Primary spinal cord melanoma is a rare disease that accounts for only 1% of all melanocytomas. Here we report a case of primary melanoma of the cervical spinal cord. In our case, 26-year-old female who were admitted to the hospital for left arm pain. Spinal magnetic resonance image (MRI) revealed a spinal cord tumor at the level of C2-3. The MRI images showed that the tumor compressed the spinal cord. At surgery, the spinal cord was under pressure and covered with shaped blackish brown neoplastic tissue. There were not any metastatic lesions. The patient is still alive six months after surgery. 33581992

Introduction Indocyanine green (ICG) is commonly used to visualize cerebral vasculature, particularly in the management of cerebral aneurysms. There have also been attempts to use ICG for visualization of tumors. Injection of ICG followed by immediate fluorescence microscopy is limited by the short time window for imaging and administration and restricted depth of imaging. Second Window Indocyanine Green (SWIG) addresses these issues by allowing for longer contrast times and the imaging of deeper regions of brain tissue. Biopsy of spinal cord lesions is often difficult for a variety of reasons, including the delicate nature of the tissue and differentiating normal from lesional tissue visually, especially in lesions with heterogeneous enhancement. Methods In this case report, we describe the use of second window ICG to facilitate the visualization of a spinal cord lesion and subsequent biopsy of the lesion. Results This patient is a 24-year-old female who had recurrence of a suprasellar germinoma. An MRI of the rest of the neuraxis was performed to assess for the presence of drop metastases. The spinal cord from C2-5 was expanded with areas of patchy enhancement; however, this lesion was asymptomatic. The patient's oncologist requested a biopsy of this lesion to help direct subsequent care of her recurrent germinoma. The day before surgery, the patient had an intravenous injection of ICG dye. She then underwent a C3-5 laminectomy for biopsy of her cervical intramedullary lesion. After opening of the dura, no visible abnormality of the spinal cord could be identified. A Stryker endoscope showed an area of ICG uptake in the cord at approximately the C3-4 level. A midline myelotomy was centered over the ICG demarcated area and several samples were taken for pathology. Final biopsy results determined the lesion to be spinal cord parenchyma with perivascular and intraparenchymal lymphocytes – not consistent with spinal cord tumor or germinoma. Conclusion Second Window ICG is effective in visualizing otherwise visually unremarkable spinal cord lesions. This technology can facilitate biopsy of these lesions and possibly their surgical resection.

Introduction: Kyphotic deformity is common after spinal tumor resection surgery. An adequate field of view is needed to resect the spinal cord tumor, and, in some cases, the facet joint must be removed during laminectomy, and fixation may be performed simultaneously. In this study, we investigated the incidence of postoperative deformity after spinal tumor resection and the factors associated with postoperative deformity. Methods: We retrospectively analyzed patients who underwent thoracic spinal cord tumor resection at a single institution between 2010 and 2017 and were followed up for at least 24 months after surgery. Fifty percent or more of the facet joint was removed during the laminectomy, and fixation was performed simultaneously. Patients were divided into two groups, with and without kyphotic deformity. Patient demographic characteristics (age at surgery and gender), whether they underwent primary surgery or reoperation, tumor level, pathological type, and surgical method were compared. Multiple linear regression analysis was performed to identify independent predictors of kyphotic deformity. Results: Thirty-one patients were found to be eligible. Thirteen patients had intramedullary spinal cord tumors. Laminectomy was performed in 52% (N=16), laminoplasty in 6% (N=2), and laminectomy and/or laminoplasty combined with fusion in 42% (N=13) of the patients. During a mean follow-up period of 66.8 months, 12 (39%) patients had postoperative kyphosis deformities, of which one patient (3%, a 12-year-old girl who underwent combined postoperative radiation therapy) underwent kyphosis correction surgery three years after surgery. The number of laminectomies was independently associated with kyphotic deformity. Conclusions: Although kyphotic deformity after spinal tumor surgery was observed in about 39% of the patients, corrective surgery was rarely required due to the progression of the deformity. The high number of laminectomies is a risk factor for postoperative kyphosis, and prophylactic fixation may be considered in cases of multiple laminectomies.

Introduction Primary spinal cord tumors are amenable to total surgical excision but the outcomes are still diverse. Objective The purpose of this study is to identify the significant factors which influence the long-term outcome of spinal cord tumors. Materials and Methods A total of 84 consecutive spinal cord tumor patients who underwent microsurgical excision during April 2003 to 2014 were retrospectively studied. Patients operated after April 2014 were not considered. Ten factors were taken and correlated with the outcome. Multiple regression analysis was used. Patients were periodically evaluated at 1, 6 months, 1, 2, and 5 years. In 72 cases, postoperative MRI scans were performed. The follow-up period ranged between 5 to 15 years. Results In our study, we had 64 extramedullary tumors and 21 intramedullary tumors with varied pathology. Axial location of the tumour (20.294/0.000), extent of the resection of the tumour (13.827/0.001), preoperative Nurick grade (11.349/0.023), and location of the tumour in the spine with respect to vertebral segments (8.151/0.017) were significant predictive factors. We had good outcomes in 65 cases and poor outcomes in 19 cases. Conclusion Our results show location of the tumor with respect to axial plane and vertebral segments were the main contributing factors. They influence the extent of excision of the spinal cord tumor. Preoperative neurological status is the fourth factor which independently determines the long-term outcome. It is one of the few papers where patients have been followed-up for a long period ranging from 5 to 15 years.

Few cases of malignant transformation of supposedly low-grade gliomas were described in patients with neurofibromatosis type 1 (NF1). A 27-year-old man with NF1 presented with weakness of his lower extremities and was radiologically found to have a spinal intramedullary tumor primarily involving the Th11 level. The tumor histologically demonstrated features diagnosed as a low-grade astrocytoma, subtype indeterminate (WHO grade II). Immunohistochemically, GFAP was positive, and IDH1 R132H and BRAF V600E were negative. ATRX immunoreactivity was retained. Five years after the surgery, the intramedullary tumor extended to the levels from Th8 to L1 and was partially resected. It showed histologic features similar to those of the first tumor. Two years after the second surgery, the residual spinal cord tumor was found to widely involve the levels from Th5 to L3. Spinal cordectomy was subsequently undertaken and revealed anaplastic glial cells infiltrating diffusely into the spinal cord parenchyma, with prominent subarachnoid spreading and nerve root involvement. Both necrosis and microvascular proliferation were observed. This recurrent tumor was histologically indistinguishable from glioblastoma. Loss of ATRX was noted in the second and third surgical specimens. This is the first histologically proven case of malignant transformation of NF1-associated astrocytoma with ATRX loss during the course.

