Background Wrist ligaments are challenging to visualize using magnetic resonance imaging (MRI). Injuries involving the scapholunate ligament (SLL), the lunotriquetral ligament (LTL), and the triangular fibrocartilage complex (TFCC) are common and difficult to diagnose, often requiring diagnostic arthroscopy. Purpose To compare the visualization of wrist ligaments on a three-dimensional (3D) sequence with two-dimensional (2D) sequences on 3-T MRI. Material and Methods Eighteen healthy volunteers were examined with a 3D SPACE (sampling perfection with application optimized contrasts using different flip angle evolution) sequence and 2D coronal, axial, and sagittal proton density-weighted (PD) sequences. Four musculoskeletal radiologists graded the anatomical visibility of the SLL, LTL, TFCC, and the image quality, using five grades in a visual grading characteristics (VGC) evaluation. After Bonferroni correction, a P value ≤0.005 was considered statistically significant. Results The 3D images were graded significantly better than the 2D images in the visualization of the dorsal and palmar parts of the SLL and the LTL. Regarding the TFCC, the 3D images were graded significantly better for visualization of the foveal attachment. 2D imaging was not found significantly superior to 3D imaging in any aspect. Conclusion The 3D SPACE sequence was scored as superior to the 2D sequences at 3 T in the assessment of the SLL, the LTL, and the foveal attachment of the TFCC. Thus, 3D SPACE can replace 2D PD sequences when these ligaments need to be assessed.

The aim of this study was to evaluate the prevalence of arthroscopic scapholunate (SL) and/or lunotriquetral (LQ) laxity and triangular fibrocartilaginous complex (TFCC) injuries in patients who have an intraarticular fracture of the distal radius and to correlate these lesions with fracture type. Fifty-seven intraarticular radius fractures, whether or not they were associated with an ulnar styloid fracture, were evaluated and treated by arthroscopy. Scapholunate and lunotriquetral ligament injuries were classified according to the EWAS classification. TFCC lesions were assessed according to Palmer's classification. Each injury was documented through preoperative X-rays and a CT scan. Fracture type and soft tissue injury were not significantly associated one to another. Arthroscopic examination revealed at least one soft tissue injury in 39 intraarticular fractures of the distal radius (68.4%). Twenty-five percent of arthroscopic SL laxities (including severe EWAS 3 injuries) were not detected on standard radiographs. Arthroscopic SL laxity was present in 8 of 11 cases (72.7%) of radial styloid fracture and in 15 of 25 cases (60%) of fractures with at least one radial styloid component. There was no association between LQ integrity and fracture type. Ulnar styloid fractures (base or tip) and TFCC lesions were significantly correlated (P0.0001). The prevalence of soft tissue lesions secondary to intraarticular fractures of the distal radius was 68.4%. However, there was no statistically significant relationship between the different types of radius fractures and soft tissue injuries. On the other hand, ulnar styloid fracture was predictive of TFCC injury.

Background Reverse perilunate injuries are rare. Contrary to perilunate injuries, the violent force would start from the lunotriquetral ligament, go reversely toward the radial side, and cause the reverse or ulnar-sided perilunate dislocation. Case Description We describe a 31-year-old man with a reverse perilunate dislocation, who presented to our institution 3 weeks after a motorcycle accident. The patient was successfully treated with the reduction and association of the lunate and triquetrum (RALT) procedure by using closed maneuver and percutaneous headless compression screw fixation. The patient can obtain a good radiographic result and satisfactory function at the 30-month follow-up. Literature Review Many case series were reported concerning the perilunate injuries. However, few cases of reverse perilunate dislocation have been reported in the literature. No cases of reverse perilunate dislocation treated 3 weeks after the injury with the RALT procedure have been reported. Clinical Relevance In this case, we found that the dislocation could still be reduced with the closed maneuver. With the RALT procedure, the carpal alignment can be maintained and the stability can be regained. Also, the functional outcomes are good.

