1. Kis méretű vesedaganatok patológiai és biológiai jellemzőinek elemzése a tumorméret alapján

Abstract: Összefoglaló. Bevezetés: A kis méretű vesedaganatok között lényegesen gyakoribbak a benignus elváltozások, és a kis malignus tumorok biológiai tulajdonságai is kedvezőbbek, mint a nagyobb daganatokéi. Célkitűzés: Szerzők a kis méretű vesetumorok tulajdonságait vizsgáltuk különböző alcsoportokban. Módszer: 2000. január 1. és 2015. január 1. között 1272 beteg esetén végeztünk műtétet vesedaganat miatt. Közülük 496 betegnek volt kis méretű vesetumora. A betegek átlagéletkora 59 ± 12 év volt. A betegeket a tumorméret alapján három csoportba osztottuk. Az 1. csoportban a daganat mérete ≤4 cm, a 2. csoportban ≤3 cm és a 3. csoportban ≤2 cm volt. Eredmények: Az eltávolított daganat nagysága átlagosan 29 ± 8 mm volt. A szövettan 418 esetben (84%) malignus, míg 78 alkalommal (16%) benignus elváltozást mutatott. A 2 cm-nél kisebb daganatoknál malignitás csak az esetek 73,2%-ában fordult elő. A malignus és a benignus tumorok méretében szignifikáns eltérés volt (p = 0,008). Rosszul differenciált daganat – grade 3. és 4. – az esetek 10,8%-ában, 14,4%-ában, illetve 20,7%-ában volt jelen, amikor a tumorméret kisebb mint 2 cm, 2,1–3 cm, illetve 3,1–4,0 cm volt. A vesecarcinomáknál az átlagosan 10 éves utánkövetési idő alatt progresszió az esetek 5,5%-ában fordult elő. Következtetés: A kis méretű vesetumor az összes vesedaganat 39%-át tette ki. Ezek nagy része malignus volt, és benignus elváltozás az esetek 16%-ában fordult elő. A malignitás előfordulása a 2 cm-nél kisebb tumoroknál volt a legalacsonyabb. A tumorméret szoros összefüggést mutatott a malignitás gyakoriságával és a daganat differenciáltságával. A kedvező patológiai és biológiai eredmények alapján a 2 cm alatti daganatoknál felmerül annak lehetősége, hogy esetükben az aktív követés vagy minimálisan invazív kezelés alkalmazása kerüljön előtérbe. Introduction: The incidence of benign lesions is more common in small renal masses (SRMs) and biological behavior of small malignancies is better compared to larger ones. Objective: The authors measured the characteristics of SRMs in different subgroups. Method: From January 1, 2000 to January 1, 2015, 1272 patients underwent surgery for renal tumors. In 496 of the 1272 cases, the patients had SRMs. The mean age of the patients was 59 ± 12 years. Based on the sizes, the SRMs were divided into three groups. The sizes of the renal tumors were ≤4 cm in Group 1, ≤3 cm in Group 2 and ≤2 cm in Group 3. Results: The mean diameter of the removed SRMs was 29 ± 8 mm. Histology confirmed renal cell carcinoma in 418 cases (84%), while benign tumor was present in 78 patients (16%). However, with the tumor size ≤2 cm, malignancy was detected in 73.2% of the cases. There was a significant difference in the sizes of the malignant and the benign masses (p = 0.008). Grade 3 or 4 tumors were present in 10.8%, 14.4% and 20.7% when the tumor size was ≤2 cm, 2.1 to 3 cm, and 3.1 to 4 cm in diameter, respectively. During the mean 10-year follow-up period, tumor progression was detected only in 5.5% of malignancies. Conclusion: In 39% of all cases, the patients had SRMs. The majority of SRMs were malignant, and benign lesion occurred only in 16% of the cases. The incidence of malignant tumors was the lowest when the size of SRMs was ≤2 cm. The size of the tumor was highly associated with probability of malignancy and tumor grading. Based on the favorable pathological and biological results in tumors below 2 cm, active surveillance or minimally invasive treatment could be the preferred management. Abstract: In the 2011 superhero movie, Captain America: The First Avenger (produced by Marvel Studios, directed by Joe Johnston) the main opponent of the title character is a Nazi officer, Johann Schmidt, who turns out to be a kind of superhuman entity, the Red Skull. Throughout the movie, viewers can follow the process of him gradually leaving behind his identity as a Nazi officer, and presenting himself as the leader of the occult-high-tech terrorist organization, the Hydra. At a certain point we can see him visited by one of the scientists working for him, Doctor Zola, whom he puts wise to his plans. During the conversation a portrait is being painted of the Red Skull, but we cannot see his face, only that the artist uses a huge amount of red paint. In the background, excerpts of Wagner’s operas are being played, which is very unusual in a Marvel movie. The question is, should the viewer recognize the diegetic music and notice the possible reference to the painting Las Meninas by Velázquez? How the detection of these intermedial references and the awareness of the act of trespassing media borders would affect the semiotic processes of interpretation? And also, how would the more precise identification of the cited materials change the semiotic modality of intermediality here? The article tries to answer these questions by interpreting the scene and the role of the references in question within the entire film through the prism of intermedial semiotics.

