BACKGROUND Currently, there is a lack of sepsis screening tools that can be widely used worldwide. Pulmonary sepsis can be of sufficient concern to physicians due to their noticeable symptoms, which usually rely less on screening tools. AIM To investigate the efficiency of the international normalized ratio (INR) for the early rapid recognition of adult nonpulmonary infectious sepsis. METHODS This is a prospective observational study. A total of 108 sepsis patients and 106 nonsepsis patients were enrolled according to relevant inclusion and exclusion criteria. Commonly used clinical indicators, such as white blood cell, neutrophil count, lymphocyte count, neutrophil-lymphocyte count ratio (NLCR), platelets (PLT), prothrombin time, INR, activated partial thromboplastin time, and quick Sequential “Sepsis-related” Organ Failure Assessment (qSOFA) scores were recorded within 24 h after admission. The diagnostic performances of these clinical indicators were analyzed and compared through multivariate logistic regression analysis, Spearman correlation, and receiver operating characteristic curve analysis. RESULTS The INR value of the sepsis group was significantly higher than that of the nonsepsis group. INR has superior diagnostic efficacy for sepsis, with an area under the curve value of 0.918, when those preexisting diseases which significantly affect coagulation function were excluded. The diagnostic efficacy of the INR was more significant than that of NLCR, PLT, and qSOFA (P < 0.05). Moreover, INR levels of 1.17, 1.20, and 1.22 could be used to categorize the relative risk of nonpulmonary infections sepsis into three categories: low, medium and high risk, respectively. CONCLUSION The INR is a promising and easily available biomarker for diagnosis, and it can be used as one of the indicators for early screening of adult nonpulmonary infectious sepsis. When its value is higher than the optimal cutoff value (1.22), high vigilance is required for adult nonpulmonary infectious sepsis.

Background This study aimed to validate the feasibility of deformable image registration (DIR) in assessing the cumulative dose distributions in combined external beam radiotherapy (EBRT) and intracavitary brachytherapy (ICBT) for cervical cancer. Methods This retrospective study included 23 patients with stage IIB disease treated with combined EBRT to the whole pelvis (50.4 Gy in 28 fractions) using an intensity-modulated radiotherapy technique with 6-MV X-ray, followed by three-dimensional (3D) ICBT (28 Gy in 4 fractions). Tumor gross target volume at diagnosis (GTV-Tinit), tumor gross target volume before brachytherapy, high-risk clinical target volume (HR-CTV), intermediate-risk clinical target volume (IR-CTV), and parametrium and organs at risk were recontoured on computed tomography images of EBRT and ICBT, respectively. The dose-volume parameters were also determined. The DIR results were reviewed using MIM Maestro (Reg Review) and modified by function (Reg Refine). To evaluate the accuracy of DIR, DIR-based cumulative dose-volume histogram (DVH) parameters and simple DVH parameter addition were compared using Wilcoxon rank-sum tests. Results The cumulative dose distributions of EBRT and four ICBT sessions were successfully illustrated using DIR. The mean tumor diameters were 68.35 cm3 at diagnosis and 29.63 cm3 at ICBT initiation. The mean tumor regression was 56.6%. The median minimum dose covering 90% (D90) of HR-CTV, GTV-Tinit, IR-CTV, and parametrium were 69.58±4.94, 68.81±7.98, 59.28±3.78, and 60.97±1.1 Gyα/β=10, respectively, for DIR and 69.11±5.68, 68.49±8.62, 58.89±3.59, and 61±1.49 Gyα/β=10, respectively, with conventional simple DVH parameter addition.No statistically significant differences in dosimetric parameters were observed between the two methods. Conclusions Although there were limitations in the DIR accuracy, DIR-based dose accumulation was significantly beneficial in visually showing the cumulative dose distribution in the target area to clinicians in combined radiotherapy for cervical cancer in routine clinical practice.

To evaluate the effect of adjuvant chemotherapy on improving the prognosis of patients with stage I triple-negative breast cancer (TNBC). TNBC patients diagnosed in the SEER 18 database from 2010 to 2015 were included. Kaplan–Meier plots and log-rank tests were used to compare the differences in breast cancer-specific survival (BCSS) and overall survival (OS) between subgroups of variables. A Cox proportional hazard model was used to determine the prognostic factors affecting BCSS and OS. A total of 9256 patients were enrolled in this study. Among these patients, 380 died from breast cancer, and 703 died from all causes. Patients who received chemotherapy had significantly better BCSS and OS than those who did not receive chemotherapy for stage T1cN0M0 (BCSS, hazard ratio (HR) = 0.68, 95% confidence interval (CI) 0.51–0.90; OS, HR = 0.54, 95% CI 0.44–0.67) and stage IB (BCSS, HR = 0.39, 95% CI 0.16–0.95; OS, HR = 0.41, 95% CI 0.19–0.87) disease. Patients who received chemotherapy did not have significantly better BCSS or OS than those who did not receive chemotherapy for stage T1aN0M0 or T1bN0M0 disease. The patients who received chemotherapy in the poorly differentiated and undifferentiated groups had better BCSS (HR = 0.68, 95% CI 0.52–0.88) and OS (HR = 0.54, 95% CI 0.44–0.66) than the patients who did not receive chemotherapy. According to current clinical guidelines, patients with stage T1bN0M0 TNBC are probably overtreated. The prognosis of these patients with stage T1aN0M0 or T1bN0M0 disease is good enough that adjuvant chemotherapy cannot improve it further.