We assessed the outcomes of isolated lunotriquetral ligament injuries in ten patients who underwent arthroscopic dorsal capsulodesis. Data from patient records, radiologic images and arthroscopic video records were evaluated. The patients were evaluated for ulnar-sided wrist pain with history and physical examination. Preoperative radiographs and MRIs for pain aetiology were assessed. The patients underwent arthroscopic dorsal capsulodesis and were evaluated 29 months (range 19–45) after surgery. Nine patients returned to their original jobs. In one patient pain was aggravated with heavy activities. Mean scapholunate angle was 44°. There were significant improvements postoperatively in pinch and grip strength and MAYO wrist, patient-reported wrist evaluation and pain scores. The wrist flexion–extension and the radial ulnar deviation were significantly improved compared with the contralateral hand. We conclude that arthroscopic dorsal capsulodesis offers effective management for isolated lunotriquetral interosseous ligament injuries. Level of evidence: IV

Background Injuries of the lunotrirquetral ligament (LT lig) could be part of an extensive carpal injury and are then often treated at the time of the injury. However, when an injury of the LT ligament occurs alone, the injury is often missed. Treatment of this injury has traditionally been by open surgery, such as reattachment of the LT ligament, ligament reconstruction, or arthrodesis of the LT joint. These procedures needed a large exposure to the carpus running the risk of damaging the external ligaments, the nerves important for proprioception, and the capsule with the potential of scarring and adhesions. Materials and Methods We describe a novel arthroscopic assisted technique for reconstruction of the LT ligament. Using this less invasive technique, there is a possible advantage of lesser scarring and faster mobilization. Results We have performed this technique in two patients with more than 30 months follow-up. They both have great improvement of the functional scores. Conclusion The novel arthroscopic assisted technique for LT lig reconstruction is a technically demanding procedure; however, this obtains good clinical results with more than 30 months follow-up due to less exposure of the carpus. The best outcome in distal radius fractures is achieved if anatomy is restored, in particular the intra-articular congruity. This is achieved partly with improved fixation, such as using volar locking plates, and partly by using an arthroscopy-assisted reduction and fixation technique. In addition to improving the intra-articular congruity, associated ligament and chondral injuries can be detected and treated. This article outlines various associated injuries with suggested management in a stepwise fashion. It is hoped that overall outcomes will be improved once patient-related and treatment-related factors have been evaluated and previously undetected associated ligament injuries have been found and treated.

Background: The purpose of our study was to compare magnetic resonance arthrography (MRA) as a diagnostic modality against the gold standard of wrist arthroscopy in the evaluation of chronic wrist pain. Materials and Methods: Thirty three patients with chronic wrist pain suspected to have ligament injuries of the wrist were prospectively recruited. They underwent MRA examinations followed by wrist arthroscopy. Arthroscopic findings were compared with radiological findings focusing on three important structures – triangular fibrocartilage complex (TFCC), scapholunate ligament (SLL), and lunotriquetral ligament (LTL). Results: For the 17 patients with TFCC tears/perforations on arthroscopy, MRA gave a sensitivity (SEN) = 88%, specificity (SPE) = 87.5%, positive predictive value (PPV) = 88%, and negative predictive value (NPV) = 87.5%. For the 13 patients with SLL tears on arthroscopy, MRA gave SEN = 77%, SPE = 100%, PPV = 100%, and NPV = 87%. For the 7 patients with LTL tears on arthroscopy, MRA gave SEN = 29%, SPE = 100%, PPV = 100%, and NPV = 84%. A composite correlation between findings on MRA and wrist arthroscopy revealed an overall SEN = 73%, SPE = 96%, PPV = 93%, and NPV = 85% for MRA, with overall accuracy = 88%. Conclusions: The presented diagnostic results of MRA are superior to those of magnetic resonance imaging quoted in literature. MRA is a potent tool for evaluating chronic wrist pain but tends to miss lesions of intrinsic carpal ligaments (SLL and LTL) more than TFCC. Wrist arthroscopy may be recommended when the clinical suspicion is strong.