Abstract: Selection of cancer patients for new targeted therapies reached its dead end as next generation sequencing based precision oncology approaches failed to deliver breakthrough improvements to oncology practice. The problem with current approaches is that the effect of a mutation can be indirect by influencing the expression of various other genes, which in turn can act as new therapy targets. A large-scale analysis of such cascades was not yet executed in breast cancer. Here, we developed an analysis tool to identify targetable genes showing an altered expression in relation to a mutation in other genes. The background database includes two independent large patient's cohorts, the TCGA and the Metabric datasets. Mutation status for each gene was determined using the VCF files from the TCGA repository. RNA-seq gene expression data for the same patients was re-normalized using a scaling normalization. Gene expression for the Metabric samples was determined using Illumina gene arrays and mutation status for the same patients is available for 174 selected genes. The Metabric database includes 1,981 patients and the TCGA breast cancer database contains 1,091 patients. Expression is linked with mutation status for each gene across all patients using Mann-Whitney test. A p We demonstrate the utility of the analysis platform by using it to uncover patient cohorts with higher expression of PD1 (PDCD1) and PD-L1 (CD274). Immune checkpoint inhibitors permbrolizumab and nivolumab target PD1. PD-L1 inhibitors include atezolizumab, avelumab, and durvalumab. None of these immunotherapy agents is approved to be used in breast cancer. In both settings, only one gene reached statistical significance. For PD1, the best performing gene was NOP14. Patients with mutation in NOP14 (1.3% of patients) had a 2.08x increased expression (4.58 in mutated vs. 2.20 in wild type) of PD1 (p=8.4e-05, FDR=0.0239). For PD-L1, the strongest gene was CCDC88A (mutated in 2.6% of patients), which had a 2.03x increased expression (10.42 in mutated vs. 5.13 in wild type) of PD-L1 (p=6.2e-05, FDR=0.0147). Both NOP14 and CCDC88A have been linked to cancer development and progression, but have not been investigated in relation to immune therapies. One can anticipate that patients with mutation in these genes will be prone to respond to immune checkpoint inhibitors. In summary, an online portal was set up capable to identify genes with altered expression in relation to a given mutation. The presented approach can help to increase speed and reduce cost of development for future anticancer treatments. The analysis tool also enables identification of patient cohorts for new agents and is accessible at www.mutarget.com. Abstract: Purpose: Pain is the most common symptom in acute pancreatitis (AP), and is part of the diagnostic criteria. Since its clinical characteristics in AP have not been detailed in the literature, we aimed to characterize acute abdominal pain in AP. Materials and methods: The Hungarian Pancreatic Study Group (HPSG) has prospectively collected multicenter clinical data of 1435 adult AP patients between 2012 and 2017. The pain was characterized by (i) its intensity (Visual Analog Scale ; 0–10), (ii) duration prior to admission (hours), (iii) localization (9 regions of the abdomen), and (iv) type (sharp, dull, cramping). Associations between the pain categories and outcome parameters were investigated Results: 97.3% of patients (n=1394) had pain on admission. It was mostly severe (VAS 7–10: 70%, n=511), cramping (61%, n=705), and epigastric (48%, n=687). Severe pain was associated with a more severe disease course (p72 h) on admission was not associated with worse outcomes. Abstract: Satellite imagery represents a unique opportunity to quantify the spatial and temporal changes of glaciers world-wide. Glacier velocity has been measured from repeat satellite scenes for decades now, yet a range of satellite missions, feature tracking programs, and user approaches have made it a laborious task. To date, there has been no tool developed that would allow a user to obtain displacement maps of any specified glacier simply by establishing the key temporal, spatial and feature tracking parameters. This work presents the application and development of a unique, semi-automatic, open-source, flexible processing toolbox for the retrieval of displacement maps with a focus on obtaining glacier surface velocities. SenDiT combines the download, pre-processing, feature tracking, and postprocessing of the highest resolution Sentinel-2A and Sentinel-2B satellite images into a semi-automatic toolbox, leaving a user with a set of rasterized and georeferenced glacier flow magnitude and direction maps for their further analyses. The solution is freely available and is tailored so that non-glaciologists and people with limited geographic information system (GIS) knowledge can also benefit from it. The system can be used to provide both regional and global sets of ice velocities. The system was tested and applied on a range of glaciers in mainland Norway, Iceland, Greenland and New Zealand. It was also tested on areas of stable terrain in Libya and Australia, where sources of error involved in the feature tracking using Sentinel-2 imagery are thoroughly described and quantified., This work is a contribution to the project Copernicus bretjeneste (Contract NIT.06.15.5), which is partly funded by the Norwegian Space Centre through Copernicus.

