BACKGROUND: Obesity is an independent risk factor for esophageal symptoms, gastroesophageal reflux disease (GERD), and motor abnormalities. When contemplating bariatric surgery, patients with obesity type III undergo esophagogastroduodenoscopy (EGD) and also esophageal manometry (EMN), and prolonged pHmetry (PHM) as part of their pre-operative evaluation. OBJECTIVE: Description of endoscopy, manometry and pHmetry findings in patients with obesity type III preparing for bariatric surgery, and correlation of these findings with the presence of typical GERD symptoms. METHODS: Retrospective study in which clinical symptoms of GERD were assessed, focusing on the presence of heartburn and regurgitation. All patients underwent EMN, PHM and most of them EGD. RESULTS: 114 patients (93 females-81%), average age 36 years old, average BMI of 45.3, were studied. Typical GERD symptoms were referred by 43 (38%) patients while 71 (62%) were asymptomatic. Eighty two patients (72% of total) underwent EGD and 36 (42%) evidenced esophageal abnormalities. Among the abnormal findings, hiatal hernia was seen in 36%, erosive esophagitis (EE) in 36%, and HH+EE in 28%. An abnormal EMN was recorded in 51/114 patients (45%). The main abnormality was a hypotensive lower esophageal sphincter (LES) in 32%, followed by ineffective esophageal motility in 25%, nutcracker esophagus in 19%, IEM + hypotensive LES in 10%, intra-thoracic LES (6%), hypertensive LES (4%), aperistalsis (2%) and achalasia (2%). Among the 43 symptomatic patients, 23 (53%) had abnormal EMN and 31/71 asymptomatic cases (44%) also presented this finding (P=0.30). PHM showed abnormal reflux in 60/114 patients (53%), with a predominance of bi-positional reflux (42%), followed by supine reflux (33%) and upright reflux (25%). Abnormal PHM was found in 26/43 symptomatic cases (60%) and also among 34/71 asymptomatic cases (48%) (P=0.19). CONCLUSION: Manometric abnormalities were common in obesity type III patients, the most frequent being hypotensive LES, followed by IEM. Most patients were asymptomatic. There was no correlation between the finding of motor abnormalities and the presence of symptoms. More than half the patients had abnormal reflux at PHM. We found no significant correlation between abnormal reflux and the presence of symptoms. RESUMO CONTEXTO: A obesidade é fator de risco independente para sintomas esofagianos, doença do refluxo gastroesofágico (DRGE) e alterações motoras. Pacientes com obesidade tipo III, candidatos à cirurgia bariátrica foram submetidos a endoscopia digestiva alta (EDA) e também realizaram esofagomanometria (EMN) e pHmetria prolongada (PHM) como parte da avaliação pré-operatória. OBJETIVO: Em um grupo de pacientes com obesidade tipo III em pré-operatório de cirurgia bariátrica, descrever os achados endoscópicos, manométricos e pHmétricos, correlacionando-os com a presença de sintomas típicos de DRGE. MÉTODOS: Estudo retrospectivo, de pacientes com obesidade tipo III, candidatos a cirurgia bariátrica. A avaliação clínica focalizou a presença de sintomas típicos de DRGE (pirose/regurgitação); todos foram submetidos a EMN, PHM e a maior parte à EDA, realizada previamente. RESULTADOS: Foram incluídos 114 pacientes, 93 (81%) do sexo feminino, média de idade de 36 anos e IMC médio de 45,3. Sintomas típicos de refluxo foram referidos por 43 (38%) pacientes e 71 (62%) eram assintomáticos. EDA foi realizada por 82 (72%) pacientes, havendo anormalidades esofagianas em 36 (42%). Entre os anormais, havia hérnia hiatal (HH) em 36%, esofagite erosiva (EE) em 36% e HH + EE em 28%. A EMN foi anormal em 51/114 (45%). Entre os anormais, predominou o esfíncter esofagiano inferior (EEI) hipotenso em 32%, seguido por motilidade esofagiana ineficaz (MEI) em 25%, esôfago em quebra-nozes (19%), EEI hipotenso + MEI (10%), EEI intra-torácico (6%), EEI hipertenso (4%), aperistalse (2%) e acalasia (2%). Dentre os 43 sintomáticos, 23 (53%) apresentavam EMN anormal, sendo que em 31 dos 71 (44%) assintomáticos a EMN também era anormal (P=0,30). A PHM revelou refluxo anormal em 60 (53%) pacientes. Predominou o refluxo anormal biposicional (42%) seguido do refluxo supino (33%) e refluxo ereto (25%). Dentre os 43 pacientes sintomáticos, 26 (60%) apresentavam PHM anormal, sendo que em 34 dos 71 assintomáticos a PHM também era anormal (48%) - P=0,19. CONCLUSÃO: Alterações manométricas foram comuns em obesidade tipo III, sendo as mais frequentes o EEI hipotenso, seguida de motilidade ineficaz. A maioria dos pacientes era assintomática. Mais da metade dos pacientes apresentou refluxo anormal à PHM. Não houve diferença significativa entre o achado de refluxo anormal e a presença de sintomas. Não houve relação entre o achado de alterações motoras e a presença de sintomas.