Study Design: Multicenter prospective study. Objectives: Although intramedullary spinal cord tumor (IMSCT) and extramedullary SCT (EMSCT) surgeries carry high risk of intraoperative motor deficits (MDs), the benefits of transcranial motor evoked potential (TcMEP) monitoring are well-accepted; however, comparisons have not yet been conducted. This study aimed to clarify the efficacy of TcMEP monitoring during IMSCT and EMSCT resection surgeries. Methods: We prospectively reviewed TcMEP monitoring data of 81 consecutive IMSCT and 347 EMSCT patients. We compared the efficacy of interventions based on TcMEP alerts in the IMSCT and EMSCT groups. We defined our alert point as a TcMEP amplitude reduction of ≥70% from baseline. Results: In the IMSCT group, TcMEP monitoring revealed 20 true-positive (25%), 8 rescue (10%; rescue rate 29%), 10 false-positive, a false-negative, and 41 true-negative patients, resulting in a sensitivity of 95% and a specificity of 80%. In the EMSCT group, TcMEP monitoring revealed 20 true-positive (6%), 24 rescue (7%; rescue rate 55%), 29 false-positive, 2 false-negative, and 263 true-negative patients, resulting in a sensitivity of 91% and specificity of 90%. The most common TcMEP alert timing was during tumor resection (96% vs. 91%), and suspension surgeries with or without intravenous steroid administration were performed as intervention techniques. Conclusions: Postoperative MD rates in IMSCT and EMSCT surgeries using TcMEP monitoring were 25% and 6%, and rescue rates were 29% and 55%. We believe that the usage of TcMEP monitoring and appropriate intervention techniques during SCT surgeries might have predicted and prevented the occurrence of intraoperative MDs.

Objectives The spinal dermoid and epidermoid cysts (SDECs) are rare entities comprising less than 1% of pediatric intraspinal tumors. The present study aims to extrapolate the clinicoradiological data, in order to identify the most plausible neural tube closure model in human and provide a retrospective representation from our clinical experience. Materials and Methods We collected the details of all histologically proven, newly diagnosed primary SDECs who underwent excision over the past 20 years. Secondary or recurrent lesions and other spinal cord tumors were excluded. Surgical and follow-up details of these patients as well as those with associated spinal dysraphism were reviewed. Clinical and radiological follow-up revealed the recurrence in these inborn spinal cord disorders. Results A total of 73 patients were included retrospectively, having a mean age of 22.4 ± 13.3 years, and 41 (56.2%) cases fell in the first two decades of life. Twenty-four (32.9%) dermoid and 49 (67.1%) epidermoid cysts comprised the study population and 20 of them had associated spinal dysraphism. The distribution of SDECs was the most common in lumbosacral region (n = 30) which was 10 times more common than in the sacral region (n = 3). Bladder dysfunction 50 (68.5%) and pain 48 (65.7%) were the most common presenting complaints. During follow-up visits, 40/48 (83.3%) cases showed sensory improvement while 11/16 (68.7%) regained normal bowel function. There was no surgical mortality with recurrence seen in eight till the last follow-up. Conclusions The protracted clinical course of the spinal inclusion cysts mandates a long-term follow-up. The results of our study support the multisite closure model and attempt to provide a retrospective reflection of neural tube closure model in humans by using SDECs as the surrogate marker of neural tube closure defect.

Intramedullary spinal cord metastasis (ISCM) is a rare event in the course of advanced malignancy. Management of these lesions remains controversial. Recently, surgery for ISCM has been advocated for selected patients. We performed a retrospective analysis of the clinical course, complications, and outcome of 30 patients surgically treated for ISCM. Patients' age, histopathological diagnoses of primary cancer, tumor size, spinal location, and extramedullary tumor dissemination were collected. Preoperative functional status, pre- and postoperative neurological status, and extent of the tumor resection were also analyzed. Predominant tumor location was thoracic, followed by cervical and conus medullaris. Lung cancer constituted the majority of primary malignancies. In 9 cases, one of the indications for spinal surgery was to obtain a histopathological diagnosis. On admission, all patients presented with neurological symptoms suggestive of myelopathy. After surgery, 18 patients exhibited improvement of symptoms in terms of pain relief and partial recovery of motor and/or sensory deficits; 6 patients were unchanged, while 6 patients exhibited postoperative deterioration. Median survival time after surgery was 9.9 months. Age > 70 years old, presence of systemic metastases, preoperative neurological non functional status, and lung cancer as primary tumor were all factors associated with a worse survival prognosis. This study did not show a clear survival difference between gross total and subtotal ISCM tumor resection. Patients who underwent gross total resection had a worse functional outcome with respect to patients with only partial resection. Gross total resection with low morbidity must be the surgical target, but when not possible, subtotal resection and adjuvant therapy are a valid therapeutic option.

Objectives The aim of this study was to evaluate the value and long-term effect of laminectomy or laminoplasty in spinal cord tumor surgery. Patients and methods Patients with spinal cord tumor treated in Department of Neurosurgery from January 2016 to October 2019 were included in this study. Posterior median approach tumor resection was preceded in 94 cases. Vertebral plate and ligament composite replant (laminoplasty group) was proceeded in 34 cases, and vertebral plate resection (laminectomy group) was proceeded in 60 cases. All patients were followed up and neurological function imagings were conducted 1 week, 3 months, and 6 months postsurgery to evaluate the surgical efficiency and spinal stability. Results Total resection was achieved in 84 patients (89.0%); subtotal resection was achieved in 10 patients (11%). There was no significant difference between thelaminectomy group and laminoplasty group in terms of operative time, surgical site, infection rate, cerebrospinal fluid (CSF) infection, CSF leak, and length of hospitalization (P > 0.05). The incidence of postoperative spinal deformity was 15.0% in the laminectomy group and 11.7% in the laminoplasty group (P > 0.05). Laminoplasty vs laminectomy was associated with a similar risk of progressive deformity. However, for the cervical patients, there is significant difference (P < 0.05) in the spinal deformity. For the patients with incision vertebral segments >3, there is no significant difference in the spinal deformity (P > 0.05). Bone fusion was achieved in 7 (20%) patients in the laminoplasty group. Laminoplasty vs laminectomy was associated with a similar risk of progressive deformity. Conclusion Vertebral plate and ligament composite replant is a simple and practical method in spinal cord tumor surgery. Neither every case got bone fusion nor positive results turned out in survival analysis, but it is still valuable in reducing spinal deformity, especially in cervical vertebra spinal cord tumor surgery.

Background and Importance: Diffuse leptomeningeal glioneuronal tumor (DLGNT) represents a provisional entity in the 2016 World Health Organization classification of tumors; it is characterized by a widespread leptomeningeal growth and oligodendroglial-like cytology. To this day, 4 pediatric patients have been reported to present with an isolated spinal cord tumor in the absence of leptomeningeal dissemination. Gross total resection (GTR) was achieved in only 1 patient. We present the clinical and technical nuances of this unique type of tumor, as well as the second reported case of GTR in a patient with DLGNT. Clinical Presentation: A 4-year-old boy presented to the emergency department after an episode of flaccid paralysis of bilateral lower extremities. MRI showed an intramedullary spinal cord tumor centered at T8. The patient was taken to the operative room, where a laminectomy and tumor resection were performed; cystic and solid tumor components were identified. Pathology report was consistent with DLGNT. After achieving GTR, patient is free of recurrence after a 15-month follow-up. Conclusion: No standard treatment for DLGNT has been identified. Current literature report surgery and chemotherapy with variable success rates. DLGNT presenting as an isolated intramedullary tumor is an uncommon condition which progression appears to be halted when treated promptly. Identifying solid and cystic components of this tumor is crucial for achieving GTR.

