Retinopathy of prematurity (RP) is a complex pathology among prematurely born children, which requires a comprehensive approach, including both prevention of the development of more severe stages of the disease, leading to poor vision and irreversible loss of visual functions, and the organization of dispensary observation and rehabilitation. Modern organization of dispensary observation, which is based on a detailed study of the etiology, pathogenesis, as well as the interaction of local and systemic factors on visual functions, can significantly reduce the visual disability of children as a result of this disease.

The pathogenesis of pulmonary fibrosis (PF) is accompanied by extracellular matrix (ECM) deposition, oxidative stress, and inflammation progression, as well as hyperactivation of amine oxidases (AOs), which contribute to disease manifestation. The present study aims to elucidate the effect of semicarbazide (SC), an inhibitor of Cu-containing AOs: lysyl oxidase (LOX), semicarbazide sensitive amine oxidase (SSAO), diamine oxidase (DAO), on PF induced in rats by bleomycin (BLM). Eighteen male Wistar rats were randomly divided into four groups: Control, rats of BLM group received BLM (5 mg/kg, intratracheally once), BLM+SC group obtained 0.005% solution of SC (about 50 µg per capita per day) for three weeks starting immediately after BLM injection, and the Control+SC group drank the same solution as BLM+SC group. The content of cross-linked collagen in total bronchi and free radicals in lung, activities of LOX, SSAO, DAO, polyamine oxidase (PAO), Cu, Zn-superoxide dismutase (SOD1), catalase (CAT) and glutathione peroxidase (GPx) in lung and blood were measured. BLM injection induced PF that was confirmed histologically and morphometrically as well as by the elevation of the content of cross-linked collagen and free radicals. The activities of LOX and SSAO involved in post-translational modification of ECM and inflammation were significantly increased (P < 0.05). The activities of DAO, and PAO that control polyamine metabolism were also essentially raised. Among antioxidant enzymes, only GPx was activated in the BLM group as compared to control. These changes were absent in the BLM+SC group. SC intake promoted the fact that the histology and morphometric parameters of lung tissue, the content of cross-linked collagen in the bronchi and free radicals in the lung, as well as the activity of the studied enzymes remained at the control level. Our data suggest that SC suppresses the development of BLM-induced PF by inhibiting AOs activities.

Purpose of the study. To study functional changes in the neuropsychological status of reproductive age patients with newly diagnosed breast cancer and to assess the possibility of using xenon-oxygen therapy to correct disorders.Patients and methods. This study included 60 reproductive age patients with newly diagnosed breast cancer who were undergoing surgical treatment at the Department of Bone, Skin, Soft Tissue and Breast Tumors of the National Medical Research Centre for Oncology of the Ministry of Health of Russia from 2018 to 2020. The main group included 30 patients with breast cancer diagnosed at the surgical stage of combined treatment, who underwent a rehabilitation course of xenon-oxygen therapy in the early postoperative period. The control group was formed from patients with the same diagnosis, without the use of this therapy. The functional state of the central nervous system in all patients was assessed by the parameters of the electroencephalography (EEG) bioelectrical activity. For the final assessment of the physiological and psychological state of the patients, a standardized questionnaires of the quality of life – ESAS, MOS-SF‑36, were used. Statistical data processing was performed using the Statistica 10 software package. Results. During the study, statistically significant differences were found in the assessment of subjective indicators, so in the group of patients using xenon-oxygen therapy, there was an improvement in well-being by 2.6 times, a decrease in depression by 2.3 times, a decrease in symptoms of nausea by 3 times, anxiety 1.9 times (p < 0.05). Against the background of an improvement in the psychophysiological state, the EEG showed a significant increase in the power of slow delta and theta rhythms, an increase in the power of the alpha rhythm and a decrease in the power of the beta rhythm, while in the patients of the control group only a decrease in the power of the beta rhythm was noted.Conclusion. The early postoperative period in patients with newly diagnosed breast cancer is characterized by the formation of a depressive symptom complex. The use of a course of xenon-oxygen therapy contributes to the normalization of the subjective feeling of physical and psychological health, increasing vital and social activity. Changes in the indicators of brain bioelectric activity and an improvement in psychophysiological state occur against the background of changes in brain activity caused by the normalizing effect of xenon.

Purpose of the study. Studying possible management of postoperative pain and normalization of adaptation status in patients with reproductive system onychopathology using xenon- oxygen therapy.Patients and methods. The study included 97 patients receiving surgical treatment for reproductive cancer at National Medical Research Centre for Oncology in 2016–2020. All patients were divided into the main and control groups depending on the performed corrective therapy with xenon- oxygen mixture. Inclusion criteria were: established diagnosis — endometrial cancer, cervical cancer or ovarian cancer at the surgical stage of antitumor treatment, absence of decompensated concomitant pathology. In addition to general clinical tests, the intensity of symptoms in patients was assessed using a standardized Edmonton questionnaire, adaptation status and data of a numerical rating scale of pain were recorded, the severity of endogenous intoxication was measured using calculated lymphocytic, leukocyte, nuclear and leukocyte shift index. Stages of the study included functional assessment of the patient's condition before surgical treatment and on the 1st and 5th days of the postoperative period.Results. An analysis showed statistically significant differences between the groups: in the group of patients receiving xenon- oxygen therapy, 12.9% of patients complained of pain, while in the control group — 34.2%, on mild exertion 17.1% and 39.9%, respectively (pConclusion. The effectiveness of the chosen therapy with xenon- oxygen mixture demonstrates the possibility of anesthesia and normalization of the adaptive status of oncogynecological patients who underwent surgical treatment for reproductive cancers.