Abstract: The addition of synthetic 4-methoxy-2-phenethyl alcohol to the known attractant phenylacetaldehyde synergized attraction of the European corn borer Ostrinia nubilalis, the blend invariably catching 3 to 5 times more than phenylacetaldehyde on its own. Highest catches were recorded by the 1:1 blend. Both females and males were attracted, supposedly in the natural sex ratio of the local population. This improved bisex O. nubilalis attractant could be more efficient and more suitable for detection and monitoring purposes than previously know lures, making possible to draw more reliable plant protection decisions.

Abstract: In a temperate climate such as that of Romania, due to the high differences between comfort parameters and temperate environmental conditions, energy is needed in order to achieve a comfortable indoor environment in both winter and summer. Yet, due to the higher initial investment cost, in Romania, the new solutions for highly energy efficient buildings are rarely used. In order to increase the awareness of the investors on the long-term advantages of these solutions, pilot projects are necessary that provide real-time monitoring on the energy performance and behaviour of energy efficient buildings. At the Politehnica University of Timisoara, an experimental programme was developed to demonstrate that applying passive house design principles could be an alternative solution for energy-efficient buildings, reflecting the Romanian local climate conditions, materials, and construction techniques. An energy-efficient house was built following the passive house design principles and was subjected to extensive monitoring. In the design phase, the discussed house is compared to a reference house designed following the energy efficiency requirements in Romania, in order to emphasise the differences in terms of energy demand and life-cycle cost. The life-cycle cost analysis results are dependent on the future growth of energy prices. The study contains the results from the monitoring campaign of the energy efficient house, including the monitoring of the energy consumption as well as of the indoor parameters. The monitoring results indicate that the studied house is meeting the passive house design target of total primary energy requirement of less than 120 kWh/m 2 year.