BACKGROUND: The high-resolution manometry has been a significant advance in esophageal diagnostics. There are different types of catheter and systems devices to capture esophageal pressures that generate variable data related to Chicago Classification (CC) and consequently influence normal values results. There are not normative data for the 24-channel water-perfused high-resolution manometry system most used in Brazil with healthy volunteers in supine posture. OBJECTIVE: To determine manometric esophageal normative values for a 24-channel water-perfused high-resolution manometry catheter in supine posture using healthy volunteers according to CC 3.0 parameters. METHODS: A total of 92 volunteers with no gastrointestinal symptoms or medications affecting GI motility underwent esophageal high-resolution manometry by standard protocol. Age, gender and manometry parameters analyzed using Alacer software were collected. The median, range, and 5th and 95th percentiles (where applicable) were obtained for all high-resolution manometry metrics. Normal value percentiles were defined as 95th integrated relaxation pressure, 5th-100th distal contractile integral, and 5th distal latency. RESULTS: The mean age was 40.5±13.2 years. Our normative metrics were integrated relaxation pressure 4 cm). For EGJ-CI the range 5th-95th was 21.7-86.9 mmHg.cm.s. CONCLUSION: This is the first report of normative data for the 24-channel water-perfused system in supine posture. It revealed higher integrated relaxation pressure and distal latency duration which suggest the need to change CC 3.0 cutoffs for this system. Background: Multiple water swallow is increasingly used as a complementary challenge test in patients undergoing high-resolution manometry (HRM). Our aim was to establish the range of normal pressure responses during the rapid drink challenge test in a large population of healthy subjects. Methods: Pressure responses to a rapid drink challenge test (100 or 200 mL of water) were prospectively analyzed in 105 healthy subjects studied in nine different hospitals from different countries. Esophageal motility was assessed in all subjects by solid-state HRM. In 18 subjects, bolus transit was analyzed using concomitant intraluminal impedance monitoring. Key Results: A virtually complete inhibition of pressure activity was observed during multiple swallow: Esophageal body pressure was above 20 mm Hg during 1 (0-8) % and above 30 mm Hg during 1 (0-5) % of the swallow period, and the pressure gradient across the esophagogastric junction was low (-1(-7 to 4) mm Hg). At the end of multiple swallow, a postswallow contraction was evidenced in only 50% of subjects, whereas the remaining 50% had non-transmitted contractions. Bolus clearance was completed after 7 (1-30) s after the last swallow, as evidenced by multichannel intraluminal impedance. Conclusions & Inferences: The range of normal pressure responses to a rapid drink challenge test in health has been established in a large multicenter study. Main responses are a virtually complete inhibition of esophageal pressures with a low-pressure gradient across esophagogastric junction. This data would allow the correct differentiation between normal and disease when using this test.

CONTEXT: Achalasia is a primary esophageal motor disorder secondary to the degeneration of ganglion cells of the inhibitory intramural myenteric plexus. It affects both sexes similarly and has two peaks of incidence, one in the 3rd to 4th decades of life and the other after 60 years of age. The effect of age on esophageal motility of patients with achalasia is not well known. Studies have shown that healthy older people, when compared to the young, have: a) a lower number of ganglion cells in the intramural myenteric plexus; b) a reduced normal relaxation of the lower esophageal sphincter; and c) a reduced esophageal peristalsis. Thus, as both age and achalasia can produce comparable degenerative changes in the intramural myenteric plexus, it is possible that advanced age could be an important factor in enhancing the clinical and manometric abnormalities commonly found in patients with achalasia. OBJECTIVES: To compare the clinical, radiological and manometric findings in young as compared to elderly (>60 years old) achalasia patients. METHODS: A retrospective study of a group of patients with untreated achalasia separated into young and elderly patients. CONTEXT: Breathing and swallowing coordination, despite the expressive number of study, remain as theme deserving further research. OBJECTIVE: To identify a coordination pattern between swallowing and the natural breathing pause that occur in association with it (swallowing apnea) and also the relevance of the vocal folds closure in this process. METHODS: Sixty-six adults, male and female, including normal health people, post-laryngectomy individuals and patients with digestive complaints without dysphagia were analyzed. The respiratory air flux interruptions produced by wet requested swallows and dry, requested and spontaneous swallows, were registered using thermo and piezoelectric receptors coupled to synectics medical manometry equipment, using Polygram upper 4.21 software. The results were analyzed with the Chi-square (3×2) and (2×2) nonparametric independency test with P = 0.05. RESULTS: Swallowing apnea is a preventive breathing stop that start just before and stay present during all deglutition pharyngeal phase. It is a well coordinated phenomena that occur as pattern in association with low elastic resistance of the lung, on the expiratory final phase until inspiration initial phase. This breathing stoppage it is usually followed by a short expiraton preceding a new breathing cycle. The swallow apnea and vocal folds closure are both independents mechanisms. CONCLUSION: It is possible to suppose that in the subconscious condition, swallowing apnea is integrated under coordination of the same control mechanism that also involves the elastic resistance of the lung.CONTEXTO: Apesar do expressivo número de estudos sobre a coordenação da respiração com a deglutição, o tema permanece aberto à pesquisa. OBJETIVO: Identificar um padrão de coordenação entre a pausa respiratória e a deglutição que ocorre em associação a esta usual apneia (apneia de deglutição) e estabelecer a importância do fechamento das pregas vocais que ocorre em associação a esta apneia. MÉTODOS: Foram estudados 66 adultos de ambos os sexos, incluindo voluntários sadios, indivíduos laringectomizados e pacientes com queixas digestivas sem disfagia. Apneias produzidas em associação com deglutições de liquido a pedido e deglutições a seco, solicitadas e espontâneas, foram registradas a partir de informações captadas por termo receptor e receptor piezelétrico acoplados a equipamento de manometria da Synectics Medical usando o software Polygran upper 4.21. Os resultados foram submetidos a teste de independência não paramétricos qui ao quadrado (3x2) e (2x2), com P = 0.05. RESULTADOS: A apneia de deglutição é uma pausa respiratória preventiva que se inicia imediatamente antes e permanece ativa durante toda a fase faríngea da deglutição. Esta apneia é um bem coordenado fenômeno padrão que ocorre em associação com a baixa resistência elástica dos pulmões, presentes nas fases final da expiração, até a inicial da inspiração. Esta pausa respiratória é usualmente seguida de uma curta expiração que antecede novo ciclo respiratório. A apneia de deglutição e o fechamento das pregas vocais são mecanismos independentes. CONCLUSÕES: É possível supor que em atividade subconsciente, a apneia de deglutição seja integrada sob a coordenação do mesmo mecanismo de controle que também envolve a resistência elástica dos pulmões.