Objective. The aim of this study was to evaluate the efficacy and safety of trastuzumab in combination with the FLOT/FOLFOX regimen in the perioperative treatment of resectable HER2-positive locally advanced gastric cancer. Methods. 15 patients were included in two groups; 4 patients were treated with trastuzumab (Herticad®, BIOCAD) + FLOT and 11 patients were treated with trastuzumab (Herticad®, BIOCAD) in combination with FOLFOX. The primary endpoint was incidence of disease stage reduction, tumor regression rate (TRG), change in HER2 tumor status, disease-free and overall survival. Results. Тrastuzumab in combination with FLOT/FOLFOX has a good therapeutic effect and a safety profile in the perioperative treatment of patients with resectable HER2-positive advanced gastric cancer (complete tumor regression [TRG1] was morphologically confirmed in 20% of patients, no pathomorphosis or minimal manifestations [TRG3/TRG4] in 40% of patients, in 40% of patients it was possible to achieve significant pathomorphological tumor regression [TRG2]). In addition, trastuzumab + FLOT gives better results in terms of tumor regression rate than FOLFOX + trastuzumab (cN+/ypN0 disease stage reduction in the T + FLOT group was 100%, whereas in the T + FOLFOX group it was only 30.0%). The HER2 status of the tumor changes against the background of preoperative PCT with the inclusion of trastuzumab (a loss of HER2/ neu protein overexpression was detected in 60% of patients).

To date, the main way to stop dysphagia for patients with unresectable esophageal cancer is stenting. Being widely accepted and effective, this technique, however, allows for oral nutrition only for an average of 3–4 months and is accompanied by the development of a wide range of complications. Recent Russian and foreign publications in the field show that evaluation of the effectiveness of the stenting technique, including analysis of possibility of complications development, is important for practical application. To improve long-term outcomes, the potential trends in the method evolution have already been identified. The stents coated with radioactive isotopes of iodine are among of the actively used novelties. Application of 3D printing for the manufacture of custom-tailored stents, as well as the inclusion of chemotherapeutic agents in the coating of self-expanding metal stents seem promising approach. The search for a «perfect stent» continues under paradigm of personalization.

The article shows the feasibility of detecting and monitoring primary breast diffuse large B-cell lymphoma using single-photon emission computed tomography (SPECT) with 99mTc-1-thio-D-glucose (99mТс-TG), a new radiopharmaceutical. The innovative radiopharmaceutical makes it possible to evaluate the metabolism of tumor tissue without using positron emission tomography. The findings of 99mTc-TG SPECT obtained during diagnosis and monitoring of the patient were confirmed by the results of computed tomography. A comprehensive examination of the patient, including 99mTc-TG SPECT, made it possible to diagnose a rare case of primary breast lymphoma. Primary breast lymphoma accounts for 1.7–2.2 % of all extranodal nonHodgkin lymphomas. Diffuse large B-cell lymphoma is the most common histological type occurring usually as a unilateral palpable tumor in middle-aged women. Extranodal lymphomas account for less than 0.5 % of all malignant neoplasms of the breast. After 6 courses of immunochemotherapy, the patient underwent 99mTc-TG SPECT, which demonstrated persistent hypermetabolic activity in the breast. The patient received radiation therapy to the remaining tumor of the left breast. The patient is in remission and followed up at the Cancer Research Institute of Tomsk National Research Medical Center. Thus, 99mTc-TG SPECT is believed to be a promising method for visualizing primary breast lymphomas and assessing their treatment outcomes. It is an alternative to the standard approach using 18F-FDG positron emission tomography. The method compares favorably with positron emission tomography with its wide availability and low cost of study.

Background: Surface reservoirs remain one of the priority sources of drinking water in the Russian Federation whereas the increasing anthropogenic effect on the environment does not exclude the possibility of contamination of surface waters with toxic compounds. The use of traditional methods of water disinfection with active chlorine aggravates the situation and leads to formation of highly toxic organochlorine compounds. In this regard, one of the urgent hygienic problems is to establish health effect of these toxicants in the population including its most sensitive groups. The objective of the study was to establish and quantify the relationship between concentrations of chloroform and its derivatives in drinking water and blood levels of organochlorine compounds in children consuming potable water with a higher content of chlorine and its derivatives. Materials and methods: The study was conducted on six territories of the Perm Region constituting an observation area where drinking water was noncompliant with hygienic standards by a number of indices, and a comparison area. The analysis of trihalomethanes in drinking water samples was carried out in accordance with the requirements of sanitary regulations SanPiN 2.1.4.1074–01. The levels of trihalomethanes (chloroform, carbon tetrachloride, dibromochloromethane, dichlorobromethane, and 1,2-dichloroethane) in children’s blood samples were analyzed by capillary gas chromatography in combination with a specific detector to increase selectivity and sensitivity of determination. We used a Kristall-5000 gas chromatograph with an Optima-5 capillary column (25 m × 0.32 mm × 0.5 μL) and a selective electron capture detector (ECD). Globalization of world markets, the formation of global supply chains, current economic trends allow us to count on further growth in world trade. The main challenge to the transport complex of the EAEU is the realization of the possibilities for reorienting part of the transported goods between the largest world markets for transportation through the territory of the EAEU. It should be noted that the attraction of transit freight traffic will remain relevant even with a decrease in world trade due to any economic changes, as it is not only about attracting additional freight traffic due to an increase in trade volumes, but also the redistribution of the structure of the existing freight traffic due to an increase in the share of the EAEU. All EAEU member states are interested in developing the transit transport market in Europe-Asia traffic and vice versa. Considering that almost all transit is container traffic, container rail transport will be mainly considered. The transportation of containers by road was not considered in the framework of the preparation of the report, due to the practical absence of such transportation across the Union from one external border to another. The existing container transportation by road is mainly a component of multimodal transportation, one of the links along with rail and water transportation. The task of the EAEU is to ensure an increase in transit traffic between the PRC and the EU, following the land route through the territory of the Union. The transit route from China to Europe through the Union has several advantages: it is faster than the sea route via the Suez Canal and cheaper than air transportation from China to Europe. Thus, there are all the prerequisites for the reorientation of part of the cargo flow to transportation through Eurasian transit corridors. The main share of container transportation in the world is in maritime transport. Due to its characteristics, maritime transport will continue to play a key role in the Europe-Asia transport market. However, there are two factors that are currently pushing for the diversification of communications and the opening of new land connections between Europe and Asia.

