SUMMARY OBJECTIVE: Early diagnosis and risk stratification may provide a better prognosis in pulmonary embolism (PE). Copeptin has emerged as a valuable predictive biomarker in various cardiovascular diseases. The aim of this study was to determine the levels of copeptin in patients with acute PE and to evaluate its relationship with disease severity and PE-related death. METHODS: Fifty-four patients and 60 healthy individuals were included in this study. Copeptin concentrations and right ventricular dysfunction were analyzed. The correlation between copeptin levels and hemodynamic and echocardiographic parameters was examined. After these first measurements, patients were evaluated with PE-related mortality at the one-year follow-up. Objective : To determine the prevalence of alternative diagnoses based on chest CT angiography (CTA) in patients with suspected pulmonary thromboembolism (PTE) who tested negative for PTE, as well as whether those alternative diagnoses had been considered prior to the CTA. Methods : This was a cross-sectional, retrospective study involving 191 adult patients undergoing CTA for suspected PTE between September of 2009 and May of 2012. Chest X-rays and CTAs were reviewed to determine whether the findings suggested an alternative diagnosis in the cases not diagnosed as PTE. Data on symptoms, risk factors, comorbidities, length of hospital stay, and mortality were collected. Results : On the basis of the CTA findings, PTE was diagnosed in 47 cases (24.6%). Among the 144 patients not diagnosed with PTE via CTA, the findings were abnormal in 120 (83.3%). Such findings were consistent with an alternative diagnosis that explained the symptoms in 75 patients (39.3%). Among those 75 cases, there were only 39 (20.4%) in which the same alterations had not been previously detected on chest X-rays. The most common alternative diagnosis, made solely on the basis of the CTA findings, was pneumonia (identified in 20 cases). Symptoms, risk factors, comorbidities, and the in-hospital mortality rate did not differ significantly between the patients with and without PTE. However, the median hospital stay was significantly longer in the patients with PTE than in those without (18.0 and 9.5 days, respectively; p = 0.001). Conclusions : Our results indicate that chest CTA is useful in cases of suspected PTE, because it can confirm the diagnosis and reveal findings consistent with an alternative diagnosis in a significant number of patients. Objetivo : Determinar a prevalência de diagnósticos alternativos corroborados por angiotomografia computadorizada (angio-TC) de tórax em pacientes com suspeita de tromboembolia pulmonar (TEP) e com resultados negativos para TEP, assim como investigar se esses diagnósticos alternativos já haviam sido identificados antes dos resultados da angio-TC. Métodos : Estudo transversal, retrospectivo, com 191 pacientes adultos que realizaram angio-TC por suspeita de TEP no período entre setembro de 2009 e maio de 2012. As radiografias de tórax e as angio-TCs foram revisadas para determinar se os achados poderiam fornecer um diagnóstico alternativo nos casos não diagnosticados como TEP. Dados sobre sintomas, fatores de risco, comorbidades, tempo de internação e mortalidade foram coletados. Resultados : A angio-TC foi positiva para tromboembolia pulmonar em 47 casos (24,6%). Entre os 144 pacientes sem tromboembolia pulmonar, achados anormais foram observados em 120 (83,3%). Tais achados foram compatíveis com um diagnóstico alternativo que explicava os sintomas do paciente em 75 casos (39,3%). Desses 75 casos, os achados não haviam sido detectados previamente na radiografia de tórax em apenas 39 (20,4%) dos casos. O diagnóstico alternativo mais frequente, identificado somente por angio-TC, foi pneumonia (em 20 casos). Sintomas, fatores de risco, comorbidades e taxa de óbito intra-hospitalar não diferiram significativamente entre os pacientes com ou sem TEP. Entretanto, a mediana de tempo de internação foi significativamente maior nos pacientes com TEP do que naqueles sem TEP (18,0 e 9,5 dias, respectivamente; p = 0,001). Conclusões : Nossos resultados demonstram que a angio-TC de tórax é útil em casos com suspeita de TEP, pois pode confirmar o diagnóstico e evidenciar achados sugestivos de um diagnóstico alternativo em um significativo número de pacientes.

