Various postoperative predictive markers following cardiovascular surgery have been examined for use in the current aging population. The controlling nutritional status (CONUT) score, which is advocated not only as a screening tool for poor nutritional status, but also as an immunonutritional assessment, has started to attract attention in several clinical settings, such as in cancer and heart failure patients. The aim of this study was to evaluate the value of the CONUT score as a postoperative prognostic marker in patients who underwent cardiovascular surgery. Objective A positive correlation is observed between the progression of renal impairment and the increasing risk of cardiovascular disease. Our aim was to examine the relationship between the renal resistive index (RRI) assessed by duplex sonography and the extent of atherosclerosis in patients without renal impairment undergoing vascular imaging studies. Methods The RRI was evaluated pre-procedurally among 106 outpatients with an estimated glomerular filtration rate (eGFR) ≥60 mL/min/1.73 m2 undergoing clinically-driven coronary computed tomography angiography (CCTA). In those subjects, a carotid artery ultrasound scan was also performed to evaluate carotid artery disease. We investigated the association between the RRI and the atherosclerotic extent, defined by the presence of coronary artery calcium (CAC)>0 and carotid intima-media thickness (cIMT)≥1.0 mm. Results Multi-site atherosclerosis (CAC>0 and cIMT≥1.0 mm) was found in 31 patients. The RRI was significantly increased with an increasing number of atherosclerotic vessels (absence of atherosclerosis: 0.65±0.04 vs. single-site atherosclerosis: 0.67±0.06 vs. multi-site atherosclerosis: 0.71±0.05, p0.70 [odds ratio (OR): 4.05, 95% confidence interval (CI), 1.37-12.0, p=0.01], cardio ankle vascular index (CAVI)≥9.0 (OR: 8.18, 95% CI: 2.47-27.1, p90 mL/min/1.73 m2 (OR: 5.89, 95% CI: 1.39-25.1, p=0.01) were associated with multi-site atherosclerosis. Conclusion The RRI, a sub-clinical renal parameter is an atherosclerotic marker in patients without renal impairment.

We report a rare case of type A dissection involving a right-sided aortic arch with an aberrant left subclavian artery originating from Kommerell’s diverticulum in a 76-year-old woman. Endovascular treatment for Kommerell’s diverticulum including intimal tear of the dissection was performed. At the 5-year follow-up, the patient was doing well, with no endoleak or dilatation of the Kommerell’s diverticulum.

Subjects Currently, hemostatic materials made from human blood components and animal-derived collagen is used for controlling operative hemorrhage in the cardiovascular surgery field. In this study, we focused on an entirely synthetic self-assembling peptide (development code: TDM-621) that gels when in contact with blood or other bodily fluids and stops bleeding upon contact with a wound site. We investigated its usefulness as a hemostatic material in animal and clinical studies. Methods Before we began the clinical study, we demonstrated the hemostasis efficacy and safety of TDM-621 in animal experimental models. Twenty-five patients (22 men, 3 women) were enrolled in the clinical study, and the following procedures were performed: 1) coronary artery bypass graft (CABG) (n = 9), 2) abdominal aortic graft replacement (n = 4), and 3) peripheral artery bypass (n = 12). The TDM-621 material was applied to a total of 33 vascular anastomotic graft sites (some patients received material at more than one site). Both hemostatic efficacy and safety were examined. Results A total of 33 anastomotic graft sites in 25 patients were evaluated, and the averaged primary and secondary efficacy rate was 94.5%. No postoperative bleeding or adverse events (including serious adverse events) with a causal relationship to treatment were observed. Conclusion This study indicated that TDM-621 is a more effective and reliable hemostat than commonly-used general hemostatic agents and, therefore, will be very useful in several cardiovascular surgery applications.

大動脈四尖弁は大動脈二尖弁と同様に大動脈弁機能不全の原因疾患となる先天性弁膜症である. 発生頻度は大動脈二尖弁より少なく稀な疾患である. 今回, 超音波検査により術前に大動脈弁四尖弁と診断し得た2症例を経験したので報告する. 症例1 : 63歳, 男性, ペースメーカー植込み術を受けている. 労作時呼吸困難を自覚するようになり入院した. 心エコー検査の結果, 重度の大動脈弁逆流を認め, 左冠尖と無冠尖の間に副尖を有する大動脈四尖弁であった. 手術で四尖が確認され大動脈弁置換術 (SJM 23 mm) を施行した. 組織像では軽度の線維増生と粘液変性が認められた. 症例2 : 58歳, 女性, 高血圧と気管支喘息のため近医で加療を受けていた. 労作時呼吸困難を自覚するようになり入院した. 心エコー検査の結果, 重度の大動脈弁逆流と大動脈弁狭窄の所見を認めたが, 大動脈弁形態は不明であった. 経食道心エコー検査を施行し, 左冠尖と右冠尖の間に副尖を有する大動脈四尖弁と判明した. 手術で四尖が確認され大動脈弁置換術 (SJM 17 mm) を施行した. 組織像では高度の線維増生と石灰化が認められた. 心エコー検査で大動脈弁膜症の原因を検索する際には大動脈四尖弁の可能性も考慮に入れ慎重に観察する必要がある.