“Nutcracker esophagus” (NE) is a primary esophageal motor disorder, first described in patients with noncardiac chest pain. In recent years NE has been associated with gastroesophageal reflux disease (GERD). In this study we compare patients with NE with and without GERD, as defined by pHmetry or endoscopy, with respect to clinical, endoscopic, radiologic, and manometric findings. Fifty-two patients with NE were studied. They were divided in two groups: GERD (17–32.6%) and non-GERD (35–67.4%) patients. Females predominated in both groups, with no significant difference in age (p > 0.05). Chest pain was the chief complaint in both groups (p > 0.05). Clinical findings in patients with and without reflux included chest pain (52.9% and 51.4%), dysphagia (58.8% and 42.8%), and heartburn (64.7% and 42.8%), followed by regurgitation, dyspepsia, ear, nose, and throat (ENT) complaints, respiratory symptoms, and odynophagia (p > 0.05). Erosive esophagitis was found in three patients (5.7%). There were no differences between groups in the findings of barium swallow studies and all manometric findings were similar for both groups (p > 0.05). We conclude that there were no differences in patients with NE with or without associated reflux disease. It is important to diagnose reflux properly so patients can be treated adequately.

RACIONAL: Nos últimos anos, estudos têm demonstrado a importância da hérnia hiatal na etiopatogenia da doença do refluxo gastroesofágico, atuando por vários mecanismos, sendo enfatizado que quanto maior a hérnia, maior seria a possibilidade de refluxo e esofagite. OBJETIVOS: Avaliar por parâmetros de pHmetria prolongada, se a presença de hérnias volumosas se correlaciona com maior intensidade do refluxo, em pacientes com a doença do refluxo erosiva e doença do refluxo não-erosiva. PACIENTES E MÉTODOS: Foram revistas as pHmetrias prolongadas anormais consecutivas de pacientes em investigação de doença do refluxo gastroesofágico (pirose como queixa principal) e analisadas as percentagens de tempo total (%TT), em posição ereta (%TE) e posição supina (%TS) com pH 5 cm. RESULTADOS: Cento e noventa e dois pacientes preencheram os critérios de inclusão, sendo 115 com doença do refluxo erosiva e 77 com doença do refluxo não-erosiva. No primeiro grupo, 94 (81%) pacientes apresentavam hérnias hiatais não-volumosas, enquanto que 21 (19%) apresentavam hérnias hiatais volumosas. No grupo com doença do refluxo não-erosiva, 66 (85%) pacientes apresentavam hérnia hiatal não-volumosa e 11 (15%) hérnia hiatal volumosa. Na doença do refluxo erosiva, as %TT, %TE e %TS foram de 13,1 + 7,1, 13,4 + 7,4 e 12,3 + 11,5 nas hérnias hiatais não-volumosas, aumentando para 20,2 + 12,3, 17,8 + 14,1 e 20,7 + 14,1 nas hérnias hiatais volumosas, respectivamente, sendo este aumento estatisticamente significante nos tempos total e supino. Na doença do refluxo não-erosiva, as %TT, %TE e %TS foram de 9,6 + 4,8, 10,8+ 6,8 e 8,6 + 7,3 nas hérnias hiatais não volumosas e de 14,6 + 13,3, 11,2 + 7,5 18,1 + 21,0 nas hérnias volumosas, respectivamente, com significância semelhante à anterior. CONCLUSÃO: As hérnias volumosas aumentam o tempo de exposição ácida esofágica exclusivamente na posição supina nos pacientes com doença do refluxo erosiva e doença do refluxo não-erosiva.

RACIONAL: As dores de origem esofagiana e coronariana são bastante semelhantes do ponto de vista clínico, havendo necessidade de exclusão desta última, que ocasiona risco de morte. A investigação esofagiana tradicional de pacientes com dor torácica de origem indeterminada, envolve emprego de endoscopia digestiva alta, esofagomanometria e pHmetria esofagiana prolongada. Esses métodos, embora de grande importância diagnóstica, muitas vezes, revelam alterações, em sua maioria, potenciais para a origem da dor. Os testes provocativos de dor esofagiana, ao reproduzirem-na em laboratório, apontam com segurança a sua origem. OBJETIVOS: Determinar a positividade dos testes de perfusão ácida, do edrofônio e da distensão esofagiana com balão em pacientes com dor torácica de origem indeterminada, e correlacionar os resultados com os testes habitualmente empregados, estabelecendo o ganho no diagnóstico da dor esofagiana comprovada. RESULTADOS: Estudaram-se 40 pacientes com dor torácica de origem indeterminada (angiografia coronária normal), sendo 80% do sexo feminino e média de idade de 54 anos. A endoscopia digestiva alta revelou esofagite erosiva em dois pacientes (5%) e úlcera péptica em um (2,4%); a esofagomanometria foi anormal em 60%; a pHmetria prolongada foi anormal em 14 (35%), com índice de sintomas positivo em 7. A dor foi considerada de origem esofagiana comprovada (índice de sintomas positivo à pHmetria) em 7 (17,5%) pacientes e 19 (47,5%) com origem esofagiana provável (8 por doença do refluxo gastroesofágico e 11 por distúrbios motores). Em 14 (35%) a origem da dor não foi demonstrada. O teste de Bernstein foi positivo em 10 (25%), o teste do edrofônio em 8 (20%) e o teste do balão em 15 (37,5%), sendo que 23 pacientes apresentaram, pelo menos, um teste provocativo positivo (57,5%). Com a adição dos testes provocativos foi possível apontar a dor como de origem esofagiana comprovada em 12 dos 19 pacientes (63,1%) em que a dor era provável e em 6 dos 14 pacientes (42,8%) com exames habitualmente empregados normais ou inconclusivos [ganho diagnóstico de 45% (18/40)]. Dois pacientes com testes provocativos negativos apresentaram o índice de sintomas positivo à pHmetria, totalizando 25 (62,5%) pacientes com dor esofagiana comprovada. CONCLUSÃO: Os testes provocativos permitiram apontar a dor como de origem esofagiana comprovada em 62,5% dos casos, o que representou um ganho diagnóstico de 45% quando comparados aos exames habitualmente empregados, constituindo ferramenta importante na investigação de pacientes com dor torácica de origem indeterminada.