Life-threatening electrolyte imbalance is not uncommon in preemies. Differential diagnosis is important for immediate treatment. The syndrome of pseudohypoaldosteronism (PHA) is characterized by increased aldosterone secretion associated with clinical signs of hypoaldosteronism reflecting mineralocorticoid resistance. There are type I, type II, and secondary type of PHA. Most secondary PHA reported in the pediatric population result from urinary infection and obstructive uropathy and extremely rarely from gastrointestinal fluid loss. Seven preemies accepted jejunostomy or ileostomy, and they suffered from high output stoma. Electrolyte imbalance with bodyweight loss or cardiac event was noted. We found a high level of aldosterone and renin and diagnosed them with secondary PHA due to excessive gastrointestinal losses. After stomal reversal, aldosterone and renin level became normalized, and electrolyte was corrected. This study reports the finding of secondary pseudohyperaldosteronism (hyponatremia, hyperkalemia, and metabolic acidosis) in a series of cases with intestinal resection and ostomy of different causes. Early stomal reversal was recommended.

Aiming at the high-temperature strain gauge on the turbine blade of aero-engine, which needs to bear the high-temperature effect and reliably carry out the strain test, the turbine blades and adhesive strength of rubber layer of adhesive strain gauge were studied by the theoretical analysis, software simulation and experimental verification method. Three-dimensional model of turbine blade was built firstly. And then, the CFD of the blade fluid domain was analyzed. The bonding position of the strain gauge was determined under the actual working condition by the coupling analysis of hydrodynamics, centrifugal force, thermal stress and aerodynamic force. The reliability of strain position simulation of turbine blade was verified by dynamic strain contact measurement experiment. The effect of high temperature and CFD analysis of turbine blades with rubber layer was carried out. The effects of different rotational speeds on the adhesive strength of the adhesive were studied. It was shown that the strain and stress distribution of the blade’s pressure surface was more uniform than the suction surface.

Purpose To evaluate the accuracy of 18F-FDG-PET/CT for the detection of recurrent and/or metastatic diseases in differentiated thyroid cancer (DTC) patients with thyroglobulin elevation and negative iodine scintigraphy. Whether PET/CT with TSH stimulation (sPET/CT) had better diagnostic performance than PET/CT without TSH stimulation (nsPET/CT) in this scenario was also evaluated. Methods PubMed and Embase databases were searched for eligible studies from January 2001 to December 2018. Only studies with clearly stated reference standard (histopathology confirmation and/or clinical/imaging follow-up) were included. Publication bias was assessed by Deeks funnel plot. The pooled sensitivity, specificity, diagnostic odds ratio (DOR) and the area under the summary receiver-operating characteristics curve (AUC) for PET/CT was determined by random-effect analysis, respectively. sPET/CT and nsPET/CT were compared pairwise for all diagnostic estimate indexes using Z-test. Results We included 17 studies with 1195 patients in this meta-analysis. The pooled sensitivity, specificity, DOR and AUC for PET/CT on patient-based data were 0.86 (95% CI: 0.79–0.91), 0.84 (95% CI: 0.72–0.91), 31.00 (95% CI: 12.00–80.00) and 0.91 (95% CI: 0.88–0.93), respectively. There was high heterogeneity (I 2 = 80% for sensitivity, I 2 = 82% for specificity) and possible publication bias (P = 0.01). Z test did not detect statistically significant difference between sPET/CT and nsPET/CT for all the diagnostic estimate indexes (all P > 0.05). Conclusions On patient-based analysis, 18F-FDG-PET/CT has high diagnostic accuracy for the detection of recurrent and/or metastatic diseases in DTC patients with thyroglobulin elevation and negative iodine scintigraphy, but existing studies were limited by high heterogeneity and possible publication bias. The diagnostic performance of sPET/CT may be not superior to nsPET/CT.