To evaluate the stabilizing effects of an antilipemic agent, bezafibrate, on coronary plaques, we carried out a prospective angioscopic and angiographic open trial. From April 1997 to December 1998, 24 patients underwent coronary angioscopy of plaques in non-targeted vessels during coronary interventions and then again 6 months later. The patients were divided into control (10 patients, 14 plaques) and bezafibrate (14 patients, 21 plaques) groups. Oral administration of bezafibrate (400 mg/day) was started immediately after the intervention and was continued for 6 months. The vulnerability score was determined based on the angioscopic characteristics of plaques and compared before and 6 months later. Background Since long-term administrations of anti-hyperlipidemic agents result in reduction in % stenosis or increase in minimum lumen diameter (MLD) of stenotic coronary segments, it is generally believed that anti-hyperlipidemic agents stabilize vulnerable coronary plaques. However, recent pathologic and angioscopic studies revealed that vulnerability of coronary plaques is not related to severity of stenosis and the rims rather than top of the plaques disrupt, and therefore, angiography is not adequate for evaluation of vulnerability.Angioscopy enables macroscopic pathological evaluation of the coronary plaques. Therefore, we carried out a prospective angioscopic open trial for evaluation of the stabilizing effects of bezafibrate on coronary plaques.Methods From April, 1997 to December, 1998, 24 patients underwent coronary angioscopy of the plaques in the non-targeted vessels during coronary interventions and 6 months later. The patients were divided into control (10 patients, 14 plaques) and bezafibrat (14 patients, 21 plaques) groups. Oral administration of bezafibrate (Bezatol SR, 400mg/day) was started immediately after the interventions and was continued for 6 months. The vulnerability score was determined based on angioscopic characteristics of plaques and it was compared before and 6 months later.Results Six months later, vulnerability score was reduced (from 1.6 to 0.8;p < 0.05) in bezafibrate group and unchanged (from 1.4 to 1.3; NS) in control group. In bezafibrate group, the changes in vulnerability score was not correlated with those in % stenosis or MLD. Conclusion The results indicate that bezafibrate can stabilize coronary plaques.

一時的バイパス設置下に切除し得た大動脈漫潤肺癌2例を経験した.症例1は64歳男性, 左上葉大細胞癌。腫瘍は下行大動脈に浸潤しており, 左鎖骨下動脈一下行大動脈間にバイパスを設置し大動脈の外膜のみを剥離して, 左上葉切除によって摘出した.症例2は48歳男性, 左下葉腺癌.腫瘍は下行大動脈に浸潤しており, 腫瘍を挟み下行一下行大動脈間にバイパスを設置して大動脈環状切除を行い, 左下葉切除によって摘出した.肺癌の大動脈浸潤例に対する一時的バイパスの設置は補助循環を使用する事なく簡便でかつ安全な方法であると考えられた.

The patient was a 64-year-old man. He developed fever and lumbago 6 months after the EVAR. Because CT showed an abscess in the aortic aneurysm surrounding the stent graft, stent-graft infection was diagnosed, and treatment with intravenous antibiotics was initiated. However, the fever and inflammatory markers persisted; therefore, CT-guided drainage catheter placement was performed. After all the pus had been discharged, the fever subsided, and the inflammatory reaction was also suppressed. One year has elapsed since the treatment, and the patient continues to visit with no complaints. We report that stent-graft infection was relieved with antibiotics and drainage.

Abdominal pseudoaneurysm is an extremely rare disease entity. Precise preoperative diagnosis is mandatory for the treatment of this disease and an accurate surgical approach that minimizes ischemia of the visceral organs has long since been needed according to origin. A 40-year-old woman with an abdominal injury was treated by blocking the abdominal artery to occlude the aneurysmal foramen between the celiac artery and superior mesenteric artery using a balloon catheter for intra aortic balloon pumping (IABP) under hypothermia. Postoperative course was uneventful. The patient was discharged from the hospital, and currently under follow-up observation.