Quality of life of 42 patients with locally advanced breast cancer (T2-4N0-3M0) after combined modality treatment including either neutron therapy or photon therapy was studied. The EORTC QLQ-30 (version 3.0) and EORTC QLQ -BR-23 questionnaires were used. A slight decrease in the physical and emotional state and nausea/vomiting were observed in the group of patients who received neutron therapy (n=20) as well as in the group of patients who received photon therapy (n=22). In patients of the neutron therapy group, an increase in the index of dyspnea by 6 points (with the baseline index of 15.0 ± 5.7 points) was observed. The dyspnea index decreased to 5.26 ± 2.9 points 12 months later in patients having no evidence of disease progression. Local pulmonary fibrosis was diagnosed in 2 (10 %) of 20 breast cancer patients. In photon therapy group patients, an increase in the index of dyspnea by 11 points (with the baseline index of 14.3 ± 5.1 points) was observed. In 12 months, the dyspnea index decreased to 7.1 ± 3.1 points. Local pulmonary fibrosis was diagnosed in 4 (18 %) of 22 patients. A moderate decline in the general health status was observed in both groups of patients after neutron therapy delivered to the anterior chest wall. However, all patients with locally advanced breast cancer subjectively noted the rise of the general health status 12 months after completion of treatment (78.3 ± 4.2 points, respectively). Thus, postoperative neutron therapy delivered to the anterior chest wall did not increase the frequency of complications and did not affect the quality of life of breast cancer patients.

OBJECTIVE of the study was to analyze the main problems and try to find the ways to improve the esophageal stenting for malignant stenosis with regard to our experience.METHODS AND MATERIALS. Palliative esophageal stenting for malignant dysphagia realized in 103 patients with observing all the patients till the fatal outcome.RESULTS. Early complications were observed in 9 patients (8.7 %) among which were: perforation of esophagus – 1 (0.97 %), minor bleeding – 2 (1.9 %), stent migration – 6 (5.8 %). Elimination of dysphagia as an immediate clinical effect was achieved in all of 103 patients (100 %) among all of early complications included. Delayed sequelae were found in 40 patients (38.8 %) among which were recurrent dysphagia because of tissue in-/overgrowth – 27 (26.2 %), stent migration – 2 (1.9 %), different kinds of esophageal fistulae – 5 (4.9 %), major bleeding – 6 (5.8 %). The most severe life-threatening complications developed at 109 days after stent placement. The mean survival rate was 131 days (from 12 till 546 days).CONCLUSION. Immediate clinical effect of stenting was achieved in 100 % of cases and delayed clinical effect was in 73.8 % of patients. Chemoradiotherapy conduces to increase the mean survival rate and, at the same time, to increase the most severe delayed life-threatening complications after esophageal stenting

Background. The development of effective methods for diagnosing multicentric breast cancer patients (BC) is of great clinical importance, because it determines the tactics of surgical and radiation treatment. The aim of study: to compare diagnostic accuracy of scintimammography (SMG), digital mammography (MMG) and ultrasound (US) in the diagnosis of multicentric BC. Materials and methods . 410 women with histologically confirmed BC were included in this analysis. SMG was performed with dual detector SPECT unit Forte (Philips, Netherlands). Mammographic digital images were obtained on Senographe DS unit (GE Healthcare, USA). In 319 of 410 evaluated women we were able to determine results of preoperative US with high-frequency (7—18 MHz), high resolution linear array transducer. The pathological report was used as the gold standard. Multicentric BC was defined as 2 or more distinct invasive tumors occupying more than one quadrant. Detection of additional grouped calcifications of malignant type occupying a small portion of breast tissue (more than 15 pieces per 1 sq. cm) was considered as another mammographic sign of multicentric BC. Results. According to histopathological examinations multicentric BC was diagnosed in 51 of 410patients. The purpose of the study was to comparatively analyze surgical outcomes in patients with histologically proven rectal cancer. Material and methods. From 1999 to 2012, 174 patients with histologically proven rectal cancer underwent abdominal-anal resection of the rectum with bringing down the sigmoid colon into the anal canal and sphincter-preserving low intra-abdominal resection of the rectum with coloanal anastomosis. Results. Two groups of patients were studied: the main group (sphincter-preserving low intra-abdominal rectal resection) and the control group (abdominal-anal resection of the rectum) . The 5-year survival rates and quality of life of the patients (Wexner scale) were compared. Improvement in fecal retention function was observed in both groups a year after surgery. However, 17 patients in the control group (25 %) had unsatisfactory function of the closure apparatus (11–20 points on the Wexner scale), sharply limiting their social activity and requiring the use of anal tampons. Fecal incontinence was observed in 95% of the control group patients 1 year after surgery. A significant improvement in the quality of life associated with adequate sphincter function was found in the main group of patients. In 40 out of 88 patients who had survived 1 year (45 %), no significant violations of the closure function were revealed. The remaining patients had symptoms of fecal incontinence or inability to control bowel movements for a long time (less than 10 points on the Wexner scale). Conclusion. The use of low anterior rectal resection with coloanal anastomosis in the surgical treatment of cancer, significantly improves the patient’s quality of life without compromising the radicality and safety of treatment.

