0.014), and added incremental information to the model containing traditional predictors of cardiovascular risk, such as left ventricular mass, ejection fraction, and clinical variables (p=0.02). CONCLUSION: LAVi is an independent predictor of prognosis in HD patients, providing incremental information to traditional clinical and Doppler echocardiographic data.