An elastic–slip interface model is proposed to investigate the effective elastic modulus of elliptical-fiber reinforced asphalt concrete, and the nonlinear factor related with the second-order term of strain is used to consider the large deformation of asphalt concrete. The representative volume element with effective isotropic medium is introduced to describe the overall properties of elliptical-fiber reinforced asphalt concrete. Combining the conformal mapping technique and polynomial function expanded method, a closed form solution of effective elastic modulus is obtained. Through numerical examples, the effects of interface material coefficients and the nonlinear factor on the effective elastic modulus under different shapes of fibers are discussed in detail. To validate the present interface model, experimental investigation on the interface effect on the elastic modulus is presented.

Negative 99mTc-pertechnetate uptake of the thyroid bed indicates the absence or a small volume of remnant thyroid tissue (RTT) after total thyroidectomy (TT). The aim of this study is to evaluate the predictive value of negative 99mTc-pertechnetate scintigraphy for excellent response (ER) to radioactive iodine therapy (RIT) in low- to intermediate-risk differentiated thyroid cancer (DTC) patients. One-hundred and eighty-nine low- to intermediate-risk DTC patients who underwent TT, RIT with a single dose of 30 mCi and suppressive therapy with thyroid-stimulating hormone (TSH) from July 2015 to February 2016 in our hospital were retrospectively evaluated. 99mTc-pertechnetate thyroid scintigraphy was performed just before RIT and images were reported dichotomously as negative or positive. The response of patients was assessed for 23.2 ± 3.8 months after RIT and dichotomized as excellent response (ER) or non-excellent response (NER). 99mTc-pertechnetate uptake, age at diagnosis, gender, multifocality, T stage, N stage, preablative stimulated thyroglobulin (ps-Tg), and TSH were explored as potential predictors for ER. 80.68% (71/88) of patients with negative 99mTc-pertechnetate uptake achieved ER. When patients were evaluated according to different ps-Tg levels, we found that 94.83% (55/58) of patients with ps-Tg < 1 ng/ml and negative 99mTc-pertechnetate uptake achieved ER. Multivariate Cox regression analysis revealed that ps-Tg (P = 0.0001) and 99mTc-pertechnetate uptake (P = 0.0473) were independent predictors for ER. In addition to ps-Tg, negative 99mTc-pertechnetate uptake is also a significant independent predictor for an excellent response in low- to intermediate-risk patients. It may be possible to omit RIT in patients with ps-Tg < 1 ng/ml and concurrent negative 99mTc-pertechnetate uptake.

Frequent cyanobacterial blooms in the eutrophic waters produce a variety of toxins such as the monocyclic heptapeptide microcystins, greatly harming aquatic ecosystems and human health. However, little information of microcystins in agricultural fields is known. This field study of three common microcystin variants (MC-LR, MC-RR, and MC-YR) in vegetables (n = 161), soils (n = 161) and irrigation water samples (n = 23) collected from southern China regions affected by cyanobacteria blooms, shows their prevalence with total concentrations up to 514 μg/L water, 187 μg/kg soil (dry weight) and 382 μg/kg vegetable (fresh weight). The internet of things (IoT) and sensor networks will dramatically improve our daily life due to the rapid growth of portable and wearable electronics. Triboelectric nanogenerator (TENG) is a promising energy harvester to power these electronics. However, high output performance and low cost TENGs still remain one of the most crucial barriers for practical applications. In this work, we reported a flexible TENG based on crumpled gold films. By introducing crumpled morphology onto the gold thin film using macrocontrol, the TENG delivered a maximum voltage of 124.6 V, a maximum current of 10.13 μA and maximum power density of 0.22 mW/cm2. Importantly, the output performance was quite stable, with no obvious degradation after 6-month storage in ambient environment due to the reliability of Au films. This simple, low-cost and controllable surface modification method is promising for the development of smaller devices for future wearable and self-powered electronics and optoelectronics applications.

For optoelectronic devices, an attractive research field involves the flexible adjustment of the band gap in semiconductor quantum well (QW) structures by strain engineering. However, rigid wafer-based technology enables lattice-misfit strain during epitaxial growth, which is biaxial, unchangeable, and not sufficient for the devices fitted on various irregular surfaces. Therefore, exploiting the strain produced by externally deformed configurations offers unique opportunities to continuously and non-defectively tune the QW's band structure. Here, we propose a strategy to induce uniaxially distributed strain in elaborately designed wavy quantum well nanoribbons (QWNRs). Meanwhile, a numerically solved strain-photonic coupling model based on the theory of elasticity and the eight-band k·p method is established to illustrate the strain distribution coupled with the strain-induced band gap shift of the wavy QWNR. The μ-photoluminescence measurements reveal a periodically varied band gap in the QW along the uniaxial tensile direction, which is consistent with the result of theoretical calculations. This model demonstrates the potential application of a wrinkled configuration in arbitrarily controlling and tuning the band gap and thus the optoelectronic performances of quantum well systems.