Introduction. The main objectives of foreign policy of any state include the extension of its influence in the international arena. In the era of information and knowledge, one of the most effective tools for this task is soft power of education. In this respect, education is seen as a promising soft power instrument, which long-term character allows a country to promote not only its cultural, scientific and technological achievements, but also its cultural values and norms of social organisation. Today, universities are also seeking to strengthen their positions in global education rankings, which are used to define the status and reputation of a particular university in the global market of educational services. This ranking serves as a guide for future students and their parents, when choosing a prestigious education location and when developing individual career paths. The aims of the present research are the following: to analyse soft power of higher education as a soft power mechanism, which allows a country to achieve and maintain leadership positions in the international arena; to specify conditions, factors and barriers that determine the position of universities in global ranking systems. Methodology and research methods. The methodological framework is based on the concept of soft power and the theory of internationalisation of higher education. The empirical study was conducted following the tradition of qualitative sociological research. In 2018, a series of semi-structured interviews was conducted among Ural Federal University employees, whose responsibilities were related to the promotion of the university on the global educational market. The Ural Federal University (UrFU) can be considered representative in terms of practices aimed at attracting foreign students to the BRICS countries. In addition, UrFU is a place where the concept of a network BRICS University was realised in 2017. A total of 5 expert interviews were conducted. The collected data were analysed in the context of the World Bank statistics on the money spent by the BRICS countries on education and R&D during the 1999-2015 period (% of GDP), as well as the data provided by the QS World University Rankings - 2019 to analyse the reputation of BRICS universities. Results and scientific novelty. The case study of Ural Federal University allowed the authors to consider the specific practices of BRICS zones for attracting foreign students. The authors analysed the specifics of actions taken by a university to promote its international leadership positions in the international space on the example of students from China. It is shown that the role of global ranking systems (institutional and faculty) in the choice of a university is steadily growing. At the same time, factors determining the attractiveness of the Ural Federal University for foreign students, in particular for Chinese students, include a reasonable level of tuition, the quality of the education and the formed ethnic social networks. For them, the Russian education is expected to be a factor in achieving a higher social status in their home countries. This demonstrates the effective role of education as a soft power instrument, although in the long-term perspective. The authors identified a number of findings concerning the barriers faced by Russian universities on the way to recognising them as global actors of higher vocational education. It is possible to enter the Top 500 best world universities; however, the task of maintaining the achieved position in global rankings is more complicated and cannot be completed without a steady increase in the performance over several years. A world-class university should attract gifted youth, qualified teachers and researchers. The reduction in the number of talented people and research results leads to the loss of geopolitical mission by the educational organisation. Inadequate funding may prevent universities from breakthrough and sustainable improvement of their reputation. Practical significance. The research findings might be useful for managers of higher education and for strategists involved in positioning and making forecasts of educational institutions development. Also, the present findings can be applied by specialists engaged in the issues of educational policy, distribution of public investments, as well as the issues of international interaction and competition between states in order to realise national interests and support national economy in the country in the long term. © 2020 Russian State Vocational Pedagogical University. Possible risks associated with the accumulation and migration of mycotoxins during germination of crops are presented. The presence of an acceptable level of surface microflora of grain can, during germination, intensify the accumulation of mycotoxins, which later migrate into the finished product, making it dangerous for the consumer. It is noted that the process of germination of grain crops cannot be carried out without the stage of disinfection. Promising methods of grain disinfection using non-thermal effects of exposure and the results of our own research are given. It is noted that the concept of food safety in world practice today is aimed at preserving the main food ingredients and their properties. Thermal effects lead to an effective reduction in the development of microorganisms, but at the same time cause significant losses of thermolabile compounds and adversely affect the organoleptic, physicochemical and functional properties of the final product. Ultrasonic exposure, nanosecond electromagnetic pulses and exposure to cold plasma are proposed as disinfecting methods. The methods presented today are widely used in world practice. They allow not only to deactivate molds, but also to destroy the already formed aflatoxins in food products (use of cold plasma). Based on the foregoing, it can be said that the danger of pathogenic microflora ingress and aflatoxin accumulation in food products (especially whole grains) is still present, since even a minimal amount of them can cause global harm to public health, therefore, the search and development of new disinfection methods is an urgent problem modern production.

By the beginning of the reproductive period, myopia accounts for 25-30% of the female population in Russia. For a long time, it has been believed that pregnancy and childbirth can provoke the development of peripheral dystrophies and, as a result, retinal detachment, which is an absolute indication for surgical delivery. However, there is literature evidence about the possibility of natural delivery for patients with retinal detachment. Detection of peripheral dystrophies during examination in pregnant women is an indication for preventive laser treatment. The modern approach to choosing the method of delivery of pregnant women, taking into account the state of the visual organ, based not on the presence of the degree of myopic refraction, but on the retinal status (the presence of PVCRD and retinal detachment), can significantly reduce the frequency of operative delivery for ophthalmological indications.

Today, food products not only should serve the source of main nourishment but also must minimize the risk of negative impact on the human body. Products enriched with antioxidants can be referred to such category. For food producers, the development of products “for health” is usually connected with significant investment, whereas the final success of innovative products does not always meet the expectations. The greatest part of such products is withdrawn from the market during the first year. It is important for manufacturers to learn consumer behavior in order to ensure sale growth and a stable market position. The purpose of this study was to study consumer reception of products containing antioxidants. (It is important to conduct market research to identify the needs of buyers in order to maintain a stable position in the market. In order to study this issue, we analyzed the consumer perception of foods with antioxidant properties.) We studied the consumer perception of products with antioxidant properties with regard to choice predictors and barriers for purchasing, which finally determine the success of the product on the market. For this purpose, we conducted a survey of 721 consumers of the South Urals. The results of the statistical analysis done with the help of SPSS proved that South Ural consumers in general are ready to purchase products containing antioxidants. Besides, producers must bring information concerning the real value of the product and win the consumer trust and confidence as far as these are the main predictors determining the choice for purchasing the products containing antioxidants. Misunderstanding of the role antioxidants play in the human body may lead to perception of risk concerning consumption of such products and rejection of the purchase.

The severity of pain and changes in the adaptational status were studied in patients with brain metastases or cervical cancer receiving xenon therapy after whole brain radiotherapy or after radical hysterectomy. Hematological indicators of the nature and tensiton of general nonspecific adaptional reactions of the body (ARs) by Garkavi-Kvakina-Ukolova, the QLQ-C15 questionnaire and a 10-point graphic visual analogue scale for the assessment of the intensity of pain were used. Xenon caused concurrent reduce in the intensity of pain and improvement of characteristics of ARs in all studied patients. The results suggested an association between the analgesic effect of xenon and the normalization of neuroimmune processes and reduced damaging effects of special antitumor treatment on the body under the influence of xenon.

We studied the effects of combined chemotherapy with doxorubicin/docetaxel on erythroid and granulocytic hematopoietic lineages with particular attention focused on their recovery in patients with stages III-IV breast cancer. Intensification of differentiation of erythroid and granulocytic CFU (even under conditions of their suppressed proliferation) provided the increase in the content of mature and morphologically differentiated elements in the bone marrow and peripheral blood. High proliferative activity of erythroid and granulomonocytic precursors resulted from enhanced production of hematopoiesis-stimulating activities by microenvironment elements.

The purpose of the study was to provide up-to-date data on the efficacy of positron emission computed tomography (PET) for the diagnosis, assessment of tumor extension and monitoring of colorectal cancer.Material and Methods. The Medline and Pubmed databases were used for searching for publications devoted to clinical trials of diagnostic value of PET with 18F-Ftordesoxyglucose (PET with 18F-FDG).Results. Data on the value of PET with 18F-FDG in visualization of the primary tumor, detection of lymph node and distant metastases and assessment of the treatment response in patients with colorectal cancer were presented.Conclusion. PET with 18F-FDG was shown to be a promising imaging method for colorectal cancer. It is the most sensitive method in the detection of liver metastases. 18F-FDG PET is particularly superior to CT and MET in the ability to assess tumor response to treatment and predict early tumor response to therapy. The development of new radiopharmaceuticals for the diagnosis, staging, monitoring and prediction of colorectal cancer is of great importance.