Background and Aims: Patients with acute or chronic wrist pain often undergo wrist arthroscopy for evaluation of chondral and ligamentous abnormalities. The purpose of this study was to compare findings of wrist arthroscopy with cone beam computed tomography arthrography and magnetic resonance arthrography. Materials and Methods: Altogether, 21 patients with wrist pain underwent cone beam computed tomography arthrography, magnetic resonance arthrography, and wrist arthroscopy. Chondral surfaces of the scaphoid, lunate, and radius facing the scaphoid and lunate were evaluated. The scapholunate ligament, the lunotriquetral ligament, and the triangular fibrocartilage complex were classified as either intact or torn. Sensitivity, specificity, positive and negative predictive values, and accuracy with 95% confidence intervals were assessed. Results: For chondral lesions (n = 10), cone beam computed tomography arthrograms showed slightly higher specificity than magnetic resonance arthrography. The sensitivity of cone beam computed tomography arthrography was also better for these lesions, except for those on the chondral surface of the lunate. For triangular fibrocartilage complex injuries (n = 9), cone beam computed tomography arthrography showed a better specificity and sensitivity than magnetic resonance arthrography. For ligamentous injuries (n = 6), cone beam computed tomography arthrograms were more sensitive, but less specific than magnetic resonance arthrography images. However, the number of lesions was very small and the 95% confidence intervals are overlapping. Conclusion: Cone beam computed tomography is an emerging imaging modality that offers several advantages over computed tomography and magnetic resonance imaging. Its usefulness particularly in ligamentous injuries should be further explored in a larger study. Cone beam computed tomography arthrography seems to offer similar sensitivity, specificity, and accuracy compared to magnetic resonance arthrography and therefore serves as a valuable option in evaluating patients with wrist pain.

Perilunate dislocations and perilunate fracture dislocations are rare injuries that occur as a result of high energy trauma. We describe a case of a volar fracture dislocation of the proximal pole of the scaphoid with an associated scapholunate and lunotriquetral ligament disruption as well as a lunate fossa fracture of the distal radius. These injuries are serious injuries that require a high degree of clinical acumen and radiographic scrutiny to allow for prompt treatment in order to avoid the sequelae of long-term complications that can arise. This case serves as a reminder of the complexity of these injuries and their associated mechanics.

Lunotriquetral ligament injury is a relatively common cause of ulnar-sided wrist pain. Injury ranges from partial stable ligament tears to extensive perilunate instability. Clinical decision-making largely depends on the chronicity, instability and cause of the ligament injury. Conservative treatment is generally regarded as first choice of treatment of mild lunotriquetral instability; however, outcome studies on conservative treatment are lacking. Temporary arthroscopic pinning and/or debridement are minimally invasive procedures of preference. In the case of more dissociative injury, surgical interventions may be performed. The literature suggests that soft tissue reconstruction is an effective procedure in this group. Arthrodesis of the lunotriquetral joint is associated with high rates of non-union (up to 57%) and the indications for surgery should therefore be very clear. Methodological issues make it hard to draw firm conclusions from the data. Studies on the effectiveness of conservative management and prospective comparative studies will further improve clinical decision-making in lunotriquetral instability. Level of evidence: N/A

Objective The aim of this study is to assess the diagnostic value of direct MR arthrography compared to conventional MR imaging in the diagnosis of different pathologic entities affecting the internal ligaments of the wrist mainly the scapholunate and lunotriquetral ligaments. Subjects and methods This study included 51 patients complaining of chronic wrist pain. Conventional MRI and MR arthrography (MRA) were done in all cases. Results A comparison of the sensitivity of conventional MRI versus MRA was done by correlating the final diagnosis of each modality with the results of arthroscopy. Regarding complete SLL tears, MRI revealed a SEN, SPE and ACC of 61.54%, 96.77%, and 86.36%, while MRA showed a SEN, SPE and ACC of 92.31%, 100%, and 97.73% respectively, partial SLL tears, un-enhanced MRI revealed SEN, SPE and ACC of 10%, 94.12%, and 75%, while MRA showed 66.67%, 85.71%, and 81.82% respectively, complete LTL tears un-enhanced MRI revealed a SEN, SPE, and ACC of 25%, 100%, and 79.5% respectively, while MRA showed 91.67%, 100%, and 97.73% respectively. Conclusion MR arthrography is a potent additional tool facilitating the diagnosis of different pathologic entities affecting the major internal ligaments of the wrist joint and helps to reduce arthroscopic interventions.

It has become clear that the stability of the scapholunate joint is not dependent wholly upon the scapholunate interosseous ligament but rather upon both primary and secondary stabilizers, which form a scapholunate ligament complex. Each case of scapholunate instability is unique and therefore should be treated with tissue-specific repairs, which may partly explain why a single procedure cannot successfully restore joint stability in every case. Not all lunotriquetral ligament tears are traumatic. The optimal treatment of symptomatic tears is still uncertain. Wrist arthroscopy has a pivotal role in both the assessment and treatment of these derangements.