OBJECTIVE Patients with gastroesophageal reflux disease (GORD) frequently have oesophageal motility disturbance. However, detailed data about bolus transport characteristics in these patients are still lacking. In the present study the new technology of concurrent impedance manometry was applied for characterization of oesophageal motor function in patients with mild GORD. METHODS Oesophageal motility testing was performed in 25 patients with mild GORD (group 1) as compared to 25 healthy subjects (group 2) employing the technique of concurrent impedancometry and manometry. Oesophageal motility as well as patterns and parameters of bolus transport after the swallowing of saline or yogurt were analysed. RESULTS According to manometry the velocity of the contraction wave was similar in both groups. Mid-distal contraction amplitude in group 1 was still in the normal range but significantly lower than in group 2 (57.4+/-4.5 mmHg vs 91.4+/-7.5 mmHg for saline, and 47+/-4.1 vs 80.7+/-9.4 mmHg for yogurt). According to impedance measurements, bolus transport was significantly slower (3.6+/-0.1 vs 4.0+/-0.1 cm/s for saline and 3.0+/-0.1 vs 3.2+/-0.1 cm/s for yogurt), and post-deglutitive impedance was significantly lower in group 1: 2110 omega+/-116 omega versus 2542 omega+/-152 omega (P

In GERD patients, ineffective esophageal motility (IEM), a hypocontractile disorder, is the most common motor abnormality. IEM has been associated with reflux in both the supine and upright position, prolonged esophageal clearance, and delayed of bolus transport. IEM has been equally present in erosive and in nonerosive GERD.Considering that reflux has been found to be more severe in erosive GERD than in nonerosive GERD patients and that IEM delays esophageal clearance, our hypothesis is that patients with erosive GERD have more severe IEM than those with nonerosive disease.A retrospective review of consecutive manometries of patients with the chief complaint of heartburn and a diagnosis of IEM were performed, and patients with both erosive and nonerosive GERD were selected. According to the number of ineffective contractions, IEM was stratified into three groups: 30% to 40%, mild; 50% to 60%, moderate; and greater than 60%, severe. We also registered the number of low amplitude, failed, and normal waves in each manometry of both groups. We evaluated 110 patients: 70 (64%) with erosive GERD and 40 (36%) with nonerosive GERD. The percentage of mild, moderate and severe IEM was similar in erosive and in nonerosive GERD patients, as well the number of low amplitude, failed or normal waves (P0.5).There were no differences between the severity of IEM in erosive and in nonerosive GERD patients.

Racional -- A disfagia é um dos principais sintomas das doenças do esôfago e sua caracterização é importante para formulação de hipóteses diagnósticas e condução da propedêutica. A anamnese freqüentemente permite diferenciar as disfagias orgânicas das funcionais, porém a literatura carece de dados caracterizando este sintoma entre os diferentes distúrbios motores do esôfago. Objetivo - Avaliar se é possível a distinção entre os diferentes distúrbios motores diagnosticados pela esofagomanometria, tomando-se por base as características da disfagia. Métodos - Foram analisadas de forma retrospectiva, as características da disfagia (relação com a consistência do bolo alimentar, caráter e localização) em 133 pacientes que realizaram esofagomanometria para esclarecimento diagnóstico. Resultados - A esofagomanometria foi anormal em 85% dos pacientes. As características da disfagia foram comparadas nos grupos de pacientes com acalásia, alterações espáticas do esôfago, distúrbios motores inespecíficos e exame normal. A distinção precisa entre os diferentes diagnósticos não foi possível, porém algumas características da disfagia puderam apontar para um ou outro grupo. Assim, nos pacientes com acalásia, a disfagia concomitante para sólidos e líquidos, de caráter constante e de localização baixa foi mais prevalente em relação a todos os outros grupos. Disfagia intermitente foi encontrada com maior freqüência nos portadores de alterações espáticas. As características da disfagia dos distúrbios motores inespecíficos se assemelharam às observadas nos pacientes com exame manométrico normal, sendo freqüentemente referida na região cervical. Conclusões - As características da disfagia podem auxiliar no diagnóstico presuntivo dos distúrbios motores, porém o estudo manométrico é necessário para o diagnóstico definitivo.

O esôfago em quebra-nozes é uma anormalidade manométrica, incluída entre os distúrbios motores primários do esôfago, caracterizada por ondas peristálticas que atingem elevada amplitude em esôfago distal, descrita inicialmente em pacientes com dor torácica não-cardíaca. Embora trabalhos posteriores tenham registrado o esôfago em quebra-nozes em pacientes com disfagia e, mais recentemente, o associado à doença do refluxo gastroesofágico, há bastante controvérsia em relação ao seu verdadeiro significado, sendo escassos os estudos clínicos envolvendo grande número de pacientes. Noventa e sete pacientes com o diagnóstico manométrico de esôfago em quebra-nozes, definido a partir de um grupo controle assintomático, foram estudados retrospectivamente quanto às características clínicas e propedêutica esofagiana, que incluiu endoscopia digestiva alta, esofagografia convencional e pHmetria esofagiana prolongada. Houve predomínio do sexo feminino (63,9 %), com média de idade de 54,3 anos. A queixa mais freqüente que determinou a realização do exame manométrico foi dor torácica, seguida de disfagia e pirose. As manifestações clínicas como um todo foram dor torácica (53,6%), disfagia (52,6%), pirose (52,6%), regurgitação (21,6%), queixas otorrinolaringológicas (15,4%), dispepsia (15,4%) e odinofagia (4,1%). A maior parte dos pacientes apresentou sintomas múltiplos, sendo queixa única observada em 28% dos mesmos. A endoscopia digestiva alta demonstrou esofagite erosiva em 8% dos pacientes e a esofagografia convencional revelou distúrbio motor esofagiano em 16,4%. A pHmetria esofagiana prolongada diagnosticou refluxo gastroesofagiano anormal em 41,2% dos exames realizados. Concluiu-se que, controvérsias à parte, são encontradas outras queixas além de dor torácica e disfagia associadas ao esôfago em quebra-nozes e que é importante definir a associação com a doença do refluxo gastroesofágico para orientar a conduta terapêutica.