Abstract: Materials used for the oil and gas industry are exposed to high pressure, high temperature and several aggressive fluids and gases. Concerning the still rising oil and gas product demand the development of new oil and gas valves is indispensable. Therefore, new reliable materials to guarantee facility safety at extreme operating conditions are needed. The presented study deals with a specific failure, the rapid gas decompression failure, which occurs due to the exposure to such extreme conditions. This failure leads to crack initiation, crack growth and in the worst case to the complete fragmentation of the component. For the characterization of this failure a hydrogenated acrylonitrile butadiene based rubber with an acrylonitrile content of 36% was exposed to several temperatures, saturation pressures, gas mixtures and different depressurization rates. Whereas, the rising testing temperature leads to decreasing volume change during the depressurization, the increase in saturation pressure, a higher decompression rate and a higher amount of carbon dioxide clearly lead to an increasing of the maximum observed volume change. Based on the observed volume change and the material ranking, determined using common testing standards NACE International (2003) and NORSOK (2001) a correlation to polymer physical principals was established.

Abstract: The aim of the study was to examine the determining factors of students' video usage and their learning satisfaction relating to the supplementary application of educational videos, accessible in a Moodle environment in a Business Mathematics Course. The research model is based on the extension of Technology Acceptance Model (TAM), in which the core TAM constructs – perceived usefulness, perceived ease of use, attitude – and internet self-efficacy were included as the explanatory factors of video usage. As regards the determinants of learning satisfaction, beside the core TAM constructs, the role of learning performance, learner-learner interaction, and learner-teacher interaction was examined. Data were collected from 89 students using a questionnaire, on which the partial least-squares structural equation modelling approach was used to evaluate the research model. The results confirmed that perceived usefulness, attitude, and internet self-efficacy had a direct effect on the video usage. Learning satisfaction was directly influenced by learner-learner interaction, perceived ease of use, and learning performance. Furthermore, the results indicated that video usage had a significant effect both on learning performance and on learning satisfaction. The findings show that the extended TAM model can be applied for predicting the university students' video technology usage and their learning satisfaction regarding the usage.

Abstract: Using online video presentations is increasingly gaining ground in higher education. Our present paper discusses the initial results of a longer research project and investigates the effect that the availability of online videos introduced to complement live presentations has on learning performance. Abstract: Nowadays the main task of scientists is to find natural ways of improving plant productivity that lead to environmentally friendly agriculture. Biofertilizers have a great potential to achieve this aim but unfortunately there is little information about their application in grape growing in Hungary. For this reason, a foliar nutrition experiment was conducted to investigate the effect of two biofertilizers (an algae product and a biostimulator, containing amino acids) on yield, leaf nutrient concentration, and quality parameters of Blaufrankish grape variety. The study was conducted in 2012 at Noszvaj in northeastern Hungary in a 24-year-old grapevine plantation on cv. Blaufrankish. Abstract: Building structures using Precast Reinforced Concrete Wall Panels (PRCWP) are widely used in Romania. Cut-out openings are often required to facilitate direct access from outside or between adjacent apartments. This paper presents a part of research program regarding the effectiveness of using the Externally Bonded Carbon Fiber Reinforced Polymer (CFRP-EBR) composites as seismic retrofitting solution for PRCWPs weakened by cut-out openings. Abstract: Local mechanical tensioning is one of the most efficient and industrially relevant stress engineering techniques to modify weld residual stress field and subsequently reduce buckling distortion. However, application of rolling load and its magnitude need to be optimised for an energy efficient rolling process. In the present study gas metal arc butt welded plates of low carbon mild steel were rolled by a dual roller in different rolling configuration (top and reverse side rolling) and with different magnitude of rolling load. All the plates were rolled post welding. Residual strain profiles of the post weld rolled plates were measured, using the SALSA strain scanner, and the in-plane stress were characterized. Average distortion of the rolled plates was correlated with the residual stress. Reverse rolling was found to be more effective in removing distortion while the stress profiles did not show any significant reduction of the peak stress.