Summary Purpose The inhibition of Mus81, a critical DNA repair gene, is recently related to the chemosensitivity of several human cancer cells such as hepatocellular carcinoma (HCC) cells. However, the role of Mus81 knockdown in chemotherapy response of colon cancer cells remains largely unknown. Methods and materials The effects of Mus81 knockdown by lentivirus-mediated short hairpin RNA in sensitivity of HCT116 and LS180 colon cancer cell lines to four therapeutic drugs, including cisplatin (CDDP), were evaluated by MTT assay as well as a mouse model. Apoptosis and cell cycle distribution of HCT116 cell line was detected by flow cytometric analysis. Western blot was also employed to determine the expression of CHK1 pathway and apoptosis-related proteins in HCT116 cells and the xenograft mouse tumors. Results Mus81 knockdown could significantly improve the chemosensitivity of colon cancer cells in vitro and in vivo , especially to CDDP. Mus81 knockdown also induced S phase arrest and elevated apoptosis in CDDP treated HCT116 cells through activating CHK1/CDC25A/CDK2 and CHK1/p53/Bax pathways, while these effects could be counteracted by CHK1 inhibition. Conclusion Mus81 knockdown improves the chemosensitivity of colon cancer cells by inducing S phase arrest and promoting apoptosis through activating CHK1 pathway.

Three-dimensional simulation methodology has been used to evaluate the performance of lattice matched InGaAs/InP double layer planar heterointerface detector arrays. The device characteristics under optical illumination and dark conditions have been computed. The modulation transfer function (MTF) profiles have been calculated with varying device geometries and carrier dynamics. It is found that the p well diffusion radius and minority carrier recombination play important roles in the MTF behaviors of dense arrays. Moderate p well diffusion dimension should be used to balance the device performances between the dark current and MTF profile. Moreover, better MTF characteristic under low light condition can be achieved with higher quality material which has longer recombination lifetime. The influences of underlying mechanisms including photon generated carriers diffusion and carrier recombination processes have been discussed. These simulation methods and results should provide a useful tool for the evaluation and improvement of imaging power of InGaAs focal plane arrays.

In the construction of water distribution pipeline system in Jiaxing City, in order to monitor the water quality and water environment, a continuous observation network is constructed according to the existing technical specifications. At present, the system runs well and produces continuous monitoring data, which provides basis for local water quality protection.

Patients and Methods: Between January 2008 and December 2015, patients with known histologies of pathologic Stage IIIA-N2 NSCLC who underwent R0 resection and received PORT concurrent with or prior to two sequential cycles of chemotherapy (“early PORT”) or with PORT administered after two cycles of chemotherapy (“late PORT”) at multiple hospitals. The primary endpoint was OS; secondary end points included pattern of the first failure, LRRFS, and DMFS. Kaplan–Meier OS, LRRFS, and DMFS curves were compared with the log-rank test. Cox regression analysis was used to determine prognosticators for OS, LRRFS, and DMFS. Results: Of 112 included patients, 41 (36.6%) and 71 (63.4%) patients received early PORT and late PORT, respectively. The median OS, LRRFS, and DMFS were longer for those who received early PORT than for those who received late PORT at the median follow-up of 29.6 months (all p < 0.05). Uni- and multi-variate analyses showed that number of POCT cycles and the combination schedule of PORT and POCT were independent prognostic factors for OS, LRRFS, and DMFS. Conclusions: Early PORT is associated with improved outcomes in pathologic Stage IIIA-N2 R0 NSCLC patients.

We propose a resonant cavity enhanced (RCE) N-structure type-II superlattice (T2SL) mid-wavelength infrared (MWIR) photodetector which can be used for the detection of methane gas at 3.3 μm. The theoretical analysis of quantum efficiency (QE) shows that the peak QE can be enhanced from 0.45 to 0.80 at 3.3 μm after 12 period AlAs0.09Sb0.91/GaSb DBR is introduced to the N-structure T2SL detector and QE exhibits the narrow bandwidth characteristic near the target wavelength. By analyzing the refractive indices of different materials and the reflectance of different DBRs, we also discuss how to determine the component materials of quarter-wavelength DBR reflectors.