A 52-year-old man with Leriche syndrome with a high aortic occlusion, in which the abdominal aorta proximal to the origin of the renal artery was thrombosed, is reported. The patient was admitted to the hospital because of intermittent claudication and sexual impotence. On aortogram the abdominal aorta just from the infrarenal portion to bilateral common iliac arteries occluded completely and the orifice of right renal artery was slightly stenotic. Operation was performed under hypothermia at 30°C of rectal temperature, and a wide operative field was secured through a modified spiral skin incision and extraperitoneal approach (by Stoney et al., 1973). Through a longitudinal aortotomy, thrombus was found locating from the proximal to the renal arteries to bilateral common iliac arteries. The orifice of the right renal artery was thrombosed, too. The long segmental thrombus was removed carefully. Aorta was reconstructed with Y-shaped Dacron graft from the infrarenal portion to bilateral common iliac arteries. The postoperative course was uneventful without renal dysfunction. On postoperative aortogram blood flow was sufficient along the prosthetic graft. In conclusion the spiral skin incision with extraperitoneal approach under systemic hypothermia is a preferable and appropriate procedure for this disease.

Reported herein is a patient with valvular dysfunctions due to thorombosis developed 14 years after mitral valve prosthesis with Bjork-Shiley valve, who could be rescued by emergency operation. A 44-year-old woman was admitted to the hospital because of hemorrhagic gastric ulcer. There was a history of mitral valve prosthesis with Bjork-Shiley valve for mitral insufficiency in 1976, and subsequent anti-coagulation therapy on an ambulant basis, after admission her symptoms of heart failure were aggravated. She was diagnosed as dysfunction of the artificial valve and underwent an emergency operation for reconstruction of the valve with 29 mm of St. Jude Medical valve. The findings at the operation revealed that a newly formed thrombosis at the minor-strut area had been inhibiting the motility of the valve.After the operation, her condition was improved and she was discharged at Day 20 of illness. This case was thus identified as having causative factors of nutritive infusion and administration of hemostatic agent upon suspending the anticoagulation therapy due to hemorrhagic gastric ulcer after 14 years of the term with no complaint of heart failure. It was also diagnosed as dysfunction of artificial valve decisively induced by a newly formed thrombosis which was assumed to becaused by the increased blood coagulation.

The significance of early and appropriate treatment is emphasized for a rare complication of pulmonary hemorrhage caused by SGC which has rapid onset and progression in the course with a resulting high mortality. The mechanisms of the onset, condition, and treatments were also discussed from a review of the literature. We reported a case of pulmonary hemorrhage induced by Swan-Ganz catheter. In a 68-year-old woman admitted for CABG in the treatment of angina after infarct, A 7F Swan-Ganz catheter was inserted under pressure monitoring and general anesthesia in the operation room. We confirmed that the tip of catheter was wedged in the pulmonary artery on the chest X-ray film and the pressure curve in the monitor. Just while the tip of catheter was pulled, massive bleeding was observed from the endotracheal tube. For this, the respirator setting was changed and hyperventilation and high PEEP were tried. These treatments were effective for hemostasis in the lung, so she could be weaned from respirator and extubated about 9 hours after operation. The patient was discharged on the 31st day after operation with a good condition.

We attempted combined use of angioscopy and intravascular ultrasonography (IVUS) to localize the coronary ostia and determine the aortic segment to be replaced in patients with annuloaortic ectasia, because these preoperative informations are important for selection of an appropriate technique for reconstructing the coronary artery, to prevent complications, and also to postoperative follow-up. Two cases with annuloaortic ectasia underwent angioscopy and IVUS both pre- and post-operatively. Structure of aortic cusps, position of coronary ostia, the extent of ectasia with very thin wall were clearly observed by IVUS. Angioscopy showed milky white luminal surface of the ectasic segment. After Cabrol’s operation, the sutured portion of native aorta and graft was clearly identified by IVUS and mural thrombus and naked surface of graft were observed by angioscopy. Complications were observed in none. The results indicate feasibility of combined use of angioscopy and IVUS for determination of surgical approach and follow-up in patients with AAE.

We recently confirmed that pemirolast potassium, an antiallergic agent, markedly inhibits migration and proliferation of vascular smooth muscle cells. It has also been reported that pemirolast inhibits intimal hyperplasia in animal experiments.To elucidate the preventive effects of pemirolast on restenosis after percutaneous transluminal coronary angioplasty (PTCA), 227 patients were enrolled in this prospective, randomized trial. A total of 205 patients who were compatible with the protocol were analyzed (pemirolast group, 104 patients with 140 lesions; control group, 101 patients with 133 lesions). Patients in the pemirolast group received 20 mg/d of pemirolast from 1 week before PTCA until the time of follow-up angiography (4 months after PTCA). Angiographic restenosis was defined as diameter stenosis/=50% at follow-up. Restenosis rates were significantly lower in the pemirolast group than in the control group (24.0% vs 46.5% of patients, 18.6% vs 35.3% of lesions, P.01, respectively). During 8 months of follow-up, there were no coronary events (death, myocardial infarction, coronary artery bypass surgery, or repeated PTCA) in 81.7% of the pemirolast group and in 63.4% of the control group (P =.013).This study suggested that pemirolast would be useful in the clinical setting to prevent restenosis after PTCA.
