1. Neutrophils contribute to ER stress in lung epithelial cells in the pristane-induced diffuse alveolar hemorrhage mouse model

Abstract

Diffuse alveolar hemorrhage (DAH), although rare, is a life-threatening complication of systemic lupus erythematosus (SLE). Little is known about the pathophysiology of DAH in humans, although increasingly neutrophils, NETosis and inflammatory monocytes have been shown to play an important role in the pristane-induced model of SLE which develops lung hemorrhage and recapitulates many of the pathologic features of human DAH. Using this experimental model, we asked whether endoplasmic reticulum (ER) stress played a role in driving the pathology of pulmonary hemorrhage and what role infiltrating neutrophils had in this process. Analysis of lung tissue from pristane-treated mice showed genes associated with ER stress and NETosis were increased in a time-dependent manner and reflected the timing of CD11b+Ly6G+ neutrophil accumulation in the lung. Using precision cut lung slices from untreated mice we observed that neutrophils isolated from the peritoneal cavity of pristane-treated mice could directly induce the expression of genes associated with ER stress, namely Chop and Bip. Mice which had myeloid-specific deletion of PAD4 were generated and treated with pristane to assess the involvement of PAD4 and PAD4-dependent NET formation in pristane-induced lung inflammation. Specific deletion of PAD4 in myeloid cells resulted in decreased expression of ER stress genes in the pristane model, with accompanying reduction in IFN-driven genes and pathology. Lastly, coculture experiments of human neutrophils and human lung epithelial cell line (BEAS-2b) showed neutrophils from SLE patients induced significantly more ER stress and interferon-stimulated genes in epithelial cells compared to healthy control neutrophils. These results support a pathogenic role of neutrophils and NETs in lung injury during pristane-induced DAH through the induction of ER stress response and suggest that overactivation of neutrophils in SLE and NETosis may underlie development of DAH.

Abstract

BACKGROUND: Empagliflozin slowed the progression of CKD in patients with type 2 diabetes and cardiovascular disease in the EMPA-REG OUTCOME Trial. In a prespecified statistical approach, we assessed treatment differences in kidney function by analyzing slopes of eGFR changes.METHODS: Participants (n=7020) were randomized (1:1:1) to empagliflozin 10 mg/d, empagliflozin 25 mg/d, or placebo added to standard of care. We calculated eGFR slopes using random-intercept/random-coefficient models for prespecified study periods: treatment initiation (baseline to week 4), chronic maintenance treatment (week 4 to last value on treatment), and post-treatment (last value on treatment to follow-up).RESULTS: Compared with placebo, empagliflozin was associated with uniform shifts in individual eGFR slopes across all periods. On treatment initiation, adjusted mean slope (eGFR change per week, ml/min per 1.73 m2) decreased with empagliflozin (-0.77; 95% confidence interval, -0.83 to -0.71; placebo: 0.01; 95% confidence interval, -0.08 to 0.10; PCONCLUSIONS: The hemodynamic effects of empagliflozin, associated with reduction in intraglomerular pressure, may contribute to long-term preservation of kidney function.

Abstract

Nasal obstruction (NO) is defined as the subjective perception of discomfort or difficulty in the passage of air through the nostrils. It is a common reason for consultation in primary and specialized care and may affect up to 30%-40% of the population. It affects quality of life (especially sleep) and lowers work efficiency. The aim of this document is to agree on how to treat NO, establish a methodology for evaluating and diagnosing it, and define an individualized approach to its treatment. NO can be unilateral or bilateral, intermittent or persistent and may be caused by local or systemic factors, which may be anatomical, inflammatory, neurological, hormonal, functional, environmental, or pharmacological in origin. Directed study of the medical history and physical examination are key for diagnosing the specific cause. NO may be evaluated using subjective assessment tools (visual analog scale, symptom score, standardized questionnaires) or by objective estimation (active anterior rhinomanometry, acoustic rhinometry, peak nasal inspiratory flow). Although there is little correlation between the results, they may be considered complementary and not exclusive. Assessing the impact on quality of life through questionnaires standardized according to the underlying disease is also advisable. NO is treated according to its cause. Treatment is fundamentally pharmacological (topical and/or systemic) when the etiology is inflammatory or functional. Surgery may be necessary when medical treatment fails to complement or improve medical treatment or when other therapeutic approaches are not possible. Combinations of surgical techniques and medical treatment may be necessary.