Background Congenital midline spinal hamartoma is a very rare tumor-like proliferation that mostly occurs during childhood. It consists of mature, well-differentiated ectodermal and mesodermal elements that present in an abnormal location and are mostly associated with neurofibromatosis type 1 and spinal dysraphism. Case Description A 5-month-old male patient presented with complaint of a bump over the lower back. The bump was already present since birth with the size of approximately 3 cm in diameter and slow growth. There were no other associated symptoms besides the bump on the lower back. In the physical examination, we found a subcutaneous mass with associated skin dimple located on the midline of the lower back. Computed tomography scan of the spine showed a mass that arose from inside the dura and a closed spinal dysraphism from L3 vertebra to the sacrum. A subtotal resection was performed, and the histopathology feature showed fat cells, cartilage, skeletal muscle fibers, nerve, and blood vessel. After 6 months follow-up, no symptoms or neurologic deficit were present, and no further growth was shown on the latest computed tomography scan. Conclusions Congenital midline spinal hamartoma is very rare and seldom addressed. Most patients present with a cosmetic defect and rarely with associated symptoms. Tissue biopsy is still the best modality to definitively diagnose hamartoma and to exclude other diagnoses. Surgical excision of the tumor is still the mainstay treatment, especially for patients who are not neurologically intact and to correct the cosmetic skin defect.

Purpose Malignant gliomas are aggressive spinal cord tumors. In this study, we hypothesized that combination therapy using an anti-angiogenic agent, bevacizumab, and hypoxia-inducible glioblastoma-specific suicide gene could reduce tumor growth. Materials and methods In the present study, we evaluated the effect of combination therapy using bevacizumab and pEpo-NI2-SV-TK in reducing the proliferation of C6 cells and tumor growth in the spinal cord. Spinal cord tumor was generated by the injection of C6 cells into the T5 level of the spinal cord. Complexes of branched polyethylenimine (bPEI)/pEpo-NI2-SV-TK were injected into the spinal cord tumor. Bevacizumab was then administered by an intraperitoneal injection at a dose of 7 mg/kg. The anti-cancer effects of combination therapy were analyzed by histological analyses and magnetic resonance imaging (MRI). The Basso, Beattie and Bresnahan scale scores for all of the treatment groups were recorded every other day for 15 days to assess the rat hind-limb strength. Results The complexes of bPEI/pEpo-NI2-SV-TK inhibited the viability of C6 cells in the hypoxia condition at 5 days after treatment with ganciclovir. Bevacizumab was decreased in the cell viability of human umbilical vein endothelial cells. Combination therapy reduced the tumor size by histological analyses and MRI. The combination therapy group showed improved hind-limb function compared to the other groups that were administered pEpo-NI2-SV-TK alone or bevacizumab alone. Conclusion This study suggests that combination therapy using bevacizumab with the pEpo-NI2-SV-TK therapeutic gene could be useful for increasing its therapeutic benefits for intramedullary spinal cord tumors.

Background Paraplegia after lumbar spinal surgery has been previously described. It was generally provoked by a missed thoracic compression because of degenerative processes, arachnoid cyst, and spinal cord tumor such as meningioma. We describe here a case of a patient with neurofibromatosis type 2 (NF-2) with multiple spinal meningiomas that developed postoperative paraplegia because of decompensation of spinal cord compression below and far from the operated level. Case Description A 54-year-old woman with NF-2 was followed-up for multiple spinal meningiomas (C7-T1, T6-7, T9-10 levels). Surgery for the symptomatic and larger lesion (C7-T1) was scheduled. Postoperatively, the patient was found to have paraplegia with sensor anesthesia below the level of the T6 vertebra. An urgent spinal magnetic resonance imaging (MRI) scan was performed revealing the absence of complication at the operated level (C7-T1) but the appearance of a marked intramedullary hyperintensity at the T6-7 level. An urgent T6-7 laminectomy and removal of the meningioma was performed. The postoperative phase was marked by a poor recuperation. Spinal MRI scan at 3 months clearly showed a severely injured spinal cord at the T6-7 level consistent with the neurologic status of the patient. Conclusions We report here the first case of acute neurologic deterioration after decompensation of a spinal cord compression below the operated level in spinal intradural surgery. Neurosurgeons must be aware of this possible complication when treating patients with multiple spinal meningiomas.

Background Due to their rarity, the clinicopathological characteristics and prognostic factors of spinal cord gliomas are still unclear. Here, we aimed to clarify these issues in a cohort of 108 spinal cord astrocytomas. Methods We characterized the clinicopathological characteristics, including 2016 World Health Organization (WHO) grade, age, sex, location, segment length, resection, pre‐ and postsurgery, Modified McCormick Scale (MMS), radio‐ and chemotherapy, and Ki‐67 and H3 K27M mutations, in 108 spinal cord astrocytomas through heatmaps. The Cox regression analysis and Kaplan‐Meier curves were used to study the prognostic value of these clinicopathological features. Results There are a total 38 H3 K27M‐mutant tumors, including 31 cases with histological grade II/III tumors. The age of low‐grade astrocytoma patients (WHO grade I/II, n = 54) was significantly younger (27.0 vs 35.5 years, P = .001) than those with high‐grade tumors (WHO grade III/IV, n = 54). All patients underwent surgical resection with neurophysiological monitoring, and the surgery did not result in significant changes in MMS. The presurgery MMS was associated with overall survival in the high‐grade subgroup (P = .008) but not in the low‐grade subgroup (P = .312). While, the high content of resection improved the survival of only patients with low‐grade astrocytomas (P = .016) but not those with high‐grade astrocytomas (P = .475). Both the low‐grade and high‐grade astrocytomas had no obvious benefit from neither adjuvant chemotherapy nor radiotherapy (all P > .05). BackgroundThe first line of treatment for most cervical intradural tumors is surgical resection through laminotomy or laminectomy. This may cause a loss of posterior pulling force leading to kyphosis, which is associated with decreased functional outcome. However, the incidence and predictors of kyphosis in these patients are poorly understood.ObjectTo assess the incidence of posterior fixation (PF), as well as predictors of radiological kyphosis, following resection of cervical intradural tumors in adults.MethodsA population-based cohort study was conducted on adult patients who underwent intradural tumor resection via cervical laminectomy with or without laminoplasty between 2005 and 2017. Primary outcome was kyphosis requiring PF. Secondary outcome was radiological kyphotic increase, measured by the change in the C2–C7 Cobb angle between pre- and postoperative magnetic resonance images.ResultsEighty-four patients were included. Twenty-four percent of the tumors were intramedullary, and the most common diagnosis was meningioma. The mean laminectomy range was 2.4 levels, and laminoplasty was performed in 40% of cases. No prophylactic PF was performed. During a mean follow-up of 4.4 years, two patients (2.4%) required delayed PF. The mean radiological kyphotic increase after surgery was 3.0°, which was significantly associated with laminectomy of C2 and C3. Of these, C3 laminectomy demonstrated independent risk association.ConclusionsThere was a low incidence of delayed PF following cervical intradural tumor resection, supporting the practice of not performing prophylactic PF. Kyphotic increase was associated with C2 and C3 laminectomy, which could help identify at-risk patients were targeted follow-up is indicated.

