Purpose To compare clinical outcomes of ultrasound-guided needle release with corticosteroid injection versus mini-open surgery in patients with carpal tunnel syndrome (CTS).Methods From January 2020 to December 2020, 40 patients (40 wrists) with CTS were retrospectively analyzed in this study. Diagnosis was based on clinical symptoms, electro-physiological and ultrasound imaging. 20 wrists were treated with ultrasound-guided needle release plus corticosteroid injection (Group A) and other 20 wrists went mini-open surgery (Group B). We evaluated Boston carpal tunnel questionnaire, electrophysiological parameters (distal motor latency, sensory conduction velocity and sensory nerve action potential of median nerve) and ultrasound parameters (cross-sectional area, flattening ratio and the thicknesses of transverse carpal ligament) both before and 3 months after treatment. Total treatment cost, duration of treatment, healing time and complications were also recorded for two groups.Results Boston carpal tunnel questionnaire, electrophysiologic and ultrasound outcomes at preoperatively and 3 months postoperatively had significant difference for each group (each P༜0.05). There were no complications such as infection, haemorrhage, vascular, nerve or tendon injuries in both groups.Conclusion Ultrasound-guided needle release and mini-open surgery are both effective measures in treating CTS patients. Ultrasound-guided needle release plus corticosteroid injection provides smaller incision, less cost, less time of treatment and faster recovery compared with mini-open surgery. Background The pandemic of COVID‐19 has a persistent impact on global health, yet its sequelae need to be addressed at a wide scale around the globe. This study aims to investigate the characteristics, prevalence, and risk factors for mid‐term (>6 months) clinical sequelae in a cohort of COVID‐19 survivors. Methods Totally 715 COVID‐19 survivors discharged before April 1, 2020, from three medical centers in Wuhan, China, were included. The longitudinal study was conducted by telephone interviews based on a questionnaire including the clinical sequelae of general, respiratory, and cardiovascular systems. Demographics and some characteristics of clinical sequelae of the survivors were recorded and analyzed. Multivariate logistic regression analysis was applied to explore the risk factors for the sequelae. Results The median time interval from discharge to telephone interview was 225.0 days. The COVID‐19 survivors' median ages were 69 years, and 51.3% were male. Among them, 29.9% had at least one clinical sequela. There were 19.2%, 22.7%, and 5.0% of the survivors reporting fatigue, respiratory symptoms, and cardiovascular symptoms, respectively. Comorbidities, disease severity, the application of mechanical ventilation and high‐flow oxygen therapy, and the history of re‐admission were associated with the presence of clinical sequelae. Conclusions Our study provides further evidence for the prevalence and characteristics of clinical sequelae of COVID‐19 survivors, suggesting long‐term monitoring and management is needed for their full recovery.

The lymph node ratio (LNR) has been considered a better prognostic factor than traditional N staging in patients with gastric cancer (GC), but its accuracy is unclear for those who receive neoadjuvant therapy (NAT). We aimed to compare the node ratio (Nr) staging with the ypN staging and to thereby develop a modified staging system incorporating Nr staging. A total of 1791 patients who underwent gastrectomy after NAT in the Surveillance, Epidemiology, and End Results database were retrospectively analyzed. ypTNrM staging was established based on the overall survival (OS). The Nr staging was generated using 0.2 and 0.5 as the cutoff values of LNR and represented patients with more homogeneous OS compared with ypN staging. The 5-year OS rates for ypTNrM stages IA, IB, II, IIIA, and IIIB were 70.2%, 54.2%, 36.0%, 21.2%, and 6.6%, respectively, compared with 58.8%, 39.1%, and 21.6% for ypTNM stages I, II, and III, respectively. On pig farms, many piglets die because they are crushed when sows roll from side to side or lie down. On average, 1.2 piglets are crushed by sows every day. To resolve the piglet mortality issue, this article proposes PigTalk, an artificial intelligence (AI) based Internet of Things (IoT) platform for detecting and mitigating piglet crushing. Through real-time analysis of the voice data collected in a farrowing house, PigTalk detects if any piglet screaming occurs, and automatically activates sow-alert actuators for emergency handling of the crushing event. We propose an audio clip transform approach to pre-process the raw voice data, and utilizes min-max scaling in machine learning (ML) to detect piglet screams. In our first contribution, the above data preprocessing method together with subtle parameter setups of the machine learning model improve the piglet scream detection accuracy up to 99.4%, which is better than the previous solutions (up to 92.8%). In our second contribution, we show how to design two cyber IoT devices, i.e., DataBank for data pre-processing and ML_device for real-time AI to automatically trigger actuators such as floor vibration and water drop to force a sow to stand up. We conduct analytic analysis and simulation to investigate how the detection delay affects the critical time period to save crushed piglets. Our study indicates that PigTalk can save piglets within 0.05 s with 99.93% of the successful rate. Such results are validated in a commercial farrowing house. PigTalk is a new approach that automatically mitigates piglet crushing, which could not be achieved in the past.

BACKGROUND Currently, rectovaginal fistula (RVF) continues to be a surgical challenge worldwide, with a relatively low healing rate. Unclosed intermittent suture and poor suture materials may be the main reasons for this. AIM To evaluate the efficacy and safety of stapled transperineal repair in treating RVF. METHODS This was a retrospective cohort study conducted in the Coloproctology Department of The Sixth Affiliated Hospital of Sun Yat-sen University (Guangzhou, China). Adult patients presenting with RVF who were surgically managed by perineal repair between May 2015 and May 2020 were included. Among the 82 total patients, 37 underwent repair with direct suturing and 45 underwent repair with stapling. Patient demographic data, Wexner faecal incontinence score, and operative data were analyzed. Recurrence rate and associated risk factors were assessed. RESULTS The direct suture and stapled repair groups showed similar clinical characteristics for aetiology, surgical history, fistula features, and perioperative Wexner score. The stapled repair group did not show superior results over the suture repair group in regard to operative time, blood loss, and hospital stay. However, the stapled repair group showed better postoperative Wexner score (1.04 ± 1.89 vs 2.73 ± 3.75, P = 0.021), less intercourse pain (1/45 vs 17/37, P = 0.045), and lower recurrence rate (6/45 vs 17/37, P = 0.001). There was no protective effect from previous repair history, smaller diameter of fistula (< 0.5 cm), better control of defecation (Wexner < 10), or stapled repair. Direct suture repair and preoperative high Wexner score (> 10) were risk factors for fistula recurrence. Furthermore, stapled repair gave better efficacy in treating complex RVFs (i.e., multiple transperineal repair history, mid-level fistula position, and poor control of defecation). CONCLUSION Stapled transperineal repair is advantageous for management of RVF, providing a high primary healing rate and low recurrence rate.