AimIt has been shown that G-protein-coupled receptor kinase 2 (GRK2) negatively regulates the insulin-like growth factor 1 receptor (IGF1R) signalling pathway in hepatocellular carcinoma (HCC). The aim of this study was to evaluate the clinicopathological and prognostic significance of GRK2 and IGF1R in HCC.MethodsExpression of GRK2 and IGF1R was first detected by tissue microarray-based immunohistochemistry in 156 patients with HCC. Staining results, termed the H-score, were then correlated with clinicopathological variables and patient survival. Finally, the prognostic value of GRK2 and IGF1R was validated in the publically available TCGA (The Cancer Genome Atlas) RNA-sequencing database.ResultsThe H-score of GRK2 staining (which was significantly lower in tumour than non-tumour tissue) was negatively associated with that of IGF1R with a reverse trend. No clinicopathological significance of the proteins was found except for a relationship between tumoral IGF1R expression and tumour–node–metastasis stage. In univariate analysis, high IGF1R expression predicted poor overall and disease-free survival, whereas GRK2 was not prognostic. In multivariate analysis, IGF1R was significant for overall survival. Furthermore, IGF1R was also of prognostic value in the TCGA database.ConclusionsOur data indicate that GRK2 and IGF1R show a negative correlation in HCC. IGF1R could be a potential marker of poor prognosis for this malignancy.

Sepsis is a life-threatening disease that affects 30 million people worldwide each year. Despite the rapid advances in medical technology and organ support systems, it is still difficult to reduce the mortality rate. Early and rapid diagnosis is crucial to improve the treatment outcome. The aim of this study was to investigate the prediction efficiency of lymphopenia and other clinical markers, such as white blood cell (WBC), neutrophil count (N#), procalcitonin (PCT), and arterial lactic acid (Lac) in the diagnosis and prognosis assessment for adult patients with nonviral infection-related sepsis. A total of 77 sepsis- and 23 non-sepsis adult patients were enrolled in this study from September 2016 to September 2018. Daily lymphocyte count (Lym) of the patients was calculated until discharge or death. The diagnostic performance of the Lym and other biomarkers were compared using the area under the receiver operating characteristic curve (ROC) value. The level of Lym was decreased significantly in the sepsis group. Lym had a high diagnostic performance for sepsis, with an area under the curve (AUC) value of 0.971 (95% CI = 0.916–0.994). The diagnostic efficacy of Lym was more significant than WBC, N#, and PCT (P 3 days indicates a higher 28-day mortality rate.

The last decades have witnessed the booming of social networks where people interact with each other and generate an unprecedented amount of content. With the online information diffusion, users publish microblog which can affect the sentiments changes of others. The aim of this paper is to analyze the sentimental influence on Sina Microblog users. In this paper, the CNN+ELM model is first proposed, in which the convolution neural network (CNN) and the extreme learning machine (ELM) are combined to identify the sentiment trend of the content. And then, the local sentiment influence rank (LSenInfRank) algorithm and the global sentiment influence rank (GSenInfRank) algorithm based on the sentiment polarity of the content, the user interactive information and the network topology, are proposed. The experimental results on the real Sina Microblog dataset show that the proposed algorithms can effectively find out the set of users with large sentimental influence and have good stability.

According to the parameters given, the main frame design of the intelligent fire truck is carried out, and the three-dimensional model is established. It is transformed into a general format by the established 3D model, and the finite element model is established. The static and mechanical analysis of the finite element model by ANSYS is performed to calculate whether the stress and deformation meet the strength and stiffness requirements; See if resonance occurs and what the mode shape is if resonance occurs.

The lip profile plays an important role in the perception of facial aesthetics; lip morphology and aesthetics research is receiving increasing attention. The advancement of research tools such as three-dimensional imaging technology has led to the clarification of lip morphologic and aesthetic characteristics. After studies of lip characteristics according to gender, ethnicity and age provided basic data, studies on lip aesthetics have been conducted by scholars worldwide. These studies could provide a basic theory to support diagnosis and treatment options, as well as the basis for evaluative criteria for precise treatment and technical improvements. Purpose To quantitatively investigate left ventricular volume and function in middle-aged healthy subjects. Materials and methods Ninety healthy volunteers underwent cardiac 3 Tesla MRI. The left ventricular end-diastolic volume (EDV) and end-systolic volume (ESV), stroke volume (SV), ejection fraction (EF), cardiac output (CO), myocardial mass (MM), and their normalized indices (EDVI, ESVI, SVI, CI, and MI, respectively) after corrected with the body surface area (BSA) were analyzed and compared at different ages. Results All subjects had successfully completed the 3-Tesla cardiac MR. Females had significantly smaller EDV (110.5 ± 9.2 versus 125.7 ± 8.3 mL), ESV (36.1 ± 3.5 versus 41.5 ± 3.8 mL), SV (74.3 ± 6.3 versus 84.2 ± 6.7 mL), CO (5.4 ± 0.8 versus 5.8 ± 0.9 l/min) and MM (73.0 ± 10.5 versus 94.8 ± 10.6 g) than males (P 0.05) difference was detected between the genders in EDVI (71.2 ± 4.3 versus 71.1 ± 4.2 mL/m2 , P = 0.882), ESVI (23.3 ± 1.9 versus 23.5 ± 1.9 mL/m2 , P = 0.733) and SVI (47.9 ± 2.9 versus 47.7 ± 3.7 mL/m2 , P = 0.698) except for CI and MI. Females had significantly (P 0.05) was detected in other parameters. Conclusion The left ventricular volume and function differs in women compared with men in the middle-aged population, and these parameters have a tendency of decrease with ageing. J. Magn. Reson. Diarrhea can occur at an early or advanced stage of acquired immunodeficiency syndrome (AIDS) as a usual symptom in people with human immunodeficiency virus (HIV) infection. While it is usually not fatal, it can influence patients' quality of life seriously. It has shown to be efficacious and improves people's immune status to a certain extent to treat HIV/AIDS-related diarrhea on the basis of syndrome differentiation and treatment or Chinese herbs plus conventional treatment. Therefore, it may have a good application potential. Here, we outline a protocol for the systematic review of this health-care intervention, with the aim to evaluate the beneficial effects and safety of Traditional Chinese Medicine (TCM) for patients who suffer from HIV/AIDS-associated diarrhea. Randomized controlled trials that compare Chinese herbs with placebo or other effective treatments will be searched and included, in spite of publication status or language. The primary outcomes include diarrhea frequency and fecal character. The databases we will search as follows: China Science and Technology Journal Database (VIP), Chinese Biomedical Literature Database (SinoMed), Wanfang Data, China National Knowledge Infrastructure, PubMed and the CENTRAL in Cochrane Library. Two authors will respectively conduct the screening of trials, data extraction, and use the Cochrane risk of bias tool to assess the methodological quality. We will analyze the data and perform a meta-analysis if possible. We intend to identify potential therapeutic modalities that may be of benefit to inform clinical practice by supplying existing evidence of the helpful effects and safety of TCM to treat patients suffering from HIV/AIDS-associated diarrhea.