Intradural epidermoid tumors of the spinal cord are commonly associated with spinal cord dysraphism or invasive procedures. The spinal epidermoid tumor, frequently located intradural extramedullary region, is typically found in the lumbar region, and its placement in the thoracic region is rare. Furthermore, it is rarely observed without underlying spinal dysraphism or invasive intervention. The first case, a 52-year-old female patient was evaluated for back pain. On the thoracic magnetic resonance imaging (MRI) conducted, a 7-mm diameter intradural extramedullary non-enhancing nodular formation posterior to the T3 vertebra was detected. This lesion was completely excised via surgical intervention because of the increased size and the resultant hypoesthesia below T4 dermatome noted at follow-up. The second case, a 31-year-old woman presented with new-onset weakness in both legs. Notably, she has a history of a spinal mass lesion in the thoracic region 5 years ago. Thoracic MRI was performed to examine the recurrence, and it revealed a hypointense mass lesion axial plane with intradural extramedullary circumferential enhancement in the left posterior T6 level. This lesion was completely removed. No recurrence was noted at follow-up, and the histopathological diagnosis confirmed an epidermoid tumor. An isolated epidermoid tumor should be considered in the differential diagnosis of intradural -extramedullary mass lesions of the thoracic region. The tumor was successfully resected in its entirety via surgical intervention without any neurological deficit. Our cases support the fact that surgical resection is a crucial factor in recurrence.

Background Subependymoma in the spinal cord is very rare and usually occurs in the cervical cord. We report an exceptional case of subependymoma that occurred at the conus medullaris with cystic formation. This article reviews the literature on subependymoma in the conus medullaris; discusses its clinical manifestations, imaging findings, and differential diagnoses; and offers an opinion about the cystic formation of the subependymoma. Case Description A 69-year-old woman experienced progressive limb weakness with a somatosensory abnormality for 3 months. Preoperative magnetic resonance imaging showed a cystic intramedullary lesion at the conus medullaris with a well-defined margin. A preliminary diagnosis of epidermoid cyst was made based on the imaging findings. During the operation, cystic formation of the tumor was found, and the tumor was completely removed. Pathology showed an uneven proliferation of glial cells, consistent with subependymal morphology, and the tumor was confirmed as subependymoma. Conclusions We present an extremely rare case of cystic formation in subependymoma at the conus medullaris. Subependymoma should be included in the differential diagnosis of intramedullary cystic lesions. The breakdown of the blood-brain barrier and excessive extravasation may be potential mechanisms of cystic formation.

Background Hemangioblastomas are well vascularized, benign CNS tumors and the third most common primary spinal cord tumor after astrocytoma/ependymoma, occurring sporadically or as a part of an autosomal dominant von Hippel-Lindau disease in which tumors are often multiple and prone to relapse. Spinal hemangioblastomas are commonly located in the cervical cord and associated with a syrinx formation. Due to location and growth trends, they may cause significant neurological deficit, impairing patient quality of life. We conducted a systematic review to understand better clinical insights of spinal hemangioblastoma in adults and compare spinal hemangioblastoma versus posterior cranial fossa hemangioblastoma. Methods Followed PRISMA guidelines for conducting systematic reviews, we performed a review of the English literature on adult spinal hemangioblastoma in the MEDLINE/PubMed database over the last 40 years. Results We reviewed 237 total scientific articles on adult spinal hemangioblastoma and analyzed national and continental distribution, clinical symptomatology, tumor location and presence of syringomyelia, treatment strategies and postoperative complications, histology and immunochemistry, as well as treatment outcomes. We also compared individual characteristics in both sporadic and von Hippel-Lindau Disease spinal hemangioblastomas. Finally, we compared features of posterior cranial fossa and spinal hemangioblastomas. Conclusion Spinal cord hemangioblastomas are most commonly located dorsally and intramedullary. Total surgical tumor resection is the first treatment option, while preoperative embolization may be performed to reduce intraoperative bleeding and surgical time. Hemangioblastomas located in the spine have decreased mortality and rate of infection, but increased rates of cardiopulmonary complications compared with those found in the posterior cranial fossa.

Background Immunoglobulin G4–related disease (IgG4-RD) is a recently identified multisystemic fibroinflammatory condition of unclear etiology. IgG4-RD of the epidural tissue causing spinal cord compression is extremely rare. Case description Here, we present a 27-year-old male with epidural mass, causing spinal cord compression at the level of D5-D6. The mass proved pathologically to be epidural inflammatory pseudotumor (IPT) related to IgG4. Spinal decompression was done. The patient was started on steroid treatment and reported a complete resolution of his symptoms over a 3 years’ follow-up period. Conclusion To the authors’ knowledge, this is the first case of IgG4-related epidural IPT and spinal cord compression in Bahrain and the Middle East. IgG4-RD should always be considered as a part of the differential diagnosis of spinal tumors.

Spinal cord tumor (SCT) is a critical disease with low incidence but high disability rate and morbidity. Surgical resection is a primary and effective treatment of SCT, but intraoperative detection of SCT highly depends on the clinical experience. Pathological studies show that microvascular proliferation (MP) occurs in some kinds of SCTs but it cannot be observed by conventional medical imaging modalities such as magnetic resonance imaging (MRI). Ultrasound microvascular imaging (UMI) has been developed to map tumor-related microvasculature. In this study, we introduced UMI into the visualization of microvessels in two types of human SCTs (i.e., ependymoma and schwannoma) and their corresponding normal tissues. For quantitative evaluation, microveseel density (MD) and power Doppler density (PDD) in the tumor region and normal tissue region of the two SCTs were calculated, respectively. Results show that the MDs and PDDs in the tumors are significantly higher than those in the normal tissues. Qualitative and quantitative results demonstrate that MP occurs in the tumor region compared with normal tissue, which is consistent with the results of immunohistochemical staining examination. In conclusion, this study demonstrate the feasibility of UMI in the intraoperative detection of human SCT.

This is the case of a 52-year-old male patient with tetraplegia and muscle respiratory failure, due to an extradural compressive cervical ganglioneuroma (C1-C2), with a history of neurological symptoms for 3 years. The tumor was completely surgically removed and the patient followed a neuro-muscular rehabilitation program (NMRP) for 4 months, consisting of rehabilitation nursing, physical and occupational therapy. At the end of NMRP the patient’s achievements were: unilateral supported walking, partial autonomous daily living functioning, the improvement of quality of life and facilitation of social and professional participation. A collision tumor is one where two neoplasms of differing type occur at the same anatomical site. We present a patient suffering from non Hodgkin small cell lymphocytic lymphoma/chronic lymphocytic leukemia (SLL/CLL) and complaining intense lumbar back pain refractory to medical treatment. Lumbosacral MRI showed an intradural extramedullary lesion in the left L2-L3 foramen with extracanalar development and compression of psoas muscle. CT showed intralesional calcification. The patient underwent resection of the lesion through a paraspinal posterolateral approach (Wiltse approach). The histology was of schwannoma with intralesional calcifications and lymphocitic infiltrates compatible with B-lineage SLL/CLL. After the operation the patient suffer from left psoas muscle motor weakness (3/5 MRC). Because of hematological disease progression, she underwent 6 cycles of chemioterapy (Fludarabine, Cyclophosphamide, Rituximab). At a six-month follow-up no recurrence or residual tumor upon lumbosacral MR imaging was detectable and the left thigh flexion returned normal. To our knowledge, this is the first described case in the literature of collision tumor between a solitary spinal Schwannoma and SLL/CLL.