The data of the ultrasonic gingiva thickness scanning are compared with the data obtained using the colorimetric probe system. We revealed that the white probe corresponds to the gingiva thickness - less than 0.5 mm; the green probe - the gingiva thickness from 0.6 to 0.9 mm; the blue probe - the gingiva thickness from 1.0 to 1.5 mm. If none of the probes is visible through the gingiva, such a biotype is regarded as very thick and its thickness reaches more than 1.6 mm. Thus, the colorimetric probe system can be used by a doctor for gingiva thickness examination the in the absence of an expensive ultrasound scanner.

This review addresses the function of the protein-transporter glycoprotein-P (Pgp, ABCB1 protein) and the expression of its encoding gene MDR1/mdr1 throughout the gestational period in humans and animals. We discuss the relationship between the transporter function and the hormone fluctuations during pregnancy. We cite studies on the association between the MDR1/mdr1 gene polymorphisms and the hemato-placental barrier permeability for Pgp substrates, as well as fetal malformations. Under a separate section, drugs commonly used in pregnant women are characterized in terms of their roles as substrates, inducers or inhibitors of the Pgp transporter.

Aim. Eribulin is an active cytostatic, associated with a wide range of mechanisms of antitumor effects, but eribulin efficiency and safety in patients with breast cancer (BC), associated with cerebral metastases are still poorly understood. Materials and methods. We analyzed the combined Russian experience of eribulin application in BC patients associated with brain metastases; the analysis included 459 Russian women with advanced BC who had received at least 2 course of eribulin during the period from 2014 to 2018; 35 of 459 patients had brain metastases (40.0% - luminal HER2-negative subtype, 31.4% - triple negative subtype and 28.6%h - HER2-positive BC). The median age was 52 years (39 - 80 years of age). In most cases, the patient had two or more metastatic brain lesions (68.6%; the median was - 3); brain radiotherapy was used in 62.8% of patients before eribulin treatment and in 5.8% of patients was held stereotactic radiation therapy during eribulin chemotherapy. We analyzed the efficiency of eribulin application (the therapy continued until disease progression, the development of unacceptable toxicity, or impossibility to apply the drug for any other reason). Results. The results showed that clinical efficacy (objective response rate + stabilization of disease lasting for more than 6 months) was 48.6%: partial response - in 20% of patients and stabilization of disease - 62.9%; tumor growth control was in 82.9%. Median PFS in all group of patients with brain metastases was 4.1 months and was similar to median PFS in patients who received radiotherapy before eribulin treatment or without eribulin - 4.1 vs 3.47 months; p=0.798. Conclusions. The application of eribulin in BC patients with brain metastasis are absolutely justified, the drug demonstrates the efficiency in a retrospective analysis in a Russian population. The determination of the optimal algorithm for the treatment of patients with metastatic BC associated with brain metastasis requires a multidisciplinary approach and further research.

Background.Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is being widely applied as a therapy for hematological malignancies. The long-term outcome of allo-HSCT depends directly on the ability of cytotoxic T-lymphocytes to recognize and eliminate the residual tumor. CTLA-4 is one of the regulatory proteins that provide control over the development of the immune response. Polymorphisms in the CTLA4 gene can affect its function and the efficiency of the antitumor response.The objective:to study the effect of non-synonymous single nucleotide polymorphism (nsSNP) c.49A>G in the donor CTLA4 gene on tumor control in the recipient of allogeneic hematopoietic stem cells (HSC).Materials and methods.Donors of HSC were genotyped for nsSNP c.49A>G in the CTLA4 gene by the real-time polymerase chain reaction using the allele-specific primers. Genotyping data was validated by Sanger’s sequencing of 22 randomly selected samples. The overall survival, the event-free survival and relapse probability were calculated using the Kaplan–Mayer method. A log-rank test was used to assess the statistical significance of group disparities. A p-value of 0.05 was considered as significant.Results.The frequencies of the CTLA4 gene c.49A>G polymorphism alleles in the observed population (102 healthy donors of HSC) correspond to the frequencies obtained by the “1000 genomes” project for the European population. The effect of the donor CTLA4 polymorphism on the tumor control was evaluated on the cohort of patients with acute leukemia after human leukocyte antigen (HLA) matched HSCT from an unrelated donor. It was shown, the three-year relapse-free survival was significantly lower for those patients who received grafts from a donor with the homozygous A/A state of nsSNP c.49A>G (p = 0.01), it was 12.7 % versus 62,8 % in group with c.49A>G G/G and A/G donor genotypes. The incidence of relapse was also significantly different for the group with A/A genotype and for the group with G/G or A/G genotypes of the nsSNP and equaled to 83.7 and 29.3 % respectively (p = 0.03).Conclusion.Patients with acute leukemia, who underwent allo-HSCT from unrelated completely HLA-matched donors with c.49A>G G/G or A/G genotypes have the significantly lower risk of relapse than patients whose donors had the A/A genotype. These results suggest practicability of the nsSNP genotyping for the optimal donor selection.