This study aimed to investigate the mental health status of nurses from low‐risk areas of novel coronavirus (COVID‐19) pandemic, its potential impact factors, and the main stressors under the normalized prevention and control in China. A mobile phone app‐based survey was conducted among registered nurses in Jiangsu province via a region‐stratified sampling method. The questionnaire consisted of items on the demographic characteristics of the nursing staff and their Depression, Anxiety, Stress Scale‐21 (DASS‐21) along with questions for self‐assessment of stressors that are associated with COVID‐19. STROBE guideline was used. Among 1803 nurses who were working in the low‐risk areas in Jiangsu, 22.0%, 29.8%, and 16.1% of them reported moderate to extreme levels of depression, anxiety, and stress, respectively. Having 11–15 years of working experience and being a fixed‐term contract nurse were associated with experiencing worse mental health outcomes while supporting‐Wuhan working experience and having mental health preparation course training were independent factors that had beneficial impact on their psychological well‐being afterward. In terms of source of pressure, a key finding of this study is that the main stressor among these nurses was the lack of patient’s understanding and cooperation (71.2%) which calls for better psychosocial communication between nurses and patients. The present findings would provide information for other regions at low risk of COVID‐19 and may aid the provision of support and interventions for the benefit of the psychological well‐being of nurses who are exposed to life‐threatening occupational risks and are more vulnerable to the pandemic than others.

BackgroundMetastasis from extramammary primary tumor to breast is extremely rare. Case SummaryA 59-year-old woman with 1-year history of rectal cancer presented with asymptomatic breast mass. At 16 months after the diagnosis of rectal mucinous adenocarcinoma, a breast mass was confirmed by ultrasonography and identified by pathology and immunohistochemistry as a metastasis from the rectal cancer. Treatments included chemotherapy (6 cycles: 300 mg irinotecan on day 1, 4.5 mg raltitrexed on day 2, 450 mg bevacizumab on day 3), radiotherapy, and surgical resection. Two years of follow-up examinations (6-months intervals) showed no evidence of recurrence or novel distant metastasis. ConclusionBreast metastasis from rectal carcinoma is a rare secondary malignancy. Final diagnosis can be established by histopathology and immunohistochemistry.

Over the past decade, significant developments have taken place in the field of deep learning. State-of-the-art convolutional neural networks (CNNs), a branch of deep learning, have been increasingly applied in various fields such as image classification, speech recognition, and natural language processing. Due to the high computational complexity of CNNs, lots of works have proposed their CNN accelerators to address this issue. Besides, a processing element (PE) array has been recently further focused and discussed since it is responsible for the entire computations as the core of CNN accelerators. Therefore, the specialized design of a PE array becomes one of the main researches on CNN accelerators for energy efficiency and high throughput. In this brief, a throughput-optimized PE array for CNNs based on the channel-oriented data pattern is proposed. The proposed PE array features fully PE interconnection which achieves scalability. Besides, any sized convolution can be processed in the PE array while maximizing the utilization of PEs by exploiting the channel-oriented data pattern. Compared to previous works, this brief achieves $1.22\times $ and $1.25\times $ improvement in the throughput density on AlexNet and VGG-16 respectively.

Distributed control is the most popular control strategy for controlling numerous slave devices. However, the performance of slave devices is hard to improve because the control algorithm is handled by the commercial slave devices. Therefore, this paper proposes a hybrid control structure that follows the concept of centralized control strategy and handles part of the control loop on the master computer in a distributed control system through EtherCAT fieldbus. In this study, different advanced control methods such as robust control and learning control were applied to handle the position control loop of a scanning mirror system in a selective laser melting (SLM) system with stringent demand of position error around 40 µm based on EtherCAT fieldbus. By designing a proper controller, the experiment results showed that the tracking performance was significantly improved. The tracking error dropped by about 80% after the proposed hybrid control structure was applied. By applying the proposed hybrid control structure, one can improve the high-performance-requirement devices, while reducing the calculation burden of the master computer for other low-performance-requirement devices by retaining the benefit of distributed control.

Microorganisms are ubiquitous in our world, and they form different communities between organisms and the surrounding environment. It is estimated that there are more than 108 bacteria in a small gram of soil, and there are more than 1013 bacteria in the human intestine. It is a complex ecosystem formed under the interaction mechanisms of resource competition, nutrient symbiosis, quorum sensing, and horizontal gene transfer. Among them, the microbial communities in the soil and hydrosphere environment play a key driving role in the global chemical cycle of carbon, nitrogen and other elements. In industrial production, the microbial communities can treat our waste and purify the environment. In the field of human diseases, the microbial community in the human intestine is closely related to the development, metabolism and immunity of the host. Therefore, studying the stability of the composition of the microbial community is of great significance for solving the problems of ecological environment, agricultural production, and human health. With the development of high-throughput sequencing technology, the diversity and stability of microbial communities in different environments have been deeply studied. Although the scientific community has paid more attention to microbial communities in recent years, there are still many difficulties before us effectively use these communities to meet social requirements and environmental challenges. Therefore, it is critical to use new scientific concepts such as synthetic biology to interpret existing scientific problems. Among them, the core cycle of synthetic biology, Design-Build-Test-Learn is expected to promote the perfection and discovery of the basic theory of microbial ecological communities, and promote the research and development of biotechnology lays a theoretical foundation for improving the application of human health, agricultural production and environmental pollution. The ecology of microbial communities in nature is highly complex, making it challenging to carry out reproducible, controllable perturbation experiments in natural ecosystems. In contrast, synthetic communities constructed in the laboratory using a “bottom-up” approach offer moderate complexity and high controllability, and can serve as a bridge between complex ecosystems and mathematical modeling. By combining synthetic microbial communities, quantitative models and sequencing techniques, we can systematically study interactions between microbial populations and predict the dynamics of microbial communities. In terms of application, research on synthetic microbial communities will provide critical insights for how to control and engineer complex microbial ecosystems, and help solve important problems in human health and agricultural production via the design of stable microbial communities with controllable functions. In this review article, we mainly focus on the construction methods and theoretical foundations of existing synthetic microbial communities and the potential applications of these communities from two aspects, namely the knowledge of creation and the application of creation. And we also discuss the potential application of synthetic biology methods in this field.