Abstract: Summary Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an inherited arterial disease of the brain recently mapped to chromosome 19. We studied 148 subjects belonging to seven families by magnetic resonance imaging and genetic linkage analysis. 45 family members (23 males and 22 females) were clinically affected. Frequent signs were recurrent subcortical ischaemic events (84%), progressive or stepwise subcortical dementia with pseudobulbar palsy (31%), migraine with aura (22%), and mood disorders with severe depressive episodes (20%). All symptomatic subjects had prominent signal abnormalities on MRI with hyperintense lesions on T2-weighted images in the subcortical white-matter and basal ganglia which were also present in 19 asymptomatic subjects. The age at onset of symptoms was mean 45 (SD [10·6]) years, with attacks of migraine with aura occurring earlier in life (38·1 [8·03] years) than ischaemic events (49·3 [10·7] years). The mean age at death was 64·5 (10·6) years. On the basis of MRI data, the penetrance of the disease appears complete between 30 and 40 years of age. Genetic analysis showed strong linkage to the CADASIL locus for all seven families, suggesting genetic homogeneity. CADASIL is a hereditary cause of stroke, migraine with aura, mood disorders and dementia. The diagnosis should be considered not only in patients with recurrent small subcortical infarcts leading to dementia, but also in patients with transient ischaemic attacks, migraine with aura or severe mood disturbances, whenever MRI reveals prominent signal abnormalities in the subcortical white-matter and basal ganglia. Clinical and MRI investigations of family members are then crucial for the diagnosis which can be confirmed by genetic linkage analysis. The disease is probably largely undiagnosed.

Abstract: Can manufacturers produce hundreds of millions of cans annually, increasingly, food cans are being opened by lifting a tab on the can end that initiates a fracture, which then propagates around a circumferential score. The damage and fracture mechanisms that lead to crack initiation and propagation in the opening process, however, are not fully understood, therefore optimisation of easy open end scores is largely based on trial and error. This paper presents an experimental analysis that concentrates on the combined shear and bending forces as applied to the particular industrial method concerning full aperture easy open ends. The influence of a gradually increasing gap measured between the score and shear force location on traditional groove geometries and depths are examined for two different packaging steels. Earlier studies have shown that the complete opening cycle depends on fracture modes I, II & III as well as their combination. Experimental results for Modes I, II & III will be presented, however attention will focus on the behaviour of the initial fracture point, whereby prior investigations have shown it to be influenced primarily by mode II shearing. After initial specimen manufacture, where the score is formed by pressing a punch into a thin steel sheet the predeformed scored specimens are loaded in shear to simulate the local stress field found during the initial opening phase. Experiments have been completed using a novel Mode II experimental technique that has been designed for use in the majority of commercially available tensile test machines. Experimental results indicate that opening forces can change radically with different gap sizes and that there is considerable potential for the industrialised process of can end manufacture to be optimised through the efficient management and control of the can ends dimensional parameters.

Abstract: The heavy metals pollution is one of the problems that arise due to the increased uses of fertilizers and other chemicals to meet the higher demands of food production for human consumption. In order to assess possible health risk of apricot (Prunus armeniaca L.) consumption, levels of Arsenic, Cadmium, Mercury and Lead were determined in fresh and dried samples of "Jumbo cot", "Tom cot", "Gold strike", "Gold bar", "Bergeron", "Bergarouge", "Sweet cot", "Yellow cot" and "Zebra" apricot cultivars. Wet digestion of samples with concentrate HNO3 – H2O2 digester mixture and inductively coupled plasma–atomic emission spectroscopy was used. Highest content of As, Cd, Hg and Pb among all cultivars, were 0.5, 0.04, 1.5 and 0.5mg/kg of dried apricot samples. Fresh fruit samples also contain 0.2, 0.016, 0.6 and 0.2 mg/kg of Arsenic, Cadmium, Mercury and Lead respectively. Daily intake of metals, hazard quotient and health risk index to reveal health risk possibility of dried and fresh fruits consumption were calculate and compared.

Abstract: Steel laminates with a poly(ethylene terephthalate) coating can provide an environmental friendlier solution for packaging applications than current lacquered processes. One of the most challenging applications is the beer and beverage can where the PET layer is subjected to high temperatures (> 100°C), high strain rates (> 3000 1/s) and high hydrostatic pressure (> 500 MPa) in the wall ironing process. Specific issues to be solved are polymer hairs in the process and scuffing or scraping of the coating. A realistic simulation of the wall ironing process is needed to understand the processing conditions that influence these phenomena. The Arruda-Boyce model proved to be a good constitutive model for PET in the as supplied state as well as the deformed state of the coating. An improvement can be made by a full thermomechanical coupling of strain rate, temperature and pressure. The major factor of influence in the wall ironing process is the entry angle of the ironing die. To overcome issues with the required small entry angle and high expansion forces in the die a special die profile has been patented.