Estudo do esvaziamento gástrico de 21 voluntários sadios, usando dieta padrão composta por homogeneizado de lipídios, carboidratos, proteínas e solução de sulfato de bário. Efetuaram-se, para cada voluntário, seis registros videofluoroscópicos em incidência póstero-anterior, com duração de um minuto, a intervalos de 12 minutos na primeira hora e, a seguir, a cada 30 minutos, até o esvaziamento total. Oito exames foram selecionados para a quantificação do esvaziamento. A análise da seqüência de registros videofluoroscópicos permitiu observar e estimar, de modo subjetivo, o comportamento funcional e o ritmo do esvaziamento gástrico. A digitalização de imagens representativas dos diversos tempos do esvaziamento gástrico permitiu quantificar as observações e estabelecer uma curva índice deste. A quantificação foi realizada com o uso de rotina desenvolvida no software Image pro-plus da Media Cybernetics. Conclusões - 1. A videofluoroscopia permitiu, com adequada resolução, a análise qualitativa da motilidade e do esvaziamento gástrico. 2. É possível quantificar as variações de forma que se observa na seqüência de imagens registradas pelo método videofluoroscópico. 3. Os resultados expressam um índice de esvaziamento gástrico, determinado pela variação temporal da projeção da área gástrica em imagens radiológicas obtidas em incidência póstero-anterior. 4. A análise quantitativa não deve prescindir da análise qualitativa. We studied the gastric emptying of 21 healthy volunteers using a standard contrasted diet composed of a lipid, carbohydrate, protein and barium sulfate solution homogenate. We did six one-minute videofluoroscopic recordings in a 12-minute interval for the first hour, and in a 30 minute interval until the complete emptying. We selected eight exams for the emptying quantification. We could observe and estimate subjectively the functional behavior and the gastric emptying. Digital representative images of each emptying time point we were able to quantify those observations and describe them as a mathematical function. We developed a macro procedure using the Media Cybernetics ImagePro-Plus software for this quantification. We conclude that: 1 - The videofluoroscopic method provides better resolution than the other methods in the analysis of the gastric motility and emptying. 2 - It is possible to computationally quantify the morphological differences observed with the sequential registration radiological methods, specially the videofluoroscopic method that generates 30 frames per second. 3 -- Our results generate a gastric emptying index, based on the area of a gastric projection in radiological images obtained in a standing position over a one-hour period (T1/2). 4 -- The quantitative analysis should not exclude the qualitative analysis.

CONTEXT: Eosinophilic esophagitis is an entity characterized by an esophageal inflammatory infiltrate of eosinophils, manifested by dysphagia, intermittent food impactions and symptoms similar to gastroesophageal reflux disease (GERD), that predominantly affects young adults. There may be association of eosinophilic esophagitis with GERD, and motor abnormalities have been described. OBJECTIVE: The main objectives of this study are to describe the findings at esophageal manometry and pH monitoring in patients with eosinophilic esophagitis. METHODS: Cross-sectional study of 20 patients with a diagnosis of eosinophilic esophagitis, submitted to esophageal manometry and 24h pH monitoring. Were analysed the manometric changes and the presence of abnormal reflux on pH monitoring. RESULTS: Twenty patients (15 men, 5 women) had a mean age of 29 years. Motility disorders were found in 25% (5/20) patients with ineffective esophageal motility being the most common finding. pH monitoring revealed abnormal reflux on 25%, without any relationship with manometric findings. CONCLUSIONS: Manometric abnormalities were observed in 25% of patients and abnormal reflux on pH monitoring also in 25%. This study showed no relationship between abnormal reflux and the presence of manometric changes. CONTEXTO: A esofagite eosinofílica é uma doença inflamatória crônica, caracterizada por infiltrado eosinofílico no esôfago e se manifesta por disfagia, impactações alimentares e sintomas similares aos da doença do refluxo gastroesofágico (DRGE), com maior incidência em adultos jovens. Pode haver associação da esofagite eosinofílica com a DRGE, e anormalidades motoras têm sido descritas. OBJETIVO: Os principais objetivos deste estudo são descrever as alterações manométricas e a presença de refluxo anormal à pHmetria esofágica em pacientes com esofagite eosinofílica. MÉTODOS: Estudo transversal de 20 pacientes com diagnóstico de esofagite eosinofílica, submetidos a esofagomanometria e pHmetria esofagiana de 24 h. Foram analisadas as alterações manométricas e a presença de refluxo anormal à pHmetria. RESULTADOS: Vinte pacientes (15 homens, 5 mulheres) com média de idade de 29 anos. Distúrbios da motilidade esofagiana foram encontrados em 25% dos pacientes, com predomínio da motilidade esofagiana ineficaz. A pHmetria revelou refluxo anormal também em 25%, sem relação entre os achados manométricos e pHmétricos. CONCLUSÕES: Anormalidades manométricas foram encontradas em 25% dos pacientes e refluxo anormal à pHmetria também em 25%. Neste estudo, não houve relação entre refluxo anormal e a presença de alterações à esofagomanometria.