OBJECTIVE. Although much attention is paid to the scapholunate ligament, lunotriquetral ligament, and the triangular fibrocartilage complex, additional intrinsic and extrinsic ligaments in the wrist play an important part in carpal stability. With improved MRI techniques, the radiologist can increasingly visualize these ligaments. CONCLUSION. The anatomy, MRI appearance, and clinical significance of the scapholunate ligament, lunotriquetral ligament, triangular fibrocartilage complex, carpal metacarpal ligaments, and volar and dorsal extrinsic ligaments are reviewed.

The wrist has intricate anatomy that can be “dissected” non-invasively with imaging. Imaging interpretations can be accomplished comprehensively and efficiently by using a systematic “checklist” approach to these anatomic structures (e.g., osteoarticular, ligamentous, musculotendinous, neurovascular). Diagnostic accuracy is maximized by mastering the problems certain to appear in patients with wrist maladies undergoing imaging. “The Top 5” classic wrist disorders are highlighted here, including fractures (e.g., scaphoid), AVN (e.g., lunate), carpal instability (e.g., scapholunate and lunotriquetral ligament tears), TFCC injuries, and common soft-tissue masses.

LEARNING OBJECTIVES After reading this article, the participant should be able to: 1. Recognize the clinical features of chronic scapholunate and lunotriquetral ligament injuries and their long-term sequelae. 2. Describe the current management and latest techniques for treating chronic scapholunate and lunotriquetral ligament injuries and their long-term sequelae, such as wrist arthritis. SUMMARY This article provides an update on the diagnosis, current management, and latest techniques for the treatment of chronic scapholunate and lunotriquetral ligament injuries and their long-term sequelae, such as wrist arthritis.

AIM: The purpose of this study was to evaluate the diagnostic value of MRI for detecting intracarpal lesions in clinical routine. PATIENTS AND MATERIALS: In a retrospective study, we reviewed the charts of 506 patients who had undergone wrist arthroscopy in our department between May 1998 and November 2002. Out of 506 patients 217 had an MRI. The MRI was performed at 31 different radiology facilities using a number of techniques. The MRI results were compared with the arthroscopic findings, taking the arthroscopic results as a “gold standard” (sensitivity = SEN, specificity = SPE, positive predictive value = PPV, negative predictive value = NPV, accuracy = ACC). RESULTS: The following results were found: For tears of the scapholunate ligament the avalues are SEN 18.5 %, SPE 95 %, PPV 71 %, NPV 66 %, ACC66 %. Not one of 16 tears of the lunotriquetral ligament was found by MRI. CONCLUSION: In our data we found a low sensitivity for unspecific requested and implemented MRI for detecting intracarpal lesions. Therefore the indication for MRI should only be made after experienced hand surgeons have examined the wrist. In our opinion, only direct MR-arthrography is equal to arthroscopy, so that only an experienced radiologist who is familiar with this technique should perform the MRI diagnostics of the hand.

Objective The purpose of this study was to determine the utility of sonography and sonoarthrography in evaluation of dorsal bands of the scapholunate ligament (SLL), lunotriquetral ligament (LTL), and triangular fibrocartilage (TFC) disk in correlation with arthrography and magnetic resonance arthrography (MRA). Methods High-resolution sonography of the SLL, LTL, and TFC disk was performed on symptomatic wrists in 16 patients referred by a hand surgeon for MRA of the symptomatic wrists. All patients then underwent arthrography and an MRA study of the same wrist. After MRA, sonography was repeated. The imaging findings of these different techniques were then compared. Four patients (25%) underwent surgery of their wrists. In these 4 patients, the surgical and imaging findings were correlated. Results For the SLL, the results were concordant for all imaging modalities in 15 patients (93.75%) and partially concordant in 1 (6.25%). For the LTL, the results were concordant for all imaging modalities in 12 patients (75%), partially concordant in 3 (18.75%), and discordant in 1 (6.25%). For the TFC disk, the results were concordant for all imaging modalities in 13 patients (81.25%), partially concordant in 2 (12.5%), and discordant in 1 (6.25%). The arthroscopic and imaging findings were concordant for 3 SLLs, 3 LTLs, and 3 TFC disks. Conclusions Our preliminary results are encouraging. Sonography may be used at least as a screening imaging modality in evaluation of the SLL and TFC disk. Sonoarthrography improves evaluation of the LTL.