Introduction This study was to develop a simple model for predicting malignancy of peripheral pulmonary lesions (PPLs) based on endobronchial ultrasonography (EBUS) and clinical findings. Methods Patients who had EBUS for PPLs were analyzed and compared on the EBUS imaging characteristics and clinical data. The malignancy prediction model was established by the logistic equation of probability of malignant PPL based on the data of 135 patients. The model was tested on an additional 50 patients for efficiency. Results Among 135 prospectively enrolled patients, 77 (57%) patients had malignant and 58 (43%) had benign lesions with the size of 36.5±19.9 mm. Univariate analysis demonstrated a significant (P 0.05) difference in age (50 years as the cutoff value) and history of extra-thoracic malignancies. Background Localized aggressive periodontitis is rare periodontitis in clinical practice, which often occurs in young adults under 35 years old, seriously affecting patients' quality of life. As a tetracycline antibacterial drug, minocycline is also considered an essential choice to treat periodontal disease. However, few reports focused on the effect of xipayi mouth rinse combined with minocycline on periodontal pathogens. The goal of this study was to investigate the clinical effect of xipayi mouth rinse combined with minocycline in the treatment of localized aggressive periodontitis and its effect on the levels of CRP, TNF-α, and IL-6. Methods Ninety-six patients with limited aggressive periodontitis were selected and randomly divided into two groups. Forty-eight patients in the control group were treated with xipayi mouth rinse after primary periodontal treatment. Then, 48 patients in the experimental group were treated with xipayi mouth rinse combined with minocycline after primary periodontal treatment. The periodontal probe was applied to detect periodontal plaque index (PLI), periodontal pocket depth (PD), sulcus bleeding index (SBI), gingival index (GL), and clinical attachment loss (CAL) before and after treatment in both groups of patients. ELISA was used for detecting the expression levels of CRP, TNF-α, and IL-6 in the serum of patients in two groups before and after treatment. We compared the recurrence rates of the two groups after a 1-year follow-up. Results Compared with the control group, the PLI, PD, SBI, GL, CAL, and total masticatory efficiency of the experimental group were significantly better than those of the control group. The levels of inflammatory factors CRP, TNF-α, and IL-6 were significantly declined, and the total effective rate of treatment was significantly elevated. After follow-up, it was found there was no noticeable difference in the recurrence rate between the two groups. Conclusions Xipayi mouth rinse, combined with minocycline in the treatment of localized aggressive periodontitis, can significantly improve the periodontal gingival condition and reduce the level of inflammatory factors. Also, the efficacy of the treatment was significant. This experiment has provided ideas for improving the clinical treatment of patients with localized aggressive periodontitis.

Despite treatment advances, radioresistance and metastasis markedly impair the benefits of radiotherapy to patients with malignancies. Functioning as molecular switches, Rho guanosine triphosphatases (GTPases) have well-recognized roles in regulating various downstream signaling pathways in a wide range of cancers. In recent years, accumulating evidence indicates the involvement of Rho GTPases in cancer radiotherapeutic efficacy and metastasis, as well as radiation-induced metastasis. The functions of Rho GTPases in radiotherapeutic efficacy are divergent and context-dependent; thereby, a comprehensive integration of their roles and correlated mechanisms is urgently needed. This review integrates current evidence supporting the roles of Rho GTPases in mediating radiotherapeutic efficacy and the underlying mechanisms. In addition, their correlations with metastasis and radiation-induced metastasis are discussed. Under the prudent application of Rho GTPase inhibitors based on critical evaluations of biological contexts, targeting Rho GTPases can be a promising strategy in overcoming radioresistance and simultaneously reducing the metastatic potential of tumor cells.

Meigs' syndrome (MS), a rare complication of benign ovarian tumors, is easily misdiagnosed as ovarian cancer (OC). We retrospectively reviewed the clinical laboratory data of patients diagnosed with MS from 2009 to 2018. Serum carbohydrate antigen 125 and HE4 levels were higher in the MS group than in the ovarian thecoma-fibroma (OTF) and healthy control groups (all P < 0.05). However, the serum HE4 levels were lower in the MS group than in the OC group (P < 0.001). A routine blood test showed that the absolute counts and percentages of lymphocytes were significantly lower in the MS group than in the OTF and control groups (all P < 0.05). However, these variables were higher in the MS group than in the OC group (both P < 0.05). The neutrophil-to-lymphocyte ratio (NLR) was also significantly lower, whereas the lymphocyte-to-monocyte ratio was higher in the MS group than in the OC group (both P < 0.05). The NLR, platelet-to-lymphocyte ratio, and systemic immune index were significantly higher in the MS group than in the OTF and control groups (all P < 0.05). The hypoxia-inducible factor-1 mRNA levels were also significantly higher, whereas the glucose transporter 1, lactate dehydrogenase, and enolase 1 mRNA levels were lower in peripheral CD4+ T cells obtained preoperatively in a patient with MS than those in patients with OTF, patients with OC, and controls (all P < 0.05). The expression of these four glucose metabolism genes was preferentially restored to normal levels after the tumor resection of MS (P < 0.001). These clinical laboratory features can be useful in improving the preoperative diagnostic accuracy of MS.