Background: Neutrophil to lymphocyte ratio (NLR) in peripheral blood is established to correlate with the morbidity and mortality of heart disease patients. We aimed to define the severity of inflammation (NLR) by observing the association of NLR with cardiac functions or myocardial damage parameters in patients with acute myocardial infarction. Methods: Data from 715 patients who underwent percutaneous coronary intervention (PCI) within 72 hours of incidence in 2016 were analysed retrospectively. Results: The NLR ranges from 0.50 to 46 (medium ± SD, 2.76 ± 2.96) in 715 patients. NLR positively correlated with myocardial damage (NLR vs. CK-mB: p 2.76 compared to those of NLR 4. The correlation was diminished between NLR and CK-mB at NLR > 2.76 or at NLR > 4, but that of NLR and EF or FS was maintained in NLR > 2.76 and at NLR > 4. EF and FS were comparable between NLR > 2.76 and NLR > 4. But myocardial damage parameters increased significantly at NLR > 4 compared to those of NLR > 2.76. Conclusion: NLR is a strong predictor of myocardial damage in acute myocardial patients. High NLR are associated with myocardial dysfunction in all the patients. Prophylactic central neck dissection (pCND) is controversial in papillary thyroid carcinoma (PTC) without clinical positive lymph nodes. The aim of this study was to investigate the effect of radioactive iodine (RAI) therapy on the clinical outcome in clinically node-negative (cN0) PTC patients treated with total thyroidectomy (TT) alone or in combination with pCND.One-hundred and sixty-seven cN0 PTC patients who underwent TT alone (TT) or in combination with pCND (TT+pCND) in our hospital from January 2014 to August 2015 were evaluated retrospectively. Adjuvant RAI therapy was recommended depending on tumor diameter, multifocality, extrathyroidal extension, the presence of positive lymph nodes, and adverse histopathologic features. Serological and imaging data were collected with a mean follow-up of 29.9±5.2 months after RAI administration. Suppressed and stimulated thyroglobulin, thyroglobulin antibody, diagnostic whole-body scintigraphy, and other imaging examinations were used to assess clinical outcome, which was defined as excellent response, indeterminate response, biochemical incomplete response, and structural incomplete response.TT was performed in 62 (37.1%) and TT+pCND in 105 (62.9%). The rate of permanent hypoparathyroidism was significantly higher in TT+pCND than that of TT alone (14.2 vs. 3.2%, P=0.0316). Because of the detection of central neck lymph node metastases by pCND, 42 (40%) patients developed higher recurrence risk stratification (from low to intermediate) and 12 (11.4%) patients were upstaged in TNM staging. RAI therapy was performed for 46 (74.2%) patients in the TT group and 87 (82.9%) in the TT+pCND group. The mean dose for patients receiving RAI in the TT+pCND group was significantly higher than that in the TT group (113.9±23.1 vs. 93.9±18.1, P0.0001). No significant difference in response to RAI therapy was found between the TT group and the TT+pCND group (P=0.9474).Although the addition of pCND to TT, with a concomitant higher frequency of permanent hypoparathyroidism, upstages 40% of patients, thereby changing the dose of RAI therapy, the clinical response to RAI therapy for TT+pCND is not superior to TT alone in cN0 PTC patients.