Abstract: Several fields of contemporary physics and chemistry, such as femtochemistry, time-resolved spectroscopy or attosecond pulse generation can benefit from ultrashort deep-UV pulses. Unfortunately, the generation of such pulses is rather challenging. Existing sources directly generating DUV pulses are rather exotic and up to now they could not generate pulses shorter than ∼100 fs. Therefore the usual approach has been to convert the ultrashort pulses generated by standard IR laser systems into the DUV range by some ultrabroadband frequency conversion technique. In this way sub-20 fs pulses could be generated in the DUV, however the pulse energy has been limited to a few microjoules [1∼4].

Abstract: As device linewidths shrink to 0.5 /spl mu/m and below the ECD (Electrical Critical Dimension) measurement technique is the best choice for conducting substrates. In this size regime, ECD is poised to replace SEM (Scanning Electron Microscope) as the standard tool of the semiconductor industry, with a measurement capability significantly better than that of the CD SEM. Keithley Instruments has developed an advanced electrical prober for use at Motorola's MOS-12 facility. This tool provides in-line CD measurements in a completely automated, hands-off "load-and-go" mode that requires only wafer loading, measurement recipe loading, and a "run" command for processing. Its expected capability will permit measurement of lines thinner than 0.5 /spl mu/m with a good accuracy, and with complete data transfer to the factory data collection and analysis system upon measurement completion. Results on the repeatability and reproducibility of fully automated measurements are presented, together with correlation to SEM measurements.

Abstract: The Portland District of the U.S. Army Corps of Engineers requested that scientists with the Pacific Northwest National Laboratory (PNNL) and the U.S. Army Engineer Research and Development Center (ERDC) conduct the hydroacoustic fish-passage studies described in this report. The ERDC also contracted with MEVATEC Corporation and Dyntel to provide staff ranging from scientists to technicians for the study. This study supports the Portland-District goal of maximizing fish passage efficiency (FPE) and obtaining 95% survival for juvenile salmon passing the Bonneville Project. This report presents results of two hydroacoustic studies of juvenile salmonids. One was a Project-wide study of fish-passage efficiency, and the other was more narrowly focused upon the approach, vertical distribution, and fish-guidance efficiency (FGE) of fish at Unit 15, where the Portland District extensively modified the gatewell and vertical barrier screen to improve gatewell flow and FGE. The goal of the larger of the two studies was to provide project-wide estimates of FPE, spill efficiency, and spill effectiveness for run-of-river fish passing the Bonneville Project during the 2001 out-migration. This type of study also provides estimates of the horizontal, vertical, and diel distributions of fish passage and FGE by turbine unit. Abstract: Purpose: An artifact typically seen in axial cone‐beam computed tomography(CBCT) reconstructions is a dark region or ‘black hole’ situated beneath the scan isocenter. We have traced the cause of the artifact to scattered radiation coming from the patient table. The artifact is most severe for offset‐detector scans and is especially significant in image‐guidedradiotherapy(IGRT) where patient tables have flat tops and are larger than those used for diagnostic radiology. To address this problem, we have developed a hybrid scatter kernel superposition (SKS) method designed to correct for scatter from both the object‐of‐interest and the patient table. Method and Materials: The proposed algorithm uses separate kernels to estimate scatter from the patient table and the object‐of‐interest. For each projection image, the location of the shadow cast by the patient table on the imager is determined. The region not shadowed by the table is processed using asymmetric kernels that best model scatter transport through body‐like objects. The patient table region is processed using a combination of the asymmetric kernels and slab‐derived symmetric kernels. The composition of these hybrid kernels is projection‐angle dependent. To test the algorithm, pelvis phantom and patient data were acquired using the Varian On‐Board Imager (OBI) and processed. Artifact intensities and HU accuracies were assessed. Results: The hybrid kernel algorithm produced substantially better scatter estimates than previous SKS methods. HU non‐uniformities in the pelvis phantom were reduced from 97HU to 29HU. In the in vivo scan, the black hole artifact was reduced by 128HU and anatomical details were made more visible. Conclusions:Radiotherapy patient tables can produce significant scatter‐related artifacts. The proposed hybrid SKS algorithm effectively addresses this problem. Scatter from both the object‐of‐interest and the patient table was accurately estimated, and image uniformity and HU accuracy were greatly improved. Employees of Varian Medical Systems.