Pilocytic astrocytomas are World Health Organisation (WHO) grade I tumors, occurring predominantly supratentorially and in the pediatric population. Although the mainstay of treatment is local therapies such as surgery, targeted systemic therapies may be necessary for recurrent or unresectable disease. The majority of sporadic pilocytic astrocytomas are associated with the BRAF-KIAA fusion gene, which results in constitutive activation of the MAP Kinase pathway. Less frequently, the BRAF V600E point mutation has been described, occurring in less than 10% of supratentorial pilocytic astrocytomas. Tumours with this mutation may respond to targeted therapy against the BRAF/MAP Kinase pathway. We report the first described case of a spinal pilocytic astrocytoma in an adult patient with a BRAF V600E mutation responding to targeted therapy using BRAF and MEK tyrosine kinase inhibitors, and share our experiences with the management of toxicity in this patient population.

Pediatric spinal tumors are unique pathologies treated by pediatric neurosurgeons. Special attention is required for the preservation of neural function and bony alignment. We reported our experience in the management of these challenging lesions. A total of 75 pediatric patients with spinal tumors treated at the National Taiwan University Hospital from 1998 to 2018 were identified retrospectively. Clinical data, radiographic image, and pathological report were reviewed for analysis. There were 37 females and 38 males. The median age was 9 years. Thirty-eight tumors (50.6%) were extradural, 20 (26.7%) intradural extramedullary, and 17 (22.6%) intramedullary. The most common pathologies were glioma, ependymoma, and neuroblastoma. The rate of total and subtotal resection was 45.3% and 21.3%. Thirty-four patients (45.3%) required post-operative adjuvant therapy. Eight patients (10.6%) with spinal deformity had simultaneous tumor excision and spinal fusion surgery. Additional six (8%) patients had subsequent spinal fixation and fusion for deformity after primary tumor operation. Eighty-four percent of patients were ambulatory 3 years after operation. For patients with intradural extramedullary and intramedullary tumors, worse survival outcome was associated with tumor derived from CSF seeding and cranial involvement of spinal tumor, while poorer functional outcome was correlated with cranial involvement and adjuvant therapy with chemotherapy or radiotherapy. Pediatric spinal tumor surgery carries low surgical morbidity and mortality under current standard of neurosurgical practice. Post-operative adjuvant therapy is required for nearly half of the cases. Spinal deformity requires special attention and sometimes surgical correction. Contemporary management of pediatric spinal tumors enables effective ablation of the lesion and delivers favorable outcome for the majority of patients.

A 30-month-old Maine Coon presented with progressive proprioceptive ataxia, paraparesis, thoracolumbar pain, and decreased appetite. An extradural mass was detected within the left side of the 13th thoracic vertebral canal that compressed the spinal cord on magnetic resonance (MR) and was considered to be mineralized on computed tomography (CT) images. The resected mass was diagnosed as a vertebral vascular hamartoma. Clinical signs improved, but recurrence was diagnosed by MR and CT imaging at 7 months after surgery. Repeated excisional surgery yielded the same diagnosis and the clinical signs abated. Fifteen months after the second surgery, there was apparent vertebral deformation, but there was no further change on CT images by 29 months.

Study Design.A case series of dual time-point 18F-fluoro-2-deoxy-D-glucose positron emission tomography/computed tomography (18F-FDG PET/CT) for the diagnosis of spinal cord sarcoidosis.Objective.The aim of this study was to illustrate three cases of spinal sarcoidosis with occult presentation and subsequent identification with the use of dual time-point 18F-FDG PET/CT.Summary of Background Data.Sarcoidosis of the spinal cord is very rare and when it occurs without systemic manifestations of disease can be a challenging diagnostic dilemma frequently resulting in the need for spinal cord biopsy in order to establish a diagnosis.Methods.Case series presentation and report.Results.This manuscript presents a case series experience of dual time-point 18F-FDG PET/CT for the diagnosis of spinal cord sarcoidosis. We review the cases of three patients who presented with myelopathy and underwent 18F-FDG DTPI as part of the evaluation for enhancing spinal cord lesions of unknown etiology for 2 years at a university-based cancer hospital. 18F-FDG DTPI was vital in making the diagnosis of sarcoidosis, and in two of the cases, the patients were able to avoid biopsy, thereby avoiding potential morbidity from an invasive procedure.Conclusion.18F-FDG PET/CT imaging is a noninvasive imaging technique that can be crucial in the diagnosis of sarcoidosis of the spinal cord and help avoid unnecessary procedures.Level of Evidence: 4. © 2019 Wolters Kluwer Health, Inc. Aim: Ultrasonic surgical aspirators are surgical instruments operated with high frequency sound waves. The brain and/or spinal cord tumors can be removed safely with minimal damaging to neurovascular structures by using tissue-selective ultrasonic aspirators. Besides its benefits to the patient, by shortening the duration of operation, it may also provide cost savings in terms of hospital management. The aim of this study is to evaluate the utility and feasibility of ultrasonic aspirator in central nervous system tumors. Material and Methods: Forty patients who apply to the Department of Neurosurgery at Duzce University Medical Faculty between March 2013 and September 2017 due to brain or spinal cord tumor and operated for a brain or spinal cord tumor were included. Ultrasonic aspirator was used during the operations, and duration of operations were recorded. Results: The total operation time was compared between the groups that their operations performed by using ultrasonic aspirator and by not using it. The mean operation time was significantly higher in the group performed operations by using ultrasonic aspirator (253.8±87.5, 195.4±48.7, p=0.014). Whereas ultrasonic aspirator usage did not change the duration of operation in patients with glioblastoma (237.8±56.3, 235.5±31.9, p=0.689). Conclusion: Technological instruments, that become a part of surgical treatment, are devices that ensure maximum efficiency with minimum damage. However, these devices require prior training on how to use them. Training of healthcare staff in the use of ultrasonic aspirator is very important. Gereç ve Yöntemler: Mart 2013 ve Eylül 2017 tarihleri arasında beyin veya omurilik tümörü nedeniyle Düzce Üniversitesi Tıp Fakültesi, Beyin ve Sinir Cerrahisi Anabilim Dalına başvuran, beyin veya omurilik tümörü nedeniyle ameliyatı yapılan 40 hasta çalışmaya alınmıştır. Ameliyat sırasında ultrasonik aspiratör kullanılmış ve ameliyat süreleri kaydedilmiştir. Bulgular: Ameliyatları ultrasonik aspiratör kullanılarak yapılan grup ile ultrasonik aspiratör kullanmadan yapılan gruplar arasında toplam operasyon süresi karşılaştırılmıştır. Ultrasonik aspiratör kullanılan grupta ortalama ameliyat süresinin anlamlı düzeyde yüksek olduğu tespit edilmiştir (253,8±87,5; 195,4±48,7; p=0,014). Oysa ultrasonik aspiratör kullanımının glioblastoma tanılı hastalarda ameliyat süresini değiştirmediği tespit edilmiştir (237,8±56,3; 235,5±31,9; p=0,689). Sonuç: Cerrahi tedavinin bir parçası haline gelen teknolojik aletler, en az hasar ile maksimum verim alınmasını sağlayan cihazlardır. Ancak bu cihazlar nasıl kullanılacakları konusunda önceden eğitim gerektirirler. Ultrasonik aspiratör kullanımında yardımcı sağlık personelinin eğitimi çok önemlidir. Bu konuda yardımcı sağlık personelinin eğitiminin ardından yapılacak daha fazla çalışmalara ihtiyaç vardır.