CONTEXT: Barrett's esophagus (BE) is characterized by intestinal metaplasia in the distal esophagus and is classified as short-segment (3 cm - LSSBE). It is suggested that LSSBE is associated with more severe esophageal motor abnormalities and increased acid exposure time than SSBE. OBJECTIVE: To evaluate the prevalence of esophageal manometriy abnormalities and acid exposure times in patients with SSBE and LSSBE. METHODS: Barrett's esophagus patients identified by upper endoscopy and confirmed by histopathology were, retrospectively, reviewed and divided into two groups: SSBE and LSBE. Demographic data, symptom duration, prevalence of hiatal hernia, lower esophagus sphincter basal pressure, prevalence of esophageal body abnormalities and acid exposure times were evaluated. RESULTS: Forty-six patients with SSBE (24 males - 52.2%, mean age of 55.2 years) and 28 patients with LSBE (18 males - 64.3%, mean age of 50.5 years). Mean symptom duration was 9.9 years for SSBE and 12.9 years for LSSBE. Hiatal hernia was present in 84.2% of SSBE, 96.3% of LSBE; average lower esophagus sphincter pressure in SSBE 9.15 mm Hg, in LSBE 6.99 mm Hg; lower esophagus sphincter hypotension in SSBE was 65.9%, in LSSBE 82.1%; aperistalsis in SSBE 6.5%, LSSBE 3.6%; mild/moderate ineffective esophageal motility in SSBE 34.8%, LSBE 46.4%; severe moderate ineffective esophageal motility in SSBE 10.9%, LSBE 7,1%; nutcracker esophagus/segmental nutcracker esophagus in SSBE 8.6%, LSBE 0%; normal body in SSBE 39.1%, in LSBE 42.9%, no statistical difference for any of these values (P

CONTEXT: Weight loss is commonly recommended as a treatment for gastroesophageal reflux; however, a relationship between excessive body weight and gastroesophageal reflux disease is not well established. OBJECTIVES: To evaluate the prevalence of obesity and over-weight in patients with heartburn, with and without erosive esophagitis; to analyze the prevalence of hiatal hernia and the intensity of abnormal gastroesophageal reflux in both groups of patients, and its relation to body mass index. METHODS: The data of pH monitoring of 362 individuals with heartburn were evaluated retrospectively. All patients had an upper gastrointestinal endoscopy and erosive esophagitis was defined by the presence of macroscopic erosion on the esophageal mucosa. Hiatal hernia was considered when the gastroesophageal junction was positioned 2 cm or more above the diaphragm. Patients with Barrett's esophagus or esophageal peptic stenosis were excluded. The population was categorized according to body mass index as normal weight (body mass index between 20 and 24.9); over-weight (between 25 and 29.9), and obese (greater than 30). The diagnosis as well as the intensity of abnormal gastroesophageal reflux were obtained through the analysis of the results of pH monitoring in patients with and without erosive esophagitis and in the various categories of body mass index. RESULTS: Among the 362 patients there were 148 (41%) with erosive esophagitis and 214 (59%) without erosive esophagitis, while the pH monitoring was abnormal in 100% and 57%, respectively. Among the 148 individuals with erosive esophagitis (61% male, median age 50 years), 41 (28%) had normal weight, 82 (55%) had over-weight and 25 (17%) were obese. There were 88 (60%) patients with hiatal hernia, which was present in 29 (71% of patients with normal weight), 45 (55% of patients with over-weight) and 14 (56% of obese individuals). In 121 patients without erosive esophagitis who had abnormal pH monitoring, diagnosed as non erosive reflux disease (38% male, median age 50 years), 51 (42%) patients had normal weight, 55 (46%) had over-weight and 15 (12%) were obese. Hiatal hernia was detected in 52 out of 121 (43%) patients and in 21 (41%) out of 51 individuals with normal weight, 24 (44%) of over-weight and 7 (47%) of obese. In the group of 93 patients without erosive esophagitis and normal pH monitoring (29% male, median age 43 years), 26 (28%) had hiatal hernia and there were 43 (46%) individuals with normal weight, 38 (41%) with over-weight and 12 (13%) were obese. The number of patients with obesity and over-weight was significantly higher in the group with erosive esophagitis compared to the ones without erosive esophagitis. The prevalence of hiatal hernia was also superior in the erosive reflux disease patients. The reflux intensity and the prevalence of hiatal hernia were similar in patients with normal weight, over-weight and obesity in the group with erosive reflux disease and non-erosive reflux disease. CONCLUSION: There was a greater prevalence of obesity and over-weight in the group of patients with erosive esophagitis compared to patients with non-erosive reflux disease. There was no difference in reflux intensity measurements in any of the body mass index categories, in both groups. Although there was a major prevalence of hiatal hernia in the group of erosive reflux disease patients, this superiority was not extended to the categories of excessive weight in both groups.

RACIONAL: As estenoses benignas de esôfago são complicações decorrentes de diversas causas. Possuem tratamentos similares, na maioria dos casos necessitando de dilatação endoscópica, no entanto a resposta terapêutica, tempo ideal de tratamento, assim como intervalo entre as sessões podem ser variáveis. OBJETIVO: Analisar, do ponto de vista endoscópico, as estenoses benignas de esôfago em 14 anos de experiência no Hospital Universitário Clementino Fraga Filho da Universidade Federal do Rio de Janeiro, RJ, avaliando etiologia, a extensão da estenose, o número de dilatações necessário para atingir resposta terapêutica satisfatória, assim como a relação entre a extensão da estenose e a resposta terapêutica. MÉTODO: Foram analisadas 2.568 dilatações endoscópicas com uso de velas de Savary-Gilliard em 236 pacientes, durante um período de 14 anos e 10 meses, até junho de 2007. RESULTADOS: A estenose péptica foi a causa mais freqüentemente encontrada, seguida pela estenose cáustica. As estenoses longas e cáusticas necessitaram de maior número de sessões para ausência de disfagia. Estenoses pépticas e curtas responderam melhor a número menor de sessões de dilatação. CONCLUSÃO: A estenose péptica foi a causa mais comum e respondeu bem à terapia endoscópica, em concordância com a literatura. As estenoses cáusticas foram as mais refratárias, principalmente as longas. Quanto maior foi a extensão da estenose, também maior foi o número de sessões necessárias. Estenoses curtas apresentaram boa evolução na maioria dos casos. O número de dilatações necessárias dependeu diretamente da causa e da extensão da estenose. BACKGROUND: Benign esophageal strictures are complications that result from different causes. They are usually similarly approached, most of the cases needing endoscopic dilation. However the response to therapy, optimal timing for treatment and interval between sessions can vary. AIM: The authors evaluate 14 years of experience with benign stricture of the esophagus from the endoscopic point of view in the "Clementino Fraga Filho" University Hospital, Federal University of Rio de Janeiro, RJ, Brazil. They evaluated etiology, length of stricture, number of dilations needed to reach satisfactory therapeutic response, and the relation between length of stricture and therapeutic response. METHODS: We analyzed 2,568 endoscopic dilations using Savary-Gilliard dilators in 236 patients. The follow up period was 14 years and 10 months, until June of 2007. RESULTS: Peptic strictures were the more frequent, followed by caustic strictures. Long strictures and caustic strictures needed more sessions to abolish dysphagia. Peptic strictures and short ones had better response to a smaller number of sessions. CONCLUSION: In this study, peptic strictures were the commonest etiology and responded best to endoscopic therapy, in accordance with published literature. Caustic strictures were the most refractory, mainly the long segments. The longer the extension of stenosis, the greater was the number dilation sessions needed for relief. Short strictures had a good prognosis in the great majority of cases. The number of dilations depended directly on the etiology and the extension of the stricture.