Chronic lunotriquetral (LT) injuries are less common than scapholunate ligament injuries and difficult to diagnose. They may be associated with positive ulnar variance. Clinical diagnostic tests elicit pain at the LT interval. Although radiographs are typically normal, MRI and wrist arthroscopy can help confirm the diagnosis. When conservative treatments fail, surgical options include LT ligament reconstruction, LT arthrodesis, and ulnar-shortening osteotomy (in patients with positive ulnar variance).

The dorsal radiocarpal ligament (DRCL) has been implicated in both volar and dorsal intercalated segmental instabilities and has a role in midcarpal instability. Tears of the DRCL are more common than previously suspected. They are best seen through a volar radial portal and are amenable to arthroscopic repair. DRCL tears appear to be part of a spectrum of radial- and ulnar-sided carpal instability as evidenced by the frequent association with scapholunate and lunotriquetral ligament injuries as well as triangular fibrocartilage tears. Isolated DRCL tears can be solely responsible for wrist pain. The presence of an associated DRCL tear when seen in combination with a scapholunate, lunotriquetral, or triangular fibrocartilage tear connotes a greater degree and/or duration of carpal instability, and portends a poorer prognosis after treatment. Good results are obtained after arthroscopic repair of isolated DRCL tears. Results of DRCL repairs are less predictable when seen in combination with other types of carpal pathology. Recognition of this condition and further research into treatment methods is needed.

Acute scapholunate and lunotriquetral interosseous ligament injuries must be stabilized for anatomically correct healing to prevent progressive carpal instability. Shortcomings of reconstruction procedures place a priority on achieving a successful initial repair. The traditional repair strategy involves open reduction and internal fixation and results in significant long-term wrist stiffness. An attempt to produce less scarring with minimally invasive techniques is currently being made using arthroscopy to guide the reduction and fixation of the intercarpal relationships during repair. One of the most important aspects of managing intrinsic ligament injuries in the wrist is making an accurate evaluation of the injury magnitude and recognizing the importance of functional stability provided by the ligament. This article provides an injury grading method based on functional stress testing of the scapholunate and lunotriquetral interosseous ligaments. The details of the arthroscopic technique have not been firmly established by the limited number of authors that have reported on the subject to date. Furthermore, long-term results in high numbers of patients are not currently available to firmly establish the efficacy of arthroscopic treatment of acute perilunate injuries.

Volar portals for wrist arthroscopy have certain advantages over the standard dorsal portals for visualizing dorsal capsular structures as well as the palmar aspects of the carpal ligaments. The volar radial (VR) portal is relatively easy to use and is an ideal portal for evaluation of the dorsal radiocarpal ligament (DRCL) and the palmar aspect of the scapholunate interosseous ligament. The volar midcarpal portal may be considered as an occasional accessory portal for visualizing the palmar aspects of the capitate and hamate in cases of avascular necrosis or osteochondral fractures. The volar ulnar (VU) portal is especially useful for the viewing and debridement of palmar tears of the lunotriquetral ligament. Copyright © 2002 by the American Society for Surgery of the Hand

Lunotriquetral instability is treated based upon degree of instability and concomitant pathology although there is no consensus on treatment of lunotriquetral instability. Lunotriquetral instability, with or without concomitant pathology, has been surgically managed with arthrodesis, ligament graft reconstruction, ligament repair, percutaneous pinning, and ulnar shortening.

The treatment of lunotriquetral interosseous (LTIO) ligament injuries is controversial. It is part of a spectrum of injuries involving the ulnar side of the wrist. Associated injuries may play a greater role in the outcome of treatment than does the LTIO injury. Arthroscopy is the most accurate diagnostic tool available for the evaluation of LTIO ligament injuries; it allows diagnosis and often treatment of associated injuries. This article describes the current approach to the diagnosis and treatment of LTIO ligament injuries anti related use of arthroscopy.

This paper presents a rare case of trans-scaphoid, transcapitate, transtriquetral, perilunate fracture–dislocation of the right wrist as a result of a motorcycle accident. Open reduction and internal fixation of the scaphoid and capitate with one screw was performed and the lunotriquetral ligament was repaired using a suture anchor.