Automatic detection and classification of abnormal medical images are very challenging in computer assisted identification of anomaly which helps the physician and the experts. The work presented in this paper attempted integrated method for automatic classification of spinal cord tumor by determining feature values of the Sample image. The integration of algorithm such as Gray Level Co-occurrence Matrix (GLCM) with Multivariate Support Vector Machine (MSVM) and K-Nearest Neighbour (KNN) classifiers approaches are producing effective results in spinal cord tumor classification. In the feature extraction stage, Gray Level Co-occurrence Matrix (GLCM) is used to compute the discriminative features. In the classification stage, the obtained features provide as input for the classification algorithm. Both approaches will classify the abnormal images along with its three types which are based on the location of the tumor existence in the spinal cord in an automatic process. Features extracted with GLCM integrated with MSVM produced 96% accuracy results. Similarly GLCM combined with KNN produced 86.5% accuracy during the classification. The performance shows the efficiency and adeptness of the integrated model.

Objective The National Cancer Database (NCDB) and the SEER (Surveillance Epidemiology and End Results) program are the 2 largest cancer registries in the United States. However, considerable differences exist between them regarding the sampling frame as well as the participating facility characteristics. In this study, NCDB and SEER are compared for primary central nervous system (CNS) tumors with the aims of discussing the implications for researchers and evaluating the generalizability of both databases. Methods Patients with primary CNS (brain and spinal cord) tumors from 2004 to 2013 were identified using the International Classification of Diseases for Oncology, Third Edition topographic codes. Patient demographics, tumor characteristics, and treatment-related variables were compared between the 2 databases using standardized differences, with values >0.1 denoting statistical significance. Results A total of 588,534 primary brain tumor (NCDB, 416,162; SEER, 172,372) and 34,827 primary spinal cord tumor (NCDB, 26,602; SEER, 8225) cases were identified. We found significant differences regarding age (sd. diff., 0.16 for brain; 0.13 for spinal cord), and Hispanic origin (sd. diff., 0.34 for brain; 0.61 for spinal cord). For NCDB and SEER patients, estimated 5-year overall survival was 59% and 64%, respectively. Conclusions Analysis of 623,361 patients with primary CNS tumors, which are identified using both the NCDB and SEER databases, showed significant differences in age, histopathologic classification of tumors, tumor behavior, and treatment of tumors between 2 databases. Overall, the differences observed between 2 databases provide helpful points for the researchers who would like to use NCDB or SEER. These observations should be taken into account when researchers design studies using these databases and discuss the generalizability of their findings.

A fair number of hospital admissions occur after 30 days; thus, the true readmission rate could have been underestimated. Therefore, we hypothesized that the 90-day readmission rate might better characterize the factors contributing to readmission for pediatric patients undergoing spinal tumor resection.The Nationwide Readmissions Database was used to study the patient demographic data, comorbidities, admissions, hospital course, spinal tumor behavior (malignant vs. benign), complications, revisions, and 30- and 90-day readmissions.Of the 397 patients included in the 30-day cohort, 43 (10.8%) had been readmitted. In comparison, the 90-day readmission rate was significantly greater; 52 of 325 patients were readmitted (16.0%; P0.04). Patients aged 16-20 constituted the largest subgroup. However, the highest readmission rate was observed for patients aged5 years (30-day, 21.7%; 90-day, 26.4%). Medicaid patients were more likely to be readmitted than were private insurance patients (30-day odds ratio [OR], 3.3 [P0.001]; 90-day OR, 2.29 [P0.02]). In both cohorts, patients with malignant tumors required readmission more often than did those with benign tumors (30-day OR, 2.78 [P0.02]; 90-day OR, 1.92 [P = 0.08]). In the 90-day cohort, the patients had been readmitted 26.4 days after discharge versus 10.6 days in the 30-day cohort. Within the 90-day cohort, 18.6% of the readmissions were for spinal reoperation, 28.3% for chemotherapy or hematologic complications, and 25.6% for other central nervous system disorders. The median charges for each readmission were ∼$50,000 and ∼$40,000 for the 30- and 90-day cohorts, respectively. Medicaid insurance, malignant tumors, and younger age were significant predictors of readmission in the 90-day cohort.The prevalence and charges associated with unplanned hospital readmissions after spinal tumor resection were remarkably high. Younger age, Medicaid insurance, malignant tumors, and complications during the initial admission were significant predictors of 90-day readmission.

Pediatric patients presenting with intramedullary spinal cord lesions often require specific diagnoses to guide their treatment plans. Though results from magnetic resonance imaging and lumbar puncture may narrow the differential diagnosis, these tests cannot always provide a definitive diagnosis. In such cases, spinal cord biopsy may be undertaken to provide a specific histopathologic diagnosis for guiding treatment. Data from the adult population show 24% of spinal cord biopsies can be nondiagnostic and the procedure may carry a 21% complication rate. Therefore, spinal cord biopsy may portend a similar high risk-to-benefit ratio in the pediatric population. Here, we review spinal cord biopsy cases scheduled for diagnosis, and not debulking, at a high volume pediatric referral center during a seventeen-year period. We report our experience with five patients who met our inclusion criteria. Due to the rarity of the procedure, statistically significant factors associated with improved diagnostic yield or peri-operative complication could not be identified. A definitive diagnosis which guided the post-operative treatment plan was obtained in four of our five patients. None of our patients developed post-operative motor deficits. However, these patients were susceptible to the same risks of open spine surgery, such as wound infections and spinal deformities. Our case series shows that intramedullary spinal cord biopsies may provide tissue for obtaining histopatholgic diagnoses. However, the potential risks of complication, and the possibility of obtaining nondiagnostic tissue, should be discussed with patients, families and their medical treatment teams.

Background Central nervous system abscesses frequently can be seen in the immunocompromised population and most commonly consist of intracranial collections in the adult patient. Spinal intradural abscesses are less commonly encountered, and there are even fewer numbers in the pediatric population with a concordant absence of documentation in the published literature. Case Description In this case report, we describe the presentation of a 2-year-old boy with a history of perinatal Escherichia coli meningitis at 2.5 months of age who was found to have an intradural spinal lesion, initially concerning for neoplasm, but later confirmed as an E. coli abscess following biopsy. He was managed with surgical aspiration of the abscess and a long course of intravenous antibiotics. The patient was treated with antibiotics with repeat imaging studies that revealed residual abscess that required re-aspiration at a later date. Conclusions Here we present an unusual disease process with an unusual disease pathogenesis in a pediatric patient currently residing in a developed country.