INTRODUÇÃO: O perfil clínico e as estratégias diagnósticas e terapêuticas nos pacientes com embolia pulmonar demonstram a prática clínica na abordagem da doença. Essas informações, escassas nos estudos nacionais, possibilitam melhor conhecimento da embolia pulmonar. MÉTODOS: Estudo multicêntrico de 727 pacientes admitidos em unidades de emergência ou terapia intensiva, com o diagnóstico de embolia pulmonar confirmado por um ou mais dos seguintes exames: arteriografia pulmonar angiotomografia computadorizada helicoidal angioressonância magnética, ecodopplercardiograma, cintilografia pulmonar ou duplex-scan venoso. Dados demográficos, comorbidades, manifestações clínicas e métodos complementares foram analisados. RESULTADOS: A média de idade foi 68 anos, sendo 42% homens. Os fatores de risco mais prevalentes foram: idade > 40 anos, repouso no leito e neoplasia. A dispnéia, taquipnéia, taquicardia, dor torácica, foram as manifestações clínicas mais frequentes. O eletrocardiograma apresentou alterações em 30%, a radiografia de tórax em 45%, o duplex-scan venoso em 69% e o ecodopplercardiograma em 37%. O D-dímero a troponina e a CKMB foram positivos em respectivamente 93, 9 e 8%. Os métodos mais utilizados para o diagnóstico foram: tomografia computadorizada: 47%, duplex-scan venoso: 14% e cintilografia pulmonar: 14%. As formas mais freqüentes de tratamento foram: heparina não fracionada 50%, heparina de baixo peso molecular 30% e trombolítico 12%. A mortalidade intra-hospitalar foi de 19,5%. CONCLUSÕES: Observou-se que a idade > 40 anos, imobilização prolongada e neoplasia foram os fatores de risco de maior prevalência e a dispnéia a apresentação clínica mais freqüente. A angiotomografia computadorizada helicoidal foi o método mais utilizado para o diagnóstico e a heparina não fracionada a principal forma de tratamento. INTRODUCTION: The clinical profile as well as the therapeutic and diagnostic strategies for patients with pulmonary embolism, describes clinical practice in the approach of the disease. Such information, scarce in national studies, enables a better understanding of pulmonary embolism. METHODS: A multicenter trial included 727 patients with pulmonary embolism who were admitted in emergency or intensive care unit. Diagnostic criteria for inclusion were: 1. Visibility of thrombus in the pulmonary artery at pulmonary arteriography, helical computer tomography, magnetic resonance or echocardiogram. 2. High probability at pulmonary scintigraphy. 3. Venous duplex-scan with thrombus and clinical manifestations of pulmonary embolism. Clinical and complementary exams were analyzed. RESULTS: Mean age was 68 years, 42% were male. Most prevalent risk factors were: age>40 years, bed rest and neoplasm. More frequent signs and symptoms were: dyspnea, tachypnea, sinus tachycardia, and chest pain. Changes were observed at electrocardiogram in 30%, at chest X-ray in 45%, at venous duplex-scan in 67%, at transthoracic echocardiogram in 37%. . D-dimer, troponin I and CKMB were positive in, respectively, 93, 9 and 8%. Most frequently used methods to confirm diagnosis were helical computer tomography and non-fractioned heparin was the treatment most used. In-hospital mortality was 19.5%. CONCLUSIONS: It was observed that age>40 years, prolonged rest and neoplasms were the most prevalent risk factors and dyspnea and tachypnea were the more frequent clinical manifestations. Helical computer tomography was the most often used method to confirm diagnosis and non-fractioned heparin was the main form of treatment.

Tromboembolia pulmonar aguda e exacerbação aguda de doença pulmonar obstrutiva crônica são doenças comuns. A doença pulmonar obstrutiva crônica é um fator clínico de risco para tromboembolia pulmonar aguda. As apresentações clínicas da tromboembolia pulmonar aguda e da exacerbação aguda da doença pulmonar obstrutiva crônica freqüentemente mimetizamse tanto que pode ser difícil distingui-las. Anormalidades estruturais nos pulmões com doença pulmonar obstrutiva crônica tornam também difícil a interpretação de testes não invasivos de diagnóstico, como o mapeamento de ventilação e perfusão pulmonares. Assim, diagnosticar tromboembolia pulmonar aguda em pacientes com doença pulmonar obstrutiva crônica é uma tarefa desafiadora. Com o objetivo de atualizar o assunto e oferecer sugestões de conduta, nós avaliamos artigos abordando este tema, incluindo relatos e séries de casos, abordagens diagnósticas de tromboembolia pulmonar aguda e fizemos algumas reflexões. A probabilidade clínica de tromboembolia pulmonar aguda em cenários de doença pulmonar obstrutiva crônica é usualmente intermediária, o mapeamento de ventilação e perfusão pulmonares é predominantemente de probabilidade intermediária e os algoritmos de conduta os deveriam assim considerar. Pulmonary thromboembolism and exacerbation of chronic obstructive pulmonary disease are common conditions. Chronic obstructive pulmonary disease is a clinical risk factor for pulmonary thromboembolism. The presentation of acute pulmonary thromboembolism and acute exacerbation of chronic obstructive pulmonary disease often mimic each other so closely that they cannot be distinguished clinically. The structural abnormalities of the lungs in chronic obstructive pulmonary disease make also difficult to interpret the results of noninvasive tests like ventilation-perfusion lung scans. Therefore, diagnosing acute pulmonary thromboembolism in patients with underlying chronic obstructive pulmonary disease is a challenging task. In order to update knowledge of the subject and offer suggestions regarding conduct, we evaluated various studies addressing this theme, including case reports and case series. In addition, we reviewed diagnostic approaches to acute pulmonary thromboembolism, and we reflect upon that topic. The clinical probability of pulmonary thromboembolism concomitant to chronic obstructive pulmonary disease is typically intermediate, as is positivity on the ventilation-perfusion lung scan. Diagnostic algorithms should take that into consideration.