Lunotriquetral ligament ruptures were treated by arthrodesis or ligament reconstruction. The outcomes were retrospectively reviewed: 17 underwent an arthrodesis and 13 a reconstruction with a strip of the extensor retinaculum.

Because the scapholunar ligament is exposed to axial constraints of mobility and loads, it represents a weak point of the proximal row. But it is protected by quite a complete system of extrinsic ligaments which contribute to lock the wrist. There are a palmar ligamentous plane, a dorsal ligamentous plane and a distal scaphoidal ligamentous complex. The different tendons around the wrist can also have a cohesion effect of the carpus. A ligamentous injury is less likely to happen if the scapholunar ligament is unaffected. In that case, the lunotriquetral ligament is most likely injured. Seldom the extrinsic ligaments can be the only one to be affected, astride on the radiocarpal or the mediocarpal spaces. Once all the biomechanical aspects are put together, it is possible to understand the different mechanisms of the carpal collapse, which result from the application of the mechanical loads to the carpus, badly absorbed or badly redirected, by bones badly locked when they should work. The different ligament injuries explain the varieties of carpal instabilities. Consequently, clinical, paraclinical and arthroscopic exams have to explore those different ligamentous bolts for the best. This aims to enable a ligamentous repair as physiological as possible.

Disruption of the lunatotriquetral ligament is a common cause of ulnar-sided wrist pain. A review of the literature of the pathophysiology and treatment of LT ligament tears is presented. The authors report a series of 20 consecutive patients with arthroscopically confirmed LT ligament tears without a VISI collapse treated with arthroscopic reduction and pinning of the LT joint. At an average follow-up of 2 years 8 months, there was an 80% success rate. Success of the procedure correlated negatively with arthroscopic findings of chondromalacia and positively with decreased postoperative active wrist motion.

Diagnosis of injuries to the ligamentous structures of the wrist can be a challenge, particularly when there is involvement of the small, complex structures of the proximal wrist. Recent advances in magnetic resonance (MR) imaging, especially in spatial and contrast resolution, have facilitated more precise visualization of these structures. However, there are a number of pitfalls that may cause difficulty in diagnosis of injuries to the triangular fibrocartilage complex (TFCC), lunotriquetral ligament, and scapholunate ligament. Use of inappropriate MR imaging sequences and MR imaging artifacts may decrease the accuracy of diagnosis of injuries to the TFCC and wrist ligaments, whereas variant anatomy of the proximal wrist structures may mimic disease of the TFCC and wrist ligaments. Knowledge of the detailed anatomy of the wrist, as well as variant patterns of structure morphology and signal intensity, can help differentiate actual disease from normal or variant appearances at assessment with MR imaging.

Wrist injuries causing coincident disruptions of the scapholunate and lunotriquetral ligaments commonly result in perilunate dislocations. This article (1) describes our management of eight patients with wrist pain after coincident scapholunate and lunotriquetral ligament disruptions in the absence of perilunate dislocation: and (2) reports the results of biomechanical testing of some of the extrinsic and intrinsic wrist ligaments and interprets these data to explain the injury seen clinically. The diagnosis of ligament failure was made on the basis of history, physical examination, arthrography and surgical exploration. Surgical treatment of seven patients consisted of concomitant scapho-trapezio-trapezoid fusion and lunotriquetral fusion. Three of seven patients were free of pain, two had pain only at the extremes of motion, and two required additional surgery. Biomechanical analysis of the scapholunate and lunotriquetral ligaments and two extrinsic wrist ligaments, the radiolunotriquetral and the radioscaphocapitate, confirmed the clinical suspicion that the intrinsic ligaments could be completely disrupted while the extrinsics are only partially injured. Such a scenario could account for the residual stability that prevents the development of perilunate dislocations. Coincident disruption of the scapholunate and lunotriquetral ligaments in the absence of perilunate dislocation is an unusual injury. Treatment with lunotriquetral fusion and scapho-trapezio-trapezoid fusion restored functional use in five of seven wrists while maintaining wrist motion.