RACIONAL: Na doença do refluxo gastroesofágico pode-se encontrar, como parte do quadro clínico da doença, queixas respiratórias, otorrinolaringológicas, dor torácica ou disfagia. As duas primeiras são intituladas de manifestações supra-esofágicas da doença. É discutível a prevalência destas alterações em pacientes com doença do refluxo gastroesofágico nas formas erosiva e não-erosiva. OBJETIVOS: Avaliar a prevalência de manifestações supra-esofágicas em pacientes com doenças do refluxo erosiva e não-erosiva. MÉTODOS: Foram revistas as fichas de pacientes que realizaram endoscopia digestiva alta, esofagomanometria e pHmetria prolongada nos últimos 5 anos, na investigação de doença do refluxo gastroesofágico (pirose como queixa principal), em que havia informações a respeito de queixas respiratórias e otorrinolaringológicas. Foram selecionados pacientes com doença do refluxo erosiva (graus I a III, pela classificação de Savary-Miller) e com doença do refluxo não-erosiva (endoscopia negativa, com pHmetria prolongada anormal). A análise estatística utilizou o teste de qui-quadrado. RESULTADOS: Duzentos e oitenta pacientes preencheram os critérios de inclusão, sendo 162 com doença do refluxo erosiva (70% com esofagite grau I) e 118 com doença do refluxo não-erosiva. No grupo total, 88 apresentavam queixas otorrinolaringológicas (31%), com predomínio de rouquidão e pigarro e 42, queixas respiratórias (15%), predominando a tosse crônica. Dos pacientes com queixas otorrinolaringológicas, 45 pertenciam ao grupo doença do refluxo erosiva (28%) e 43 ao grupo doença do refluxo não-erosiva (36,4%). Em relação aos com queixas respiratórias, 21 pacientes (13%) pertenciam ao grupo doença do refluxo erosiva e 21 (18%) ao grupo doença do refluxo não-erosiva. CONCLUSÃO: Não houve diferença entre a prevalência das queixas supra-esofágicas em pacientes com doenças do refluxo erosiva e não-erosiva.

It has been gradually accepted that esophageal diverticula result from esophageal motor disorders rather than from primary anatomic abnormalities. Twenty-seven patients with these diverticula were evaluated with respect to pathogenesis, clinical aspects, diagnostic tests, therapy, and natural history for a mean of 27 months of followup. Thirteen diverticula were midesophageal, 11 were situated in the distal third of the esophagus, and 3 were in both regions. Esophageal dysmotility was observed in 85% of patients. Specific esophageal motor disorders were more frequent in association with diverticula of the distal third than in midesophageal diverticula, suggesting that they result from a pulsion mechanism. Traction was the possible mechanism in 27% of midesophageal diverticula. Endoscopic esophagitis was seen in one patient and abnormal acid reflux in 25% of the cases, mainly in patients with distal diverticula. Distal diverticula presented with more severe symptoms than did midesophageal diverticula, and 27% of those required surgical treatment. Patients with midesophageal diverticula seem to have a better prognosis than those with more distal disease.