Supplemental Digital Content is available in the text., Background and Purpose— In the EMPA-REG OUTCOME trial (Empagliflozin Cardiovascular Outcome Event Trial in Type 2 Diabetes Mellitus Patients), empagliflozin added to standard of care in patients with type 2 diabetes mellitus and high cardiovascular risk reduced the risk of 3-point major adverse cardiovascular events, driven by a reduction in cardiovascular mortality, with no significant difference between empagliflozin and placebo in risk of myocardial infarction or stroke. In a modified intent-to-treat analysis, the hazard ratio for stroke was 1.18 (95% confidence interval, 0.89–1.56; P=0.26). We further investigated cerebrovascular events. Methods— Patients were randomized to empagliflozin 10 mg, empagliflozin 25 mg, or placebo; 7020 patients were treated. Median observation time was 3.1 years. Results— The numeric difference in stroke between empagliflozin and placebo in the modified intent-to-treat analysis was primarily because of 18 patients in the empagliflozin group with a first event >90 days after last intake of study drug (versus 3 on placebo). In a sensitivity analysis based on events during treatment or ≤90 days after last dose of drug, the hazard ratio for stroke with empagliflozin versus placebo was 1.08 (95% confidence interval, 0.81–1.45; P=0.60). There were no differences in risk of recurrent, fatal, or disabling strokes, or transient ischemic attack, with empagliflozin versus placebo. Patients with the largest increases in hematocrit or largest decreases in systolic blood pressure did not have an increased risk of stroke. Conclusions— In patients with type 2 diabetes mellitus and high cardiovascular risk, there was no significant difference in the risk of cerebrovascular events with empagliflozin versus placebo. Abstract

Optical diffraction fields have a not easy spatial structure, some times optical diffraction fields can generate a focusing region or caustic region, in this contribution, we describe the focusing region associated with highly symmetric transmittances, we analyze its associated phase function and show that generic features can be studied from a differential equation for a focusing geometry, which is obtained through angular representation for diffraction fields, the diffraction field presents focusing region whose geometry and spatial evolution can be described with the analysis of the phase singularities avoiding the integral representation. We show that in general the diffracted field has a decomposition in optical fields propagating along three optical axis mutually perpendicular. The decomposition is in terms of the Pearcey and Airy functions and generalized Airy function. Experimental results are shown.

Abstract

Aims We previously reported that in the EMPA-REG OUTCOME(R) trial, empagliflozin added to standard of care reduced the risk of 3-point major adverse cardiovascular events, cardiovascular and all-cause death, and hospitalization for heart failure in patients with type 2 diabetes and high cardiovascular risk. We have now further investigated heart failure outcomes in all patients and in subgroups, including patients with or without baseline heart failure. Methods and results Patients were randomized to receive empagliflozin 10 mg, empagliflozin 25 mg, or placebo. Seven thousand and twenty patients were treated; 706 (10.1%) had heart failure at baseline. Heart failure hospitalization or cardiovascular death occurred in a significantly lower percentage of patients treated with empagliflozin [265/4687 patients (5.7%)] than with placebo [198/2333 patients (8.5%)] [hazard ratio, HR: 0.66 (95% confidence interval: 0.55-0.79); P < 0.001], corresponding to a number needed to treat to prevent one heart failure hospitalization or cardiovascular death of 35 over 3 years. Consistent effects of empagliflozin were observed across subgroups defined by baseline characteristics, including patients with vs. without heart failure, and across categories of medications to treat diabetes and/or heart failure. Empagliflozin improved other heart failure outcomes, including hospitalization for or death from heart failure [2.8 vs. 4.5%; HR: 0.61 (0.47-0.79); P < 0.001] and was associated with a reduction in all-cause hospitalization [36.8 vs. 39.6%; HR: 0.89 (0.82-0.96); P = 0.003]. Serious adverse events and adverse events leading to discontinuation were reported by a higher proportion of patients with vs. without heart failure at baseline in both treatment groups, but were no more common with empagliflozin than with placebo. Conclusion In patients with type 2 diabetes and high cardiovascular risk, empagliflozin reduced heart failure hospitalization and cardiovascular death, with a consistent benefit in patients with and without baseline heart failure., This work was supported by Boehringer Ingelheim and Eli Lilly and Company. Abstract