Volar portals for wrist arthroscopy have certain advantages over the standard dorsal portals for visualizing dorsal capsular structures as well as the palmar aspects of the carpal ligaments. The volar radial portal is relatively easy to use and is an ideal portal for evaluation of the dorsal radiocarpal ligament and the palmar aspect of the scapholunate interosseous ligament. The volar midcarpal portal may be considered as an occasional accessory portal for visualizing the palmar aspects of the capitate and hamate in cases of avascular necrosis or osteochondral fractures. The volar ulnar portal is especially useful for the viewing and debridement of palmar tears of the lunotriquetral ligament.

To evaluate high-resolution MRI of the proximal zone of the lunotriquetral ligament (LTL) using a microscopy surface coil with a 1.5 T scanner.The proximal zone of the LTL was reviewed in 90 subjects (23 asymptomatic normal volunteers and 67 patients with suspicion of triangular fibrocartilage complex injury) with high-resolution MRI using a 47-mm microscopy surface coil. High-resolution MR images were obtained with gradient recalled echo (GRE) T2*-weighted sequence and short tau inversion recovery imaging, with a 1- to 1.5-mm slice thickness, a 50-mm field of view, an imaging matrix of 140-224x512 using zero fill interpolation, and 3-4 excitations. As a qualitative analysis, the LTL was classified in shape and signal intensity.The triangle-shaped low-signal-intensity LTL was identified in 77 of 90 subjects (85.6%) on GRE images. The triangle was classified as regular (41.1%), broad-based (20.0%), narrow-based (6.7%), or asymmetrical (17.8%). The bar-shaped ligament was seen in one patient, and unclassified ligaments were seen in 12 patients. All volunteers showed triangle-shaped LTL. The MR signal intensity of the proximal zone in the LTL was characterized as homogeneously low intensity (type 1; 33.8%), linear intermediate or high signal intensity traversing the distal surface of the LTL (type 2; 45.5%), and linear intermediate or high intensity traversing both distal and proximal surfaces of LTL (type 3; 20.8%).The proximal zone of the LTL showed a broad spectrum of normal variations in shape and signal intensity on high-resolution MR images with a microscopy coil.

Magnetic resonance imaging (MRI) has a very established role in imaging many pathologic conditions in the wrist. MRI provides useful clinical information regarding neoplastic processes. It also provides valuable information in the evaluation of tendons and synovium in rheumatoid arthritis. However, it has been and is less useful when early ligamentous injuries, including grade I and grade II scapholunate and lunotriquetral ligament lesions, are suspected. Also, information regarding cartilage injuries and degeneration, especially in wrist instabilities, would be more than desirable for treating orthopedists and hand surgeons. However, at the current stage, this information is missing, and only the more advanced changes are seen in MRI. This article describes and explains clinical entities in which that information would help patient care and management.

Triquetrumfrakturen: Frakturen des Os triquetrum konnen unterteilt werden in solche der dorsalen Kortikalis und des Triquetrum-Korpus. Die unproblematischen Frakturen der dorsalen Kortikalis (Chip-Frakturen) konnen mit meist gutem Erfolg konservativ durch kurzfristige Ruhigstellung behandelt werden. Undislozierte Triquetrum-Korpus-frakturen machen eine vier- bis sechswochiger Ruhigstellung erforderlich. Die noch selteneren dislozierten Korpusfrakturen und transtriquetralen perilunaren Luxationsfrakturen erfordern ein operatives Vorgehen mit Reposition, Osteosynthese und evtl. Bandrekonstruktion. Lunotriquetrale Bandlasionen: Partielle Bandrupturen und dynamische Instabilitaten sind durch alleinige Ruhigstellung zu behandeln. Komplette, mit einer Fehlstellung einhergehende Bandrupturen (bei perilunarer Luxation) erfordern ein operatives Vorgehen mit Bandrekonstruktion und Bohrdrahttransfixation sowie eine langerfristige Ruhigstellung.

Patients with a tear of the horizontal part of the triangular fibrocartilage complex and ulna plus variance were treated by excision of the distal part of the ulnar head. Three of the seven consecutive patients also had lunotriquetral ligament injuries and had fusions of that joint. Six of the seven patients had either complete relief or marked improvement of symptoms at the distal radioulnar joint. When ulnar shortening is indicated, ulnar head shortening should be considered.