Abstract: Groups of CBA mice immunosuppressed with anti-thymocyte serum (ATS) treatment were xeno-transplanted with either HeLa human cervical carcinoma cells or genetically modified cells expressing the human tumor necrosis factor-alpha (TNF) gene (All cells). Both cell lines were highly resistant to the cytotoxic effects of TNF. If 3 x 10(6) tumor cells were inoculated s.c. into female mice, HeLa cells grew progressively into large tumors and killed 74% of the recipients, while TNF-expressing All cells caused fatal tumor growth only in 22% of the mice. 3 x 10(6) or 1.5 x 10(7). All cells produced progressive tumor growth and lethality in all male recipients. In sera of all the A11-cell-transplanted mice, biologically active TNF was detected shortly (4.5 h) after tumor inoculation (6 39 U/ml), decreasing to below detection level in the circulation by day 3. In recipients of 15 million A11 cells, circulating TNF reappeared and reached high levels (12-1000 U/ml) 3 to 7 weeks later, when the animals bore large tumors (14-23 mm). Generally, such mice became cachectic, severely anemic, hypothermic, and soon died. On account of calcium mobilization from bones, their serum Ca levels were high. Electron microscopy revealed severe liver damage, but there were no signs of chronic arthritis. These results suggest that ATS-treated mice xenotransplanted with TNF-gene-transfected A11 human tumor cells provide a new model for studying the pathophysiological and anti-tumor effects of TNF.

Abstract: To determine whether once-daily, in the morning, topical application of the new ocular hypotensive prostaglandin analogue, latanoprost, yields nocturnal intraocular pressure (IOP) reduction similar to its diurnal IOP reducing efficacy.Placebo- controlled, randomized, and double-masked study on hospitalized patients with ocular hypertension or glaucoma. Patients in group 1 (n=9) were maintained on twice-daily applications of 0.5% timolol maleate. Patients in group 2 (n=10) terminated their timolol treatment 3 weeks before the beginning of the study. In both groups the test drug (0.005% latanoprost) and its vehicle (placebo) was applied by hospital staff every morning for 9 days.After 4 days of ambulatory treatment, patients were hospitalized, and IOP values were obtained in the supine and sitting positions with a handheld electronic tonometer (Tono-Pen XL, Bio-Rad, Glendale, Calif) and a Goldmann's applanation tonometer, covering every 2-hour interval, around the clock, but not more than at four time points per day during a 5-day period.The mean nocturnal IOPs (Goldmann's applanation tonometer) collected for 5 days were mean +/-SEM 17.9+/-0.6 vs 20.2+/-0.6 mm Hg and 16.8+/-0.3 vs 20.6+/-0.5 mm Hg for the study vs the control eyes in group 1 and group 2, respectively. These nocturnal IOP reductions were statistically significant (P.001, two-tailed paired Student's t test). The differences between diurnal and nocturnal IOP reductions (handheld electronic or Goldmann's applanation tonometer) were minimal (0.3 mm Hg) and statistically not significant (P.31, two-tailed paired Student's t test).Once-daily latanoprost treatment provides uniform circadian (around-the-clock) IOP reduction by itself, or in combination with timolol.