The side-reactor column (SRC) configuration is an effective integrated reaction separation technique, but its complex nature such as many nonlinear equations and integer type variables raises difficulty for process optimization. Here, a mixed-integer nonlinear programming (MINLP) approach coupled with simulation software (Aspen Plus) was proposed to design an SRC configuration. The MINLP model was developed to optimize the structural/operating variables simultaneously. The use of Aspen Plus provided high convergence and accuracy of rigorous process model simulation. We improved stochastic algorithm bat algorithm to solve the MINLP model. The model and algorithm were both implemented in the programming software MATLAB. The capability of this optimization approach was manifested by two case studies of methyl acetate and methyl laurate production systems.

Multi-wavelength lasing, with dynamic switching functionality, high spectral purity and contrast, plays an essential role in photonic devices. Lanthanide(Ln3+)-doped upconversion nanocrystals(UCNCs) featured with plentiful energy levels act as ideal candidates for the gain medium. However, it remains a daunting challenge to develop a tunable Ln3+-based single-mode laser across a wide wavelength range due to the absence of an appropriate mode-selection mechanism. Here, we have demonstrated the first active control of unidirectional single-mode lasing with switchable wavelength spanning beyond a record range (~ 300 nm). This is accomplished through the integration of two size-mismatched coupled microdisks cavity and inversely designed dual-mode UCNCs incorporated with selective sensitizer-pair and activator ions. By asymmetric pumping, a reversible and crosstalk-free ultraviolet-to-red single-mode operation were formed respectively from such UCNCs-based system through changing the pumping wavelengths from 980 nm to 808 nm. The results enlighten the rational design of luminescent materials and microcavity. With remarkable doping flexibility, our approach would pave an avenue to a class of UCNCs-based photonic devices for on-chip optical filter, switch, sensor and other devices.

Background New evidence from clinical and fundamental researches suggests that SNHG7 is involved in the occurrence and development of carcinomas. And the increased levels of SNHG7 are associated with poor prognosis in various kinds of tumors. However, the small sample size was the limitation for the prognostic value of SNHG7 in clinical application. The aim of the present meta-analysis was to conduct a qualitative analysis to explore the prognostic value of SNHG7 in various cancers. Methods Articles related to the SNHG7 as a prognostic biomarker for cancer patients, were comprehensive searched in several electronic databases. The enrolled articles were qualified via the preferred reporting items for systematic reviews and meta-analysis of observational studies in epidemiology checklists. Additionally, an online database based on The Cancer Genome Atlas (TCGA) was further used to validate our results. Results We analyzed 2418 cancer patients that met the specified criteria. The present research indicated that an elevated SNHG7 expression level was significantly associated with unfavorable overall survival (OS) (HR = 2.45, 95% CI: 2.12–2.85, p p p p p p Conclusions The present analysis suggests that elevated SNHG7 is significantly associated with unfavorable OS, tumor progression, LNM and DM in various carcinomas, and may be served as a promising biomarker to guide therapy for cancer patients.

Background: Congenital sucrase-isomaltase deficiency (CSID) is an autosomal recessive inherited disease that leads to the maldigestion of disaccharides and is associated with mutation of the sucrase-isomaltase (SI) gene. Cases of CSID are not very prevalent in China or worldwide but are gradually being identified and reported.Case Presentation: We report a case involving a 14-month-old male who presented with failure to thrive that had begun after food diversification and was admitted for chronic diarrhea. We used a whole-exome sequencing (WES) approach to identify mutations in this patient's genome. WES revealed two novel heterozygous mutations in the SI gene, c.2626C > T (p.Q876*) and c.2872C > T (p.R958C), which were confirmed by Sanger DNA sequencing. With a strict sucrose- and starch-restricted diet, the patient's diarrhea was resolved, and he began to gain weight.Conclusions: We report a case of novel variants in the SI gene that caused CSID. This report provides valuable information for the clinical field, especially in China.

Mitochondria are the primary organelles which can produce adenosine triphosphate (ATP). They play vital roles in maintaining normal functions. They also regulated apoptotic pathways of cancer cells. Given that, designing therapeutic agents that precisely target mitochondria is of great importance for cancer treatment. Nanocarriers can combine the mitochondria with other therapeutic modalities in cancer treatment, thus showing great potential to cancer therapy in the past few years. Herein, we summarized lipophilic cation- and peptide-based nanosystems for mitochondria targeting. This review described how mitochondria-targeted nanocarriers promoted highly efficient cancer treatment in photodynamic therapy (PDT), chemotherapy, combined immunotherapy, and sonodynamic therapy (SDT). We further discussed mitochondria-targeted nanocarriers’ major challenges and future prospects in clinical cancer treatment.

Background: The genetic susceptibility to ischemic stroke (IS) is still not well-understood. Recent genome-wide association studies (GWASes) found that several single nucleotide polymorphisms (SNPs) in the Diacylglycerol acyltransferase 2 gene (DGAT2) and monoacylglycerol O-acyltransferase 2 (MOGAT2) cluster were associated with serum lipid levels. However, the association between the DGAT2-MOGAT2 SNPs and serum lipid phenotypes has not yet been verified in the Chinese people. Therefore, the present study was to determine the DGAT2-MOGAT2 SNPs and gene-environment interactions on serum lipid profiles and the risk of IS.Methods: Genotyping of 5 SNPs (DGAT2 rs11236530, DGAT2 rs3060, MOGAT2 rs600626, MOGAT2 rs609379, and MOGAT2 rs10899104) in 544 IS patients and 561 healthy controls was performed by the next-generation sequencing technologies. The association between genotypes and serum lipid data was determined by analysis of covariance, and a corrected P-value was adopted after Bonferroni correction. Unconditional logistic regression analysis was performed to assess the association between genotypes and the risk of IS after adjustment of potential confounders.Results: The rs11236530A allele was associated with increased risk of IS (CA/AA vs. CC, OR = 1.45, 95%CI = 1.12–1.88, P = 0.0044), whereas the rs600626G-rs609379A-rs10899104G haplotype was associated with decreased risk of IS (adjusted OR = 0.67, 95% CI = 0.48–0.93, P = 0.018). The rs11236530A allele carriers had lower high-density lipoprotein cholesterol (HDL-C) concentrations than the rs11236530A allele non-carriers (P < 0.001). The interactions of rs11236530-smoking, rs3060-smoking and rs10899104-smoking influenced serum apolipoprotein B levels, whereas the interactions of rs11236530- and rs3060-alcohol affected serum HDL-C levels (PI < 0.004–0.001). The interaction of rs600626G-rs609379A-rs10899104G-alcohol (OR = 0.41, 95% CI = 0.22–0.76) and rs600626G-rs609379C-rs10899104T-alcohol (OR = 0.12, 95% CI = 0.04–0.36) decreased the risk of IS (PI < 0.0001).Conclusions: The rs11236530A allele was associated with decreased serum HDL-C levels in controls and increased risk of IS in patient group. The rs600626G-rs609379A-rs10899104G haplotype, the rs600626G-rs 609379A-rs10899104G-alcohol and rs600626G-rs609379C-rs10899104T-alcohol interactions were associated with decreased risk of IS. The rs11236530 SNP may be a genetic marker for IS in our study populations.