Purpose : To evaluate the use of post-arthrography computed tomography in examining wrist ligament injuries. Materials and Methods : Thirty consecutive patients who had a history and clinical findings suggestive of ligamentous injuries of the wrist were examined. Fourteen men and sixteen women (average age 35 years) were enrolled in this study. The evaluation concentrated on the detection and precise localization of the ligament lesions in the triangular fibrocartilage (TFC), the scapholunate ligament (SLL) and the lunotriquetral ligament (LTL). Results : For TFC, SLL and LTL lesions, arthro-CT showed a sensitivity 96%, 90% and 85.7%, and a specificity 80%, 90% and 91.3%, and an accuracy 93.3%, 90% and 93.3%, respectively. Conclusion : Arthro-CT may be a useful imaging method for evaluating intra-articular ligament injuries of the wrist.

Purpose: To analyze the findings of midcarpal and radiocarpal arthroscopic examinations in the diagnosis of wrist trauma and pathology focusing on interosseous ligament laxity. Materials and Methods: Forty-one midcarpal arthroscopic examinations were performed in conjunction with radiocarpal arthroscopic examinations. Thirty-one wrists with distal radius fracture and ten wrists with triangular fibrocartilage complex tear were included. Scapholunate and lunotriquetral ligament laxity was graded by using the Geissler classification and radiocarpal and midcarpal arthroscopic findings were compared. Results: For scapholunate findings midcarpal arthroscopy confirmed radiocarpal findings in 14 cases and provided a diagnosis in 6 cases. For lunotriquetral findings midcarpal arthroscopy confirmed radiocarpal findings in 13 cases and provided a diagnosis in 6 cases. Conclusion: Midcarpal arthroscopy added more statistically significant information to the results of radiocarpal examination than wrist arthroscopy performed without a midcarpal examination.

Eighty-four wrist arthrograms were performed on 76 patients with posttraumatic wrist pain. Instillation of a small amount of contrast media into the radiocarpal joint under fluoroscopic control was helpful in delineating specific areas of pathology about the wrist. Comparison of the arthrographic findings with the plane films of 74 patients and the operative findings of 24 patients revealed that the arthrogram was most helpful in delineating perforations of the scapholunate ligament, the lunotriquetral ligament, the triangular fibrocartilage complex, and the wrist joint capsule.

Wrist arthrography is often used to evaluate the integrity of the scapholunate and lunotriquetral ligaments. This may be difficult when the ligaments are obscured by contrast material in standard radiographic projections, despite filming under fluoroscopic observation during the injection of contrast material. When superimposition occurs, we recommend fluoroscopic positioning of the wrist to optimally visualize the individual ligaments in profile. We applied this technique in performing 240 wrist arthrograms, 88 (37%) of which had scapholunate and/or lunotriquetral ligament tears. As a result, a definite diagnosis of the specific site of ligamentous disruption was possible in all but one case. Using this technique, we also identified six short-segment or "pinhole" ligamentous tears. Identification of commonly interrupted ligaments in profile by using fluoroscopy after contrast material has been injected may allow identification of specific ligamentous disruptions or short-segment tears, as well as a diagnosis of a specific anatomic abnormality that otherwise would not be possible.

There have been numerous studies on surgery of wrist ligament injuries, but a quick assessment reveals few with a high level of evidence (LoE). The primary aim of this study was to categorize the study type and LoE of studies on repair and reconstruction of the scapholunate ligament, the lunotriquetral ligament and the triangular fibrocartilage complex by applying the LoE rating system proposed by the Oxford Centre for Evidence-Based Medicine. The secondary aims were to evaluate the journal- and geographic- distribution of the included studies. An electronic literature search of articles published 1985–2016, in PubMed, Embase, and Cochrane Library was carried out in May 2016 and updated in April 2017. Therapeutic studies written in English were included. The PRISMA checklist guided the extraction and reporting of data. A total of 1889 studies were analyzed, of which 362 were included. Three journals represented 40% of the included studies and American authors dominated. Most studies (97%) had low LoE (IV-V). No studies of LoE I-II were found. There is insufficient evidence to recommend one technique over the other in terms of wrist ligament surgery in clinical practice. There is an immense lack of comparison studies with high level of evidence in the area of wrist ligament repair and reconstruction.

Wrist arthrography is a valuable procedure for the identification of scapholunate or lunotriquetral ligament tears. The addition of sequential filming during the injection of contrast material permits the arthrographer to determine accurately the specific ligament which is ruptured.