Aim. The aim of the study is to examine the efficacy and safety of eribulin in HER2-negative metastatic breast cancer (BC) in Russian clinical practice. Materials and methods. The analysis included 459 patients with advanced BC from 44 federal and municipal medical clinics in Russia and received at least 2 courses of treatment with eribulin in accordance with the registered indications for drug. The average age of women was 56 years (between 29 and 81 years), 83% of patients had HER2-negative tumor subtype (49.9% - luminal BC and 33.1% - triple-negative BC) HER2-positive biological tumor subtype was registered in 17% of patients. Visceral metastases were diagnosed in 73% of patients and three-zone and multiple zone metastases were diagnosed in 41.6% of cases. The median number of prior lines of therapy in patients with disseminated disease was 2; anthracycline and taxane chemotherapy was applied in 94.3% of patients, and 38.1% of patients were recived CT plus capecitabine. Standard treatment regimen with eribulin was cotinuing (1.4 mg/m² as a 2-5-minute intravenous infusion administrated on days 1, 8 of a 21-day cycle) until disease progression, unacceptable toxic effects, or impossibility of the drug administration for any other reason. We estimated the efficacy and safety of treatment with eribulin in Russian patients with HER2-negative BC. Results. Objective response rate was achieved in 20.5% of cases, complete response rate was in 3.2%, partial - 17.3%, and the stable disease rate was marked in 52.7% of women, and in 19.7% of these cases was prolonged more than 6 months. Aim. To analyze the current situation in the staffing of functional diagnostics by doctors in the Russian Federation.Material and methods. The method of descriptive statistics was used to analyze the data of the Federal statistical observation № 30 “Information about the medical organization” on the staffing of doctors of functional diagnostics for the period from 2012 to 2017.Results. During the analyzed period, the staffing of doctors of functional diagnostics in the Russian was low, although there was a positive trend. In the calculation of staffing for the number of occupied positions in almost all Federal districts, parameter was higher than 80,0%, with the exception of the Southern Federal District (76,0%).Conclusion. The dynamics of the staffing parameter for the number of occupied positions was negative.

Currently, 18F FDG-PET and 18F FDG-PET/CT are widely used for diagnosis and monitoring of lymphomas. The majority of aggressive lymphomas are characterized by high glycolytic activity, which enables the visualization by using 18F FDG-PET/CT. The use of PET/CT makes it possible to clarify the stage of the disease in 10–30 % of patients, with additional tumor sites typical for advanced stage of lymphomas, which in turn effects on treatment and disease prognosis. The 18F FDG-PET/CT has the advantage over other methods of radiation diagnosis in detecting bone marrow lesions in patients with lymphomas. It has been shown that 18F FDG-PET/CT performed at early stages of chemotherapy allows differentiating patients with favorable lymphoma, which is sufficient for standard therapy and high-risk patients who require more intensive treatment with high-dose regimens of chemotherapy. After completion of therapy over 60 % of patients with HL and 40 % with aggressive non-Hodgkin’s lymphomas, have residual masses containing necrotic and/or fibrotic tissue and residual neoplastic cells. 18F FDG-PET and 18F FDG-PET/CT has been shown to be useful in identifying residual masses in 30–64 % of patients, by demonstration of persistent metabolic activity on FDG-PET. Between 62–100 % of patients with residual FDG-positive masses have been shown to relapse after first-line chemotherapy. Identification of patients with partial response to chemotherapy indicates the need for continued treatment. New radiopharmaceuticals for the diagnosis of lymphoma and evaluation of therapy effectiveness are developed. Such promising radiopharmaceuticals are 18F fluorothymidine, a biomarker of cellular proliferation and Ga-68 CXCR4, a chemokine receptor imaging biomarker.

Casting yards of blast furnaces are most powerful sources of atmospheric air pollution among big non-organized pollution sources of steel industry. The task to create effective systems of those sources aspiration is rather actual.Analysis of known technical solutions on blast furnace casting yards aspiration presented. Positive and negative sides of the projects determined. A conclusion made that aspiration systems effectiveness is determined by designs of local suction facilities and covers for localization of emitted dust and gas convection flows. Main directions of effective aspiration system creation formulated. At the example of JSC MMK BFs No. 9 and No. 10, four variants of technical solutions of general aspiration system for casting yards quoted, accompanied by calculation of convection flows and emissions volumes, maximum necessary productivity of aspiration facility and aero dynamical calculation of an aspiration system. Plan and algorithms of the system valves operation studied. Effectiveness of localization (capture degree) of non-organized emissions estimated as about 90%.

We studied myelotoxicity of modern schemes of chemotherapy for breast cancer (docetaxel/doxorubicin and cyclophosphamide/doxorubicin/5-fluorouracil) towards granulocytopoiesis, the mechanisms determining the differences of hematological effects of these schemes, and the efficiency of correction of the observed changes with granulocyte CSF (filgrastim). Granulocytopoiesis stimulation with filgrastim during the treatment with docetaxel/doxorubicin combination was more pronounced than during cyclophosphamide/doxorubicin/5-fluorouracil therapy. The observed differences were found at all levels of granulocyte lineage organization (central and peripheral), which is related to different effects of the cytostatic substances used in the proposed protocols on the structures controlling hemopoiesis.

We review the role of dopamine in the mechanisms of development of primary headaches, including migraine. Some data are given about the involvement of various types of dopamine receptors in the regulation of cranial blood flow and control of nociceptive transmission in the trigeminal nerve system. We discuss the role of the basal ganglia in the neurobiology of cephalgias. One part of the review is focused on dopaminergic drugs that may be used in the therapy of migraine.

The incidence and mortality of skin melanoma in the world and in Russia is growing every year. It should be noted that the proportion of patients with melanoma of late stages remains high. Until recently, only ineffective chemotherapy regimens were available for such patients. That is why the possibility of using modern drugs in real clinical practice for the treatment of this cohort of patients is an urgent issue of modern clinical oncology. The appearance in a real clinical practice of a new group of immunological drugs (anti-CTLA4 and anti-PD-1) for the treatment of patients with metastatic melanoma allows to increase both the time to progression and the life span of this cohort of patients.In this article, we present the experience of St. Petersburg Regional Clinical Hospital “City Clinical Oncology Clinic” in the treatment of patients with metastatic melanoma under the conditions of real clinical practice with the preparation of ipilimumab as part of the expanded access program from February 2014 to March 2017. In total, 31 patients with disseminated melanoma (12 men and 19 women) were treated with ipilimumab during this period. The average age was 57 years (from 39 to 81 years), in 8 patients there were metastases to the brain. Patients with ECOG status 1 at the time of initiation of treatment were the majority 28 patients (90%), three patients (10%) had ECOG status 2. All patients had previously received 1 to 4 lines of systemic chemotherapy. The median of the observation period is currently 12 (3-35) months. The median time to progression was 6 months. At 5 of 31 patients at the time of analysis, signs of progression were not identified. The median overall survival (OB) was 12 months 9 of 31 patients were alive at the time of the analysis. The annual OM was 56.5%, the 2-year period was 29.4%.The article presents a clinical case of treatment of a patient with metastatic melanoma, which will have an effect against the progression of the disease after four lines of therapy, including ipilimumab. The patient was included in the program of expanded access to the PD-1 inhibitor, nivolumab. In all, the patient received 65 nivolumab injections. The life expectancy of the patient from the moment of diagnosing the metastatic stage of the disease is 3.5 years, while 2.5 years on the background of nivolumab therapy. Patient monitoring continues.