Objective Low triiodothyronine syndrome (LT3S) is a common endocrine disease in preterm neonates. Various serious acute or chronic diseases result in LT3S. Few studies have investigated the causal relationship between perinatal factors and LT3S in preterm neonates with a gestational age (GA) of 28–35 weeks. The present study comprehensively analyzed the perinatal factors of LT3S in preterm neonates. Methods This was a retrospective study of neonates with and without LT3S from January 2018 to November 2019. Compared to 206 preterm neonates without LT3S, 158 neonates were diagnosed with LT3S, excluding neonates with congenital malformations, other endocrine diseases, genetic diseases and inherited metabolic diseases. Results Five perinatal risk factors for LT3S were confirmed using univariate and multivariate analyses: smaller gestational age, lower birth weight, respiratory distress syndrome (RDS), neonatal sepsis, and dopamine use. Conclusions LT3S in preterm neonates was associated with multiple perinatal factors, including smaller gestational age, lower birth weight, RDS, sepsis, and dopamine use. Background Bone morphogenetic protein 9 (BMP9) has been identified as a crucial inducer of osteoblastic differentiation in mesenchymal stem cells (MSCs). Although microRNAs (miRNAs) are known to play a role in MSC osteogenesis, the mechanisms of action of miRNAs in BMP9-induced osteoblastic differentiation remain poorly understood. Methods In this study, we investigate the possible role of the miR17-92 cluster in the BMP9-induced osteogenic differentiation of MSCs by using both in vitro and in vivo bone formation assays. Results The results show that miR-17, a member of the miR17-92 cluster, significantly impairs BMP9-induced osteogenic differentiation. This impairment is effectively rescued by a miR-17 sponge, an antagomiR sequence against miR-17. Using TargetScan and the 3′-untranslated region luciferase reporter assays, we show that the direct target of miR-17 is the retinoblastoma gene (RB1), a gene that is pivotal to osteoblastic differentiation. We also confirm that RB1 is essential for the miR-17 effects on osteogenesis. Conclusion Our results indicate that miR-17 expression impairs normal osteogenesis by downregulating RB1 expression and significantly inhibiting the function of BMP9.

With the development of high-throughput sequencing technology, the posttranscriptional mechanism of alternative splicing is becoming better understood. From decades of studies, alternative splicing has been shown to occur in multiple tissues, including the brain, heart, testis, skeletal muscle, and liver. This regulatory mechanism plays an important role in physiological functions in most liver diseases. Currently, due to the absence of symptoms, chronic pediatric liver diseases have a significant impact on public health. Furthermore, the progression of the disease is accelerated in children, leading to severe damage to their liver tissue if no precautions are taken. To this end, this review article summarizes the current knowledge of alternative splicing in pediatric liver diseases, paying special attention to liver damage in the child stage. The discussion of the regulatory role of splicing in liver diseases and its potential as a new therapeutic target is also included.

Oesophageal liposarcomas are particularly rare, accounting for 1.2–1.5% of all gastrointestinal liposarcomas. Surgical resection is the usual treatment. Endoscopic resection is minimally invasive but still controversial. This current case report describes a rare case of a large oesophageal liposarcoma in a 52-year-old male that presented with 10-year history of dysphagia for dry and solid food that was exacerbated by a recent common cold. Thoracoscopic and laparoscopic oesophagectomy was performed. He did not have any dysphagia or dyspnoea 1 week postoperatively. The excised specimen consisted of a polypoid mass measuring 21.0 cm × 5.1 cm. Histological examination confirmed that it was an oesophageal liposarcoma. At 1-year postoperatively, there was no sign of recurrence. Thoracoscopy and laparoscopy can be used to treat large oesophageal masses. Long-term follow-up is required as oesophageal liposarcomas tend to recur.