In order to facilitate government making timely decision for multiple internet public opinion emergencies with interval fuzzy indicators, we first determine the rational weight of each emergency decision index based on fuzzy information entropy of interval index. Then we aggregate all the interval index values to obtain the comprehensive alarm interval of each emergency. Finally, we can get the priority of emergency operation of all the internet public opinion emergencies according to the possible degree between their alarm intervals.

This paper presents an application-layer attack detection method based on hidden semi-markov models. In this method, the keywords of an application-layer protocol and their inter-arrival times are used as the observations, a hidden semi-markov model is used to describe the application-layer behaviors of a normal user who is using some application-layer protocol. This method is also based anomaly detection. In theory, application-layer anomaly detection can identify the known, unknown and novel attacks happened on application-layer. The experimental results show that this method can identify several application-layer attacks, and has high detection accuracy and low false positive ratio.

When the car is running in high speed, the energy consumption of the air resistance is responsible for 60%~80% of the total energy consumption. If the energy consumed by windward side is converted into electric energy and stored in the battery, the goal of energy conservation and environmental protection can be achieved. The wind scooper structure to achieve the goal was designed, the software FLUENT is used for numerical simulation of fluid analysis. The best wind scooper structure is determined.The power coefficients of different number of blades are analyzed by using sliding mesh technology of CFD through 2D transient method.

BACKGROUND Although there is increasing recognition of the role of somatic mutations in genetic disorders, the prevalence of somatic mutations in neurodevelopmental disease and the optimal techniques to detect somatic mosaicism have not been systematically evaluated. METHODS Using a customized panel of known and candidate genes associated with brain malformations, we applied targeted high-coverage sequencing (depth, = 200x) to leukocyte-derived DNA samples from 158 persons with brain malformations, including the double-cortex syndrome (subcortical band heterotopia, 30 persons), polymicrogyria with megalencephaly (20), periventricular nodular heterotopia (61), and pachygyria (47). We validated candidate mutations with the use of Sanger sequencing and, for variants present at unequal read depths, subcloning followed by colony sequencing. RESULTS Validated, causal mutations were found in 27 persons (17%; range, 10 to 30% for each phenotype). Mutations were somatic in 8 of the 27 (30%), predominantly in persons with the double-cortex syndrome (in whom we found mutations in DCX and LIS1), persons with periventricular nodular heterotopia (FLNA), and persons with pachygyria (TUBB2B). Of the somatic mutations we detected, 5 (63%) were undetectable with the use of traditional Sanger sequencing but were validated through subcloning and subsequent sequencing of the subcloned DNA. We found potentially causal mutations in the candidate genes DYNC1H1, KIF5C, and other kinesin genes in persons with pachygyria. CONCLUSIONS Targeted sequencing was found to be useful for detecting somatic mutations in patients with brain malformations. High-coverage sequencing panels provide an important complement to whole-exome and whole-genome sequencing in the evaluation of somatic mutations in neuropsychiatric disease. For a temperature rebound in the irrigated high arch dam during the construction period, using finite element method to research the influence of environment temperature to temperature rebound of the irrigated high concrete arch dam. Assuming that the initial temperature of the dam concrete is the joint grouting temperature. This paper presents a simulation of the temperature rebound of the high arch dam which is caused by the monthly average environment temperature, and performs sensitivity analysis to research the influence of thermal conductivity and surface heat preservation on the temperature rebound. According to the analysis,when the high arch dam sealing temperature is lower than the dam site annual average temperature, the dam site temperature inevitably would flow backward,slowly,to the irrigation area.The larger the thermal conductivity is,the quicker the temperature rebound speed will be. And benzene board insulation can reduce concrete temperature rebound rate, if the heat preservation benzene board is thicker,the temperature rebound rate is smaller.

Considering the feature of roller capacity with dynamics change in round bar production, a hot-rolling batch planning problem is studied. Firstly, the problem is formulated as a different capacities vehicle routing problem (DCVRP). And then, a mathematic model is built with two optimization objectives, minimization for total set-up time and maximization for utilization of roller. Secondly, an algorithm based on constraint satisfaction technique is present with variable selection rules and value selection rules for billet selecting, grouping and sequencing, and constraint propagation for searching-space reducing and rolling-unit partitioning. Meanwhile, the current capacity of online roller is updated dynamically during the solving process of algorithm. Finally, comparing results indicated that the proposed model and algorithm are effective and efficient.