Topicality. Given the increasing demand in herbal drugs, it is very important to develop a base of medicinal plants raw materials for the pharmaceutical industry. Another relevant issue is the substitution of imported medicinal plant by domestic. This allows, on the one hand, to expand the range of products in order to maximize the needs of the Ukrainian population for high-performance medicines and dietary supplements based on herbal drugs, on the other hand, domestic substance of medicinal plants is cheaper than imported, which will allow them to compete successfully in the price of the finished product.Aim. To study the mineral composition of the herbal drugs of immortelle (Helichrysum bracteatum) using the atomic-emission spectrographic method.Materials and methods. The herbal drugs of immortelle were used for the study. The analysis was carried out as for flowers, herb and roots. To study the qualitative composition and content of minerals, one of the modern and sensitive methods of analysis as atomic-emission spectrographic method was used.Results and discussion. The presence of fifteen minerals and their quantitative content were determined. These experimental data indicate a diverse mineral composition in the herbal drugs being investigated, and the value of herbal drugs in terms of quantitative indicators. The results of study the trace elements content indicated that roots of the immortelle contain more minerals than herb, and flowers – less. The total of all elements in the investigated herbal drugs (mg/100 g): herb – 4653.25; flowers – 2647.89. roots – 6835.25.Conclusions. For the first time an analysis of the mineral composition of flowers, herbs and roots of the immortelle were carried out. Undoubtedly, such a rich mineral composition will cause high biological activity and this plant can be considered as promising for a complete phytochemical study with the purpose of identifying biologically active substances and determining the types of their activity and creating on the basis of immortelle medicinal drugs.

Aim. A comparative analysis of mertoplasty results for treatment of uterine scar leak by different approaches. Materials and methods. A retrospective analysis of hospital records of 51 patients was made, which were divided into three groups depending on the surgical approach used: group 1 – laparotomy access, group 2 – laparoscopic access, group 3 – transvaginal access. All patients had hysteroscopic and ultrasound signs of uterine scar leak after cesarean section (thickness of the myometrium in the scar area less than 4 mm, discontinuity of the scar). One month after the surgery, all the patients were examined by an obstetrician-gynecologist, all had a control ultrasound to assess the condition of the uterine scar. Incidence, severity, and risk factors for hemorrhagic cystitis (HC) were assessed in 267 patients undergoing allogeneic hematopoietic stem cell transplantation (allo-HSCT). HC was diagnosed in 14.6% (39 patients) within 1-139 days after allo-HSCT (median duration – 39 days). Chemotherapy-related HC was diagnosed in 4 patients only. The majority (19⁄35) of patients developed late HC of viral aetiology. Median time from a day of HC diagnosis to clinical symptoms resolution was 25 days (range: 6 to 133 days). Using a multivariate analysis, allo-HSCT from mismatched unrelated/haploidentical donor was found to be a risk factor of HC (р=0.01). The analysis also showed that 82.1% of patients with HC received cyclophosphamide as a part of conditioning regimen or +3/+4 days after allo-HSCT.

Idiopathic generalized dystonia is a group of neurodegenerative diseases primarily charecterized by dystonic hyperkinetic disorder (irregular slow involuntary movements of different body parts accompanied by specific muscle tone changes and pathological poses). A 54-year-old female was admitted with various complaints. The leading syndrome was extrapyramidal due to which the differential diagnosis included Parkinson’s disease, multiple system atrophy, corticobasal degeneration, etc. The diagnosis of idiopathic generalized dystonia, in the form of lower extremity hyperkinesia with dystonic equinus of the left foot and hallux extension and cheirospasm, was made by a method of exclusion. The clinical case described in the article illustrates variety of clinical manifestations of the disease imitating clinical presentation of various neurological and psychiatric disorders, complexity of diagnostic search and importance of timely diagnosis of this disease.

The designs of miniature waveguide loads of the millimeter wave band, that ensure an acceptable matching in a wide frequency band are proposed. Microwave one-ports with the volume-inhomogeneous dissipative filling are analyzed and optimized using 3D numerical models. Three experimental sample loads covering a frequency band of 53.57−178.4 GHz are fabricated and studied.

Background. Improving nursing methods of newborns with birth weight less than 2500 g resulted in an increase in the proportion of children at risk for the development of severe disabling conditions. Objective: Our aim was to investigate the risk factors of the mother, associated with the birth of a child with a birth weight less than 2500 g. Methods. In a retrospective cohort study included 572 children born to 566 mothers in 5 perinatal centers. Risk assessment was carried out taking into account birth weight (in groups of children with normal, low, very low and extremely low birth weight). Results. Sample of mothers (n = 489), whose children had birth weight 2500 g, characterized by a mean age of 31 (26; 35) years, the growth of 164 (159; 168) cm, weighing when registering for pregnancy 62 (53; 74) and 70 kg (60; 80) kg just before birth. Among the independent predictors of birth weight infants were allocated as follows: maternal education (higher/more), mother occupation (housewife/other), presence of chronic diseases, anemia during pregnancy, the threat of premature births, growth and weight of the mother’s body before birth, weight gain body during pregnancy, body mass index (BMI) when registering for antenatal care in pregnancy and before delivery. Conclusion. Identified risk factors for maternal birth weight infants, which can be used to stratify pregnant women at risk.

Novel, small-sized broadband matched loads in the form of disordered photonic crystals containing nanometer metallic and insulating layers have been developed. It has been proposed to form insulating layers with different permittivities and low dissipation factors in both centimeter and millimeter wavelength ranges from the composites that consist of insulating matrices filled with air inclusions.