Background: Hormone receptor positive (HR+)/human epidermal growth factor receptor 2 negative (Her2-) is the most prevalent subtype of breast cancer, representing 70% of the cases with metastatic disease. Endocrine therapy plus FDA-approved cyclin-dependent kinases (CDK) 4/6 inhibitors (palbociclib, ribociclib and abemaciclib) have improved the treatment of HR+/Her2- advanced breast cancer patients. However, patients develop clinical resistance (acquired) during the long-term treatment, and in some cases, they do not respond within the first 3 months of treatment (intrinsic resistance), leading to disease progression. Although these inhibitors have the same nominal targets, CDK4/6, abemaciclib can inhibit other kinases that are not inhibited by palbociclib/ribociclib, such as CDK9/7/2/1, GSK3α/β and CAMK2γ/δ. As such, abemaciclib has been shown to inhibit cell growth in Rb deficient cells, in which palbociclib/ribociclib are ineffective. Therefore, we hypothesized that 1) mechanisms driving palbociclib acquired resistance are distinct than those mediating abemaciclib resistance, 2) palbociclib resistant models may be responsive to abemaciclib, and 3) pathways underlying these mechanistic differences can be exploited as alternative therapies to overcome resistance. Methods: We generated several models to examine mechanisms of resistance to different CDK4/6 inhibitors: 1) MCF7 and T47D (ER+) palbociclib resistant breast cancer cells with increasing concentrations of palbociclib, starting at 1.2 µM (initial IC 50) up to 5 µM (plasma concentrations in patients) in a stepwise manner over a 6 month period, 2) MCF7 abemaciclib resistant cells (0.5, 1 and 1.5 µM), and 3) organoids derived from patient derived xenografts (PDXs) with a similar molecular profile to palbociclib acquired resistance models. Cell cycle changes were evaluated by western blot analysis and flow cytometry. Results: Western blot analysis revealed a dose dependent downregulation of ERα, Rb, p-Rb and p27, while levels of cyclin E and p-CDK2 increased in a stepwise fashion in palbociclib resistant cells, which were only partially cross resistant to abemaciclib and sensitive to CDK9 inhibitors LDC067 and SNS032. FACS analysis of abemaciclib resistant cells showed an increase in G2/M accompanied by a drastic reduction of p-Rb and a gradual decrease of Rb. ERα levels were slightly decreased in abemaciclib resistant cells compared to the complete loss in palbociclib resistant cells. As in the palbociclib resistant cells, a dose dependent increase in cyclin E and p-CDK2 were also observed. However, Rad51, a key mediator of DNA repair, was downregulated in a stepwise manner only in abemaciclib resistant cells, but not in palbociclib resistant cells. Therefore, we evaluated if a combination therapy targeting DNA repair pathways plus abemaciclib could render palbociclib resistant models more responsive to targets other than cell cycle inhibitors. To this end, we examined the combination of abemaciclib and niraparib in organoid cultures generated from PDX models that are transcriptomic surrogates of palbociclib resistance cells and found a significant reduction of viability, number and density of these organoids. In vivo, using the same PDX models, the combined treatment reduced the tumor growth rate and increased overall survival. Conclusion: Our results suggest that mechanisms underlaying palbociclib and abemaciclib acquired resistance exhibit differentially altered pathways, such as DNA damage/repair pathways, which can be exploited to overcome CDK4/6 inhibitors therapy resistance in breast cancer patients. The aim of the present study was to evaluate the microcirculation perfusion in patients with acute radiation-induced liver damage (RILD) and explore the feasibility of non-invasive evaluation of RILD using quantitative analysis of contrast-enhanced ultrasound (CEUS). Patients who successfully underwent three-dimensional conformal intensity-modulated radiotherapy for abdominal tumors were selected. CEUS was performed on the liver prior to and 2, 3 and 4 weeks after exposure, and the time-intensity curve (TIC) was obtained by quantitative analysis of CEUS. The spinal cord is composed of gray matter and white matter. It is well known that the properties of these two tissues differ considerably. Spinal diseases often present with symptoms that are caused by spinal cord compression. Understanding the mechanical properties of gray and white matter would allow us to gain a deep understanding of the injuries caused to the spinal cord and provide information on the pathological changes to these distinct tissues in several disorders. Previous studies have reported on the physical properties of gray and white matter, however, these were focused on longitudinal tension tests. Little is known about the differences between gray and white matter in terms of their response to compression. We therefore performed mechanical compression test of the gray and white matter of spinal cords harvested from cows and analyzed the differences between them in response to compression. We conducted compression testing of gray matter and white matter to detect possible differences in the collapse rate. We found that increased compression (especially more than 50% compression) resulted in more severe injuries to both the gray and white matter. The present results on the mechanical differences between gray and white matter in response to compression will be useful when interpreting findings from medical imaging in patients with spinal conditions.

Microlasers and waveguides have wide applications in the fields of photonics and optoelectronics. Lanthanide-doped luminescent materials featuring large Stokes/anti-Stokes shift, long excited-state lifetime as well as sharp emission bandwidth are excellent optical components for photonic applications. In the past few years, great progress has been made in the design and fabrication of lanthanide-based waveguides and lasers at the micrometer length scale. Waveguide structures and microcavities can be fabricated from lanthanide-doped amorphous materials through top-down process. Alternatively, lanthanide-doped organic compounds featuring large absorption cross-section can self-assemble into low-dimensional structures of well-defined size and morphology. In recent years, lanthanide-doped crystalline structures displaying highly tunable excitation and emission properties have emerged as promising waveguide and lasing materials, which substantially extends the range of lasing wavelength. In this minireview, we discuss recent advances in lanthanide-based luminescent materials that are designed for waveguide and lasing applications. We also attempt to highlight challenging problems of these materials that obstacle further development of this field.

The present study was designed to evaluate the dynamic survival and recurrence of remnant gastric cancer (RGC) after radical resection and to provide a reference for the development of personalized follow‐up strategies. A total of 298 patients were analyzed for their 3‐year conditional overall survival (COS3), 3‐year conditional disease‐specific survival (CDSS3), corresponding recurrence and pattern changes, and associated risk factors. The 5‐year overall survival (OS) and the 5‐year disease‐specific survival (DSS) of the entire cohort were 41.2% and 45.8%, respectively. The COS3 and CDDS3 of RGC patients who survived for 5 years were 84.0% and 89.8%, respectively. The conditional survival in patients with unfavorable prognostic characteristics showed greater growth over time than in those with favorable prognostic characteristics (eg, COS3, ≥T3: 46.4%‐83.0%, Δ36.6% vs ≤T2: 82.4%‐85.7%, Δ3.3%; P 0.05). Time‐dependent logistic regression analysis showed that the AJCC stage independently affected recurrence within 2 years after surgery (P, Patients with RGC usually have a high likelihood of death or recurrence within 3 years after radical surgery. We developed a postoperative follow‐up model for RGC patients of different stages.