Laminectomy (LAMT) and laminoplasty (LAMP) have been widely applied on patients with spinal cord tumors (SCTs). However, the clinical efficacy of LAMP versus LAMT remains controversial. The purpose of this study is to assess the safety and efficacy of LAMP compared to LAMT in the treatment of SCTs.We searched several English and Chinese databases (PubMed, EMBASE, the Cochrane Library, CBM, CNKI and Wan Fang) to identify relevant randomized controlled trials or observational studies. The quality of included studies was assessed by the Cochrane Collaboration's tool and the Newcastle-Ottawa Scale. The outcome measures included the primary and secondary outcomes. Subgroups analysis was performed to explore the impact of study type, age, type of tumor, tumor size, surgical levels, follow-up time, surgical methods (whether with fusion) on the outcome measures.Sixteen studies of 1096 patients with SCTs were included in this meta-analysis. The results showed statistically significant differences between the LAMP and LAMT groups in terms of effective recovery rate (P = 0.003), blood loss (P0.00001), hospital stays (P = 0.006), spinal deformity (P = 0.01), and cerebrospinal fluid leak (P0.00001). However, there was no significant differences in total resection rate of tumor (P = 0.21) and operation time (P = 0.14). In the subgroup analysis, the results indicated that age, type of tumor, follow-up time, surgical levels, and methods were the influence factors for the incidence of spinal deformity.LAMP might be a safer and more effective surgical method in the treatment of SCTs. In addition, the advantage of fusion in preventing the occurrence of spinal deformity should not to be ignored. However, because of the lack of high quality randomized controlled trials and adequate data, the safety and validity of LAMP was undermined.

Background Primary central nervous system (CNS) tumors are often associated with high symptom burden and a poor prognosis from the time of diagnosis. The purpose of this study is to describe patient-reported outcomes (PRO) data from long-term survivors (LTS; ≥5-year survival post-diagnosis). Methods Clinical/treatment/molecular characteristics and PROs (symptom burden/interference (MDASI-BT/SP), perceived cognition (Neuro-QoL), anxiety/depression (PROMIS), and general health status (EQ-5D-3L)) were collected on 248 adult LTS between 9/2016 and 8/2019. Descriptive statistics and regression analysis were used to report results. Results Participants had a median age of 47 years (19-82) and were primarily White (83%) males (51%) with high-grade tumors (59%) and few mutations. Forty-two percent of the 222 brain tumor LTS reported no moderate-to-severe symptoms, whereas 45% reported three or more; most common symptoms were fatigue (40%), difficulty remembering (29%), and drowsiness (28%). Among spine tumor LTS (n = 42), nearly half reported moderate-to-severe weakness, pain, fatigue, and numbness/tingling, with 72% experiencing activity-related interference. Severe anxiety, depression, and cognitive symptoms were reported in up to 23% of the sample. Brain tumor LTS at higher risk for severe symptoms were more likely to be young, unemployed, and have poor KPS (Karnofsky Performance Status), whereas high symptom-risk spinal cord tumor LTS had poor KPS and received any tumor treatment. Conclusions Findings indicate LTS fall into distinct cohorts with no significant symptoms or very high symptom burden, regardless of tumor grade or mutational profile. These LTS data demonstrate the need for survivorship care programs and future studies to explore the symptom trajectory of all CNS tumor patients for prevention and early interventions.

Background Autonomic dysreflexia (AD) is an abnormal reflex of the autonomic nervous system normally observed in patients with spinal cord injury from the sixth thoracic vertebra and above. AD causes various symptoms including paroxysmal hypertension due to stimulus. Here, we report a case of recurrent AD associated with cervical spinal cord tumor. Case presentation The patient was a 57-year-old man. Magnetic resonance imaging revealed an intramedullary lesion in the C2, C6, and high Th12 levels. During the course of treatment, sudden loss of consciousness occurred together with abnormal paroxysmal hypertension, marked facial sweating, left upward conjugate gaze deviation, ankylosis of both upper and lower extremities, and mydriasis. Seizures repeatedly occurred, with symptoms disappearing after approximately 30 min. AD associated with cervical spinal cord tumor was diagnosed. Histological examination by tumor biopsy confirmed the diagnosis of gliofibroma. Radiotherapy was performed targeting the entire brain and spinal cord. The patient died approximately 3 months after treatment was started. Conclusions AD is rarely associated with spinal cord tumor, and this is the first case associated with cervical spinal cord gliofibroma. AD is important to recognize, since immediate and appropriate response is required.

Intradural spinal lipomas are very rare and constitute less than 1% of all spinal tumors. Such tumors are usually associated with spinal dysraphism and occur mostly in the lumbosacral or cervical region. Intradural spinal lipomas tends to be intramedullary or subpial. Meningeal melanocytoma is further rarer cases that comprise less than 0.1% of cases. These usually occur in the fifth or fifth decade and chances of malignant transformation are high.Here, we report an extremely rare case (first to the best of our knowledge) of a 9 years female child who presented to us with rapid progressing paraparesis. She was operated and found to have an intradural purely extramedullary spinal lipoma without spinal dysraphism. Moreover, she had melanin pigment deposits all over her meninges which is further rare. On presentation, the patient was bedridden but after surgery, the patient improved and could walk without support.To the best of our knowledge, this is the first case of spinal cord lipoma in dorsal location along with melanin pigments in the meninges. We discuss the pathogenesis, presentation and management of intradural extramedullary spinal lipomas.

Three years old boy presented with recurrent severe abdominal pain. He was treated for constipation and the pain resolved. Later he developed a limitation of neck movements. Investigations revealed a spinal cord mass. After surgical resection, histopathology confirmed pilocytic astrocytoma.

Background and importance Intramedullary spinal cord metastases (ISCMs) are rare and no cases of ISCM from an adenoid cystic carcinoma (ACC) of the external auditory canal (EAC) have been reported. Clinical presentation We report a 54-year old man complaining backpain and worsening myelopathy. He had an ACC of the EAC resected years prior. A spinal MRI demonstrated a contrast-enhancing intramedullary lesion within the conus medullaris. The histopathological diagnosis of the patient was consistent with the patient's primary cancer. At 3 months follow-up, the neurological condition of the patient was unchanged. Conclusions This is the first reported case of ISCM from a primary ACC of the EAC.

Diffuse midline glioma with histone H3 lysine27-to-methionine mutation (H3 K27M mutation) is a rare, aggressive tumor that is designated as World Health Organization (WHO) grade IV regardless of histologic features. Preoperative diagnosis remains challenging due to limited evidence regarding distinctive clinical and imaging characteristics. We describe the case of a young woman who presented with progressively worsening headaches due to communicating hydrocephalus. MR imaging with contrast of the cervical and thoracic spine revealed diffuse leptomeningeal enhancement with focal areas of intramedullary and subarachnoid T2 hyperintensity and enhancement, suggestive of a potential infectious process. Intraoperatively, no epidural pathology was identified, and with the differential diagnosis remaining broad, a second procedure was conducted involving intradural exploration and biopsy of a lesion. This was then identified as a diffuse midline glioma with H3 K27M mutation. The nonfocal clinical presentation in the setting of communicating hydrocephalus as well as the significant exophytic tumor growth and imaging findings made the initial diagnosis unique and challenging. This case, therefore, emphasizes the rare presentation of this tumor, and the need for further understanding of the clinical and imaging characteristics of this disease as well as the need for effective therapeutics.