Nowadays starch has become an important food and technical product widely used in various sectors of food industry. The article provides the materials of research studies of Russian and foreign scientists in the field of modification of starch as an instrument of enhancing its technical properties. Modified starch, which is several-fold cheaper, allows to significantly reduce the base cost of the final product. It is used for: sausage production, binding of free moisture; production of sauces, ketchup, mayonnaise for relevant consistency; production of yoghurts or kefir for desired texture; production of confectionery and bakery products for better appearance, etc. Regardless its field of use, the modified starch, which is manufactured in such countries as Russia, China, USA, India and others, acts as thickener, emulsifier, and stabilizer. The main problem while using starch is the increase of viscosity with ageing of aqueous solutions; this happens due to partial crystallization of polymer chains. Contemporary research studies on development of efficient ways of targeted alteration of natural properties of native starch have recently been actively progressing, since starch modification through chemical (acid, oxidizing hydrolysis), biochemical (enzymatic hydrolysis), and physical activation (mechanical, temperature, ultrasonic and wave) is not only of scientific value, but may also be used in industry. It is worth noting that methods of gene engineering are not used for the modified starch manufacture. However, the Expert Committee assembled under such organizations as FAO (Food and Agriculture Organization of the United Nations) and WHO (World Health Organization) recommended unlimited use of only the starches which undergo enzymatic processing. Other kinds of chemically-processed starches need further studying. That is why physical methods of modification are of major interest for they allow to activate starch in a non-agent way and still alter its properties.

Introduction . The basic contradiction between market relations and social sphere is distinguished from the main sources of negative phenomena among young people and failures in realization of youth policy. Substitution of social production goals by its means, a sharp distortion towards a primacy of market economy with its only indicator of success in the form of money deform a process of formation of a personality of a young specialist both in professional, and in sociocultural plans. As a rule, management teams of enterprises expect the generation of young workers to possess initiative and role behaviour which consists in highly skilled independent performance of production functions. At the same time, creative importance of «Person – Person» relations is underestimated. In this regard, questions of social-moral education of young personnel are brought into focus, whereas providing and further increase in competitiveness of production in many respects depends on their decision. The aim of the present article is to conceptualize spiritual and moral foundations of youth policy implementation in a real sector of the economy. Methodology and research methods . The research methodology involves a comprehensive application of anthropological, ontological and axiological approaches. Dialectical method, method of historical and logical unity, statistical and comparative data analyses are used. Results and scientific novelty . Spiritual and moral characteristics of young workers which formation demands system educational measures are one of the most important development provisions of enterprises in new social and economic reality. The questions of realization of youth policy in real sector of economy, including Sverdlovsk region, are considered. The following concepts and aspects are analyzed from social and philosophical perspectives: the concept «spirituality»; features of mental phase of psychology and spiritual stages of consciousness; the concept «perfection» as a source of spirit; «spirit» as the ideal gene of socialization and foundation of mental health; «morality» as a source of human community and creative practice. It is stated that all directions and forms of work with youth at the enterprises have to promote formation of high spiritual and moral qualities; a person can not take place neither as a citizen, nor as an expert without having reached necessary spiritual level. Morality expands the horizon of consciousness of young people, allows them to understand social importance of common collective affairs; attainment of professional honour and responsibility for the results of own work by a worker are impossible without morality. Practical significance . Work organization of industrial enterprises for development and deployment of ethical codes and norms, implementation of special programs of moral education of working youth with attraction for this purpose of various experts (doctors, psychologists, sociologists, theologians, etc.) will surely turn back positive social and economic effect: increase in economic stability of an enterprise (also region and country in general); considerable reduction of turnover of staff; reduction of cases of industrial injuries; decrease in number of violations of labour discipline; absence without leave, and other deviations in behaviour of young workers. The materials and results of the investigation can be used as follows: organization of work with youth in real sector of economy; development of social technologies and science-based recommendations for the implementation of corporate youth policy; process of teaching of social philosophy, sociology, social psychology, management of professional career.

The paper presents results of clinical application of autologous stromal-vascular fraction (SVF) cells in patients with degenerative osteoarthritis (OA) of the knee, grade II and III (Kellgren–Lawrence scale).We recruited six patients with knee OA (3 men and 3 women; median age 64 years) with mean disease duration of 7 years. All the patients were administered a single intra-articular injection of autologous nucleated SVF cells at an average dose of 16.8±0.9 × 106 per joint (a total of 11 joints). The patients did not experience any serious side effects (allergic, toxic or inflammatory) related to the knee injection. Patient surveys at 1 month after SVF administration revealed a decrease in the severity of pain, as measured by a visual analog scale (VAS) and a specialized 100-point scale KOOS (subscale "pain") (p < 0.05 on both scales). Moreover, the patients reported improvement in the joint functions and quality of life related to affected joints on a KOOS scale (p < 0.05). These positive clinical changes persisted during 6 month follow up. Significant improvements were noted in ultrasound findings, with increased thickness of the cartilage layer at 3 months (in 73% of cases) and at 6 months (in 82%). Our pilot study demonstrated the safety and tolerability of intra-articular injection of autologous SVF cells in patients with moderate to severe OA. The results obtained also indicate a significant antiinflammatory effect of autologous adipose tissue SVF cells, which is manifested at the early stages of cell therapy. Our further investigations will be focused on exploring the SVF stimulatory effects on regeneration of damaged joints.

Paraneoplastic neurological syndrome involves the concurrent development of cancer and neurologicaldiseases. Breast cancer is the second most common cancer associated with paraneoplastic damage to the central and peripheral nervous system. Autoimmune genesis of the disease is characterized by the presence of highly specific onconeural antibodies, which selectively affect neurons in the brain cord, spinal cord and spinal ganglia, and cause the onset of neurological symptoms within 2 years before cancer is detected. Six well-characterized onconeural antibodies detected in the blood serum of breast cancer patients can be used for the laboratory diagnosis of paraneoplastic neurological syndrome. Of them, anti-Hu, anti-CV2 and anti-amphiphysin antibodies cause polyneuropathy most often. Anti-Yo antibody is usually associated with cerebellar degeneration. Multiple neuronal autoantibodies can be simultaneously detected in a patient. Removal of the tumor may lead to stabilization and even partial regression of the neurological symptoms in 70 % of patients. Therefore, the surgical treatment of cancer should consider not only the tumor extension, but also the severity and progression of neurological deficit. We present a case of paraneoplastic cerebellar degeneration and paraneoplastic polyneuropathy in a 50-year-old woman with the neurological symptoms appeared 5 months before breast infiltrating ductal carcinoma was detected. The current approaches to the diagnosis of paraneoplastic neurological syndrome, as well as feasibility of radical removal of the tumor due to progression of neurological deficit were discussed.