Study design: A systematic review and meta-analysis. Objective: The objective of this meta-analysis was to evaluate the association between calcium-phosphorus balance and adolescent idiopathic scoliosis (AIS). Methods: Databases, including PubMed, OVID database, Web of Science, CBM database and CNKI database were searched for the relevant case control studies and cross-sectional studies. Two authors selected studies and extracted data independently. Data analysis was performed by Review Manager Software 5.0. Subgroup analysis was performed on the serum level of vitamin D according to gender and menstruation. Results: Five studies were included, with a total of 646 cases of AIS and 791 controls. AIS group had a lower serum level of vitamin D compared to control group [MD = −6.74, 95% CI (−9.47, −4.00)]. Gender and menstruation condition were thought to have no effect on the primary outcome of vitamin D level by subgroup analysis [MD = −5.97, 95% CI (7.61, −4.34)]. The AIS group had a lower calcium level [SMD= −0.77, 95% CI (−1.51, −0.02)] and calcitonin level compared to control group. There was no statistical difference in phosphorus level [SMD=0.5, 95% CI (−0.46, 0.57)] and parathyroid hormone level [SMD = −0.11, 95% CI (−0.54, −0.31)]. Meanwhile, the observational indexes, including serum levels of calcium, phosphorus, parathyroid hormone and calcitonin were within normal limits. Conclusion: Vitamin D deficiency may be involved in the pathogenesis of AIS by influencing the regulation of calcium-phosphors metabolism on human bone. Residual cancer after incomplete ablation remains a major problem for radiofrequency ablation (RFA). We aimed to investigate the synergetic treatment efficacy of RFA combined with ultrasonic cavitation for liver tumor. Sixty rabbits with VX2 liver tumor were randomly divided into three groups. Group A was control group without any treatment. Combined ultrasonic cavitation with RFA was performed for group B1. Group B2 underwent RFA alone. The histopathological results were compared at the 5th, 11th, and 18th day of experiment, and the survival time and metastasis were assessed. The tumor volume growth rate, percentage of necrosis area, microvessel density, and apoptosis index showed significant differences among these groups at the 5th day, 11th day, and 18th day of experiment (P < .05). In contrast, the difference of metastatic score was not significant at the 5th and 11th day (P > .05). At the 18th day, the metastatic score of group A was significant higher than that of group B1 (P < .05), whereas the differences between group A and group B2, or group B1 and group B2 were not significant (P > .05). The median/range interquartile of survival time in groups A, B1, and B2 were 25/8 days, 50/19 days, and 48/20 days, respectively, and there was significant difference between groups A and B1 or B2 (P < .05). The difference between groups B1 and B2 was not significant (P > .05). Ultrasonic cavitation after incomplete RFA for liver tumor improved the antitumor effect, which could be considered as a potentially useful combined therapeutic strategy for liver malignancy. Evaluate environmental load of the agriculture industry by taking Chongming Island of Shanghai as a case study. Propose feasible approaches to reduce the environmental impact of agriculture industry based on the research findings. The research boundary of our study covers from raw material extraction to agriculture product (cradle to product). Two stages are identified based on life cycle thinking: agricultural material production and planting process. In the agriculture material stage, we evaluated the consumption of agricultural machinery and equipment for diesel, gasoline and electricity; then, the embodied environmental impact of these energy sources are calculated based on life cycle assessment (LCA). For the planting stage, we calculated the environmental impact caused by agriculture sowing, fertilizing, and spraying of pesticides also according to the LCA method. The environmental impacts of Chongming Island in 2015 are found as ecosystem damage (2.44E+11 species year), human health (2.86E+10 DALY), and resources consumption (1.15E+09 $). The primary environmental impacts in the agricultural material production stage are revealed as marine ecological toxicity (1.01E+06), freshwater eutrophication (5.26E+05), and human toxicity (4.04E+05), of which 90% of the impacts are caused by the production of nitrogen fertilizer, phosphorus fertilizer, and potassium fertilizer. The main environmental impacts in the agricultural planting stage are freshwater ecotoxicity (1.27E+06), freshwater eutrophication (7.70E+05), and terrestrial ecotoxicity (7.70E+05), which are mainly caused by pesticide residues and loss of nitrogen fertilizer, phosphorus fertilizer, and potassium fertilizer. The results of the environmental impact analysis of the Chongming agricultural production of 2008–2015 showed that there was a significant decline trend in 2011 and 2012, mainly due to the significant reduction in the use of nitrogen fertilizer. The main measures for the practitioner to reduce the potential ecological damage to agricultural production are controlling the N, P, and K fertilizer application and pesticide spray volume. In addition, the ecological environment can be improved by exploring techniques and measures for efficient utilization of agricultural resources such as optimizing the amount of fertilizer and application rate.

PURPOSE: CDK4/6 inhibitors are currently used in combination with endocrine therapy to treat advanced hormone receptor-positive, HER2-negative breast cancer. While this treatment doubles time to progression compared to endocrine therapy alone, about 25–35% of patients do not respond, and almost all patients eventually acquire resistance. Discerning the mechanisms of resistance to CDK4/6 inhibition is crucial in devising alternative treatment strategies. EXPERIMENTAL DESIGN: Palbociclib resistant cells (MCF-7 and T47D) were generated in a step-wise dose-escalading fashion. Whole-exome sequencing, genome-wide expression analysis and proteomic analysis were performed in both resistant and parental (sensitive) cells. Pathway alteration was assessed mechanistically and pharmacologically. Biomarkers of altered pathways were examined in tumor samples from palbociclib treated breast cancer patients whose disease progressed while on treatment. RESULTS: Palbociclib resistant cells are cross resistant to other CDK4/6 inhibitors and are also resistant to endocrine therapy. IL-6/STAT3 pathway is induced while DNA-repair pathways are downregulated in the resistant cells. Combined inhibition of STAT3 and PARP significantly increased cell death in the resistant cells. Matched tumor samples from breast cancer patients who progressed on palbociclib were examined for deregulation of estrogen receptor, DNA repair, and IL-6/STAT3 signaling and results revealed that these pathways are all altered as compared to the pre-treatment tumor samples. CONCLUSION: Palbociclib resistance induces endocrine resistance and alteration of IL-6/STAT3 and DNA damage response pathways in cell lines and patient samples. Targeting IL-6/STAT3 activity and DNA repair deficiency using a specific STAT3 inhibitor combined with a PARP inhibitor could effectively treat acquired resistance to palbociclib. TRANSLATIONAL RELEVANCE: The majority of breast cancer deaths are due to progression of metastatic ER-positive disease. Identification of targetable biomarkers to predict treatment strategies to circumvent resistance to CDK4/6 class of inhibitors which are currently used in combination with endocrine therapy in ER-positive metastatic breast cancer patients will be instrumental in improving survival. We show that ER-positive breast cancer cells acquire resistance to palbociclib (CDK4/6 inhibitor) by downregulation of ER protein and DNA repair machinery and upregulation of IL-6/STAT3 pathway, which is overcome by treatment with STAT3 and PARP inhibitors. Matched biopsies from breast cancer patients who progressed on palbociclib showed deregulation in DNA repair, ER and IL-6/STAT3 as compared to their pre-treatment biopsy samples. By identifying and validating these mediators (or drivers) of palbociclib resistance, we propose that patients who progress on palbociclib can be targeted using clinically available inhibitors to STAT3 and DNA repair to circumvent resistance and improve clinical outcomes.