We analyze the diffraction field generated by coherent illumination of a three-dimensional transmittance characterized by a slit-shape curve. Generic features are obtained using the Frenet-Serret equations, which allow a decomposition of the optical field. The analysis is performed by describing the influence of the curvature and torsion on osculating, normal, and rectifying planes. We show that the diffracted field has a decomposition in three optical fields propagating along three optical axes that are mutually perpendicular. The decomposition is in terms of the Pearcey and Airy functions, and the generalized Airy function. Experimental results are shown.

Abstract

Birdsfoot trefoil (Lotus corniculatus L.) is a highly variable and widely distributed Old-World perennial forage legume found in wild and naturalized populations throughout temperate regions of Europe, Asia Minor, North Africa, North and South America, Australia, and New Zealand. Understanding the relationships among birdsfoot trefoil morphologic, ecogeographic, and genetic characteristics may provide insights for better utilizing exotic germplasm. The objectives of this research were to (i) compare morphologic and random amplified polymorphic DNA (RAPD) classifications of 28 exotic and ecologically diverse genotypes from the USDA National Plant Germplasm System (NPGS) birdsfoot trefoil collection, and (ii) determine the relationships between genotype classifications and collecting-site ecogeographic features. Eighteen morphologic characteristics, 130 RAPD bands, and eight collecting-site ecogeographic characteristics were used to classify the genotypes. The relatedness of genetic, morphologic, ecologic, and geographic distances among the genotypes was measured using the product moment correlation. Genotype morphology was related to collecting-site distances from one another and ecologic similarity. Genetic relatedness was also associated with collecting-site ecology, and specific morphologic characteristics were associated with different ecogeographic features. The similarity between the genetic and ecologic classifications suggested that genotypes adapted to similar habitats, even if geographically distant, have acquired similar phenotypes. Birdsfoot trefoil (Lotus corniculatus L.) is a widely distributed polymorphic Old-World perennial forage legume found in wild and naturalized populations throughout temperate regions of Europe, Asia Minor, North Africa, and North and South America. Exotic birdsfoot trefoil germplasm has rarely been used for birdsfoot trefoil genetic enhancement, and information about its crossing ability with other exotics and commercial quality germplasm is not available. The objectives of this research were to (i) characterize the crossing ability of 27 exotic birdsfoot trefoil genotypes with two genetically diverse hybridization testers, and (ii) determine if crossing ability among genotypes was related to their genetic background measured by random amplified polymorphic DNA (RAPD) markers and their ecogeographic origins. Crossing ability was determined using reciprocal crosses with one commercial-quality germplasm and one exotic genotype tester. All possible crossing combinations for an eight-genotype subset were also determined. Crossing ability was measured as the percentage of pollinated flowers that set pods, [F.sub.1] progeny pollen viability, pod length, and seeds per pod. Self-genotype pod set and pollen viability were not correlated. Intermediate bridge crosses were identified that could potentially overcome specific cross incompatibilities and be used to obtain progeny for any combination of genotypes. Genotype-crossing ability was associated with ecogeographic features of the collecting site, but not with morphologic characteristics. This differs from findings that other genotype morphologic characteristics are associated with ecogeographic origins and genetic similarities based on RAPD markers. Abstract

We describe the interaction between focusing regions and analyze the resulting physical features. The study is supported by the fact that focusing regions exhibit particle-like behavior and the interaction among these presents features similar to inelastic collision. When focusing regions are generated in a medium with random refractive index, the irradiance distribution along the focusing regions changes according to the diffusion process and the collision between them generates vortex-like effects. This study was implemented by solving the irradiance transport equation. Experimental results are in good agreement with the theoretical model performed.