Background Transection‐induced axonal retrograde degeneration, in contrast to Wallerian degeneration, has not been widely recognized in clinical practice. Aims of the Study To assess a potential of corticospinal tractography for detecting axonal retrograde degeneration. Methods We assessed the corticospinal tractography of a 74‐year‐old woman with monoplegia of the lower limb due to a unilateral thoracic spinal cord tumor. Results The tractography revealed integrity reduction of the corticospinal tract in the cerebra contralateral to the spinal cord tumor. Being able to obtain morphometric quantification (size, location, growth rate) of the tumor, edema, and cavity can result in improved monitoring and treatment planning. Such quantification requires the segmentation of these structures into three separate classes. However, manual segmentation of three-dimensional structures is time consuming, tedious and prone to intra- and inter-rater variability, motivating the development of automated methods. Here, we tailor a model adapted to the spinal cord tumor segmentation task. Data were obtained from 343 patients using gadolinium-enhanced T1-weighted and T2-weighted MRI scans with cervical, thoracic, and/or lumbar coverage. The dataset includes the three most common intramedullary spinal cord tumor types: astrocytomas, ependymomas, and hemangioblastomas. The proposed approach is a cascaded architecture with U-Net-based models that segments tumors in a two-stage process: locate and label. The model first finds the spinal cord and generates bounding box coordinates. The images are cropped according to this output, leading to a reduced field of view, which mitigates class imbalance. The tumor is then segmented. The segmentation of the tumor, cavity, and edema (as a single class) reached 76.7 ± 1.5% of Dice score and the segmentation of tumors alone reached 61.8 ± 4.0% Dice score. The true positive detection rate was above 87% for tumor, edema, and cavity. To the best of our knowledge, this is the first fully automatic deep learning model for spinal cord tumor segmentation. The multiclass segmentation pipeline is available in the Spinal Cord Toolbox (https://spinalcordtoolbox.com/). It can be run with custom data on a regular computer within seconds.

A spinal cord tumor is any tumor involving the spinal cord or immediate surrounding area. Tumors typically are classified as extradural, intradural extramedullary, or intramedullary intradural. Many spinal cord tumor resections attempt to balance tumor removal with preservation of neurologic function. It is important that anesthesiologists be familiar with the common perioperative risks involved in resection of spinal cord tumors as well as how to form an anesthetic plan that takes intraoperative neuromonitoring and patient comorbidities into consideration. Other risks of prolonged spinal tumor resection include postoperative visual loss, acute on chronic pain, and delayed awakening.

Schwannoma is one of the most common pathologies that occur in the intradural sub-arachnoidal space. Spinal schwannomas present in various sizes, shape, and location. With proper adjustments, strategies and techniques used in spinal schwannoma surgeries could be applied to other tumors in the intradural sub-arachnoidal space. Spinal schwannomas can be classified into three categories, which are pre-ganglionic, ganglionic, and post-ganglionic type, according to their nerve root origin. Schwannomas arise from a Schwann cell and involves a single nerve fascicle. Thus theoretically, all of them could be removed without sacrificing the neighboring nerve fibers. In surgeries of ganglionic or post-ganglionic-type schwannomas, it is often not easy to isolate the tumor from other nerve tissues as in that of pre-ganglionic-type schwannomas. Suppose all schwannomas, regardless of their types, are actually located in the intradural sub-arachnoidal space, then it is quite helpful to find and secure the dissection plane between the tumor and the adjacent nerve fibers. With carefully planned surgical strategies and proper techniques, spinal schwannomas can be removed completely without causing neurologic deficits.

Spinal cord hemangioblastomas are benign, high vascular tumors and are considered intramedullary spinal tumors, but they are anatomically located in the pial-extra parenchymal and are considered exactly juxtamedullary tumors. The focus of the surgical procedure on this tumor is to perform a complete resection while preserving the surrounding normal neural tissue. Most authors believe that surgical procedures require circumferential coagulation around the tumor that has been advocated to control intraoperative bleeding. However, circumferential coagulation (CC) could not avoid the collateral damage to the spinal cord tissue itself. Hence, instead of circumferential coagulation, circumferential dissection with minimal use of coagulation (CD) has been applied to prevent unwanted damage to the spinal cord from coagulation. The efficacy and limitations of digital subtraction angiography and embolization as a preoperative procedure should be considered. Efficacy of intraoperative indocyanine green fluorescence angiography is sufficient to replace digital subtraction angiography. Intraoperative neuromonitoring is also necessary because the neurological changes during surgery can be identified immediately.

Neuraxial anesthesia is preferred over general anesthesia for cesarean delivery when appropriate because the latter is associated with a higher incidence of airway complications and an increased need for neonatal resuscitation. Common complications of neuraxial anesthesia include backache and headache, whereas feared but rare complications include paraplegia, intraspinal hemorrhage, cauda equina syndrome, nerve injury, and epidural hematoma. This case report presents a 40-year-old female with undiagnosed and asymptomatic ependymoma who presented with concerning neurological symptoms after receiving spinal anesthesia for elective cesarean delivery. Stat lumbar and thoracic spine magnetic resonance imaging (MRI) were performed on postoperative day 13 and were suggestive of a large hypervascular mass with areas of hemorrhage, concerning for ependymoma. An emergent laminectomy and mass resection were performed, resulting in an improvement in the patient's neurological symptoms.

Background. We would like to introduce epidemiology of spinal cord tumors based on academic nationwide studies. Methods. Korean National Health Insurance (NHI) Corporation’s database and nationwide cancer registry (KCCR) were used for epidemiology. Newly diagnosed spinal cord tumor was investigated between 2009 and 2012 from NHI data. In KCCR data, primary spinal cord and appendage tumors (PSCATs) were from 2006 to 2010. Results. Of 1600 primary spine tumors diagnosed from 2009 to 2012, 373 (23.3%) were malignant and 1227 (76.7%) were nonmalignant. Differences in primary malignant, primary nonmalignant spine tumor incidence by sex were significant (P = 0.004, < 0.001, respectively). The most common histological type was neurilemomas (41.3%), followed by meningiomas (20.1%) and ependymomas (7.6%).

I’ve had spinal cord tumor surgery for quite some time. During about 25 years, 321 intramedullary tumors, 929 intradural extramedullary tumors, and 182 extradural tumors were operated. While I worked as a neurosurgeon, I learned a lot from patients and image like as MR. I have changed many of my practices. I used to use steroids and insert drain. However, currently, steroids are never used, and if dura is opened, no drain is added. In schwannoma, you have to meet endoneurial fluid. And spinal cord meningioma is intradural extra-arachnoidal tumor. In hemangioblastoma, it is not cauterization but dissection. In ependymoma, the tumor should be dissected from gray matter of spinal cord. The principle of cavernous malformation surgery is to open the shortest distance from the lesion. I would like to share my experience and ideas with others in this book. I’ve also changed and developed a lot of spinal cord tumor surgery, but I think there’s still a lot to change. I hope my experiences and ideas in this book will help surgeons performing spinal cord tumor surgery.