As air conditioning system long-time running, microorganism like bacterium and fungus propagates easily in high humidity environment such as filter equipment. After the microorganism entering indoor environment through air conditioning system, the air quality would be affected seriously. Air conditioning system which is regarded as potential microbial pollution source is becoming more attention. The study was about isolation and identification of fungal microorganisms on the filter surface of the central air conditioning system in a gymnasium, and then researching on the colonies and mycelium grown and reproduce regular of fungal microorganisms in different thermal environment using thermal methods, aim to lay groundwork of propagation and diffusion mechanism study of fungal microorganisms in air conditioning system and effective air microbial contamination solve by thermal methods. By physiology biochemistry experiment and molecular biological identification, it was shown that the dominant fungi were Penicillium spp. and Cladosporium spp., colonies were 600 cfu/cm2 and 140 cfu/cm2 respectively.

Complete video-assisted thoracoscopic surgery (c-VATS) for left upper lobectomy is difficult due to the branching pattern of the left pulmonary artery. Our purpose was to report outcomes of a modified technique of c-VATS left upper lobectomy. We retrospectively compared the outcomes of 83 patients with stage I/II non-small-cell lung cancer (NSCLC) who received left upper lobectomy between 2008 and 2011; 32 underwent conventional c-VATS and 50 received modified c-VATS. In the modified procedure, the order in which hilum of lung was treated was from the lingular segmental artery to the superior pulmonary vein to the bronchus, and then finally the pulmonary artery. The mean patient age was 63.6 ± 8.4 years, and no differences were observed in age, gender, and largest tumor diameter between the two groups. No conversion occurred in either group. With the enlargement of the scope of cloud computing application, the number of users and types also increases accordingly, the special demand for cloud computing resources has also improved. Cloud computing task scheduling and resource allocation are key technologies, mainly responsible for assigning user jobs to the appropriate resources to perform. But the existing scheduling algorithm is not fully consider the user demand for resources is different, and not well provided for different users to meet the requirements of its resources. As the demand for quality of service based on cloud computing and cloud computing original scheduling algorithm, the computing power scheduling algorithm is proposed based on the QoS constraints to research the cloud computing task scheduling and resource allocation problems, improving the overall efficiency of cloud computing system.

This paper describes the image based on dark channel prior to fog. Dark colors from the statistical laws of outdoor priori no fog image database, it was such a key observation was based on the fact that - the vast majority of each local area outdoor image without fog are present strength of certain of at least one color channel low value pixels. Using this model building, directly estimate the concentration of fog and mist removal recover high quality interference image. Experiments show that the dark channel prior to remove the image haze becomes simple and effective.

Aiming at billet stacking problem of the rolling mill, an IBF algorithm is used to assign the billet to the warehouse positions. The utilization of non-empty warehouse positions and shuffles are considered in the algorithm. Computational experiments demonstrate that the proposed algorithm is feasible and effective, competitive and able to find more high-quality solutions for problem instances than artificial method.

Considering the feature of round steel enterprises with the contradiction between mass production size and the multi-variety and small-batch market demands, an order-grouping problem is studied with transportation optimization. Two objectives that to minimize the delivery destination difference and delivery date difference are presented, and an algorithm improved k-means algorithm procedure is proposed to solve the problem. In this algorithm, a method is applied to generate representative initial centers of k-means algorithm for optimization. Experiment results show that the algorithm is feasible and effective.

Patients and Methods: We searched the Cochrane Library, MEDLINE, CENTRAL, EMBASE and manual searches. The meta-analysis was performed to compare overall survival, pattern of failure, and toxicity among the homogeneous studies. Subdivided analyses were also performed. Results: Sixteen studies containing 11540 patients were included in the meta-analysis. Among these studies, 9 were propensity-score matched (PSM) cohort studies, and 7 were cohort studies. Sublobar resection, compared with radiotherapy (either conventional fraction radiation therapy or stereotactic body radiation therapy), significantly improved the overall survival regardless in both PSM and non-PSM analyses (all p 0.05). Conclusions: Sublobar resection was associated with improved outcomes in high-risk elderly patients with Stage I NSCLC, which supports the need to compare both treatments in large prospective, randomized, controlled clinical trials.

The stoppage and lifetime of high speed motorized spindle was related to many influence. A total of 31 statistical lifetime data of imported motorized spindles in cylinder head production line were presented, according to these data, and the average cut operation lifetime could reach 8277 hours, and the 8000 hours of average design precision lifetime was validated. It was found that the main influencing factors of motorized spindle lifetime in initial stage were the original manufacturing defects, also in precision lifetime stage and fatigue lifetime stage, the main influencing factors were the service conditions which exceeded design allowable conditions and own unique structural characteristics of motorized spindle respectively. according to 7 data and GM(1,1) grey theory, the precision lifetime prediction model of motorized spindle was built and the. model error was tested. The reliability of prediction model is validated by 2 inchoate reality observation data which relative differences were 0.57% and 2.03% respectively.