We demonstrate a simple and direct method to precisely characterize the optical beam shear angle of polarizing prisms. Applying nanometer localization analysis for determining the central positions of the two sheared spots with orthogonal polarizations, we show that it allows for resolving the shear angle beyond the optical diffraction limit. We use Nomarski prisms to verify our technique and obtain a resolution below 0.5 μ rad. Our method provides a stand-alone characterization of polarizing prisms with a high accuracy for many quantitative imaging technologies such as differential interference contrast microscopy.

Myocardial fibrosis detected by cardiac magnetic resonance (CMR) has been reported in patients with desmin‐related myopathy, although its characteristics remain unclear. Here, we describe a case of desmin‐related restrictive cardiomyopathy wherein CMR imaging revealed myocardial oedema, ischaemia, and fibrosis in the left ventricle; the different types and processes of myocardial injury were detected by CMR. Middle wall left ventricular enhancement may be a feature of late gadolinium enhancement, and the lateral wall is often involved in cases of myocardial injury. CMR is useful for the early detection of cardiac involvement and the prediction of prognosis in patients diagnosed with desmin‐related myopathy.

Background: The current study aimed to investigate the effects of synaptotagmin-like 3 (SYTL3) and solute carrier family 22 member 3 (SLC22A3) single nucleotide polymorphisms (SNPs) and gene-environment (G × E) interactions on blood lipid levels as well as the risk of coronary artery disease (CAD) and ischaemic stroke (IS) in the Southern Chinese Han population.Methods: The genetic makeup of 6 SYTL3-SLC22A3 SNPs in 2269 unrelated participants (controls, 755; CAD, 758 and IS, 756) of Chinese Han ethnicity was detected by the next-generation sequencing techniques.Results: The allele and genotype frequencies of the SYTL3 rs2129209 and SLC22A3 rs539298 SNPs were significantly different between the case and control groups. The SLC22A3 rs539298 SNP was correlated with total cholesterol (TC) levels in controls, the rs539298G allele carriers maintained lower TC levels than the rs539298G allele non-carriers. At the same time, the SLC22A3 rs539298 SNP interacted with alcohol consumption reduced the risk of CAD and IS. The SYTL3-SLC22A3 A-C-A-A-A-A, G-T-C-G-C-A and A-T-A-A-C-A haplotypes increased and the A-C-A-A-C-G haplotype reduced the risk of CAD, whereas the SYTL3-SLC22A3 A-C-A-A-A-A, G-T-C-G-A-G and A-T-A-A-C-A haplotypes increased and the A-C-A-A-A-G and A-C-A-A-C-G haplotypes reduced the risk of IS. In addition, several SNPs interacted with alcohol consumption, body mass index ≥ 24 kg/m2 and cigarette smoking to affect serum lipid parameters such as triglyceride, high-density lipoprotein cholesterol, TC, and apolipoprotein A1 levels.Conclusions: Several SYTL3-SLC22A3 variants, especially the rs539298 SNP, several haplotypes, and G × E interactions, were related to blood lipid parameters and the risk of CAD and IS in the Southern Chinese Han population.

Background Understanding the long-term effects of coronavirus disease 2019 (COVID-19) on cognitive function is essential for monitoring the cognitive decline in the elderly population. This study aims to assess the current cognitive status and the longitudinal cognitive decline in elderly patients recovered from COVID-19. Methods This cross-sectional study recruited 1539 COVID-19 inpatients aged over 60 years who were discharged from three COVID-19-designated hospitals in Wuhan, China, from February 10 to April 10, 2020. In total, 466 uninfected spouses of COVID-19 patients were selected as controls. The current cognitive status was assessed using a Chinese version of the Telephone Interview of Cognitive Status-40 (TICS-40) and the longitudinal cognitive decline was assessed using an Informant Questionnaire on Cognitive Decline in the Elderly (IQCODE). Cognitive assessments were performed 6 months after patient discharge. Results Compared with controls, COVID-19 patients had lower TICS-40 scores and higher IQCODE scores [TICS-40 median (IQR): 29 (25 to 32) vs. 30 (26 to 33), p p p p p p p p p p Conclusions Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection is associated with an increased risk of long-term cognitive decline in elderly population. COVID-19 patients, especially severe patients, should be intensively monitored for post-infection cognitive decline.

Traditional diagnosis technology on earlier detection of some deadly liver diseases has many disadvantages. These shortcomings are due mainly to inadequate accuracy, which usually leads to failing to give liver patients timely treatment. In order to solve this problem, this paper used Classification and Regression Tree (CART) as a weak classifier of the AdaBoost framework to propose a Classification and Regression Tree-Adaptive Boosting (CART-AdaBoost) model. Moreover, the authors trained and verified the model basing on the Indian Liver Patient Dataset (ILPD) of UCI. The results showed that the model's accuracy was 83.06%, and its precision was 84.31%. Besides, F1-score could reach 80.75%, and the recall metric was 77.48%. All the former three indicators were higher than those produced by single models or combination models (weak classifier + AdaBoost) listed in this paper. Besides, it is worth noting that the prediction accuracy and precision of the CART-AdaBoost model were improved by a maximum value of 18.60% and 23.84%, respectively. Therefore, the suggested model is of great benefit in enhancing the early detection effect of liver diseases.