Scans of Catphan® and electron density (Model 062A, CIRS) phantoms have been compared with OBI scans. Hounsfield unit (HU) accuracy was checked by changing the z scan length and the phantom diameters for pelvis (125kVp, 680mAs, 45cm dia.) acquisitions. The Catphan was imaged using doses ranging between 5–20mGy (CTDIw). Projections acquired using clinical OBI units were also reconstructed for comparison using the new reconstructor.Results: The new CBCT system has higher dose efficiency and higher HU accuracy. When the volume length is reduced from 160mm to 90mm or when the phantom diameter is reduced from 330mm to 180mm, the HU values measured for the same inserts differ by −190 to +80HU for OBI scans and by –60 to +50HU for Trilogy MX scans (for electron densities between 0.2 and 1.2). The contrast detectability of the 1% contrast objects in the Catphan phantom improves from 9mm to 4mm diameter when using the same CTDIwdose as OBI. Clinical images exhibit much better uniformity, elimination of streaks and better definition of the skin surface. Conclusions: The new reconstruction algorithm makes substantial improvements in CBCTimage quality, reduces patient dose and increases HU accuracy. The new system produces CBCTimages, which are much better suited to image guidance and which may be suitable for other tasks such as adaptive RT planning.

48. Maintained Intraocular Pressure Reduction With Once-a-Day Application of a New Prostaglandin F2α Analogue (PhXA41)

Abstract: To eliminate uncertainties about compliance, 15 patients with glaucoma (intraocular pressure [IOP]22 mm Hg and40 mm Hg) were hospitalized to participate in a clinical trial of the ocular hypotensive effectiveness of the new prostaglandin F2 alpha analogue prodrug, PhXA41 (13,14-dihydro-17-phenyl-18, 19, 20-trinor-PGF2a-isopropyl ester; latanoprost [World Health Organization generic name]). At 9 PM on each of five consecutive days, one of the investigators applied one drop of a 0.006% solution of PhXA41 (representing approximately 2 micrograms of PhXA41 per treatment) to one eye of nine patients and one drop of placebo to one eye of six patients. This was followed by an evaluation of potential local side effects at 9:30 PM. Complete examinations, including tonometry (Goldmann), were also performed at 8 AM and 8 PM on days 1 to 6, as well as at noon and 4 PM on days 1, 2, and 6. Except for mild conjunctival hyperemia in two PhXA41-treated eyes (once each at 8 AM), no side effects were observed or reported by any patient. Starting with the first IOP measurement after the first treatment (8 AM on day 2), IOP was reduced by 20% to 30% in the eyes treated with PhXA41. This reduction was highly significant (P.01 at 12 time points and P.05 at the remaining two measurements) throughout the study. The IOP reduction did not become attenuated during the 23 hours after treatments. At 11 hours after the last treatment, the mean (+/- SD) IOP difference between PhXA41-treated and contralateral control eyes was -5.5 +/- 2.8 mm Hg, as compared with -6.1 +/- 1.8 mm Hg 12 hours later. PhXA41 must, therefore, be regarded as an excellent candidate for use as a once-a-day glaucoma medication.

Abstract: Twenty-two pregnant patients were exposed, 12±1 days after their missed menstrual period, to a single intrauterine dose of 5mg PG F2 α , to provoke a PG ″Impact″ (PGI) and through it legal abortion. The PGI, delivered through the cervix during 10 minutes, induced 83±9mm Hg contracture in 20±3 minutes. Cyclic IUP reached 102±10mm Hg only in 116±14 minutes, it was then sustained for about 2 hours and subsequently declined. During the evolution of IUP uterine bleeding appeared, progesterone (P) and estradiol 17 β (E 2 ) started to decrease and continued decreasing. At 24 hours after PGI, P-withdrawal was 44% (P None of the patients aborted during the short lived period of exogenous PG stimulation. However, the continued steroid withdrawals indicated that PGI damaged the endocrine function of the ovum and the luteotrophic support of the corpus luteum. Therefore, if it was PG which completed abortion eventually, it had to be the endogenous compound, having become effective in physiological concentrations, due to the threshold-lowering action of P-withdrawal.