BACKGROUND: Portal hypertension in patients with liver cirrhosis causes manifestations such as esophageal varices, ascites and edema. Some studies have been conducted about the role of esophageal varices in the development of esophageal motor disorders and abnormal gastroesophageal reflux in these patients. Ascites could be a factor promoting gastroesophageal reflux and it has been questioned whether reflux would favor the rupture of varices. However there are a few studies using ambulatory esophageal pH recording in the evaluation of these patients. AIMS: Evaluate gastroesophageal reflux by pH recording in cirrhotic patients with esophageal varices and possible predictors. METHODS: Fifty one patients (28 men, 23 women, mean age of 54 years) with liver cirrhosis, diagnosed by clinical, laboratorial, image and histological findings were prospectively evaluated. All patients had esophageal varices confirmed by endoscopy and were submitted to a questionnaire about typical gastroesophageal reflux disease symptoms (heartburn and or acid regurgitation). pH recording was performed with the probe placed 5 cm above the superior lower esophageal sphincter limit, as determined by manometry. Abnormal reflux (% total time with pH < 4 >4.5%) was related to the size of varices, congestive gastropathy, ascites, severity of cirrhosis and typical gastroesophageal reflux disease symptoms. RESULTS: The caliber of the varices was considered to be small in 30 patients (59%), medium in 17 (33%) and large in 4 (8%), 21 (41%) congestive gastropathy. Ascites was observed in 17 (33%), 32 patients (63%) were classified as Child-Pugh A, 17 (33%) Child-Pugh B and 2 (4%) Child-Pugh C. Twenty seven patients (53%) presented with typical gastroesophageal reflux disease symptoms. Abnormal reflux at pH recording was found in 19 patients (37%). One of them presented with erosive esophagitis at endoscopy. There was no relation between ascites, variceal size, congestive gastropathy and Child-Pugh score and abnormal reflux. There was a correlation between typical gastroesophageal reflux disease symptoms and abnormal reflux. CONCLUSION: Abnormal gastroesophageal reflux was found in 37% of the patients with hepatic cirrhosis and esophageal varices. Only typical gastroesophageal reflux disease symptoms predicted these findings. RACIONAL: A hipertensão porta que acomete os pacientes com cirrose hepática é causa de varizes de esôfago, ascite e edema. Alguns estudos têm sido realizados para avaliar a importância das varizes de esôfago no desenvolvimento dos distúrbios motores esofagianos e do refluxo gastroesofágico anormal neste grupo de pacientes. A ascite pode ser um fator promotor de refluxo gastroesofágico e tem sido questionado se o refluxo anormal poderia favorecer a rotura das varizes de esôfago. Entretanto, são poucos os estudos que utilizam a pHmetria esofagiana prolongada ambulatorial na avaliação destes pacientes. OBJETIVO: Avaliar a presença de refluxo anormal a pHmetria esofagiana prolongada ambulatorial em pacientes cirróticos com varizes de esôfago e seus possíveis fatores preditivos. MÉTODOS: Cinqüenta e um pacientes (28 homens, 23 mulheres, média de idade de 54 anos) com cirrose hepática diagnosticada por métodos clínicos, laboratoriais, de imagem e histopatológicos foram avaliados de forma prospectiva. Todos os pacientes apresentavam varizes de esôfago à endoscopia digestiva alta e foram submetidos a um questionário para avaliação da presença de sintomas típicos da doença do refluxo gastroesofágico (pirose e/ou regurgitação ácida). pHmetria esofagiana prolongada ambulatorial foi realizada posicionando-se o cateter 5 cm acima do limite superior do esfíncter esofagiano inferior, determinado previamente pela esofagomanometria. Refluxo anormal (% tempo total com pH < 4 >4,5%) foi relacionado com o tamanho das varizes, gastropatia congestiva, ascite, gravidade da cirrose e presença de sintomas típicos da doença do refluxo gastroesofágico. RESULTADOS: O calibre das varizes foi considerado pequeno em 30 pacientes (59%), médio em 17 (33%) e grosso em 4 (8%), 21 (41%) gastropatia congestiva. Ascite foi observada em 17 (33%); 32 pacientes (63%) foram classificados com Child-Pugh A, 17 (33%) Child-Pugh B e 2 (4%) Child-Pugh C. Vinte e sete pacientes (53%) apresentavam sintomas típicos da doença do refluxo gastroesofágico. Refluxo anormal a pHmetria esofagiana prolongada ambulatorial foi demonstrado em 19 pacientes (37%). Apenas um deles apresentava esofagite erosiva à endoscopia digestiva alta. Não houve relação entre ascite, calibre das varizes, gastropatia congestiva e classificação de Child-Pugh com refluxo anormal. Houve correlação entre a presença dos sintomas típicos da doença do refluxo gastroesofágico e refluxo anormal. CONCLUSÃO: Refluxo anormal foi demonstrado em 37% dos pacientes com cirrose hepática e varizes de esôfago. Apenas os sintomas típicos foram preditores de refluxo anormal.

RACIONAL: A cirrose hepática apresenta como uma das principais causas de morbimortalidade, a hipertensão porta com o desenvolvimento de varizes esofagianas, possibilidade de hemorragia digestiva alta e agravamento da insuficiência hepática. É importante identificar fatores preditivos causais ou agravantes desta condição e se possível, preveni-los. Nos últimos anos tem se observado a associação de distúrbios motores de esôfago e de refluxo gastroesofágico em pacientes cirróticos com varizes de esôfago. OBJETIVOS: Estudar a prevalência dos distúrbios de motilidade esofagiana e, entre eles, da motilidade esofagiana ineficaz, neste grupo de pacientes e seus possíveis fatores preditivos. MÉTODOS: Avaliaram-se de maneira prospectiva, 74 pacientes com cirrose hepática e varizes esofagianas diagnosticadas por endoscopia digestiva alta, virgens de tratamento endoscópico terapêutico. Todos foram submetidos a um protocolo de investigação clínica, a esofagomanometria e 55 pacientes também realizaram pHmetria esofagiana ambulatorial. RESULTADOS: Alterações da motilidade esofagiana foram observadas em 44 pacientes (60%), sendo a mais prevalente a motilidade esofagiana ineficaz, verificada em 28%. Refluxo anormal foi encontrado em 35% dos pacientes. Não houve correlação entre anormalidade manométrica em geral e motilidade esofagiana ineficaz, em particular, e a presença de sintomas esofagianos ou típicos de doença do refluxo, refluxo anormal, a gravidade da doença, a presença de ascite e o calibre das varizes. CONCLUSÕES: A maioria dos cirróticos com varizes esofagianas não submetidos a tratamento endoscópico apresenta distúrbios motores do esôfago, sem fatores preditivos identificáveis. A importância clínica desses achados necessita de maior aprofundamento na questão, para elucidar seu papel definitivo.

Two hundred and forty Brazilian patients with chest pain and normal cardiac evaluation were submitted to computerized esophageal manometry. Endoscopic examination and/or swallow barium studies had excluded obstructive lesions. Motor disorders were found in 63% of patients; non-specific motors disorders and hypotensive lower esophageal sphincter were the most common. The finding of nutcracker esophagus in only 6% of the patients is a quite different rate from what has been previously described in the literature. Esophagitis was observed at endoscopy in 13.4% of the patients, hiatus hernia in 19.7% and peptic gastric or duodenal ulcer in 4.9%. It should be emphasized that after excluding pain as being of cardiac origin an abnormal manometry result points to the esophagus as the probable site of origin of the pain; esophageal investigation is important for establishing proper treatment for these patients.

The most important etiologies of achalasia are idiopathic and related to Chagas' disease. The lower esophageal sphincter pressure (LESP) in idiopathic achalasia (Id Ach) is higher compared with a healthy group, but there are different reports in Chagasic achalasia (Ch Ach). We compared the LESP of patients with both forms of achalasia and a control group. The LESP of 213 achalasia patients without previous treatment and 32 healthy volunteers were assessed. In 126 patients, the etiology could be demonstrated using serologic tests (Id Ach, 94 and Ch Ach, 32).
